Analysis of environmental factors affecting the incidence of testicular cancer, with particular emphasis on testicular germ cell tumors (TGCTs). Literature review

Introduction: Testicular cancer is the most common malignancy among men in the 20-44 age group. Most of the incidence is testicular germ cell tumor. In recent years there has been a steady increase in the incidence of this cancer. For this reason, we analyzed available articles on environmental risk factors that may influence the incidence of increased testicular malignancies Material and methods: Review of the specialized literature in the field of urology and articles available on PubMed, searching by the keywords “testicular cancer,” “testicular cancer risk factors,” “germ cell tumor,” “tgct‘’Aim of the study: The purpose of the study is to review the available literature and research papers regarding risk factors for malignant tumors of the testis. Summary: In recent years, there has been an increase in the incidence of testicular cancer, especially among men aged 20-44 years. The vast majority of it is germ cell tumors (TGCT). This article discusses the influence of environmental factors on the development of testicular cancer, with particular emphasis on the induction of TGCT. Studies indicate an association between exposure to trichloroethylene and other solvents and the risk of TGCT induction. The role of exposure to pesticides (especially fungicides), organic chlorine compounds, and viruses was also indicated to increase the risk of developing TGCTs,. The association of neoplasia with marijuana and tobacco use has also been studied, but needs further research. There is a need for continued research regarding heavy metals, infertility and factors related to pregnancy and the perinatal period. It is necessary to know the mechanisms that induce the process of carcinogenesis in the testis, which will enable the use of prevention and prophylaxis.

Three cases of gingival cancer associated with peri‐implantitis and their management

AbstractBackgroundDental implant treatment is effective for missing teeth, but it can cause peri‐implantitis. Chronic inflammation may be involved in the development of malignancy. We report three cases of gingival cancer that may have been associated with peri‐implantitis.Case PresentationAll three patients were referred to our department for detailed examination of a mass or ulcerative lesion around implants placed in their family dental clinics. One case of cancer was associated with oral lichen planus and peri‐implantitis. Two cases of cancer were associated with peri‐implantitis. Surgery for gingival cancer was successfully performed. All three pathological diagnoses were well‐differentiated squamous cell carcinoma.ConclusionChronic inflammation is a risk factor for malignant tumors, suggesting the importance of its maintenance.

The predictive value of the thyroid nodule benign and malignant based on the ultrasound nodule-to-muscle gray-scale ratio.

To investigate the efficacy of the ultrasonic nodule to muscle gray scale ratio as a predictive tool for distinguishing between benign and malignant thyroid nodules. A retrospective study was undertaken at the First People's Hospital of Hangzhou, affiliated with the Zhejiang University School of Medicine, analyzing ultrasound and pathological data of patients with thyroid nodules between May 2020 and December 2022. The study extracted ultrasound features of nodules and employed univariate and multivariate logistic regression analyses to identify independent risk factors for malignant tumors in the nodules. Subsequently, a predictive model for distinguishing benign and malignant thyroid nodules was developed. A total of 466 patients were included in this retrospective study, of which 275 cases were malignant tumors. Univariate and multivariate logistic regression analyses showed that the nodular-muscle gray-scale ratio, nodule diameter, margin status, aspect ratio, and calcification were closely related to thyroid malignant tumors. The area under the curve (AUC) of training group was 0.832, with a sensitivity, specificity, and accuracy of 85.5%, 67.4%, and 76.6%, respectively. The AUC of the external validation group was 0.819, with a sensitivity, specificity, and accuracy of 76.4%, 74.5%, and 75.7%, respectively. The calibration and decision curves showed that the model had good diagnostic value. The research findings indicate that ratio is significantly associated with the malignant nature of thyroid nodules. The application of a line chart model based on these parameters exhibits a high level of predictive performance.

Predictive Factors of Nonmalignant Pathological Diagnosis and Final Diagnosis of Ultrasound-Guided Cutting Biopsy for Peripheral Pulmonary Diseases.

This study aimed to explore the predictive factors of nonmalignant pathological diagnosis and final diagnosis of ultrasound-guided cutting biopsy for peripheral pulmonary diseases. A total of 470 patients with peripheral lung disease diagnosed as nonmalignant by ultrasound-guided cutting biopsy in the First Affiliated Hospital of Guangxi Medical University from January 2017 to May 2020 were included. Ultrasound biopsy was performed to determine the correctness of pathological diagnosis. Independent risk factors of malignant tumor were predicted by multivariate logistic regression analysis. Pathological biopsy results showed that 162 (34.47%) of the 470 biopsy data were specifically benign, and 308 (65.53%; malignant lesions: 25.3%, benign lesions: 74.7%) were nondiagnostic findings. The final diagnoses were benign in 387 cases and malignant in 83 cases. In the nondiagnostic biopsy malignant risk prediction analysis, lesion size (OR = 1.025, P = 0.005), partial solid lesions (OR = 2.321, P = 0.035), insufficiency (OR = 6.837, P < 0.001), and presence of typical cells (OR = 34.421, P = 0.001) are the final important independent risk factors for malignant tumors. In addition, 30.1% (25/83) of patients with nonmalignant lesions who were finally diagnosed with malignant tumors underwent repeated biopsy, and 92.0% (23/25) were diagnosed during the second repeated biopsy. 59.0% (49/83) received additional invasive examination. Nondiagnostic biopsy predictors of malignant risk include lesion size, partial solid lesions, insufficiency, and presence of atypical cells. When a nonmalignant result is obtained for the first time, the size of the lesion, whether the lesion is subsolid, and the type of pathology obtained should be reviewed.

Nomogram based on the O-RADS for predicting the malignancy risk of adnexal masses with complex ultrasound morphology

ObjectiveThe accurate preoperative differentiation of benign and malignant adnexal masses, especially those with complex ultrasound morphology, remains a great challenge for junior sonographers. The purpose of this study was to develop and validate a nomogram based on the Ovarian-Adnexal Reporting and Data System (O-RADS) for predicting the malignancy risk of adnexal masses with complex ultrasound morphology.MethodsA total of 243 patients with data on adnexal masses with complex ultrasound morphology from January 2019 to December 2020 were selected to establish the training cohort, while 106 patients with data from January 2021 to December 2021 served as the validation cohort. Univariate and multivariate analyses were used to determine independent risk factors for malignant tumors in the training cohort. Subsequently, a predictive nomogram model was developed and validated in the validation cohort. The calibration, discrimination, and clinical net benefit of the nomogram model were assessed separately by calibration curves, receiver operating characteristic (ROC) curves, and decision curve analysis (DCA). Finally, we compared this model to the O-RADS.ResultsThe O-RADS category, an elevated CA125 level, acoustic shadowing and a papillary projection with color Doppler flow were the independent predictors and were incorporated into the nomogram model. The area under the ROC curve (AUC) of the nomogram model was 0.958 (95% CI, 0.932–0.984) in the training cohort. The specificity and sensitivity were 0.939 and 0.893, respectively. This nomogram also showed good discrimination in the validation cohort (AUC = 0.940, 95% CI, 0.899–0.981), with a sensitivity of 0.915 and specificity of 0.797. In addition, the nomogram model showed good calibration efficiency in both the training and validation cohorts. DCA indicated that the nomogram was clinically useful. Furthermore, the nomogram model had higher AUC and net benefit than the O-RADS.ConclusionThe nomogram based on the O-RADS showed a good predictive ability for the malignancy risk of adnexal masses with complex ultrasound morphology and could provide help for junior sonographers.

Peritumoral edema in preoperative magnetic resonance imaging is an independent prognostic factor for hepatocellular carcinoma

BackgroundPeritumoral edema is an independent prognostic risk factor for malignant tumors. Therefore, assessment of peritumoral edema in preoperative magnetic resonance imaging (MRI) may provide better prognostic information in patients with hepatocellular carcinoma (HCC). AimTo determine whether peritumoral edema in preoperative MRI is a prognostic factor for HCC. MethodsA retrospective analysis of 90 patients with HCC confirmed by surgical pathology was performed. All patients' peritumoral edema in preoperative MRI was reviewed by two radiologists. The association of disease recurrence with peritumoral edema and clinicopathological features was assessed using the Cox proportional hazards model. Interobserver agreement for evaluating peritumoral edema was determined using Cohen's κ coefficient. ResultsRecurrence and non-recurrence after an average 20.8 month follow-up was 25.6% (23/90) and 74.4% (67/90), respectively. The ratio of peritumoral edema of 90 patients with HCC in preoperative MRI was 35.6% (32/90). In univariate Cox regression analysis, peritumoral edema [hazard ratio (HR) 11.08, P < 0.001], tumor diameter (HR 4.12, P = 0.001), microvascular invasion (HR 2.78, P = 0.020), gender (HR 0.29, P = 0.006), cirrhosis (HR 2.45, P = 0.049), ascites syndrome (HR 2.83, P = 0.022), aspartate aminotransferase(AST)/alanine aminotransferase(ALT) (HR 5.07, P = 0.003) were indicators for HCC recurrence. In multivariate Cox regression analysis, the tumor diameter (HR 2.53, P = 0.032) and peritumoral edema (HR 8.71, P < 0.001) were independent prognostic factors of HCC. The sensitivity, specificity, positive predictive value and negative predictive value of peritumoral edema and tumor diameter were 82.6%&60.9%, 80.6%&77.6%, 59.4%&48.3%, and 93.1%&85.3%, respectively. ConclusionPeritumoral edema in preoperative MRI may be considered as a biomarker of prognostic information for patients with HCC.

Microsatellite instability is inversely associated with type 2 diabetes mellitus in colorectal cancer.

BackgroundMicrosatellite instability (MSI) is a clonal change in the number of repeated DNA nucleotide units in microsatellites. High-frequency MSI (MSI-H) colorectal cancers (CRCs) are known to have different clinicopathological features compared with microsatellite stable (MSS) CRCs. In addition, previous studies have shown that type2 diabetes mellitus (T2DM) is a risk factor for malignant tumors including CRCs. The aim of this study was to investigate the relationship between T2DM and MSI-H colorectal cancer.MethodsThe study design is a single center, cross-sectional study. Data from a series of 936 patients with CRCs were collected and MSI status was assessed.ResultsIn total, 29 (3.1%) and 907 (96.9%) tumors were classified as having MSI-H and low-frequency microsatellite instability or being MSS (MSS), respectively. Of the 936 patients, 275 (29.6%) were associated with T2DM. One (3.4%) of the 29 MSI-H patients and 274 (30.2%) of the 907 MSS patients had T2DM. Thus, the incidence of T2DM was significantly less frequent in MSI-H compared with MSS patients (Fisher’s exact test: p = 0.0007).ConclusionsWe conclude that MSS tumors are significantly more common than MSI-H tumors among individuals with T2DM.

Modern view on the risk factors for malignant tumors of the thyroid gland: a systematic review

The morbidity of thyroid carcinoma (TC) tends to annual growth. The results of the multifactorial studies of the development of thyroid carcinogenesis testify the insufficient knowledge about the influence and significance of certain TC factors, which requires systematization in order to improve the oncoepidemic situation in the country.The objective: of this study was to conduct an analytical systematic review of literature to resume risk factors of TC in the last 10 years.Materials and methods. The generalization of literary sources, founded by key words in Google Scholar, PubMed and eLIBRARY.ru for 2007 to 2017 was carried out. We analyzed the value of the odds ratio (OR), the relative risk (RR), the hazard ratio (HR), standardized incidence ratio (SIR) and an excess relative risk (ERR) with respective confidence intervals (95% CI), as well as the reliability value (p).Results. The most significant risk factors for TC are: ionizing radiation (increases the incidence in 1,3–5 times); previous thyroid disease (associated with TC in 30–94% cases, most goiter, adenoma and AIT – in 29,7%, OR=2,12; RR=1,4), heredity (presence of TC in I degree relatives increases the chance of occurrence in &gt; 4-times), systemic autoimmune disease (SLE increases the risk 1,8 times), diabetes (TC risk increases on 18–20%), menstrual and reproductive factors (increased risk in 1,2–2,4 times), anthropometric factors (high height – increases the risk in 1,2–1,6 times, BMI ≥30 kg/m2 – in 1,35–1,7 times, and the waist &gt;90 cm in women – in 1,3 times); benign breast disease (TC risk is higher on 38–56%), breast cancer (TC risk increases in 1,4–2,3 times), deficiency of vitamin D (TC risk increases 2–6 times), the persistence of virus, insufficient iodine intake, lifestyle – the use of certain products and physical activity, anthropogenic and occupational factors.Conclusion. The primary care physician should take into account the current understanding of the risk ТС factors in patients, especially in high-risk groups.

Body fat composition as predictive factor for treatment response in patients with newly diagnosed multiple myeloma - subgroup analysis of the prospective GMMG MM5 trial.

Introduction/BackgroundObesity is a well-known risk factor for malignant tumors and increased body mass index (BMI) is correlated to the risk of developing multiple myeloma (MM). The correlation of body fat composition with disease activity, adverse events and treatment response of MM patients has not been investigated yet.Patients and MethodsA subgroup of 108 patients from a single institution enrolled in the prospective GMMG-MM5 trial, who received a whole-body low-dose computed tomography (WBLDCT) before induction therapy, were included in this study. Body fat composition was measured in WBLDCT for each patient, divided in the compartments abdomen, pelvis, thigh and further categorized in subcutaneous (SAT) and visceral adipose tissue (VAT). The correlation of these parameters with disease activity (M protein, plasma cell count, LDH, CRAB-criteria), adverse cytogenetics, adverse events and treatment response were evaluated.ResultsSignificant reciprocal correlation was found between adverse cytogenetics and VAT of the abdomen and pelvis, respectively (gain 1q21: p=0.009 and p=0.021; t(4;14): p=0.038 and p=0.042). No correlation of VAT or SAT with adverse events was observed. Significant reciprocal correlation was observed between abdominal (p=0.03) and pelvic (p=0.035) VAT and treatment response. Abdominal VAT remains significant (p=0.034) independently of revised ISS stage and treatment. The BMI did not show a significant correlation with treatment response or investigated cytogenetics.ConclusionBased on the clinically relevant difference in treatment outcome depending on VAT and SAT, excessive body fat of abdomen and pelvis might be a predictive factor for poor treatment response. Further influences in this context should be considered as well, e.g. chemotherapy dosing and body fat metabolism. Further studies are necessary to investigate this hypothesis.

Stratifying Risk of Urinary Tract Malignant Tumors in Patients With Asymptomatic Microscopic Hematuria

ObjectiveTo identify patients who could safely avoid unnecessary radiation and instrumentation after the detection of microscopic hematuria. Patients and MethodsWe conducted a prospective cohort study of patients who were referred to urologists and underwent a full evaluation for asymptomatic microscopic hematuria during a 2-year period in an integrated care organization in 3 regions along the West Coast of the United States. A test cohort and validation cohort of patients with hematuria evaluations between January 9, 2009, and August 15, 2011, were identified. Patients were followed passively through their electronic health records for a diagnosis of urothelial or renal cancer. The degree of microscopic hematuria, history of gross hematuria, smoking history, age, race, imaging findings, and cystoscopy findings were evaluated as risk factors for malignant tumors. ResultsThe test cohort consisted of 2630 patients, of whom 55 (2.1%) had a neoplasm detected and 50 (1.9%) had a pathologically confirmed urinary tract cancer. Age of 50 years or older and a recent diagnosis of gross hematuria were the strongest predictors of cancer. Male sex was also predictive of cancer, whereas smoking history and 25 or more red blood cells per high-power field on a recent urinalysis were not statistically significant. A Hematuria Risk Index developed from these factors had an area under the receiver operating characteristic curve of 0.809. In the validation cohort of 1784 patients, the Hematuria Risk Index performed comparably (area under the curve = 0.829). Overall, 32% of the population was identified as low risk and 0.2% had a cancer detected; 14% of the population was identified as high risk, of whom 11.1% had a cancer found. ConclusionThese results suggest that a considerable proportion of patients could avoid extensive evaluations with the use of the Hematuria Risk Index.

Obesity and Colorectal Cancer

Obesity worldwide is constantly increasing. Obesity acts as an independent significant risk factor for malignant tumors of various organs including colorectal cancer. Visceral adipose tissue is physiologically more important than subcutaneous adipose tissue. The relative risk of colorectal cancer of obese patients is about 1.5 times higher than the normal-weight individuals, and obesity is also associated with premalignant colorectal adenoma. The colorectal cancer incidence of obese patients has gender-specific and site-specific characteristics that it is higher in men than women and in the colon than rectum. Obesity acts as a risk factor of colorectal carcinogenesis by several mechanisms. Isulin, insulin-like growth factor, leptin, adiponectin, microbiome, and cytokines of chronic inflammation etc. have been understood as its potential mechanisms. In addition, obesity in patients with colorectal cancer negatively affects the disease progression and response of chemotherapy. Although the evidence is not clear yet, there are some reports that weight loss as well as life-modification such as dietary change and physical activity can reduce the risk of colorectal cancer. It is very important knowledge in the point that obesity is a potentially modifiable risk factor that can alter the incidence and outcome of the colorectal cancer.
