Laser surgery for soft tissue purulent diseases.

The original techniques of soft-tissue purulent lesion treatment by means of a continuous-wave CO2 laser are elaborated. The following laser treatment advantages--minimal tissue damage, prophylaxis of local dissemination of infection, haemostatic and sterilizing effects--have been proved experimentally. Laser utilization in clinical trial (360 cases) made it possible to obtain significantly improved outcomes and marked shortening of treatment.

Gunnar Holmgren (1875-1954).

GUNNAR HOLMGREN was born in Uppsala on Sept 27, 1875. He grew up in one of that period's most liberal homes, and was the son of a professor of physiology, Frithjof Holmgren— well known from the time of Scandinavianism—and his equally distinguished wife, Ann Margret Holmgren, one of Sweden's pioneer feminists. During his early student years, Gunnar Holmgren came into close contact with social and political problems which subsequently were to be fundamental for many of his initiatives. After completing his medical studies[ill] partly in Uppsala and partly at the Caroline Institute in Stockholm, he received his doctorate of medicine in 1909 with a dissertation on the purulent diseases of the inner ear. This was an outstanding work on one of the most dreaded and fatal complications of middle ear inflammation at that time and, in fact, until the era of chemotherapeutics. For a successful surgical intervention on the lungs, it is necessary to take into account the state of not only the affected organ, but also the whole organism. Of particular importance in chronic purulent diseases and neoplasms of the lungs in this regard is the functional diagnostics of the respiratory and cardiovascular systems, which are functionally a single whole.

INTRA-AORTIC INJECTION THERAPY FOR HEPATIC AND CHOLECYSTIC DISEASES.

Hypertonic glucose solution injected into the lower portion of the thoracic aorta brought about a marked elevation of liver function. The result appeared to indicate that penicillin or a sulfaminide drug would prove more infallibly effective when injected in glucose solution into the aorta than when otherwise administered and that intra-aortic injection of a mixture of hypertonic glucose solution and penicillin or a sulfaminide drug would be a very reasonable therapy for the liver, gall-bladder, etc, with purulent diseases. On this assumption a mixture of hypertonic glucose solution and penicillin or a sulfaminide drug was injected into the lower portion or the thoracic aorta in purulent diseases of the liver and gall-bladder. The result was excellent and fever inreducible by any other drug therapy was often reduced instantaneously by this therapy. Even in cases in which this therapy fails to bring about a complete cure, a condition safe for surgical treatment will be induced by eliminating acute inflammation by this therapy.

Regarding the article by Dr. Aydarov

Dr. Aydarov's article correctly raises the question of the abnormal formulation of medical education in that part of it, which concerns outpatient surgery and purulent surgery in particular. The main forms of purulent surgical diseases should be studied in the third year in general surgery classes, the department of which should be provided with clinical beds and an outpatient clinic. Until recently, there was no such clinic in Kazan, just as there was no department or an outpatient clinic. Thus, teaching the basics of general surgery and, in particular, teaching students of purulent surgery stood on the wrong path of predominantly theoretical education. Until recently, there was no fixed polyclinic, where it would be possible to widely and correctly organize the teaching of 5th year students in outpatient and, in particular, purulent surgery.
