The aim of this study is to compare the incidence rates of BC after radiation-therapy and surgery for localized PCa at Kyoto University Hospital. METHODS A total of 1241 prostate cancer patients treated at our Institution during 1997-2009 periods were evaluated. The RT cohort consisted of 835 patients (30.7% of patients treated by conventional RT or 3D-CRT, while the rest of them were by IMRT) and the surgery cohort of 406 patients. The incidence of BC, time to disease onset, and clinic-pathological characters of BC were studied retrospectively by reviewing medical records. RESULTS Median age, follow-up period, and initial PSA were 72 years (49-88), 56 months (1-168), and 15.3 ng/ml (2.2-150), in the RT cohort and 67 years (46-77), 72 months (2-154), and 8.75 ng/ml, in the RP cohort, respectively. Among 835 RT patients, 13 cases (1.57%) developed BC, while only one case (0.25%) of the RP cohort was found, which was significantly different (P=0.015). Compared to BC incidence rates among sixty years or older Japanese population (0.57%) according to data from National Cancer Center, Japan, the incidence rate of BC after RT against PCa at our Institution was about 3 times higher. The median time to disease onset after RT was 34 months (9-104), and all of 13 cases after RT suffered from macrohematuria. The pathology of BC in RT cohort demonstrated cis in 2 patients, pTa in 8 patients, pT1 in 2 patients and pT3b in one patient The localization of BC within the urinary bladder included bladder neck (3 cases), right side wall(4 cases), anterior wall(2 cases), posterior wall(2 cases), and unknown(2 cases). There was no specific tendency in the localization of BC occurrences. CONCLUSIONS The incidence of BC was significantly high in RT patients compared to patients undergoing radical prostatectomy.
