Filling Ability and Flow of Root Canal Sealers: A Micro-Computed Tomographic Study.

This study evaluated by micro-computed tomography (μCT) the filling ability in curved root canals, besides the flow of AH Plus (AHP) and Neo MTA Plus (NMTAP) sealers using different methodologies. Mandibular molars mesial roots with two root canals and degree of curvature between 20° and 40° were selected. The specimens were prepared with the ProDesign R system up to size 35.05 and were filled with the sealers by a continuous wave of condensation technique, Thermo Pack II (n=12). The teeth were scanned using μCT after root canal preparation and obturation. The volumetric percentage of filling material and voids were calculated. Flow was evaluated based on ISO 6876/2012 (n=10). Flow and filling were also evaluated in μCT using a glass plate with a central cavity and four grooves from the central cavity (n=6). Flow was linearly calculated into the grooves. The central cavity filling (CCF) and lateral cavity filling (LCF) were calculated in mm³. Data were submitted to non-paired t test with a significance threshold at 5%. The percentage of filling and voids between the root canals filled with AHP or NMTAP was similar (p>0.05). NMTAP presented the lowest flow in conventional test (p<0.05). Using μCT, sealers had similar CCF, LCF and linear flow (p>0.05). In conclusion, NMTAP and AHP had similar filling ability in curved mesial root canals of mandibular molars without presence of isthmus. Although AHP presented better flow than NMTAP using ISO methodology, there was no difference between these materials regarding volumetric filling when evaluated by μCT.

Interstitial fluid drainage from rat apical area takes place via vessels in the mandibular canal.

We sought to investigate the transport route for protein-rich fluid from the apical area towards the draining lymph nodes. The first mandibular molar root canals in 24 female Wistar rats were instrumented and filled with radioactive-labelled human serum albumin. The rats were sacrificed at different intervals beginning after 10min (time 0) and continuing up to 72h. Three jaw segments, gingiva around the first molar, blood samples, submandibular and cervical lymph nodes were collected and analyzed for radioactivity. The starting volume of tracer (control) for all experiments was calculated from measurements at time 0. At time 0, radioactivity was only detected in the jaw segments. Within lymph nodes and serum, the tracer was found after 4h, with the highest amount recorded in serum up to 24h. Lymphatics were found within the mandibular canal along blood vessels and nerves and exiting via foramen mandibularis, after immunohistochemical staining in four untreated rats. Our results show tracer distribution from the apical area towards the mandibular canal in a posterior direction. The tracer washout rate was low, and the fluid was mainly absorbed into blood vessels. The lymphatics in the mandibular canal may be more important for immune cell transport than for fluid drainage.

Accuracy of an endoscope to detect root canal anastomoses in mandibular molar teeth: a comparative study with micro-computed tomography

Objective: The aim of this study was to investigate the accuracy of endoscopic visualization to detect root canal anastomoses at the coronal half of the mesial root canals of mandibular molars using micro-computed tomographic (micro-CT) images as reference.Material and methods: Seventy-four mesial roots of mandibular first molars with (n = 47) or without (n = 27) intercanal anastomosis were selected based on the micro-CT scans of 269 mandibular first molars at a pixel size of 10 µm. The specimens were mounted on the mannequins and their root canals were evaluated using dental operating microscope (DOM) and endoscope. The endoscopic probe was inserted into the main mesial root canals and 2 blinded observers evaluated the presence of a divergence point of anastomosis (where the branching occurs) as ‘present’ or ‘absent’. The scorings were compared with the three-dimensional reconstructed images of the specimens and recorded as ‘correct’ or ‘incorrect’ evaluation. Degree of agreement between evaluators was assessed with Kappa test and the accuracy of endoscopic visualization according to the presence and location of anastomosis was compared using Fisher exact tests with a significance threshold at 5%.Results: High inter-examiner reliability was reported (0.91). None of the divergence points were identified using DOM whereas 11 divergence points were detected using endoscope, corresponding the 23.4% of the intercanal anastomoses. The endoscope also showed the absence of an intercanal anastomosis correctly in all of the specimens without an anastomosis. Detectability of a divergence point using endoscope was not affected by its location within the coronal half of root canal (p > .05).Conclusions: The endoscopes were able to visualize the divergence points of 23.4% of the intercanal anastomoses located at the coronal halves of root canals.

Shaping Ability, Cyclic Fatigue Resistance and Fractographic Analysis of Hybrid and Reciprocating Nickel Titanium Endodontic Instruments

The aim of this study was to investigate the shaping ability and cyclic fatigue resistance of Genius (GN) and WaveOne (WO) nickel-titanium (NiTi) instruments. Forty mesial root canals of mandibular molars were randomly divided into two groups, according to the instrument system used for root canal preparation (n = 20): GN and WO. Root canals were prepared to the full working length using the crown-down technique, with up to 40.04 instruments for GN group and 40.08 instruments for WO group. Specimens were scanned with a µCT before and after instrumentation. The changes in 3D parameters and degree of canal transportation were evaluated in the apical, middle and coronal thirds of root canals. Size 25, taper 0.04 (GN) and 0.08 (WO) instruments (n = 20) were used in simulated root canals until fracturing, and the number of cycles to failure (NCF) was recorded. The fractured files were investigated under a scanning electron microscope to characterize fractured surfaces. Data were analyzed using ANOVA post hoc Tuckey’s tests with the significance level set at 5%. The GN system was able to prepare the higher percentage of the canal surface with less canal transportation in all root canal thirds and displayed the higher resistance to cyclic fatigue in comparison to WO instruments (p &lt; 0.05). However, the length of the fractured segments was not significantly different among the groups; meanwhile, SEM investigations revealed the typical topographic characteristics of fractured surfaces. Hybrid GN instruments demonstrated superior shaping ability and resistance to cyclic fatigue. However, both NiTi instrument systems prepared the root canals with no significant shaping errors, considerable changes in 3D parameters and no significant differences in the topographic appearances of the instrument surfaces after fractures.

Assessment of the Performance and Surface Alteration of Two Single-file Systems According to the Number of Uses

The aim of this study was to assess the performance and surface alteration of two single-file systems according to the number of uses. Ten WaveOne Primary files (25/.08) and ten One Shape NG files (25/.06) were used for instrumentation of mesial root canals of mandibular molars. Each instrument was cleaned and sterilized after the preparation of two root canals and then reused. The time spent for instrumentation of each root canal was recorded and analyzed by using t-test. Microphotographs of the surface of the instruments were taken with a scanning electron microscope at different magnifications (i.e. 18x, 160x, 500x and 1000x) and at a distance of 4 mm from the instrument’s tip before being analyzed by four observers. Presence of disruption of cutting edge, crack, craters and unwinding was assessed and submitted to Fisher’s exact test. Both systems showed manufacturing defects and cracks. The presence of disruption of cutting edges was major in WaveOne files from the sixth use, whereas One Shape NG files showed more unwinding. Only preparation time using WaveOne files was influenced by the number of uses. One can conclude that WaveOne files had more disruption of cutting edges and took longer time for root canal preparation from the sixth use onwards, whereas the One Shape NG files showed more unwinding at the tenth use.

Apical Transportation and Centering Ability After Root Canal Filling Removal Using Reciprocating and Continuous Rotary Systems: A CBCT Study.

Objective To evaluate the apical transportation and centering ability promoted by reciprocating and continuous rotary systems after root canal filling removal. Materials and Methods After obturation, 40 mesial root canals of mandibular molars were distributed into four groups ( n = 20) for filling material removal: PTU group–F2 instrument (25.08) of ProTaper Universal system; R25 group–R25 instrument (25.08) of Reciproc system; X2 group–X2 instrument (25.06) of ProTaper Next system and X3 group–X2 instrument (25.06) of ProTaper Next system, followed by X3 instrument (30.07). Cone-beam computed tomographic analysis was performed before and after filling material removal for acquisition of apical images. Apical transportation (AT) and its direction, and centering ability (CA), were assessed using the equations AT = (X1–X2)–(Y1–Y2) and CA = (X1–X2/Y1–Y2 or Y1–Y2/X1–X2), respectively. Data were submitted to the nonparametric Kruskal–Wallis and Dunn multiple comparison tests ( p < 0.05) for statistical analysis. Results There was no statistically significant difference among groups for AT ( p > 0.05), with a tendency toward transportation in the distal direction. Also, there was no statistically significant difference among groups regarding CA ( p > 0.05). Conclusions The different systems, including ProTaper Next, caused AT within the acceptable clinical limit after filling removal. In addition, none of the tested systems presented adequate CA.

Accuracy of High-resolution Small-volume Cone-beam Computed Tomography in Detecting Complex Anatomy of the Apical Isthmi: Ex Vivo Analysis

IntroductionThe purpose of this study was to evaluate the accuracy of small-volume cone-beam computed tomography (CBCT) to detect and measure isthmi in the apical root canals of mandibular molars by using micro–computed tomography (μ-CT) as the reference standard. MethodsForty mandibular first molars selected on the basis of μ-CT scan and presenting isthmi in the apical 3-mm mesial roots were scanned by using the highest-resolution settings of a small-volume CBCT unit. Isthmi lengths were measured and compared between both μ-CT and CBCT images to study the accuracy of CBCT readings. Quantitative data for sensitivity rate were depicted as percentage value with 95% confidence interval. Results were analyzed by using linear regression between true lengths (μ-CT) and CBCT lengths, Bland-Altman plot and t test, at α = 0.05. ResultsCBCT sensitivity for isthmi detection was 65% (95% confidence interval, 0.4667–0.8333). An average of 74.7% of the lengths could be measured, and differences among the lengths in μ-CT and CBCT were significant (P < .05; mean, 0.756 ± 0.655; t test), showing that there was no agreement between both methods. ConclusionsAccuracy of identifying apical isthmi of mandibular molars was highly influenced by the evaluation method. Small-volume CBCT imaging could not detect and measure apical isthmi length accurately. Moreover, using high-resolution settings in CBCT, it was not reliable to forecast the actual apical root canal anatomy.

Antibacterial Efficacy of Synthetic and Natural-Derived Novel Endodontic Irrigant Solutions

The aim of this study was to compare the efficacy of grape seed extract (GSE), calcium hypochlorite [Ca(ClO)2], and sodium hypochlorite (NaOCl) irrigant solutions with rotary or reciprocating instrumentation for disinfection of root canals inoculated with Enterococcus faecalis. The mesiobuccal root canals of mandibular molars were prepared and inoculated with Enterococcus faecalis for 21 days. The roots were then randomly divided into the following eight experimental groups (n=11) according to the instrumentation technique and disinfection protocol: ProTaper Next or Reciproc R25 with sodium chloride (control group), 6% NaOCl, 6% Ca(ClO)2, or 50% GSE used for irrigation during instrumentation. The antimicrobial activity was determined on the basis of a reduction in colony-forming units (CFUs) counted on bacterial samples collected before and after root canal instrumentation and expressed as a percentage of reduction. Data were evaluated by two-way ANOVA followed by Tukey HSD post-hoc tests (p<0.05). No significant differences were observed in bacterial reduction between the ProTaper Next and Reciproc R25 systems (p>0.05), regardless of the irrigant solution used. Furthermore, all active solutions (6% NaOCl, 50% GSE, and 6% Ca(ClO)2) showed similar potential to reduce bacterial counts (p>0.05) and were significantly more effective than sodium chloride (control) (p<0.05). The results suggest that the GSE and Ca(ClO)2 have potential clinical application as irrigant solutions in endodontic therapy since they present bactericidal efficacy against Enterococcus faecalis.

Impacts of Contracted Endodontic Cavities on Primary Root Canal Curvature Parameters in Mandibular Molars

IntroductionThe purpose of this study was to provide information regarding the debate on contracted endodontic cavities (CECs); their impacts on angle, location, and radius of the primary canal curvature (PCC) were assessed in type IV mesial root canals of mandibular molars at different stages of instrumentation. Impacts on treatment time were also assessed. MethodsTwenty-four teeth were matched by radiographic and micro–computed tomographic criteria and accessed via CECs (CEC, n = 12) or nonextended traditional endodontic cavities (TECs, n = 12). PCC parameters were radiographically determined using a repositioning apparatus before glide path preparation (PI), after glide path preparation, and after final instrumentation (FI). Instrumentation was performed with PathFiles (13/.02, 16/.02; Dentsply Maillefer, Ballaigues, Switzerland) and ProFile Vortex files (Dentsply Tulsa Dental Specialties, Tulsa, OK) to size 30/.04 at the working length under copious irrigation. Changes in PCC were measured with ImageJ (National Institutes of Health, Bethesda, MD). The instrumentation time was recorded. Data were analyzed with 2-way repeated measures analysis of variance (α < .05) and Tukey honest significant difference tests. ResultsA significant (P < .001) decrease in the mean angle and increase in the mean radius were detected at each instrumentation stage for both CECs (angle: PI = 42.57°± 8.00°, FI = 32.61°± 5.17°; radius: PI = 6.48 ± 1.81 mm, FI = 10.55 ± 1.48 mm) and TECs (angle: PI = 38.80°± 7.15°, FI = 30.08°± 6.99°; radius: PI = 6.97 ± 2.31 mm, FI = 11.01 ± 2.20 mm). PCC location shifted apically (P < .001). Changes in PCC parameters did not differ significantly between CECs and TECs (P > .05). The treatment time was significantly (P < .0001) longer for CECs (83.17 ± 6.71 minutes) than for TECs (33.18 ± 9.20 minutes). ConclusionsInstrumentation of curved mesial canals reduced the severity and abruptness of PCC and shifted the PCC location apically similarly in mandibular molars with CECs and those with nonextended TECs. The extended treatment time with CEC merits consideration when debating CECs versus TECs.

Mandibular mesial root canal morphology using micro-computed tomography in a Brazilian population.

To assess the root canal system morphology of mandibular molars by means of micro-computed tomography, one hundred and four mandibular first molars were investigated and the following were evaluated: Vertucci's classification, the presence of lateral canals and the number of apical foramina. Mesial root canal surface area and volume were obtained by the CTAn and CTvol software. The type of root canal isthmi was classified applying the Hsu & Kim and Fan criteria. The most frequent root canal configuration was Vertucci type IV (46.2%). Lateral canals were observed in all root canal thirds and the average number of apical foramina was 3.15 ± 1.77. Regarding the presence of isthmus, Types I and II were the most prevalent, according to Hsu & Kim and Fan et al. criteria, respectively. Mesial root canals of mandibular molars showed high morphology variability.

Prevalence of technical errors and periapical lesions in a sample of endodontically treated teeth: a CBCT analysis.

The aims of this study are to identify the most frequent technical errors in endodontically treated teeth and to determine which root canals were most often associated with those errors, as well as to relate endodontic technical errors and the presence of coronal restorations with periapical status by means of cone-beam computed tomography images. Six hundred eighteen endodontically treated teeth (1146 root canals) were evaluated for the quality of their endodontic treatment and for the presence of coronal restorations and periapical lesions. Each root canal was classified according to dental groups, and the endodontic technical errors were recorded. Chi-square's test and descriptive analyses were performed. Six hundred eighty root canals (59.3%) had periapical lesions. Maxillary molars and anterior teeth showed higher prevalence of periapical lesions (p < 0.05). Endodontic treatment quality and coronal restoration were associated with periapical status (p < 0.05). Underfilling was the most frequent technical error in all root canals, except for the second mesiobuccal root canal of maxillary molars and the distobuccal root canal of mandibular molars, which were non-filled in 78.4 and 30% of the cases, respectively. There is a high prevalence of apical radiolucencies, which increased in the presence of poor coronal restorations, endodontic technical errors, and when both conditions were concomitant. Underfilling was the most frequent technical error, followed by non-homogeneous and non-filled canals. Evaluation of endodontic treatment quality that considers every single root canal aims on warning dental practitioners of the prevalence of technical errors that could be avoided with careful treatment planning and execution.

Micro-CT Evaluation of Gutta-Percha Removal by Two Retreatment Systems.

Introduction:The aim of the present ex vivo research was to compare the remaining filling material and the volumes of dentine removed after retreatment of curved canals with two rotary systems naming ProTaper Universal Retreatment and Mani NRT-GPR using micro-computed tomography (micro-CT). Methods and Material: Forty mandibular molars containing two completely separated canals, with curvature angle of 25-35° and a curvature radius <10 mm were prepared to the Mtwo instrument 35/0.04 and filled with warm gutta-percha and AH-Plus sealer. The teeth were randomly divided into 2 groups (n=20), according to the retreatment system evaluated: ProTaper Universal Retreatment (PR group) or Mani NRT-GPR (MR group). Retreatment was considered complete when the working length was reached and when smooth dentinal walls were observed, with no evidence of filling material adhered to instruments or in the irrigating solution. Preoperative and postoperative micro-CT images were obtained with an isotropic voxel size of 11.88 µm to observe the volume of residual filling material in the canals and dentine removed after retreatment. Statistical analysis was performed by Student’s t-test (P<0.05). Results:The mean percentage of remaining filling material was 12.96% for PR group and 24.26% for MR group (P=0.0056). The percentage of dentin removal was greater in the PR group (5.02%) than MR group (1.36%) (P=0.0028). Both systems failed to completely remove the filling material from the canals. Conclusion: ProTaper Universal Retreatment system was more effective than Mani NRT-GPR system regarding removal of root filling material and also removed significantly more dentine after retreatment of curved mesial root canals of mandibular molars.

Comparison of PathFile and ProFinder Systems to Create a Glide Path in Curved Root Canals.

Objective:Root canal shaping is as important as irrigation and filling when attempting to obtain a high success rate in endodontic treatment. The creation of a glide path before the use of rotary instruments reduces the risk of posterior iatrogenic errors. The objective of the present study was to evaluate instrumentation time and root canal transport after using 2 different glide path rotary systems.Methods:In total, 60 mesiobuccal root canals of mandibular molars, with curvature angles between 11° and 82°, were standardized to measure 15 mm. The specimens were divided into 2 groups, depending on their angles of curvature (11º–38º and 39º–82º), and further divided into 4 groups (n=15). Two groups were instrumented using the PathFile system and the other 2 using the ProFinder system. The angle and radius of curvature were measured at the most abrupt angle of curvature before and after instrumentation. Both measurements were analyzed and compared using AutoCAD software to determine canal transportation. Curvature angles were compared using Student’s t test and the radii of curvature using the Wilcoxon test. The time for instrumentation was also evaluated using Student’s t tests.Results:There were no statistically significant differences between the two systems with respect to root canal transport (P>0.05); however, the ProFinder system took a longer time to create a glide path (P=0.004).Conclusion:Both systems were equally effective in creating a glide path; however, the PathFile system proved to be faster than the ProFinder system.

Evaluation of Mesial Root Canals of Mandibular Molars Obturated with Gutta-Percha and Resilon Techniques

Objective:The aim of this study was to evaluate the obturation of mesial root canals of mandibular first molars performed with different filling techniques and materials (gutta-percha and resilon).Methods:Seventy-eight mesial root canals of human mandibular first molars were prepared using the K3 rotary system, and the apical preparation was set up to size 35.04. The root canals were obturated with single cone, System B, Thermafil and Real Seal 1 techniques using either gutta-percha/ThermaSeal Plus (n=13) or Resilon/Real Seal SE (n=13). Rhodamine B dye was incorporated into the sealers. Each specimen was horizontally sectioned at 2 milimeters (mm), 4 mm and 6 mm from the apex, and the samples were examined under a stereomicroscope to evaluate the presence and type of isthmuses and the percentage areas of gutta-percha/Resilon, sealer and voids. Confocal laser scanning microscopy (CLSM) was used to evaluate the sealer penetration into dentinal tubules. The Kruskal-Wallis and Dunn’s tests were used to analyse the stereomicroscope data, while the ANOVA and Tukey tests were used to analyse the CLSM data (P<0.05).Results:Thermafil and Real Seal 1 fillings showed more gutta-percha/Resilon and less sealer (P<0.05) at the 2 mm level, but the percentage of voids was similar in all groups (P>0.05). At the 4 mm level, more sealer (P<0.05) was found in the single cone groups using both materials. The System B groups exhibited better performance at the 6 mm level. The percentage of sealer penetration showed no statistically significant differences among the obturation techniques for all evaluated levels. Similar results (P>0.05) were found for both material/sealers.Conclusion:None of the materials or techniques completely filled the mesial root canals of mandibular molars, but the plasticised techniques were more efficient. The obturations using both materials and sealers were similar.

Effect of canal preparation with TRUShape and Vortex rotary instruments on three‐dimensional geometry of oval root canals

The purpose of this study was to assess the geometry of non-round root canals after preparation with TRUShape (a novel instrument with s-shaped longitudinal design) in comparison to conventional rotary instrumentation using micro-computed tomography. Twenty distal root canals of mandibular molars were randomly distributed in two groups to be shaped with either TRUShape or Vortex rotaries. Percentages of unprepared surface and volume of dentin removal for the entire canal and for the apical 4mm were calculated. Canal transportation and the structure model index (SMI) were assessed. Data were compared with Student t-tests. Shaping with both techniques resulted in similar prepared surface and volume of dentin removed, as well as the extent of canal transportation. The SMI shape factor was significantly lower for TRUShape preparations (P=0.04) suggesting less rounding during rotary preparation. Although both instruments were suitable for the preparation of oval canals, TRUShape appeared to better conform to the original ribbon-shaped anatomy.

Retrospective Assessment of Healing Outcome of Endodontic Treatment for Mandibular Molars with C-shaped Root Canal.

A thorough knowledge of the anatomic morphology of the root canal system is necessary for the long-term success of the root canal therapy. The occurrence of C-shaped root canal configuration is one such variation. Achievement of favorable prognosis after commencing root canal therapy in such teeth is one of the challenges imposed on the endodontist. Hence, we evaluated the healing occurring after endodontic therapy in patients with C-shaped root canals in mandibular molars. The present study was conducted in the Department of Conservative Dentistry of the institution and included assessment of all the patients who underwent root canal treatment of the mandibular first and second molars. Endodontic therapy was performed in all the cases by experienced endodontist. Final postoperative radiographs were taken. Recording of the data of the personal and clinical details of a total of 250 patients was done. All the clinical and radiographic details of the patients, such as tooth location in the jaw, presence or absence of C-shaped canals, status of the pulp tissue, presence or absence of the fractures, and other details of the patients were recorded. Radiographic and clinical examination of the tooth of the patients was done during the baseline visit and further during the follow-up visits. The presence of C-shaped root canals was confirmed using the radiographs. Periapical index (PI) scoring system was used. Categories defined for enlisting the healing after the root canal treatment with the assessment of the PI score. All the results were analyzed by Statistical Package for the Social Sciences software. Vital pulp tissue was encountered in majority of the cases. C-shaped root canal configuration was observed in 40% of the cases included in the present study. After completion of the endodontic therapy, complete crown placement was done in only 22% of the cases. In cases of vital teeth with C-shaped root canals configuration, most of the teeth showed complete healing. Significant results were obtained while comparing the complete coverage crown parameter in relation to the healing process in teeth with C-shaped root canals. In the present study, no significant effect of the C-shaped root canal configuration was found on the healing rate of the endodontic therapy in mandibular molars. Meticulous endodontic therapy with special techniques should be done while preparing teeth with C-shaped root canals.

Prevalence of C-shaped canals in mandibular second and third molars in a central India population: A cone beam computed tomography analysis.

Introduction:To evaluate the prevalence of C-shaped root canals in mandibular molars using cone beam computed tomography (CBCT) in a subpopulation of Central India.Materials and Methods:CBCT scans of patients from diagnostic imaging center were selected in accordance with the criteria given by Fan et al. (2004) for C-shaped canals. A total of 238 CBCT scans fulfilled the inclusion criteria and thereby divided into two groups: Group 1: Images showing C-shaped canal configuration in mandibular second molars. Group 2: Images showing C-shaped canal configuration in mandibular third molars. The frequency and distribution of canals and their configuration along with the position of lingual/buccal grooves in the images were evaluated, and the data was analyzed.Results:CBCT evaluation showed that 9.7% of second molars and 8% of third molars had C-shaped canals. A prominent buccal groove was seen in these teeth. The data showed a significant difference (P = 0.038) for the presence of such anatomy on the right side for mandibular third molars.Conclusion:The study showed a significant prevalence of C-shaped canal configuration in the subpopulation studied.
