Abnormal 18F-FDG uptakes in the prostate due to two different conditions of urine reflux: a mimicker of prostate cancer.

A 69-year-old man with lung cancer underwent 18F-fluorodeoxyglucose (FDG) positron emission tomography (PET)/CT for staging. FDG PET/CT showed high uptakes in the prostate gland with calcification, and magnetic resonance imaging was recommended to check the prostatic malignancy. T2-weighted images revealed midline cystic lesion at the base to midgland level and cystic lesion in right apical peripheral zone. We suspected urine reflux conditions. Voiding cystourethrography demonstrated those cystic lesions were communicating with the urethra. Therefore these lesions were diagnosed as the prostatic utricle cyst and the dilated prostatic duct in peripheral zone. We conclude that the urine reflux condition should be recognized as a prostate benign lesion with FDG accumulation.

The prostatic utricle cyst with huge calculus and hypospadias: A case report and a review of the literature.

Prostatic utricle cysts with calculus and hypospadias are rare. There are a few reported cases. We present a case of a prostatic utricle cyst with huge calculus in a 25-year-old male. He had a history of left cryptorchidism and surgery for penoscrotal hypospadias in his infancy. He was referred for frequent micturition, urgency of urination, urine pain, terminal hematuria, and dysuria. A computed tomography (CT) revealed a retrovesical cystic lesion of low density, showing a 5 × 5-cm calcification. Retrograde urethrocystography showed a 5 × 5-cm high-density shadow in the posterior urethra. The cyst was incised by transperineal approach and the stone was clearly observed and removed. Urethral stricture repair was performed simultaneously. The patient recovered smoothly after surgery.

MRI in diagnosis of a giant prostatic utricle.

A prostatic utricle cyst is an epithelial lined diverticulum arising from the prostatic urethra and usually asymptomatic when small. When enlarged, it may be symptomatic and is typically accompanied by hypospadias. We present a case of a markedly enlarged prostatic utricle in a neonate without hypospadias, demonstrated on voiding cystourethrography (VCUG), ultrasound, and 1.5 Tesla MRI.

Cysts of the Lower Male Genitourinary Tract: Embryologic and Anatomic Considerations and Differential Diagnosis

Cysts of the lower male genitourinary tract are uncommon and usually benign. These cysts have different anatomic origins and may be associated with a variety of genitourinary abnormalities and symptoms. Various complications may be associated with these cysts, such as urinary tract infection, pain, postvoiding incontinence, recurrent epididymitis, prostatitis, and hematospermia, and they may cause infertility. Understanding the embryologic development and normal anatomy of the lower male genitourinary tract can be helpful in evaluating these cysts and in tailoring an approach for developing a differential diagnosis. There are two main groups of cysts of the lower male genitourinary tract: intraprostatic cysts and extraprostatic cysts. Intraprostatic cysts can be further classified into median cysts (prostatic utricle cysts, müllerian duct cysts), paramedian cysts (ejaculatory duct cysts), and lateral cysts (prostatic retention cysts, cystic degeneration of benign prostatic hypertrophy, cysts associated with tumors, prostatic abscess). Extraprostatic cysts include cysts of the seminal vesicle, vas deferens, and Cowper duct. A variety of pathologic conditions can mimic these types of cysts, including ureterocele, defect resulting from transurethral resection of the prostate gland, bladder diverticulum, and hydroureter and ectopic insertion of ureter. Accurate diagnosis depends mainly on the anatomic location of the cyst. Magnetic resonance imaging and transrectal ultrasonography (US) are excellent for detecting and characterizing the nature and exact anatomic origin of these cysts. In addition, transrectal US can play an important therapeutic role in the management of cyst drainage and aspiration, as in cases of prostatic abscess.

An abnormally large prostatic utricle cyst associated with unilateral renal agenesis

Prostatic utricle cyst is an uncommon congenital disorder associated with urogenital anomalies. We present a case of an abnormally large prostatic utricle cyst filling the whole of the abdominal cavity with unilateral renal agenesis in an 8-year-old male child.

Cystic lesions of the prostate are rare and often asymptomatic. Medial cysts are conventionally divided into so-called Müllerian duct cysts and prostatic utricle cysts which seems rather questionable in light of recent studies. We report a case of a patient with a 10-year history of dysuria. Diagnostic imaging was performed using 3T prostate MRI with a body coil. Because of the potential risk of retrograde ejaculation with subsequent infertility and of incontinence by the transurethral resection, the interventional therapy of his utricle cyst was abandoned.

The Role of Cross-sectional Imaging in Male Infertility: A Pictorial Review

Infertility is a common problem. The role of imaging in assisting clinical evaluation is discussed. Ultrasound and magnetic resonance imaging are first-line, noninvasive imaging techniques that provide accurate definition of anatomical causes of infertility. This affords an opportunity to deliver timely and appropriate treatment. This pictorial review illustrates normal imaging anatomy and various causes of male infertility, and focuses on congenital and acquired testicular abnormalities and post-testicular obstruction, such as congenital absence of the vasa deferentia, seminal vesicle cysts, prostatic utricle cysts, Mullerian cysts, ejaculatory duct cysts (Wolffian cysts), and epididymal obstruction.

Prostatic utricle cyst – a case report and review of current literature

We present a 23 year old mentally challenged male with hypospadias who presented with haematuria. Digital rectal examination revealed a large firm non-tender midline swelling whose upper limit could not be reached, palpable just above the prostate. CT scan showed a homogenously hypodense thick walled cystic lesion in the region of prostate, enhancing with contrast. Cystoscopy revealed a small opening at the summit of verumontanum in the midline. Intra-operatively, the thick walled hollow cavity was found in the retro-vesical region, adherent to the prostate and right seminal vesicle.Histopathological examination done showed that the wall of a cavitary lesion was lined by inflammatory granulation tissue suggestive of a prostatic utricle cyst. Enlarged prostatic utricles are commonly seen in patients with hypospadias, cryptorchidism and intersex. The posterior sagittal rectum-retracting approach has been described as one of the most suitable approaches for the surgical management of this condition.

Perineal US in the diagnosis of a prostatic utricle cyst in an infant

The formation of a prostatic utricle cyst is a congenital abnormality in the involution of Müllerian ducts. This report describes an infant boy with bilateral epididymitis and a prostatic utricle cyst detected using a perineal US approach. According to our literature review, the use of US to detect this abnormality is rare. The perineal approach provides a better visualization of the cyst than the abdominal approach, and the relation between the cyst and the adjacent anatomic structures can be determined.

New classification of midline cysts of the prostate in adults via a transrectal ultrasonography‐guided opacification and dye‐injection study

To reclassify midline cysts (MLCs) of the prostate according using the results from transrectal ultrasonography (TRUS)-guided opacification and dye injection. Eighty-six patients (mean age 60.9 years) who had MLCs detected in the pelvis by TRUS were investigated. In all patients the size of the MLC was measured and they had transperineal aspiration under TRUS guidance. After aspiration of the MLC a mixture of water-soluble contrast medium and indigo carmine dye was injected to check for communication with the urethra or seminal tract by endoscopic and pelvic X-ray examination. We classified MLCs into four categories: (i) type 1 (nine cases), MLC with no communication into the urethra (traditional prostatic utricle cyst); (ii) type 2a (60 cases), MLC with communication into the urethra (cystic dilatation of the prostatic utricle, CDU); (iii) type 2b (14 cases), CDU which communicated with the seminal tract; (iv) type 3 (three cases), cystic dilation of the ejaculatory duct. The location, shape and volume of the MLC, and the prostate-specific antigen level of MLC fluid, did not influence the classification. The most common type of MLC was CDU. A new classification that depends on the communication with the urethra or seminal tract is proposed.

A CLINICAL ENTITY OF CYSTIC DILATATION OF THE UTRICLE ASSOCIATED WITH HEMOSPERMIA

We investigated the mechanism of hemospermia in patients with a midline cyst (MLC) of the prostate, focusing on cystic dilatation of the utricle (CDU) as a possible causative lesion. Of 138 patients with hemospermia 30 (22%) had an MLC, of whom 19 underwent transperineal needle aspiration of the MLC and bilateral seminal vesicles to determine the site of bleeding. Following MLC aspiration a mixture of dye and contrast medium was injected. The verumontanum was observed endoscopically and pelvic x-ray was done. Seminal vesicle fluid on 1 or 2 sides was hemorrhagic in 13 of the 19 patients (aspiration failed in 6) and fluid from the MLC was nonhemorrhagic in 5 of the 19 (aspiration failed in 7). The MLC communicated with the urethra (CDU) in 15 patients (79%) and with 1 or 2 ejaculatory ducts in 11 (58%). In 5 of 11 patients with communication with the ejaculatory duct hemospermia persisted for more than 1 year. Four of these patients were cured by transurethral unroofing of the CDU. CDU is a significant underlying lesion for hemospermia. Communication with the ejaculatory ducts was frequent in patients with chronic hemospermia and transurethral unroofing was effective for eliminating hemospermia.

Male genital malformations associated with right renal agenesis

A 30-year-old man visited our department complaining of discomfort on micturition. Hypoplasia of the bilateral epididymis, impalpable vas deferens, and prostatic tenderness were noted. Diagnostic imaging revealed agenesis of the right kidney, cystic dilation of the right seminal vesicle, and a prostatic utricle cyst. Examination of the genital tract confirmed aplasia of the left seminal vesicle and vas deferens and hypoplasia of the right seminal vesicle. On the basis of postoperative extravasation of contrast medium to the right epididymis, cystic dilation of the right seminal vesicle was diagnosed.

RETROVESICAL MASS IN MEN: PITFALLS OF DIFFERENTIAL DIAGNOSIS

Purpose We review the differential diagnosis and treatment of retrovesical masses in men. Materials and Methods During the last 8 years 21 male patients 3 to 79 years old (mean age 47.1) presented with symptoms or signs of a retrovesical mass. Clinical features and diagnostic findings were reviewed, and related to surgical and histopathological findings. Results The retrovesical masses included prostatic utricle cyst in 3 cases, prostatic abscess in 1, seminal vesicle hydrops in 6, seminal vesicle cyst in 2, seminal vesicle empyema in 3, large ectopic ureterocele in 1, myxoid liposarcoma in 1, malignant fibrous histiocytoma in 1, fibrous fossa obturatoria cyst in 1, hemangiopericytoma in 1 and leiomyosarcoma in 1. In 17 patients various symptoms were seen and in 4 the mass was incidentally detected. A mass was palpable on digital rectal examination in 16 cases and visible on sonography in 20. For a cystic mass medial location relative to the bladder neck was suggestive of prostatic abscess or utricle cyst, while lateral location was suggestive of seminal vesicle cyst/hydrops or empyema, ectopic ureter or ureterocele. In 6 patients diagnosis was established only by exploratory laparotomy and histopathological examination. Conclusions Digital rectal examination and sonography reliably detect a retrovesical mass. Nevertheless, clinical signs and median or lateral location relative to the bladder neck on ultrasound are diagnostic only for cystic lesions. Computerized tomography and magnetic resonance imaging are useful for staging malignant tumors. However, needle or open biopsy is required in most cases to establish a histopathological diagnosis. Exploratory laparotomy and histopathological examination are the procedures of choice when other findings are equivocal.

Evaluation of 42 patients with chronic abacterial prostatitis: are there any underlying correctable pathologies?

Forty-two young men with abacterial prostatitis underwent physical examination, semen analyses, rectal US (TRUS) and magnetic resonance imaging (MRI) of the pelvis. Findings on TRUS showed no distinct differences like capsular irregularity, dilatation of the seminal vesicles and calcified foci in the prostate. In MRI pathologic conditions were noted as transitional zone cysts, prostatic utricle cysts and abscesses of the seminal vesicles. MRI had been found superior to TRUS in imaging the prostate and to predict associated pathologic conditions. It is also concluded that some additional factors as a cause of prostatitis must be determined in patients with chronic abacterial prostatitis.

Cystic lesions of the prostate.

As the use of ultrasound (US) for evaluating the prostate gland has grown, so has the variety of cystic prostatic lesions that can be identified with US. We review many of these cystic lesions seen in our department over the past several years. Lesions discussed include müllerian duct cyst, prostatic utricle cyst, ejaculatory duct cyst, cystic degeneration in benign prostatic hyperplasia, prostatic retention cyst, cavitary prostatitis, and prostatic abscess. The defect from transurethral resection of the prostate gland, which may have a sonographic appearance similar to that of a cystic prostatic mass, is also discussed. A diagnostic algorithm based on a combination of sonographic findings and the results of transperineal needle aspiration is presented.

Prostatic Utricle Cysts (Müllerian Duct Cysts)

A series of 88 male patients with prostatic utricle cysts (müllerian duct) has been compiled by adding our 3 patients to 85 cases reported. Irritative lower urinary tract symptoms were the most common presenting complaint (42 per cent), while lower obstructive symptoms were noted in 29 per cent. Of the 20 clinical case reports in boys 25 per cent presented with epididymitis often in an undescended gonad. A cystic rectal mass found in half of the patients represents the most common presenting sign, while hypospadias occurred in a fourth. Unilateral renal dysgenesis or agenesis was associated in 10 per cent of the reports. While the suprapubic surgical approach to excision has been used most commonly it has proved unsatisfactory in two-thirds of the cases. The posterior approach was used successfully in 2 boys with preservation of erectile potency in both. The wide spectrum of histopathology of these cysts prevents clear characterization. A 3 per cent incidence of malignancy in prostatic utricle cysts is noted.

Unusually large prostatic utricle cyst

Prostatic utricle cyst is one of the uncommon conditions and only a few cases have been reported. We present a case of unusually large prostatic utricle cyst in a 13- year- old male. He presented with burning urination and post-void dribbling of urine. A cystic mass was felt on digital per rectal examination. Ultrasound pelvis revealed a well-defined midline cystic mass posterior to the urinary bladder. Subsequent magnetic resonance imaging (MRI) of the pelvis demonstrated fluid containing cystic lesion communicating with posterior urethra. Surgical resection of the cyst was performed through the posterior sagittal approach. Follow up of the patient after three months of operation, there was complete resolution of the symptoms.
