Iloperidone for the Treatment of Schizophrenia

To review the pharmacology, pharmacokinetics, efficacy, and safety data of iloperidone for the treatment of schizophrenia. Data were selected by searching Pre-MEDLINE, MEDLINE, and International Pharmaceutical Abstracts (1966-January 2010). Abstracts, scientific posters, and unpublished data provided by the manufacturer in the English language were also assessed. All published data including pharmacologic, pharmacokinetic, pharmacodynamic, and clinical studies related to iloperidone were considered for inclusion. Selected studies included randomized controlled trials, abstracts, and posters presented at national scientific meetings providing pertinent data. Iloperidone is a benzisoxazole phenylethanone with a higher affinity for serotonin-2a than dopamine-2 receptors. The recommended therapeutic total daily dose is 12-24 mg divided in 2 doses titrated over 1 week to avoid orthostasis. Acute, 6-week, randomized, placebo-controlled, and active-controlled studies demonstrated iloperidone's efficacy in reducing psychotic symptoms according to changes in the total positive and negative symptom scale (PANSS-T) score from baseline. A long-term maintenance trial demonstrated similar efficacy with haloperidol in preventing time to relapse. Pharmacogenomic studies reported possible single nucleotide polymorphisms related to QT interval prolongation and efficacy with iloperidone. Common adverse effects included dizziness, dry mouth, and sustained orthostasis occurring more frequently with higher doses. Weight gain is possible at any dose. Additionally, studies showed that QTc interval prolongation may be dose related. The incidence of extrapyramidal symptoms appears to be low across all dosage ranges; however, akathisia may be more frequent with higher doses. Iloperidone demonstrated efficacy in acute exacerbations and long-term maintenance in adults with schizophrenia. Caution may be warranted in elderly patients and patients with cardiac disease, due to orthostasis. Further studies regarding pharmacogenomic testing related to the drug's efficacy and tolerability are needed to justify its routine use in practice.

Response by Sex to Statin and Ezetimibe Versus Statin Monotherapy: Pooled Analysis from 22,913 Hyperlipidemic Patients

Synopsis: Coronary heart disease (CHD) remains the leading cause of mortality in women, highlighting the importance of preventive interventions in women. Purpose: To assess sex-related efficacy of statin + ezetimibe (EZ) vs statin in a broad, pooled database. Methods: Data were pooled from 27 double-blind, active, or placebo-controlled studies that randomized adult patients with hypercholesterolemia to statin alone or statin + EZ. Consistency of treatment effect among sexes was tested with ANCOVA with terms for treatment, first/second-line therapy status (FSLT), race, sex, CHD, statin potency, age, body mass index, baseline (BL) low-density lipoprotein cholesterol, BL high-density lipoprotein cholesterol (HDL-C), BL triglycerides, BL response variable, diabetes, trial within FSLT, treatment by FSLT, and treatment by sex interactions. Results: BL lipids and high-sensitivity C-reactive protein (hs-CRP) were generally greater in women than men. Both therapies significantly reduced low-density lipoprotein cholesterol, non-HDL-C, TG, apoB, TC, and hs-CRP, with significantly greater reductions seen with statin + EZ vs statin for prespecified dose comparisons. In general, treatment group differences for women were slightly smaller. HDL-C increased more for women than men in both treatment groups. There were no between-group differences in adverse events or creatine kinase elevations in the overall population. A greater incidence of alanine aminotransferase elevations was seen in statin + EZ vs statin patients in the overall population (0.6% vs 0.3%). Some minor differences, although not clinically relevant, were noted between sexes in the incidence of some AEs. Conclusions: This pooled database describes a large clinical experience with statin + EZ for women in which significantly greater changes in lipid and hs-CRP levels were seen with statin + EZ vs statin in both sexes, although women experienced smaller reductions than men for some lipids. Both treatments were generally well tolerated without major sex-related safety issues. Inter-institutional Development of a Poster-Based Cancer Biology Learning Tool

There is a paucity of African-American Cancer researchers. To help address this, an educational collaboration was developed between a Comprehensive Cancer Center and a distant undergraduate biology department at a minority institution that sought to teach students introductory cancer biology while modeling research culture. A student-centered active learning curriculum was established that incorporated scientific poster presentations and simulated research exercises to foster learning of cancer biology. Students successfully mined primary literature for supportive data to test cancer-related hypotheses. Student feedback indicated that the poster project substantially enhanced depth of understanding of cancer biology and laid the groundwork for subsequent laboratory work. This inter-institutional collaboration modeled the research process while conveying facts and concepts about cancer.

Breaking ground on translational stem cell research

Sponsored by the New York Stem Cell Foundation (NYSCF), the "Fourth Annual Translational Stem Cell Research Conference: Breaking Ground" convened October 13-14, 2009 at The Rockefeller University in New York City to discuss translational stem cell research. Attracting over 400 scientists, patient advocates, and stem cell research supporters from fifteen countries, the two-day conference featured an afternoon of panel discussions, intended for a broad audience, followed by a second day of scientific talks and poster presentations. This report summarizes both days of this exciting conference.

Student focused oral health promotion in Residential Aged Care Facilities

Objectives: A pilot oral health promotion student placement program was developed in February 2009 to provide final year Bachelor of Oral Health students with an opportunity to experience a community based aged care placement, and to provide health promotion to residents and staff within these Residential Aged Care Facilities (RACF). Design: Students in the final year of the Bachelor of Oral Health, University of Newcastle were placed in Residential Aged Care Facilities (RACF) that volunteered to participate in the program. Management at the RACF were interviewed to determine existing levels of oral health awareness and services provided, and the participating students completed pre-and post-participation surveys to determine knowledge and experience with older adults. Results: A total of 54 students participated and were placed in 17 RACF across the Central Coast area of New South Wales, Australia. Students produced oral hygiene teaching aids and a scientific poster as assessment tasks linked with the placement program, and assisted residents with their oral hygiene care. Students identified improvement in their skills and knowledge as a result of the experiences during the placement program. Conclusions: The student placement program provided a supportive teaching and learning environment where students built on and improved their communications skills and oral health knowledge pertaining to the older person.

Strategy for strengthening scientific capacity in developing countries on water and sanitation related issues

It is generally accepted that scientific research offers a tool for meeting the development demands in the area of water and sanitation. It is stressed by the authors that research should be performed by researchers in the countries most affected, on topics identified by these researchers and in their own environment. For researchers in developing countries the resources and scientific infrastructure constitutes a hindrance to scientific research. To address the situation this paper presents a strategy for scientific capacity strengthening. The strategy builds on a number of activities including; training courses on scientific methodology, literature review, fundraising, sampling, laboratory practices, statistical methods, experimental designs, participatory approaches, oral presentation, posters and manuscript development, as well as thematic workshops, workshop for revision of proposals, contacts with end-users, research grants, networks, purchasing and maintaining scientific equipment and scientific conferences. The strategy builds heavily on collaboration with local organisations. It has been concluded through assessments of activities arranged in Bangladesh, Benin, Burkina Faso, Cameroon, Ivory Coast, Ghana,Mali, Niger, Nigeria, Nepal, Sri Lanka and Togo that the strategy has been successful.

The presentation of scientific posters gives young scientists the opportunity to present their data in the setting of a medical congress. In preparation of the organization of the 116th Congress of the German Society of Internal Medicine (DGIM) 2010, the authors evaluated the poster rounds at the 115th Congress of the DGIM 2009 by using a questionnaire that was given to poster presenters, poster chairmen, and visitors. The authors sought to receive an instructive criticism for the organization in 2010. Distribution of questionnaires containing ten questions with preformulated response options and an additional field for further comments to all presenters, chairmen, and visitors of the poster rounds during the 115th Congress of the DGIM (April 2009). 159 questionnaires were returned and evaluated. Almost all respondents quoted the poster presentation as being important for their scientific work (98%). In general, they were satisfied with the discussion at the poster rounds (83%). The amount of posters within one round was criticized by 41%, as was the inadequate adherence to time constraints and time frame and room conditions themselves. The poster exhibition of the 115th Congress of the DGIM 2009 was evaluated positively by most of the respondents to the survey. Nevertheless, helpful hints were retrieved as how to further improve poster rounds. They should be respected when planning the exhibition at the congress in 2010.
