Evaluation of continuous positive airway pressure adherence and its contributing factors

BackgroundContinuous positive airway pressure (CPAP) adherence may vary based on the study design and country. ObjectivesTo investigate long-term CPAP adherence and elucidate associated factors. MethodsA multicenter retrospective observational cohort study was conducted over two years. The study included patients diagnosed with obstructive sleep apnea who were prescribed CPAP (n = 1261). Adherence was assessed for 90 days and 365 days. Centers for Medicare and Medicaid Services (CMS) criteria (≥4 h/night on ≥70 % of nights) and research adherence criteria (≥4 h/night during the observational period) were used. Factors influencing CPAP adherence (CMS criteria) during 90 and 365 days were also evaluated. Continuous variables were categorized into low, middle, and high classes, and logistic regression analysis with interaction was performed. ResultsCPAP adherence to CMS criteria for 90 and 365 days was 45.6 % and 44.7 %, respectively, whereas adherence to research criteria was 57.0 % and 54.1 %, respectively. Middle age, high age, and high apnea ratio independently increased CPAP adherence. Conversely, a high rapid eye movement apnea-hypopnea index ratio decreased CPAP adherence. Although high body mass index (BMI) had no independent effect on adherence, its interaction with high 3 % oxygen desaturation index increased 90-day CPAP adherence. However, high BMI decreased 90-day CPAP adherence in males. The interaction between middle age and high BMI was associated with decreased CPAP adherence over 365 days. ConclusionsAdherence to CPAP in real-world settings is modest. Various factors influence CPAP adherence. BMI may exert varied effects on CPAP adherence depending on the accompanying factors.

0526 Effect of Body Position and Sleep State on Sleep Disordered Breathing Distribution in Infants Ages 6 to 12 months

Abstract Introduction The effect of body position and sleep state on events distribution in infants ages 6 to 12 months needs further study. We aimed to study this distribution and determine the difference of body position in infants with and without sleep disordered breathing. Methods This was a single center, retrospective study that included infants ages 6 to 12 months who were evaluated for suspected sleep disordered breathing and underwent a full overnight polysomnography in an academic sleep disorders center over a period of 10 years. Infants with ages 6 to 12 months with obstructive sleep apnea hypopnea index (OAHI) and central apnea index (CAI) ≥ 1 (sleep disordered breathing [SDB] group) were compared with infants with normal AHI who represented the control group (CG). Total OAHI, CAI, apnea hypopnea index (AHI) in different body positions, rapid eye movement (REM), and Non-REM sleep were collected. Results A total of 36 infants had SDB and 18 infants had normal AHI. The median age was not significantly different between both groups with a median age of 10 months (IQR 7.75-11) in SDB group and 8.5 months in CG (IQR 7.25- 10.00) (p=0.258). Male sex was predominant in both groups (58.3% in SDB group and 61.1% in CG). There were no significant differences between both groups in the BMI, median total sleep time, REM% sleep time, and Non-REM% sleep time. The supine sleep time in the SDB group [258.90 minutes (IQR 149.35-381.25)] was significantly higher than the supine sleep time in the CG [165.90 minutes (IQR 54.55 to 285.05)] (p=0.035). In the SDB group, REM AHI [8.55 (IQR 5.02 – 15.25] was significantly higher than Non-REM AHI [0.50 (IQR 0.30-1.20)] (p< 0.001). In the SDB group, there were no significant differences in the supine vs. off-supine distribution of respiratory events. Conclusion Obstructive and central respiratory events in infants ages 6 to 12 months occurred predominantly in REM sleep compared to Non-REM sleep. Infants with SDB significantly spent more time in supine position when compared to infants without SDB but the respiratory events distribution in infants with SDB did not have a significant positional distribution. Obstructive sleep apnea in aboriginal Australians: polysomnographic outcomes and symptom perception post-continuous positive airway pressure implementation.

Obstructive sleep apnea (OSA) is reported to be highly prevalent among Aboriginal Australians. However, no studies have assessed the implementation and efficacy of continuous positive airway pressure (CPAP) therapy in this population. Hence, we compared the clinical, self-reported perception of sleep quality and polysomnographic (PSG) characteristics among Aboriginal patients with OSA. Adult Aboriginal Australians who underwent both diagnostic (Type 1 and 2) and in-lab CPAP implementation studies were included. Total of 149 patients were identified (46% female, median age 49 years, body mass index 35 kg/m2). The OSA severity was 6% mild, 26% moderate, and 68% severe on the diagnostic PSG. On application of CPAP, there were significant improvements in; total arousal index (diagnostic 29 to 17/h on CPAP), total apnea-hypopnea index (AHI) (diagnostic 48 to 9/h on CPAP), non-rapid eye movement AHI (diagnostic 47 to 8/h on CPAP), rapid eye movement (REM) AHI (diagnostic 56 to 8/h on CPAP) and oxygen saturation (SpO2) nadir (diagnostic 77% to 85% on CPAP) (p < 0.001 for each). Following a single night of CPAP, 54% of patients reported sleeping "better than normal" compared to 12% following the diagnostic study (p = 0.003). In multivariate regression models, males had a significantly lesser change in REM AHI than females (5.7 events/hour less change (IQR 0.4, 11.1), p = 0.029). There is substantial improvement in several sleep-related domains on the application of CPAP among Aboriginal patients with a good initial acceptance of treatment. Whether the positive impact observed in this study translates to better sleep health outcomes with long-term adherence to CPAP therapy is yet to be assessed.

The firstly visited department affects the acceptance of CPAP in patients with obstructive sleep apnea: a cohort study

BackgroundContinuous positive airway pressure (CPAP) therapy is the first-line treatment for obstructive sleep apnea (OSA). However, the low acceptance rate of CPAP remains a challenging clinical issue. This study aimed to determine the factors that influence the acceptance rate of CPAP.MethodsThis retrospective cohort study was conducted at the sleep center of Shuang-Ho Hospital. Initially, 1186 OSA patients who received CPAP therapy between December 2013 and December 2017 were selected, and finally, 1016 patients were analyzed. All patients with OSA received CPAP therapy for at least 1 week, and their acceptance to treatment was subsequently recorded. Outcome measures included patients’ demographic and clinical characteristics (sex, age, BMI, comorbidities, history of smoking, and the medical specialist who prescribed CPAP treatment), polysomnography (PSG) results, and OSA surgical records.ResultsPatients with a lower CPAP acceptance rate were referred from otolaryngologists (acceptance rate of otolaryngology vs. others: 49.6% vs. 56.6%, p = .015), in addition to having a lower apnea–hypopnea index (AHI) (acceptance vs. non-acceptance: 55.83 vs. 40.79, p = .003), rapid eye movement AHI (REM-AHI) (acceptance vs. non-acceptance: 51.21 vs. 44.92, p = .014), and arousal index (acceptance vs. non-acceptance: 36.80 vs. 28.75, p = .011). The multiple logistic regression model showed that patients referred from otolaryngology had a lower CPAP acceptance rate (odds ratio 0.707, p = .0216) even after adjusting for age, sex, BMI, AHI, REM-AHI, arousal index, comorbidities, and smoking status.ConclusionsBefore their initial consultation, patients may already have their preferred treatment of choice, which is strongly linked to the type of medical specialists they visit, and consequently, affects their rate of acceptance to CPAP therapy. Therefore, physicians should provide personalized care to patients by exploring and abiding by their preferred treatment choices.

0858 Under reporting of Apnea Hypopnea Index (AHI) in patients with predominant REM sleep disordered breathing on antidepressants

Abstract Introduction The prevalence of Rapid Eye Movement (REM) related Sleep Disordered Breathing (SDB) ranges from 13.5% to 36.7% in patients suspicious to have SDB.1 It is reported that patients with REM related SDB often have associated depression and are commonly on anti-depressants.2 Antidepressants suppress overall REM sleep duration. Significant sleep disordered breathing is known to occur during REM sleep. These patients commonly present with excessive daytime sleepiness and fatigue. Obstructive sleep apnea (OSA) diagnosis is often missed in these patients secondary to REM sleep suppression and subsequent underreporting of AHI.1. Conwell W, Patel B etal 2012 PMID: 21614575. 2. Geckil AA, Ermis H. 2020 PMID: 30949927. Report of Cases: Case 1 is a 53-year-old obese white female with stable depression on citalopram 20mg presented with witnessed loud snoring and apneas, sleep onset and maintenance insomnia, daytime fatigue and hypersomnolence. Her Epworth Sleep Score (ESS) was 13/24. Her in lab baseline polysomnogram demonstrated overall AHI 3.8/hour and REM only AHI 24.3/hour. To further explore her persistent daytime sleepiness a Mean Sleep Latency Test (MSLT) was ordered. Citalopram was held 2 weeks prior to MSLT. In the PSG her overall AHI increased to 7.6/hour diagnosing her with mild OSA. PAP therapy was initiated that improved patients nocturnal sleep, daytime fatigue and hypersomnolence. Case 2 is a 54-year-old overweight white female with past medical history of chronic stable depression on Wellbutrin SR 100 mg daily presented with significant day time fatigue and hypersomnolence (ESS 15/24). Her baseline PSG reported AHI 1.7/hour with REM only AHI 10.7/hour. However due to persistent excessive daytime sleepiness MSLT was ordered. Her Wellbutrin was held 2 weeks prior to testing. Her overnight PSG demonstrated increase in her AHI to 7.9/hour and REM AHI to 34.2/hour. Mild obstructive sleep apnea was diagnosed, and PAP therapy was initiated that improved her hypersomnolence. Conclusion These cases illustrate that antidepressants likely mask obstructive sleep apnea by suppressing REM sleep. Given the underreported AHI in patients with REM SDB who are on anti-depressants it may be prudent to hold anti-depressants for 2 weeks to establish an accurate diagnosis of sleep apnea. Proportion of rapid eye movement sleep related obstructive sleep apnea (REM related OSA) in patients with sleep disordered breathing: A cross sectional study.

Background and Objectives:Obstructive sleep apnea (OSA) during rapid eye movement (REM) stage of sleep is gaining importance in recent years. This study was done to determine the proportion of REM-related OSA and its associated polysomnographic features.Methods:One hundred forty-two patients were included in the study. REM-related OSA was defined based on previously established broad and strict criteria (REM apnea–hypopnea index [AHI]/non-REM [NREM] AHI ratio ≥2 and REM AHI >5 with NREM AHI <5, respectively), and its association with polysomnographic features was studied using appropriate statistical tools.Results:The proportion of REM-related OSA in the study was 56.3% and 25.3% as per broad and strict criterion, respectively. The REM-related OSA group had a mean younger age (47.4 ± 13.2 years) as compared to NREM-related OSA group (52.6 ± 15.8 years). Females (34 out of 45; 75.6%) were more likely to have REM-related OSA as compared to males (46 out of 107; 47.4%). Supine AHI, arousal index, oxygen desaturation index, length of the longest event, and the lowest oxygen saturation recorded during sleep had a significant association with REM-related OSA. 74% of patients with overall AHI <5 and 87% patients with overall AHI 5 to 15 satisfied the criteria for REM-related OSA as per broad criterion.Conclusion:REM-related OSA was quite prevalent in the study population (56.3%) and was more common in the mild and moderate severity subgroups of OSA.

411 REM predominance of OSA: Associated with supine position, but not with CPAP adherence

Abstract Introduction Obstructive sleep apnea (OSA) is a heterogeneous disease dependent on many factors including the sleep stage and the body position. OSA is often more severe during the rapid eye movement (REM) sleep stage, a phenomenon known as REM predominance. Prior studies suggested associations of higher REM predominance of OSA with younger age, higher obesity, and lower adherence to continuous positive airway pressure (CPAP) therapy, but these studies had small cohort sizes. Here we leverage home-based sleep tests (HST) that estimate REM sleep and measure body position to study REM predominance in a larger cohort of OSA patients. Methods We retrospectively reviewed patients who took HST at our clinic using devices based on peripheral arterial tonometry (WatchPAT, Itamar Medical). The HST results included estimated REM sleep periods and measured body positions. Auto-titrating CPAP therapy was prescribed for the majority of OSA patients diagnosed by the HST. Our inclusion criteria were: apnea-hypopnea index (AHI) above 5 /hour, estimated REM sleep time above 30 minutes, oxygen saturation below 90% (T90) for less than 10 minutes, and successful retrieval of CPAP usage data. CPAP adherence was defined as the percentage of nights with CPAP usage above four hours, and REM predominance as the ratio between REM AHI and non-REM AHI. Additionally, the percentage of estimated sleep time in supine position was calculated. Results Among 292 consecutive patients whose HST were reviewed, 113 patients met the inclusion criteria. The 25th-75th percentile ranges of age, body mass index (BMI), AHI, REM predominance, CPAP adherence and supine sleep percentage were 36–56 years, 28.1–38.4 kg/m2, 8.9–25.9 /hour, 1.27–2.89, 40%-97% and 28%-72%, respectively. REM predominance was not associated with CPAP adherence (P &amp;gt; 0.05), but was significantly associated with lower age, higher BMI, and higher supine sleep percentage (all P &amp;lt; 0.01). Conclusion We found that REM-predominant OSA is relatively more prevalent not only in young and obese patients, but in patients who sleep relatively more in the supine position. This association of REM predominance with body position is a novel finding to our knowledge. Contrary to prior studies, we did not find association of REM predominance with adherence to CPAP therapy. Higher Apnea-Hypopnea Index (AHI) and Oxygen Desaturation Index (ODI) Were Independently Associated with Increased Risks of Hypertension in Patients with T2DM: A Cross-Sectional Study.

Objective The current study aimed to explore the relationship between OSAS and hypertension and whether polysomnography (PSG) indices were independently associated with hypertension in patients with type 2 diabetes (T2DM). Methods This study recruited 316 T2DM patients. Multivariable logistic regression analyses were performed to determine the independent association of apnea-hypopnea index (AHI) and oxygen desaturation index (ODI) with hypertension with adjustment for potential confounders. Results Among 316 patients, 130 (41.1%) and 204 (64.6%) had hypertension and OSAS, respectively. T2DM patients with hypertension showed significantly increased levels of AHI ((median (interquartile range)): 17.2 (5.7–34.9) vs. 5.7 (2.1–17.3) events/hour, p < 0.001), nonrapid eye movement AHI (NREM-AHI) (17.6 (5.5–36.5) vs. 5.2 (2.2–16.6) events/hour, p < 0.001), ODI (48.4 (21.9–78.0) vs. 22.6 (10.8–48.1) events/hour, p < 0.001), and severities of OSAS and decreased levels of lowest SaO2 ((mean ± standard deviation): 74.0 ± 10.4 vs. 77.3 ± 9.8, p = 0.004). Multivariable logistic regression analysis showed that higher levels of AHI, NREM-AHI, and ODI were significantly associated with increased risks of hypertension, and the adjusted odds ratios (ORs) with 95% CI were 1.026 (1.008–1.044, p = 0.004), 1.026 (1.009–1.044, p = 0.003), and 1.005 (1.001–1.010, p = 0.040), respectively. Compared with non-OSAS, severe OSAS was significantly associated with the risk of hypertension with the adjusted OR (95% CI) of 3.626 (1.609–8.172, p = 0.002), but associations of rapid eye movement AHI (REM-AHI) and lowest SaO2 with hypertension were not statistically significant. Conclusion Increased AHI, NREM-AHI, ODI, and severities of OSAS were significantly associated with higher risks of hypertension in T2DM patients. Detection and treatment of OSAS are needed to prevent hypertension in T2DM patients.

Review of Pediatric Polysomnographic Studies over a 2-year Period in a Tertiary Care Hospital in Bengaluru, India

ABSTRACT Introduction Pediatric polysomnography is the gold standard in diagnosing sleep-related breathing disorders, the most common cause of this being obstructive sleep apnea (OSA). Instituting good sleep hygiene measures, early recognition of symptoms and signs of sleep-disordered breathing, and timely referral for polysomnography play a vital role in improving a child's quality of life. We report our experience with sleep disorders in children and the challenges faced during diagnosis. Aims To review indications for pediatric sleep studies done over 2 years and their outcomes in a tertiary care center in Bengaluru, India. Methods Retrospective descriptive analysis of 65 pediatric sleep studies done between 2018 and 2020 (2-year period) for various indications. Results The total numbers of studies included in the analysis were 65, of which complete studies were done in 58 children and portable studies in 7 children. The most common indication for performing a sleep study was suspected OSA, followed by referrals for children with spinal muscular atrophy and Duchenne muscular dystrophy, respectively. Average age of the patient was 93.56 months (range 3–216 months). The number of boys and girls included in the study was 45 and 20, respectively. The average duration of sleep was 375.35 minutes, of which rapid eye movement (REM) sleep contributed to 45.33 minutes (12.13%) (range, 0–90 minutes). The average sleep latency time was 21.36 minutes (range, 0.5–138 minutes). The total apnea-hypopnea index (AHI) was 12.64/hour with 26 (40%) children having mild OSA, defined as an AHI of less than 5/hour; 17 (26.1%) having moderate OSA, defined as an AHI of 5 to 10/hour; 22 (33.8%) having severe OSA defined as an AHI of 15 to 30/hour. The mean REM AHI was 29.67/hour. Thirty-four children were started on bilevel positive airway pressure (BiPAP) following the study and four children had their BiPAP setting titrated following the study with one child whose high flow oxygen was continued. Conclusion Of the 65 pediatric sleep studies included, 39 children (60%) underwent some form of intervention following the study, indicating the clear benefit of performing polysomnography and indicating a high prevalence of sleep disorders in children. Correlation between the clinical severity of laryngomalacia and endoscopic findings.

Objectives:To correlate the clinical severity of laryngomalacia (LM) with endoscopic findings, swallowing evaluations and polysomnography in a cohort of patients .Methods:We conducted a retrospective analysis between 2017-2018 on a cohort of patients diagnosed with upper airway obstruction (UAO), stridor, noisy breathing or laryngomalacia. This study took place at the Pediatric Pulmonology Department, Riley Children’s Hospital, Indianapolis, United States of America.Results:There were 157 patients with laryngomalacia included in the study. Patients with severe LM were significantly younger than those with mild LM (p=0.0214) and moderate LM (p=0.0220). Subjects with type I of LM were significantly older than type III (p=0.0051).When associations were tested between polysomnogram (PSG) variables and clinical severity, there were significant associations with age at PSG. The overall apnea-hypopnea index (AHI) in mild (p=0.0103) and moderate (p=0.0242) were significantly lower than the severe group. The rapid eye movement (REM) AHI was significantly lower in moderate cases than severe (p=0.0134). The end-tidal carbon dioxide (EtCO2) peak was significantly lower in mild cases than severe (p=0.0141).The total sleep time (TST) peripheral capillary oxygen saturation (SpO2) 90% occurs in both mild (p=0.0197) and moderate (p=0.0498)) were significantly lower than the severe group.Conclusions:The severity of the clinical manifestations of LM did not correlate with the different endoscopic types in our study. The presence of cyanosis was associated with type III LM. Rapid eye movement AHI and EtCO2 in polysomnogram were remained significantly associated with clinical severity.

Individuals were considered to have REM OSA if the REM AHI was greater than 5 events/hour, and the ratio of REM AHI / non-rapid eye movement (NREM) AHI was greater than 2. Demographic variables and the recommended CPAP pressure were analyzed using paired Student’s T-Tests. Results: Our study included a total of 16 individuals with REM OSA and equal number of AHI, gender, and BMI matching controls. Both groups had similar demographic and polysomnographic characteristics. Individuals with REM OSA required similar CPAP pressures as controls (7.5 cm H 2O vs 7.4 cm H 2O p=0.78). Conclusion: Individuals with REM might require similar CPAP pressures as their AHI, gender, and BMI matching controls.

Associations Between the Apnea-Hypopnea Index During REM and NREM Sleep and Cognitive Functioning in a Cohort of Middle-Aged Adults.

Prior research has linked obstructive sleep apnea (OSA) to varied cognitive deficits. Additionally, OSA in rapid eye movement (REM) versus non-rapid eye movement (NREM) sleep has been shown to be a stronger predictor of outcomes such as hypertension. The present study aimed to investigate whether OSA-as characterized by the apnea-hypopnea index (AHI)-during REM and NREM sleep is associated with performance on a range of cognitive tasks. We also investigated whether the presence/absence of the apolipoprotein E4 allele (APOE4) modifies the associations between AHI during REM and NREM sleep and cognitive performance. A cross-sectional sample of 1,250 observations from 755 community-dwelling adults (mean [standard deviation] age, 62.3 [8.2] years) participating in the Wisconsin Sleep Cohort study was carried out by means of overnight polysomnography, paper-and-pencil cognitive tasks, and genetic data. Linear mixed effects models with repeated measures estimated associations of AHI during REM and NREM sleep with cognitive outcomes, stratified by APOE4 status (carrier versus noncarrier). No significant associations were found between REM AHI and cognitive outcomes for either APOE4 carriers and non-carriers. Higher NREM AHI was associated with worse memory retention among APOE4 carriers; among noncarriers of APOE4, higher NREM AHI was associated with worse performance on a test of psychomotor speed, but better performance on two tests of executive function. Sleep state-specific (REM, NREM) OSA may be differentially associated with varying dimensions of cognitive deficits in middle-aged to older adults, and such associations are likely to be modified by genetic factors, include APOE polymorphisms.

Protective effects of REM sleep without atonia against obstructive sleep apnea in patients with idiopathic REM sleep behavior disorder

BackgroundThis study investigated the protective effect of rapid eye movement (REM) sleep without atonia against obstructive sleep apnea (OSA) in patients with idiopathic REM sleep behavior disorder (RBD). MethodsIn this case–control study, patients with idiopathic RBD and OSA (RBD-OSA) were consecutively enrolled and OSA controls without RBD were matched for age, sex, and apnea–hypopnea index (AHI). Clinical and polysomnographic characteristics were compared between RBD-OSA patients and OSA controls. Additionally, differences in AHIs depending on sleep state and posture were analyzed. ResultsIn total, 109 OSA patients (81 males and 28 females) with idiopathic RBD were included in the study. In OSA controls without RBD, AHI and respiratory distress index (RDI) were significantly higher during REM sleep than during non-rapid eye movement (NREM) sleep (p < 0.01). In RBD-OSA patients, however, AHI and RDI were slightly lower during REM sleep than during NREM sleep (p < 0.05). During REM sleep, AHI and RDI were significantly lower in RBD-OSA patients than in OSA controls (p < 0.001). Differences in apnea severity between RBD-OSA patients and OSA controls increased in supine REM sleep but disappeared or lessened in non-supine REM sleep. The prevalence of REM-related OSA was lower in RBD-OSA patients (9.2%) than in OSA controls (33.0%). ConclusionsREM sleep without atonia has protective effects against OSA in patients with idiopathic RBD. These protective effects are much more potent in supine sleep than in non-supine sleep.

Obstructive sleep apnea during rapid eye movement sleep is associated with early signs of atherosclerosis in women.

Although obstructive sleep apnea (OSA) is associated with overall cardiovascular disease and mortality, the association with atherosclerotic cardiovascular disease is less clear, especially in women. Recently, it has been suggested that OSA during rapid eye movement (REM) sleep, associated with long apneas and deep desaturations, could have severe cardiometabolic consequences. The aim of this study was to investigate whether OSA during REM sleep is associated with early signs of atherosclerosis in a population-based sample of women. In the community-based "Sleep and Health in Women" (SHE) cohort study, 400 women underwent polysomnography, anthropometric measurements, blood sampling, blood pressure measurement, and answered questionnaires. Ten years later, 201 of the original participants, free of known atherosclerotic disease at baseline and without continuous positive airway pressure treatment for OSA, underwent a high-frequency ultrasound of the common carotid artery to assess the individual thickness of the layers of the artery wall. Severe OSA during REM sleep (REM apnea-hypopnea index [AHI] ≥ 30) was associated with a thicker intima. This association was still significant after adjustment for age, body mass index, alcohol, and smoking, as well as for further adjustment for systolic blood pressure, low-density lipoprotein, C-reactive protein, and diabetes (β-coefficient, 0.008; p-value, 0.022). The association between a REM AHI of ≥30 and intima thickness was also seen in women with no or mild OSA and normal non-REM AHI. In this study of a community-based sample of women, severe OSA during REM sleep was independently associated with early signs of atherosclerosis.

Rapid Eye Movement-Related Obstructive Sleep Apnea: A Study on the Pathogenesis through Clinical and Polysomnographic Features

Purpose: The aims of this study were to evaluate the differences of clinical and polysomnographic features between rapid eye movement (REM)-related obstructive sleep apnea (OSA) and not-REM-related OSA, and to suggest the pathogenesis according to the REM dependency of OSA. Methods: One hundred ninety consecutive patients diagnosed with OSA were evaluated clinical features and performed full night polysomnography. The patients were divided into REMrelated (REM apnea-hypopnea index [AHI] at least two times higher than their non-REM AHI) and not-REM-related (a REM AHI less than two times higher than their non-REM AHI) OSA groups and evaluated the differences in age, body mass index (BMI), neck circumference, Epworth Sleepiness Scale score, and parameters of polysomnography. Results: REM-related patients were younger and showed higher sleep efficacy, low percentage of light sleep stage (stage 1 sleep), and low rate of positional OSA. Age was significantly associated with REM dependency of OSA and REM AHI were significant correlated with BMI, neck circumference, percentage of sleep in supine position, and percentage time of snoring. Conclusions: Our results showed that REM-related OSA patients showed less severe polysomnographic parameters than not-REM-related patients. However, significant risk factors were differed depending on the REM dependency and OSA severity, and the clinical features correlated with REM AHI and non-REM AHI were also showed differently. We suggest that the occurrence of OSA according to the REM dependency can be based on different mechanisms.

Treatment Outcomes of Mandibular Advancement Devices between Rapid-Eye-Movement (REM)-Related and Not-REM-Related OSA Patients

Purpose: Mandibular advancement devices (MAD) are used effectively and widely for the treatment of obstructive sleep apnea (OSA) and rapid-eye-movement (REM) dependency of the patients can affect the treatment outcome of OSA. The aim of this study was to compare treatment outcomes of MAD between REM-related and not-REM-related OSA patients. Methods: Fifty-six consecutive patients with OSA who received MAD therapy were evaluated using full night polysomnography before and after insertion of the MADs. The patients were divided into REM-related (REM apnea-hypopnea index [AHI] at least two times higher than their non-REM AHI) and not-REM-related (REM AHI less than two times higher than their non-REM AHI) OSA groups. Results: MAD is used for the treatment of OSA effectively. In respect of AHI, MAD therapy were effective both in REM-related OSA and not-REM-related OSA, but MAD therapy was more effective in not-REM-related OSA than REM-related OSA in overall sleep and non-REM sleep. SpO2 saturations were improved after MAD therapy, but were not different between two groups. Epworth sleepiness scale scores were not improved after MAD therapy. Percentage of REM sleep was increased after MAD therapy but was not different between two groups. Conclusions: MAD therapy was more effective in not-REM-related OSA than REM-related OSA and REM dependency can be a predictive factor of treatment outcome of oral appliance for OSA patients.

Effect of Sleep State and Position on Obstructive Respiratory Events Distribution in Adolescent Children.

This study aimed to examine the effect of sleep state (rapid eye movement [REM] versus non-rapid eye movement [NREM]) and position (supine versus non-supine position) on obstructive respiratory events distribution in adolescent population (ages 12 to 18 y). This was a retrospective study that included 150 subjects between the ages of 12 to 18 y with an apnea-hypopnea index (AHI) > 1/h. Subjects using REM sleep-suppressant medications and subjects with history of genetic anomalies or craniofacial syndromes were excluded. The median age was 14 y with interquartile range (IQR) of 13 to 16 y, 56% of patients were males and the median body mass index (BMI) z-score was 2.35 (IQR: 1.71-2.59) with 77.3% of patients fulfilling obesity criteria. Respiratory obstructive events were more common in REM sleep. The median REM obstructive AHI (OAHI) was 8.9 events per hour (IQR: 2.74-22.8), whereas the median NREM OAHI was 3.2 events per hour (IQR: 1.44-8.29; p < 0.001). African American adolescents had more REM obstructive events with median REM OAHI of 13.2 events per hour (IQR: 4.88-30.6), which was significantly higher than median REM OAHI of 4.94 (IQR: 2.05-11.36; p = 0.004) in white adolescents. Obstructive events were more common in supine position with higher median supine OAHI of 6.55 (IQR: 4-17.73) when compared to median non-supine OAHI of 2.94 (IQR: 1-6.54; p < 0.001). This study shows that sleep related obstructive respiratory events in the adolescents (12 to 18 y of age) occur predominantly in REM sleep and in supine position.

Abstract 12314: The Long-Term Effect of Coronary Atherosclerosis and Apnea Hypopnea Index During Rapid Eye Movement on Posttraumatic Stress Disorder

Introduction: Posttraumatic-stress-disorder(PTSD) is associated with subclinical-atherosclerosis as well as rapid-eye-movement(REM) apnea-hypopnea-index(AHI). This study investigated the relation of coronary-atherosclerosis measured by coronary-artery-calcium(CAC), REM-AHI with cardiovascular(CV)-mortality in PTSD. Hypothesis: There is a direct relation between CAC and REM-AHI with increased CV-mortality in PTSD. Methods: This study is consist of 634 veterans without-known-CAD(age:58±11years,86%men) underwent CAC-scanning and overnight poly-somnography for clinical-indications. Psychological-health-status(PTSD vs. non-PTSD)was measured and they were followed for median of 36-months. Multivariable-Cox-regression-analyses were employed to assess the relation of REM-AHI and CAC, with CV-mortality in PTSD. Results: At follow-up (figure 1), the death-rate was higher in the PTSD(17%) than non-PTSD(10.4%)(p=0.001). Regression-analyses revealed a significant-linkage between PTSD and CAC as well as PTSD and REM-AHI with CV-mortality. The relative-risk of CV-mortality was 5.72(95%CI:2.46-13.34,p=0.0001) in PTSD and CAC&gt;0 as compared to non-PTSD with CAC=0. Similarly, the relative risk of CV-mortality was 3.22 (95%CI:1.17-8.85, p=0.02) in PTSD and REM AHI≥30 as compared to non-PTSD with REM AHI0 which was more prominent in PTSD(p=0.001). A significant linkage of CAC and REM-AHI with increased CV-mortality in PTSD was noted in which the relative-risk of CV-mortality in PTSD was 6.44 (95%CI:3.64-11.37,p=0.001) in combined CAC&gt;0 and REM AHI≥30 as compared to non-PTSD. Conclusions: 1)PTSD is associated with the coronary-atherosclerosis and increased CV-mortality, 2)PTSD is associated with REM obstructive-sleep-apnea and predicts CV-mortality and 3) There is an independent significant linkage of PTSD with REM obstructive-sleep-apnea as well as coronary-atherosclerosis, and increased risk of CV-mortality.
