Social class in the mental health center

To test whether the preference for middle and upper class clients, found in psychotherapeutic clinics during the 1950s and early 1960s, exists in mental health centers, the relationships between social class and various aspects of psychiatric treatment are investigated in forty mental health centers which vary in several organizational dimensions. In general, clients are given treatment with little or no regard to social class. The variables that account for the different patterns of treatment in the two types of psychiatric treatment organizations are (1) the composition of the staffs; (2) the types and amount of individual psychotherapy given; and (3) the primary organizational goal.

The phenomenon of splitting in a small psychiatric organization: A case report

It is evident from previous reports that splitting of the staff group is a powerful and continual occurrence in the daily life of inpatient psychiatry services. It seems critical that a sophisticated psychiatry staff be able to tolerate and master the ability to work with these phenomena. If the staff may lead a work culture in which split off parts of the group and of the individual are integrated, then a fundamental need of psychiatric patients is being met. We present a method by which a staff may work at developing this competence in a brief, intensive conference.

Application group: broadening the patient experience in a psychiatric organization.

The Tavistock conference model of Group Relations Training, described in some depth by Rioch (1970) and O'Conner (1971), provides participants with a focus on the problems of leadership and authority as they develop and emerge in a variety of relatively unstructured group contexts within a time-limited laboratory setting. The major conference experience involves participation in ‘here and now’ groups with the use of staff in a consultative role, i.e. the staff function being to focus exclusively on the covert or unconscious processes in the group. So the usual consultant role involves no comments to individuals but a rigorous focus on the dynamics of the group as a whole; a role which has been described in more clinical settings by Bion (1968), Coffey (1966) and Ezriel (1950). ‘One of the major aims of the conferences is to contribute to people's ability to form serious work groups committed to the performance of clearly defined tasks …. A second major aim … is the development of more responsible leadership and fellowship in group life’ (Rioch, 1970, p. 347).

Is there a power elite in Canadian psychiatry?

This paper poses a number of questions regarding the recent history of psychiatric organization in Canada. Using public documents regarding organizational structure, it was suggested that during the decade 1956–1966, there was considerable evidence of a national shaping of policy regarding psychiatric services by a small but influential group of reformers. For other questions such as the organization and dispensing of research funds, access to more privileged sources would be required. In its organization, Canadian psychiatry is of no greater interest than many other professional organizations. However, because of its critical role at a time when health services generally were in transition, the psychiatric elite played an extremely important part in influencing the restructuring of psychiatric services around private practice, around general hospital and medical integration and the ideology that mental illness is identical with physical illness. The blurring of differences between psychiatry and mental health were discussed briefly and the apparent lack of conflict between bureaucratic and professional roles.

Care for the mentally ill in the U. S. has traditionally been hospital-based; there are four times as many psychiatric beds per 1,000 population as in the U.S.S.R., where the district psychoneurological dispensary, emphasizing outpatient services and aftercare, is the focal point of treatment. These dispensaries, which the author describes, have features in common with the community mental health centers to be built in the U. S. The organizational structure for psychiatric services in this country is more complex than in the U.S.S.R. for various historical reasons; the success of the community mental health movement here will require a creative and flexible approach which can break down antiquated boundaries of responsibility.
