Automated sample preparation for electrospray ionization mass spectrometry based on CLOCK-controlled autonomous centrifugal microfluidics.

We report a centrifugal microfluidic device that automatically performs sample preparation under steady-state rotation for clinical applications using mass spectrometry. The autonomous microfluidic device was designed for the control of liquid operation on centrifugal hydrokinetics (CLOCK) paradigm. The reported device was highly stable, with less than 7% variation with respect to the time of each unit operation (sample extraction, mixing, and supernatant extraction) in the preparation process. An agitation mechanism with bubbling was used to mix the sample and organic solvent in this device. We confirmed that the device effectively removed the protein aggregates from the sample, and the performance was comparable to those of conventional manual sample preparation procedures that use high-speed centrifugation. In addition, probe electrospray ionization mass spectrometry (PESI-MS) was performed to compare the device-treated and manually treated samples. The obtained PESI-MS spectra were analyzed by partial least squares discriminant analysis, and the preparation capability of the device was found to be equivalent to that of the conventional method.

Rapid determination of glyphosate and glufosinate in human blood by probe electrospray ionization tandem mass spectrometry

In forensic science, glyphosate (GLYP) and glufosinate (GLUF), a class of non-selective broad-spectrum herbicides, have been frequently encountered in many fatal poisoning and suicide cases due to their widespread availability. Therefore, it is essential to develop an effective method for detecting these compounds. Some conventional methods, such as gas chromatography–mass spectrometry (GC–MS) or liquid chromatography–mass spectrometry (LC–MS), have been reported to detect these compounds. However, these methods are not ideal for their time-consuming and non-sensitive feature. Herein, probe electrospray ionization (PESI) tandem mass spectrometry (MS/MS), a fast and sensitive technique, was applied for the determination of GLYP and GLUF in human blood, which can obtain analytical results within 0.5 min without derivatization and chromatographic separation. After protein precipitation of blood samples, the supernatant was mixed with isopropanol and ultra-pure water (1:1 v/v). Then, 8 μL of the mixture was introduced into the plastic sample plate for PESI-MS/MS analysis. The limits of detection (LODs) of the method were 0.50 μg/mL and 0.25 μg/mL for two analytes, and the limits of quantitation (LOQs) were both 1.00 μg/mL, which are higher than the concentration of reported poisoning and fatal cases. In the linear range of 1–500 μg/mL, the regression coefficients (r2) for GLYP and GLUF were over 0.99. The matrix effects ranged from 94.8 % to 119.5 %, and the biases were below 4.3 %. The recoveries ranged between 84.8 % and 107.4 %, and the biases were below 7.6 %. Meanwhile, the method was effectively utilized to detect and quantify the blood, urine, and other samples. Consequently, the results suggest that PESI-MS/MS is a straightforward, fast, and sensitive method for detecting GLUF and GLYP in forensics. In the future, PESI-MS/MS will become an indispensable technique for polar substances in grassroots units of public security where rapid detection is essential.

Development of a rapid-fire drug screening method by probe electrospray ionization tandem mass spectrometry for human urine (RaDPi-U).

Drug screening tests are mandatory in the search fordrugs in forensic biological samples, and immunological methods and mass spectrometry (e.g., gas chromatography-mass spectrometry and liquid chromatography-tandem mass spectrometry) are commonly used for that purpose. However, these methods have some drawbacks, and developing new screening methods is required. In this study, we develop a rapid-fire drug screening method by probe electrospray ionization tandem mass spectrometry (PESI-MS/MS), which is an ambient ionization mass spectrometrymethod, for human urine, named RaDPi-U. RaDPi-U is carried out in three steps: (1) mixing urine with internal standard (IS) solution and ethanol, followed by vortexing; (2) pipetting the mixture onto a sample plate for PESI; and (3) rapid-fire analysis by PESI-MS/MS. RaDPi-U targets 40 forensically important drugs, which include illegal drugs, hypnotics, and psychoactive substances. The analytical results were obtained within 3min because of the above-mentioned simple workflow of RaDPi-U. The calibration curves of each analyte were constructed using the IS method, and they were quantitatively valid, resulting in good linearity (0.972-0.999) with a satisfactory lower limit of detection and lower limit of quantitation (0.01-7.1ng/mL and 0.02-21ng/mL, respectively). Further, both trueness and precisions were 28% or less, demonstrating the high reliability and repeatability of the method. Finally, we applied RaDPi-U to three postmortem urine specimens and successfully detected different drugs in each urine sample. The practicality of the method is proven, and RaDPi-U will be a strong tool as a rapid-fire drug screening method not only in forensic toxicology but also in clinical toxicology.

Probe Electrospray Ionization Tandem Mass Spectrometry for the Detection and Quantification of Benzodiazepines.

Legally prescribed benzodiazepines (BZDs) and designer BZDs are widely misused and must be determined in multiple contexts (eg, overdose, drug-facilitated sexual assaults, or driving under the influence of drugs). This study aimed to develop a method for measuring serum BZD levels using probe electrospray ionization (PESI) mass spectrometry and an isotope dilution approach. A tandem mass spectrometer equipped with a probe electrospray ionization source in multiple reaction monitoring mode was used. Isotope dilution was applied for quantification using a deuterated internal standard at a fixed concentration for alprazolam, bromazepam, diazepam, nordiazepam, oxazepam, temazepam, zolpidem, and zopiclone. This method included designer BZDs: clonazolam, deschloroetizolam, diclazepam, etizolam, flualprazolam, flubromazepam, flubromazolam, meclonazepam, nifoxipam, and pyrazolam. Sample preparation was done by mixing 10 µL of serum with 500 µL of an ethanol/ammonium formate 0.01 mol/L buffer. Complete validation was performed, and the method was compared with liquid chromatography coupled with mass spectrometry (LC-MS/MS) and immunoassays (IC) by analyzing 40 real samples. The analysis time for identification and quantification of the 18 molecules was 2.5 minutes. This method was fully validated, and the limits of quantification varied from 5 to 50 mcg/L depending on the molecule. In the 40 real samples, 100% of molecules (n = 89) were detected by both LC-MS/MS and PESI-MS/MS, and regression analysis showed excellent agreement between the 2 methods (r2 = 0.98). On IC, bromazepam and zolpidem were not detected in 2 and 1 cases, respectively. PESI-MS/MS allows serum BZD detection and measurement. Given the isotope dilution approach, a calibration curve was not required, and its performance was similar to that of LC-MS/MS, and its specificity was higher than that of IC.

Plastic probe electrospray ionization mass spectrometry developed for rapid fingerprint profile of biological samples without pretreatment.

A triangular-shaped flat plastic substrate probe was prepared for direct electrospray ionization mass spectrometry (ESI-MS) for analysis of untreated chemical and biological samples including liquids (Met-Arg-Phe-Ala peptide, reserpine, and dodecyl aldehyde), solids (biological samples, traditional Chinese medicine), and powders (roasted coffee, rhizoma coptidis, lotus plumule, and Schisandra sphenanthera). Quantitative analysis of reserpine in water yielded a detection limit of 1ngmL-1, dynamic response range within 1-500ngmL-1, and linearity of signal response ˃0.9925. Compared to the conventional capillary ESI, this plastic probe ESI offers lower cost of analysis (US $0.0056 per probe), higher sensitivity, lower sample consumption, longer signal duration (>6min), better reproducibility, signal stability, and higher speed of analysis (<10s per sample, including sample loading). Overall, the results indicate the potential of ESI-MS based on flat plastic probes as a versatile method for simple, sensitive, and stable analysis of untreated biological sample analysis.

μ-PESI-based MS profiling combined with untargeted metabolomics analysis for rapid identification of red wine geographical origin.

The commercial value of red wine is strongly linked to its geographical origin. Given the large global market, there is great demand for high-throughput screening methods to authenticate the geographical source of red wine. However, only limited techniques have been established up to now. Herein, a sensitive and robust method, namely probe electrospray ionization mass spectrometry (μ-PESI-MS), was established to achieve rapid analysis at approximately 1.2 min per sample without any pretreatment. A scotch near the needle tip provides a fixed micro-volume for each analysis to achieve satisfactory ion signal reproducibility (RSD < 26.7%). In combination with a machine learning algorithm, 16 characteristic ions were discovered from thousands of detected ions and were utilized for differentiating red wine origin. Among them, the relative abundances of two characteristic metabolites (trigonelline and proline) correlated with geographical conditions (sun exposure and water stress) were identified, providing the rationale for differentiation of the geographical origin. The proposed μ-PESI-MS-based method demonstrates a promising high-throughput determination capability in red wine traceability.

Promotional role of tungstate in the integrated synthesis of C2 and C3 alcohols and understanding the bond functionality for a series of cascade reactions

AbstractIn this work, TiO2 nanowires (TNWs) were synthesized using a plasma‐assisted method, and then Pt clusters were impregnated onto these TNWs. The nanowires supported Pt when combined with various tungsten‐based co‐catalysts, which improved the catalytic performance. The direct conversion of cellulose to C2‐C3 alcohols using tungsten‐based co‐catalysts was enhanced, even at low temperatures. X‐ray photoelectron spectroscopy (XPS) and Raman analysis showed oxygen vacancy (Ov) enrichment on the surface of Pt/TiO2 in the presence of tungsten co‐catalysts, which improved their catalytic activity. The role of metallic platinum (Pto) was also investigated and was found to have a linear relationship with their activity as follows: H2WO4 &gt; (NH4)6H2W12O40 · xH2O &gt; H3PW12O40. Maximum yields of 32.33% and 51.52% of ethanol and propane‐2‐ol at optimum temperatures of 220 and 250 °C, respectively, were obtained with H2WO4. A catalytic reaction performed using tandem catalytic system gave a high ethanol yield of 25.56%, which is low in comparison with an integrated catalytic system. Cellulose conversion was also quantified here by probe electrospray ionization (pESI) coupled with Fourier transform mass spectroscopy (FTMS), which was never reported earlier for these reactions. This method provides evidence of negligible fragmentation of high molecular‐weight compounds, and the maximum cellulose conversion reported here is 100%. A reaction pathway is proposed, based on the experimental results, which elaborates the activation and cleavage of specific C‐C and C‐O bonds. © 2023 Society of Industrial Chemistry and John Wiley &amp; Sons Ltd.

Matrix-Compatible Solid-Phase Microextraction Pin Coupled Directly to Mass Spectrometry using Probe Electrospray Ionization.

A solid-phase microextraction (SPME) pin device with a biocompatible coating on the tip was developed for direct coupling to mass spectrometry (MS) via a vertical dipping-and-spray strategy using an automated probe electrospray ionization (PESI) interface. The developed method provides superior sensitivity compared to standard PESI-MS due to the enrichment effects of SPME and the significant increase in the volume of sample and/or solvent collected during dipping due to the SPME pin's notably larger size. The tips of the SPME pins were coated with a biocompatible coating consisting of small sorbent particles embedded into a polyacrylonitrile (PAN) binder. This coating enables the extraction of small molecules, while preventing larger molecules such as tissue fragments, proteins, and cell matter from coming into the sorbent. The developed SPME pin-PESI-MS method also features much lower matrix effects compared to PESI-MS for the analysis of complex biology samples. When applied for the analysis of 8 drugs of abuse in urine samples, the SPME pin-PESI-MS method provided good linearity (R2 ≥ 0.9997), high sensitivity with limits of detection between 0.003 to 0.03 ng/mL, and good reproducibility with RSD% ≤ 6%. The vertical design of the SPME-PESI-MS direct-coupling interface allows the potential fully automation of the system using a conventional autosampler.

Validation of a non-targeted method devoted to identification and quantitation of toxicologically relevant compounds in plasma with HRMS

The objective of this study was to develop and validate a simple method using liquid chromatography hyphenated to high resolution mass spectrometry (HRMS) allowing both the performance of a non-targeted screening and the simultaneous quantification of 29 compounds of interest in clinical and forensic toxicology.Extraction was done with QuEChERS salts and acetonitrile, after addition of internal standard to 200 μL of human plasma samples. The mass spectrometer was an Orbitrap, with a heated electrospray ionization (HESI) probe. The analyses were carried out in full scan experiment with a nominal resolving power of 60,000 FWHM within the 125–650 m/z mass range, followed by four cycles of data dependent analysis (DDA) with a mass resolution of 16,000 FWHM.The untargeted screening was evaluated using 132 compounds, mean limit of identification (LOI) was 8.8 ng/mL (min = 0.05 ng/mL, max = 500 ng/mL) and mean limit of detection (LOD) was 0.25 ng/mL (min = 0.05 ng/mL, max = 5 ng/mL). The method was linear in the 5 to 500 ng/mL range (0.5 to 50 ng/mL for cannabinoids, 6-acetylmorphine and buprenorphine) with correlation coefficients > 0.99, intra- and inter-day accuracy and precision were < 15% for all compounds. The method was successfully applied to 31 routine samples.

An effective pretreatment technique based on multi-walled carbon nanotubes to reduce the matrix effect in plasma samples analyzed by a new type probe electrospray ionization method

The quantitative analysis of drug plasma samples plays an important role in the drug development and drug clinical use. Our research team developed a new electrospray ion source―Micro probe electrospray ionization (μPESI) in the early stage, which was combined with mass spectrometry (μPESI-MS/MS) showing good qualitative and quantitative analysis performance. However, matrix effect severely interfered the sensitivity in μPESI-MS/MS analysis. To solve this problem, we recently developed a Solid-phase purification method based on multi-walled carbon nanotubes (MWCNTs), which was used for removing matrix interfering substances (especially phospholipid compounds) in the preparation of plasma samples, so as to reduce the matrix effect.In this study, aripiprazole (APZ), carbamazepine (CBZ) and omeprazole (OME) were used as representative analytes, the quantitative analysis related to the plasma samples spiked with the analytes above and the mechanism of the MWCNTs to reduce matrix effect were both investigated. Compared with the ordinary protein precipitation, MWCNTs could reduced the matrix effect for several to dozens of times, which resulting from the removement of phospholipid compounds from the plasma samples by MWCNTs in the selective adsorption manner. We further validated the linearity, precision and accuracy of this pretreatment technique by the μPESI-MS/MS method. These parameters all met the requirements of FDA guidelines. It was showed that MWCNTs have a good application prospect in the drug quantitative analysis of plasma samples using the μPESI-ESI-MS/MS method.

Introducing direct probe electrospray ionization tandem mass spectrometry in apiculture: Comparison with other mass spectrometric methods for the determination of antibiotics in bees and honey

Amphenicols and tetracyclines (TCs) antibiotics use in apiculture is mainly focused on the treatment of bee brood diseases. Despite their efficiency, the serious genotoxic effects that some elicit led to their banning for use in apiculture and for chloramphenicol (CAP) in all food-producing animals. In this work, three tandem mass spectrometric methods were developed and validated for the analysis and quantification of CAP, thiamphenicol (TF), florfenicol (FF), tetracycline (TC), oxytetracycline (OTC) and chlortetracycline (CTC) in honey and honeybees. With the first, we have demonstrated the first use of probe electrospray ionization (PESI) in conjunction with tandem mass spectrometry (MS/MS) for antibiotics determination in honey and bees entailing a minimum sample preparation for bees, resulting in rapid chemical analysis for both commodities. The second was a reversed-phase HPLC-ESI-MS/MS method sharing the same scope. With the third chiral HPLC-ESI-MS/MS method, it was attempted to separate CAP stereoisomers in both commodities since reports have shown that only the D-threo isomer displays antimicrobial properties. The analytical methods fulfilled validation criteria demonstrating recoveries in the range of 74–92%, managing to address the minimum required performance limit (MRPL) for CAP, with the exception of PESI-MS/MS. The latter displayed a detection capability (CCβ) for all compounds in proximity to 1 ng/g, and an overall runtime of 1.5 min. In total, 68 samples of honeybees and honey were evaluated for amphenicols and TCs residues. CAP was detected only in two honeybee samples, while one honey sample was positive to TC and OTC was detected in traces in another sample. PESI-MS/MS managed to quantify both TC in honey and CAP in bees in proximity to the values assigned by the conventional HPLC-ESI-MS/MS methods. With regard to the separation of CAP's isomers, the chiral method managed to resolute all isomers and quantify CAP in one bee sample in the form of its L-threo stereoisomer. Overall, ABCs detection in honey and related matrices still constitutes a challenge, considering their prevalence and potential applications in major honey-producing countries.

High-throughput analysis of anthocyanins in horticultural crops using probe electrospray ionization tandem mass spectrometry (PESI/MS/MS).

Plant secondary metabolites exhibit various horticultural traits. Simple and rapid analysis methods for evaluating these metabolites are in demand in breeding and consumer markets dealing with horticultural crops. We applied probe electrospray ionization (PESI) to evaluate secondary metabolite levels in horticultural crops. PESI does not require pre-treatment and separation of samples, which makes it suitable for high-throughput analysis. In this study, we targeted anthocyanins, one of the primary pigments in horticultural crops. Eighty-one anthocyanins were detected in approximately 3minutes in the selected reaction-monitoring mode. Tandem mass spectrometry (MS/MS) could adequately distinguish between the fragments of anthocyanins and flavonols. Probe sampling, an intuitive method of sticking a probe directly to the sample, could detect anthocyanins qualitatively on a micro-area scale, such as achenes and receptacles in strawberry fruit. Our results suggest that PESI/MS/MS can be a powerful tool to characterize the profile of anthocyanins and compare their content among cultivars.

Solvent effects of N,N-dimethylformamide and methanol on mass spectrometry imaging by tapping-mode scanning probe electrospray ionization.

Mass spectrometry imaging (MSI) is an effective technique for visualizing the distribution of lipids in tissues. The direct extraction-ionization methods using minute volumes of solvent for local components have the advantage of rapid measurement without any sample pretreatment. For effective MSI of tissues, it is necessary to understand the effect of solvent physicochemical properties on ion images. In this study, we report solvent effects on the lipid imaging of mouse brain tissue by tapping-mode scanning probe electrospray ionization (t-SPESI) which is capable of extraction-ionization using sub-pL solvents. To precisely measure lipid ions, we developed a measurement system incorporating a quadrupole-time-of-flight mass spectrometer. The differences in signal intensity and spatial resolution of lipid ion images were investigated using N,N-dimethylformamide (non-protic polar solvent), methanol (protic polar solvent) and their mixture. The mixed solvent was suitable for the protonation of lipids, and it provided high spatial resolution MSI. Results indicate that the mixed solvent improves the extractant transfer efficiency and minimizes charged droplets from an electrospray. The solvent selectivity study revealed the importance of solvent selection based on physicochemical properties for the advancement of MSI by t-SPESI.

Ultrafast measurement of drugs using Probe Electrospray Ionization Mass Spectrometry (PESI-MS) and isotope dilution: An example with metformin

Probe Electrospray Ionization (PESI) is a miniaturized electrospray (ESI) with a capillary of the nanoelectrospray converging on a solid needle with a tip of several hundred nanometers. This probe was developed to be coupled directly to a mass spectrometer (MS) without the need for chromatographic separation, allowing ultrafast analysis with minimal sample preparation. Metformin is the first-line treatment for type 2 diabetes. Lactic acidosis (LA) is a frequent complication that can be either induced by or associated with elevated metformin plasma concentrations. Therefore, a rapid measurement plays an important role in helping clinicians to diagnose overexposure to metformin. In the present study, our objective was to develop a PESI-MS method for an ultra-rapid determination of metformin in plasma by using an isotope dilution approach. Analysis was carried out with a PESI source installed on a triple quadrupole MS (LCMS 8060®, Shimadzu). A multiple reaction monitoring mode (MRM) with 2 transitions for metformin and metformin-D6 (internal standard) was used. To be able to apply an isotope dilution protocol, the main MS parameters were optimized to ensure that the m/z 130.15 > 71.10 (metformin) and m/z 135.95 > 77.15 (metformin-D6) transitions exhibited the same intensity. The method was fully validated with 6 concentration levels (0.5 to 50 mg/L). The matrix effect was evaluated for each level, and the specificity was tested by analysing 6 plasmas spiked with a mix of 44 potential co-medications. Finally, the performance was compared with 2 classical LC-MS/MS and LC-DAD methods used in external labs, by analysing samples from 29 patients. The sample preparation consisted in mixing 10 μL of plasma with 1000 μL ethanol/ammonium formate buffer including metformin-D6 at a fixed concentration of 5 mg/L. Total run time was 0.31 min. The limit of detection and quantification was fixed at 0.5 mg/L. Isotope dilution gave satisfactory results in accuracy and precision for each concentration level (min-max: −12.1–15.8% and 1.0–7.1%, respectively). Thanks to the internal standard the matrix effect was fully corrected (bias less than 1%). The specificity study also reported satisfactory results. Finally, in the representative group of 29 patients (55% with a concentration <5 mg/L, 38% with a concentration >5 mg/L and 7% not detected), we observed almost identical results when comparing LC-DAD and LC-MS/MS to PESI-MS (r2 > 0.99). The PESI-MS approach needs no sample preparation. Combined with a isotope dilution protocol, that requires no calibration curve building, offers the best possibility to design specific, sensitive, accurate and ultra-fast solutions. Our study presents a method which allows an accurate determination of metformin in about 30 seconds that will be helpful in a core lab when rapid diagnosis of LA is needed.

Detection of drugs in urine by ambient direct ionization mass spectrometry

尿液作为一种易于获取的体内毒品检材,在吸毒人员快速筛查中被广泛应用。针对传统快速筛查技术存在假阳性率高、定量能力不足以及实验室质谱技术在快速检测中存在前处理复杂、检测耗时长、使用环境苛刻等问题,该文提出了一种基于敞开式直接电离质谱技术的生物样本快速检测方法。该研究采用探针式电喷雾离子源与便携式质谱仪联用快速检测平台,优化了喷雾电压和质谱入口毛细管温度,开发了高效快速的前处理技术。基于该平台和前处理技术,5种常规毒品(甲基苯丙胺、氯胺酮、可卡因、O6-单乙酰吗啡和3,4-亚甲双氧甲基苯丙胺)的尿液加标溶液的检出限为0.5~30 ng/mL,且其中4种毒品定量检测的线性相关系数大于0.99。除此之外,5种常规毒品在3个不同水平下的加标回收率为56.1%~103.7%,多次检测结果的相对标准偏差为9.0%~27.8%,说明联用检测平台与前处理方法结合可以达到良好的准确度。为了进一步检验该联用仪器的实战能力,测试了某社区戒毒康复中心40份阳性和110份阴性实际尿液样本,总体检测的准确率接近99%,且通过一次进样在20 s内可同时检测多种毒品。该研究成果有利于推动快速检测技术的发展,促进敞开式直接电离质谱仪技术的推广应用,提升一线执法服务水平。

Ultrafast Measurement of Metformin in the Clinical Setting Using Probe Electrospray Ionization Mass Spectrometry.

Metformin (MtF) is a treatment used for type 2 diabetes. Lactic acidosis (LA) is a frequent complication that can be either induced by or associated with elevated MtF plasma concentrations. When coupled with a mass spectrometry (MS) system, the probe electrospray ionization (PESI) method allows direct and rapid analysis of different types of matrices without pretreatment. In this study, we developed a PESI-MS method for the determination of MtF in plasma. We used a tandem mass spectrometer equipped with a PESI source in the reaction monitoring mode for the quantitation of MtF. MtF-d6 was chosen as the internal standard (IS), following an isotope dilution (ID) approach. The method was fully validated with six concentration levels (0.5-50 mg/L). The matrix effect was evaluated for each level, and the specificity was tested with a mix of potential co-medications. Using patient samples, the performance was compared with two classical LC-MS-MS and LC-diode array detector (DAD) methods used in external labs. Sample preparation consisted in mixing 10 µL plasma in 1,000 µL ethanol/ammonium formate buffer including MtF-d6 at a fixed concentration of 5 mg/L. The total run time was 0.31 min. ID gave satisfactory results of accuracy and precision (min-max: -12.1 to 15.8% and 1.0-17.1%, respectively). The matrix effect was fully corrected by the internal standard (bias < 1%). The specificity study also reported satisfactory results. Finally, in a representative group of 29 patients (55% with a concentration <5 mg/L, 38% with a concentration >5 mg/L and 7% not detected), we observed almost identical results when comparing LC-DAD and LC-MS-MS to PESI-MS (r2 > 0.99). We propose a specific, sensitive, accurate and ultrafast solution for the measurement of MtF in patient plasma, with no sample preparation or calibration curve building. This could be helpful in a core lab when rapid diagnosis of LA is needed.

Fast Classification of Thyroid Nodules with Ultrasound Guided-Fine Needle Biopsy Samples and Machine Learning

A rapid classification method was developed for the malignant and benign thyroid nodules with ultrasound guided-fine needle aspiration biopsy (FNAB) samples. With probe electrospray ionization mass spectrometry, the mass-scan data of FNAB samples were used as datasets for machine learning. The patients were marked as malignant (98 patients), benign (110 patients) or undetermined (42 patients) by experienced doctors in terms of ultrasound, the B-Raf (BRAF) gene, and cytopathology inspections. Pairwise coupling was performed on 163 ions to generate 3630 ion ratios as new features for classifier training. With the new features, the performance of the multilayer perception (MLP) classifier is much better than that with the 163 ions as features directly. After training, the accuracy of the MLP classifier is as high as 92.0%. The accuracy of the single-blind test is 82.4%, which proved the good generalization ability of the MLP classifier. The overall concordance is 73.0% between prediction and six-month follow-up for patients in the undetermined group. Especially, the classifier showed high accuracy for the undetermined patients with suspicious for papillary carcinoma diagnosis (90.9%). In summary, the machine learning method based on FNAB samples has potential for real clinical applications.

Rapid diagnosis of papillary thyroid carcinoma with machine learning and probe electrospray ionization mass spectrometry.

Frozen section examination could provide pathological diagnosis for surgery of thyroid nodules, which is time-consuming, skill- and experience-dependent. This study developed a rapid classification method for thyroid nodules and machine learning. Total 69 tissues were collected including 43 nodules and 26 nodule-adjacent tissues. Intraoperative frozen section was first performed to give accurate diagnosis, and the rest frozen specimen were pretreated for probe electrospray ionization mass measurement. By multivariate analysis of mass scan data, a series compounds were found downregulated in the extraction solution of papillary thyroid carcinoma (PTC), but some were found upregulated by mass spectrometry imaging. m/z 758.5713 ([PC[34:2] + H]+ ), m/z 772.5845 ([PC[32:0] + K]+ ), and m/z 786.6037 ([PC[36:2] + H]+ ) were firstly identified as potential biomarkers for nodular goiter (NG). Machine learning was employed by means of support vector machine (SVM) and random forest (RF) algorithms. For classification of PTC from NG, SVM and RF algorithms exhibited the same performance and the concordance was 94.2% and 94.4% between prediction and pathological diagnosis with positive and negative mass dataset, respectively. For the classification of PTC from PTC adjacent tissues, SVM was better than RF and the concordance was 93.8% and 83.3% with positive and negative mass dataset, respectively. With the identified compounds as training features, the sensitivity and specificity are 87.5% and 88.9% for the test set. The developed method could also correctly predict the malignancy of one medullary thyroid carcinoma and one adenomatous goiter (benign). The diagnosis time is about 10min for one specimen, and it is very promising for the intraoperative diagnosis of papillary thyroid carcinoma.

Versatile Mass Spectrometry-Based Intraoperative Diagnosis of Liver Tumor in a Multiethnic Cohort

Currently used techniques for intraoperative assessment of tumor resection margins are time-consuming and laborious and, more importantly, lack specificity. Moreover, pathological diagnosis during surgery does not often give a clear outcome. Recent advances in mass spectrometry (MS) and instrumentation have made it possible to obtain detailed molecular information from tissue specimens in real-time, with minimal sample pre-treatment. Probe Electro Spray Ionization MS (PESI-MS), combined with artificial intelligence (AI), has demonstrated its effectiveness in distinguishing liver cancer tissues from healthy tissues in a large Italian population group. As the MS profile can reflect the patient’s ethnicity, dietary habits, or particular operating room procedures, the AI algorithm must be well trained to distinguish different groups. We used a large dataset composed of liver tumor and healthy specimens, from the Italian and Japanese populations, to develop a versatile algorithm free from ethnic bias. The system can classify tissues with discrepancies &lt;5% from the pathologist’s diagnosis. These results demonstrate the potential of the PESI-MS system to distinguish tumor from surrounding non-tumor tissues in patients, with minimal bias from race/ethnicity or etiological characteristics or operating room procedures.

Arterial thromboxane A2-induced transient contraction after IL-1β exposure

The involvement of thromboxane A2 (TXA2) in systemic inflammation and infection is well recognized. However, there are few reports on the involvement of prostanoids in warm shock (the initial pathology of sepsis). Previous studies showed that interleukin (IL)-1β causes a rapid inducible nitric oxide synthase/nitric oxide (iNOS/NO)-mediated relaxation in peripheral blood vessels during warm shock. Furthermore, a transient contraction was seen before this relaxation occurred. The present study aimed to elucidate the mechanism of this transient contraction. We measured isometric tension changes in the superior mesenteric arteries from normal male Wistar rats by adding IL-1β at the point of maximum contraction by phenylephrine (Ph). The same study was performed for each vessel pretreated with various inhibitors, including SQ29548, a TXA2 receptor antagonist, 30 min before Ph contraction. In addition, the concentration of thromboxane B2 (TXB2) in SMA was measured by probe electrospray ionization. Treatment of endothelial vessels with cyclooxygenase 1 (COX1)/2 inhibitors SC560/NS398 and TXA2 receptor antagonist SQ29548 suppressed IL-1β–induced transient contractions. This transient contraction reaction was derived from TXA2. Additionally, gene expression of COX2/TXA2 synthetase and the concentration of TXB2 were significantly increased in IL-1β-exposed vessels. It was demonstrated for the first time in inflamed blood vessels that endothelial cell-derived COX2/TXA2 is induced before iNOS and causes transient contractions. TXA2 may be considered an early sign of warm shock or as a biological defense mechanism in the early stages of septic shock.
