Impact of the COVID-19 Pandemic on Emergency Room Attendances: Experience at a Tertiary Care Centre in Pakistan

Objective: Since the emergence of the COVID-19 pandemic, several reports have shown markedly reduced Emergency Department utilization. We aimed to examine the trends in Emergency Department visits during the early phase of the COVID-19 pandemic in a tertiary care center in Pakistan. Methods: A retrospective, observational analysis of Emergency Department visits was performed at Shifa International hospital over the 3-month study period (March to May 2020) in the early phase of the COVID-19 pandemic. The same 3-month period in 2019 was used as a reference. Total visits, as well as priority level at arrival, diagnosis, and disposition at discharge along with age and gender, were recorded and compared, and the changes in Emergency Department visits were assessed descriptively. Results: During the study period, the total Emergency Department volume was 40% lower than that of 2019. What Have Been the Public Health Impacts of Cannabis Legalisation in the USA? A Review of Evidence on Adverse and Beneficial Effects

To summarise empirical research on the adverse and beneficial public health impacts of cannabis legalisation in states in the USA. The most consistent finding from surveys of drug use is that the legalisation of medical cannabis has so far not had an effect on rates of use or cannabis use disorders among youth, but it has increased the frequency of use among adult cannabis users. There are conflicting findings on the impact of legalisation of medical and recreational cannabis use on the following: cannabis use disorders in adults, rates of motor vehicle accidents in which the driver was impaired by cannabis, rates of suicide, and opioid-related harms. The legalisation of recreational cannabis use has increased emergency room attendances for cannabis-related medical conditions (acute adverse psychological effects, hyperemesis, and accidental poisoning of children). There is no evidence to date on the impact of medical or recreational legalisation on the prevalence of mental disorders such as psychoses, depression, and anxiety. There is suggestive evidence that cannabis legalisation is associated with a range of public health consequences. However, current evidence is limited in the capacity to confidently conclude that these changes are the result of cannabis legalisation. The impacts on public health may take some years to become apparent.

Clinical deterioration during neonatal transport in California.

ObjectiveIdentify clinical factors, transport characteristics and transport time intervals associated with clinical deterioration during neonatal transport in California.Study designPopulation-based database was used to evaluate 47,794 infants transported before 7 days after birth from 2007 to 2016. Log binomial regression was used to estimate relative risks.Results30.8% of infants had clinical deterioration. Clinical deterioration was associated with prematurity, delivery room resuscitation, severe birth defects, emergent transports, transports by helicopter and requests for delivery room attendance. When evaluating transport time intervals, time required for evaluation by the transport team was associated with increased risk of clinical deterioration. Modifiable transport intervals were not associated with increased risk.ConclusionOur results suggest that high-risk infants are more likely to be unstable during transport. Coordination and timing of neonatal transport in California appears to be effective and does not seem to contribute to clinical deterioration despite variation in the duration of these processes.

Fresh frozen plasma for on-demand hereditary angioedema treatment in South Africa and Iran

BackgroundInternational guideline-recommended on-demand treatments for hereditary angioedema (HAE) include: C1-esterase inhibitor (plasma-derived or recombinant), or bradykinin-receptor antagonists. In most low- and middle-income countries (LMIC) these products are not registered or are unaffordable. Solvent-detergent, fresh or freeze-dried plasma therapy is thus the only available on-demand treatment in these settings; but published data on efficacy and safety are limited. This study evaluated the efficacy and safety of on-demand plasma treatment of acute HAE in two LMICs. MethodsA retrospective folder or patient registry review of acute swelling episodes necessitating emergency room attendance amongst known HAE patients was conducted at treatment centers in South Africa and Iran. Data collected included the site of angioedema, timing and amount of fresh frozen plasma (FFP) administered, time-to-resolution, hospital length of stay and adverse events. ResultsThere were 176 acute swelling episodes amongst 43 HAE patients; 98 were treated with FFP. The face, upper airway, and abdomen were involved in 15.3% (15/98), 53.1% (52/98) and 29.6% (29/98) of episodes treated with FFP respectively. Median (interquartile range ([IQR]) of FFP administered was 400 (280–560) mLs. In all episodes except two, FFP led to resolution, with median (IQR) hours to resolution 4 (2–12). Five transfusion reactions occurred, with one case of anaphylaxis and no deaths; giving an adverse reaction rate of 5%. Differences between South Africa and Iran included: (1) proportion of HAE type II(2) median (IQR) hours to FFP administration and hospitalization, (3) number of intubations after FFP infusion. Healthcare cost for FFP treatment was USD369- 791 in South Africa and USD275-550 in Iran, largely influenced by hospital length of stay. ConclusionsPlasma (fresh-frozen) remains the only available effective on-demand treatment for acute HAE in many countries. FFP is effective and safe, but time-to-resolution is slower and adverse events are more frequent than published data on targeted therapies. Overall healthcare cost of FFP approaches that of targeted therapies – now available through global access programs – when hospitalization is prolonged.

Greetings from Emily! The effects of personalized greeting cards on tipping of hotel room attendants

Purpose In hotels, room attendants are often invisible to hotel guests. This study aims to understand how customers would increase their voluntary tips when there was less or no personal interaction and communication between customers and service providers. Specifically, the purpose of this study is to investigate whether providing different greeting cards in hotel rooms would affect hotel guest tipping behavior. Design/methodology/approach A field study was conducted in an upscale independent hotel. Four types of greeting cards through two personalized factors, perceived effort and personalization, were placed in the hotel rooms. The tipping amount for each room-night was recorded during the data collection. Findings There were 3,285 room-nights tip records collected in this study. The results indicated that non-personalized housekeeping greeting cards did not increase the likelihood of guests to tip, but they may increase the average tipping amount; the personalization of greeting cards from room attendants had positive effects on guest tipping behavior; the hand-written greeting card and name-introduction greeting card were predictors that can significantly increase the likelihood of hotel guests to tip. Research limitations/implications The empirical research results support social presence theory. With more consistent tipping in hotel rooms, attendants may be able to predict tips through their job performance; thus, creating a win-win in the lodging industry. Originality/value This study contributes to understanding guest-tipping behavior in the hotel rooms.

Fully Automated Classroom Attendance System

<p class="0abstract">This paper is based on study and implementation of the pilot project for customized, automated, highly secured class room management system known as JIC CMS (Jubail Industrial College Class room management System). In this research paper, the strategies, design, development and implementation of a pilot project for class room attendance management systems is explained in detail. As a pilot project, mobile app is built on Android platform using Xamarin programming. Prototype of proposed system provides high level of authentication by embedding face recognition and biometric verification together with RFID system. Though given model features are specially designed for our college needs, but this model can be modified for general usage and can be applied for all academic levels i.e. school, college, universities. Among the other features of system, one important feature is selection of system to work in smart mode in which system closes after predefined time and uploaded automatically to the web server.</p>

Influence of Gamma-Hydroxybutyric Acid-Use and Gamma-Hydroxybutyric Acid-Induced Coma on Affect and the Affective Network

Background: Gamma-hydroxybutyric acid (GHB) is a drug of abuse associated with increased emergency room attendances, due to GHB-induced comas. Withdrawal from GHB often increases social anxiety and is linked to alterations in emotion processing. However, little is known about the effects of GHB-use and GHB-induced comas on affect regulation in humans. Objectives: We aimed to assess the effect of GHB-use and GHB-induced comas on the affective network. Method: We recruited 27 GHB users with ≥4 GHB-induced comas (GHB-Coma), 27 GHB users without a GHB-induced coma (GHB-NoComa), and 27 polydrug users who never used GHB (No-GHB). Participants completed self-report questionnaires assessing negative affect (depression, anxiety and stress) and performed an emotional face matching task during functional magnetic resonance imaging to probe activity of the amygdala and the hippocampus. Results: The GHB-Coma group reported higher levels of depression, anxiety, and stress; showed decreased activity of the hippocampus; and increased functional connectivity of the left hippocampus with the left fusiform gyrus and a cluster on the left temporal-parietal-occipital junction, when compared with the 2 other groups. The GHB-NoComa group showed decreased functional connectivity of the left hippocampus with the amygdala in comparison with the No-GHB group. Conclusions: GHB-use but in particular GHB-induced comas, are associated with altered emotion identification and hippocampal functioning. Awareness campaigns are required to raise consciousness about the adverse effects of GHB-induced comas on affect regulation, despite the absence of subjective side effects.

An investigation of healthcare utilization and its association with levels of health literacy in individuals with chronic pain.

Chronic pain patients are frequent and recurrent users of health services, which may have an impact on levels of health literacy (HL). Therefore, the aim of the present study was to investigate associations between healthcare utilization and varying levels of HL in individuals with and without chronic pain. A cross-sectional questionnaire was distributed in three pain clinics in Dublin, Ireland, comprising a demographic section, a validated HL assessment tool (Newest Vital Sign) and self-reported healthcare utilization in the previous year (i.e., general practitioner [GP] visits, accident and emergency room attendance, hospital services and allied health services). Patients with chronic pain, and a control group (no pain) were recruited. Overall, 262 participants were recruited: those with chronic pain (n=131) and controls (n=131). Those in the chronic pain group were more likely to be female (p=0.004), have less education (p=0.01), be unable to work (p<0.001), have a lower monthly income (p=0.001), be more likely to have a medical card (i.e., free access to public health services) (p=0.002) and have a greater number of comorbidities (p<0.0001). Although bivariate analyses demonstrated increased healthcare utilization in chronic pain patients (i.e., GP visits, hospital services and allied therapies; p<0.05), there was no difference in HL levels between groups in multivariate analysis (chronic pain: 54%, n=71; control group 49%, n=64; p=0.39). Higher educational attainment, greater levels of income and being younger remained independently associated with higher levels of HL. Further research is needed to understand the nature of how HL is acquired, both from individual and organizational perspectives. Once this has been established, it may facilitate the development or advancement of current HL-sensitive management strategies.

Recreational use of GHB is associated with alterations of resting state functional connectivity of the central executive and default mode networks.

Gamma‐hydroxybutyrate acid (GHB) is a recreational drug with a high addictive potential. Severe side effects such as GHB‐induced coma are common and linked to increased emergency room attendances. Task‐based functional‐imaging studies have revealed an association between the regular use of GHB and multiple GHB‐induced comas, and altered neurocognitive function. However the effects of multiple GHB‐induced comas and regular GHB‐use on intrinsic brain connectivity during rest remain unknown. The study population consisted of 23 GHB‐users with ≥4 GHB‐induced comas (GHB‐Coma), 22 GHB‐users who never experienced a GHB‐induced coma (GHB‐NoComa) and 24 polydrug users who never used GHB (No‐GHB). Resting‐state scans were collected to assess resting‐state functional‐connectivity within and between the default mode network (DMN), the bilateral central executive network (CEN) and the salience network (SN). The GHB‐NoComa group showed decreased rsFC of the right CEN with a region in the anterior cingulate cortex (p FWE = 0.048) and decreased rsFC between the right CEN and the DMN (p FWE = 0.048) when compared with the No‐GHB group. These results suggest that regular GHB‐use is associated with decreased rsFC within the right CEN and between the right CEN and the DMN. The presence of multiple GHB‐induced comas is not associated with (additional) alterations in rsFC.

Avoidance of unnecessary hospital admissions through the implementation of a digital health programme

Introduction: 62% of hospital bed days are occupied by older adults 65 and older in the UK and it is estimated that there is a cost to the NHS of £820 million1. Elderly patients admitted to hospital have worse morbidity and mortality outcomes and often have a poorer experience2,3,. To reduce emergency admissions Tameside and Glossop Integrated Care NHS Foundation Trust developed a Digital Health Service, which supports frail people to stay within the community where their care can be managed, avoiding an Emergency room attendance and potential emergency admission. Practice change: To enhance the value of care provided to the local community a digital health service has been implemented in care homes. The service provides digital technology that enables care home staff to access a hospital specialist for advice and guidance on residents who have a deterioration in health before considering an Ambulance/ GP visit. Aim and theory of change: The aim is to use technology to enable care home staff to maintain older people with complex co-morbidities, within their home setting and avoiding unnecessary hospital admissions. Timeline: The programme was launched in March 2017 and will run for 3 years Highlights: In the period April and September 2017 94 Emergency and 258 GP attendances were avoided. Total hospital bed days saved 1268. Deflection rate of Emergency room attendance is currently 70% Comments on sustainability: The programme has now been expanded across the whole of the residential care home sector and will launched as a 24/7 service this year. Limitations: Awaiting more long term finding, ongoing evaluation is required to ensure that early results are sustainable overtime. Comments on transferability A joint End of Life bid is being developed. Conclusions: Elderly people suffer worsening health and poor experience when in hospital 3, 5. There is also a significant financial cost to systems treating frail elderly patients in hospitals 3. The Digital health programme enabled care staff to access acute hospital specialists for advice and guidance, which has led to a significant deflection in attendance. Discussions: The programme has proven successful at reducing admissions and in Emergency room attendance within the population group, data appears to show that the service has the potential to support people to remain within their place of residence rather than be treated in hospital. Achieving Academic Excellence in Private unaided Engineering Colleges in India

In this paper an attempt has been made to explain the strategies adopted to improve the overall performance of a private engineering college. For this purpose, a comprehensive model with a two pronged strategy has been suggested. First, the focus was on the student related issues such as the improvement of class room attendance, student mentoring program, student feedback etc., This was followed by the steps taken to enhance the examination results including the supply of question banks. The second strategy is related to the teachers. How the academic leadership helped the teachers in improving not only the classroom teaching but also in implementing several other student related programs is explained. The details regarding the tremendous improvement of R&D output due to the motivation given by the leadership are also explained. The procedures used for attaining academic excellence using various methodologies have been discussed in detail so that interested people may think of adopting.

&lt;p&gt;&lt;em&gt;This study aims to : ( 1 ) determine how the motivation and performance of the Department Housekeeping room attendant at the Novotel Nusa Dua Bali ( 2 ) determine the relationship between motivation and performance room attendant and how much the relationship. This research was conducted at the Novotel Nusa Dua Bali hotel and resident. This type of research is descriptive quantitative by using primary data originating from field research and secondary data through library research. Data was collected through observation, interviews , questionnaires and literature. The information consists of Floor supervisors and Room Attendant at the Novotel Nusa Dua Bali. The data analysis technique used is descriptive quantitative methods of data analysis product moment correlation pearson to calculate the relationship between two variables, motivation and performance of the second variable interval scale. While the indicator of the performance is loyalty, work, teamwork, ability, initiative and responsibility. From the research results between motivation and performance have a correlation coefficient of 0.71 coefficient interval 0.71 to 0.90 which has a high level of relationship or a strong and reliable. The calculation result of hypothesis test obtained t counted = 4.157 &amp;gt; t table = 2,110 then accepted Ha Ho is rejected. This means that between motivation and performance room attendant significant correlation. Exposure to organophosphate flame retardants of hotel room attendants in Wuhan City, China

Indoor environments provide sources of exposure to organophosphate flame retardants (PFRs), which are artificially synthesized fire-protecting agents used as additives in interior products. As public spaces, hotels are required to meet stricter fire-precaution criteria. As such, room attendants may be exposed to higher levels of PFRs. Our goal was to characterize the exposure of hotel room attendants to PFRs by measuring metabolites in their urine and the corresponding parent PFRs in dust and hand-wipes collected from 27 hotels located in Wuhan City, China. The exposure of the attendants was found to be omnipresent: urinary metabolites of PFRs, such as DPHP (diphenyl phosphate), BDCIPP (bis(1,3-dichloro-2-propyl) phosphate), and DoCP (di-o-cresyl phosphate) & DpCP (di-p-cresyl phosphate) were detected with high frequency (87%, 79% and 87%, respectively). We observed that metabolites in post-shift urine were consistently present at higher levels than those in the first morning voids (p < 0.05 for BDCIPP and DPHP). Regarding external exposure, 10 PFRs were determined in both dust samples and hand-wipes, with TCIPP (tris(2-chloroisopropyl) phosphate) being the most abundant compound in both matrices. The levels of PFRs in hand-wipes and dust samples were not correlated. PFRs in dust and their corresponding urinary metabolites were not significantly correlated, while a moderate significant correlation of TDCIPP (tris(1,3-dichloro-2-propyl) phosphate) in hand-wipes and its urinary metabolite, BDCIPP, was observed in both morning void samples (p = 0.01) and post-shift urine (p = 0.002). Moreover, we found that participants from high-rise buildings (defined as > 7 stories) had significantly higher BDCIPP and DPHP concentrations than those from low-rise buildings. A possible reason is that high-rise buildings may use high-grade fireproof building materials to meet stricter fire restrictions. Overall, these results indicate that PFRs exposure in hotels is a contributor to the personal exposure of hotel room attendants.

Incidence and pattern of dog bite injuries treated in the emergency room of a teaching hospital South East Nigeria

Dog bite injury treated in the emergency room varies from and within subregions in pattern and potential risk of transmission of rabies. This variation has implications in its morbidity and mortality. The aim of this study was to determine the incidence and pattern of dog bite injuries treated in a teaching hospital emergency room setting of a developing country. This was a retrospective study of the entire patients with dog bite injury treated in the emergency room of Federal Teaching Hospital Abakaliki from January 2006 to December 2015. Dog bite injury necessitated visit in 74 patients with an incidence of 2 per 1000 emergency room attendances, and a male to female ratio of 1:1.1. The mean age of the patients was 25.5 ± 1.87 years, and peak age group incidence was 5-9 years. Lower extremity was involved in 77.5% of the injuries, and buttock was the predominant site of injury in 0-4 years old. Fifty-one (68.9%) owned dogs and 23 (31.1%) stray dogs were involved in the attack. There was unprovoked attack in 81.1% of cases, and 51 (68.9%) sustained Grade II injury. Twenty-eight (37.8%) of the dogs had anti-rabies vaccination. Fifty-four (73%) patients had no prehospital care while 64 (86.5%) received postexposure anti-rabies vaccine. Majority of the patients 73 (98.7%) recovered fully. One (1.4%) patient that presented with clinical rabies self-discharged against medical advice. The incidence of dog bite injury is within worldwide range though the female gender bias is unprecedented. We recommend preventive strategies based on the observed pattern and improvement in the rate of prehospital care and higher coverage of anti-rabies vaccination of dogs.

Hotel room attendants’ delivery of quality service

Illuminating the physical difficulties experienced by hotel room attendants in delivering service quality at five-star hotels on the Gold Coast region of South East Queensland, Australia, our research was founded on socialist-feminist and critical theory epistemologies. In-depth interviews were conducted with a sample of 46 hotel room attendants. We used a qualitative, social-constructivist, grounded theory methodology to render the empirical material into a basic social structural process of tasking. Tasking offers a conceptual framework of room attendants’ daily employment experiences as they transform hotel rooms to prescribed standards. Tasking illustrates the high physical demands placed on room attendants, tantamount to exploitation from their own perspectives. Tasking contributes to research on employment experiences in hospitality, particularly on hotel praxis, delivery of quality customer service and the operational demands placed on room attendants. Exposure level of ergonomic risk factors in hotel industries

Ergonomic Risk Factors (ERFs) which contribute to Musculoskeletal Disorders (MSDs) among room attendants were considered as a problem or trouble since these ERFs would affect their work performance for hotel industries. The purpose of this study was to examine the exposure level of ERFs among room attendants in hotel industries. 65 of respondents were obtained from selected hotels in Peninsular Malaysia. Data were collected by direct observation via Workplace Ergonomic Risk Assessment (WERA) and Quick Exposure Checklist (QEC). There were 36 males and 29 females room attendants involved throughout the research. Most of room attendants experienced high exposure level for back, leg, forceful and vibration based on the exposure level evaluation through WERA while QEC results showed that all room attendants were found to have moderate exposure level for risk factors including back for movement use, shoulders/arms, wrists/hands and neck. All the results obtained showed that the related ERFs for MSDs were associated and essential ergonomic interventions are needed in order to eliminate risk of exposures to MSDs among room attendants in hotel industries.

Examining job stress and burnout of hotel room attendants: Internal marketing and organizational commitment as moderators

ABSTRACTHousekeeping is a support department that helps provide and support quality service and products for hotel customers. Room attendants, however, face as much job stress as other staff. Therefore, this study examines the causal relationships among job stress, job burnout, and turnover intentions, while also verifying internal marketing and organizational commitment as moderators for the effect of job stress on burnout. Study results indicate that room attendants who suffer job stress do have higher job burnout, which, in turn, affects their turnover intentions. Further, internal marketing and organizational commitment significantly moderates job stress and job burnout.

Real-life characteristics of asthma inhaler device use in South Korea

Background and Aims: Historically, dry powder inhalers (DPIs) were considered to provide better airway distribution, easier identification of empty devices, and easier handling when compared to pressurized metered dose inhalers (pMDIs). Prior research into the major handling errors with inhaler device use has shown that errors result in comparable impairment of asthma control in both DPIs and pMDIs. Our own research has demonstrated that patients who are prescribed similar types of preventer inhaler devices to their reliever have better asthma control. Patients prescribed pMDI inhaled corticosteroid/long acting beta agonist relievers could benefit from switching from a DPI to a pMDI. The Health Insurance Review and Assessment (HIRA) database provides coverage of medical claims for over 50 million people in Korea and offers the opportunity to study asthma control on a national basis. The aim of this study was to provide a review of the possibilities of the Korean HIRA database in preparation for a study to investigate the effect on asthma control when patients switch inhaler types. Methods: Methodology from previous literature describing the HIRA database was analysed. We focused on the identification of asthma patients, their clinical characteristics (e.g. exacerbations, comorbidities), real-life medication switching behaviour, healthcare resource use (including medication) and associated costs. Results: Patient medical history could be constructed from primary and secondary diagnosis associated with individual database entries. Asthma exacerbations could be proxied by a prescription of acute oral corticosteroids, hospital admission or emergency room attendance associated with a diagnosis of asthma, lower respiratory infection or respiratory failure. Patients could be considered switch patients if they received a prescription of a pMDI after prescription of ≥2 DPI inhalers. Additional variables that were available included medication and hospitalisation cost. Conclusions: The HIRA database will allow studies analysing switch success of inhaler types in terms of persistence, asthma control and healthcare resource utilisation.
