Pre-operative mechanical bowel preparation and prophylactic oral antibiotics for pediatric patients undergoing elective colorectal surgery: a protocol for a randomized controlled feasibility trial

BackgroundInfections after elective colorectal surgery remain a significant burden for patients and the healthcare system. Adult studies suggest that the combination of oral antibiotics and mechanical bowel preparation is effective at reducing infections after colorectal surgery. In children, there is limited evidence for either of these practices and the utility of combining oral antibiotics with mechanical bowel preparation remains uncertain.MethodsThis study aims to determine the feasibility of conducting a randomized controlled trial assessing the efficacy of oral antibiotics, with or without mechanical bowel preparation, in reducing the rates of post-operative infection in pediatric colorectal surgery. Participants aged 3 months to 18 years undergoing elective colorectal surgery will be randomized pre-operatively to one of three trial arms: (1) oral antibiotics; (2) oral antibiotics and mechanical bowel preparation; or (3) standard care. Twelve patients will be included in each trial arm. Feasibility outcomes of interest include the rate of participant recruitment, post-randomization exclusions, protocol deviations, adverse events, and missed follow-up appointments. Secondary outcomes include the rate of post-operative surgical site infections, length of hospital stay, time to full enteral feeds, reoperation, readmission, and complications.DiscussionIf the results of this trial prove feasible, a multi-center trial will be completed with sufficient power to evaluate the optimal pre-operative bowel preperation for pediatric patients undergoing elective colorectal surgery.Trial registrationClinicalTrials.gov: NCT03593252.

Antibiotic impregnated catheters and intrathecal antibiotics for CSF shunt infection prevention in children undergoing low-risk CSF shunt surgery

BackgroundCerebrospinal fluid (CSF) shunts allow children with hydrocephalus to survive and avoid brain injury (J Neurosurg 107:345-57, 2007; Childs Nerv Syst 12:192-9, 1996). The Hydrocephalus Clinical Research Network implemented non-randomized quality improvement protocols that were shown to decrease infection rates compared to pre-operative prophylactic intravenous antibiotics alone (standard care): initially with intrathecal (IT) antibiotics between 2007–2009 (J Neurosurg Pediatr 8:22-9, 2011), followed by antibiotic impregnated catheters (AIC) in 2012–2013 (J Neurosurg Pediatr 17:391-6, 2016). No large scale studies have compared infection prevention between the techniques in children. Our objectives were to compare the risk of infection following the use of IT antibiotics, AIC, and standard care during low-risk CSF shunt surgery (i.e., initial CSF shunt placement and revisions) in children.MethodsA retrospective observational cohort study at 6 tertiary care children’s hospitals was conducted using Pediatric Health Information System + (PHIS +) data augmented with manual chart review. The study population included children ≤ 18 years who underwent initial shunt placement between 01/2007 and 12/2012. Infection and subsequent CSF shunt surgery data were collected through 12/2015. Propensity score adjustment for regression analysis was developed based on site, procedure type, and year; surgeon was treated as a random effect.ResultsA total of 1723 children underwent initial shunt placement between 2007–2012, with 1371 subsequent shunt revisions and 138 shunt infections. Propensity adjusted regression demonstrated no statistically significant difference in odds of shunt infection between IT antibiotics (OR 1.22, 95% CI 0.82–1.81, p = 0.3) and AICs (OR 0.91, 95% CI 0.56–1.49, p = 0.7) compared to standard care.ConclusionIn a large, observational multicenter cohort, IT antibiotics and AICs do not confer a statistically significant risk reduction compared to standard care for pediatric patients undergoing low-risk (i.e., initial or revision) shunt surgeries.

Comparison of Efficacy of Prophylactic Antibiotics in Preventing Infection with no Antibiotics Given for Removal of Impacted Mandibular Third Molar Tooth

Objective: to compare pre-operative prophylactic antibiotics with no antibiotics for removing an impacted mandibular third molar tooth to avoid Infection. Study Design: Quasi-experimental study Place and Duration of Study: Ayyub Teaching Hospital Abbottabad, Pakistan from Jan 2018 to June 2018 Methodology: Two hundred four study participants were divided into two groups: those with their third mandibular molar teeth impacted and those without. One Group was given an antibiotic before surgery; the other received a placebo. Both groups were followed after surgery and were observed for the development of post-operative Infection. Results: There were 204 participants in our study whose age group ranged from 17-30 years, (mean age 23.89±2.39 years). Of 204 individuals, 182(89.2%) were treated well, whereas 22 individuals were diagnosed with Infection (10.8%). 18 individuals (17.6%) developed Infection in the Placebo Group, whereas 4(3.9%) patients developed infection in the Antibiotic Group. Conclusion: The frequency of post-operative infections is reduced in individuals with an impacted third mandibular molar tooth who are given antibiotics before surgery.

Single stage pediatric airway reconstruction in solid organ transplant recipients

ObjectivesTo describe results of single stage laryngotracheal reconstruction (ssLTR) in patients with solid organ transplants and to discuss modifications which need to be considered in this subset of patients pre-operatively, intra-operatively and post-operatively. Study designWe performed a retrospective case series review of children undergoing single stage laryngotracheal reconstruction in the context of prior solid organ transplant. SettingA tertiary care academic setting. MethodsPediatric cases undergoing Laryngotracheal reconstruction over a 3-year period. Demographic data including age, sex, presenting symptoms, operative details. ResultsTwo cases of ssLTR in solid organ transplant patients were found, one each with renal and cardiac transplants respectively. Both patients successfully underwent ssLTR for Grade 2 subglottic stenosis. The care of these patients was multidisciplinary and required alterations in their preoperative prophylactic antibiotics. While they did not require changes to the LTR post-operative sedation protocol, their immunosuppressant doses and target ranges were lowered. Special care was taken to avoid nephrotoxic and cardiotoxic medications throughout their hospital stay. ConclusionsAlthough traditionally considered for double stage laryngotracheal reconstruction, single stage laryngotracheal reconstruction is a viable option in patients with solid organ transplant. These patients require a multidisciplinary approach and pharmacological protocol alterations pre-, intra-, and post-operatively.

Preoperative Prophylactic Antibiotics Administration Does Not Influence Culture Yield in Revision Shoulder Arthroplasty.

The importance of administrating prophylactic antibiotics prior to a surgical procedure is well established. Given the difficulty in diagnosing shoulder periprosthetic infections, which are more indolent in nature, some advocate holding prophylactic antibiotics prior to obtaining cultures as there is a concern antibiotics may lead to a false negative culture result. The purpose of this study is to determine whether administration of antibiotics prior to obtaining cultures in revision shoulder arthroplasty influences culture yield. This was a retrospective analysis of revision shoulder arthroplasty cases performed at a single institution between 2015 and 2021. During the study period, each surgeon had a standardized protocol that dictated whether antibiotics were given or held prior to each revision surgery. Each case was categorized into either a Pre-culture antibiotic group, if antibiotics were administered prior to incision, or a Post-culture antibiotic group if antibiotics were administered after incision and obtaining cultures. Basic demographics and the International Consensus Meeting (ICM) scoring criteria provided by the Musculoskeletal Infection Society was used to categorize the probability of periprosthetic joint infection for each case. Culture positivity was calculated as the ratio of positive cultures and total number of cultures obtained. One hundred and twenty-four patients met inclusion criteria. There were 48 patients in the Pre-culture group and 76 patients in the Post-culture group. No significant difference in patient demographics or ICM criteria (p=0.09) was observed between the two groups. With regards to culture positivity, there was no difference between the Pre-culture antibiotic group and the Post-culture antibiotic group (16% vs 15%, p=0.82, confidence interval= 8-25% vs. 10-20%, respectively). In the setting of revision shoulder arthroplasty, timing of antibiotic administration did not significantly influence culture yield. This study supports the use of prophylactic antibiotics prior to obtaining cultures in revision shoulder arthroplasty.

Laparoscopic cholecystectomy (LC) in patients admitted with acute cholecystitis is considered the preferred, feasible and safe mode of managing gallstone disease. The objective of this study is to evaluate the role of single-dose pre-operative prophylactic antibiotics in patients undergoing emergency LC for mild to moderate acute cholecystitis. All randomized control trials (RCTs) reporting the use of single-dose pre-operative prophylactic antibiotics in patients undergoing acute cholecystectomy were retrieved from the search of standard medical electronic databases and analysis was conducted by using the principles of meta-analysis on the statistical software RevMan version 5. Standard medical databases search produced only 3 RCTs on 781 patients undergoing acute cholecystectomy. There were 384 patients in single dose pre-operative antibiotics group whereas 397 patients were recruited in the no-antibiotics group. In the random effects model analysis, the use of single-dose preoperative prophylactic antibiotics in patients undergoing acute cholecystectomy for mild to moderate cholecystitis failed to demonstrate any extra advantage of reducing the risk of [risk ratio (RR) =0.69; 95% confidence interval (CI): 0.46-1.03; Z=1.80; P=0.07] infective complications. There was no heterogeneity [Tau2 =0; Chi2 =1.74, df =2 (P=0.42; I2=0%)] among included studies. A preoperative single dose of prophylactic antibiotics in patients undergoing acute LC for mild to moderate acute cholecystitis does not offer extra benefits to reduce infective complications.

FTP7.14 A systematic review and meta-analysis evaluating the role of single-dose preoperative versus extended use of antibiotics therapy following acute cholecystectomy

The objective of this study is to evaluate the role of single dose pre-operative prophylactic antibiotics versus extended antibiotics therapy up to 5th post-operative days in these patients undergoing acute cholecystectomy for mild to moderate acute cholecystitis. Materials and methods RCTs reporting the use of single dose pre-operative prophylactic antibiotics versus extended antibiotics therapy up to 5th post-operative days following acute cholecystectomy were retrieved from the search of standard medical electronic databases and analysis was conducted by using the principles of meta-analysis on the statistical software RevMan version 5. Results The search of medical databases yielded 4 RCTs on 949 patients undergoing acute cholecystectomy. There were 481 patients in in single dose pre-operative antibiotics group whereas 468 patients received extended doses antibiotics therapy. In the random effects model analysis, the extended use of post-operative antibiotics in patients undergoing acute cholecystectomy for mild to moderate acute cholecystitis does not confer any extra benefit [risk ratio 1.05, 95%, CI (0.73, 1.51), Z = 0.25, P=0.80] in terms of reducing the infective complications. There was no heterogeneity (Tau2 = 0; Chi2 = 0.40, df = 3; (P = 0.94; I2 = 0 %) among included studies. Conclusion Post-operative extended use of antibiotics in patients undergoing acute cholecystectomy for mild to moderate acute cholecystitis does not demonstrate extra benefit to reduce infective complications.

Second-Line Antibiotic Agents in Patient-Reported Penicillin or Cephalosporin Allergy Have No Negative Impact on Antibiotic Resistance After Hip and Knee Arthroplasty

BackgroundThe aim of the present study was to compare causative bacteria and their antibiotic resistance profiles in patients developing a periprosthetic joint infection (PJI) based on preoperative prophylactic antibiotic regimens in primary total hip (THA) and primary total and unicompartmental knee arthroplasty (TKA/UKA). MethodsWe reviewed all cases of PJI occurring after primary THA and primary TKA/UKA, between 2011 and 2020 in a tertiary referral hospital. The standard preoperative prophylactic antibiotic for primary joint arthroplasty was cefuroxime and recommended second-line agent was clindamycin. Patients were divided by the replaced joint and analyzed independently. ResultsIn the THA group, culture-positive PJI was detected in 61 of 3,123 (2.0%) cefuroxime-administered cases and 6 of 206 (2.9%) noncefuroxime-administered cases. In the TKA/UKA group, culture positive PJI was identified in 21 of 2,455 (0.9%) cefuroxime-administered cases and in 3 of 211 (1.4%) noncefuroxime administered cases. The most commonly isolated bacteria in both groups were coagulase negative staphylococci (CNS). There were no statistically significant differences of pathogen spectrum depending on the preoperative antibiotic regimen detected. Antibiotic resistance of isolated bacteria was significantly different in 4 of 27 (14.8%) analyzed antibiotics in THA and in 3 of 22 (13.6%) analyzed antibiotics in TKA/UKA. In all cohorts, a high occurrence of oxacillin-resistant CNS (50.0 to 100.0%) and clindamycin-resistant CNS (56.3 to 100.0%) has been observed. ConclusionThe use of the second-line antibiotic did not influence the pathogen spectrum or antibiotic resistance. However, an alarmingly high proportion of CNS strains was resistant to clindamycin.

Role of Prophylactic Dose of Oral Amoxicillin Trihydrate in Prevention of Bacteremia Post Surgical Removal of Impacted Mandibular Third Molar: A Clinical Study.

The current study is designed to evaluate the efficacy of pre-operative prophylactic antibiotic for prevention of bacteremia after surgical removal of impacted mandibular third molar SRIMTM. The study was carried out as an open clinical trial. Fifty patients who fulfilled the criteria were included. Twenty five randomly selected patients (study group) were administered two grams oral dose of amoxicillin trihydrate, one hour prior to the procedure. Twenty five patients (control group) were not administered any antibiotic prior to the procedure. All patients underwent surgical removal of impacted mandibular third molar through intraoral approach under local anesthesia. Blood culture for detection of aerobic and anaerobic micro-organisms was done for all 50 patients. Eleven out of the twenty five patients (44%) in the control group showed positive growth, including mixed growth (aerobic and anaerobic). Seven patients showed purely anaerobic, 2 patients purely aerobic and two mixed growth. In the study (antibiotic) group, 7 out of the 25 patients (28%) showed positive growth, including mixed growth. Three patients showed purely anaerobic, 2 patients purely aerobic and 2 mixed type of growth. The study suggests that the judicious use of antibiotic prophylaxis, in conjunction with optimal oral health care serves to minimize the adverse effects of antibiotic therapy; and at the same time reduce the significant morbidity and mortality associated with systemic infections that might occur due to bacteremia resulting from surgical procedures of the oral cavity.

Proper Timing for Administration of Prophylactic Intravenous Antibiotics for Elective Surgical Procedures

Background: Preoperative antibiotic prophylaxis is administering antibiotics before performing surgery to help decrease the risk of postoperative infections. The antibiotic selected should only cover the likely pathogens. It should be given at the correct time. The timing of antibiotic administration may vary, but the goal of administering preoperative systemic prophylactic antibiotics is to have the concentration in the tissues at its highest at the start and during surgery. The literature supports at least 30 minutes, but no greater than 60 minutes before the skin incision is made to the optimal timing for the preoperative administration of most commonly used antibiotics. Aims and Objectives: To obtain precise information on the optimal time window for surgical antimicrobial prophylaxis (SAP). Methodology: A retrospective observational study design was used to investigate the effect of timing of SAP on the risk of surgical site infection (SSI) from September 2021 until September 2022. Patients with Inguinal hernia who performed elective inguinal hernia repair were eligible for enrollment in the study. The risk of SSI was compared between patients, categorized by the time interval between SAB administration and incision, and adjusted for confounding through Univariate Analysis logistic regression. Hakeem data base was used to collect the required data. This study was performed in King Hussein Medical Center (KHMC) at the Royal Medical Services in Amman/Jordan. Results: Data included in the current study was collected for the period between September 2021 until September 2022, in which 500 patients were randomly assigned into two arms; Patients in arm A received the in a time window of 60 to 30 minutes before the scheduled incision while Patients in arm B received the antibiotics in time window of less than 30 minutes to 0 minutes before the scheduled incision. Preoperative Prophylactic Antibiotics in Mandibular Fractures and Surgical Site Infection.

Compared with other facial fractures, mandibular fractures have the highest rate of postsurgical site infection. There is strong evidence to suggest that postoperative antibiotics do not reduce rates of surgical site infections (SSIs) regardless of duration. However, there are conflicting data in the literature regarding the role of prophylactic preoperative antibiotics in reducing rates of SSIs. This study reviews the rate of infections in patients who underwent mandibular fracture repair who received a course of preoperative prophylactic antibiotics compared with those who received no or 1 dose of perioperative antibiotics. Adult patients who underwent mandibular fracture repair at Prisma Health Richland between 2014 and 2019 were included in the study. A retrospective cohort review was conducted to determine the rate of SSI, comparing 2 groups of patients who underwent mandibular fracture repair. Patients who received more than 1 dose of scheduled antibiotics before surgery were compared with those who did not receive any antibiotic treatment before surgery or received only a single dose of antibiotics within 1 hour of incision time (perioperative antibiotics). The primary outcome was the rate of SSI between the 2 groups of patients. There were 183 patients who received more than 1 dose of scheduled antibiotics before surgery and 35 patients who received a single dose of perioperative antibiotics or did not receive any antibiotics. The rate of SSI was not significantly different in the preoperative prophylactic antibiotics group (29.3%) compared with the patients who received a single perioperative dose or no antibiotics (25.0%). Extended regimens of preoperative prophylactic antibiotics beyond a single dose at time of surgery do not reduce SSIs after surgical repair in mandibular fractures.

Analyzing complications and implementing solutions in a pediatric inguinal hernia cooperation program in Equatorial Guinea: a prospective cohort study

BackgroundFew studies have evaluated the efficacy of short-term medical missions. This study was aimed to evaluate complication rates and determine the effects of protocol changes in a pediatric inguinal hernia campaign in Equatorial Guinea and analyze post-operative follow-up capacity.MethodsIn this prospective observational cohort study, we evaluated two patient cohorts (group A, 2017–2018; group B, 2019) treated during campaigns in Equatorial Guinea for congenital inguinal pathology (hernia, hydrocele, and cryptorchidism). Patients aged < 18 years treated in referral campaigns were included. Complications occurring up to 6 months post-operatively were evaluated. Two stages were defined: Stage 1, wherein, complication rate in group A was compared to that in a control group from a tertiary hospital in Spain (with a case–control ratio of 1:2, paired according to age, sex and diagnosis); stage 2, wherein, complication rates between groups A and B were compared. Group B received a single dose of prophylactic amoxicillin-clavulanic acid. Follow-up capacity was assessed through follow-up appointments.ResultsIn stage 1, complication and surgical site infection (SSI) rates were 21.3% and 7.4% in group A (n = 94), and 5.8% (p < 0.001) and 0.5% (p = 0.012) in the control group, respectively. Group A had 20.2% loss-to-follow-up. In group B (n = 62), 6-month postoperative follow-up could not be assessed owing to restrictions due to the COVID-19 pandemic, so only early complications were considered in stage 2, were complication and surgical site infection rates were 18.1% and 7.4% in group A and 11.3% (p = 0.350) and 1.6% (p = 0.150) in group B.ConclusionOur results showed higher than expected complication rates. Pre-operative prophylactic antibiotic could not show to reduce SSI. Further studies are needed to reduce complication rates in these campaigns. Patient loss-to-follow-up ratio warrants considering new strategies.

Preoperative topical benzoyl peroxide treatment is effective in reducing Cutibacterium acnes in shoulder surgery: a systematic review.

Cutibacterium acnes (C acnes) colonization can have a significant impact on patients undergoing both arthroscopic and open shoulder surgery with regard to postoperative infection. Its resistance to standard preoperative skin preparations and prophylactic antibiotics has led to a need for a more targeted therapy. Topical benzoyl peroxide (BPO) has been used by dermatologists in the treatment for acnes due to its bactericidal and penetrative effects through the dermal layer. The aim of this systematic review is to review the effectiveness of topical BPO preoperatively in shoulder surgery in reducing C acnes colonization and postoperative infection. A review of the online databases Medline and Embase was conducted on December 15, 2021, according to Preferred Reporting Items for Systematic Reviews and Meta-analyses (PRISMA) guidelines. The review was registered prospectively in the PROSPERO database. Clinical studies reporting superficial and deep sample microbiology and postoperative complications were included. The studies were appraised using the revised Cochrane Risk of Bias 2 (ROB 2) tool for randomized studies and the Methodological Index for Non-Randomized Studies (MINORS) tool. The search strategy identified 10 studies for inclusion (6 randomized control trials, 2 prospective cohort studies, and 2 case series), including a total of 482 patients. Seven studies were comparable, testing BPO against alternative standard skin preparations. Of the 10 studies, 7 showed a decrease in the load of C acnes on the skin and/or deep tissues, of which 6 demonstrated statistical significance. Men were shown to have a statistically significant increase in the colonization rate of C acnes. Scheer etal (2021) demonstrated 4500 colony-forming units/mL in males and 900 colony-forming units/mL in females. In studies where the number of BPO applications was higher, BPO appeared more effective. Dizay etal demonstrated C acnes elimination in 78.9% with more than 1 application compared with 66.7% if only applied once. Three studies looked at the effectiveness of BPO during the operative timeline with 1 demonstrating its statistically significant effectiveness at reducing colonization 2 hours into the operation (P=.048). BPO is effective as a topical treatment at reducing C acnes colonization before shoulder surgery. However, the relationship between duration of treatment, frequency of application, and gender requires further research.

The Quantitative and Qualitative Analysis of Antibiotic Use in Bone Fracture Patients in a Public Hospital in Indonesia

Introduction: Prophylactic antibiotic therapy has an important role in facilitating optimal postoperative healing. Inappropriate use of antibiotics in hospitals will increase the cost of therapy and the incidence of nosocomial infections of infectious microorganisms. Preoperative prophylactic antibiotics are believed to reduce the incidence of surgical site infections. Objective: This study aims to determine the pattern of antibiotic use and analyse the use of antibiotics qualitatively by the Gyssen method and quantitatively by the defined daily dose (DDD) method in inpatients with bone fractures at the Haji General Hospital Surabaya, Indonesia. Methods: This study was a descriptive observational study conducted on 89 samples. Data were collected retrospectively through medical records in the period January-December 2019. Antibiotic use data were then analysed qualitatively and quantitatively using the Gyssen and DDD methods. Results: In this study, the antibiotic that was widely used as a prophylactic and therapeutic antibiotic in patients with bone fractures was ceftriaxone. Quantitative analysis using the DDD method showed that the value of ceftriaxone was 45.6/100 patient-days and cefazoline was 3.1/100 patient-days. Analysis of antibiotic use in bone fracture patients using the Gyssen method showed that the rational use of antibiotics was 84.3%, the use of antibiotics was not timely as much as 4.5%, and the interval of antibiotic administration was not appropriate as much as 11.2%. Conclusion: the use of antibiotics in fracture surgery patients is classified as rational use of antibiotics but the use of antibiotics still exceeds WHO standards. Furthermore, it is necessary to conduct a similar study with prospective data collection so that it can observe the incidence of surgical wound infection.

Effectiveness of postoperative oral administration of cephalexin and metronidazole on surgical site infection among obese women undergoing cesarean section: a randomized, double-blind, placebo-controlled, parallel-group study—phase III

BackgroundCesarean section (CS) is the most frequently performed surgery in the United States. Compared to vaginal delivery, CS has a higher risk of maternal and neonatal mortality, morbidities, and complications, among which surgical site infection (SSI) is the most common. We aimed at evaluating the effectiveness of postoperative oral administration of cephalexin and metronidazole on SSI among obese women undergoing CS.MethodsWe conducted a randomized, double-blind clinical trial comparing the prophylactic effect of oral cephalexin and metronidazole vs cephalexin and placebo on SSI following CS among obese women. who had received preoperative prophylactic cephalosporin antibiotics. The study was conducted at the Ommolbanin Hospital, affiliated with Mashhad University of Medical Sciences from April 2019 to February 2020.ResultThe participants were randomized into the intervention group (n = 210) and the control group (n = 210). At week-1 follow-up, the outcomes were significantly lower in the intervention group as compared to the control group in terms of fever (9% vs 19%, p = 0.003), abnormal discharge from the incision (serous: 8.6% vs 10.5%, purulent: 2.9% vs 16.7%, p < 0.001), incision separation (1% vs 7.1%, p = 0.001), and cellulitis (4.8% vs 13.3%, p = 0.002). At week-2 follow-up, there were no patients in the intervention group with fever, abnormal discharge from the incision, incision separation, or cellulitis and there was a statistically significant difference for fever, abnormal discharge from the incision, and incision separation between the two groups (p < 0.001, p = 0.001, p = 0.014, respectively).ConclusionPost-operative administration of cephalexin and metronidazole for 48-h post-cesarean delivery among obese women, in addition to the standard pre-operative prophylaxis, reduced the overall rate of surgical site infection and wound infection symptoms in a 2-week follow-up.Trial registration The study protocol was approved by the Iranian Registry of Clinical Trials (IRCTID: IRCT20200608047685N2) on 2021-03-15.

Cesareans antibiotic prophylaxis aims to prevent the clinical manifestations of SSI expected to occur. Theoretically, Cefazoline is considered more advantageous as a cesarean prophylactic antibiotic than Ceftriaxone. This study aims to compare the effectiveness of Cefazoline and Ceftriaxone as cesarean prophylactic antibiotics at RSIA Trisna Medika Tulungagung from October–December 2021. The study used a cross-sectional research design using 214 medical records data of patients with preoperative prophylactic antibiotics Cefazoline and Ceftriaxone (1:1). Data analysis includes antibiotic use profiles and therapeutic effectiveness data. Statistical analysis used the Mann-Whitney U Test. Research results showed that Cefazoline and Ceftriaxone have the same effectiveness as cesarean prophylactic antibiotics at RSIA Trisna Medika Tulungagung October – December 2021 based on pulse frequency, breath frequency, 10d post-op wound pain, purulent, dry wound, SSI, and LOS.&#x0D;

Propensity score analysis for the efficacy of preoperative antibiotics in patients with resected primary lung cancer: Levofloxacin versus cefazolin

ObjectiveThe use of cefazolin (CEZ) is recommended as a preoperative prophylactic antibiotic, but other antibiotics may be used for various reasons. We adopted a fluoroquinolone (levofloxacin; LVFX) as a preoperative prophylactic antibiotic because of reduced supply of CEZ worldwide, while the efficacy of LVFX in preventing infectious complications including surgical site infection (SSI), empyema, and pneumonia has not been fully investigated. MethodsThe medical records of 260 patients who underwent primary lung cancer resection between April 2018 and July 2020 were retrospectively reviewed. Eighty-nine patients before May 2019 were intravenously received a single dose of CEZ with additional administration every 3 ​h during surgery (the CEZ group) and 171 patients after that date were orally received a preoperative single dose of LVFX (the LVFX group). The efficacy of preventing infectious complications was compared between two groups. The propensity score matching (PSM) method was also applied to minimize selection bias. ResultsInfectious complications were observed in 3.1% (8/260) of patients, with no significant difference between the LVFX group (2.9%) and the CEZ group (3.4%) regardless of subtypes such as SSI and empyema. After PSM, 77 patients each were matched from the two groups, and there was also no significant difference in the incidence of infectious complications (the LVFX group; 2.6% vs. the CEZ group; 3.9%). ConclusionLVFX has comparable efficacy to CEZ for preventing infectious complications, and may be an alternative to preoperative antibiotics for patients with primary lung cancer who underwent pulmonary resection.

P31. How positive cultures affected clinical outcomes in patients undergoing primary lumbar fusion

<h3>BACKGROUND CONTEXT</h3> Occult bacteria have been linked to deep and chronic infections in spine surgery. These organisms are commensal to dermal and deeper tissue layers, including the intervertebral disc and bone. We hypothesize that standard surgical skin preparation nor preoperative antibiotics properly access and eradicate these organisms prior to primary spine surgery. These organisms can potentially adhere to the biofilm surrounding the hardware and limit bone formation. <h3>PURPOSE</h3> To determine the colonization rate of occult bacteria in the superficial and deep tissue layers of healthy patients undergoing primary instrumented lumbar spine surgery. <h3>STUDY DESIGN/SETTING</h3> This prospective, consecutive cohort of patients undergoing primary lumbar spine surgery at a single tertiary center. All patients met the inclusion/exclusion criteria which included between the ages of 18 and 80, undergoing a posterior approach with pedicle screw instrumentation with no evidence of a prior or current fracture, trauma, tumor, or an active systemic or local infection. Patients were also excluded if they were immunosuppressed or had received prior surgery at any lumbar level with hardware. This study was approved by our institutional review board and ethics review panel. <h3>PATIENT SAMPLE</h3> Fifty-four patients. <h3>OUTCOME MEASURES</h3> Culture swabs, fusion rates, revision rates. <h3>METHODS</h3> All patients received preoperative antibiotics and skin preparation. Culture samples were subsequently obtained from the superficial skin, dermal wound-edge following incision, hypodermis and the vertebral pedicle prior to instrumentation implantation. Control culture samples were taken from scalpel, tap and suction prior to incision. The primary outcome was the rate of positive culture samples from the various layers traversed during standard surgical spine exposure. Clinical outcomes of fusion status and revision rate were collected. <h3>RESULTS</h3> A total of 525 culture samples were obtained, and samples were positive in 33.3% of patients (18/54). Culture-positive patients had on average 3.1 positive samples per case. Superficial skin samples were positive in 13.0% (n=7), dermal layer samples in 16.7% (n=9), hypodermis samples in 13.0% (n=7), and vertebral samples in 20.4% (n=11) of cases. All control samples taken were culture-negative. C. acnes was the most common organism isolated, in 83.3% of culture-positive cases. Significantly more males were culture-positive than females. Patients with positive cultures were treated with antibiotics. At one-year postoperative, patients in culture-positive group had similar fusion rates to culture-negative groups. However, delayed fusion was observed at a higher rate in the culture-positive group. <h3>CONCLUSIONS</h3> Occult bacteria was detected in one-third of patients undergoing primary instrumented posterior spine surgery. Deep culture samples of the vertebral pedicle were more often positive than dermal layer samples. The data collected supports our hypothesis that occult bacteria persist in the skin and deeper tissue layers regardless of the efforts made by preoperative skin preparation and prophylactic antibiotics. The clinical sequela for culture-positive patients, although addressed perioperatively, was a higher incidence of delayed fusion. <h3>FDA DEVICE/DRUG STATUS</h3> This abstract does not discuss or include any applicable devices or drugs.
