Rectovesical Fistula Following an Ovarian Dermoid with Recurrent Vesical Calculus: A Case Report

Coincident pneumaturia and cystitis emphysematosa.

Although Boismont mentioned spontaneous production of gas in the bladder in 1825, according to Kelly and MacCallum (1898), it was not until 1860 that the first case of pneumaturia not due to a recto-vesical fistula was described by Raciborski. In 1883 Guiard reported pneumaturia in association with diabetes and said that the other predisposing condition for primary pneumaturia was urinary obstruction. Cystitis emphysematosa was first noted in man as a pathological entity by Mills in 1930, and Bailey in 1961 reviewed the world literature, finding 50 cases of this condition, of which only 13 were known to have had glycosuria. At the same time he found only 46 cases of primary pneumaturia, of which 29 had had glycosuria. The present case is reported because of the relative infrequency with which either of these conditions has been described on its own, and the even greater infrequency in which they have been recorded together.

Development and Classification of Anorectal Anomalies

The current classification of congenital anorectal anomalies was established by Partridge and Gough, 1 who divided them into two principal groups: low abnormalities, in which the termination of the bowel is below the pelvic floor (covered anus, ectopic anus, stenosed anus, anal membrane); and high abnormalities, in which the termination of the bowel is above the pelvic floor (anorectal agenesis, with or without fistula, rectal atresia, cloaca, multiple abnormalities). In their discussion of the development of this region, they said of Wood-Jones's theory (q.v. infra), It might be reasonable to suggest that if (it) were true, one might occasionally find a rectourethral or rectovesical fistula without anorectal agenesis. This is not recorded. The case described in this paper is the equivalent of such an anomaly in a female. Imperforate anus

The usual type of imperforate anus in the female baby can usually be repaired at six months of age by the perineal approach illustrated in this article. The usual type of imperforate anus in the male infant with a rectovesical fistula can be repaired at one to two days of age by an abdominoperineal pullthrough procedure. Both types of imperforate anus require special technic which is best accomplished by one experienced in these operations. Rapid transportation usually makes this possible. If delay is necessary in instances of the latter type, a transverse colostomy is much preferred to an inexperienced attempt at repair, as no amount of skill is likely to achieve good results with the second attempt.

Management of imperforate anus.

Experience with 89 cases of imperforate anus treated at Charity Hospital of New Orleans and the Ochsner Clinic has emphasized the importance of more standardized management of this anomaly. The importance of the observation period in determining the best plan of treatment has been stressed. Imperforate anus has been classified into four types. The surgical procedure to be employed depends on the type of anomaly, the associated anomalies and the degree of surgical risk. For type I digital dilation is usually sufficient except in cases of hard fibrotic rings when anoplasty may be required. An intrasphincteric procedure should be performed for type II anomalies. In patients with type III anomalies in which the distance between the rectal pouch and surface of the skin exceeds 1.5 cm., in those with rectovesical, recto-urethral or rectovaginal fistulas above the hymen and in those with type IV anomalies, the abdomino-intrasphincteric pull-through procedure is recommended. The anal sphincter-preserving operation is the procedure of choice for rectovaginal fistulas that open below the hymen into the fossa navicularis and for all rectoperineal fistulas. The success of immediate definitive treatment depends upon the skill of the surgeon, and the presence or absence of prematurity and associated anomalies. The various surgical technics have been described.

The treatment of congenital malformations of the anus and rectum

A small series of cases of congenital malformations of the anus and rectum are presented with observations thereon. The perineal approach may be used if the bowel is close to the perineum. When there is an associated rectovesical, rectourethral or rectoureteral fistula, or when the bowel is not close to the perineum, the combined one-stage abdominoperineal operation is indicated. In rectovaginal fistulas and in some cases of rectoperineal fistulas, a two-incision perineal approach, credited to Ortiz, is used. A slight modification in the technic of the latter has been suggested. Colostomy alone is seldom necessary.

Imperforate anus and recto-urethral fistula in a twenty-seven year old man; report of a case.

Imperforate anus—some form of developmental defect of the anorectal bowel, varying from simple stenosis of the anus to absence of anus and rectum— occurs once every 6,000 to 10,000 births. The types of malformations have been classified into four categories by Ladd. 1 When the additional developmental anomaly of rectourethral of rectovesical fistula conjunction with the imperforate anus occurs, in the male, the fistulous termination of the rectum the bladder or urethra unless relieved is followed by early death from pyelonephritis. 2 After a thorough survey of the literature and personal communications with American surgeons, 3 I believe the present case to be the only one which a man with a Ladd type 3 imperforation and rectourethral fistula has survived without definitive surgical therapy until the age of 27 years.
