Section 1: Handwashing

Handwashing is the single most important procedure for preventing nosocomial infections. Handwashing is defined as a vigorous, brief rubbing together of all surfaces of lathered hands, followed by rinsing under a stream of water. Although various products are available, handwashing can be classified simply by whether plain soap or detergents or antimicrobial-containing products are used. Handwashing with plain soaps or detergents (in bar, granule, leaflet, or liquid form) suspends microorganisms and allows them to be rinsed off; this process is often referred to as mechanical removal of microorganisms. In addition, handwashing with antimicrobial-containing products kills or inhibits the growth of microorganisms; this process is often referred to as chemical removal of microorganisms. Cost-effective application of the centers for disease control guideline for prevention of surgical wound infections

Surgical wound infections present a serious hazard to patients and an important legal and economic liability to health care providers. This Guideline furnishes infection control practitioners with several excellent cost-saving concepts and, if implemented, would result in a reduction in the rates of surgical wound infections.

Antimicrobial Prophylaxis in Pediatric Surgical Patients

A major use of antimicrobial agents in hospitalized children is prophylaxis of postoperative wound infections. In view of this frequent use and the emerging consensus on recommendations for prevention of surgical wound infections,1 the American Academy of Pediatrics has developed guidelines for surgical antimicrobial prophylaxis in children. Prophylaxis is defined as use of antimicrobial drugs in the absence of suspected or documented infection. FREQUENCY OF ANTIMICROBIAL PROPHYLAXIS In an extensive study of patients in acute care general hospitals in Pennsylvania, 36% of antimicrobial courses were initiated for prophylaxis of infection after surgery or an invasive procedure, such as cystoscopy or cardiac catheterization.2 The prevalence and reasons for antimicrobial use have been studied primarily in general or adult hospitals, but the patterns of use in children are similar to those in adults.3-5 Two studies have demonstrated that prophylaxis accounts for approximately 75% of the antibiotic use on pediatric surgical services.4,6 The efficacy of antimicrobial prophylaxis in lowering the incidence of postoperative infection after certain types of surgery has been demonstrated amply in controlled clinical trials.7 These and earlier studies by Miles et al8 and Burke9 in experimental animals have delineated the principles for effective use of antimicrobial agents in surgical prophylaxis, including choice of drugs and when and how long they are to be given. INAPPROPRIATE ANTIMICROBIAL PROPHYLAXIS Prophylaxis has been incriminated as a major cause of inappropriate use of these antimicrobial agents in both adults and children.4,5,10-12 In 74% of antimicrobial courses given for prophylaxis in the Pennsylvania study, the duration exceeded 48 hours, an interval beyond which continuation has not been proven beneficial.2

In Reply.— Dr Miller makes an important point. The available literature always must be reviewed in the context of one's own practice experience. The patient population studied and described in the literature must be compared with one's own patient population in making judgments about the applicability of reported findings to one's own practice. There have been numerous recent reviews in the literature of the appropriateness in selected settings of prophylactic antibiotic use. I think the most useful single resource in this area is the recent guidelines coming from the Centers for Disease Control in Atlanta. Of particular value is the Guidelines for Prevention of Surgical Wound Infections , published in March 1982. 1 This guideline does discuss prophylactic antibiotics and includes a useful bibliography in the whole area of surgical wound infection prevention and control.

Despite the widespread use of antibiotics, surgical wound infections continue to cause patient discomfort and drain on health care finances. More serious local complications often develop concomitantly (eg, cellulitis) or later (eg, incisional hernia). Inadequate treatment of an illness or poor host defenses may lead to serious systemic complications (septicemic shock, multiple organ failure). Therefore, it is essential that every effort be made to minimize the likelihood of infection. The Centers for Disease Control in Atlanta convened a group of physicians and surgeons knowledgeable in the practice of infection control. This group developed a set of guidelines that is thought to reflect the state of the art in 1982. The trials, which were objective and preferably randomized or even blinded, determined present options for surgical infection control.

Guideline for Prevention of Surgical Wound Infections

Patients who undergo a surgical operation are at high risk of having one or more nosocomial infections. These infections develop in more surgical patients (8%) than in any other patient group, and about 70% of all nosocomial infections throughout the hospital develop in patients who have an operation. Most infections in surgical patients, however, are not related to the wound but to instrumentation of the urinary and respiratory tracts. Thus, personnel who take care of these patients should be aware of measures to prevent nosocomial infections at all sites. Moreover, to prevent surgical wound infections, personnel who perform the operation must take the lead in instituting prevention measures, because the most important measures involve use of good surgical technique and are not easily instituted simply by making changes in hospital policy.

Operative wound infection is examined through classifications based on estimation of frequency, severity, and sources of infection. These classifications help in identifying preventive and corrective measures. All surgeons are concerned with postoperative infection because it can convert a superior technical result into a disaster. The analysis of postoperative infection is complicated by the complex and constantly changing relations among host, challenging microbe, and the existing antimicrobial measures. The effect of physical, pharmacologic, and biological variables is estimated with emphasis on measures that promise further control of infection in the surgical patient. Proposals are offered for further efforts in controlling this hazard.
