Realizing Digital Signatures for Medical Imaging and Reporting in a PACS Environment

According to Taiwan's legislation pertaining to the protection of electronic data, the creators of electronic medical records (EMR) are solely responsible for the security of EMR. However, actual implementations that fulfill the security standards and requirements for electronic medical record systems are still lacking. Most EMR created from picture archive and communication system are not considered secure, as security protection mechanisms have not yet been granted legal status. This paper describes the details of establishing a digital signature system using Taiwan health professional cards. A digital signature system has been included to ensure quality assurance (QA) operations are controlled by technicians, and reporting capabilities have been provided for radiologist. Six imaging modalities and eight types of radiology reports have also been included in the system. Results indicate that the process of creating QA signatures does not have an adverse effect on the workflow of the facility, requiring less time for the signing and verification of radiology reports. This system has already been used routinely online in a real clinical setting for more than 2 years.

Teen Ambassador Leadership Kit (TALK)

Teen Ambassador Leadership Kit, (TALK), is an annual weekend retreat designed for teens interested in promoting and marketing 4-H in their communities. TALK organizers felt teens would benefit from an intensive weekend retreat focused on communication. TALK produces a network of educated and excited 4-H teens that are available to help with 4-H promotion and marketing. Participants arrive on Friday evening for team building activities, on Saturday they participate in one of the workshops offered and on Sunday morning each workshop group has the opportunity to share their completed projects and what they learned. At the conclusion of the retreat, teens are designated "County 4-H Ambassadors" and certificates of completion, professional business cards and polo shirts are presented. The TALK teen participants return home to share what they learned with their local county 4-H staff and help promote and market 4-H in their communities.

Risk evaluation and security analysis of the clinical area within the German electronic health information system

Germany is currently introducing a nationwide health information infrastructure. This infrastructure connects existing information systems of various service providers and health insurances via a common network. An essential step towards the implementation of this system will be the introduction of an electronic health care smart card (eHC) for patients and a counterpart health professional card (HPC) for care providers. This article provides a risk analysis on the handling of these cards by both patients and physicians from an organizational point of view. On the basis of the information security audit methodology of the Federal Office for Information Security (BSI), the currect security status of German healthcare telematics on the clinical side is evaluated. For this purpose, an appropriate framework specifically designed for the clinical area is first developed and explained in detail. Based on these perceptions it is possible to precisely check the workflows “patient admission”, “accessing emergency data” and “prescription of medicine” for inherent organizational threats. As a result, we proposed appropriate steps to mitigate potential risks and derived valuable hints for future process re-eingineering by the introduction of the new smart cards in hospitals. The electronic locum record for general practitioners: Outcome of an evaluation study in the Netherlands

Background A locum practitioner is an out-of-hours general practitioner who needs access to the electronic health record of visiting patients. The electronic locum record is a summary of the electronic health record available to the locum practitioner and includes the most significant health problems, the most recent records of the patient's visits to the practice, current medication data and information on allergies and intolerances. The locum practitioner returns a locum medical note to the electronic health record at the general practitioner with his or her diagnosis, treatment or referral of the visiting patient. A pilot project of the electronic locum record was implemented in the Twente region of the Netherlands. Objective To obtain policy information for the nationwide implementation of the electronic locum record as a first component of the electronic health record in the Netherlands. Methods First, evaluation aspects were collected from parties involved in the pilot implementation process. Aspects were taken from the work flow to operationalise the electronic health record. Secondly, indicator questions were formulated and normative levels agreed for each indicator by a panel with experts from the medical and information technology domain. Third, the actual values were rated either by measurement (technical indicators) or by structured interviews (process indicators) with the general practitioners who joined the pilot study. Finally, a cross case analysis was performed by checking for (in-)consistencies among the respondents. Results Eight out of the 15 key indicators scored positive, three failed and four remained inconclusive. The indicators that failed the norms related to the guideline for electronic registration of patient information, the process of acquiring the healthcare professional identification card and card-related services. Indicators that remained inconclusive referred to storing and archiving of identification cards and codes, the use of the identification repository at the general practice post and the usefulness and ease-of-use of the electronic locum record. Conclusions The study demonstrates that many processes, systems, services and practices for nation-wide implementation of the electronic locum record for general practitioners in the Netherlands are in place. However, significant improvements are required on a number of aspects. For example general practitioners need to be trained more in applying the guideline for electronic registration of patient data. Also the process for general practitioners to acquire their unique healthcare professional identification cards should require less effort. We also recommend that a strong regional information technology support group should be in operation during roll-out.

A theoretical analysis of a buffer frame size conversion algorithm for audio applications ensuring minimum latency

In implementing digital signal processing (DSP) algorithms for audio real-time applications, one is frequently faced with problems regarding incompatibilities between the hardware buffer length (the internal buffer of a professional sound card) and the software buffer size imposed by the underlying algorithm (due to i.e. multirate or FFT constraints). This mismatch is solved by proper frame size conversion algorithms which inevitably introduce delay. In this context, this paper presents a buffering scheme together with a theoretical proof of the minimum delay property shown by it. Some examples derived from frequently encountered issues in DSP applications are reported.

Recording and online analysis of auditory steady state responses (ASSR) in Matlab

Auditory steady state responses (ASSR) are a current research focus because of their potential use as a diagnostic tool. Research platforms are required to test user defined stimuli and algorithms for the analysis of electrophysiologic responses. Commercially available ASSR devices are not adequately flexible. To enable a larger group of scientists to pursue ASSR research, we introduce a cost-efficient and flexible ASSR setup. ASSR recording and online analysis software in Matlab (The Mathworks, Inc.) was developed for a standard PC equipped with an external professional sound card, audiometric headphones, and an EEG biosignal preamplifier.

Ethical implications faced by teachers supervising the clinical practice of students of infirmary

During clinical practice the teaching nurse is doomed to act in situations that involve making ethical decisions in front of the patient and be responsible for the consequences of this decision. The objective of the research was to determine the more frequent implications in which the infirmary professional is involved in hospitals, while supervising the student practice in the city of Bogota. Research on this topic is scars, taking into account that this subject becomes, every time, more and more important in the professional exercise. As a tool for data collecting, a semi-structured interview was designed and recordings of experiences by the participant professionals were made. The results determined that the implications most frequently faces by the teacher in the places of practice are of ethical type; no penal denunciation, before the court of Ethics of Infirmary, followed by a sanction or the dismissal, withdrawal of the professional card or indemnification of prejudices was presented. The situations confronted by students with the care subject generate experience in moral reasoning and in the importance of values for the taking of ethical decisions. The necessity to establish a continuous communication within the institutions, where the practice is realized, will allow a proper coordination of the attention of the care subject and a consensus in the decision making, when required by the situation.

A Central Architecture and Solution for Health Smart Cards in Hospitals – The IQ-Medi-LOG Product as Alternative to gematik Concepts

The mandatory introduction of Health Professional Cards (HPC) in the German health care system induces major challenges for IT infrastructures as well as organisational structures in hospitals. This paper presents a new approach for integrating HPCs in hospital processes and infrastructures that is realized in the product IQ-Medi-LOG. The objective is to leverage efficiency and effectiveness potentials associated with the electronic health cards and to enable entirely new services in hospitals. Using a conceptual comparison the authors are able to show the advantages of this concept. In closing they outline further areas of application and future development trends.

Analizing the scandal taking place in the summer of 1840 in Balatonfüred, one of the famous thermal bath sites in Hungary at the time, the essay describes the underworld of a big town. József Román, a solicitor, made a complaint in 1843 about a 'solicitor', Dániel Prodanovits, in reality a criminal, who hit him because of not settling a card play debt. Zala county had the case investigated. During the course of investigation it became evident that it was not only aristocrats, wealthy noble people and citizens who showed up at the site, but prostitutes, marrige swindlers, card sharpers, waiters from notorious places of amusement as well. The bustling and lively town attracted adventurers. Card sharpers from Pest got József Román to play cards and collaborating they took him into debts. The investigation made it clear that Dániel Prodanovits and his friends, as professional card sharpers, lived at thermal baths in summer and Pest at the Wurm caffee in winter. József Román was not innocent either. He violated the law many times as a leaseholder. He misused his authority getting involded in bribery..Preserved in the Zala County Archives, documents relating to matters under dispute reveal one of the aspects bath life in Balatonfüred.

Such comparison motivates sorting across market segments and reshapes the whole marketplace. The Internet provides an excellent example: it introduces substantial search cost savings over brick and mortar retail stores but imposes new obstacles for sellers to convey quality. Using sports card trading as a case study, we provide empirical evidence on (1) the sorting of product quality between the online and offline segments, (2) the changes for retail outlets after the Internet came into place, and (3) how supporting industries such as professional grading and card manufacturing adapted to take advantage of the new market. Design and implementation of a smart card based healthcare information system

Smart cards are used in information technologies as portable integrated devices with data storage and data processing capabilities. As in other fields, smart card use in health systems became popular due to their increased capacity and performance. Their efficient use with easy and fast data access facilities leads to implementation particularly widespread in security systems. In this paper, a smart card based healthcare information system is developed. The system uses smart card for personal identification and transfer of health data and provides data communication via a distributed protocol which is particularly developed for this study. Two smart card software modules are implemented that run on patient and healthcare professional smart cards, respectively. In addition to personal information, general health information about the patient is also loaded to patient smart card. Health care providers use their own smart cards to be authenticated on the system and to access data on patient cards. Encryption keys and digital signature keys stored on smart cards of the system are used for secure and authenticated data communication between clients and database servers over distributed object protocol. System is developed on Java platform by using object oriented architecture and design patterns.

This article presents an overview of existing currently active or planned model projects in healthcare-telematics in Germany. To this purpose a couple of test regions have been established in order to prepare the introduction and implementation of smart cards (patient cards, healthcare professional cards) in the German healthcare system. At its end, the article does also include a table presenting an overview of web pages where further information with respect to these projects can be found.

Use of an infection control pocket reference card for healthcare providers in a university-based medical center

ISSUE: There is a great need for continual education and reinforcement of infection control (IC) practices that is rapid and easily accessible. This is particularly true in large medical centers where the number and turnover of house staff and employees is substantial. These issues contributed to the need for an easy, point-of-care reference card for isolation categories and IC policies. Enhanced collaboration between the healthcare providers and the IC team is also needed. PROJECT: Pocket-sized reference cards focused on IC topics were developed. Two versions were created, the first one aimed at physicians and the second at other frontline healthcare professionals, particularly nursing staff. Both versions of the card contain descriptions of the Centers for Disease Control and Prevention (CDC) isolation categories including organism-specific guidelines, the manner in which isolation may be discontinued, certain protocols, important reminders, and IC contact information. The physician card contained additional detailed information regarding Tuberculosis isolation. The cards were distributed at staff orientation, during formal educational lectures, and at the point of care by the IC team. The intent of the cards was to aid the healthcare provider in making accurate and timely decisions regarding IC at the point of care. RESULTS: To date, 600 physician and 1200 heathcare professional cards have been distributed throughout the medical center. The cards have been strongly accepted by the medical and nursing staff. Several patient care units requested additional cards and had them laminated for the staff. Subjectively, there has been a decrease in the number of calls to IC regarding basic isolation procedures. IC staff is beginning to be seen as an important part of the patient care team. This is imperative as governmental and patient safety organizations are looking for evidence that IC is an active member of the healthcare team. LESSONS LEARNED: The card's acceptance and ease of use demonstrates the need for more quick reference and educational material that focuses on IC policies and practices. The success of this basic intervention suggests that other modes of information delivery, such as use of personal data assistant (PDA) devices and an IC Website, may also be useful tools for dissemination of IC-related information.

Scalable and portable visualization of large atomistic datasets

A scalable and portable code named Atomsviewer has been developed to interactively visualize a large atomistic dataset consisting of up to a billion atoms. The code uses a hierarchical view frustum-culling algorithm based on the octree data structure to efficiently remove atoms outside of the user's field-of-view. Probabilistic and depth-based occlusion-culling algorithms then select atoms, which have a high probability of being visible. Finally a multiresolution algorithm is used to render the selected subset of visible atoms at varying levels of detail. Atomsviewer is written in C++ and OpenGL, and it has been tested on a number of architectures including Windows, Macintosh, and SGI. Atomsviewer has been used to visualize tens of millions of atoms on a standard desktop computer and, in its parallel version, up to a billion atoms. Para-professional agents known as auxiliaries, or community animal health workers, provide low-cost basic veterinary services to communities of livestock producers. A 2003 survey of 16 Sub-Saharan African countries, carried out as part of the Pan-African Programme for the Control of Epizootics, showed that in the majority of cases, the official Veterinary Services have no (or very few) links with the auxiliaries, although they are well aware of their existence and in some cases the auxiliaries have been trained by officials of the Veterinary Services. However, there are isolated cases of countries establishing more formal links, for example, recognising the status of an auxiliary, recognising auxiliaries with no definition of a status, attaching auxiliaries to Veterinary Service staff, establishing agreements for the provision of auxiliary services through livestock producer associations, harmonising auxiliary training programmes, issuing professional auxiliary cards, and setting up a consultation framework on the issue of auxiliaries. Unlike private veterinarians, agents of the official services do not generally perceive auxiliaries as competitors, and sometimes collaboration develops at this level. The authors propose several measures to improve links between the official Veterinary Services and auxiliaries, as follows: the fields of competence of auxiliaries should be defined and their curriculum harmonised, the status of auxiliaries should be recognised, a monitoring and assessment mechanism should be established at senior level in the Veterinary Services, training for livestock producers should be improved.

Simm v Louisiana State Board of Dentistry, 2002 US Dist LEXIS 3195, is a case that should give some of us pause for reflection. In short, the dentist used the designation MAGD (Master, Academy of General Dentistry) on his professional stationery, cards, and letterhead, as well as in his Yellow Pages advertising; these were all disseminated to the general public. The state board took exception to this practice, stating that it was misleading to the public because it might imply that the doctor either was a specialist in some field or had an advanced academic degree. The question that should concern us is, as a profession, what acronyms, if any, should we be allowed to use, and are they in and of themselves misleading?
