Osteoarthritis of zygapophysial joints as a cause of back pain and neck pain: a scoping review.

Zygapophysial joints (Z joints) can be a source of back pain and of neck pain, but the cause of pain is not known. Some authors attribute the pain to osteoarthritis but without citing evidence. The present review was undertaken to determine if there was sufficient evidence of association between spinal pain and osteoarthritis of Z joints to justify osteoarthritis being held to be the cause of pain. The null hypothesis was that osteoarthritis of Z joints does not cause back pain or neck pain. Relevant studies were ones that provided primary data on the association between pain and osteoarthritis of Z joints. These could be population studies, diagnostic studies, or case-control studies. The database of PubMed was searched using the terms: Lumbar or cervical, zygapophysial or facet, pain, and osteoarthritis or degeneration or degenerative. Data pertinent to the research question were extracted from original articles and tabulated for reporting. Odds ratios for associations were calculated, as were the prevalence rates of osteoarthritis in subjects with pain, and conversely the prevalence rates of pain in subjects with osteoarthritis. The searches retrieved 11 population studies, 4 diagnostic studies, and 3 cases control studies. No study showed any positive association between osteoarthritis of Z joints and pain. All studies found pain to be independent of the presence or severity of osteoarthritis. Osteoarthritis was as common in subjects with no pain as in subjects with pain. The null hypothesis was not refuted. The published evidence does not support the belief that osteoarthritis causes Z joint pain. All the evidence contradicts this belief.

A REVIEW ON UTILITY OF DWIVIDHOPAKRAMANIYA CHIKITSA IN SANDHISHOOLA

Dwividhopakraminiya chikitsa explained in Ashtanga Hridaya Sutra Sthana chapter-14 as the two kinds of significant line of treatments as Santarpana and Aptarpana chikitsa in the form of Brimhana and langhana for two types of the human body. One is nourishing therapy, and another is depleting therapy. Sandhishoola is the common symptom found in the Sandhigata vata, Amavata or Vatarakta diseases in the different age groups of people. Among vataj gatavat vyadhis, Sandhigata vata is the most commonly observed condition correlated to Osteoarthritis (OA), predominantly seen in degenerative disease involving peripheral joints cartilage damages overgrowth of the bone. The prevalence rate of OA has been observed to increase with the advancing age, and the possibility of women being affected by the disease is greater than men with respect to gender. In the Amavata and Vatarakta also, Sandhishoola is observed along with inflammation of joints. Hence, in this article, the application of dwividhopakraminiya chikitsa for sandhishoola in different diseases has been explained. The mode of action of Santarpana and Apatarpana chikitsa in managing degenerative and inflammatory conditions of the joints.

CLINICAL STUDY OF GUGGULU GOLD AND GUM SHALLAKI ALONG WITH PANCHKARMA THERAPIES IN THE MANAGEMENT OF SANDHIGHATA VATA W.S.R OSTEOARTHRITIS

In Vriddhavastha, most of the Dhatus Kshaya, which in turn leads to Vataprakopa and thus making individuals prone to many diseases. Among these diseases, Sandhigata Vata is one type of Vata-Vyadhi, which is the commonest form and occurs mainly due to sitting jobs, air conditioner atmosphere, lack of proper healthy diet, travelling and old age etc. Sandhigata Vata is one of the most common Vatavyadhi, which can be correlated with osteoarthritis. The prevalence rate of Osteoarthritis is about 14.8 %, in which knee osteoarthritis prevalence rate is 10.8% which is more than the arthritis of other joints1. The incidence of osteoarthritis in India is as high as 12%. It is estimated that approximately four out of 100 people are affected by it. Osteoarthritis is the most common articular disorder which begins asymptomatically in the 2nd & 3rd decades and is extremely common by age 70. Almost all persons by age 40 have some pathologic change in the weight-bearing joint. 25% of females & 16% of males have symptomatic osteoarthritis. When we talk about healthy joints, there is a coating of tough but smooth and slippery tissue, called cartilage, which covers the surface of the bones and helps the bones to move freely against each other. When a joint develops osteoarthritis, part of the cartilage thins and the surface becomes rougher, which in turn doesn't allow the joint to move as smoothly as it should. When cartilage becomes damaged, all the tissues within the joint become more active than normal as the body tries to repair itself. But these repair processes are going to change not only the structure of the joint but will often allow the joint to work normally and without any pain and stiffness. Acharya Charaka has mentioned "Sandhi Gataanila”, which means vata gets located in the Sandhi and thereafter results in Sandhigatavata. Allopathic treatment has its limitation in managing this disease. It can provide either conservative or surgical treatment and can only subside the pain but not cure the cause of diseases. While traditional life science Ayurveda has the most effective solution over this. Local Abhyanga and Nadi Sweda were selected for the present study in one group, as it has shown best for the Vata Vyadhis. Here local Abhyanga was given with Bala Taila because Bala Taila and Nadi Sweda are having Vatashamaka and Rasayana properties. While in the case of another group, Guggulu gold along with gum shallaki followed by local Abhyanga and Nadi Sweda were given to the patients. Prevalence of osteoarthritis in individuals with COPD: a systematic review.

The objective of this review was to examine the prevalence of osteoarthritis (OA) in individuals with COPD. A computer-based literature search of CINAHL, Medline, PsycINFO and Embase databases was performed. Studies reporting the prevalence of OA among a cohort of individuals with COPD were included. The sample size varied across the studies from 27 to 52,643 with a total number of 101,399 individuals with COPD recruited from different countries. The mean age ranged from 59 to 76 years. The prevalence rates of OA among individuals with COPD were calculated as weighted means. A total of 14 studies met the inclusion criteria with a prevalence ranging from 12% to 74% and an overall weighted mean of 35.5%. Our findings suggest that the prevalence of OA is high among individuals with COPD and should be considered when developing and applying interventions in this population.

Osteoarthritis of the hip and knee in former male professional soccer players.

Professional soccer (PS) players are at great risk of osteoarthritis (OA) of the knee and hip. Following the PRISMA guidelines, the key words 'osteoarthritis' and 'soccer' or 'football' were matched with 'players' or 'former' or 'retired' and with 'hip' or 'knee' on December 24, 2017 in the following databases: PubMed, Cochrane, Google scholar, Embase and Ovid. Only comparative studies reporting the prevalence rate of OA of both hip and knee joint in former PS athletes (fPSa) and age and sex matched controls were considered. In fPSa, the prevalence rate of OA of both hip and knee is significantly higher compared to age and sex matched controls. The pathological pathways responsible for the development of OA of the hip and knee in PS athletes (PSa) are still not clearly understood. The prevalence rate of clinical OA of the hip was 8.6% in fPSa and 5.6% in controls (odd ratio (OR) = 1.5; 95% CI: 1.06-2.31). The radiographic rate of OA was 21.2% in fPSa and 9.8% in controls (OR = 2.4; 95% CI: 1.66-3.69). A total of 14.6 and 53.7% of fPSa presented clinical and radiographic signs of OA of the knee, respectively, vs 12.9% (OR = 1.16; 95% CI: 0.86-1.55) and 31.9% (OR = 2.47; 95% CI: 2.03-3.00) of controls. Sonographic evidence of OA of the knee was found in 52% of fPSa and 33% of controls (OR = 2.2; 95% CI: 1.24-3.89). Preventive training programmes should be developed to reduce the number of fPSa presenting early OA.

The Association between Osteoarthritis and Occupational Clusters in the Korean Population: A Nationwide Study.

Osteoarthritis (OA) is a considerable health problem worldwide. It is known to be associated with certain occupational risk factors. We examined the prevalence rate of OA by occupational cluster. Data were collected from the Korea National Health and Nutrition Examination Surveys (2010–2013). The total number of unweighted sample size was 9,905 participants: 4,460 men and 5,445 women, and OA prevalence was 5.3% and 18.4% respectively. OA patients were defined as participants with knee/hip joint pain and radiographic change of knee/hip joint. Occupational type was classified as either white, pink, blue, or green collar based on the occupational characteristics following physical demand: white for manager and professionals; pink for clerks and service/sales workers; blue for craft/trade workers, machine operators and assemblers, and elementary manual workers; and green for agricultural/fishery workers. We calculated the odds ratios (ORs) and 95% confidence intervals (95% CI) for the odds of a participant’s having OA according to the occupational cluster, with gender stratification. The multiple logistic regression model showed that, compared to the white collar group, the ORs of the pink, blue, and green collar workers were 1.23 (95% CI 0.64–2.36), 1.85 (95% CI 1.18–2.88), and 2.91 (95% CI 1.86–4.54), respectively, in males, and 2.53 (95% CI 1.71–3.73), 2.86 (95% CI 1.94–4.21), and 3.90 (95% CI 2.60–5.83), respectively in females. The prevalence rate of OA was associated with the occupational cluster, in order from highest to lowest: green, blue, pink, and white collar.

Health problems in former elite female football players: Prevalence and risk factors.

The aim of this study was to assess the prevalence of health problems and associated risk factors in former elite female football players. A cross-sectional research design was employed, using an online questionnaire on personal characteristics and health complaints during/after the career. One hundred fifty-two (response rate: 62.0%) former first German league players answered the survey. Around 70% described their current health as good or very good. Over half (57.9%) reported knee problems during the last 4weeks while exercising and a third (33.6%) during normal daily activities. The second most common location for complaints was the head (53.3%). Almost one quarter (23.7%) of players suffered from osteoarthritis (OA). Regression analysis showed that OA in knee/ankle and physical complaints (PC) in knee/ankle/head were significantly predicted by number and severity of previous injuries (P<0.05). Further, increases in age, training volume, and level of play were associated with an increased likelihood of presenting with OA (P<0.05), but not PC. In conclusion, a football career may lead to specific long-term health problems in elite female players. Prevention strategies should focus on knee, ankle, and head injuries. Future studies are needed to clinically assess the prevalence rates of OA and possible neurocognitive changes.

Prevalence of osteoarthritis in former elite athletes: a systematic overview of the recent literature.

The aim of this study was to systematically review the recent scientific literature to explore the prevalence of osteoarthritis (OA) in former elite athletes from team and individual sports. A systematic review of observational studies was conducted. Based on three categories of keywords (and synonyms), a sensitive search strategy was built in order to search MEDLINE and SPORTDiscus from 2000 to 2014. The data from included studies presenting a prevalence rate of OA as an outcome measure were extracted using a standardised extraction form, and their methodological quality was described. Fifteen studies were identified through our search strategy, studies being rather heterogeneous with regard to their study population and to the definition and assessment of OA. Hip OA ranged from 2 to 60%, and knee OA from 16 to 95%. Prevalence rates of general, lower limbs or hip/knee OA ranged from 1 to 59%. Prevalence of neck, back, shoulder and ankle OA was reported only in single studies (16, 18, 33 and 3%, respectively). This study showed that prevalence of OA, especially in their lower limbs, seems to be high among former elite athletes from team and individual sports compared to the general population and other occupational sectors.

Knee Osteoarthritis Prevalence in Hospitalized Elderly Patients: A Retrospective Study

This study aimed to determine the prevalence rate of knee osteoarthritis (OA) and the risk factors for OA in hospitalized elderly patients. We conducted this retrospective study in elderly patients (aged 65 years and older) who were hospitalized in the Geriatric Ward of General Hospital of Guangzhou Military Command of the People's Liberation Army between January 2011 and June 2013, including general condition, present history, past history, physical examination, X-ray results, and disease diagnosis. The prevalence, awareness, and treatment rates of knee OA in hospitalized elderly patients were calculated. Risk factors were computed using multiple logistic regression analysis. Of a total of 267 (17.4%) hospitalized elderly patients diagnosed with knee OA, the prevalence rate of OA was 9.95% in males and 37.76% in females. The rate of awareness among those with OA was 51.68%; the rate of treatment was 83.33%; and the rate of control was 77.39%. The medical expenses for both females (1143±315 yuan month-1) and males (1192±357 yuan month-1) in knee OA patients are higher than that of the non-knee OA group (989±274 yuan month-1, 1038±295 yuan month-1). The risk factors for knee OA include gender (OR=2.448), age (OR=1.124), transportation mode (OR= 8.972), exercise (OR=7.374), bowel evacuation position (OR=5.767), family history of knee OA (OR=2.195), and body mass index (OR=2.469). The prevalence of knee OA is unexpectedly high in hospitalized elderly patients, and the rates of awareness and treatment are less than desirable. Prevention and control measures should be taken in patients with concomitant risk factors.

European Project on Osteoarthritis (EPOSA): methodological challenges in harmonization of existing data from five European population-based cohorts on aging

BackgroundThe European Project on OSteoArthritis (EPOSA), here presented for the first time, is a collaborative study involving five European cohort studies on aging. This project focuses on the personal and societal burden and its determinants of osteoarthritis (OA). The aim of the current report is to describe the purpose of the project, the post harmonization of the cross-national data and methodological challenges related to the harmonization processMethodsThe study includes data from cohort studies in five European countries (Germany, Italy, the Netherlands, Spain and the United Kingdom) on older community-dwelling persons aged ≥ 59 years. The study design and main characteristics of the five cohort studies are described. Post harmonization algorithms are developed by finding a "common denominator" to merge the datasets and weights are calculated to adjust for differences in age and sex distribution across the datasets.ResultsA harmonized database was developed, consisting of merged data from all participating countries. In total, 10107 persons are included in the harmonized dataset with a mean age of 72.8 years (SD 6.1). The female/male ratio is 53.3/46.7%. Some variables were difficult to harmonize due to differences in wording and categories, differences in classifications and absence of data in some countries. The post harmonization algorithms are described in detail in harmonization guidelines attached to this paper.ConclusionsThere was little evidence of agreement on the use of several core data collection instruments, in particular on the measurement of OA. The heterogeneity of OA definitions hampers comparing prevalence rates of OA, but other research questions can be investigated using high quality harmonized data. By publishing the harmonization guidelines, insight is given into (the interpretation of) all post harmonized data of the EPOSA study.

Prevalence of radiologically defined osteoarthritis in the finger and wrist joints of adult residents of Tecumseh, Michigan, 1962-65

Radiographs of the fingers and wrists of adult participants in the Tecumseh Community Health Study in 1962–1965 were examined for signs of osteoarthritis (OA). The severity of OA for each of 32 joints of the fingers and wrists was recorded for each individual. Attention was restricted to the 3035 participants who were 32 years of age or older and for whom a diagnosis of OA was available for each of 32 joints. Joint-specific prevalence rates of OA increased sharply with age for both sexes, and at the older ages, the prevalence rates for most joints were higher for females. Older individuals with OA also had a greater number of affected joints, with females having a greater number of affected joints than males. Of those individuals aged 44 years or younger, only 6.2% had one or more joints affected with OA. The percentages were 21.6 and 42.0% for those aged 45–59 years and 60 or more years, respectively. The distal interphalangeal (DIP) joints were the most frequently affected joints in all age categories for both sexes and OA in the proximal interphalangeal (PIP) joints was positively associated with OA in the DIP joints. However, controlling for the number of affected DIP joints, the PIP joints of older subjects were more likely to exhibit OA than the PIP joints of younger subjects. Though there is an association between OA in the DIP and PIP joints, there was only a small, nonsignificant association (OR= 1.24, 95% CI=0.83, 1.84) between disease in the DIP and PIP joints of the same finger.
