264. SIGNIFICANCE AND USEFULNESS OF PLASMA SUBSTANCE P CONCENTRATION IN PREDICTING HIGH-RISK PATIENTS FOR ASPIRATION AFTER ESOPHAGEAL CANCER SURGERY

Abstract Background The greatest challenge in postoperative complications of esophageal cancer is the prevention of pulmonary complications, especially aspiration pneumonia. Although the introduction of minimally invasive surgery and intervention in breathing and swallowing rehabilitation can improve the motor function of breathing and swallowing, we reported the possibility that a decline in the swallowing reflex may play a significant role in esophageal cancer patients, many of whom are elderly. In fact, about 80% of esophageal cancer patients in Japan are over 60 years old, and 53% of them have asymptomatic cerebral infarction, about half of which are reported to be found near the basal ganglia, which control the swallowing reflex. The swallowing reflex is controlled by the secretion of substance P, synthesized in the cervical sympathetic ganglia, into the pharynx and trachea upon stimulation of dopamine synthesized in the basal ganglia. Methods To test the hypothesis that patients with preoperative subclinical cerebral infarction are more likely to develop aspiration postoperatively due to decreased substance P secretion. To compare the association between the presence or absence of cerebral infarction near the basal ganglia and serum substance P levels and the development of postoperative aspiration or aspiration pneumonia due to videoendoscopy and video fluorography in patients with thoracic esophageal cancer scheduled for video-assisted esophagectomy by preoperative brain MRI imaging and blood sampling. Results We report the results of an interim analysis of 54 cases in which the measurement of substance P was completed. Male: 39 cases, female: 15 cases. Mean age: 69.3 (49-82) years. Tumor location: Ut/Mt/Lt=13/25/14. MRI showed infarction in or near the basal ganglia in only 2 cases. Median preoperative blood substance P concentration: 48.2 pg/ml. Eleven one patients had postoperative aspiration. Preoperative substance P levels ≥48.2 pg/ml were defined as the High group, and those below 48.2 pg/ml were defined as the Low group. Postoperative aspiration was observed in 2/27 (7.4%) patients in the High group, while 9/26 (34.6%) patients in the Low group, which was significantly higher (p=0.015). Conclusion In fact, subclinical cerebral infarction was much less common than reported and was not useful as a predictor of postoperative aspiration. On the other hand, patients with low preoperative blood substance P levels had a very high risk of developing aspiration postoperatively due to impaired swallowing reflexes, suggesting that preoperative blood substance P levels may be useful in assessing postoperative aspiration risk in patients with thoracic esophageal cancer.

Prevention of pulmonary complications in sedated patients undergoing interventional procedures in the nonoperating room anesthesia setting.

Nonoperating room anesthesia (NORA) procedures have expanded in number, variety, and complexity. NORA involves all age groups, including frail older adults and patients often considered too sick to tolerate traditional surgical interventions. Postoperative pulmonary complications are a significant source of adverse events in the perioperative setting. We present a review focused on preventing pulmonary complications in the interventional NORA setting. NORA locations should function as independent, autonomous ambulatory units. We discuss a strategic plan involving a thorough preoperative evaluation of patients, including recognizing high-risk patients and their anesthetic management. Finally, we offer guidance on the challenges of conducting sedation and anesthesia in patients with coronavirus disease 2019 (COVID-19) or a history of COVID-19. The demands on the interventional NORA anesthesia team are increasing. Strategic planning, checklists, consistent staffing assignments, and scheduled safety drills are valuable tools to improve patient safety. In addition, through quality improvement initiatives and reporting, NORA anesthetists can achieve reductions in periprocedural pulmonary complications.

Algorithm for the prevention of pulmonary complications after thoracic surgeries

Annotation. The biggest problem after operations on the thoracic cavity is the number of pulmonary complications (according to various authors 26-38% of cases). Despite the development of numerous methods for the prevention and treatment of these complications, they remain one of the leading causes of hospital mortality and may be an independent risk factor for worse long-term survival. Therefore, the aim of the work was to improve the results of treatment of patients after operations on the thoracic cavity by developing an algorithm for anesthesia for perioperative prevention of pulmonary complications. Participated in the study 192 patients with diseases of the thoracic cavity (esophagus, lungs, mediastinum), operated on in the thoracoabdominal department of the National Institute of Surgery and Transplantology named after O.O. Shalimov. Retrospective comparison group - 96 patients after thoracic surgery, which used conventional methods of perioperative management. The study group consisted of 96 patients after thoracic surgery, who used a perioperative anesthesia algorithm for the prevention of pulmonary complications. Statistical software EZR v. Was used for statistical calculations. 1.54 (graphical user interface for statistical software R version 4.0.3, R Foundation for Statistical Computing, Vienna, Austria). When conducting a multifactor analysis, it was found that the proposed method of prevention of pulmonary complications can reduce (p=0.001) the risk of development, HS=0.27 (95% CI 0.13-0.58) compared with the control group (with standardization by sex, weight, ASA of the patient). Pulmonary complications developed in 33 (34.4%) patients of the control group and in 13 (13.5%) patients of the study group, the difference was statistically significant, p=0.001. Thus, the application of the proposed technique reduces (p = 0.001) the risk of complications, BP=0.39 (95% CI 0.22 - 0.70) compared to traditional methods.

IMPACTO DA ESPIROMETRIA DE INCENTIVO NA REDUÇÃO DE COMPLICAÇÕES RESPIRATÓRIAS NO PÓSOPERATÓRIO DA LAPAROTOMIA: REVISÃO SISTEMÁTICA

Contexto: Laparotomia é uma cirurgia que envolve uma incisão na parede abdominal para aceder à cavidade abdominal. O benefício da implementação de exercícios respiratórios profiláticos, na prevenção de complicações pulmonares após cirurgia abdominal é controverso. Objetivo: Analisar a evidência acerca do efeito do inspirómetro de incentivo na redução de complicações no pós-operatório de pacientes submetidos a laparotomia. Metodologia: Foram incluídas nas pesquisas diversas bases de dados: CINAHL Plus with Full Text, PubMed e Scopus, assim como literatura cinzenta para estudos publicados desde 2008 comparando pacientes adultos submetidos a laparotomia com inspirómetro de incentivo como profilaxia no pós operatório e pacientes adultos submetidos a cirurgia abdominal sem exercícios respiratórios no pós operatório e também pacientes adultos submetidos a laparotomia com inspirómetro de incentivo como profilaxia no pós operatório e pacientes adultos submetidos a cirurgia abdominal com outro tipo de exercícios respiratórios no pós operatório. Resultados: Dos 160 estudos analisados, 1 estudo prospetivo randomizado foi incluído. Este concluiu que não há diferença significativa entre a utilização de inspirómetro de incentivo e pressão positiva intermitente. Conclusão: Apesar da eficácia do inspirómetro de incentivo ter sido demonstrada, é necessária mais investigação para explorar o seu impacto nos pacientes submetidos a cirurgia abdominal. Physiotherapeutic procedures used in patients after surgical treatment of maxillofacial and occlusal disorders

The treatment of patients with craniofacial defects is interdisciplinary and long term. In order to restore the function of the stomatognathic system (chewing, swallowing, speech, breathing) and to improve the aesthetic appearance of the face, patients with malformations and acquired defects require combination therapy. It is necessary to cooperate with a surgeon, orthodontist, phoniatrist, speech therapist, prosthetist, psychologist and physiotherapist. The earlier treatment of facial cranial defects is started, the better final results can be obtained. The aims of rehabilitation are prevention of pulmonary complications, prevention of deep vein thrombosis, restoration of the efficiency of the stomatognathic system, removal of functional and organic consequences of surgery. The implementation of these goals begins on the first day after surgery and continues until the patient is discharged from the hospital. The next step is ambulatory rehabilitation.

Impact of laparoscopy on the prevention of pulmonary complications after thoracoscopic esophagectomy using data from JCOG0502: a prospective multicenter study

BackgroundPostoperative pulmonary complications (PPCs) are the most common causes of serious morbidity after esophagectomy, which involves both thoracic and abdominal incisions. Although the thoracoscopic approach decreases PPC frequency after esophagectomy, it remains unclear whether the frequency is further decreased by combining it with laparoscopic gastric mobilization. This study aimed to determine the impact of laparoscopy on the prevention of PPCs after thoracoscopic esophagectomy using data from the Japan Clinical Oncology Group Study 0502 (JCOG0502).MethodsJCOG0502 is a four-arm prospective study comparing esophagectomy with definitive chemo-radiotherapy. The use of thoracoscopy and/or laparoscopy was decided at the surgeon’s discretion. PPCs were defined as one or more of the following postoperative morbidities grade ≥2 (as per Common Terminology Criteria for Adverse Events v3.0): pneumonia, atelectasis, and acute respiratory distress syndrome.ResultsA total of 379 patients were enrolled in JCOG0502. Of these, 210 patients underwent esophagectomy via thoracotomy with laparotomy (n = 102), thoracotomy with laparoscopy (n = 7), thoracoscopy with laparotomy (n = 43), and thoracoscopy with laparoscopy (n = 58). PPC frequency was reduced to a greater extent by thoracoscopy than by thoracotomy (thoracoscopy 15.8%, thoracotomy 30.3%; p = 0.015). However, following thoracoscopic esophagectomy, laparoscopy failed to further decrease the PPC frequency compared with laparotomy (laparoscopy 15.5%, laparotomy 16.3%; p = 1.00). Univariable analysis showed that thoracoscopy (shown above) and less blood loss (<350 mL 16.3%, ≥350 mL 30.2%; p = 0.022) were associated with PPC prevention, whereas laparoscopy showed a borderline significant association (laparoscopy 15.4%, laparotomy 26.9%; p = 0.079). Multivariable analysis also showed that thoracoscopy and less blood loss were associated with PPC prevention.ConclusionThoracoscopic approach to esophagectomy significantly reduced PPC frequency with minimal additional effect from laparoscopic gastric mobilization.

Noninvasive Ventilation During Immediate Postoperative Period in Cardiac Surgery Patients: Systematic Review and Meta-Analysis.

ObjectiveTo verify the effectiveness of noninvasive ventilation compared to conventional physiotherapy or oxygen therapy in the mortality rate and prevention of pulmonary complications in patients during the immediate postoperative period of cardiac surgery.MethodsSystematic review and meta-analysis recorded in the International Prospective Register of Ongoing Systematic Reviews (number CRD42016036441). The research included the following databases: MEDLINE, Cochrane Central, PEDro, LILACS and manual search of the references of studies published until March 2016. The review included randomized controlled trials with patients during the immediate postoperative period of cardiac surgery, which compared the use of noninvasive ventilation, BiLevel modes, continuous positive airway pressure, intermittent positive pressure breathing and positive pressure ventilation with conventional physiotherapy or oxygen therapy, and assessed the mortality rate, occurrence of pulmonary complications (atelectasis, pneumonia, acute respiratory failure, hypoxemia), reintubation rate, ventilation time, time spent in the intensive care unit (ICU), length of hospital stay and partial pressure of oxygen.ResultsAmong the 479 selected articles, ten were included in the systematic review (n=1050 patients) and six in the meta-analysis. The use of noninvasive ventilation did not significantly reduce the risk for atelectasis (RR: 0.60; CI95% 0.28-1.28); pneumonia (RR: 0.20; CI95% 0.04-1.16), reintubation rate (RR: 0.51; CI95%: 0.15-1.66), and time spent in the ICU (-0.04 days; CI95%: -0.13; 0.05).ConclusionProphylactic noninvasive ventilation did not significantly reduce the occurrence of pulmonary complications such as atelectasis, pneumonia, reintubation rate and time spent in the ICU. The use is still unproven and new randomized controlled trials should be carried out.

Effects of preoperative respiratory muscle exercise on the prevention of postoperative pulmonary complications in patients with abdominal operation: a Meta-analysis

Objective To evaluate the effect of preoperative respiratory muscle exercise on the prevention of postoperative pulmonary complications in patients with abdominal operation. Methods The published randomized control trails (RCT) on preoperative respiratory muscle exercise for the prevention of pulmonary complications after abdominal operation were collected in the databases of PubMed, the Cochrane Library, Chinese BioMedical Literature Database (CBM) by computer from January 1, 1975 to March 1, 2016. Two researchers extracted the data and assessed the quality of studies according to the revised edition of Jadad scale independently. Then meta-analyses were performed on included studies. Results A total of 6 RCTs involving 385 patients were included. The results of meta-analyses showed that the preoperative respiratory muscle exercise could effectively decrease the incidence of pulmonary infection [OR=0.38, 95%CI (0.16, 0.91), P=0.03], pulmonary atelectasis [OR=0.17, 95%CI (0.05, 0.56), P=0.004]. Conclusions The preoperative respiratory muscle exercise can effectively decrease the incidence of pulmonary infection and pulmonary atelectasis. While providing reserve time for dificult airway management, preoxygenation with pure oxygen increases the risk of pulmonary complications due to absorption atelectases. The authors explored when it could be appropriate to prevent atelectases by preoxygenation with decreased FiO₂. ASA I-II elective gynecological surgery patients were randomized among five groups (n = 22 each) with preoxygenation using FiO₂ 100, 70, 60, 60% + PEEP 5 mbar and 50%. Even FiO₂ 70% led to decrease. in safe apnea time (i.e. time interval to Sp²O₂ 95%) by two, while FiO₂ 50% - by more than three times. Furthermore, in five similar additional groups of women with same techniques ofpreoxygenation (n = 10 each) it was shown that for FiO₂ 5 70% very fast pattern of SpO2 fall after the first change ofpulseoxymeter figure (100% by 99%) is typical: interval to SpO2 90% was less than 1 min, while for FiO₂ 100% it lasts for 200 s. Since critical problem is "Cannot intubate, cannot ventilate", the authors tried to focus on the difficultfacemask ventilation prognosis. In the group of 71 elective general surgery patients (31 males, 40 females, ASA I-III) original prognostic model based on seven simple bedside tests (removable dentures, beard, snoring, Mallampati class 2-4, age > 50 y.o., BM > 30 kg/m², sternomental distance < 12 cm) demonstrated the reliability of difficult facemask ventilation negative prognosis of 97,5%. The authors suggest that only in patients with reliable prognosis of easy facemask ventilation prevention ofpulmonary complications by preoxygenation with FiO₂ 50-60% could be safely recommended.

Effects of Iron Chelators on Pulmonary Iron Overload and Oxidative Stress in β-Thalassemic Mice

Aim: To evaluate the effect of iron chelators on iron-related pulmonary pathology and oxidative stress in an animal model of β-thalassemia. Methods: Pulmonary iron overload was induced in heterozygous β-globin knockout mice (^muβ^th-3/+, BKO). Over a period of 2 weeks, 180 mg of iron/mouse was loaded by intraperitoneal injection of iron dextran, and subsequently treated daily via intraperitoneal with either deferoxamine (DF) or deferiprone (L1) at an equimolar concentration of iron binding (0.2 and 0.6 μmol/g body weight, respectively) for 7 days. Results: Iron loading resulted in iron deposition in peribronchial regions, septa and also in alveolar macrophages with a grading score of 3. This iron burden resulted in lung epithelial injuries, fibrosis and corresponded with increased lipid peroxidation and decreased tissue catalase activity. Treatment with DF or L1 resulted in a reduction of iron-laden alveolar macrophages and decreased oxidative stress and tissue damage, showing the iron mobilizing ability of both compounds. Conclusion: Iron chelation therapy, with DF and L1, may protect against pulmonary damage by sequestering catalytic iron and improving oxidative status. It may be beneficial in the prevention of pulmonary complications in thalassemia.

A Review of the Impact of Preoperative Chemoradiotherapy on Outcome and Postoperative Complications in Esophageal Cancer Patients

Preoperative chemoradiotherapy has emerged in the treatment of esophageal cancer as a means to down-stage tumors, improve local control, and possibly improve overall survival. However, there are concerns that postoperative complications may be increased by preoperative chemoradiotherapy. We review the rationale for preoperative chemoradiotherapy. We review the literature to identify the potential postoperative complications, the risk of complications, and the risk factors for complications. Although individual and previous studies have shown an increased risk of postoperative complications, the 4 most recent randomized trials published after the year 2000 have not shown an increase in postoperative complications and mortality rates in patients treated with preoperative chemoradiation compared with patients treated with surgery alone. Pulmonary complications are frequently reported, and we focus on dosimetric factors that can be used to minimize lung toxicity. Several dose-volume-histogram parameters, including V10≥40%, V15≥30%, V20≥20%, have been shown to correlate with 32% to 35% of pulmonary complications including pneumonia and acute respiratory distress syndrome. More recent evidence has suggested that an absolute volume of lung spared doses of > 5 Gy (VS5) correlates with pulmonary complications. As these data show, low-dose volume may be more important in the prevention of pulmonary complications than high-dose volume. These dosimetric constraints can be used by physicians to prevent postoperative pulmonary complications in patients treated with preoperative chemoradiotherapy.

The effect of non-invasive positive airway pressure therapy following thoracic surgical procedures: protocol for a systematic review.

BackgroundThoracic surgical procedures impair respiratory function, decreasing ventilation and oxygenation and increasing the risk of acute respiratory failure and pulmonary complications. To prevent these clinical repercussions, positive airway pressure therapy is widely used to increase pulmonary ventilation, decrease muscle overload, and ensure adequate oxygenation; however, the benefit of this therapy remains unclear.Methods/designA systematic search of the literature including PubMed, CINAHL, AMED, PsycINFO, LILACS, Scielo, Scopus, PEDro, and the Cochrane Library will identify the randomized and quasi-randomized trials that used CPAP, Bilevel, or IPPB compared with a control without intervention, a sham treatment or other lung expansion techniques following thoracic surgical procedures. From these trials, we will extract data on a predefined list of outcomes, including oxygenation, ventilation, respiratory failure, pulmonary complications, and time of resolution of the clinical condition. The methodological quality of each trial included will be assessed using the PEDro scale. The strength of the recommendations will be summarized using the GRADE scale. Meta-analyses will be performed, if appropriate.DiscussionThis review aims to promote greater knowledge regarding the efficiency of the use of non-invasive positive airway pressure on recovery of respiratory function and on prevention of pulmonary complications following thoracic surgical procedures. Effect of respiratory rehabilitation before open cardiac surgery on respiratory function: a randomized clinical trial.

Introduction: Prevention of pulmonary complications after coronary artery bypass graft is attended as a very important issue. The aim of this study was to evaluate the role of pulmonary rehabilitation before surgery for reducing the risk of pulmonary complications after surgery. Methods: In a randomized clinical trial, 60 patients undergoing heart surgery were randomly divided into two groups A and B. Chest physiotherapy was performed before and after surgery on group A patients however it was done on group B’s, only after surgery. Effects of preoperative pulmonary rehabilitation were compared between two groups, using spirometry and arterial blood gas (ABG). Results: Thirty nine males (65%) and 21 females (35%) with mean age of 8.10 ± 9.56 were analyzed. The mean differences were statistically significant for predicted forced vital capacity (FVC) (CI 95%:1.3 to 8.7) and Predicted Peak Flow indices (PEF) (CI 95%: 1.9 to 9.4) of spirometry indicator, PCO2 index (of ABG parameter) (CI 95%: 1.4 to 8.9) and mean oxygen saturation (mean Spo2) (CI 95%: 0.6 to 1.7) of ABG index in two groups. Conclusion: The performance of pulmonary rehabilitation program before surgery is recommended, as it may result in the reduction of complications of heart surgery

Efficacy of ambroxol combined with penehyclidine hydrochloride for prevention of pulmonary complications after surgery due to acute abdomen in pediatric patients with upper respiratory tract infection

Objective To investigate the efficacy of ambroxol combined with penehyclidine hydrochloride for prevention of pulmonary complications after surgery due to acute abdomen in pediatric patients with upper respiratory tract infection.Methods Fifty pediatric patients with acute abdomen complicated with respiratory infection,of ASA physical status Ⅰ or Ⅱ,aged 6 months-5 yr,were randomly divided into test group (group A) and control group (group B),with 25 cases in each group.Immediately after admission to the operating room,ambroxol 1 mg/kg and penehyclidine hydrochloride 0.02 mg/kg were injected intravenously in group A,and atropine 0.02 mg/kg was given in group B.Extubation time and development of laryngospasm,bronchospasm and hypoxemia were recorded.The patients were followed up for 72 h after surgery and the development of postoperative pulmonary complications was recorded.Results Compared with group B,the incidence of hypoxemia and pulmonary complications was significantly decreased,and extubation time was shortened in group A.Conclusion Ambroxol combined with penehyclidine hydrochloride can prevent pulmonary complications after surgery due to acute abdomen and is helpful for prognosis in pediatric patients with upper respiratory tract infection. Pulmonary Complication of Liver Cirrhosis

Purpose: We report a case of hepatic hydrothorax; one of the rare pulmonary complications of liver cirrhosis. Case Report: A 69-year-old female with significant past medical history of primary biliary cirrhosis presented with increased shortness of breath with decreased exercise tolerance for six months, which was associated with loss of appetite and increased abdominal girth. She was a non-drinker, non-smoker and never use any illicit drugs. She had a history of upper GI bleed secondary to esophageal varices and Porto-hypertensive gastropathy. Physical examination was significant for decreased air entry at left lower and mid zone, and right lower zone. She also had mild abdominal distension with periumbilical hernia and was positive for free fluid. She did not have lower extremity edema. She had obstructive pattern abnormal LFT with normal renal function. CXR as well as CT chest showed bilateral moderate size pleural effusion with no active lung disease. Diagnostic thoracentesis was done, which showed transudate type pleural fluid. Echocardiogram was done to rule out underlying cardiac cause, which showed normal EF of 55-60%. She had therapeutic thoracentesis for symptom control and then started on diuretic therapy. Discussion: Hepatic hydrothorax is one of the rare pulmonary complications of liver cirrhosis. It is pleural effusion, usually >500 ml in patients with cirrhosis and without primary cardiac, pulmonary or pleural disease. Pathophysiology is incompletely understood, probably due to the passage of ascites from the peritoneal to the pleural cavity through small diaphragmatic defects located in the tendinous portion of the diaphragm. A rare but important complication of hepatic hydrothorax is acute tension hydrothorax, which may present with severe dyspnoea with hypotension and SBEM [spontaneous bacterial empyema], in which case mortality is 20%, even with antibiotic treatment. The main goal of treatment is symptom control with sodium restriction and diuretics, and prevention of pulmonary complications until liver transplantation can be performed. In case of refractory hydrothorax, i.e., persistent hydrothorax despite diuretic and those who developed diuretic related complication thoracentesis can be done for symptom control.

Breathing Exercises with Positive Expiratory Pressure after Abdominal Surgery–The Current Physical Therapy Practice in Sweden

Objectives: In Sweden breathing exercises with Positive Expiratory Pressure (PEP) are commonly recommended for the prevention of pulmonary complications after abdominal surgery. Scientific documentation of the effects of PEP treatment is limited. The aim of this national survey was to describe the current physical therapy practice of PEP treatment after abdominal surgery in Sweden. Methods: A questionnaire was sent by e-mail to the 45 physical therapists who work with abdominal surgery patients in all seven university hospitals in Sweden. The questionnaire contained questions about the usage of PEP after abdominal surgery. Results: In total, 24 (54%) of the physical therapists answered the questionnaire. All reported using PEP as a treatment option after abdominal surgery. The most commonly used PEP device was the Blow bottle system and the PEP ventil system connected to a mouthpiece. Recommendations regarding treatment frequency and implementation varied significantly across respondents. The number of breaths per treatment varied considerably. Conclusion: All respondentsreported using PEP as a postoperative treatment on abdominal surgery wards. The treatment is most often recommended hourly during the first postoperative days. The common first-choice PEP devices were the Blow bottle system, Pep/Rmt set with mouthpiece or mask, Breathing exerciser/PEP valve system 22, and the Mini-PEP.

Effect evaluation of evidence-based nursing on prevention of pulmonary complications in postoperative elderly lung cancer patients

Objective To discuss the effect of evidence-based nursing on prevention of pulmonary complications in postoperative elderly lung cancer patients.Methods Twenty elderly lung cancer patients from January to June 2010 were randomly divided into the observation group and the control group with 10 patients in each group.The control group was given regular care methods.The observation group received evidence-based clinical nursing methods.The preventive effect of pulmonary complications was observed in both groups.Results One case was found to have pulmonary complications in the observation group,while four cases in the control group.The total cure rate of the observation group was higher than that of the control group.Conclusions Evidence-based nursing methods can satisfy the medical demand of patients and their family members to the utmost degree,reduce the incidence of pulmonary complications and contribute to the early recovery of patients.It is worthy of clinical application. Perioperative Management of Lung Resection Patients

Lung resection has various and commonly occurring postoperative complications. Pulmonary complication is well known as one of the most important among them, exerting a negative influence on the postoperative course and resulting in mortality. Thus, the prevention of pulmonary complication after lung resection is very important. To prevent postoperative pulmonary complication, the perioperative management must be optimal. Perioperative management begins long before the surgery and does not end until the patient leaves the hospital. The goal of perioperative management is to identify the high-risk patients, to provide appropriate intervention, to prevent postoperative complications, and to obtain the best outcomes.

Fisioterapia respiratória em crianças com doença falciforme e síndrome torácica aguda

OBJETIVO: Realizar uma revisão sistemática da literatura médica para identificar as técnicas de fisioterapia respiratória aplicadas em crianças com doença falciforme e síndrome torácica aguda, bem como descrever seu nível de evidência e recomendação. FONTES DE DADOS: Revisão bibliográfica nos bancos de dados Medline, Lilacs, SciELO e Cochrane no período de 1995 e 2009, com os descritores: "doença falciforme", "síndrome torácica aguda", "fisioterapia", "criança", "inspirometria de incentivo", em português e inglês, excluindo-se os estudos de revisão. Os artigos foram classificados por nível de evidência. SÍNTESE DOS DADOS: Foram encontrados cinco artigos; destes, três utilizaram a inspirometria de incentivo e observaram que ela evita as complicações pulmonares associadas à síndrome torácica aguda (nível de evidência II, II e IV), um deles (evidência II) comparou a inspirometria de incentivo com o dispositivo de pressão expiratória, sem diferenças entre ambos. Um artigo utilizou uma rotina de cuidados, incluindo a inspirometria de incentivo (evidência V), e observou redução do tempo de internação hospitalar e do uso de medicação oral para dor. Outro estudo com a ventilação não invasiva para crianças com desconforto respiratório e com incapacidade de realizar a inspirometria de incentivo relatou melhora da oxigenação e do desconforto respiratório (nível de evidência V). CONCLUSÕES: As técnicas de fisioterapia respiratória com dispositivos de inspirometria de incentivo, de pressão expiratória e a ventilação não invasiva podem ser aplicadas em crianças com doença falciforme e síndrome torácica aguda; o nível de recomendação é C.

Prevention of Pulmonary Complications After Aortic Surgery: Evaluation of Prophylactic Noninvasive Perioperative Ventilation

In 40% of the cases, pulmonary complications are encountered after aortic surgery in patients suffering from chronic obstructive broncho-pneumopathy (COBP). The factors aggravating the occurrence of these complications are cumulated tobacco addiction of ≥40 packets per year and surgery for aneurysm. The aim of our prospective study was to evaluate the effect of pre- and postoperative noninvasive ventilation (NIV) preparation on the respiratory function of patients presenting with high pulmonary risks. Between September 2007 and May 2010, 30 patients were included in the present study. Inclusions criteria were male gender, patients suffering from COBP with tobacco addiction of ≥40 packets per year, and aortic surgery for aneurysm with or without occlusive lesions. In all, 14 of the 15 patients had effectively performed the preparation before and after surgery according to a fixed protocol (NIV group). We compared the respiratory rate complications and the mean hospital length of stay in intensive care with the non-NIV control group (15 patients). Mean age, severity of COBP according to the classification of the French Society for Pneumology, surgery duration, and blood losses were comparable between the two groups. Conversely, pulmonary complications were significantly lower in the NIV group (0/14 [0%] vs. 5/15 [33%], p = 0.004). Hospital length of stay in intensive care was shorter in the NIV group (2.5 vs. 6.5 days, p < 0.001). Preliminary results are encouraging, in favor of pulmonary preparation before aortic surgery with NIV at home, and carried on postoperatively in hospital for patients with COBP. These results prompt us to propose a multicenter study to validate these first observations.
