One type of work accident that often occurs is road transport accidents. The Ministry of Transportation shows a significant increase in the number of road transport accident victims from year to year. In 2022, the number of victims will reach 204,447 people, an increase of 33% compared to the previous year. During this period, 163,686 people recorded accident victims with minor injuries, 27,531 people died, and 12,230 people with serious injuries. This final assignment reviews identifying hazards and controlling the risk of work accidents in the crude oil distribution process at PT Sangkakala Niaga Energi in 2024. This research aims to identify potential hazards, analyze risk factors, and outline risk control measures. This type of research uses qualitative data research methods obtained from interviews, observations, and document analysis. From the results of the observation sheet, you can get an overview of the crude oil distribution process with 9 indicators and 73 statement items. The results of the observations identified 22 hazards and potential events in the crude oil distribution process. The Frequency Level (Possibility) and Severity Level (consequences) have various levels. There are 14 planned control actions that have been implemented, while there are 8 actions that do not include control actions. Researchers suggest that companies provide regular K3 training and disseminate knowledge about PPE in crude oil distribution work and provide sanctions for workers who violate company operational standards (SOP), can equip workers' facilities and provide action on hazards that have not received control measures so that safety programs and occupational health is going well

As vital elements of company assets, employees played a significant role in contributing to the company to achieve optimal performance and effectively compete. Evaluation of employee performance was conducted through various aspects, such as quality, quantity, time efficiency, and cooperation in achieving the goals set by the company. Employee performance was not only an indicator of the company's success in achieving its goals but also a crucial factor in the overall organizational objectives. In maintaining employee safety, the company needed to pay attention to various factors, including one of them being the Occupational Health and Safety (OHS) program at PT Semen Baturaja Tbk, with the main goal of ensuring and enhancing the level of workplace safety for employees. This study used interviews, documentation, and library research with descriptive quantitative analysis. The results showed a strong commitment from the company in maintaining safety through the implementation of the OHS program, which included policies, risk evaluations, safety guarantees, and safety promotions. Employees responded positively to the safety culture reinforced by effective training and communication. Although successful, the evaluation identified challenges in implementation, such as variations in awareness, resistance to change, and the need for improved communication. This study recommended enhancements in training, communication, and supervision to strengthen the OHS program and ensure sustainable workplace safety

A preparation program for psychological safety of hospitalized adolescents

IntroductionThe study aimed to investigate the impact of an information-based preparation program on the psychological safety of adolescents admitted to pediatric wards, emphasizing the importance of enhancing patient safety.MethodsThis quasi-experimental study was conducted among 98 adolescents admitted to pediatric wards at Namazi Hospital, managed by Shiraz University of Medical Sciences, in 2021. The participants were randomly assigned to either an intervention group or a control group using an electronic randomization table. The intervention group received an information-based preparation program, while the control group followed routine care. Adolescents completed the Psychological Safety Questionnaire after admission and at discharge. Data were analyzed using SPSS (Version 22), with a significance level of 0.05.ResultsThe mean psychological safety scores before the intervention were 136.73 ± 17.30 in the control group and 141.03 ± 16.34 in the intervention group, with no significant difference between the two groups (p = 0.20). After the intervention, the mean scores were 136.65 ± 19.01 in the control group and 145.50 ± 14.05 in the intervention group. A comparison of the mean psychological safety scores showed a significant difference between the two groups after the intervention (p = 0.01).ConclusionsThe findings of this study indicate that the information-based preparation program positively affected the psychological safety of hospitalized adolescents. Therefore, it is recommended that nurses incorporate this method into therapeutic programs for hospitalized adolescents to enhance their psychological safety effectively.

Determinant Factors of Patient Satisfaction in the Orthopedic Surgery Ward

Background: Patient satisfaction is a crucial measure of health service performance. The fluctuations in this level of accomplishment can serve as empirical evidence for developing quality and patient safety programs. This study seeks to assess and analyse demographic characteristics, the degree of patient satisfaction, and the relationship between respondent characteristics and satisfaction at three orthopedic surgery wards over a span of three time periods: 2021 to 2023. Methods: This study employs a quantitative research methodology adopting a descriptive comparative technique. The independent variables of this study were time period, quality dimensions, and patient characteristics. The dependent variable of this study was patient satisfaction. The data were examined using Kruskal-Wallis’s analysis, the central tendency, and multiple regression analysis. Results: The study showed that the level of patient satisfaction has increased every year (p-value = 0.005), with no significant difference between the three wards (p-value = 0.893). Also, there is no significant correlation with patient satisfaction for each year (p-value > 0.05). Nevertheless, the findings of this study indicate that age significantly influences patient satisfaction in 2021 (p-value = 0.021) and 2023 (p-value = 0.007). Conclusion: It is recommended this research be expanded by exploring other patient characteristic variables and exploring the key elements that significantly influence patient satisfaction in many patient care settings.

Artificial intelligence related safety issues associated with FDA medical device reports

The Biden 2023 Artificial Intelligence (AI) Executive Order calls for the creation of a patient safety program. Patient safety reports are a natural starting point for identifying issues. We examined the feasibility of this approach by analyzing reports associated with AI/Machine Learning (ML)-enabled medical devices. Of the 429 reports reviewed, 108 (25.2%) were potentially AI/ML related, with 148 (34.5%) containing insufficient information to determine an AI/ML contribution. A more comprehensive approach is needed.

Remarks on the origin of almost-Gaussian scaling in asymptotically safe quantum gravity

The gravitational asymptotic safety program envisions the high-energy completion of gravity through an interacting renormalization group fixed point, the Reuter fixed point. The predictive power of the construction is encoded in the spectrum of the stability matrix which is obtained from linearizing the renormalization group flow around this fixed point. A key result of the asymptotic safety program is that parts of this spectrum exhibit an almost-Gaussian scaling behavior, entailing that operators which are classically highly UV irrelevant do not induce new free parameters. In this article, we track down the origin of this property by contrasting the structure of the stability matrix computed from the Wetterich equation and the composite-operator equation within the realm of f(R) truncations. We show that the almost-Gaussian scaling is not linked to the classical part of the beta functions. It is a quantum-induced almost-Gaussian scaling originating from the quantum corrections in the flow equation. It relies on a subtle interplay among the analytic structure of the theory’s two-point function and the way the Wetterich equation integrates out fluctuation modes. As a byproduct we determine the parts of the eigenmode spectrum that is robust with respect to changing the regularization procedure. Published by the American Physical Society 2024

Possibilities for an internationalised, inclusive pedagogy for transnational sport education: the case of a safe sport education programme in Lithuania

ABSTRACT Global education is a priority in the Higher Education (HE) system of all nations. In sport degree programmes in HE, the internationalisation trend is also noticeable. However, working in transnational education settings may bring to light, and generate existential and pedagogical contemplation of some of the foundational principles and assumptions about effective teaching. This paper aims to extend theoretical understanding of the confluence of internationalisation and educational modalities to fostering critical thinking. And, illustrate how building language-sensitivities in variable delivery may yield effects upon positive classroom experiences and establishing a ‘safe’ space for communication and learner-informed/learner-led dialogue. Based on a case study of a safe sport education programme for university student-athletes in Lithuania, this paper features a narrative that amalgamates: (1) descriptions of the practical context, session structure and delivery; (2) reflections on the sessions from the lead author; and, (3) focus group data from student participants. The case presented reveals that irrespective of an educator’s efforts to acculturate to the distinct context, continued uncertainties regarding the teaching environment, language and cultural differences, and learners’ understandings of subject content remained perennial concerns. Such issues necessitated adopting careful session design that incorporated a breadth of methods to make students’ experiences more meaningful and inclusive. Key here was the consideration of language and conceptual assumptions (e.g. vis-a-vis ‘safe’ sport), and wider Western-hegemonic frameworks of what sport is/is not. The analysis illustrates how efforts to consider language, use student-co-creation and blended learning can be effective in generating a comfortable space for interaction, communication and learning. Thus, this paper contributes to continuing debates on transnational education, internationalisation in HE and roles sport content may play in generating global dialogue on critical contemporary issues.

EFFECTS OF A WHOLE-BODY VIBRATION EXERCISE PROGRAM ON PHYSICAL PERFORMANCE AND POSTURAL BALANCE OF OLDER ADULTS

With the burgeoning elderly population, there is an escalating demand for effective and safe exercise programs to enhance physical performance and avert age-related complications. This study aimed to evaluate the effects of a whole-body vibration (WBV) exercise program on the physical performance and postural balance of older adults. The study enlisted 21 participants aged 65 and older, divided into three groups: WBV, foam mat (FM), and control. Over four weeks, participants engaged in a structured exercise regimen designed to improve lower limb muscle strength, cardiovascular endurance, flexibility, coordination, and postural balance. The low-impact, elderly-friendly program included resistance exercises tailored to each group’s conditions. Pre- and post-program evaluations comprised physical performance tests (chair stand, 2[Formula: see text]-min step, chair sit-and-reach, figure-of-8 walk) and postural balance assessments (limit of stability (LOS), Timed Up and Go (TUG), One Leg Stance (OLS)). Results showed significant improvements across all groups, with the WBV group exhibiting the most pronounced enhancements. WBV group improvements included a 62.8% increase in lower-limb muscle strength and a 31.7% rise in cardiovascular endurance. The WBV group also demonstrated the greatest advancements in postural stability and dynamic balance, as indicated by significant changes in OLS and TUG test times. These findings suggest WBV exercises can effectively enhance physical performance and balance in older adults, presenting a promising alternative to traditional exercise programs that may impose greater physical burdens. Further research with larger sample sizes and controlled variables is recommended to confirm these benefits and refine WBV exercise protocols.

The Safety, Feasibility, and Efficacy of a Structured Individual Exercise Program for Community-Dwelling Stroke Patients.

Stroke is a leading cause of mortality and long-term disability worldwide, often resulting in low levels of physical activity post-discharge. A comprehensive exercise program could be a safe method to increase stroke patients' physical activity and exercise volume. This pilot study examines the effectiveness, safety, and feasibility of a comprehensive exercise regimen for stroke patients residing in the community after discharge. This single-group pilot study included patients who received inpatient rehabilitation for motor impairment following a stroke. Community-dwelling participants engaged in a comprehensive exercise program, which was structured and individualized, consisting of stretching, aerobic exercise, strength training, and balance exercises, conducted under the supervision of a health exercise specialist in a dedicated living lab within the hospital. The program involved 20 sessions, each lasting 50 min, conducted three to five times weekly. The pre- and post-intervention evaluations were comprehensive, including safety and validity assessments, along with physical function tests such as cardiopulmonary exercise testing and various fitness tests (6 min walk test, timed up and go test, chair stand test, figure-of-8 walk test, grip strength test, and sit-and-reach test). Depression levels were assessed using the Geriatric Depression Scale (GDS). A total of 22 subjects were recruited, with 21 completing the study after one dropout due to fatigue and visiting distance. Among the 21 completers, one subject experienced a mild adverse event (hypoglycemia), which was not serious, confirming the program's safety. The exercise participation rate was 95.5%, and the adherence rate averaged 95.2%, with all completers achieving over 80% adherence. Significant improvements were observed in VO2 peak (p = 0.000), 6 min walk test (p = 0.000), timed up and go test (p = 0.000), chair stand test (p = 0.000), figure-of-8 walk test (p = 0.000), and grip strength (p = 0.001). However, changes in the peak respiratory exchange ratio (p = 0.078), sit-and-reach test (p = 0.228) and GDS scores (p = 0.275) were not significant. This study demonstrates that the comprehensive exercise program is sufficiently safe and valid for stroke rehabilitation patients. The program significantly enhances cardiopulmonary endurance, walking ability, balance, muscle strength, muscle endurance, and coordination. Further research with larger sample sizes and control groups is needed to confirm these findings and explore additional benefits.

Evaluation of the safety of T-DXd in the Treatment of HER2-positive breast cancer

Abstract. Breast cancer (BC) is one of the most common diseases in women today, and HER2-positive BC has attracted the attention of researchers. Anti-her2 targeted therapy is an important tool in the treatment of HER2. In recent years, antibody-coupling drugs have entered people's field of vision and triggered a boom, and the research on T-DXd targeted therapy for HER2-positive BC is also gradually advancing and improving. This paper mainly discusses the mechanism of HER2-positive BC, explores the mechanism of action of T-DXd against HER2-positive BC, analyzes the side effects and safety evaluation of T-DXd targeted therapy for adult female patients with HER2-positive BC, and concludes that T-DXd is a relatively safe drug. However, it can be found that the pulmonary interstitial disease after treatment is a serious side effect that should not be ignored. Future studies can focus on discussing the principle of the side effect, ways to reduce the probability of its occurrence and more accurate and safe medication programs.

Process safety in bioenergy with carbon capture and storage systems (BECCS)

AbstractIn response to the climate crisis, the United States has embarked on an ambitious program to achieve 100% carbon‐free electricity generation by 2035 and net‐zero greenhouse gas emissions by 2050. The implementation of bioenergy with carbon capture and storage (BECCS) systems is an essential component of that strategy. BECCS is broadly defined as the utilization of biomass energy (from the processing of solids, liquids, or vapors) with the capture of carbon dioxide and subsequent permanent storage in a deep geological formation. There are numerous potential technologies and flowsheets for implementing BECCS, and the supply chains rely upon support from the agricultural, forestry, and solid waste industries. Inherent in BECCS systems are the hazards associated with combustible dusts, spontaneous ignition and smoldering of combustible solids, flammable liquids, flammable vapors and gases, toxic gases, and more. For BECCS to be deployed commercially across the United States, it is imperative that process safety risks are controlled. A risk‐based process safety (RBPS) program can help manage the risks of a BECCS facility and minimize process safety incidents. In this paper, we present two representative bioenergy technologies as mini‐case studies to illustrate the range of process hazards encountered. Process safety strategies required by regulation are briefly reviewed and potential gaps are identified. We then demonstrate how RBPS can be implemented in a practical and effective manner to fill the gaps.

Promoting Workplace Health, Safety, and Well-Being Among Essential Agricultural Workers Through Vaccine-Preventable Infectious Diseases Training in the Rio Grande Valley

ABSTRACT Introduction Health, safety, and well-being training programs provide essential education on anticipating, identifying, and mitigating exposures like infectious diseases. Gaps in infectious diseases awareness and education became especially apparent during the COVID-19 pandemic and subsequently were exacerbated by mis- and disinformation. Methods Vaccine-preventable infectious diseases training (influenza, hepatitis A and B, and tetanus infections, including COVID-19) was developed, delivered, and evaluated among 1,043 farmworkers, bodega workers, and production management in the Rio Grande Valley using mobile-learning technologies. The Kirkpatrick Four-Level Training Evaluation Model was utilized to evaluate training satisfaction (Level 1), effectiveness (Level 2), and effect on behavior (Level 3). Results The mean score on the pretest before training was 67.8% (SD 17.5), and the mean score on the same test immediately after the video training was 77.2% (SD 17.9). A paired t-test revealed that knowledge improved significantly from pre- to post-training (p < .05). We observed a difference between mean pre- and post-test scores relative to the pooled standard deviation, resulting in an effect size estimate of 0.53 indicative of a medium learning effect. Conclusion There is no “silver bullet” for training migrating bodega and farmworkers. Our findings suggest that the utilization of m-learning techniques continues to be a successful mechanism for delivering health, safety, and well-being awareness training content to agricultural workers in remote and challenging work environments. There is a long overdue need for offline capable software with features that allow equitable access to training, even in remote farming regions.

Genomic Surveillance of Foodborne Pathogens: Advances and Obstacles

Context: The genomic surveillance of foodborne pathogens in the United States has grown exponentially in the past decade, grounded in a powerful combination of novel sequencing technologies, bioinformatic approaches, data-sharing networks, and metadata harmonization efforts. This practice report examines recent advances in genomic epidemiology as applied to food safety programs and delineates State, Tribal, Local, and Territorial infrastructure necessary for continued life-saving improvements in public health. Program: National databases of foodborne pathogen genomes, along with data sharing and evaluation networks such as GenomeTrakr and PulseNet, have transformed how connections are made among isolates and how root causes of outbreaks are determined, allowing much more timely interventions to protect public health. Freely available bioinformatics tools such as GalaxyTrakr and the National Center for Biotechnology Information Pathogen Detection database have allowed laboratories with limited local computing resources to participate in surveillance efforts and contribute to traceback investigations. Implementation: In this report, we describe advances in genomic epidemiology that have occurred over the past decade and examine obstacles to fully implementing this technology within State, Tribal, Local, and Territorial public health systems. Evaluation: Despite a clear return on investment from governmental expenditures on genomic surveillance of foodborne pathogens, we identify significant obstacles to further sustained progress. These obstacles include workforce gaps, ineffective data sharing, and lack of constitutive and sustained funding. Discussion: Many public health laboratories face major obstacles to widespread and routine adoption of genomic surveillance technologies. While whole genome sequencing has become an integral part of routine public health microbiology, the seamless integration of these protocols into the existing practices, laboratory workflows, and information systems remains challenging. Centralized efforts to address these issues include (1) support through the Food and Drug Administration Laboratory Flexible Funding Model, (2) training and proficiency assessments, (3) open-source, standardized protocols for collecting high-quality genomic data, and (4) open access informatics software.

The implementation of an occupational health and safety (K3) program can be an effort for mining companies to make efforts to prevent accidents in mining work activities such as coal hauling. Based on this, the purpose of this study was to analyze the implementation of K3 (occupational health and safety) on coal hauling activities by PT. XYZ with HAZOP and HIRARC methods. The research was carried out using data collection methods such as interviews, observations, and document studies. Furthermore, the data were analyzed using the HAZOP and HIRARC methods, which are data analysis methods that aim to determine the potential hazards that exist in mining work activities. The results showed that through the HAZOP analysis showed that there were 12 sources of danger in 5 work activities in the Coal hauling section. These 12 hazard sources are divided into 4 hazard categories with 56% high risk, 24% moderate risk, 15% extreme conditions, and 5% low risk. Based on this, it can be seen that the potential hazard by causing work accidents is quite significant. Meanwhile, the HIRARC analysis shows that there are 6 sources of danger with 9 potential work accidents which are divided into 4 categories of hazards as much as 51% high risk 4 potential risks, 21% extreme risk with 1 high value potential risk, 18% low risk with 3 potential risks, and 10% medium risk with 1 potential hazard. This potential and source of danger has and will continue to allow for the occurrence of losses, both material and physical injury to employees.

First Nations Peoples' perspectives on telehealth physiotherapy: a qualitative study focused on the therapeutic relationship.

Relationships are the core of Indigenous Peoples' spiritual and cultural identities, and therapeutic relationships are an integral part of the physical rehabilitation process, directly influencing health outcomes. However, participating in therapeutic relationships can be difficult for First Nations Peoples, particularly in the virtual landscape. There is limited understanding of First Nations Peoples' perspectives on this issue, and this understanding is crucial to developing culturally safe and effective telehealth physiotherapy programs. Therefore, the purpose of this study is to explore the perspectives of First Nations Peoples from British Columbia, Canada, on telehealth physiotherapy, with an emphasis on the virtual therapeutic relationship. A narrative qualitative study that utilized one-on-one, semistructured interviews was conducted with 19 First Nations adults from remote and rural First Nations communities in north-central British Columbia, Canada. Interviews were recorded, transcribed verbatim, and analyzed using an inductive approach to reflexive thematic analysis. Three themes emerged from data analysis. 'Therapist's attitude and rapport' captures participants' perceptions of what matters the most in how physiotherapists relate to First Nations Peoples and carry out their work. 'Remote nature of virtual care' encompasses the main challenges of virtual care visits, particularly how these were perceived to impact establishing and maintaining solid therapeutic relationships. 'Fostering culturally appropriate and safe telehealth physiotherapy' focuses on what is needed to advance telehealth physiotherapy in a manner that respects and reflects First Nations cultures, equipping all involved parties to provide comprehensive and sensitive services. Our findings advocate a hybrid model that combines in-person and telehealth visits to address communication barriers and the absence of physical interaction. Bridging the digital health literacy gap through training and collaboration with local support staff is crucial (as it is to bridge the possible cultural literary gap of therapists), and the incorporation of cultural elements holds promise for enhancing the engagement and effectiveness of telehealth services in these communities. The pursuit of equitable health care for First Nations communities demands not only increased access but also a thoughtful, culturally safe, trauma-informed, and holistic approach. This approach must be tailored to the unique needs of First Nations Peoples, emphasizing the integration of cultural elements and community support. A hybrid model combining in-person and telehealth visits is recommended to address logistical challenges and enhance the therapeutic relationship, ensuring that care is both effective and respectful of cultural values and practices.

Domains of four-step technique training program for laparoscopic colorectal surgery.

Many surgeons have begun learning about colorectal surgery using laparoscopy rather than laparotomy. The domains of four-step technique training program (DOF) for laparoscopic colorectal surgery have been designed and implemented by our institute since 2011, and they are expected to provide a safe and effective program for trainees with limited experience in laparoscopic colorectal surgery. The DOF were established with standard surgical procedures, a four-step technique, and learning passports using point credits after the procedure was completed. Patients who underwent minimally invasive colorectal resection at the Chang Gung Memorial Hospital between January 2013 and April 2019 were enrolled. Overall, 2604 and 478 patients were enrolled in the non-training and training groups, respectively. Multivariable logistic regression analysis revealed that the postoperative short-term outcomes were not significantly associated with the training or non-training groups. However, the training group had a significantly longer operative time than the non-training group in the linear regression model. Once the trainee passed the proficiency point (passed 100 points or 30 cases), no significant difference in postoperative short-term outcomes was found between the patients in the non-training and training groups that underwent the entire surgery performed by the trainee. When patients' safety was evaluated in the training cases under the DOF, no higher rates of postoperative morbidity and mortality were found compared to those in cases performed by experienced surgeons. Additionally, trainees who passed the proficiency point during DOF could safely perform the entire laparoscopic colorectal surgery under supervision without further adverse effects on the patients.

Risk Factors for Hearing Loss in the Railway Transportation Sector: Literature Review

Introduction: Hearing loss, a significant global health problem, can significantly impact quality of life. The railway sector, with its high noise levels, is particularly vulnerable. To address this, it is crucial to focus on mitigating risk factors through noise control, PPE promotion, and regular hearing assessments. Objective: The study aims to identify factors related to hearing loss in railway sectors. By understanding these factors, the research seeks to contribute to improved workplace safety and hearing conservation programs. Method: This study employed a literature review methodology, utilizing sources from Scopus and Google Scholar. Articles were searched using the keywords "hearing loss," "NIHL," "hearing impairment," "railway transport," "railway industry," and "train transportation" published within the last 10 years from 2014 to 2024. Result: The result of the article search revealed 10 articels that match the criteria set. The result show that various factors were identified that can increase the risk of hearing loss in the railway sector. Overall, risk factors for hearing loss among workers include duration of noise exposure, age, sex, individual habits, and the use of personal protective equipment (PPE). Conclusion: The need for comprehensive hearing conservation programs in the railway sector that address not only noise control, but also individual risk factors and the effective use of personal protective equipment. Further research is needed to investigate the long-term effects of combined noise and vibration exposure and the potential benefits of advanced audiometric techniques in the early detection and prevention of hearing loss.
