Implementation of RedCap data systems in Kenyan psychiatric rehabilitation: a community case study

Data collection systems are vital in implementation of interventions within patient care as they allow us to better understand and improve the care given. This is equally true in the field of psychiatric rehabilitation, which has been steadily growing in the medical world for the past 20 years. It has recently been a focus of psychiatric medicine in Kenya with the Kenya Mental Health Action Plan 2021-2025 calling for improvement of care in the field. With psychiatric rehabilitation growing in low- and middle-income countries in recent years studies have come out showing the effects of newly employed programs and stressing the need for data collection practices that can be used to encourage best practices in local communities. This paper describes the implementation of data systems for the Moi Teaching and Referral Hospital Nawiri Skills and Recovery Centre (Nawiri) in Eldoret, Kenya. The goals of the data systems were 1) To collect and organize information for continuous quality improvement of services at Nawiri, 2) To collect information for reporting to MTRH leadership, Ministry of Health as well as private funders, 3) To provide data to assess outcomes of people who receive services at Nawiri to create evidence for effective interventions to help create policy to make psychiatric rehabilitation more available to people living with Severe mental illness in Kenya. Lessons learned from data systems implementation included insights into ease of data collection, adaptability of data instruments to fit facility needs, and ability to reproduce RedCap data collection in other settings.

Inter- and intraspecific blood-related biomarkers and chemical exposure in confined and free-living sea turtles.

The relevance of recovery centers and head-starting programs for rescue, rehabilitation, rearing, and conservation of sea turtles is recognized worldwide. In addition, these centers contribute to generating biochemical and physiological data needed to identify health markers and provide baseline values. Because of the marine ecosystems' deterioration, biomarker identification is a global priority for sea turtle conservation; nevertheless, information on specific endpoints, such as neurotoxicity and mutagenesis, is still limited in sea turtles. This study aimed to contrast a set of non-invasive blood biomarkers with ecotoxicological and clinical applications in confined green sea turtles (Chelonia mydas) compared with free-living ones from the Mexican Caribbean. Additionally, interspecific (green, hawksbill (Eretmochelys imbricata), loggerhead (Caretta caretta) turtles) differences were also evaluated. Plasmatic organochlorine pesticides (OCs) and polychlorinated biphenyl (PCBs) were also determined. The concentration ranges of uric acid, total proteins, lipids (cholesterol and triglycerides), and thyroxine of both confined and free-living green turtles fell outside the reference intervals for the species. Additionally, confined green turtles had the highest number of erythrocytic nuclear abnormalities (ENA) and elevated levels of hemoglobin, lipid peroxidation, and activity of glutathione S-transferase, glutathione peroxidase, and carboxylesterase (CE). Contrasts among confined species identified hawksbill turtles with the lowest glutathione reductase activity, green turtles with the lowest ENA frequency and CE activity, and loggerhead turtles with the highest plasmatic concentrations of PCBs and OCs. The information here provided can be used as information in health monitoring programs and for conservation and management policies at regional, national, and international level.

Evaluating changes in recovery in people living with severe and persistent mental illness after psychiatric rehabilitation services at Moi Teaching and Referral Hospital, Eldoret, Kenya.

People living with severe and persistent mental illness (SPMI) in Kenya lack access to recovery-based services. In this study, we assessed changes in recovery in people living with SPMI in Kenya 6 months after receiving services at the Moi Teaching and Referral Hospital Nawiri Recovery and Skills Centre (Nawiri). A retrospective evaluation was conducted using a pretest/posttest design analyzing Nawiri care program data collected on admission and 6 months after discharge for recovery metrics. Thirty patients, with an average age of 33 years and of whom 57% are female, met criteria for the study, with the most common mental diagnoses being schizophrenia (60%) and bipolar mood disorder (30%); 76% of participants met the definition of extreme poverty and had a median of two psychiatric admissions in the 12 months before admission. Patients improved significantly on recovery outcomes 6 months after receiving care at Nawiri, including decreased psychiatric hospitalizations (from 1.33 to 0.07), improved rates of independence in life skills (75.9%-96.7%), improved engagement in income generating activities (23.3%-63.3%), improved food security (69.0%-96.7%), decreased days of functional impairment from symptoms (3.7 to 1.7 days in past week), decreased substance use (53.3%-13.8%), and improved engagement in outpatient mental health care (50.0%-93.3%). People living with SPMI had improved recovery 6 months after receiving residential psychiatric rehabilitation services in western Kenya. A more robust evaluation of program effectiveness and implementation is recommended to help explore generalizability and scalability to other resource limited settings. Peran Badan Pemulihan Aset di Kejaksaan Negeri Donggala

This research explains the role of the Asset Recovery Agency (formerly known as the "Asset Recovery Center") at the Donggala District Attorney's Office. In Law Number 16 of 2004 concerning the Attorney General's Office of the Republic of Indonesia jo. Law Number 11 of 2021 concerning Amendments to Law Number 16 of 2004 concerning the Attorney General's Office of the Republic of Indonesia in Article 2 paragraph (1) essentially states that the Attorney General's Office of the Republic of Indonesia is a government institution that exercises state power in the field of prosecution and other authorities based on the law. The research results show that the purpose of the establishment of the Asset Recovery Agency (formerly known as the "Asset Recovery Center") at the Donggala District Attorney's Office is based on the principles of social justice and legal certainty as stipulated by the Prosecutor's Law, whereby the return of assets to the rightful owners or to the State is carried out to achieve this goal, thereby providing a deterrent effect on criminal offenders. Because the illegal acquisition of something or an asset, the asset recovery handling procedure at the Donggala District Attorney's Office is carried out in accordance with the laws and regulations but lacks transparency, as evidenced by the absence of socialization of the Asset Recovery Center (PPA) or Asset Recovery Agency (BPA) to the general public both online and offline. As a result, the general public will have difficulty accessing information regarding the clarity of the status of confiscated goods or assets related to criminal offenses managed by the prosecutor's office. The author believes this because, based on a comparison with the Asset Recovery Center (PPA) and the Asset Recovery Agency (BPA) of the Donggala District Attorney's Office and the Palu District Attorney's Office, information regarding the Asset Recovery Center (PPA) and the Asset Recovery Agency (BPA) and Standard Operating Procedures (SOP) is easily accessible online. To achieve this goal, the PPA and BPA should innovate so that the public can easily access the information provided by the prosecutor's office.

Legal Measures to Support the Process of Asset Recovery in the State-Owned Enterprises Banking Sector Post-Corruption Cases

This study aims to identify obstacles and formulate strategic measures to optimize asset recovery in the State-Owned Enterprises Banking sector post-corruption cases. This research employs a normative legal research method with a comparative approach, where the collected legal materials are analyzed qualitatively to describe the problems and answer the research objectives. The results show that asset recovery faces multidimensional challenges, including weaknesses in the legal framework, limitations in the capacity and integrity of law enforcement officers, lack of facilities and infrastructure, and efforts by the accused to avoid asset recovery. Optimizing asset recovery requires a comprehensive strategy encompassing the enactment of the Bill on Criminal Asset Forfeiture, strengthening the legal and institutional framework, enhancing the capacity and integrity of law enforcement, improving facilities and infrastructure, optimizing the civil route, and strengthening international cooperation. Therefore, it is recommended that the Government, Directors of State-Owned Enterprises Banking, the Asset Recovery Center, and law enforcement agencies undertake various optimization efforts, such as accelerating the enactment of the Bill on Criminal Asset Forfeiture, increasing the budget, strengthening corruption prevention systems, enhancing the competence of law enforcement officers, and strengthening international cooperation. Furthermore, the Public Prosecutor is expected to maximize the use of Article 18 of Law Number 31 of 1999 in indictments, enabling judges to issue optimal rulings for asset recovery in the State-Owned Enterprises Banking sector post-corruption cases.

Circulating Levels of Vitamins A, C, and E-Alpha in Organ Donors After the Neurologic Determination of Death.

The antioxidant effects of vitamins may attenuate the oxidative stress on organs imposed by ischemia-reperfusion injury during the process of organ transplantation from brain-dead donors. Circulating levels of vitamins A, C, and E-α in donors after brain death and their relationships to donor demographics, management, organ utilization, and recipient outcomes are largely unknown. An observational, prospective, cohort study of 84 consecutive brain-dead organ donors managed at a single organ procurement recovery center was conducted. Vitamin levels were drawn immediately prior to procurement. Levels of serum vitamins A and E-α and plasma vitamin C were below normal in 80%, 85%, and 92% of donors and deficient in 40%, 62%, and 63%, respectively. Vitamin C deficiency was associated with a longer time between death and specimen collection (P = .004). Death from head trauma and stroke were associated with lower levels of vitamin A than from anoxic causes (P = .003) and smokers had greater vitamin C deficiency (P = .03). During donor management, vitamin C deficiency was associated with longer vasopressor support (P = .03) and normal levels of vitamin E-α were associated with reaching a lower alanine transferase compared to those with subnormal levels (P < .05). Donors deficient in vitamin E-α were less likely to have a liver recovered for transplantation (P = .005). Vitamin levels were not associated with the recipient outcomes examined. Circulating vitamins A, C, and E-α is profoundly low in brain-dead organ donors, associated with relevant demographic features of the donor, and may influence donor management and organ utilization.

Effects of 8 weeks aquatic exercises on balance recovery strategies and center of pressure sways in blind women: A randomized controlled trial

Visual impairment presents significant challenges to mobility and daily activities, often resulting in a sedentary lifestyle and decreased physical fitness. This study aimed to investigate the effects of an 8-week aquatic exercise protocol on balance recovery strategies and center of pressure (COP) sways in blind women. A total of 30 participants were randomly assigned to either an experimental group (EX, n = 15) or a control group (CO, n = 15). Balance recovery parameters, including hip sways, ankle sways, and the ratio of hip sways to ankle sways, were assessed using motion analysis, while COP sways were measured using a force platform. The aquatic exercise protocol focused on enhancing the somatosensory system through a variety of targeted exercises. The results revealed a significant improvement in balance recovery strategies among participants in the EX group, characterized by reduced hip sways, increased ankle sways, and a decreased hip sways/ankle sways ratio in both anterior–posterior and posterior–anterior directions. Moreover, COP sways significantly decreased, as indicated by reductions in ellipse area and traveled distance in both directions. These findings suggest that the aquatic exercise protocol facilitated a shift from a hip-dominant to an ankle-based strategy, indicating improved adaptive balance recovery mechanisms. The reduced COP sways further indicate enhanced postural stability. These positive outcomes underscore the effectiveness of targeted aquatic interventions in enhancing physical capabilities and overall balance in blind women. Including such exercises in rehabilitation programs may contribute to reducing fall risks and improving the quality of life for individuals with visual impairments.

The growth and diversity of the evidence base for the clubhouse model.

The Clubhouse Model is a long-standing form of psychosocial rehabilitation that has been in existence for over 75 years. Today, over 350 Clubhouse programs in 33 countries affiliate with Clubhouse International and operate as nonclinical community-based recovery centers for adults and young adults living with mental illness. Clubhouses provide a strengths-based approach to recovery and offer participants, referred to as members, a variety of supports and services including assistance with obtaining and maintaining community-based employment, education, housing, social integration, outreach and advocacy, wellness and health promotion activities, and linkages to medical and psychiatric services. There is evidence and support for the Clubhouse Model in improving quality of life and social functioning, reducing hospitalization(s) and/or psychiatric symptoms, and promoting employment. This special issue has nine articles that highlight ongoing research collaborations from across the globe that examine the impact of the Clubhouse Model on a variety of novel outcomes. While more research is needed, the articles in this special issue reflect the growth and diversity of the evidence base for the Clubhouse Model. Analysis of Evolution of the State Nutritional, of there Duration of Stay and Mortality of Children 0-59 Months in Mbujimayi Case of the General Hospital of Tudikolela, Democratic Republic of Congo

Objective The objective of this analysis is to evaluate the evolution of the nutritional status of a cohort admitted to nutritional recovery center at the Tudikolela hospital, in Mbujimayi, and to identify the risk factors for hospital malnutrition as well as its consequences on the morbidity and length of hospitalization. Material and methods Our study is prospective, transversal, carried out in a center of nutritional recovery of malnutrition.It was carried out in a Unit Nutritional Therapeutic intensive (UNTI) of the Tudikolela General Hospital in Mbujimayi (Democratic Republic of Congo). It concerns 275 subjects. Results The presence study reveals that diarrhea on admission (p0.009) constitutes a mortality factor in malnourished children.

An international, multicenter post hoc analysis comparing in-person and virtual medication management strategies in post-ICU recovery clinics.

In an effort to expedite the publication of articles, AJHP is posting manuscripts online as soon as possible after acceptance. To compare the incidence of ICU pharmacist interventions in intensive care unit recovery center (ICU-RC) in-person and virtual clinic visits. This was a post hoc analysis of interventions implemented by ICU pharmacists among adult patients who were referred to 12 ICU-RCs across the United States and the United Kingdom between September 2019 and July 2021, as reported in a previously published study "An International, Multicenter Evaluation of Comprehensive Medication Management by Pharmacists in ICU Recovery Centers." That study included patients who received a comprehensive medication review by an ICU pharmacist. Medication-related interventions performed by an ICU pharmacist during ICU-RC in-person clinic visits were compared to those performed during virtual clinic visits. There were 507 patients referred to an ICU-RC, of whom 474 patients attended a clinic visit. Of those, 472 received a comprehensive medication review, with 313 patients attending in-person visits and 159 patients attending virtual visits. The incidence of medication-related interventions implemented was higher in the ICU-RC in-person clinic group compared to the virtual clinic group (86.5% vs 79.2%, P = 0.04). There was no difference in the median number of ICU pharmacist interventions per patient between the in-person and virtual clinic groups (2 vs 2, P = 0.13). An ICU admission diagnosis was an independent predictor of medication-related interventions among all patients. The incidence of ICU pharmacist interventions was higher at ICU-RC in-person clinic visits compared to virtual clinic visits. Pharmacists aid in meeting the complex pharmacologic challenges of post-intensive care syndrome in both settings.

Self-reported longitudinal impacts of the COVID-19 pandemic on adults with acquired communication disorders

Purpose Adults with acquired communication disorders are particularly vulnerable to potential negative effects from the changes that arose during the COVID-19 pandemic. In this longitudinal study, we asked: How did the self-perceived impacts of the pandemic on adults with acquired communication disorders change over time? Method We developed an online survey to investigate changes in day-to-day behaviours, psychosocial elements, and communication abilities among members of a community-based stroke recovery centre in the USA. Participants included adults with aphasia or other cognitive-communication disorder from stroke or brain injury. We compared survey responses from summer 2020 (n = 50) and summer 2021 (n = 24) using a concurrent mixed methods design. Result The impacts of the pandemic changed over time. By 2021, participants reported a more “normal” amount of time spent on in-person socialising and healthcare, improved quality of life and connectedness to others, and an improvement in overall communication ability. Across both years, participants shared comments regarding changes in routine, increased use of technology, and the positive benefit of therapy groups. Conclusion Self-perceived psychosocial elements and communication abilities among stroke and other brain injury survivors changed over the course of the pandemic. Clinicians and researchers should continue providing this population with support and opportunities for engagement.

A multi‐objective sustainable closed‐loop supply chain network problem with hybrid facilities

AbstractA sustainable closed‐loop supply chain network requires conjunctive implementation of reverse logistics in the supply chain, with decisions that consider economic, environmental, and social factors. In real life, the problem needs to be addressed by prioritizing targets or interacting between them to give a range of solutions to the decision maker. In this context, this work proposes a novel multi‐objective sustainable closed‐loop supply chain network problem based on the revised network design model with hybrid recovery centers minimizing (1) the total economic cost, (2) the CO2 emission of vehicles used, and (3) the total obnoxious distance. The latter objective is a novel implementation of the social dimension of a sustainable model. A sensitivity analysis of the multi‐objective model is developed through ANOVA. A dataset of instances was generated to test the model and the solution methods, which are configured with AUGMECON2, a linear programming relaxation implemented to improve the CPU time, and AUGMECON2‐EXTENDED to obtain more solutions to avoid exploring all space of the solution. The results show that an AUGMECON2‐EXTENDED implementation outperforms all the selected performance metrics. These performance metrics include NPS, CPU time, RPOS, QM, and HV. The results show an improvement on average of at least , , , , and , respectively, in those metrics, in comparison to other implementations.

Description of Mother's Behavior and Nutritional Status of Toddlers After the Nutrition Recovery Program in Makassar City, South Sulawesi

The nutrition recovery program is one of the efforts to overcome malnutrition in children by involving mothers or families. Mothers are provided with knowledge so that they can become independent in caring for and providing additional food that has been provided or recommended by the health center. During the three months, there were still children who did not experience changes in nutritional status, namely very thin, and only a few children experienced changes in nutritional status to become thin and normal. This type of research is a survey with a cross-sectional research design and aims to determine the description of maternal behavior and the nutritional status of toddlers after the nutritional recovery program in Makassar City. The method used is descriptive analysis. Data was collected by direct interviews with mothers using questionnaires. The child's weight was measured using a step scale and the child's height using a measuring board and microtoise. Nutritional status is measured by the BB/TB indicator. The research results showed that most of the mothers' knowledge was in the sufficient category, the mothers' attitudes were in the sufficient category, and the mothers' actions were in the sufficient category. Officers are expected to further increase mothers' knowledge through disseminating information about nutrition recovery centers, conducting outreach regarding malnourished children, MP-ASI, Nutrition Aware Families and providing motivation to increase mothers' knowledge and awareness of the nutritional status of toddlers.

First report of Enterocytozoon bieneusi in Erinaceus europaeus and Hemiechinus auritus hedgehogs from recovery centers of Portugal.

Enterocytozoon bieneusi microsporidia are emerging pathogens infecting a wide range of vertebrate and invertebrate hosts, known to have zoonotic features since they infect both wild and domestic animals, and humans. Despite their significance, there is very limited epidemiological data on microsporidia in hedgehogs, especially European hedgehogs (Erinaceus europaeus) and long-eared hedgehogs (Hemiechinus auritus), the former known as synantropic hedgehogs, and the latter suited as pets. As such, the present study aimed to assess the presence of E. bieneusi in hedgehogs from Portugal. For this purpose, fecal samples from 110 hedgehogs of three species-E. europaeus (n=106), H. auritus (n=1), and Atelerix albiventris (n=3)-were collected and tested for E. bieneusi by PCR targeting the internal transcribed spacer region and the flanking small and large subunits of the rRNA. We found an overall occurrence of 22.7% (25/110; 95% confidence interval [CI]: 15.28-31.70), with 22.6% (24/106; 95% [CI]: 15.08-31.79) in E. europaeus, 100% (1/1) in H. auritus, and 0% in A. albiventris. Interestingly, three novel genotypes were identified, all belonging to the potentially zoonotic Group 1. Our findings highlight the importance of hedgehogs as potential reservoirs for E. bieneusi and emphasize the need for further research to understand their role in transmission dynamics and assess the associated risks to public and veterinary health.

Leadership Style and Work Performance among Security Officers of Narcotic Addiction Recovery Centre, Malaysia

This study explores on leadership style and work performance among the security officers of the Narcotic Addiction Recovery Center (NARC). This research uses quantitative methods with the distribution of questionnaires to the officers in NARC. The study employs research tools on the Leadership Style Questionnaire (LSQ) by Northouse (2015) for the independent variable and the Individual Work Performance Questionnaire (IWPQ) by Koopmans (2014) for the dependent variable. A total of 89 respondents were involved in this study. The reliability analysis was conducted on the leadership style had a Cronbach&amp;#39;s alpha value of 0.819. Meanwhile, the work performance had a Cronbach&amp;#39;s alpha value of 0.726. The results of this study found that there is a significant relationship between leadership style and work performance among NARC officers where the correlation value (r=0.426, p=0.000).

Former landfills as environmental liabilities in central Chile: Evidence from a pilot study in Lepanto

AbstractThe former Lepanto landfill in the Santiago Metropolitan Region in Central Chile ceased operations in 2002 after 24 years. Currently, a biogas recovery center operates on this site. The objective of this pilot study was to evaluate the atmospheric concentrations of two polycyclic aromatic hydrocarbons (PAHs, i.e., phenanthrene and fluoranthene) at three points downwind from the former Lepanto landfill using passive air samplers and polyurethane foam disks to provide 3‐month average concentrations of contaminants during winter months. The results showed concentrations ranging between 2 and 65 ng m−3 and decreasing with distance from the former landfill. Given that the distance to other potential sources was similar in all sampling sites, the evidence found suggested that the former Lepanto landfill may contribute to the local PAH inventory. Thus, we suggest that former landfills are potential candidates for long‐term monitoring as they can represent a less‐known environmental liability for residents. Additionally, we provide insights on how the Chilean environmental protection system can establish reclamation steps for their management after closure.

Enhancing patient-clinician collaboration during treatment decision-making: study protocol for a community-engaged, mixed method hybrid type 1 trial of collaborative decision skills training (CDST) for veterans with psychosis

BackgroundPatient participation in treatment decision making is a pillar of recovery-oriented care and is associated with improvements in empowerment and well-being. Although demand for increased involvement in treatment decision-making is high among veterans with serious mental illness, rates of involvement are low. Collaborative decision skills training (CDST) is a recovery-oriented, skills-based intervention designed to support meaningful patient participation in treatment decision making. An open trial among veterans with psychosis supported CDST’s feasibility and demonstrated preliminary indications of effectiveness. A randomized control trial (RCT) is needed to test CDST’s effectiveness in comparison with an active control and further evaluate implementation feasibility.MethodsThe planned RCT is a hybrid type 1 trial, which will use mixed methods to systematically evaluate the effectiveness and implementation feasibility of CDST among veterans participating in a VA Psychosocial Rehabilitation and Recovery Center (PRRC) in Southern California. The first aim is to assess the effectiveness of CDST in comparison with the active control via the primary outcome, collaborative decision-making behavior during usual care appointments between veterans and their VA mental health clinicians, and secondary outcomes (i.e., treatment engagement, satisfaction, and outcome). The second aim is to characterize the implementation feasibility of CDST within the VA PRRC using the Practical Robust Implementation and Sustainability Model framework, including barriers and facilitators within the PRRC context to support future implementation.DiscussionIf CDST is found to be effective and feasible, implementation determinants gathered throughout the study can be used to ensure sustained and successful implementation at this PRRC and other PRRCs and similar settings nationally.Trial registrationClinicalTrials.gov NCT04324944. KAZUO ISHIGURO’S NEVER LET ME GO: AN ANALYSIS THROUGH THE LENS OF PLACE

This paper explores Kazuo Ishiguro's Never Let Me Go through the lens of spatial theory, examining how the construction and perception of place shape the novel's exploration of identity, memory, and human relationships. Drawing on Yi-Fu Tuan's concepts of space and place and Edward Relph's theory of place authenticity, the study investigates how key locations—such as Hailsham, the Cottages, and the recovery centers—function as more than mere settings, instead serving as active participants in the characters' emotional and existential journeys. The paper argues that these spaces embody the tensions between belonging and alienation, safety and confinement, and the known and the unknowable. By analyzing how the characters' sense of self is tied to their spatial experiences, this study sheds light on the broader implications of place in the novel, particularly in relation to themes of societal control, personal agency, and the inevitability of loss. Ultimately, the research highlights Ishiguro's nuanced portrayal of place as a medium through which human connections and existential truths are both forged and fractured.

Efforts of The Prosecutor as State Attorney to Recovery Recovery of State Losses Through Replacement Money

Purpose: The Corruption Law in Indonesia faces technical and normative challenges in collecting compensation for state losses. Technical obstacles include concealment of assets, inability of convicts or heirs to pay compensation, and rejection by heirs or third parties. In addition, corruptors often prefer to serve additional prison sentences as a substitute for compensation money. This law also lacks legal certainty and is disproportionate in regulating additional prison sentences. This research aims to find out the efforts of the Prosecutor as State Attorney in the effectiveness of returning compensation for State losses through the prosecution and execution of compensation for State losses resulting from criminal acts of corruption. Methode: This research is a normative-juridical type of research. Supported by 4 (four) approaches, philosophical approach, legislative approach, conceptual approach, and case approach.
