The Effect of Group Sexual Counseling Based on Cognitive Behavioural Approach on Sexual Satisfaction of Women

Introduction: Sexual satisfaction is an important factor in the prevention of sexual dysfunction in women. It is an important factor in the healthy emotional and physiological aspects of sexual relationship. Aim: The aim of this study was to assess the effect of sexual counselling by cognitive behavioural approach on sexual satisfaction of women. Materials and Methods: This randomised clinical trial was conducted in two groups with pre-test and post-test on 198 women referred to selected health centres of Hamadan, 2016. The centres were randomised and taken as experimental and control group, three centres were selected as a control group and three as experimental group. The participants were selected by simple random sampling. The intervention group received four sessions (two hours) of group sexual counselling. Before and after the intervention, the demographic and sexual satisfaction questionnaires were completed. Results: After intervention, there were significant difference between two groups regarding sexual satisfaction (p<0.001). The mean score of the experimental group significantly decreased (p<0.001) and the mean score of sexual dissatisfaction in the control group didn’t changed significantly (p=0.625). Conclusion: The research findings showed that group sexual counseling based on CBT had an effect on the sexual satisfaction and increased women’s sexual satisfaction. © 2018, Journal of Clinical and Diagnostic Research. Postpartum Sexual Function; Conflict in Marriage Stability: A Systematic Review

Background: One of the most important issues affecting the stability of marriage is sexual function, so its problem can lead to divorce or separation of the couple. Pregnancy and delivery as one the most important periods of women's life can have significant effects on sexual function. This study reviews the postpartum sexual function and its related factors in Iran.Methods: This study is a systematic review of the sexual function after childbirth in Iran. By using of valid keywords and searching in databases such as Google scholar, SID, Magiran, Medlib, Irandoc, Iranmedex, the total number of 15 articles between 2005 and 2012 years have been evaluated. Results were reported quantitatively and qualitatively.Results: Total Sample was 4109 women, with an average of 274 samples per study. Plenty of studies in Tehran was 46% and other cities was 54%. The majority of studies showed no relation between mode of delivery and sexual function (P=0.14), but there were significant relation between lactation and postpartum sexual function (P<0.05) as, breastfeeding decreased sexual function. Also sexual function score has decreased with increasing parity.Conclusion: According to the effects of lactation and parity on women sexual function, therefore high risk for divorce, sex education after childbirth, especially in the first six months after delivery, maybe helpful in prevention of sexual dysfunction after delivery.

Post-treatment surveillance in locally advanced squamous cell carcinoma (SCCA) of the anal canal: A global subspecialist survey.

573 Background: Squamous cell carcinoma of the anal canal will be diagnosed in 28,000 individuals globally. Considered a rare cancer, the incidence of anal carcinoma continues to rise annually. Majority of patients present with locally advanced disease in which concurrent chemoradiation (CRT) will be provided with curative intent. The Anal Cancer IRCI Working Group was created to pursue novel approaches to the diagnosis, treatment, and surveillance of patients. Yet, surveillance following completion of CRT is not well established and varies as per NCCN (v2.2015) and ESMO guidelines. Methods: A 22-question survey provided to anonymous international GI subspecialists in various fields. Questions pertained to timing and type of diagnostic imaging, surveillance interval, approach to detection of complete response (CR), and prevention of sexual dysfunction. Results: Survey distribution was initiated in March 2015 and still ongoing. Thus far, 96 physicians responded. Conclusions: Carcinoma of the anal canal continues to rise in incidence annually. Surveillance following completion of CRT is warranted; however, global variability in practice patterns for surveillance exists. As such, the IRCI Anal Cancer Working Group intends to provide an international consensus surveillance statement. [Table: see text]

Current approach to episiotomy: Inevitable or unnecessary?

AbstractAn episiotomy is a surgical incision through the perineum made to provide sufficient area for the delivery of the infant. About 10 to 95% of women who deliver will have episiotomy depending on which part of the world they are having delivery.Too early episiotomy causes extensive bleeding and too late episiotomy causes the excessive stretching of a pelvic floor and lacerations could not be prevented.According to widely accepted arguments, there are many benefits of episiotomy for the neonate: prevention of injuries, shoulder dystocia and mental retardation of the infant. Benefits for the mother are: reduction of severe lacerations, prevention of sexual dysfunction, prevention of urinary and fecal incontinence. But those things could also be complications of episiotomy, if it is being used nonrestrictively. Some other complications are also extensive bleeding, hematoma or infection.There are many different opinions in literature about using episiotomy restrictively or routinely, so it is the right doubt arisen: is it inevitable or unnecessary?There is a wide variation in episiotomy practice, and the decision of performing it or not depends of actual clinical situation.There is still a great need for continuous obstetrics education according to the evidence based guidelines for the patient’s safety.

Additional publications were identified from the bibliographies of these articles. Randomized controlled trials were considered evidence of the highest quality, followed by cohort studies.Grey (unpublished) literature was identified through searching the websites of health technology assessment and health technology assessment-related agencies, clinical practice guideline collections, clinical trial registries, and national and international medical specialty societies. ValuesThe quality of evidence was rated using the criteria described in the Report of the Canadian Task Force on Preventive Health Care (Table). Recommendations1.Vaginal hysterectomy is generally considered the first choice of surgical approach for most benign indications for hysterectomy, as it is associated with lower rates of morbidity, fewer postoperative complications, and a faster recovery time than abdominal hysterectomy. (I-A)2.Women contemplating a vaginal, laparoscopic, or abdominal hysterectomy for the management of benign uterine disease should be reassured that hysterectomy is usually associated with improved quality of life, including improved sexual function, whether or not the cervix is removed. (I-B)3.Supracervical hysterectomy should not be recommended as a superior technique to total abdominal hysterectomy for the prevention of postoperative lower urinary tract symptoms. (I-B)4.Although supracervical hysterectomy may be associated with less blood loss and a shorter surgical time, these parameters have not been found to be clinically significant, and supracervical hysterectomy should not be recommended as a superior technique to total abdominal hysterectomy for the prevention of peri- and postoperative complications. (I-B)5.Women considering a supracervical hysterectomy should be counselled that they may continue experiencing cyclic vaginal bleeding following the surgery. A systematic review on clinical management of antipsychotic-induced sexual dysfunction in schizophrenia

Sexual dysfunction frequently occurs in patients with schizophrenia under antipsychotic therapy, and the presence of sexual side effects may affect compliance. The aim of this study was to review and describe clinical findings relating to the appropriate management of such dysfunctions. The research was carried out through Medline (from 1966 to March 2005), PsycInfo (from 1974 to March 2005), and Cochrane Library (from 1965 to March 2005) and included any kind of study, from case reports to randomized trials. The most common sexual dysfunctions found in the literature were libido decrease, difficulties in achieving and maintaining erection, ejaculatory dysfunction, orgasmic dysfunction, and menstrual irregularities. Thirteen papers were found: eight of them were open-label studies, four were descriptions of cases, and only one was a randomized clinical trial. All of them were short-term and had small sample sizes. The agents used were: bromocriptine, cabergoline, cyproheptadine, amantadine, shakuyaku-kanzo-to, sildenafil and selegiline. There was no evidence that those agents had proper efficacy in treating the antipsychotic-induced sexual dysfunction. An algorithm for managing sexual dysfunction induced by antipsychotics is suggested as a support for clinical decisions. Since the outcome from schizophrenia treatment is strongly related to compliance with the antipsychotics, prevention of sexual dysfunction is better than its treatment, since there is a scarcity of data available regarding the efficacy of intervention to deal with these problems.

Styles of sexual expression in women: clinical implications of multivaritanalyses.

Two different multivariate techniques were applied to self-reported trait and behavioral data concerning sexual behavior from a sample of North American women. Canonical correlation delineated three styles in which certain demographic attributes and cognitive indices of sexual arousal were related to forms of sexual expression. The first style was characterized by respondents who experienced high rates of intercourse and orgasm in a cohabitation arrangement but who did not necessarily report satisfaction with their sexual responsivity. A second style was characterized by older women currently without partners who expressed a liking for erotic literature and media, direct genital stimulation, and frequent masturbation but who were dissatisfied with their sexual responsivity. The third style, which included respondents most satisfied with their sexual responsivity, was characterized by women who were more aware of physiological changes during sexual arousal and who enjoyed gently seductive erotic activities, breast stimulation, and genital stimulation. A discriminant analysis showed that those women who were most highly satisfied with their sexual responsivity experienced frequent intercourse and orgasm consistency, enjoyed gently seductive erotic activities and breast stimulation, but were unresponsive to erotic literature and media. A second discriminant analysis showed that women who achieved orgasm most consistently were older, were more aware of physiological changes during sexual arousal, reported higher frequencies of masturbation and intercourse, and were less likely to be aroused by erotic preliminaries. These results are discussed within the context of the literature on the treatment and prevention of sexual dysfunction.
