Rare case of anomalous right coronary artery from pulmonary artery (ARCAPA syndrome)

Anomalous right coronary artery from pulmonary artery (ARCAPA) is a rare congenital anomaly of right coronary artery, most of the patient are asymptomatic, although myocardial infarction and sudden cardiac arrest has been reported. Herein we present a 5 month old infant presented with chest infection, during routine examination cardiac murmur detected, anomalous right coronary artery from pulmonary artery suspected by transthoracic Echocardiography which showed dilated (4mm) left main stem of coronary artery with mild left ventricular dilatation, but the origin of right coronary artery couldn’t be detected. Coronary angiography done which confirm the diagnosis and the child successfully operated on for reimplantation of right coronary artery to the aortic cusp.

Modified Repair for Huge Unruptured Aneurysm of the Right Sinus of Valsalva

Unruptured aneurysm of the sinus of Valsalva is a rare abnormality. In the present case, the huge aneurysm of the sinus of Valsalva was only involved in the right coronary sinus with the ostium far from its normal place. We designed a modified repair technique, with a scalloped Intergard woven vascular patch combined with aortic flap, to reconstruct the sinus of Valsalva and avoid the reimplantation of right coronary artery.

Reimplantation of anomalous right coronary artery arising from the pulmonary trunk leading to normal coronary flow reserve late after surgery

We report a case of successful reimplantation of the right coronary artery from the pulmonary trunk into the aorta with a 10-year follow-up. The finding of a normal coronary flow reserve late after surgery suggests that direct reimplantation of the right coronary artery into the aorta is the preferable surgical technique.
