Nonhypovascular pancreatic ductal adenocarcinomas: CEUS imaging findings and differentiation from other types of solid pancreatic lesions.

To observe and assess the diagnostic value of contrast-enhanced ultrasound (CEUS) in patients with iso-/hypervascular solid pancreatic lesions. 70 pancreatic lesions (all confirmed by surgery or biopsy pathology) that manifested iso-/hyperenhancement on CEUS were retrospectively studied from January 2018 to January 2022, including 24 pancreatic ductal adenocarcinomas (PDAC), 15 mass-forming pancreatitis (MFP), 24 pancreatic neuroendocrine tumors (PNET) (14 hyper-PNETs, 10 iso-PNETs), and 7 solid pseudopapillary tumors of pancreas (SPTP). 65 pancreatic ductal adenocarcinomas (PDAC) that manifested hypoenhancement on CEUS were retrospectively studied from January 2020 to January 2022. CEUS patterns and the clinical and pathologic features were analyzed, and the diagnostic ability of CEUS for iso/hyperenhanced solid pancreatic lesions was assessed. Centripetal enhancement, heterogeneous enhancement, early washout, and hypoenhancement in the late phase mostly appeared in iso-/hyper-PDACs (p < 0.05). Heterogeneous enhancement in small lesions (< 3 cm) as the diagnostic criterion for iso-/hyper-PDACs had an accuracy of 74.3% and a specificity of 91.3%. Iso-PNETs more commonly had larger tumor sizes and more often showed heterogeneous enhancement than hyper-PNETs (p = 0.007, p = 0.035, respectively). The characteristics of the combination of isoenhancement, homogeneous enhancement, and synchronous wash-in/out for MFP had a high accuracy of 90%. Capsular enhancement with heterogeneous enhancement inside for SPTP had an accuracy of 97.1%. CEUS enhancement patterns have potentially great value in the differentiation of iso-/hyperenhanced pancreatic lesions.

Role of fine needle aspiration cytology (FNAC) in diagnosing Pancreatic lesions in a tertiary care institution

OBJECTIVE- FNAC is an accurate and rapid technique for diagnosing pancreatic masses. Pancreatic carcinoma represents the seventh leading cause of cancer death in the world, responsible for more than 300,000 deaths per year.[1] Worldwide, both the incidence and death rates of pancreatic cancer are increasing. The aim of this study was early diagnosis of these lesions and to evaluate its burden and study the global, regional, and national patterns. These would further aid in policy making , better resource allocation for controlling pancreatic cancer risk factors and formulating more effective treatments.[2]&#x0D; METHODS- This was a retrospective observational study performed at a tertiary care hospital over a period of one and a half year. A total of 86 patients with pancreatic lesions were subjected to image guided FNA . 35 of these patients underwent diagnostic histopathology in addition to FNA.&#x0D; RESULTS- The 86 cases analyzed were in the age group of 61-70 years with mean age distribution of 57.34 years. Male predominance was seen with M:F ratio of 2.18:1. Head was the most common area to be aspirated followed by tail and least number of lesions were in body of pancreas. Maximum cases were found to be malignant (55) in etiology. Only 2 were non diagnostic because one of them had insufficient cellularity and other had necrosis mainly on histopathology. Adenocarcinoma was the most common malignancy found followed by one case each of lymphoma and solid pseudo papillary neoplasm of pancreas. Histo pathological correlation was obtained in 91.42% cases proving that FNA is a very useful tool in early diagnosing pancreatic lesions.

Solid-pseudopapillary neoplasia of the pancreas (analysis of three cases)

The objective was to improve the early diagnosis efficiency of large pseudopapillary tumor of pancreas through the demonstration and analysis of three clinical cases.Methods and materials. In this work, the observation period was two years, in which three cases were diagnosed and treated a large number of pseudopapillary tumors.Results. With sufficient information on the characteristics of this disease, over the past 5 years we have had an opportunity to determine a diagnosis in two patients before the tissue examination.Conclusion. The age of patients, clinical manifestations, tumor size and the age of tumor patients are not reliable criteria for predicting the possibility of malignant tumors. Performing the operation in compliance with the principles of radicalism provide the best opportunity for recovery and reduces the early incidence of postoperative complications.

Solid Pseudopapillary Neoplasm of the Pancreas - A rare entity with emphasis on the differential diagnosis.

Solid Pseudopapillary Neoplasm of Pancreas (SPNP) is a rare, low-grade malignant solid-cysticneoplasm with papillary architecture. It accounts for 2% to 3% of pancreatic neoplasms and 0.9% to2.7% of exocrine pancreatic neoplasms. It occurs almost exclusively in young women and has anexcellent postsurgical curative rate. Metastasis is rare although it may be locally aggressive. Thesolid pseudopapillary neoplasm of the pancreas pose a diagnostic challenge both clinically andradiologically as it has a nonspecific clinical presentation with vague radiologic features.Histopathological evaluation and immunohistochemistry remains the gold standard in reaching adefinitive diagnosis. Due to its low incidence, the clinical and pathologic features of SPNP have notbeen extensively studied. We report a case of a 32-year-old lady with solid pseudopapillaryneoplasm of the pancreas that was suspected on abdominal CECT as a well-defined mass in theampullary-periampullary region abutting head of the pancreas and confirmed on histopathologicalevaluation with immunohistochemistry.

Comparison of abdominal ultrasonographic findings with endoscopic ultrasonographic findings of solid pseudopapillary tumor of pancreas

Purpose To compare abdominal ultrasonographic (AU) findings with endoscopic ultrasonographic (EUS) findings of solid pseudopapillary tumor (SPT) of pancreas. Methods Thirteen patients (M:F = 3:10; mean age, 36 years) with surgically proven SPT were included, who underwent both preoperative AU and EUS. Ultrasonographic findings of 2 modalities were compared according tointernal echo texture, calcification, demarcation, internal septum, and main pancreatic duct dilatation. Results Eight cases showed hypoechogenicity on both AU and EUS. Remaining 5 cases showed different echogenicity on both modalities; hypoechogenicity (n = 3) and isoechogenicity (n = 2) on AU, hyperechogenicity (n = 2), unremarkable internal echogenicity due to dense rim calcification (n = 2), isoechogenicity (n = 1) on EUS. In two cases with dense rim calcification, evaluation of the internal content was more difficult on EUS, compared to AU. Additionally, central punctate calcifications of 2 cases were well seen on both AU and EUS, but 1 case of peripheral calcification was only seen on EUS. All cases showed good demarcation without main pancreatic ductal dilatation or internal septum. Conclusions EUS was superior to AU in the evaluation of internal echo texture (solid or cystic) of pancreatic SPT. However, AU was superior to evaluate the larger size tumor and tumors with dense rim calcification.

Distal Pancreas and Spleen-preserving Central Pancreatectomy in a Locally Aggressive Solid Pseudopapillary Neoplasm of Pancreas: A Novel Extended Warshaw Technique

Introduction: Central pancreatectomy (CP) is a well-described procedure done for neck and proximal body tumors of the pancreas. It can be done for benign lesions where an adequate length of the normal distal pancreas will be left leading to organ preservation. The currently described benefit of the procedure is decreased long-term morbidity due to retention of both the spleen and the preservation of functioning pancreas. This is usually dependent on the preservation of distal pancreatic vascularity by splenic artery preservation. Many studies have described splenic preservation by Warshaw technique by safeguarding the short gastric (SGA) and left gastroepiploic (LGEA) vessels in case of distal pancreatectomy. However, distal pancreatic preservation during CP with splenic vessels ligation is not given a significant mention in the current literature in relation to Warshaw technique Method: Here, we present a 19-year-old girl diagnosed with an exophytic solid pseudopapillary tumor of the pancreatic body that was selected for central pancreatectomy. In view of splenic vessels involvement, she underwent ligation of the splenic vessels and splenic preservation was based on the LGEA and SGA. Distal pancreas was anastomosed with a roux en loop of jejunum and intra-operatively, we were able to demonstrate the back flow in the splenic vessels. Result: Postoperative computed tomography showed adequate enhancement of the spleen along with retrograde blood flow into the distal splenic artery with enhancement of the distal pancreas. Her postoperative period went uneventful. Conclusion: Thus CP with extended Warshaw technique is a safe and feasible procedure where indicated.

The imaging features of pancreas solid pseudopapillary tumor in children

Objective To analyze the imaging features of solid pseudopapillary neoplasm of pancreas (SPN) in children, and to improve the awareness of the disease. Methods From January 2007 to December 2016, 12 patients with SPN proven pathologically were enrolled in the study, of whom 12 cases underwent CT scanning and 3 cases underwent MRI scanning. The imaging data of SPN were analyzed retrospectively. The tumor parameters included the location, size, shape, margin, capsule, form, inhancement degree, and presence of calcification, biliary obstruction, surrounding, ascites, lymph node metastasis, as well as distant metastasis, which were comparative analyzed with that of pathology. Results CT showed that 3 cases were located in the head of pancreas, 7 cases were located in the body/tail of pancreas, and 2 cases were ectopic. Twelve cases were circular tumors, the diameter of which ranged from 28—76 mm (median diameter 48 mm) . Capsules were showed in 10 cases, calcification was seen in 4 cases and hemorrhage was seen in 1 case. Three cases were solid, 8 cases were solid and cystic mixed, and 1 case was cystic. The tumors were heterogeneous, and the solid portion of SPN was moderately to obviously enhanced gradually whereas the cystic part remained unenhanced. Nine cases showed that the tumors growed to the outside of pancreas, in which 4 cases covered by the normal pancreas tissue, 1 case located inside of the pancreas. One case caused biliary obstruction and the collateral circulation of splenic arteriovenous was established in 2 cases due to tumor compressed. Ascites was seen in 2 cases. MRI showed that the cystic components of tumor in 3 cases showed low signal in T1WI and high signal in T2WI, with no enhancement. The solid components of the tumor showed equal signal in T1WI and slightly higher signal in T2WI, with obvious enhancement. Capsules were showed in 3 cases with low signal in T1WI and T2WI, which were obviously enhanced in 2 cases and without enhancement in 1 case; hemorrhage was showed high signal in T1WI in 1 case. No lymph node metastasis and distance metastasis were observed in 12 cases. Conclusions The characteristic imaging findings of SPN in children are boundary clear, capsules, calcification, circular pancreas tumors, which grow to the outside of pancreas, with varied degrees of hemorrhage and necrosis. Analysis of diagnosis and treatment of solid pseudopapillary neoplasm of pancreas in 12 children

Objective To analyze the experience in diagnosis and surgical treatment for solid pseudopapillary neoplasm(SPN) of pancreas in children. Methods A retrospective study was performed in 12 pediatric patients with SPN who had been admitted to Affiliated Cancer Hospital of Zhengzhou University during January 2004 to December 2016, and their general data, demographic data, types of operations, postoperative complications and follow-up were analyzed. Results Among the 12 patients, 3 cases were male and 8 cases were female, with average age 14.3 years old(11-17 years old). The main clinical manifestations included abdominal pain(4/12 cases, 33.3%), abdominal mass(2/12 cases, 16.7%) and trauma(2/12 cases, 16.7%). In those 12 patients, 33.3%(4/12 cases) SPN was located at the head of the pancreas, and 66.7%(8/12 cases) at the body and tail of it.The tumors were usually large, the largest diameter ranged from 4.0 to 15.3 cm(average largest diameter, 8.2 cm). The color ultrasonography indicated heterogeneous echogenic mass and clear boundary.CT scanning indicated that the tumor was a low-density cystic mass with a clear boundary, with enhanced tumor real component and irregular reinforcement.No calcification was found in the patients.Dynamic enhanced magnetic resonance imaging scan revealed gradual strengthening solid components in tumor.All the patients received surgical resection, with distal pancreatectomy in 4 patients, pancreaticoduodenectomy in 4 patients, spleen-preserving distal pancreatectomy in 2 patients, Enucleation in 1 patient, and distal pancreatectomy and self-splenic slices transplantation in 1 patient.Lymphadenectomy was performed in 4 patients, and all the 21 removed lymph nodes were all negative.Pathological diagnosis confirmed the SPN in all the patients, among them 3 cases were malignant SPN, and one of them with tumor rupture and hemorrhage.The mean follow-up duration was 57.7 months(19-156 months) and no recurrence was found. Conclusion SPN is a rare neoplasm in children who go to see doctors because of clinical symptoms.Surgical resection, especially organs-preserving resection, may improve the long-term results. Clinical observation of pancreatoduodenectomy combined with Tegafur chemotherapy for solid pseudopapillary tumor of the pancreas

Objective To investigate the clinical effect of pancreatoduodenectomy combined with Tegafur in the treatment of solid pseudopapillary tumor of the pancreas. Methods Retrospective analysis the cases of 84 patients with solid pseudopapillary tumor of the pancreas from Baoji People′s Hospital from January 2010 to January 2015. Among them, 34 patients received adjuvant chemotherapy such as fluorouracil before and after treatment (combined treatment group), and 50 cases were not treated with chemotherapy after operation surgical treatment group. The effective rate and control rate of patients with WHO solid tumors were compared and analyzed by Kaplan-Meier criteria. The survival rate of patients was calculated by the Karnofsky method, and the quality of life was compared and analyzed by using the score criteria. Measurement data were represented as ±s, and comparsion between groups was analyzed using the t test.Count data were evaluated by the percentage, and comparsion between groups was analyzed using the chi-square test. Results The effective rate and control rate of the combined treatment group were 85.29% and 97.06%, the surgical treatment group were 72.00% and 86.00%. The difference was statistically significant (P 0.05). After treatment, the level of Karnofsky score of two groups were significantly increased (P 0.05). The survival rate was 91.18% in the combined treatment group and 88% in the surgical treatment group, and the difference was statistically significant (P<0.05). Conclusion Solid pseudopapillary tumor of the pancreas in patients after pancreaticoduodenectomy combined with fluorouracil chemotherapy alone and surgery were compared, no significant difference in quality of life, but it can significantly improve the survival rate of patients. Laparoscopic management of solid pseudo papillary neoplasm of pancreas in tertiary care center from south India

BackgroundSolid pseudo papillary neoplasm (SPN) of the pancreas which predominantly affects young women is a relatively indolent entity with favorable prognosis. Resection through minimal access approach helps to achieve better short term benefits. The aim of this study is to describe our experience in laparoscopic management of this disease. MethodsA retrospective review of our prospectively maintained database revealed that 17 patients with SPN were managed with surgical resection between March 2009 and October 2016. The clinical data of these patients were then analyzed. ResultsAmong the 17 cases of SPN, 14 were females and 3 were males. The mean age at presentation was 26.1 years (11–46 years). The most common presenting symptom was an abdominal pain (n = 10; 58.8%). A tumor was incidentally detected in 5 patients. The neoplasm was localized in the pancreatic head/neck in 6 patients and in the body/tail in 11. The median diameter of the tumors was 7.5 cm (2–13 cm). Five patients underwent pancreaticoduodenectomy, 10 had distal pancreatectomy, while median pancreatectomy or enucleation was performed in one each. All the patients were offered laparoscopic surgery; one distal pancreatectomy was converted to open in view of bleeding. The median length of stay was 7 days (5–28 days). The patients were followed up for a median period of 31 months (3–62 months). ConclusionSPN is a rare neoplasm with low malignant potential and has an excellent prognosis. In our experience, laparoscopic surgical resection is safe and feasible, even for larger lesions.

SPLEEN PRESERVING DISTAL PANCREATECTOMY FOR AN UNCOMMON LARGE PANCREATIC MASS: A CASE REPORT

Solid pseudo papillary tumour is a rare primary neoplasm of the pancreas with low malignant potential that typically affects young Asian women. We herein report a case of a teen aged girl who presented with a large painless progressively increasing asymptomatic abdominal lump. On radiological investigation, there was a solitary large heterogenous mass arising from the tail of pancreas. An ultrasound guided fine needle aspiration cytology revealed a solid pseudopapillary epithelial neoplasm. The patient underwent spleen preserving distal pancreatectomy. The histopathology confirmed the diagnosis of solid pseudopapillary neoplasm of pancreas. The patient had an uneventful recovery and is under follow up.

Solid pseudopapillary tumor of pancreas: 7 cases report and literature review

目的 探讨胰腺实性假乳头状肿瘤(SPT)的病理学特征、诊断要点、预后及鉴别诊断。 方法 回顾性分析7例SPT患者的临床资料，观察其组织学特征、免疫组织化学结果，并进行随访。 结果 7例SPT中，女性6例，男性1例，年龄16～41岁，平均年龄23.3岁，胰腺各部位均有发生。镜下肿瘤细胞排列成实性片状或围绕纤维血管轴心形成假乳头结构，胞质内可见嗜酸小体．免疫组织化学显示β-catenin特征性表达于细胞核及细胞质， CD10、vimentin、孕激素受体(PR)、Syn、CD56、α-抗胰蛋白酶(AAT)均弥漫或部分阳性，CK呈阳性或阴性表达，E-cadherin、CgA均阴性，Ki-67阳性率一般<5%。手术后随访，无复发。 结论 SPT是一种少见的低度恶性肿瘤，好发于年轻女性患者，总体预后良好，应积极采取手术治疗，并进行长期随访。

Clinical application of binding pancreaticogastrostomy in the central pancreatectomy

Objective To investigate the clinical application of binding pancreaticogastrostomy (BPG) in the central pancreatectomy (CP). Methods The clinical data of 62 patients with benign and low-grade malignant lesions in the neck and body of pancreas who received CP combined with BPG from January 2010 to October 2014 were retrospectively analyzed. Fifty-six patients with space-occupying lesions of the head and neck of pancreas were confirmed by postoperative pathological examinations, including 21 solid pseudopapillary tumors of pancreas (SPTPs), 19 pancreatic neuroendocrine neoplasms (PNENs) (13 non-functional islet cell tumors), 16 pancreatic cystic tumors (12 serous cystadenomas and 4 mucinous cystadenomas) and 6 ruptures in the head and neck of pancreas. CP combined with BPG was performed. The central pancreas was resected via upper and anterior approaches after surgical exploration, and digestive tract reconstruction was applied using BPG. The operation time, volume of intraoperative blood loss, time of postoperative gastrointestinal function recovery, drainage tube removed time, duration of hospital stay and postoperative complications were recorded. Patients were followed up by outpatient examination and telephone interview up to January 2015, and follow-up included the level of blood glucose, conditions of pancreatic exocrine function and with or without pancreatic pseudocyst. Results All the patients underwent successful operation without perioperative death. The average operation time, average volume of intraoperative blood loss, average time of postoperative gastrointestinal function recovery, average drainage tube removal time and average duration of postoperative hospital stay were 155 minutes (range, 125-230 minutes), 300 mL (range, 210-425 mL), 3.0 days (range, 2.0-5.0 days), 6.0 days (range, 4.0-10.0 days) and 10.5 days (range, 9.0-21.0 days), respectively. Seven patients with delayed gastric emptying were cured by non-surgical treatment. Of 6 patients complicated with pancreatic fistula, 4 patients (Grade A) had healed pancreatic fistulas during hospitalization, 2 patients (Grade B) with drainage tubes were discharged from hospital and then drainage tubes were removed after confirming healed pancreatic fistula by imaging examination. Of 2 patients with intraperitoneal hemorrhage, 1 underwent under gastroscope cauterisation for hemostasis and 1 underwent open reoperation for hemostasis. All the patients were followed up for 3-36 months with a median time of 25 months and without high blood glucose, pancreatic exocrine function insufficiency and pancreatic pseudocyst. Conclusions CP with the advantages of minimal invasion and quick recovery can be used as a priority surgical method for benign or low-grade malignant tumors and injures in the neck and body of pancreas. BPG is safe and feasible as well as reduce the incidence of pancreatic fistula after CP, and it is an ideal reconstruction. Abstract 560: Distinct protein expression patterns of solid pseudopapillary neoplasm of pancreas

Abstract Solid pseudopapillary neoplasm (SPN), is an uncommon pancreatic tumor with distinct clinicopathologic features. SPNs are characterized by mutation in exon 3 of CTNNB1. However, little is known about SPN tumorigenesis but CTNNB1 mutation. In order to identify the characteristic gene expression patterns of SPN, we performed proteome analysis of SPN and compared to gene expression data. We analyzed pooled SPNs and pooled non-neoplastic pancreas tissues using LTQ-Orbitrap. In total, we identified 862 (444 up-regulated and 418 down-regulated) proteins showed differential expression in SPN compared to non-neoplastic pancreas. We compared proteome profiling compared to mRNA expression of SPNs, and then selected 544 significant proteins presented the same expressional tendency. Many of the overexpressed proteins are related to the activated signaling pathways of SPN consistently with our previous study. Specifically, DKK4 (6.1-fold), APC (1.5-fold), RUVBL1 (1.4-fold) and β-catenin (1.3-fold) involved in Wnt signaling were up-regulated in protein level of SPNs compared to non-neoplastic pancreas. Besides Wnt signaling, we detected several up-regulated proteins including FUS and NONO. The interaction of β-catenin with FUS was reported that involved in the regulation of pre-mRNA splicing. We identified that FUS and NONO interacted directly with β-catenin by immunoprecipitation in β -catenin active cell lines (SW480, HCT116). We found that molecules involved in glycolysis, metabolism and sumoylation were activated in SPNs; HK1 (3.2-fold), PKM2 (3.3-fold), ENO2 (2.1-fold), PDHA (1.5-fold) and SUMO2 (1.6-fold). We also detected that several proteins involved in epithelial-mesenchymal transition were up or down-regulated including CTSB (3.98-fold), VIM (1.98-fold) and JUP (-2.29-fold). Our proteomic profiling study of SPNs provides 1) SPN-specific molecular mechanism for solid-pseudopapillary neoplasm tumorigenesis similar with mRNA expression pattern including Wnt signaling, 2) SPN-specific epithelial-mesenchymal transition, and 3) a metabolism is activated in SPNs. Ultrasound-guided fine-needle aspiration cytology of solid pseudopapillary tumor of the pancreas: a tertiary care centre experience

Background: Solid pseudopapillary tumours (SPT) of pancreas is an unusual neoplasm with uncertain malignant potential. It is seen predominantly in adolescent girls and young women. We present the clinical, ultrasound-guided fine needle aspiration (US-FNA) cytologic features, differential diagnosis and immunohistochemical findings in SPT. Methods: A retrospective 4-year (2007–2010) fine needle aspiration samples of 8 solid pseudopapillary tumours of pancreas were reviewed. Results: The entire cases were cured with exploratory laparotomy. This study includes 8 cases (1 male, 7 female) of solid pseudopapillary tumours of pancreas. All these cases showed characteristic cytomorphological features displaying hypercellular smears with presence of several papillary fragments lined by multilayered anisomorphic atypical cells having fine chromatin with delicate nuclear groove, myxoid stroma and foamy macrophages in a haemorrhagic background. Conclusions: Correct preoperative cytological diagnosis of solid pseudopapillary tumours of pancreas may be accurately possible by US-FNA technique and also aids in treatment of the surgically curable cancer in the patients.

A Solid Pseudopapillary Tumor of the Pancreas Threated with Distal Pancreatectomy and Splenectomy: Report of A Case

Solid pseudopapillary tumor of the pancreas is a rare epitelial tumor with a low potential for malignancy constitutes approximately 1% of all exocrine pancreatic tumors. We aimed to present a case with a solid pseudopapillary tumor of pancreas treated with distal pancreatectomy and splenectomy. A 18-year-old female patient was admitted to our general surgery department with a complaints of abdominal pain, distension and dysorexia. Abdominal computerized tomography revealed a solid mass located in the pancreatic tail. Distal pancreatectomy and splenectomy was performed. Histopathological examination revealed a solid pseudopapillary tumor of pancreas. Low volume pancreatic fistula was occurred in postoperative period and the patient was discharged on the ninth day. Solid pseudopapillary tumor of the pancreas a rare neoplasm which frequently effects the young women. Surgical resection is generally curative.
