WHO method for estimating congenital syphilis to inform surveillance and service provision, Paraguay.

ProblemIn Paraguay, incomplete surveillance data resulted in the burden of congenital syphilis being underestimated, which, in turn, led to missed opportunities for infant diagnosis and treatment.ApproachThe prevalence of congenital syphilis, as defined by the World Health Organization (WHO), was estimated for Paraguay using the WHO congenital syphilis estimation tool. This tool was also used to monitor progress towards the elimination of mother-to-child transmission of syphilis.Local settingThe burden of syphilis in Paraguay has historically been high: its prevalence in pregnant women was estimated to be 3% in 2018.Relevant changesThe incidence rate of congenital syphilis estimated using the WHO tool was around nine times the reported prevalence. Subsequently, Paraguay: (i) provided training to improve diagnosis and case reporting; (ii) strengthened information systems for case monitoring and reporting; and (iii) procured additional rapid dual HIV–syphilis and rapid plasma reagin tests to increase syphilis testing capacity. In addition, the Ministry of Health prepared a new national plan for eliminating mother-to-child transmission of syphilis, with clear monitoring milestones.Lessons learntHealth-care providers’ reporting and surveillance procedures for congenital syphilis may not adequately reflect national and international case definitions. Use of the WHO congenital syphilis estimation tool in Paraguay drew attention to congenital syphilis as a national public health problem and highlighted the importance of comprehensive national surveillance systems and accurate data. Ongoing use of the WHO tool can track progress towards the elimination of mother-to-child transmission of syphilis by helping improve syphilis service coverage and national surveillance.

Gaps in the congenital syphilis prevention cascade: qualitative findings from Kern County, California

BackgroundCongenital syphilis is preventable through timely access to prenatal care, syphilis screening and treatment of pregnant women diagnosed as infected. In 2018, California had the second highest number of congenital syphilis cases in the United States (U.S.), a nearly twofold increase in cases since 2014. This study assessed gaps in preventing congenital syphilis in the high morbidity region of Kern County, California.MethodsBetween May 2018 and January 2019, we conducted five focus group discussions with pregnant/postpartum women and ten semi-structured interviews with prenatal care providers in Kern County. Focus group and interview data were recorded, transcribed, and analyzed to identify emergent themes pertaining to facilitators and barriers at each step (prenatal care, syphilis screening and treatment) in the congenital syphilis prevention cascade.ResultsGaps in congenital syphilis prevention discussed in focus group discussions with pregnant/postpartum women were related to limited prenatal care access, social-, economic-, and cultural-barriers, and substance use and co-occurring intimate partner/domestic violence. The gaps identified from interviews with prenatal care providers included social economic vulnerabilities of pregnant women and stigma and shame around the vulnerabilities, distrust in medical system, prenatal substance use, limited prenatal substance use disorder treatment facilities, and inadequate provider training on context-specific congenital syphilis management strategies. Gaps in partner notification, screening and treatment for syphilis were brought up by pregnant/postpartum women and prenatal care providers.ConclusionsCongenital syphilis continues to increase in Kern County and throughout the U.S. In high syphilis morbidity areas, comprehensive and tailored public health approaches addressing setting-specific gaps in prenatal screening and treatment are needed.

Characterization of reports of Congenital Syphilis in Pernambuco from 2018 to 2019

Syphilis is a Sexually Transmitted Infection (STIs) caused by the bacterium Treponema Pallidum, which can be controlled through effective public health actions and measures, due to effective and low-cost treatment. The congenital form of syphilis is a serious outcome, since most newborns have physical, sensory or developmental sequelae, when they do not result in fetal and perinatal loss. This is a public health problem that remains an important challenge in Brazil. To characterize the prevalence of congenital syphilis in the state of Pernambuco between 2018 and 2019. This is a descriptive, observational, quantitative study, using secondary data from the Database of the Unified Health System - DATASUS/Ministry of Health. Tabbed by TABNET. Through the database of the Notifiable Diseases Information System (SINAN), notifications for congenital syphilis in the state of Pernambuco were evaluated from 2018 to 2019. Background: Syphilis is one of the most serious sexually transmitted diseases worldwide, and has tremendous consequences for the mother and her developing fetus if left untreated. The burden of morbidity and mortality due to congenital syphilis is high. Purpose: Screening and prompt to know the importance of treatment of syphilis during pregnancy. Case: A 32-year-old multigravida in 5 months of pregnancy presented with multiple raised lesions over her labia. It was accompanied by fluor and smelly fishy odor. There was no history of genital ulcers in either spouse and no history of sexual promiscuity. History of antenatal care in public health showed non-reactive status of HIV, syphilis and hepatitis B. Clinical examination revealed multiple flat, moist warts over her labia mayora and minora, and multiple roseola syphilitica on the plantar pedis sinistra. Darkfield microscopic examination presence spirochete, venereal disease research laboratory (VDRL) titer was 1:16 and T. pallidum particle agglutination assay (TPHA) titer was 1:2560. Obstetric ultrasonography examination was suggestive no mayor congenital abnormalities. Both of serology VDRL and TPHA were non-reactive in her husband. Significant of lesion improvement and decrease a fourfold titer serologic in VDRL (1:4) and TPHA (1:320) as follow-up 3 months after being treated with single intra-muscular injections of benzathine penicillin 2.4 million units. Discussion: Coordinated prenatal care and treatment are vital. It’s implemented before the fourth month of pregnancy to reduce vertical transmission and all associated side effects of congenital syphilis. Penicillin is highly efficacious in maternal syphilis and prevention of congenital syphilis. Conclusion: Universal screening and adequate pregnancy care must be a priority.

A qualitative assessment of structural barriers to prenatal care and congenital syphilis prevention in Kern County, California.

Congenital syphilis is the result of placental transmission from mother to fetus of Treponema pallidum. Although congenital syphilis is preventable through timely treatment, the rate of new infections in the United States (US) has increased each year since 2013, and is increasing at a noticeably greater pace in California (CA). Most research into congenital syphilis has focused on individual psychosocial and behavioral factors that contribute to maternal vulnerability for syphilis. The aim of this study was to evaluate structural barriers to prenatal care access and utilization and congenital syphilis prevention in Kern County, CA. Transcripts from 8 in-depth interviews with prenatal care providers and 5 focus group discussions with 42 pregnant and postpartum persons were examined using thematic analysis. Structural barriers experienced by pregnant and postpartum persons to prenatal care access and utilization included (1) burdens of poverty; (2) stigma around substance use in pregnancy; (3) citizenship status; (4) lack of healthcare coverage; (5) low sexual health literacy; and (6) gender inequality Structural barriers experienced by prenatal care providers in congenital syphilis prevention included (1) limited guidance on clinical management of syphilis in pregnancy; (2) decay in public health infrastructure; and (3) inadequate support for managing patients’ social comorbidities. The response to congenital syphilis prevention will require an examination of the complex context of social determinants of health in which persons diagnosed with syphilis live in.

Syphilis remains a common congenital infection over the globe. There has been a tremendous rise in the number of congenital syphilis cases worldwide in the last 20 years. It affects large numbers of pregnant mothers in high burden regions causing a significantly high perinatal mortality and morbidity, which can be easily prevented by early antenatal screening and treatment.Diagnosis of maternal syphilis in pregnancy mainly based on clinical symptoms, serological tests and direct identification of treponemes in clinical samples. However, the diagnosis can be challenging due to the relapsing-remitting nature of the disease. The early stage of the infection is usually symptomatic which is commonly followed by an asymptomatic latent phase but infectious and as a result serological tests will be positive. The risk of transplacental transmission is high during the second and third trimester.Obstetric complications of syphilis include spontaneous miscarriage, non-immune hydrops, stillbirth, preterm labour, low birth weight, increased neonatal mortality and congenital syphilis among neonates.Penicillin is the drug of choice for treatment and should be commenced immediately. Babies born to mothers with syphilis should also be treated with penicillin. Early detection and prompt intervention are the key to the prevention and successful control of congenital syphilis. The aim of this review is to highlight the impact of syphilis infection on pregnancy and discuss the current trends in diagnosis and management of maternal and congenital syphilis.

Congenital syphilis in East Baton Rouge parish, Louisiana: providers\u2019 and women\u2019s perspectives

BackgroundCongenital syphilis is completely preventable through screening and treatment, but rates have been rising in the United States. Certain areas are at particularly high risk. We aimed to assess attitudes, knowledge, and barriers around effective prevention of congenital syphilis among health care providers and community women potentially at risk.MethodsTwo parallel studies were conducted: in-depth interviews with health care providers and focus groups with community women in the area of Baton Rouge, Louisiana. Each group was questioned about their experience in providing or seeking prenatal care, knowledge and attitudes about congenital syphilis, sources of information on testing and treatment, perceptions of risk, standards of and barriers to treatment. Results were transcribed into QSR NVivo V10, codes developed, and common themes identified and organized.ResultsProviders identified delays in testing and care, lack of follow-through with partner testing, and need for community connection for prevention, as major contributors to higher rates of congenital syphilis. Women identified difficulties in accessing Medicaid contributing to delayed start of prenatal care, lack of transportation for prenatal care, and lack of knowledge about testing and prevention for congenital syphilis.ConclusionsProviders and community members were in broad agreement about factors contributing to higher rates of congenital syphilis, although some aspects were emphasized more by one group or another. Evidence-based interventions, likely at multiple levels, need to be tested and implemented to eliminate congenital syphilis.

INTRODUCTION: The current epidemiological situation in Porto Alegre is worrying due to the high rates of vertical transmission of HIV, syphilis in pregnant women and congenital syphilis, making the evaluation of the prevalence of congenital syphilis in newborns exposed to HIV relevant. It is estimated that an analysis of the data found may lead to measures capable of assisting in the prevention of both infections. OBJECTIVES: To describe pregnancy coinfection HIV/syphilis data and the consequences to their newborns. METHODS: A retrospective cohort study of live births of HIV-positive mothers with and without gestational syphilis, born at Hospital de Clínicas of Porto Alegre from January 1st, 2013 to December 31th, 2017. RESULTS: A total of 440 newborns from HIV-positive mothers were divided into two groups: the first, with 392 infants of HIV-positive mothers without gestational syphilis, resulting in 3 HIV-infected infants (vertical transmission 0,7%); the second group, with 48 infants born from mothers with HIV/syphilis coinfection (prevalence of gestational syphilis 10,9%), with 64,6% needing hospitalization for congenital syphilis treatment, and 4 HIV-infected newborns (vertical transmission 8,3%). When compared the two groups, it was verified that this difference was statistically significant. CONCLUSIONS: The gestational and congenital syphilis prevalence in our hospital is alarming, and coinfection HIV/syphilis seems to be an independent risk factor to the vertical transmission of HIV.

Congenital infections (syphilis and human immunodeficiency virus) in a federal hospital in Rio de Janeiro

Introduction: Congenital infections are related to a higher risk of morbidity and mortality in the neonatal period and can cause serious complications in the newborn. Among the sexually transmitted infections (IST) present in this group, syphilis and vertical exposure to the human immunodeficiency virus (HIV) are still a challenge in the 21st century. Objective: To evaluate the prevalence of congenital syphilis and exposure to HIV among congenital infections. Methods: Cross-sectional, analytical study with data collected from medical records of newborns admitted to the Conventional Neonatal Intermediate Care Unit, Kangaroo and Joint Accommodation, in the database of the service of the Federal Hospital from Bonsucesso, from January 2015 to December 2018. Results: During the study, 2,202 newborns were discharged from the hospital and 474 were positive for congenital infection (21.8%). In cases of congenital infection, congenital syphilis (398–84%) and maternal HIV infection (40–8,4%) were the most frequently encountered clinical situations. The relationship between prenatal care and the absence of congenital infection was evident. Among the newborns, 117 (5.3%) were premature, 352 (16.3%) were older than 37 weeks and 95 (23.7%) had low birth weight. Conclusion: Congenital infection was one of the main causes of neonatal morbidity; syphilis and HIV had the highest prevalence, being associated with reducible deaths when adequate care is provided for women during pregnancy. Perinatology services demand a great effort in order to manage these avoidable and undesirable situations.

1142. Increased Odds of Psychiatric Illness Among Mothers of Infants with Congenital Syphilis

BackgroundSyphilis can be transmitted mother to child during pregnancy leading to multisystem birth defects if untreated. In Illinois, screening is mandated for pregnant women at first and third trimesters. The University of Illinois Hospital (UIH) serves a vulnerable patient population with a high syphilis prevalence. An understanding of risk factors associated with maternal syphilis infection can guide prevention of congenital syphilis (CS) with early prenatal diagnosis and treatment. The aim of this retrospective case control study is to describe maternal risk factors associated with CS in a clinical setting.MethodsUsing a database used for health department reporting from 2014-2018 at UIH, 106 maternal syphilis diagnoses were identified. Medical records were reviewed for CS infant diagnosis, sociodemographic information, medical history, and potential risk factors, including multiple sex partners, HIV status, drug use, history of incarceration or sex work, and having sex with men who have sex with men (MSM). Cases were matched with controls of pregnant women with syphilis testing that was not indicative of infection.ResultsOf the maternal syphilis diagnoses identified, there were 8 cases in which CS was possible or highly probable, 68 in which CS was less likely or unlikely, and 30 that were lost to follow up. Of the possible and probable infants’ mothers, 38% had a psychiatric illness (6.80 OR, 95% CI 1.06-43.48) and 25% were homeless (12.00 OR, 95% CI 0.94-153.89). Late or scant prenatal care was seen in 75% (4.15 OR, 95% CI 0.72-23.95) and 75% had inadequate syphilis treatment. None were HIV positive or reported incarceration, intravenous drug use, sex work, or having sex with MSM.ConclusionAmong infants with probable or possible CS, there was a 6.80 increased odds of maternal psychiatric illness compared to those born to mothers not diagnosed with syphilis, which may have complicated prenatal care and delayed diagnosis or treatment. Psychiatric illness outnumbered several other known risk factors; however, these may be less often discussed during clinical encounters. Psychiatric illness history may be a risk factor and means to identify women in the clinical setting who need close follow up and outreach after a prenatal syphilis diagnosis to prevent or mitigate congenital transmission.Disclosures All Authors: No reported disclosures

Contact tracing of syphilis-seropositive pregnant women and syphilis-infection among their male partners in Bao'an district, Shenzhen, China

BackgroundUntreated male partners are a critical source of maternal re-infection. Contact tracing is a good way to identify infection among partners and reduce risk of mother-to-child transmission related to maternal re-infection. This study aimed to analyze the current situation and related factors of contact tracing of syphilis-seropositive pregnant women and syphilis-infection among their male partners.MethodData of syphilis-seropositive pregnant women and their male partners attending clinic for syphilis-screening were obtained from the Shenzhen Program for Prevention of Congenital Syphilis. Contact tracing rate of syphilis-seropositive pregnant women and syphilis prevalence among male partners were counted, and related factors were also analyzed using a random-effects logistic regression model.ResultOf the 1299 syphilis-seropositive pregnant women, 74.1% (963/1299) had their male partners receiving syphilis-screening and 19.1% (184/963) of male partners were syphilis-infected. For pregnant women, being divorced (adjusted odds ratio [AOR] =0.39; 95%CI: 0.17–0.87), seeking for emergency services at their first antenatal clinics visits (AOR = 0.58; 95%CI: 0.44–0.77), reporting willingness to notify partner(AOR = 7.65; 95%CI: 4.69–12.49), multi-partners (AOR = 1.38; 95%CI:1.03–1.86) and having a history of drug abuse (AOR = 0.37; 95%CI: 0.14–1.00)were independently associated with successful contact tracing. For male partners, of minority ethnicity (AOR = 4.15; 95%CI: 1.66–10.34), age at first sex>20(AOR = 0.57; 95%CI: 0.37–0.87), reporting multi-partners (AOR = 1.60; 95%CI: 1.04–2.46), having a history of drug abuse (AOR = 4.07; 95%CI: 1.31–12.64) were independently associated with syphilis-infection. In addition, pregnant women with TRUST titer ≥1:8 (AOR = 2.81; 95%CI: 1.87–4.21), having a history of adverse pregnancy outcomes (AOR = 1.70; 95%CI: 1.14–2.53), reporting multi-partners (AOR = 0.43; 95%CI: 0.29–0.64) and reporting the current partner as the source of syphilis (AOR = 5.05; 95%CI: 2.82–9.03) were independently associated with partners’ syphilis-infection.ConclusionContact tracing is feasible and effective in identifying syphilis-infected partners among syphilis-seropositive pregnant women. Contact tracing is associated with many factors such as women’s marital status, services at their first antenatal clinics visit and willingness of partner notification. Partners’ ethnicity, age at first sex, multi-partners and history of drug abuse as well as women’s levels of TRUST titer were associated with partners’ syphilis-infection.

Avaliação epidemiológica da sífilis congênita na região Nordeste do Brasil

O presente estudo tem por objetivo estudar a prevalência de sífilis congênita no Nordeste do Brasil de 2009-2018. Trata-se de um estudo epidemiológico, retrospectivo e descritivo de cunho populacional, utilizando-se dados secundários, no qual foi realizada uma pesquisa de casos de Sífilis congênita, obtidos no Sistema de Informação de Agravos de Notificação (SINAN). Avaliando aspectos relacionados ao ano de diagnóstico de sífilis congênita no recém-nascidos, na idade gestacional, em informação sobre realização de pré-natal da mãe, escolaridade, faixa etária da gestante e o momento do diagnóstico. Os dados foram Tabulados, utilizando os programas TABNET e Microsoft Office Excel 2019. No período em estudo foram confirmados 56985 casos de recém-nascidos com sífilis na Região Nordeste do Brasil, com média de 5.698,5casos por ano. Em 2018, foi registrado o maior número de casos novos (n=14705), e em 2009, o menor número (n=1983) e apontou-se um aumento na frequência de casos de sífilis congênita a partir de 2009 até 2018. O maior número de casos de sífilis ocorreu em gestantes com fundamental incompleto e de 20 a 39 anos com 3.278 e 14.109 em 2018 respectivamente. A identificação dos casos de sífilis materna foi realizada principalmente no momento do parto (53,2%). Destacou-se ainda a crescente prevalência de pré-natal de 2009 a 2017, com maior ano de 2017, com 5640 notificações. Assim, o aumento da incidência no período gestacional e a consequente elevação de casos de sífilis congênita só poderá ser minimizada e controlada quando as medidas de prevenção e controle forem satisfatoriamente aplicadas.

Incidence of congenital syphilis in the State of Rio Grande do Norte and Brazil: a retrospective study

Introduction: Syphilis is an infection with increasing incidence in Brazilian regions, and the congenital form is liable to lead to cerebral palsy and musculoskeletal deformity. The objective of this study was to analyze the prevalence of congenital syphilis in the State of Rio Grande do Norte, Brazil.
 Methods: An exploratory and retrospective study was performed from 2015 to 2017. Public domain data and unrestricted access were used in the DATASUS computer department through the TABNET application. The following variables were collected: age range, clinical evolution (congenital syphilis late, late congenital syphilis, stillbirth / syphilis abortion, ignored / blank, discarded; live birth), mother's schooling, performed prenatal and residence zone.
 Results: In Brazil, there were 75,733 cases, whereas in the State of Rio Grande do Norte there were 1,436 cases of congenital syphilis, out of which a greater number of confirmed cases affected age group of 0-6 days (97.6%). The highest number of cases was observed in the city of Natal (75.8%), followed by Mossoró (7.8%) and Parnamirim (5.7%).
 Conclusion: The study shows that the incidence of congenital syphilis occurs predominantly in incomplete elementary schooling and in the urban area. It was also verified that the diagnosis of most cases of congenital syphilis occurred in the age group up to 6 days of life, which contributes to the rates of evolution with the living child. In addition, a greater number of confirmed cases of congenital syphilis were observed, and prenatal care was performed, indicating a serious failure in care.

Missed Opportunities for Prevention of Congenital Syphilis - United States, 2018.

Congenital syphilis is an infection with Treponema pallidum in an infant or fetus, acquired during pregnancy from a mother with untreated or inadequately treated syphilis. Congenital syphilis can cause miscarriage, stillbirth, or early infant death, and infected infants can experience lifelong physical and neurologic problems. Although timely identification and treatment of maternal syphilis during pregnancy can prevent congenital syphilis (1,2), the number of reported congenital syphilis cases in the United States increased 261% during 2013-2018, from 362 to 1,306. Among reported congenital syphilis cases during 2018, a total of 94 resulted in stillbirths or early infant deaths (3). Using 2018 national congenital syphilis surveillance data and a previously developed framework (4), CDC identified missed opportunities for congenital syphilis prevention. Nationally, the most commonly missed prevention opportunities were a lack of adequate maternal treatment despite the timely diagnosis of syphilis (30.7%) and a lack of timely prenatal care (28.2%), with variation by geographic region. Congenital syphilis prevention involves syphilis prevention for women and their partners and timely identification and treatment of pregnant women with syphilis. Preventing continued increases in congenital syphilis requires reducing barriers to family planning and prenatal care, ensuring syphilis screening at the first prenatal visit with rescreening at 28 weeks' gestation and at delivery, as indicated, and adequately treating pregnant women with syphilis (2). Congenital syphilis prevention strategies that implement tailored public health and health care interventions to address missed opportunities can have substantial public health impact.

Evaluating coverage of maternal syphilis screening and treatment within antenatal care to guide service improvements for prevention of congenital syphilis in Countdown 2030 Countries.

BackgroundCountdown to 2030 (CD2030) tracks progress in the 81 countries that account for more than 90% of under-five child deaths and 95% of maternal deaths in the world. In 2017, CD2030 identified syphilis screening and treatment during antenatal care (ANC) as priority indicators for monitoring.MethodsCountry-reported data in the UNAIDS Global AIDS Monitoring System (GAM) system were used to evaluate four key syphilis indicators from CD2030 countries: (1) maternal syphilis screening and (2) treatment coverage during ANC, (3) syphilis seroprevalence among ANC attendees, and (4) national congenital syphilis (CS) case rates. A cascade analysis for CD2030 countries with coverage data for the number of women attending at least 4 antenatal care visits (ANC4), syphilis testing, seroprevalence and treatment was performed to estimate the number of CS cases that were attributable to missed opportunities for syphilis screening and treatment during antenatal care.ResultsOf 81 countries, 52 (64%) reported one or more values for CS indicators into the GAM system during 2016-2017; only 53 (65%) had maternal syphilis testing coverage, 41 (51%) had screening positivity, and 40 (49%) had treatment coverage. CS case rates were reported by 13 (16%) countries. During 2016-2017, four countries reported syphilis screening and treatment coverage of ≥95% consistent with World Health Organization (WHO) targets. Sufficient data were available for 40 (49%) of countries to construct a cascade for data years 2016 and 2017. Syphilis screening and treatment service gaps within ANC4 resulted in an estimated total of 103 648 adverse birth outcomes with 41 858 of these occurring as stillbirths among women attending ANC4 (n = 31 914 408). Women not in ANC4 (n = 25 619 784) contributed an additional 67 348 estimated adverse birth outcomes with 27 198 of these occurring as stillbirths for a total of 69 056 preventable stillbirths attributable to syphilis in these 40 countries.ConclusionThese data and findings can serve as an initial baseline evaluation of antenatal syphilis surveillance and service coverage and can be used to guide improvement of delivery and monitoring of syphilis screening and treatment in ANC for these priority countries.

Clinical characteristics and outcomes for paediatric patients admitted with congenital or acquired syphilis: a population-based cohort study

Sífilis congênita no município de Santa Inês – MA: caracterização epidemiológica

A sífilis é causada por uma bactéria chamada Treponema pallidum, gênero Treponema, da família dos Treponemataceae, caracterizando-se por ser sistêmica, de evolução crônica e com transmissão por meio de relação sexual ou materno-fetal. A infecção é transmitida pela via sexual (sífilis adquirida) e verticalmente (sífilis congênita) pela placenta da mãe para o feto. O presente estudo teve como objetivo analisar o perfil epidemiológico de sífilis congênita na cidade de Santa Inês, Maranhão. Trata-se de uma coorte retrospectiva de caráter exploratório, descritivo e quantitativo, desenvolvido no município de Santa Inês no Estado do Maranhão, no período transcorrido de janeiro de 2010 a dezembro de 2018. Observou-se uma tendência crescente em todos os anos analisados, com um aumento substancial a partir de 2017 (n=21), principalmente no Nordeste e no estado do Maranhão, já no município de Santa Inês no ano de 2013, observou-se uma queda considerável nos casos com um aumento no ano subsequente e com maiores incidência no ano de 2018 com um número de casos superior aos casos do nordeste do estado do Maranhão. Assim, Mediante os dados apresentados, pode-se caracterizar a incidência da sífilis congênita no município de Santa Inês através da análise epidemiológica do corte temporal de 08 anos.
