Product development methodology: non-quality caused in production by mistakes in product development, its measurement and improvement integration in the product development process

A new quality focused product development (NPD) methodology, applying lean manufacturing principles and Industry 4.0 paradigms is described in this paper. This methodology has been tested in a tailor-made equipment manufacturer for the retail sector, where tight product development times are needed. This requires a high agility in the processes, meaning the overlap of phases, the integration of processes, cooperation, and automation of tasks. We have generated a system concept that collects all the manufacturing incidences related to NPD, integrated in workshops stations, creating a system that enables real time data capture and a valuable feedback loop to NPD processes. Its implementation showed an improvement over 10% in the quality of the NPD process in a period of 3 months, an increase of the productivity in both product development and manufacturing areas, and a boost of the internal customer satisfaction in manufacturing area with the product development area.

Post-Operative Remote Monitoring for Same-Day Discharge Elective Orthopedic Surgery: A Pilot Study.

The purposes of this pilot study are to utilize digital remote monitoring to (a) evaluate the usability and satisfaction of a wireless blood pressure (BP) and heart rate (HR) monitor and (b) determine whether these data can enable safe mobilization at home after same-day discharge (SDD) joint replacement. A population of 23 SDD patients undergoing unicompartmental knee arthroplasty (UKA), total knee arthroplasty (TKA), or total hip arthroplasty (THA) were given a cellular BP/HR monitor, with real-time data capture. Patients took three readings after surgery, observing for specific blood pressure decreases, HR increases, or hypotensive symptoms. If any criteria applied, patients followed a hydration protocol and delayed ambulation. Home coaching was also provided to each patient. Patient experience was surveyed, and responses were assessed using descriptive statistics. Of 18 patients discharged (78%), 17 returned surveys, of which 100% reported successful device operation. The mean “ease of use” rating was 8.9/10; satisfaction with home coaching was 9.7/10; and belief that the protocol improved patient safety was 8.4/10. A total of 27.8% (n = 5) had hypotensive readings and appropriately delayed ambulation. Our pilot findings support the feasibility of and confirm the satisfaction with remote monitoring after SDD arthroplasty. All patients with symptoms of hypotension were successfully remotely managed using a standardized hydration protocol prior to safe mobilization.

Emerging epidemics: is the Zanzibar healthcare system ready to detect and respond to mosquito-borne viral diseases?

BackgroundEffective control of emerging mosquito-borne viral diseases such as dengue, chikungunya, and Zika requires, amongst other things, a functional healthcare system, ready and capable of timely detection and prompt response to incipient epidemics. We assessed the readiness of Zanzibar health facilities and districts for early detection and management of mosquito-borne viral disease outbreaks.MethodsA cross-sectional study involving all 10 District Health Management Teams and 45 randomly selected public and private health facilities in Zanzibar was conducted using a mixed-methods approach including observations, document review, and structured interviews with health facility in-charges and District Health Management Team members.ResultsThe readiness of the Zanzibar healthcare system for timely detection, management, and control of dengue and other mosquito-borne viral disease outbreaks was critically low. The majority of health facilities and districts lacked the necessary requirements including standard guidelines, trained staff, real-time data capture, analysis and reporting systems, as well as laboratory diagnostic capacity. In addition, health education programmes for creating public awareness and Aedes mosquito surveillance and control activities were non-existent.ConclusionsThe Zanzibar healthcare system has limited readiness for management, and control of mosquito-borne viral diseases. In light of impending epidemics, the critical shortage of skilled human resource, lack of guidelines, lack of effective disease and vector surveillance and control measures as well as lack of laboratory capacity at all levels of health facilities require urgent attention across the Zanzibar archipelago.

Cloud based analysis of Drinking Water quality parameters

The contamination of water bodies becomes a very big threat to the world. This happens due to pollution, domestic and industry waste and over population. This threat will lead to low rainfall, famines etc. In recent years, the research on monitoring and analyzing water quality is increasing. The water quality has to be examined in people living area, ponds, lakes, rivers and also the water bodies near the industries. People not having safe drinking water will lead to many diseases. The older methods of officials collecting sample from the water bodies, analyzing in labs, providing the results and taking control actions will be costly and takes more time. The main disadvantage is the lack of real time data analysis is not possible in the traditional systems. In this paper, a real time water quality method is analyzed and the results are verified with the traditional methods. This method uses high level processor, sensors, networking and measurement devices to give the real time data. The important parameters of water like temperature, turbidity and pH levels are monitored continuously. The user can verify the data anywhere anytime using the web interface provided in the method. The data analysis is also provided in tabulation and graphical representations. This paper provides an optimal method for monitoring the water quality in an efficient way. Traditional methods that rely on collecting water samples, testing and analyses in water laboratories are not only costly but also lack the capability for real-time data capture, analyses and fast dissemination of information to relevant stakeholders for making timely and informed decisions.

Precision Nutrition Assessment of Risk for Metabolic Syndrome in Active Duty Service Members

ObjectivesPrecision nutrition leverages the specificity of molecular and phenotypic differences in personalizing diet and lifestyle interventions.Objective for this phase of the study: 1) examine the effectiveness of gene-based nutrition counseling on behavior change measured by weight, body mass index (BMI), blood glucose, lipids, 25-hydroxyvitamin (OH) D, %body fat (BF), waist circumference, and blood pressure. MethodsIn this prospective multisite RCT, a baseline genomic profile from 70 diet-responsive genes/80 variants, is augmented by biomarkers specific to metabolic syndrome (MetS) risk for each subject. Treatment group (TG) receives gene-based nutrition counseling for six weekly sessions; Control group (CG) receives evidence-based nutrition content in pamphlets directed at preventing metabolic syndrome. A digital app provides real-time health data capture with continuous feedback and is validated by in-person interviews. Primary outcome is weight loss at 12 weeks. ResultsArmy NW cohort has enrolled 90 subjects to date; 49 are assigned to the TG. Sample demographics: males (70%), mean age 32 yrs, 58% married, 75% Caucasian, non-Hispanic, and 78% report some college education. In females, mean BMI 28.3, %BF 34.2, waist circumference 32.2 in; males mean BMI 30.1, %BF 29, waist circumference 40 in. For MetS components, 27/90 subjects show no abnormal components yet 41/90 have 2 or 3 alterations. Elevations noted as follows: fasting blood glucose in 47%, blood pressure in 38%, and waist circumference in 30%. High variant presence is noteworthy for genes with a role in obesity and hypertension. In 71% of subjects baseline 25(OH) D ≤ 30 ng/mL. Preliminary results for primary outcome of weight loss at 12 weeks reveal no change in TG (n = 21), an average loss of 5 lbs in CG (n = 21) within groups, and a significant difference between groups; TG 197.4 (39.3) vs CG 192.6 (40.3), p < .001. ConclusionsDigital health integration, along with genomic data and family history, can reveal early signals of risk in a young, generally healthy, military population. 16 What’s a Connected Barn and How Do We Use It?

Abstract Internet enabled swine facilities with smart controllers are forging the future directions of swine management, data analytics, and research. Today’s smart controllers have capabilities far beyond just ventilation operation, they are sophisticated data acquisition and control systems with cloud-based platforms for data aggregation and visualization. This technology has promoted the concept of a connected barn – a facility with autonomous, real-time data capture, analysis, and decision making. Connected barns can monitor indoor environmental conditions, personnel traffic, ventilation function, power consumption, bin weights, weather, refrigerators, etc. This new abundance of customizable measurement options sampled at a high-resolution will fundamentally change swine production. There is often a general lack of information related to conditions/events the pigs experience inside a barn that impact short-/long-term productivity and provide context for interpreting conventional data. Information is now superimposable with other data streams and new relationships between, for example, pig activity, environment, feed intake, daily gain, water consumption, etc. can be discovered. This will enable new avenues in nutrition, health, genetics, technology, husbandry, and engineering because a unified data stream, across numerous spaces is possible. Connected barns provide the link between pigs, data, and their environment. While opportunities are limitless, several key challenges need addressing: representativeness and accuracy of data; diligent management of advanced technology; and cultivation of transdisciplinary knowledge to understand limitations and make thoughtful inquiries. A connected barn is no longer a future concept, but a tangible production system ripe with a myriad of opportunities that will shape the course of animal production.

IoT platforms for the Mining Industry: An Overview

Industry 4.0 and the Internet of Things are now very common concepts as solutions that can revolutionize the industry. Constanttechnological progress increases the possibilities of using computer tools and solutions to support processes in industry and productionoptimization. The use of the Internet of Things is particularly important in complex processes in mining, enabling the extractionof valuable information from data. The integration of physical facilities in the enterprise enables the digitization of productionprocesses and the increase of efficiency and security.This article presents an overview of the selected internet of things platforms and analytical tools that can be used in industry, withparticular emphasis on the mining sector. Adolescents\u2019 sedentary time, affect, and contextual factors: An ecological momentary assessment study

BackgroundFew adolescents achieve sufficient levels of physical activity, and many are spending most of their time in sedentary behavior. Affective response following sedentary time may influence motivation to remain sedentary. Ecological Momentary Assessment (EMA) is a real-time data capture methodology that can be used to identify factors influencing sedentary time, such as the context of the home setting, and resulting affective state within a free-living setting. The purpose of this study was to evaluate the relationship between context at home and adolescent sedentary time, and the relationship of sedentary time and subsequent affect.MethodsAdolescents (n = 284; 10–16 y) participated in an EMA study that used random, interval-based sampling methods. Adolescents each received 22 unannounced surveys over 7-days through a smartphone application. One survey was randomly sent within each 2-h time-period. These time-periods occurred between 4:00 pm-8:00 pm on weekdays and 8:00 am-8:00 pm on the weekend. This 15-question survey included a series of questions on context (indoors/outdoors, alone/not alone) and positive affect. Adolescents concurrently wore an accelerometer at the hip, and the 30-min bout of accelerometry data prior to each survey was used in analyses. Mixed-effect location scale models were used to examine the association between context at home and sedentary time (stage 1) and the adjusted sedentary time and positive affect (stage 2), with each model adjusted for covariates.ResultsAdolescents were 12.6 ± 1.9 y of age on average, about half were White (58%), and engaged in high levels of sedentary behavior during the 30 min prior to the survey (21.4 ± 6.8 min). Most surveys occurred when adolescents were with others (59%) and indoors (88%). In Stage 1, both being alone and being indoors at home were positively associated with sedentary time (p < 0.001 for both). In Stage 2, adjusted sedentary time was not related to positive affect. Age was negatively related to positive affect (p < 0.001).ConclusionsBoth contextual factors, being alone and indoors at home, were related to additional time spent sedentary compared to being with someone or outdoors. After adjustment, sedentary time was not related to subsequent positive affect, indicating other factors may be related to adolescent’s positive affect in home settings.

Ethical Issues in Democratizing Digital Phenotypes and Machine Learning in the Next Generation of Digital Health Technologies

Digital phenotyping is the term given to the capturing and use of user log data from health and wellbeing technologies used in apps and cloud-based services. This paper explores ethical issues in making use of digital phenotype data in the arena of digital health interventions. Products and services based on digital wellbeing technologies typically include mobile device apps as well as browser-based apps to a lesser extent, and can include telephony-based services, text-based chatbots, and voice-activated chatbots. Many of these digital products and services are simultaneously available across many channels in order to maximize availability for users. Digital wellbeing technologies offer useful methods for real-time data capture of the interactions of users with the products and services. It is possible to design what data are recorded, how and where it may be stored, and, crucially, how it can be analyzed to reveal individual or collective usage patterns. The paper also examines digital phenotyping workflows, before enumerating the ethical concerns pertaining to different types of digital phenotype data, highlighting ethical considerations for collection, storage, and use of the data. A case study of a digital health app is used to illustrate the ethical issues. The case study explores the issues from a perspective of data prospecting and subsequent machine learning. The ethical use of machine learning and artificial intelligence on digital phenotype data and the broader issues in democratizing machine learning and artificial intelligence for digital phenotype data are then explored in detail.

Factors influencing peri-operative delays in neurosurgery operating theatres: a prospective study

BackgroundReduction of perioperative delays optimises theatre efficiency, and may provide an opportunity to improve the quality of healthcare delivered. The aim of this quality improvement project was to elucidate the prevalence, causes and impact of perioperative delays in Neurosurgery theatres at our local institution. MethodsA prospective audit analysing elective Neurosurgery cases was conducted from 1st Sept ‘18 to 28th Feb ’19 (6 months).Quantitative and qualitative analysis was performed on data from the Operating Room Scheduling Office System (ORSOS) real-time data capture forms and a custom-made survey filled out by theatre staff. NHS Benchmarking Network national neurosurgery theatre data were utilised as a control group. ResultsA total of 461 (90%) cases started late (average delay of 49 minutes and 21 seconds), 32% (n=164) cases finished early and 31 minutes was the average turnaround time. There was 16.8%(n=124) cancellation rate and 76% theatre utilisation. The most commonly cited causes for late start were lack of patient readiness (62.1%), change in list order (5.3%), anaesthetic delay (5.3%), surgeon delay (4.5%), turnaround delay (2.3%), bed issues (2.3%), emergency (1.5%), fire alarm (0.8%), kit delay (0.8%) and multi-factorial (15.2%). The total cost of late start delays over the study period was £304,266.18. ConclusionsThis study showed late-starts and early-finishes as part of sub-optimal theatre utilisation. Cost-saving could be immense if corporate factors improve such as availability of appropriate beds along with clinical ‘human’ factors. To ameliorate these delays, we are currently implementing strategies to improve compliance of lumbar spinal surgery as day-case guideline. In order to ensure long-term sustainability, this implementation will be re-audited using a Plan-Do-Study-Act cycle in 6 months’ time.

Ecological momentary assessment to explore fatigue, mood and physical activity levels in people receiving peritoneal dialysis: A study protocol.

Fatigue is a frequent and debilitating symptom for people with end-stage kidney disease (ESKD) receiving dialysis. Ecological momentary assessment (EMA) allows real-time data capture of day-to-day and diurnal variations. EMA has been used to study haemodialysis-related fatigue but not in people receiving peritoneal dialysis who are unique in their physical, environmental and logistical characteristics. The aim of this study is to explore the real-time associations between fatigue and mood (EMA mobile application) and objective physical activity levels (accelerometry) in people with EKSD receiving peritoneal dialysis. A 7-day intensive longitudinal study will be conducted. People receiving peritoneal dialysis within South Australia will be invited to participate. Five times throughout the day, participants will be prompted to answer 18 questions relating to fatigue (Visual Analogue Scale to Evaluate Fatigue Severity) and a single question for mood (Visual Analogue Mood Scale). Participants will continuously wear a GENEActiv accelerometer to capture physical activity levels during the 7-day period. At the completion of the data collection, participants will answer questions to evaluate the feasibility and acceptability of using EMA. This study will be the first to explore the real-time relationships between fatigue, mood and physical activity in people with ESKD receiving peritoneal dialysis. Understanding the fluctuations people experience and the relationships between mood and physical activity and fatigue will inform clinical management and well-being intervention development.

A qualitative exploration of the patient experience of erosive and non-erosive hand osteoarthritis

BackgroundMany patients with hand osteoarthritis (HOA) experience reduced health-related quality of life. This study sought to better understand the disease and treatment experience of individuals with HOA, explore any differences in experiences between erosive and non-erosive HOA sub-types, and evaluate content validity of the Michigan Hand Outcomes Questionnaire (MHQ) in HOA.MethodsThirty subjects from the United States (n = 15 erosive HOA; n = 15 non-erosive HOA) participated in semi-structured interviews: concept elicitation explored symptoms/impacts important to patients; cognitive interviews assessed understanding and relevance of the MHQ. A sub-sample participated in real-time data capture (RTDC) activities via a smartphone/tablet app over 7 days. Verbatim transcripts were coded using Atlas.ti software and thematically analyzed. Concept saturation and MHQ content validity were evaluated.ResultsMost participants reported experiencing pain, swelling and stiffness, symptoms that most commonly had a direct impact on physical functioning. Substantial impacts on activities of daily living, emotional functioning, sleep and work were also reported. RTDC findings corroborated concept elicitation findings. There were no notable differences between erosive and non-erosive HOA, except nodules were reported more frequently in erosive disease. Most participants used analgesic treatments, but effects were short-lived. Pain was the symptom most frequently reported as most bothersome and important to treat. Concept saturation was achieved. MHQ items and instructions were well understood and relevant to most participants; stiffness and swelling were reported as important symptoms not included in the MHQ.ConclusionsThis study characterizes key symptoms of HOA which are burdensome for patients and not well controlled by current therapies, highlighting an unmet treatment need. Although the study is limited by a small sample size that may not be representative of the broader erosive and non-erosive HOA population, concept saturation was achieved, and our findings suggest that disease experience is similar for patients with erosive and non-erosive HOA. Evaluation of stiffness and swelling items in conjunction with the MHQ may enhance relevance and improve measurement precision to assess important domains of HQRoL in an HOA population.

The prevalence of dyads in social life.

A salient objective feature of the social environment in which people find themselves is group size. Knowledge of group size is highly relevant to behavioural scientists given that humans spend considerable time in social settings and the number of others influences much of human behaviour. What size of group do people actually look for and encounter in everyday life? Here we report four survey studies and one experience-sampling study (total N = 4,398) which provide evidence for the predominance of the dyad in daily life. Relative to larger group sizes, dyads are most common across a wide range of activities (e.g., conversations, projects, holidays, movies, sports, bars) obtained from three time moments (past activities, present, and future activities), sampling both mixed-sex and same-sex groups, with three different methodological approaches (retrospective reports, real-time data capture, and preference measures) in the United States and the Netherlands. We offer four mechanisms that may help explain this finding: reciprocity, coordination, social exclusion, and reproduction. The present findings advance our understanding of how individuals organize themselves in everyday life.

A perspective on the use of ecological momentary assessment and intervention to promote stroke recovery and rehabilitation

ABSTRACT Motivated by recent advances in technologies, ecological momentary assessment (EMA), and ecological momentary intervention (EMI) have seen a rise in behavioral medicine research that in real-time, informs the context for the behavior and prompts interventions to change that behavior in the natural setting when necessary. However, EMA and EMI have yet to be fully embraced in the field of stroke rehabilitation. Our objective is to provide a theoretically based perspective for the combined and synergistic use of EMA and EMI to promote person-centered, recovery-based durable changes in functional movement behaviors of stroke survivors. Research abounds for non-stroke populations with emerging evidence for the benefits of using real-time data capture techniques (i.e. EMA) coupled with EMI to better customize the content and timing of interventions to the inherent fluctuations in state and context that encompass the target behavior. We review existing EMA and EMI literature broadly in behavioral medicine and psychological science to identify how real-time repeated sampling technology has been used in the context of stroke rehabilitation and to delineate the pros and cons of this approach in general with non-stroke populations. We propose a coupled EMA and EMI strategy be used in conjunction with existing stroke recovery and rehabilitation practices. There is tremendous potential to effectively personalize recovery-promoting interventions to achieve durable behavior change, and importantly, shift the focus of rehabilitation practice from the health-care provider and clinical environment to the individual and their lived experience in the home and community.

Assessment of VaxTrac electronic immunization registry in an urban district in Sierra Leone: Implications for data quality, defaulter tracking, and policy

BackgroundIn 2016, the Sierra Leone Ministry of Health and Sanitation (MoHS) piloted VaxTrac, an electronic immunization registry (EIR), in an urban district to improve management of vaccination records and tracking of children who missed scheduled doses. We aimed to document lessons learned to inform decision-making on VaxTrac and similar EIRs’ future use. MethodsTen out of 50 urban health facilities that implemented VaxTrac were purposively selected for inclusion in a rapid mixed-method assessment from November to December 2017. For a one-month period, records of six scheduled vaccine doses among children < 2 years old in VaxTrac were abstracted and compared to three paper-based records (register of under-two children, daily tally sheet, and monthly summary form). We used the under-two register as the reference gold standard for comparison purposes. We interviewed and observed 10 heath workers, one from each selected facility, who were using VaxTrac. ResultsOverall, VaxTrac captured < 65% of the vaccine doses reported in the paper-based sources, but in the largest health facility VaxTrac captured the highest number of doses. Two additional notable patterns emerged: 1) the aggregated data sources reported higher doses administered compared to the under-two register and VaxTrac; 2) data sources that need real-time data capture during the vaccination session reported fewer doses administered compared to the monthly HF2 summary form. Health workers expressed that the EIR helped them to shorten the time to manage, summarize, and report vaccination records. Workflows for data entry in VaxTrac were inconsistent among facilities and rarely integrated into existing processes. Data sharing restrictions contributed to duplicate records. ConclusionAlthough VaxTrac helped to shorten the time to manage, summarize, and report vaccination records, data sharing restrictions coupled with inconsistent and inefficient workflows were major implementation challenges. Readiness-to-introduce and sustainability should be carefully considered before implementing an EIR.

Design and Modeling of Resource-constrained IoT based Body Area Networks

Due to the recent advancement and development of sensing, wireless, and communication technologies, therehas been a shift in attention towards Body Area Networks (BANs). One of the most important services of BAN is the remote monitoring of patients, enabling doctors to observe, diagnose, and prescribe the patients without being physically present. Various vital signs are being monitored by body sensing devices installed inside, on or off the body of patients, but most of these devices are constrained in terms of resources such as storage, processing, bandwidth, and energy due to their smaller size. This paper aims at highlighting the key findings related to BAN applications, constrained resources, and various resource management techniques. The paper also presents the design and modeling of a resource-constrained BAN system and discusses the various scenarios of BAN in the context of resource constraints. It further proposes an Advanced Edge Clustering (AEC) approach to manage the resources such as energy, storage, and processing of BAN devices while performing real-time data capture of critical health parameters and detection of abnormal patterns. The comparison of the AEC approach is done with the Stable Election Protocol (SEP) through simulations and empirical data analysis. The results show an improvement in energy, processing time and storage requirements for the processing of data on BAN devices in AEC as compared to SEP.

Real Time Data Capture: A Response to Unsustainable Dumping of Disposable Diapers and Sanitary Pads in Gweru City, Zimbabwe

The sustainable management of used disposable diapers and sanitary pads (Absorbent Hygienic Products) is undoubtedly a topical issue in municipalities across the developing world. This article is based on a bigger study which investigated how the city of Gweru can best manage this waste using a model that adjusts itself in response to real time data. Purposive and cluster sampling were employed to select the wards and the respondents respectively. Data was gathered using both qualitative and quantitative techniques. Particularly, semi-structured questionnaires were augmented by observations and in-depth interviews of key informants. The study found out that the municipality in Gweru was fighting a problem they had not yet measured; hence they were acting on vague data. Results of this study are likely to stimulate further research on how real time data on waste can be gathered. The results are also likely to shape the future of waste management in cities across the developing world. The main contribution of this study to the existing body of literature is its recommendation on the usage of real time data to drive waste management.

Treatment group receives gene-based nutrition counseling; control group receives evidence-based nutrition pamphlets. A digital app is used for real-time health data capture, validated by in-person RD interviews. After the initial 6 weeks, both groups use study resources independently and return for final body composition, BP, and serum biomarkers at 12 weeks. Results Preliminary findings: Northwest cohort has enrolled 26 of 75 active duty soldiers. Sample has slightly more males (54%), mean age 29.6 years, 64% married, 77% Caucasian, non-Hispanic, and 60% report some college. About 50% lost an average of 15 lbs to join the Army. Mean anthropometrics for females: BMI 27.4, 30.3% body fat, waist circ 32.2 in; for males: BMI 30.7, 28% body fat, waist circ 40 in. High variant presence noted for obesity and hypertension. Signals of abnormal biomarkers are insignificant although 60% of sample has baseline serum vitamin D less than 30 ng/mL. Conclusions This initiative has begun to advance the precision nutrition agenda in the military setting where advanced practice RNs and RDs will soon use genomic risk data to guide diet and lifestyle counseling to support personal and military readiness and optimize mission performance.
