Radiographs and aluminium: a pitfall for the unwary

Ingestion of radio-opaque foreign bodies is common. We highlight the need for a careful radiological examination and endoscopy if symptoms of obstruction persist. A 70 year old man presented to the local hospital while on holiday, having accidentally swallowed part of the metallic tab of a soft drinks can. Sonographic detection of foreign bodies of the extremities

A prospective study to assess the accuracy of ultrasound in the detection and localization of foreign bodies of the extremities was undertaken. Forty-five patients with clinical suspicion of foreign bodies were included in the study. Among 20 patients with radio-opaque foreign bodies, ultrasonic detection of foreign bodies was achieved in 19 patients. In 25 patients, with no radiographic evidence of foreign body, ultrasonic detection of foreign bodies was made in seven patients and among these foreign bodies were confirmed and removed at surgery in five patients. Accurate pre-operative ultrasonic localization of foreign bodies was carried out in three patients.

HIDE AND SEEK: PRE‐OPERATIVE ULTRASONIC LOCALIZATION OF NON RADIO‐OPAQUE FOREIGN BODIES

Ultrasound has long been used as a diagnostic and therapeutic tool in surgery. We have extended its use to hand surgery, where is has several applications. Non radio-opaque foreign body extraction is invariably a frustrating exercise of 'hide and seek'. Accurate pre-operative localization with ultrasound illustrating size, shape, depth, soft tissue and bony relationships can ensure rapid and complete removal. Several cases are presented to demonstrate the use of ultrasound for the detection of non radio-opaque foreign bodies. The technique used will be described. We feel pre-operative localization by ultrasound is a useful technique to assist with the removal of non radio-opaque foreign bodies.

Esophageal foreign bodies.

In a review of 40 children with esophageal foreign bodies, between 1983 and 1986, 25(62.5%) were males and 15(37.5%) females, of them 34 (85%) were below five years of age. Inorganic material was the causative agent in 39(97.5%). Coins being the commonest present in 34 (85%) patients. A history of foreign body ingestion was obtained in all the patients. Radioopaque foreign bodies were seen in 39 (97.5%). All patients had their foreign body removed 38(95%) endoscopically and 2(5%) by esophagotomy. There was no mortality in this study.

Foreign bodies in the pediatric airway.

In a review of 34 children with foreign bodies in the airway, between 1978 and 1982,24 were males and 10 females. Eighteen of them were between one and two years of age. Vegetable matter was the causative agent in 76–47 percent with 47.06 percent due to a portion of groundnut A history of foreign body aspiration was obtained in 24 patients and obstructive emphysema demonstrated on plain X-ray of the chest in 21 patients. Radioopaque foreign bodies were seen in only 6 patients. Foreign bodies were seen with almost equal frequency on both sides, 13 in the right bronchus and 14 in the left bronchus. All patients had their foreign bodies removed-one through the endotracheal tube, 29 bronchoscopically. 2 by tracheotomy and 2 by bronchotomy. There was no mortality. The details of the diverse clinical presentations, findings and method of treatment are also discussed.

Patients with foreign bodies inside soft tissues are common in a surgeon's daily practice,. Radio-opaque foreign bodies can easily be located with radiography but radio-lucent foreign bodies cannot be located with X-ray, where Ultrasonography especially, high resolution ultrasonography can be used to locate it. Ultrasonography, being easily available, cost-effective and radiation-hazard free, can be done repeatedly for foreign bodies which move inside tissues. VALUE OF ROUTINE ROENTGENOGRAPHIC STUDIES OF WAR INJURIES OF THE HEAD AND NECK

The case to be presented gives pictorial evidence of the value of routine roentgenographic studies in dealing with all war injuries of the head and neck in which the presence of a foreign body is remotely suspected. Routine roentgenographic examination of all injuries of the head and neck has been adopted as a matter of policy in the otolaryngologic, ophthalmologic and maxillofacial departments of the evacuation hospital at which I am stationed since its first admission of war casualties in February 1942. The use of this additional diagnostic safeguard has amply rewarded the medical officer for the care expended by rendering valuable information concerning the retention of radio-opaque foreign bodies in war injuries of the head and neck in patients in whom the presence of these foreign bodies would otherwise have passed unnoticed. In several instances it has saved the inevitable clinical complications and professional embarrassment that accompany the omission
