The effect of histamine on gastric secretion

According to G. Modena and L. Fernandez'y (Arch, fr Verdauungskr., Bd. 42, No. 1-2) in histamine we not only have an excellent stimulant, with the help of which we obtain pure juice for studying the secretory function of the stomach, but it is also an excellent means for recognizing achilia, namely, the absence of its action: indicates a serious organic lesion, for example, stomach cancer and gastritis in malignant anemia.

Hydrochloric Acid and Total Chlorine Content of Pure Gastric Juice Produced After Histamine Injection.

The acid curve obtained from a fractional analysis of an ordinary test meal is at present in a stage of reinterpretation. This new interpretation is based on Boldyreff's theory about the automatic regulation of the acidity in the stomach.1 Clinically this theory is being supported chiefly by work from Guy's Hospital (Bolton and Goodhart;2 Baird, Campbell and Hern;3 Ryle4). The chief point in the interpretation is that the total chlorides of the gastric content represent a true expression of the acid production in the stomach. The difference between the curve expressing the free hydrochloric acid and the total chloride curve, therefore, according to this theory represents the extent of neutralization of acid either by combining with protein or by neutralization with regurgitated alkaline duodenal content, particularly pancreatic juice. DIRECT EXAMINATION OF THE GASTRIC JUICE

For a study of gastric conditions we are able to avail ourselves of test meals, such as the Boas-Ewald, Riegel-Leube or Boas, which give a certain amount of evidence regarding both the motor and the secretary functions of the stomach. But if we are concerned with gastric motility alone, this can be examined more easily and accurately by roentgenographic methods, while in practice it has been demonstrated that the test meal method affords only partial information concerning irregularities in gastric secretion. In making use of the test meal we can only obtain a mixture of food and stomach secretions, never the pure gastric juice alone. And because of this dilution and chemical admixture with the ingested food, it is impossible to make any accurate estimate as to the precise amount of secretion given off, this difficulty being still further enhanced by the fact of its fractional withdrawal. The speaker, having previously said about the relevant literature, reported several cases of somotherapy by Hricourt'y and Ch. Bichet for pulmonary tuberculosis. In one case from private practice in case of tuberculosis of the 1st degree with congenital dbilitas, meat juice (muscle plasma) was used in amounts from 200.0 to 500.0 pure juice (obtained without insisting meat with water) and from 100.0 to 500.0 meat ...
