Impact of Improper Waste Disposal on Surface and Ground Water

Pollution in water occurs when harmful substance (often chemicals or biological) contaminate streams, river, lakes, oceans or other bodies of water, degrading water quality and rendering it unhealthy to humans, animal and the environment. The widespread problem of water pollution is jeopardizing our health. Water pathogens, in form of disease-causing bacteria and viruses from human and animal waste, are a major reason of illness from contaminated drinking water. Owing to the improper treatment facilities, waste is often discharged into the surface and ground water sources. Water bacteria can be limited by recycling used plastic, standard disposal of chemicals, oils or non-biodegradable items and proper collection and transportation of solid waste to a processed disposal site. In addition, waste water should be treated properly to prevent adverse health risk of the user of both surface water and ground water in the aquatic ecosystems.

Multi-Decadal Impact of Mine Waters in Przemsza River Basin, Upper Silesian Coal Basin, Southern Poland

Anthropogenic increases in the salinity of surface waters are referred to as secondary salinization. In surface waters, salinity levels can vary significantly due to various natural and anthropogenic influences. This article presents multi-decadal observations of changes in surface water salinity in the highly industrialized region in southern Poland. The case study of the Przemsza River is an example of the significant impacts of industrial, mainly coal mining, activities that have changed the chemical and biological characteristics of water bodies. The presented research revealed that impacts on salinity and water body status due to mining discharges will be difficult or even impossible to restore, considering the process of transition of the coal sector. In the Przemsza river basin, almost 42% less mine water was discharged in 2023 than in 1991. Parallelly, the salinity of mine waters discharged from deeper levels of active coal mines has increased due to the geochemical gradient (the total load of chlorides and sulfates was 534.8 MgCl−+SO42− per day in 1991, while in 2023 the total salinity load was 480.1 MgCl−+SO42− per day). Moreover, of the 19 active mine water discharges in 1991, only 11 remain in 2023, while the observed salinity of surface water in the Przemsza watershed increased rapidly from an average of 2000 µS·cm−1 to 6700 µS·cm−1 due to the significant drought and adverse hydrological conditions, which represent low flows never observed before (three times lower flows in the mouth of the Przemsza River in the period 2021–2023 compared to the previous decades 1991–2020). Impacts on water bodies will continue to occur regardless of mining activities in the area—it should be noted that at the end of exploitation, mine water rebound and flooding do not automatically reduce long-lasting impacts on surface waters. Therefore, salinization is a growing threat that might be amplified by climate change. While industrial and urban impacts on surface water change its characteristics, the future challenge of proper water management with a holistic approach is necessary with proper monitoring data collection and river flow-dependent and surface water salinity-dependent discharge of wastewater in the river basin.

Colorectal screening program implementation in primary health care, in Loures-Odivelas, Portugal

Abstract Problem In 2020, Portugal had the highest cumulative incidence of colorectal cancer (CRC) in the European Union (103 new cases/100,000 inhabitants). The Colorectal Cancer Screening Program (CRCS) is based on fecal occult blood testing (FOBT) as the primary screening method. With the delivery of the stool sample collection device; instruction on its use is provided by the Primary Health Care team. The sample is processed at the Clinical Pathology laboratory of the Hospital Center (HC). In case of a positive result, the general practitioner communicates the result and refers the individual to the Follow-up and Treatment Center. Description The general objective of this project was to implement the CRCS in the 19 Family Health Units (FHU) of Loures-Odivelas Health Center Group, between September 2022 and April 2023. The following activities were carried out: 1. Sessions to present the patient pathway, record platform, and stool sample collection device; 2. Application of a checklist of essential points for the start of screening; 3. Preparation and dissemination of monitoring reports to the FHU; 4. Follow-up visits to clarify doubts. Results By March 2023, CRCS had been implemented in 15 of the 19 FHU (79%). Between September 2022 and March 2023, 1,430 devices were delivered and 959 FOBT samples were received (67%). Among the devices received with biological samples, it was not possible to issue a result for 5%. Lessons The activities carried out allowed for the initiation of CRCS in most FHU. The involvement of professionals in these activities contributes to increased patient adherence and proper collection technique, reducing inconclusive FOBT results. It is necessary to continue this strategy of personalized support for the FHU to consolidate the implementation of CRCS in LO HCG. Ensuring that the necessary materials are available and that result issuance and referral deadlines are met are important factors for maintaining professional motivation. Key messages • The objective was to implement the Colorectal Cancer Screening Program (CRCS) in 19 Family Health Units (FHU) in Loures-Odivelas. By March 2023, CRCS had been implemented in 15 of 19 FHU (79%). • It is important to continue these activities in order to put into operation the CRCS in all FHU and to tackle the problem of colorectal cancer.

Assessing the Feasibility of a Cancer Biomarker Testing Navigator in the Laboratory

Abstract Introduction/Objective Cancer biomarker testing is a crucial component of precision oncology, as it can guide medical treatment decisions with targeted therapies and immune checkpoint inhibitors. However, complex, fragmented, and inefficient operational processes involved in biomarker testing lead to delays, errors, and variability in testing. One potential solution is to introduce a novel role in the pathology laboratory: the cancer biomarker testing navigator (BTN). Methods/Case Report The American Society for Clinical Pathology (ASCP) conducted a mixed-methods project to assess the current state of biomarker testing operations and explore how a BTN may improve biomarker testing processes. ASCP conducted an online survey, focus groups, and worked with two hospital-based cancer centers on a feasibility pilot. Results (if a Case Study enter NA) Survey and focus group insights reveal several operational challenges around the biomarker testing process. 47 completed the survey and 16 participated in the focus groups. Participants report that biomarker test orders may be incorrect or incomplete. 37% of survey respondents experience delays preparing tissue samples for send-out cancer biomarker testing. 42% of survey respondents are unable to track the processing status of send-out tests. Excessive time is required to coordinate and facilitate send-out tests, especially when multiple reference laboratories are used since each has its own portal, requisition form, specimen requirements, etc. 87% agree they would benefit significantly from having a dedicated person to coordinate and manage cancer biomarker tests. The cancer centers that piloted this role found that they could catch and correct defects such as incorrect test ordered, incorrect specimen types marked on the test order, outdated requisition used, etc. The BTN also found instances where the test report was incomplete or had not been uploaded into the EHR. The BTN role could be justified by evaluating metrics such as task efficiency, workplace satisfaction in the lab, and levels of service satisfaction from oncologists. Patient care can be improved by reducing delays in testing and by reviewing other metrics of quality care delivery. Conclusion The BTN role in the laboratory is a novel solution for ensuring the proper collection and triage of pathology samples for biomarker testing. This role could serve as a liaison between the laboratory and ordering clinicians, provide collection and testing guidance, and help with benchmarking and quality measurement initiatives.

GC-MS quantification of fecal short-chain fatty acids and spectrophotometric detection of indole: Do rectal swabs produce comparable results as stool samples? - A pilot study.

The exploration of the gut microbiome and related metabolites holds an exciting future in health science. The challenges associated with fecal sample testing are proper sample collection, sterile transportation, optimal transport conditions, and processing as all these factors could potentially change the microbiome composition, further exacerbated by the patient's customary discomfort regarding feces samples. The study aimed to compare the usage of rectal swabs and stool samples for short-chain fatty acid estimation using gas chromatography-mass spectrometry (GC-MS) and indole estimation using spectrophotometry. From May 2022 to June 2022, three women were recruited from the Department of Obstetrics and Gynecology (OBG) in a secondary care hospital in coastal Karnataka. During their clinical visit, a rectal swab was collected, and the stool sample was transported to the hospital from the patient's home in sterile containers provided. After the extraction, short-chain fatty acids (acetate, propionate, and butyrate) were quantified using GC-MS. The fecal indole concentration was determined using a hydroxylamine-based assay. The GC-MS analysis failed to detect the concentrations of short-chain fatty acids in rectal swab samples. Indole concentrations in stool and swab samples were significantly different. The study's findings do not support the use of rectal swabs to analyze gut metabolites.

The Effects of Diet, Dietary Supplements, Drugs and Exercise on Physical, Diagnostic Values of Urine Characteristics.

Background/Objectives: This review summarizes the current knowledge about factors that affect the physical characteristics of urine. It highlights proper urine sample collection and displays factors like diet, hydration status, and medications that can alter urine color, odor, clarity, specific gravity and pH. Results: Urinalysis is a minimally invasive examination of a patient's health, especially concerning nephrological and endocrinological abnormalities, as well as dietary habits and stimulants used. Certain deviations in appearance, composition or frequency/pain during urination may indicate an ongoing disease process in the body. Based on laboratory results, further medical treatment is determined. The reason for a change in the color of the urine, for its clouding or intense odor may be a disease, as well as the consumption of food, medication, intensive physical exercise or inadequate hydration of the body. Well-standardized procedures for collecting, transporting, preparing and analyzing samples should become the basis for an effective diagnostic strategy in urinalysis. It is worth noting that pharmacists in pharmaceutical care are often the first people to whom a patient turns for health advice and for the interpretation of simple laboratory tests. Acquiring the ability to interpret the results of laboratory tests and the principles of proper sampling for laboratory tests is indispensable in the process of possible counseling and providing reliable answers to patients' questions. Conclusions: Although urinalysis is not recommended as a routine screening tool for the general population, it can prove to be a valuable source of patient health data in some cases as the data will be useful to physicians and pharmacists to more effectively diagnose and better care for patients.

Assessing the readiness of Greek municipalities to manage bioplastics in municipal waste management streams

Each year citizens of the European Union (EU) generate 2.5 billion tonnes of waste; equating to five tonnes of waste per person per year. To ensure optimal material recovery, and to support the move towards a circular economy, proper separate collection of waste is necessary. The current trend of increasing collection rates, encouraged by EU legislation, is promising; however, progress is uneven both across and within EU member states. While European and national targets provide the overarching driver for better waste collection, regional and local implementation is crucial for achieving continued progress. Therefore, it is vital to consider the disparity among the different EU countries when introducing new materials, such as bioplastics (bio-based and/or biodegradable plastics), into current national waste management streams, and any new introductions should be carefully evaluated before further actions are recommended by local authorities. As a member state that is heavily reliant on tourism, proper solid waste management is of vital importance in Greece, especially during the peak summer season. Considering the rapid expansion of bioplastics in the European market, it is important to assess the readiness of municipalities in Greece to manage this new category of solid waste. Data collected from 51 Greek municipalities via questionnaire (distributed to public authorities or waste management companies) showed that the current situation and capacities for adapting waste management streams to process bio-based and/or biodegradable plastics is still limited in Greece. Therefore, the readiness of Greece to process an increasing proportion of bioplastics within the waste stream (using the current waste management systems) is very low, as a result, such innovative materials are currently seen more as a problem than a ready-to-implement solution.

Bacterial pathogens and antimicrobial susceptibility in ocular infections: A study at Boru-Meda General Hospital, Dessie, Ethiopia

IntroductionThe eye consists of both internal and external compartments. Several variables, including microbes, dust, and high temperatures can cause eye illnesses that can result in blindness. Bacterial eye infections continue to be a major cause of ocular morbidity and blindness, and their prevalence is periodically rising. The objective of the study was to detect bacterial pathogens and assess their susceptibility profiles to antibiotics in the ophthalmology unit of Boru-meda Hospital in Dessie, Ethiopia.MethodsA hospital-based cross-sectional study was conducted from February 1 to April 30, 2021, among 319 study participants with symptomatic ocular or peri-ocular infections who were enrolled using a consecutive sampling technique. After proper specimen collection, the specimen was immediately inoculated with chocolate, blood, and MacConkey agar. After pure colonies were obtained, they were identified using standard microbiological methods. The Kirby Bauer disk diffusion method was used to test antimicrobial susceptibility patterns, based on the guidelines of the Clinical and Laboratory Standards Institute.ResultsThe majority of participants developed conjunctivitis 126 (39.5%), followed by blepharitis 47 (14.73%), and dacryocystitis 45 (14.1%). Overall, 164 (51.4%) participants were culture positive, six (1.9%) participants had mixed bacterial isolates, giving a total of 170 bacterial isolates with an isolation rate of 53.3%. The predominant species was CoNS 47 (27.6%), followed by S. aureus 38 (22.4%) and Moraxella species 32 (18.8%). The overall Multi-Drug Resistance (MDR) rate was 62.9%, with 33 (44.6%) being gram-negative and 74 (77.1%) being gram-positive isolates.ConclusionConjunctivitis was the dominant clinical case and CoNS, was the predominant isolate. A higher rate of MDR isolates, particularly gram-positive ones, was observed. Efficient peri-ocular or ocular bacterial infection surveillance, including microbiological laboratory data, is necessary for monitoring disease trends.

Honey bee larval toxicity study designs: Applicability of the current study protocols and endpoints asa predictor of pesticide hazard for pollinators.

The assessment of pesticide risks to bees in North America currently relies in part on Tier 1 honey bee laboratory toxicity studies to support the registration and registration review processes for crop protection chemicals. For immature stages, the studies follow two standardized test designs recommended by the Organization for Economic Cooperation (OECD), evaluating acute (seven-day single-dose, TG OECD 237) and chronic (22-day repeated-dose, GD OECD 239) toxicity in bee larvae. In this article, we aim to evaluate the current approach for generating and interpreting honey bee larval toxicity data, enhancing pesticide risk assessment for pollinators. First, by considering that the repeated-dose larval study covers all stages of honey bee brood development up to adult emergence, we compared endpoints (larval LD/ED50 and LC/EC50 values) from seven-day acute exposure studies with the 22-day chronic exposure studies. Our goal was to identify the study design offering greater sensitivity in assessing pesticide toxicity to immature bees. Our second objective involved analyzing available weight data from emerged adults and comparing it to survival endpoints (e.g., NOEL and LD50) to determine if the weight after adult emergence would accurately represent a sensitive indicator of pesticide effects on developing honey bees. Our analysis determined that the use of a single 22-day chronic exposure study adequately covers all immature stages and that the toxicity values based on cumulative dose are more accurate and representative measures of exposure for immature bees than using endpoints based on estimated daily doses. Furthermore, our analysis suggests that measuring the weight of emerged adults was a more sensitive indicator than mortality of treatment-related effects in 22% of the compounds included in our analysis. Here we also discuss the importance of standardized protocols for proper collection of weight after emergence and the need for further discussion on the relevance of this parameter at risk assessment scheme. A Descriptive Analysis of Electronic Waste Management among Electronic Repair Vendors in Mwanza City, Tanzania

Electronic waste (e-waste) is a rapidly growing global problem, with Africa generating 2.9 million metric tons of e-waste in 2019. In Tanzania, Mwanza faces a significant amount of e-waste and poor management of e-waste collection and disposal. This study conducted a descriptive analysis of e-waste management practices among electronic repair vendors in Mwanza City, Tanzania. The objective was to describe their current e-waste management practices, identify challenges, and highlight areas for improvement. The lack of awareness and inadequate practices among electronic repair vendors hinder effective e-waste management, contributing to the environmental and health risks associated with the improper disposal of hazardous materials. A literature review revealed the importance of stakeholder engagement, improved awareness, and sustainable e-waste management practices in various African countries. This study employed a descriptive research design, utilizing both quantitative and qualitative methods. Data will be collected through questionnaires, interviews, and observations targeting a sample of electronic repair vendors in Mwanza City. Data analysis involved descriptive statistics and thematic analysis. The findings indicate that using e-waste as a spare part of other products and selling non-repairable e-waste to third parties are current e-waste management practices. Moreover, e-repair vendors have been found to contribute to e-waste management, particularly repair, collection, and storage. Based on these findings, the study recommends that an e-waste awareness campaign be instituted in the e-repair industry, that e-waste management be undertaken sustainably to combat unnecessary disposal, and that the deposition of e-waste and e-waste collection equipment(s) be provided to e-repair vendors for proper e-waste collection and management. Also, there is a need to conduct further studies to assess strategies used by local authorities to support e-repair vendors in e-waste management.

Guidelines of the Brazilian Society of Surgical Oncology for anatomopathological, immunohistochemical, and molecular testing in female tumors.

Precision medicine has revolutionized oncology, providing more personalized diagnosis, treatment, and monitoring for patients with cancer. In the context of female-specific tumors, such as breast, ovarian, endometrial, and cervical cancer, proper tissue collection and handling are essential for obtaining tissue, immunohistochemical (IHC), and molecular data to guide therapeutic decisions. To establish guidelines for the collection and handling of tumor tissue, to enhance the quality of samples for histopathological, IHC, genomic, and molecular analyses. These guidelines are fundamental in informing therapeutic decisions in cancer treatment. The guidelines were developed by a multidisciplinary panel of renowned specialists between June 12, 2013 and February 12, 2024. Initially, the panel deliberated on critical and controversial topics related to conducting precision medicine studies focusing on female tumors. Subsequently, 22 pivotal topics were identified within the framework and assigned to groups. These groups reviewed relevant literature and drafted preliminary recommendations. Following this, the recommendations were reviewed by the coordinators and received unanimous approval. Finally, the groups made the final adjustments, classified the level of evidence, and ranked the recommendations. The collection of surgical samples requires minimum quality standards to enable histopathological, IHC, genomic, and molecular analyses. These analyses provide crucial data for informing therapeutic decisions, significantly impacting potential survival gains for patients with female tumors.

Characterization of the Dominant Stages at Which Gas Flaring Is Introduced: Impacts and Policy Options to Ameliorate Them

Information on associated gas flaring in the Niger Delta is analyzed to characterize the various stages in the lifecycle of an oil exploration and production facility through which gas flaring is introduced into an area, leading to ecological, health, and economic impacts, and possible policies and implementation strategies for the reduction of the flared gas are offered. Gas flaring is currently occurring at a rate of 5318 million m3 per year in the Niger Delta region. The impacts of gas flaring on the ecological, human, and economic systems include poor air quality, acid rain, and soil degradation. Other impacts include decreased crop yield, increased incidences of respiratory diseases amongst inhabitants in the area, and losses of billions of USD yearly in economic costs. Three dominant stages in the introduction of gas flaring are characterized in this study, using the concept of the management of the invasion of non-native species into an ecosystem and it is determined that the cheapest method of minimizing gas flaring is to incorporate mitigation measures during the design stage of the oil exploration/production facility, rather than after flaring has commenced, as is the common emphasis. During the early periods of oil production, gas flaring is largely localized, and the gas utilization measures can still be implemented to good effect. However, once the facility is established, gas flaring becomes pervasive, stretching kilometers beyond the flare points. Current policies like the imposition of flare-out years on the oil producers have helped, but they have been insufficient as the intensity of gas flaring in the region continues at double the average global levels. Working with relevant stakeholders, a realistic flare-out deadline should be agreed upon, clear and specific gas flaring legislation should be drawn up that includes measures to mitigate gas flaring for each proposed oil facility, and proper data collection and management services would enable the implementation of the policies to be based on robust evidence. These policies would shift the costs of gas flaring away from the inhabitants of the region back to the operators of the facilities that are the sources of the flares.

Development of human remains investigations at fatal fire scenes: Bridging anthropology and fire sciences in the United States

AbstractAnthropological assessment of a scene for fatal fire victims can prove crucial in the search, recovery and identification of human remains from fire‐related death investigations. On scene contributions of the anthropologist include, but are not limited to, searching for remains in burnt structures, vehicles, roadside dumps, campfire accidents, and other fire investigations, as well as the proper documentation and collection of remains. While patterning and sequencing for all fire events is not standardized, thermal alteration can be classified and consistent terminology can be used to reflect commonalities and evaluation of thermally altered remains. Further, the anthropologist has the ability to evaluate damage to the remains in terms of possible associated trauma (e.g., antemortem, postmortem). However, scene investigators do not regularly call upon anthropologists to assist at the scene and often do not know the contributions and benefits of having an anthropologist on scene. This perspective article focuses on the development of fatal fire investigations within the United States, key terminology, and core concepts within anthropological sciences and provides recommendations for how fire investigators and anthropologists can collaborate–bridging the gap between the two fields. International variation in injury burden and comparative outcomes: How injury looks like in different parts of the world

Injury is reported as one of the leading causes of morbidity and mortality globally. However, its burden stays under-reported in low- and middle-income countries, which prevents understanding of the true distribution and impact of injury worldwide. This dearth of data not only includes the burden of trauma-associated mortality but also involves the identification of long-term patient-reported outcomes after injury. High-income countries have shown that the development of trauma registries can achieve proper data collection, the findings from which can ultimately decrease trauma-associated morbidity and mortality. Expanding such an initiative to low-resource settings with high injury proportions can aid in understanding the long-term outcomes of post-trauma patients, identifying the factors that continue to contribute to its injury burden, and developing strategies catered to reducing it.Here, we discuss the variation in the global injury burden and go a step beyond mortality to highlight the importance of capturing long-term patient-reported outcomes post-trauma. We describe the process of developing trauma registries in high-income countries and extrapolate the findings from this experience to set up similar registries in resource-limited settings. Lastly, we demonstrate the feasibility and initial impact of using its data on identifying predictors of in-hospital mortality and determining long-term patient-reported outcomes in the context of low-resource settings. Through these findings, we share the outlook of an evolved trauma care system in both high- and low-income regions with reduced morbidity and mortality globally.

"EFFECT OF WEBINAR REGARDING IMPACT OF BIOMEDICAL WASTE ON ENVIRONMENT AND IT’S PREVENTION AMONG NOVICE NURSING STUDENTS OF SELECTED COLLEGES OF ODISHA"

Biomedical waste, sometimes known as hospital trash, is created during the treatment of people as well as during biologics research, that contains potentially contagious materials and affect the environment due to improper disposal of waste. As nurses are backbone of the health care delivery system though they are working 24 hours a day. Novice nursing students have less knowledge on impact of biomedical waste on environment& its prevention. The current Pre-experimental one group pre-test post-test research study conducted in 3 nursing colleges of Odisha with 500 first year nursing students who are selected randomly by using convenience sampling technique. The result of the current study revealed that, majority of nursing students in Post-test (77.33%) are having good knowledge than Pre-test knowledge (3.33%).There was a significant mean difference (7.1) exists between pre-post test knowledge score and 0.1 SD difference exists between the pre and post-test. The present study revealed that, there is lack of adequate knowledge among nursing students regarding management of biomedical waste in view of its impact on the different aspects of the environment. The management of biomedical waste should be assessed as soon as possible in order to raise awareness for proper collection, segregation and disposal to prevent contamination of waste towards environment amongfuture nursing officers at their training courses.

Comparative evaluation of uranium isotope ratios by peak-jumping and static multi-collector measurements in thermal ionization mass spectrometry

The 235U/238U isotope ratios of uranium standard reference materials and an environmental uranium sample recovered from seawater were measured by thermal ionization mass spectrometry using the single-collector peak-jumping and static multi-collection modes. All the 235U/238U isotope ratios measured in the peak-jumping mode were systematically lower than the values obtained in the static measurements. The lowering of the measured isotope ratios became more remarkable with the decrease in the 235U isotopic abundance, resulting in the maximum 2.2‰-lower value comparable to isotope mass fractionation. The lowering of isotope ratios is not due to the apparent loss of 235U ion currents caused by the time constant of the Faraday amplifier used but attributable to the actual decrease of ion currents by ion-beam changes in the mass switching step of peak-jumping mode. Furthermore, the proper collection of ion beams to the Faraday cup was achieved by allowing sufficient delay time for eliminating the effect of ion-beam changes, which was significantly longer than the time needed for the response of the amplifiers.

Viral load in hospitalized infants with respiratory syncytial virus bronchiolitis: a three-way comparative analysis.

Viral load measurement ofRespiratory syncytial virus (RSV) in acute bronchiolitis depends on specimen collection, viral load quantification, and transport media.The aim of this study wasto investigate viral load in three-way-comparative analyses; nasal swab versus nasal wash, quantitative real-time polymerase chain reaction (RT-PCR) versus cell tissue culture, and various transport media.A prospective cohort study of infants aged < 12 months, admitted to the Soroka Medical Center, due to acute bronchiolitis, was conducted. Two nasal swabs and two nasal wash samples (in UTM and VCM) were collected from each infant upon admission and after 48h. Samples were immediately stored at -80 °C and tested at Viroclinics DDL (Rotterdam, Netherlands). Quantitative RT-PCR and quantitative virus culture were performed using tissue culture infective dose (TCID50). Spearman's correlation coefficient test assessed the correlation between the different methods, viral load, and clinical severity score. One hundred samples were collected from 13 infants (mean age 5.7 ± 3.8 months, 46% males). Twelve patients were RSV-A positive, and one was RSV-B positive. A high correlation was found between transport media- UTM and VCM (0.92, P < 0.001) and between nasal swabs and nasal wash samples (0.62, P = 0.02). RSV signals were higher in nasal wash than in swabs. PCR signals were lower in the second collection compared to the first. No correlation was found between viral load and clinical severity. Conclusion: RSV viral load is comparable across nasal wash, nasal swabs, and various transport media. However, it did not correlate with clinical severity, probably due to the limited sample size. Broader analyses are warranted. Determining the rate and reasons for specimen rejection among specimens referred for HIV viral load determination through referral linkage to Debre Tabor Comprehensive Specialized Hospital, north-central, Ethiopia,2023: Retrospective study

BackgroundThe preanalytical phase encompasses the time between the clinician's test order to the sample being ready for analysis. Of all errors during the laboratory diagnostic process,70 % appeared in the pre-analytical phase. In clinical laboratories, it is crucial to ensure proper specimen collection and handling, which is essential to guarantee quality assessment, monitoring process standardization, improving performance, and ensuring patient safety. Despite this importance, no study has been conducted in the study area to investigate the rate and reasons for human immunodeficiency virus viral load sample rejection. ObjectiveTo determine the rate of human immunodeficiency virus viral load sample rejection (number of preanalytical errors) documented during the preanalytical phase and articulate possible causes for specimen rejection. Material and methodsA retrospective study was conducted at Debre Tabor Comprehensive Specialized Hospital from January 1st to January 31, 2023. During the study period, 5950 samples were extracted from the human immunodeficiency virus viral load laboratory sample tracking log books, which were sent to the hospital for viral load testing between August 2021 to November 2022. The collected data were cleaned and entered into EPI data version 4.6 before transferred it to STATA version 14.0 for analysis. Descriptive statistics such as frequencies, percentages, and cross-tabulations were used to summarize the findings. ResultsThe study found that improper sample handling was common during the preanalytical phase. According to the current study, 3.6 % of the sample was rejected at pre analytical stage. The most common reasons for specimen rejection were using inappropriate containers (64.0 %) uncentrifuged specimens (20.4 %); hemolyzed specimens (7.0 %); insufficient specimen volume (6.2 %); clotted specimens (1.9 %); and specimen labeling problems (0.5 %). ConclusionThis study found that the most common preanalytical error was using an inappropriate sample collection container, followed by uncentrifuged samples, Therefore, it is recommended that mentorship programs be developed to educate staff on the preanalytical phase of laboratory testing, specifically on sample collection, storage, and transportation for HIV viral load testing. Additionally, the quality management system of laboratory processes should be strengthened to ensure accuracy and minimize errors.

If it is Possible to be Non-Compliant: Why Should it be Compliant?

This study examines how people’s moral and ethical behavior influences tax compliance. The research involved interviews with four taxpayers in East Nusa Tenggara Province. Taxes play a crucial role in funding public services, and their proper collection is vital for community welfare. The study suggests that more than a robust tax system is needed; incorporating moral and cultural values is essential to encourage compliance. The findings reveal that taxpayers adhere to tax obligations not just because of the system but also due to a sense of responsibility for their business sustainability.

Multiple transcriptome analyses reveal mouse testis developmental dynamics

The testes are the organs of gamete production and testosterone synthesis. Up to date, no model system is available for mammalian testicular development, and only few studies have characterized the mouse testis transcriptome from no more than three postnatal ages. To describe the transcriptome landscape of the developing mouse testis and identify the potential molecular mechanisms underlying testis maturation, we examined multiple RNA-seq data of mouse testes from 3-week-old (puberty) to 11-week-old (adult). Sperm cells appeared as expected in 5-week-old mouse testis, suggesting the proper sample collection. The principal components analysis revealed the genes from 3w to 4w clustered away from other timepoints, indicating they may be the important nodes for testicular development. The pairwise comparisons at two adjacent timepoints identified 7,612 differentially expressed genes (DEGs), resulting in 58 unique mRNA expression patterns. Enrichment analysis identified functions in tissue morphogenesis (3-4w), regulation of peptidase activity (4-5w), spermatogenesis (7-8w), and antigen processing (10-11w), suggesting distinct functions in different developmental periods. 50 hub genes and 10 gene cluster modules were identified in the testis maturation process by protein-protein interaction (PPI) network analysis, and the miRNA-lncRNA-mRNA, miRNA-circRNA-mRNA and miRNA-circRNA-lncRNA-mRNA competing endogenous RNA (ceRNA) networks were constructed. The results suggest that testis maturation is a complex developmental process modulated by various molecules, and that some potential RNA-RNA interactions may be involved in specific developmental stages. In summary, this study provides an update on the molecular basis of testis development, which may help to understand the molecular mechanisms of mouse testis development and provide guidance for mouse reproduction.
