Primary synovial chondromatosis of the first tarsometatarsal joint: a rare case report

Primary synovial chondromatosis is a rare condition and commonly involves a single large joint, although it may involve any synovial joint. It occurs as a result of metaplasia and focal formation of cartilage in the intimal layer of the synovial membrane. Diagnosis could be made after thorough clinical and radiological assessment. Diagnosis must be confirmed by histopathological examination, because malignant transformation has been reported. Here, we report a case of primary synovial chondromatosis on an extremely rare location involving the first tarsometatarsal joint.

Uncommon Primary Synovial Chondromatosis Involving Only the Infrapatellar Fat Pad in an Elderly Patient.

Primary synovial chondromatosis is a rare condition of idiopathic synovial chondrometaplasia and usually occurs during the third to fifth decades of life. Conversely, secondary synovial chondromatosis results from the growth of separated particles from articular cartilage or osteophytes in patients with joint diseases, such as degenerative osteoarthritis, and occurs mostly in elderly people. We describe here a 76-year-old male histopathologically confirmed as having primary synovial chondromatosis with no calcification of the infrapatellar fat pad (IFP) of the knee joint. To our knowledge, this is the first description of primary synovial chondromatosis of the knee joint confined to the IFP in a patient >60 years old.

A Rare Case of Primary Synovial Chondromatosis of the Ankle

ABSTRACT Synovial chondromatosis is a condition where the synovial lining of joints, tendons, or bursa undergoes metaplasia into cartilaginous loose bodies. Primary synovial chondromatosis of the ankle joint is very rare and less commonly reported in literature. We report a case of primary synovial chondromatosis of the ankle that we encountered in our clinic. A 33-year-old male patient presented with a history of multiple swellings around his right ankle joint, along with dull aching pain. Both the clinical examination and X-rays were suggestive of primary synovial chondromatosis of the ankle. We performed an open loose body removal and synovectomy of the ankle. Histopathology confirmed the diagnosis of primary synovial chondromatosis. We report this case owing to its rarity of presentation. How to cite this article Chathoth V, Sankaranarayanan S. Bakers Cyst with Synovial Chondromatosis of Knee - A Rare Case Report.

Introduction:Synovial chondromatosis is a rare intraarticular benign condition arising from the synovial membrane of the joints, synovial sheaths or bursae around the joints. Primary synovial chondromatosis typically affects the large joints in the third to fifth decade of life, although involvement of smaller joints and presentation in younger age group is also documented. The purpose of this case report is to document this rare extra articular synovial pathology present inside the baker’s cyst which required open synovectomy and debridement to eradicate it.Case Report:A 43 yearold male presented with a two year history of pain, swelling and restriction of right knee joint. After the clinical and radiological assessment, open synovectomy, removal of cyst and thorough joint debridement procedure was performed. Histopathological study confirmed the findings of synovial chondromatosis.Conclusion:Synovial chondromatosis is a rare benign condition. Complete synovectomy offers reliable cure rate.

Synovial Chondromatosis of the Ankle Joint: Clinical, Radiological, and Intraoperative Findings.

Synovial chondromatosis, also termed synovial osteochondromatosis, is a rare benign disorder characterized by the presence of cartilaginous nodules in the synovium of the joints, tendon sheaths, and bursae. It most commonly involves large joints, such as the knee, hip, and shoulder, but its presence in smaller joints has also been reported. Nevertheless, ankle involvement is unusual. The diagnosis is commonly made following a thorough history, clinical, physical, and radiographic examination. We report a case of a young patient with primary synovial chondromatosis of the ankle joint and present the clinical, radiographic, and intraoperative findings.

Surgical Treatment of Synovial Chondromatosis of the Long Biceps Tendon Sheath

Objectives:Primary synovial chondromatosis (PSC) is a rare benign neoplasm that affects diarthrodial joints. The reported cases of PSC in the literature are mostly localized in the knee, followed by the hip, elbow, wrist and shoulder. We report the surgical treatment of a rare case of PSC localized in the long biceps tendon sheath.Methods:A 66-year-old woman presented severe pain in her left shoulder without any traumatic event. The pain was rated on Visual analog scale (VAS) as 4 to 7. At the physical examination, a palpable mass was found medial to the bicipital groove of the left shoulder. Functional examination of the left shoulder revealed painful limitation at 150 abduction, at 130 forward elevation and at 40 external rotation. Constant Murley Score was measured as 65. A magnetic resonance image (MRI) revealed the presence of 2x2x1.8 cm lesion within the biceps tendon sheath, rotator cuff tear and a bone cyst in the great tubercule. The treatment of choice was the open surgical treatment with excision of the lesion, curettage and spongious allografting of the bony cyst and repair of the rotator cuff with two suture anchors. The histological examination was consistent with PSC without any malignant transformation. The shoulder was immobilized postoperatively for 2 days in a sling. Immediate passive ROM exercises were started at third day for 6 weeks.Results:At the 6 week follow-up, the patient achieved full symmetric restoration of motion. She had returned to full activity. Visual analog scale was rated 0 to 1 and Constant Murley Score was measured as 92. Functional examination of the left shoulder reveled 170 of abduction, at 160 forward elevation and at 40 external rotation. At 1-year follow-up, clinical findings were unremarkable, with no sign of recurrence.Conclusion:PSC is a benign, idiopathic lesion of the synovial membranes of the diarthrodial joints. PSC is observed rarely in extra-articular shoulder involvement. Although the diagnosis of PSC is often clear with the results of radiological and intraoperative findings, it needs to be verified histologically. Surgical treatment is preferred to avoid the risk of malignant transformation, possible damage to the neighboring anatomical structures and functional disability as seen in our patient.

A case report of a primary synovial chondromatosis of temporomandibular joint, producing medial displacement of the articular disc

AbstractSynovial chondromatosis in the temporomandibular joint (TMJ) is a rare benign cartilaginous metaplasia characterized by the formation of loose bodies within the synovial joint space. This condition presents a specific triad of semiological characteristics featuring pain, swelling, and limitation of function. We will present a clinical case of a 49 years‐old female patient who presented to the clinica Davila's maxillofacial service with severe functional alterations in her right TMJ. Clinically, the patient had a maximum aperture of 15 mm wide, alteration of mandibular dynamics and preauricular pain. Posterior conducting a complete clinical examination and conclusive magnetic resonance imaging (MRI) findings it was decided to perform surgery on the joint. Articular cavity was exposed through a preauriclar approach, where the joint capsule presented no evident alteration. After opening the capsule, we were able to identify and extract multiple cartilaginous bodies both above and below the disc space. The articular surfaces were in good condition, while the articular disc was shifted to the medial portion of the cavity. The removal of the loose bodies was held the meniscopexy of the articular disc with a small joint anchor. The immediate post‐operative period was uneventful, and the patient did not present any alteration with respect to motor function of facial expression. At the 6‐week control, we recorded an increase in maximum mouth opening of 30 mm and a reduction of the pain. Six months later, we recorded an even wider mouth opening with no signs or evidence of recurrence.

Biceps Tenoscopy in Arthroscopic Treatment of Primary Synovial Chondromatosis of the Shoulder

Primary synovial chondromatosis (PSC) of the shoulder is a rare condition and usually necessitates operative therapy. Arthroscopic partial synovectomy with removal of loose osteochondromas may be regarded as the current surgical treatment of choice. However, involvement of the biceps tendon sheath (BTS) occurs in almost half of the patients and required additional open surgery in all previously reported cases. We successfully performed tenoscopy of the BTS and long head of the biceps tendon during arthroscopic treatment of PSC in a 26-year-old male competitive wrestler. Biceps tenoscopy enabled minimally invasive partial (teno)synovectomy and removal of all osteochondromas within the BTS. The symptoms of PSC fully subsided within 2 postoperative weeks. There were no functional restrictions at the 3-month follow-up examination. These preliminary results support the feasibility, safety, and efficacy of biceps tenoscopy as a complement in arthroscopic treatment of PSC of the shoulder, dispensing with the need for additional open surgery. The spectrum of indications for biceps tenoscopy has still to be defined. Conceivable indications are proposed. This first report of a diagnostic and interventional biceps tenoscopy entails a detailed step-by-step description of the surgical technique.

Condrosarcoma secundario: correlación radiopatológica

Chondrosarcomas are malignant bone tumors originating in cartilage. Chondrosarcoma is the third most common malignant bone tumor after multiple myeloma and osteosarcoma.About 75% of chondrosarcomas are primary lesions. The remaining 25% belong to special categories such as histologic variants and secondary forms.A secondary chondrosarcoma is one that appears in a pre-existing benign chondral lesion; the different types of secondary chondrosarcomas include solitary osteochondroma, multiple osteochondromatosis, enchondroma, the different types of enchondromatosis, and primary synovial chondromatosis. The incidence of this malignant transformation varies widely in function of the type of lesion.In this article, we discuss and illustrate the different types of secondary chondrosarcomas, placing special emphasis on the imaging findings that should alert to these lesions and give radiologists a key role in the diagnosis, management, and follow-up of these patients.

Synovial chondrosarcoma arising in synovial chondromatosis.

Primary synovial chondromatosis (SC) is a rare proliferative disorder that causes pain, swelling, and restriction of movement to the joints it affects. The disease frequently runs a protracted course, often requiring multiple surgical procedures to obtain some control. Few reports exist detailing the natural history of SC, although malignant transformation to synovial chondrosarcoma (CHS) is recognized to be a rare event. The aim of our study was to review a large orthopaedic oncology database in order to evaluate the incidence of CHS arising from SC. We identified 78 patients who have presented to our centre with primary synovial chondromatosis (SC). Of those patients, 5 went on to develop malignant change. This represents a 6.4% incidence of developing synovial chondrosarcoma (CHS) within preexisting primary synovial chondromatosis. The patients had a mean age of 28 years at first diagnosis with synovial chondromatosis with the median time from original diagnosis to malignant transformation being 20 years (range 2.7–39 yrs).

Synovial chondromatosis of the hip - Management with synovectomy and partial removal of loose bodies: Case study

Primary synovial chondromatosis is an uncommon disorder, in which involvement of the hip joint is still rare. Clinical symptoms are usually non-specific, clinical diagnosis of synovial chondromatosis of the hip may be difficult and delayed, especially before the ossifying nodules become evident. Currently, the recommended management is surgical removal of the loose bodies and synovectomy without dislocating the hip joint. We report a case of synovial chondromatosis of the hip in a 70 year old male patient, managed with an open synovectomy and removal of the loose bodies. We opine this is an easy and safe method for management of this disorder.

Giant Solitary Synovial Osteochondromatosis of The Knee in a Young Female

Introduction: Giant Synovial osteochondromatosis of Hoffa’s body is very uncommon. Although described as a benign disease, it can be destructive and can cause severe osteoarthritis and pain. Case report: We report an 18 year old female patient presented with a calcified mass inside the Hoffa’s body. Clinically, patient presented with eight-month history of progressively worsening left knee pain with associated swelling. The bony mass in the Hoffa’s body was evident on the X-ray. MRI showed synovial affectation. During the arthroscopy, all pathological synovial was removed and the bony mass was extirpated through a mini-arthrotomy. Diagnosis of a giant synovial osteochondromatosis was confirmed by histology and malignancy was ruled out. Five years after surgery the patient has been asymptomatic and motion range is complete. Conclusion: This case of primary synovial chondromatosis is interesting because it was presented in an age, gender and unusual location. At 5 years of postoperative follow-up the patient has had no recurrence and has showed excellent performance of the knee joint.

Arthroscopic Management of Primary Synovial Chondromatosis of the Hip

We evaluated clinical outcomes after hip arthroscopy in patients with primary synovial chondromatosis (SC). We retrospectively assessed 11 patients who underwent hip arthroscopy for primary SC at a mean follow-up of 22 months (range, 12 to 36 months). Clinical preoperative and postoperative evaluation was performed with the Harris hip score. The preoperative evaluation included plain radiographs and magnetic resonance (MR) scanning to detect number and positioning of intra-articular radiopaque loose bodies. The osteochondral damage was graded using the Kellgren-Lawrence classification. The chondral surfaces of both the acetabulum and femoral head were graded according to the Outerbridge scale. The clinical score improved postoperatively. There were statistically significant differences between preoperative and postoperative Harris hip scores (P < .05). Outcomes were rated as very satisfactory and satisfactory in 3 and 5 of 11 patients, respectively. The osteochondral damage ranged between stages 1 and 2. No complications related to surgical procedures were observed. Hip arthroscopy for the treatment of patients with primary SC showed good clinical results without any complications related to the surgical procedure. A rare shoulder disease of primary synovial chondromatosis

Primary synovial chondromatosis is a rare, benign condition which affects the synovial membranes. It commonly involves knee, elbow and hip in young adults but is rare in shoulder. We reviewed a rare case of synovial chondromatosis of the shoulder. A young female came to our hospital with pain and swelling at right shoulder joint. Radiograph, ultrasound and MRI with contrast were obtained. It helped to make a diagnosis of synovial chondromatosis. The shoulder was operated and multiple oval loose bodies measuring up to 8 mm were removed. Partial synovectomy was done.

PRIMARY SYNOVIAL CHONDROMATOSIS OF THE SHOULDER (CASE REPORT)

Primary synovial chondromatosis of a shoulder is quite rare in clinical practice. The diagnostics of this pathology has to include the histological examination of intraarticular material to detect the signs of tissue malignant transformation. Currently surgical intervention of this disease involves the removal of intra-articular cartilaginous bodies and local synovectomy of the affected joint by arthroscopic or open methods. The authors describe the clinical, histologic and arthroscopic features of primary synovial chondromatosis of the shoulder in order to assess the possibility of its minimally invasive correction. The clinical example of diagnostics and treatment of patient with this pathology was presented.

WRIST SYNOVIAL CHONDROMATOSIS: CASE REPORT AND LITERATURE REVIEW

Primary synovial chondromatosis is a proliferation of cartilaginous bodies within the synovial membrane, tendon sheath or bursa. It is a rare orthopaedic entity especially when it occurs in the distal radioulnar joint. We report a 27-year-old man with recurrent synovial chondromatosis, nine years after his first operation. Although rare, synovial chondromatosis must be considered in the differential diagnosis in a patient with pain and swelling of the distal radioulnar joint. From our literature review, we report a 17% (5/30) recurrence rate for synovial chondromatosis involving the wrist joint which has not been documented in the medical literature previously.

Fatty lesions in intra-articular loose bodies: a histopathological study of non-primary synovial chondromatosis cases

Intra-articular loose bodies (ILBs) are not uncommon conditions in patients with various joint diseases. Their morphological alterations have been investigated in detail, but little attention has been paid to their fatty lesions. In this study, we examined fatty bone marrow, fat cells without bone marrow structures (extramedullary fat cells), and their necrotic changes in 55 ILBs surgically removed from 42 patients, excluding primary synovial chondromatosis cases. The presence of viable re-vascularized vessels with or without enchondral ossification could discriminate 19 re-attached ILBs from 36 true free ILBs. Fatty bone marrow was found in 25 ILBs, and its necrosis was recognized in 11 (44.0%) of them. Extramedullary fat cells, characterized by single or clustered fat cells focally or multifocally distributed in fibrous or cartilaginous stroma, were identified in seven true free ILBs (7of 55, 12.7%), and all of them were focally necrotic. Unique lipomembranous changes were detected in 7 (12.7%) of 55 ILBs; they were found in 4 (36.4%) of 11 ILBs of necrotic bone marrow and were associated with 3 (42.9%) of 7 necrotic extramedullary fatty lesions. These changes were highlighted by periodic-acid Schiff and Masson's trichrome stain more clearly. We concluded that extramedullary fat cells represent lipometaplasia in ILBs with no blood supply. We considered that lipomembranous changes in ILBs can be a useful hallmark for necrotic bone marrow or necrosis of extramedullary lipometaplastic lesions.
