Clindamycin Resistance Detection among MRSA at a Tertiary Care Hospital: A Cross-sectional Study

Introduction: Antimicrobial resistance is on the rise, posing a global threat to the success of modern surgical procedures. A common and notorious superbug causing community and hospital-acquired infections is Methicillin-resistant Staphylococcus aureus (MRSA). Its potential for easy and rapid spread leads to increased mortality in hospitals. Clindamycin, a relatively inexpensive and effective drug, is empirically prescribed for the treatment of MRSA infections. Due to the risk of misidentifying clindamycin resistance as susceptibility, leading to treatment failure, its judicious use after D-test is advocated. Aim: To isolate Staphylococcus aureus (S.aureus) from various clinical specimens and to identify MRSA using cefoxitin disc (30 μg), further determining the incidence of inducible clindamycin resistance among MRSA by the disc diffusion method (D-test). Materials and Methods: The present cross-sectional study was conducted in the Department of Microbiology, Government Mohan Kumaramangalam Medical College Hospital (GMKMCH), Salem, Tamil Nadu, India, from June 2023 to August 2023. Study was conducted using 185 S.aureus isolates collected from samples received from patients of all genders treated at the present study hospital. Various clinical samples such as pus, sputum, blood, urine and fluids collected from patients treated at the study Institute Salem during the study period were included. A cefoxitin disc (30 μg) was used to detect MRSA among the 185 S.aureus isolates by disc diffusion method as per Clinical and Laboratory Standards Institute (CLSI) guidelines, which were further tested for clindamycin resistance using clindamycin (2 μg) with erythromycin (15 μg) discs by the D-test. The data of the study were analysed using Statistical Package for the Social Sciences (SPSS) software version 21.0. Further descriptive statistics and a chi-square test with a level of significance, p-value ≤0.005 (statistically significant), were used. Results: Among 185 S.aureus isolates, 130 (70.27%) were Methicillin-sensitive S.aureus (MSSA) and 55 (29.7%) were MRSA. Inducible clindamycin resistance was observed in 22 (40%) isolates, while 17 (30.9%) isolates showed constitutive resistance, 9 (16.36%) showed the MS phenotype, and the remaining 7 (12.72%) showed the susceptible phenotype. Among the clindamycin resistance patterns in MRSA, inducible clindamycin resistance was reported predominantly. Conclusion: The majority of S.aureus was isolated from pus samples, highlighting its importance as a pyogenic microorganism. The current study records a high rate of MRSA resistance among S.aureus isolates. The present study reports a higher rate of inducible clindamycin resistance among MRSA isolates when compared to other phenotypes of clindamycin resistance. Therefore, routine D-testing must be implemented to identify inducible resistance to clindamycin in MRSA to avoid treatment failure.

Peculiarities of antibiotic resistance of Staphylococcus aureus strains isolated from nosocomial infections

Aim. To study of the antibiotic resistance of S. aureus strains isolated from nosocomial infections (pneumonia, surgical site infections and sepsis) in a multidisciplinary surgical clinic.
 Methods. Microbiological testing of sputum in 41 patients with pneumonia, of samples (wound, abscess, drainage) obtained from 40 patients with surgical site infections (SSI) and of blood from 46 patients with signs of sepsis was performed. The obtaining cultures were identified by conventional methods (including morphological, cultural, biochemical features, etc.). Disc diffusion method was used to detect methicillin-resistant S. aureus (MRSA) strains. Inducible clindamycin resistance (ICR) of S. aureus strains was determined by double disk approximation test (D-test).
 Results. Methicillin-resistant S. aureus was found in 14.3% (2 of 14) of the patients with surgical site infections, in the sputum 27.3% (3 of 11) of the patients with pneumonia, and in the blood 50.0% (7 of 14) of the patients with sepsis (p 0.05). The rate of inducible clindamycin resistance of isolated S. aureus strains in patients with surgical site infections (2 of 14 cases, 14.3%) and with pneumonia (2 of 11 cases, 18.2%) did not statistically significant difference with the rate of methicillin resistance (p 0.05). However the rate was significantly lower septic infections 7.1% and 50.0% respectively (p=0.0328).
 Conclusion. Among S. aureus strains isolated from nosocomial infections, the rate of methicillin-resistant S. aureus had not depended on the type of nosocomial infection; the rate of inducible clindamycin resistance in septic infections was lower than resistance to methicillin.

Clindamycin Resistance among Methicillin Resistant <i>Staphylococcus aureus</i> Isolated from Human and Respective Household Swine in Greater Kabale Region—South Western Uganda

Introduction: S. aureus is recognized as the common cause of nosocomial and community-acquired infections. Macrolide-Lincosamide-Streptogramin B (MLSB) is thought to be alternative therapies against MRSA infections. Clindamycin is the most favored agent because of exceptional pharmacokinetic characteristics. However, increasing resistance to clindamycin among MRSA strains is a serious challenge. The current study investigated the profile of clindamycin resistance among MRSA isolates from Humans, and their respective livestock (in particular swine) using D-test in greater Kabale region. Materials and Methods: Three hundred phenotypic MRSA isolates previously isolated from Humans and swine were confirmed by mecA PCR. We performed D-test using erythromycin (15 μg) and clindamycin (2 μg) discs in accordance to Clinical and Laboratory Standards Institute (CLSI) protocol. Results: Of all 300 MRSA isolates, 6% (n = 18) were sensitive to Erythromycin and Clindamycin (S). The rate of inducible clindamycin resistance (iMLSB) was 42% (n = 125) and 38% (n = 115) was resistance to both Erythromycin and clindamycin (cMLSB). However, 14% (n = 42) were resistant to erythromycin but sensitive to clindamycin (MS) without “D” zone negative. Conclusion: Clindamycin resistance (both cMLSB and iMLSB) among MRSA was high and “D” test should be adopted routinely during antimicrobial susceptibility testing by disc diffusion testing to rapidly detect iMLSB and cMLSB.

Antibiotic resistance and biofilm production among the strains of Staphylococcus aureus isolated from pus/wound swab samples in a tertiary care hospital in Nepal

BackgroundThe increasing drug resistance along with inducible clindamycin resistance, methicillin resistance and biofilm production among the strains of Staphylococcus aureus are present as the serious problems to the successful treatment of the infections caused by S. aureus. So, the main objectives of this study were to determine the antimicrobial susceptibility patterns along with the rates of inducible clindamycin resistance, methicillin resistance and biofilm production among the strains of S. aureus isolated from pus/wound swab samples.MethodsA total of 830 non-repeated pus/wound swab samples were processed using standard microbiological techniques. The colonies grown were identified on the basis of colony morphology, Gram’s stain and biochemical tests. Antimicrobial susceptibility testing was performed by Kirby–Bauer disc diffusion technique. Detection of inducible clindamycin resistance was performed by D test, while detection of methicillin resistant S. aureus (MRSA) was performed by determination of minimum inhibitory concentration of oxacillin by agar dilution method. Similarly, detection of biofilm formation was performed by microtiter plate method. Strains showing resistance to three or more than three different classes of antibiotics were considered multidrug resistant.ResultsTotal 76 samples showed the growth of S. aureus, among which 36 (47.4%) contained MRSA and 17 (22.4%) samples were found to have S. aureus showing inducible clindamycin resistance. Among the S. aureus isolated from outpatients, 41.9% were MRSA. Highest rates of susceptibility of S. aureus were seen toward linezolid (100%) and vancomycin (100%). Similarly, S. aureus isolated from 35 (46.1%) samples were found to be biofilm producers. Higher rate of inducible clindamycin resistance was seen among MRSA in comparison to methicillin susceptible S. aureus (MSSA). Similarly, higher rates of multidrug resistance and methicillin resistance were found among biofilm producing strains in comparison to biofilm non producing strains.ConclusionsThe rate of isolation of MRSA from community acquired infections was found to be high in Nepal. Increased rate of inducible clindamycin resistance as compared to previous studies in Nepal was noted. So for the proper management of the infections caused by S. aureus, D test for the detection of inducible clindamycin resistance should be included in the routine laboratory diagnosis. Further, detection of biofilm production should also be included in the routine tests. Linezolid and vancomycin can be used for the preliminary treatment of the serious infections caused by S. aureus.

Assessment of Clindamycin and Erythromycin Resistance, and Inducible Clindamycin Resistance in Streptococcus Group B Isolated from Urinary Samples of Outpatient Women in Tehran

Background: Streptococcus group B (GBS) or Streptococcus agalactiae is typically associated with neonatal disease and infection in pregnant women. Mortality of GBS sepsis in neonates is over 50% and is particularly high in preterm infants. GBS also causes invasive infection in pregnant and non-pregnant women including urinary tract infection (UTI). Penicillin-derived antibiotics remained as choice drugs for treatment of GBS infection; however, Erythromycin and Clindamycin are useful in cases of allergic to Penicillin. The aim of this study was to investigate the resistance to Erythromycin and Clindamycin, especially inducible Clindamycin resistance, in GBS isolated from urinary samples of women who attended medical offices in Tehran, Iran. M aterials and Methods: This study was conducted on 5000 urine samples from Jan. 2011 to Oct. 2012 that 104 GBS were isolated. The isolates were identified as GBS using laboratory criteria. Antimicrobial susceptibility test was done by Erythromycin disk 15µg and Clindamycin disk 2µg for observation inducible resistant D-zone test by double-disk diffusion method with Erythromycin and adjacent Clindamycin. R es ults: Among the 5000 urine samples 104 (2.08%) were Beta hemolytic GBS. Of the 104 isolated GBS, 22 (21.2%) were resistance, 24 (23%) were intermediate, and 58 (55.8%) were susceptible to Erythromycin; however, 24 (23%) were resistance, 5 (4.8%) were intermediate, and 75 (72.2%) were susceptible to Clindamycin. Of the 22 Erythromycin-resistant isolates, 10 (9.5% in total GBS isolated) displayed the D zone; it means they have inducible Erythromycin resistant to Clindamycin. C onclusion: Various studies in other countries report lower rates of inducible Clindamycin resistance; it indicates the use of more macrolides in the treatment of UTI.

High rate of inducible clindamycin resistance in Staphylococcus aureus isolates--a multicenter study in Tokyo, Japan.

The resistance of Staphylococcus aureus (S. aureus) to antibiotics is an increasing problem. Clindamycin has been used as empiric therapy for the rising incidence of community-acquired methicillin-resistant S. aureus (MRSA). As such, the local rate of inducible resistance against clindamycin is an important consideration. This multicenter study was conducted to identify the incidence of inducible clindamycin resistance of S. aureus isolates in Tokyo, the most populous city in Japan. A total of 2408 adult and pediatric samples were collected from a university hospital and two pediatric hospitals between January 2011 and December 2011. Among the 2341 samples analyzed, the incidence of inducible clindamycin resistance in erythromycin-resistant and clindamycin-susceptible/intermediate isolates was found to be 91% (n = 585), a figure much higher compared to most reports from other countries. In conclusion, we found a very high rate of inducible clindamycin resistance in macrolide-resistant S. aureus isolates in our geographic area.

Detection of inducible clindamycin resistance in Staphylococcus aureus and coagulase-negative staphylococci - a study from South India

Background: Inducible clindamycin resistance is a major concern for the use of clindamycin to treat staphylococcal infections. Aims: To determine the prevalence of inducible clindamycin resistance in clinical isolates of Staphylococcus spp. and the susceptibility pattern of the isolates. Materials and Methods: A total of 300 isolates of Staphylococci spp. recovered from different clinical specimens were studied. All the Staphylococcus spp. were identified by conventional microbiological methods. Inducible clindamycin resistance was detected by double disk approximation test (Dtest).Results: Of the 300 isolates, 176 were identified as S. aureus, while 124 were coagulase negative staphylococci (CoNS). The rates of inducible clindamycin resistance in methicillin resistant S. aureus (MRSA), methicillin sensitive S. aureus (MSSA), methicillin resistant CoNS (MR-CoNS) and methicillin sensitive CoNS (MSCoNS) were 75.0%, 24%, 18.8% and 11.1%, respectively. The inducible clindamycin resistance was significantly more among MRSA compared to methicillin sensitive S. aureus (MSSA) (P value < 0.0001). Majority of the MRSA isolates were susceptible to clindamycin, vancomycin and linezolid, while most of them were resistant to erythromycin, gentamicin, ciprofloxacin, tetracycline and sulfamethoxazole-trimethoprim. Conclusion: In view of the significant in vitro inducible clindamycin resistance in Staphylococcus spp., we recommend that D test should be used as a mandatory method in microbiology laboratories to avoid misinterpretation of clindamycin result.

Significance of MRSA strains in community associated skin and soft tissue infections

The study was conducted to determine the antibiotic susceptibility profile of community-associated methicillin resistant Staphylococcus aureus (CAMRSA) strains isolated from infections. S. aureus strains were isolated from clinical specimens using the standard procedures. CDC definition was used to classify CAMRSA. Antibiotic susceptibility test was done using Kirby-Bauer disk diffusion method. Double disk diffusion method (D-test) was used to detect inducible macrolide, lincosamide and streptograminB resistance (inducible MLSB resistance). A total of 83 CAMRSA were isolated from abscesses and other skin infections in persons without known risk factors for MRSA infection. All CAMRSA were susceptible to vancomycin. Out of 83 CAMRSA, 13 (15.65%) were D-test positive (inducible MLSB positive) and 6 (7.23%) were positive for constitutive MLSB resistance. Eight strains (9.63%) were resistant to tetracycline and 26 (31.32%) strains were resistant to erythromycin. Increased rate of inducible clindamycin resistance among CAMRSA indicates the importance of identification of such strains by D test to avoid treatment failure when clindamycin is used.

Phenotypic detection of inducible clindamycin resistance among Staphylococcus aureus isolates by using the lower limit of recommended inter-disk distance

Clindamycin is one of the important alternative antibiotics in the therapy of Staphylococcus aureus, particularly in methicillin-resistant S. aureus (MRSA) infections. Inducible clindamycin resistance (iMLS B--inducible Macrolide-Lincosamide-Streptogramin B resistance) is a critical factor in antimicrobial susceptibility testing. To know the rate of inducible clindamycin resistance among clinical isolates of Staphylococcus aureus in our hospital by Disk approximation test (D-test) using the average recommended inter-disk distance and comparing the results with that of D-test using the lower limit of recommended inter-disk distance. A total of 51 erythromycin-resistant and clindamycin-susceptible S. aureus isolates were subjected to disk approximation testing with 21 +/- 1 mm and 15 mm edge-to-edge distance between the clindamycin and erythromycin disks. Z-test levels. Among 51 erythromycin-resistant and clindamycin-susceptible S. aureus isolates, 25 (49%) were recorded as inducible clindamycin resistant by D-test with 21 +/- 1 mm edge-to-edge distance between the clindamycin and erythromycin disks. When we re-tested all the 51 strains by D-test with 15 mm inter-disk distance, we identified 14% more iMLS B strains previously reported as D-test negative. Z-test for MRSA indicates that 15 mm edge-to-edge distance has significant advantage. Since the incidence of inducible clindamycin resistance is high (63% in our study), accurate identification of inducible clindamycin resistance is important to prevent therapeutic failure in infections caused by these strains. We suggest the use of D-test with 15 mm edge-to-edge inter-disk distance for detecting iMLS B .

Influence of Disk Separation Distance on Accuracy of the Disk Approximation Test for Detection of Inducible Clindamycin Resistance inStaphylococcusspp

We undertook this study to assess the accuracy of the clindamycin-erythromycin disk approximation test (D-test) for detection of inducible clindamycin resistance in Staphylococcus spp. One hundred sixty-three Staphylococcus aureus and 68 coagulase-negative Staphylococcus (CoNS) spp. which were erythromycin nonsusceptible but clindamycin susceptible were tested using the D-test performed at both 15-mm and 22-mm disk separations and compared with genotyping as the "gold standard." The rate of inducible clindamycin resistance was 96.3% for S. aureus and 33.8% for CoNS spp. The sensitivities of the D-tests performed at 15 mm and 22 mm were 100% and 87.7%, respectively, and specificities were 100% for both. The use of 22-mm disk separation for the D-test to detect inducible clindamycin resistance results in an unacceptably high very major error rate (12.3%). All isolates with false-negative results harbored the ermA gene, and the majority were methicillin-resistant Staphylococcus aureus. False-negative results were associated with smaller clindamycin zone sizes and double-edged zones. We recommend using a disk separation distance of </=15 mm. There is wide geographic variation in the rates of inducible clindamycin resistance, and each laboratory should determine the local rate before deciding whether to either perform the D-test routinely or else report that all erythromycin-resistant S. aureus isolates are also clindamycin resistant.
