Fetal Cardiology Bioethics: An Innovative New Curriculum for Cardiology Trainees.

Decision-making in fetal cardiology is fraught with ethical issues yet education in bioethics for trainees is limited or nonexistent. In this innovation report, we describe the development of a fetal cardiologybioethics curriculum designed to address this gap. The curriculum was developed to supplement the core curriculum for cardiology fellows and fetal cardiology subspecialty trainees. The series combines didactic and interactive teaching modalities and contains 5 key components: (1) introduction to bioethics and its role in fetal cardiology, (2) counseling and pathways for compassionate terminal care, (3) case vignette-based ethical analysis and discussion cases, (4) fetal counseling considerations for shared decision-making and recommendations, (5) facilitated communications role play. The curriculum was refined using session evaluations from end users. This report describes the innovative curriculum as a starting point for further incorporation and study of bioethical education in pediatric cardiology and fetal training programs.

Does study counselling foster STEM intentions and reduce the STEM gender gap? Evidence from a randomized controlled trial

ABSTRACT Low interest in STEM (science, technology, engineering and mathematics) among high school students in general and women in particular is a highly prevalent phenomenon in many industrialized countries. This study analyses whether study counselling – guiding students to select a major in higher education through self-exploration and fostering their confidence in completing higher education – increases their aspirations toward STEM. We pay particular attention to gender differences in this association. To identify the causal impact of the intervention, we employ a Randomized Controlled Trial (RCT) with pre–post design. Our results show that students participating in the counselling intervention more frequently develop aspirations toward life and environmental sciences subjects such as biology. Moreover, this effect is stronger for female students. In light of these results, we discuss the potential role of study counselling in reducing gender segregation in higher education.

THE ROLE OF GROUP COUNSELING WITH FILOSOFI TERAS IN SOLVING BULLYING BEHAVIOR IN PANTI ASUHAN KELUARGA YATIM MUHAMMADIYAH (PAKYM) SURAKARTA IN 2022

Abstract: This study aimed to find out the role of Group Counseling with Filosofi Teras in solving Bullying behavior in Panti Asuhan Keluarga Yatim Muhammadiyah (PAKYM) Surakarta in 2022. To this end, A 4-cycle guidance and counseling action research was applied. Each cycle consisted of four stages: planning, action, observation, and reflection. The data source in this study were the bullies, orphan keeper, and the caregiver. Interviews, observation, and documentation were performed to collect the data. Techniques and sources of data in this study were triangulated to ensure the validity. The collected data were analyzed using the interactive data analysis technique. In this study, 6 participants were indicated to commit bullying behavior due to their poor regulation of emotion. Following interventions performed in each cycle, this study found that during the pre-cycle percentage of solved bullying behavior was 56.66%, Cycle I was 59,18%, Cycle II was 69.16%, Cycle III was 78.34%, and Cycle IV was 86.66%, respectively. Based on the study result, the group counseling with Filosofi Teras is applicable in overcoming bullying behavior in Panti Asuhan Keluarga Yatim Muhammadiyah (PAKYM) Surakarta in 2022. PSYCHOANALYTIC INDIVIDUAL COUNSELING SERVICE: AN EFFORT TO OVERCOME 11TH-GRADE STUDENTS’ KLEPTOMANIAC BEHAVIORS IN SMKN MATESIH IN THE 2021/2022 ACADEMIC YEAR

Abstrac : This study aimed to see the role of the psychoanalytic individual counseling in overcoming students’ kleptomaniac behaviors in Class XI SMKN Matesih in the 2021/2022 academic year. To this end, A 3-cycle guidance and counseling action research was applied. Each cycle consisted of four stages: planning, action, observation, and reflection. The data were collected using interviews, observation, documentation, and Who am I psychological test. The collected data were analyzed using a qualitative descriptive technique with the technique triangulation. Three students were identified to exhibit mild kleptomaniac disorders, commonly known as hypomania. The following percentage of kleptomaniac behavioral characteristics were obtained in each cycle: Precycle: 46%, cycle I: 44.6%, cycle II: 33.32%, cycle III: 22.8%, and cycle IV: 14.72%. This result shows that psychoanalytic individual counseling is effective in treating individuals with mild kleptomaniac symptoms, allowing them to control the urge to steal. In silico analysis of mutation spectrum of Ehlers–Danlos, osteogenesis imperfecta, and cutis laxa overlapping phenotypes in Iranian population

BackgroundEhlers–Danlos syndrome (EDS), osteogenesis imperfecta (OI), and cutis laxa (CL) are three rare and heterogeneous connective tissue disorders. Patients with these syndromes have similar manifestations and unpredictable prognosis, making a misdiagnosis highly probable. Some of their subtypes are inherited in autosomal recessive patterns, so they are expected to be prevalent in populations like Iran, where consanguineous marriages are common. In the current work, a cohort of Iranian patients with overlapping phenotypes of the EDS/OI/CL and their mutation spectrum was defined. Based on this, in silico analysis was conducted to anticipate further probable genetic variations. Pathogenicity of EDS, OI, and CL variants in Iranian patients was evaluated using Web servers. A protein interaction network was created by String database and visualized using a Python-based library. The Iranome database was used to predict other genetic mutations in all reported genes of EDS, OI, and CL syndromes.ResultsIn the EDS/OI/CL overlap phenotype, 32 variants in 18 genes have been involved. At least 59% of patients were from families with consanguineous marriages. Interaction analysis showed that COL1A1, COL1A2, CRTAP, LEPRE1, PLOD1, and ADAMTS2 have the most significant impact within the protein network of EDS/OI/CL overlap phenotype. Analyzing the Iranome database revealed 46 variants of EDS, OI, and CL genes potentially disease causing.ConclusionThe overlapping phenotype of EDS, OI, and CL syndromes requires genetic testing (e.g., whole-exome sequencing) to reveal respective variants, which helps to diagnose more accurately and manage the disease more effectively. Particularly in populations with high rates of consanguineous marriages, such as Iran, genetic screening plays a crucial role in premarital and prenatal counseling to prevent the transmission of these rare connective tissue disorders.

GUIDANCE AND COUNSELING MANAGEMENT: A SCIENTIFIC APPROACH TO IMPROVING STUDENTS' MENTAL HEALTH

The objective of this research is to explore the management of guidance and counseling as a scientific approach to enhancing students' mental health. The method employed in this study is qualitative research, specifically using a case study design. The choice of a case study is justified as it allows the researcher to gain in-depth insights into the context, processes, and complexities of the phenomenon under investigation. The findings of this research illustrate that the role of guidance and counseling (BK) teachers in state high schools in West Kalimantan, Indonesia, in safeguarding students' mental health is not merely reactive but also proactive. BK teachers apply a scientific approach involving observation and interviews to understand the holistic needs and challenges faced by students. Additionally, they utilize a variety of counseling techniques, including individual counseling, group counseling, cognitive-behavioral approaches, and art and play therapy, to provide tailored support based on individual student needs.

A scientific paper that reviews the literature on the role of group counseling on young people's peer conformity attempts to develop a novel intervariable influence study hypothesis that will be used to future psychology counseling studies. Short-term Changes in Health-related Quality of Life of Patients Undergoing Radical Surgery for Upper Urinary Tract Urothelial Carcinoma: Results from a Prospective Phase 2 Clinical Trial

Background and objectiveThe possible negative impact of radical surgery on patients’ health-related quality of life (HRQoL) plays an important role in preoperative counseling. Here, we analyzed the HRQoL of patients treated for upper urinary tract urothelial carcinoma (UTUC) in the context of a single-arm phase 2 multicenter study, in which the safety and efficacy of a single preoperative intravesical instillation with mitomycin C were investigated. Our objective was to investigate early changes in HRQoL in patients undergoing radical surgery for UTUC and identify factors associated with these outcomes. MethodsPatients with pTanyN0-1M0 UTUC were prospectively included. HRQoL was assessed using the European Organisation for Research and Treatment of Cancer Quality of Life Questionnaire C30 (EORTC QLQ-C30) questionnaire at baseline, and at 1 and 3 mo after surgery. A linear mixed model was used to evaluate the changes in HRQoL over time and identify the variables associated with these outcomes. The clinical effect size was used to assess the clinical impact and level of perceptibility of HRQoL changes for clinicians and/or patients based on given thresholds. Key findings and limitationsBetween 2017 and 2020, 186 patients were included. At baseline, 1 mo after surgery, and 3 mo after surgery, response rates were 91%, 84%, and 78%, respectively. One month after surgery, a statistically significant and clinically relevant deterioration was observed in physical, role, and social functioning, and for the included symptom scales: constipation, fatigue, and pain. An improvement in emotional functioning was observed. At 3 mo, HRQoL returned to baseline levels, except emotional functioning, which improved at 1 mo and persisted to be better than that before surgery. Age >70 yr was associated with worse physical functioning, but better social and emotional functioning. Male patients reported better emotional functioning than females. Postoperative complications were negatively associated with social functioning. Conclusions and clinical implicationsUTUC patients treated with radical surgery experienced a significant, albeit temporary, decline in HRQoL. Three months following surgery, HRQoL outcomes returned to baseline levels. This information can be used to counsel UTUC patients before undergoing radical surgery and contextualize recovery after surgery. Patient summaryWe investigated the changes in quality of life as reported by patients who underwent surgery for upper tract urothelial carcinoma (UTUC). We found that patients experienced a decline in quality of life 1 mo after surgery, but this was temporary, with full recovery of quality of life 3 mo after surgery. These findings can help doctors and other medical staff in counseling UTUC patients before undergoing radical surgery.

Role of Career Counselling in Medical Profession: Perceptions of Medical Students

Objective: To determine the perception of medical students regarding the role of career counselling in medical profession.Methodology: It was a cross sectional study done between April and October 2023, where140 medical students from Central Park Medical College, Lahore were enrolled via convenience sampling. After receiving formal IRB approval, data was collected via self-designed google forms and WhatsApp groups from MBBS students in their first to final academic year who volunteered to participate after piloting with Cronbach Alpha score of 0.81. After collecting data, SPSS software version 26 was utilized for analysis. The outcomes were presented as frequency tables and bar graphs.Results: A total of 140 responses were collected with the average age of the respondents being 21+1.7SD years. About 59 (42.1%) of the students who responded were from 2nd year MBBS, 38 (27.1%) were from 4th year MBBS, 26 (19%) were from 1st year MBBS, 09 (6%) were from 5th year MBBS and remaining 08 (6%) were from 3rd year MBBS. About 60 (42.9%),52 (37.1%), 133 (95%) and 131 (93.6%)of the students said that career counselling helped identify their capabilities. About 130 (93 %), 115 (82 %), and 113(81%) gave positive responses when asked about the impact on gaining perspective, developing strategies, and managing conflicts. There was a significant relationship between the role of career counselling in identifying opportunities and building the CV 109(95%) (p=0.01), understanding interest59(99%) capabilities49(94%) (p=0.00), broadening one's perspective 126(95%) (0.00), adopting the right strategies 126(97%) (0.00), and resolving conflicts 108(95%) (0.00). Conclusion: Career counselling should be conducted in the early years of medical professional's career, so that students can identify their areas of interest in order to choose the appropriate specialization in the future.

Optimizing Recovery After Carpal Tunnel Syndrome Release Surgery: The Role of Counseling in Pain Management and Perioperative Functional Enhancement.

Preoperative patient education is pivotal in improving patient outcomes during the perioperative phase, involving a thorough explanation of what patients can expect. This enhances adherence and reduces perioperative anxiety. In orthopedics, carpal tunnel syndrome, a common and painful hand condition, is effectively managed through surgical release under local anesthesia. Inadequate counseling before such procedures may intensify intraoperative anxiety and increase pain responses. Thus, this research aims to investigate the effects of comprehensive preoperative counseling on various parameters in carpal tunnel release surgery. A case-control study design was adopted for this study. A retrospective analysis of patients who underwent carpal tunnel release surgery was performed. These patients were categorized into two distinct groups: one group received comprehensive counseling during their clinic visits, while the other group reported receiving less effective counseling. Evaluation encompassed patient-related factors, disease-related aspects, and perioperative variables for both groups. The study comprised 681 participants, with 526 (77.2%) being females, 421 (61.8%) undergoing surgery on the right side, and 519 (76.2%) employed in non-manual occupations. Moreover, 559 (82.1%) were non-smokers, while approximately two-thirds of the cohort had both diabetes and hypertension. The average age of participants was 52 years, and they reported a mean functional disability score of 7.2 out of 10. The comprehensive preoperative counseling group consisted of 333 patients, while the other group included 348 patients. The analysis revealed statistically significant differences between the two groups, including reduced intra-procedural anxiety (p = 0.043), decreased intraoperative pain (p = 0.005), lower rates of wound complications (p = 0.022), and improved self-reported pain relief (p = 0.3). Our study emphasizes the crucial role of preoperative counseling in improving patient experiences during perioperative care, leading to reduced anxiety, milder pain responses, fewer complications, decreased reliance on postoperative pain medication, and increased self-reported pain relief. Students’ Perception on the Role of Guidance and Counselling Services on Behavior in Private Secondary Schools in Bugesera District, Rwanda

Despite availability of guidance and counselling services in many secondary schools, many students are reluctant to seek those services. Students` attitude is important in determining the uptake of guidance and counselling services. Therefore, this study sought to determine the perception of students on the role of guidance and counselling services on behavior change among students in selected private secondary schools in Bugesera District in Rwanda. The study’s objectives were: to determine students’ perception on the aims of guidance and counselling services; to examine the attitude of students towards guidance and counselling; and to interrogate the students’ perception on the role of guidance and counselling services on behavior change among students. The study was grounded on Gestalt Theory of Perception and Maslow Hierarchy of Needs. The target population was 860 students from two private secondary schools. A sample of 274 was drawn using both convenient and probability systematic sampling techniques. Data was collected using self-administered questionnaires. The research employed descriptive survey design. The study established that most students perceived the aims of guidance and counselling services as that of helping them improve their academic performance (84.2%); prevent early pregnancies (81.6%); provide information in making career choices (81.6%) as well as raising students’ self-esteem (79.5%). The study also revealed that the attitude of students was positive as 73.1% were willing to seek counselor’s help; 70.6% were ready to see a counselor or referred elsewhere when necessary while 70.6% were willing to be referred to see a counselor. The study revealed that most students perceived the role of guidance and counselling in behavior change as effective in reducing crimes in schools (86.2%); helping students perform better in co-curriculum activities (82%); improve students’ study habits (80.9%) as well as increases students` self-esteem (80. 9%).This implied that most students perceived the role of guidance and counselling as effective in influencing behavior change. The study recommended that the school administrators should sustain efforts to promote positive attitudes towards seeking counseling help.

Role of Counselling in Helping Neglected Boy Children in Lusaka District, Zambia.

The exclusion and neglect of boy children in Zambia, particularly in Lusaka district, has resulted in increased conflicts between boys and girls, a rise in illiteracy among boys, heightened crime rates, more street boys, and increased drug abuse among boy children (Kambilima, 2014). Despite societal focus on protecting the girl child, the neglect of boys poses future risks to the very girls being safeguarded. This study explores the role of counselling in mitigating the effects of boy child neglect in Lusaka. Guided by Empowerment Theory, which emphasizes enhancing individuals’ capacity to make choices, the study aimed to: (a) describe the factors causing boy child neglect in Lusaka, (b) explore experiences of boy children due to societal neglect, and (c) establish how counselling can address these outcomes. Employing a qualitative and descriptive research design, data was collected using in-depth interview guide and focus group discussions with 50 participants, including 20 elderly men and women and 30 boys. Elderly participants were selected using simple random sampling. Data was thematically analyzed, revealing that one primary factor of boy child neglect is the preferential care and support given to girls in the which is influenced by cultural societal systems. Boys are often expected to be more independent, leading to reduced attention and neglect. The findings highlighted educational disadvantages as a significant experience among neglected boys. The study recommends enhancing counselling services by increasing the availability of qualified counselors trained to meet the unique needs of neglected boy children. These counselors should utilize culturally sensitive approaches and tools to effectively engage with this demographic.

Looking beyond year 1 in the molecular era of pediatric brain tumor diagnosis: confirmatory testing of germline variants found on tumor sequencing.

Somatic molecular profiling of pediatric brain tumors aids with the diagnosis and treatment of patients with a variety of high- and low-grade central nervous system neoplasms. Here, we report follow-up targeted germline evaluation for patients with possible germline variants following tumor only testing in the initial year in which somatic molecular testing was implemented at a single institution. Somatic testing was completed for all tumors of the central nervous system (CNS) undergoing diagnostic workup at Seattle Children's Hospital during the study period of November 2015 to November 2016. Sequencing was performed in a College of American Pathologists-accredited, Clinical Laboratory Improvements Amendments-certified laboratory using UW-OncoPlex™ assay (version 5), a DNA-based targeted next generation sequencing panel validated to detect genetic alterations in 262 cancer-related genes. We tracked subsequent clinical evaluation and testing on a subgroup of this cohort found to have potential germline variants of interest. Molecular sequencing of 88 patients' tumors identified 31 patients with variants that warranted consideration of germline testing. To date, 19 (61%) patients have been tested. Testing confirmed germline variants for ten patients (31% of those identified for testing), one with two germline variants (NF1 and mosaic TP53). Eight (26%) patients died before germline testing was sent. One patient (13%) has not yet had testing. Clinically validated molecular profiling of pediatric brain tumors identifies patients who warrant further germline evaluation. Despite this, only a subset of these patients underwent the indicated confirmatory sequencing. Further work is needed to identify barriers and facilitators to this testing, including the role of genetic counseling and consideration of upfront paired somatic-germline testing.
