Can Primigravid Stress Urinary Incontinence be Predicted by Translabial Urethral Motion Profile Ultrasonography?

Aims: To determine SUI prevalence and its association with urethral and bladder neck mobility among late third-trimester primigravid women. Introduction: Stress urinary incontinence (SUI) is a relatively common urogynecological disorder in pregnant women that significantly disrupts their quality of life. Although its prevalence is increasing along with gestational age, there have been reports of underreporting due to various reasons. Therefore, a diagnostic modality is needed to determine the presence of SUI in such a population. Methods: A total of 209 late third-trimester primigravid women included in the study between November 2016 and July 2019 were examined by translabial ultrasound. Bladder neck descent (BND), retrovesical angle (RVA), urethral rotation (RoU) and funneling were observed in each patient. SUI was diagnosed using a cough stress test and Questionnaire for Urinary Incontinence Diagnosis (QUID). Thirty-five subjects from each group were randomly selected for further analysis. Results: Among 209 late third-trimester primigravid women, SUI was observed in 57 patients (prevalence 27.3%). The RVA and funneling of the SUI group were significantly higher than the control group (P < 0.05). The BND and RoU were similar between groups. Identified risk factors of SUI were body mass index >23 kg/m2 and the presence of funneling in the translabial ultrasound. Conclusions: The prevalence of SUI among late third-trimester primigravid was 27.3%. Positive funneling and higher BMI were shown to be the independent risk factors for SUI.

The Effect of Kegel Exercises and Pelvic Floor Physiotherapy on the Improvements of Stress Urinary Incontinence and Urge Incontinence in Women with Normal Vaginal Delivery

Objectives: Pregnancy and normal vaginal delivery, are the main risk factors for urinary incontinence in women. A variety of treatments such as bladder training and pelvic floor exercises are effective for urinary incontinence. This interventional study has been conducted to compare the influence of Kegel exercises and pelvic floor physiotherapy on the improvements of urge and stress incontinence in women with first or second singleton pregnancies who have experienced normal vaginal delivery. Methods: One hundred fifty women with urinary incontinence in the postpartum period, who were divided randomly into 3 groups of 50 patients, participated in this prospective interventional study. The women were primigravida or it was their second labor. None of them complained of urinary incontinence before pregnancy. All of them had a normal vaginal delivery. The first group included females who were asked to perform Kegel exercises. The second group had 8 weekly physiotherapy appointments. The control group, included mothers who received routine care. We compared the prevalence of urge and stress incontinence between groups. Results: We found that the relative risk of urge and stress incontinence among mothers in the control group was more than in the intervention groups, but this difference was only statistically significant in the subject of stress incontinence between the Kegel and control groups. The reduction of urge incontinence prevalence among mothers in the Kegel and physiotherapy groups, was more than control group, although this reduction was not significant. Conclusion: There is a significant decrease in the prevalence of stress urinary incontinence at the end of the 8th and 12th weeks postpartum compared to the control group. Although the prevalence of urge incontinence among women in intervention groups was lower than in the control group, it was not significant. It seems the effect of Kegel exercises and pelvic floor physiotherapy, in reducing stress incontinence at the end of the 8th week, is almost equal.

Association Between Added Sugar Intake and Urinary Incontinence in Females: A Cross-sectional Population-based Study.

Urinary incontinence (UI) is a widespread issue in women that severely impacts quality of life. The addition of sugar is associated with multiple adverse effects on health. This study examined the potential association between added sugar intake and UI. Adult females from the National Health and Nutrition Examination Survey database (2005-2018) were included in this study. The primary outcomes were the prevalence of stress urinary incontinence (SUI), urge urinary incontinence (UUI), and mixed urinary incontinence (MUI). Weighted logistic regression, stratified logistic regression, restricted cubic spline regression, and sensitivity analyses were utilized to determine whether added sugar was associated with UI after multivariate adjustment. A total of 14,927 participants met the inclusion criteria. The results revealed a heightened prevalence ofSUI, UUI, and MUI in the fourth quartile of added sugar energy percentage (OR = 1.304, 95% confidence interval [CI] = 1.105-1.539; OR = 1.464, 95% CI = 1.248-1.717; OR = 1.657, 95% CI = 1.329-2.065 respectively). The effect was more pronounced in young women and the subgroup analyses did not reveal any noteworthy interaction effects. According to the sensitivity analyses, the results for SUI and the MUI were consistent with those of the primary analyses. The excessive intake of added sugar among women may increase their risk of SUI and MUI. Our study highlights the negative effects of added sugar on female genitourinary health and highlights the need for universal access to healthy diets.

Prevalence of stress urinary incontinence and urge urinary incontinence in multiples sclerosis patients: a systematic review and meta-analysis.

Urinary incontinence (UI), encompassing stress urinary incontinence (SUI) and urge urinary incontinence (UUI), is a prevalent and debilitating condition in patients with multiple sclerosis (MS), profoundly impacting their quality of life. This systematic review and meta-analysis aimed to elucidate the worldwide prevalence rates of SUI and UUI among MS patients. This study was conducted by examining observational studies published between 2000 and 2023. An exhaustive literature search was conducted across databases such as PubMed, MEDLINE, Web of Science, Scopus, ProQuest, and Google Scholar. The Meta-prop method facilitated pooled prevalence estimation of UUI and SUI, while Egger tests assessed publication bias. In total, 27 studies with 15,052 participants were included in the meta-analysis. The findings revealed a high random effect pooled prevalence of UUI at 41.02% (95% Confidence Interval [CI]: 30.57-51.89; I2 = 99%, p < 0.001) and SUI at 25.67% (95% CI: 19.30-32.58%; I2 = 94.9%, P < 0.001). Additionally, the pooled prevalence of mixed urinary incontinence (MUI) was reported at 18.81% (95% CI: 7.55-33.48; I2:95.44%, p < 0.001). The high heterogeneity observed suggests variable prevalence across populations and highlights the intricate nature of UI in MS. These findings underscore the critical need for dedicated supportive, therapeutic, and rehabilitative interventions to manage this common complaint in MS patients effectively.

STRESS URINARY INCONTINENCE: A TEN YEARS SYSTEMATIC REVIEW

Introduction: Stress urinary incontinence (SUI) and pelvic organ prolapse (POP) affect a substantial percentage of women, significantly impacting their quality of life. This review explores the prevalence, definitions, and pathophysiology of SUI, POP, and associated urinary incontinence, emphasizing the need for medical attention and offering insights into current treatment trends over the past 5 years. Methods: The researchers in this study followed the 2020 Preferred Reporting Items for Systematic Review and Meta-Analysis (PRISMA) guidelines to ensure that their work met the required standards. This was done to ensure the precision and reliability of the conclusions derived from the research. Result: Our search produced 21 results. After looking at the titles and summaries, we found 9 papers that fit our criteria. At first, we excluded several articles because they were written in review style. But after reading the full papers carefully, we included five papers in our final analysis. These papers included a retrospective observational study, prospective study, and randomized control trial. Conclusion: In this decade-long systematic review on stress urinary incontinence (SUI), notable findings include a 2.2 prevalence of SUI operations post-hysterectomy, persistent links between vaginal hysterectomy (VH) and SUI operations, and significant associations with preceding pelvic organ prolapse (POP), vaginal deliveries, laparoscopic hysterectomy (LH), and urinary incontinence (UI) visits. Combining pelvic floor muscle (PFM) strengthening with hip synergic muscle exercises improves daily urine loss frequency, while modifiable risk factors such as achieving an ideal body weight and preventing traumatic vaginal delivery are identified for SUI prevention. Cost analysis indicates that savings from avoiding unnecessary surgical treatments through urodynamic investigations (UDIs) in uncomplicated patients cover associated expenses, emphasizing the need to consider potential legal actions in a comprehensive cost assessment of negative externalities

The association between weight-adjusted-waist index and stress urinary incontinence in female adults: a population-based study.

Obesity has a significant impact on female stress urinary incontinence. The weight-adjusted-waist index (WWI), a newly explored metric for measuring physical obesity and stress urine incontinence, has not been investigated. Cross-sectional data were obtained from the 2011 to 2018 National Health and Nutrition Examination Survey (NHANES) of participants with complete data for WWI and SUI. The independent connection between WWI and SUI was examined using weighted multivariable logistic regression analyses. Smoothed curve fitting was used to study their relationship. In addition, subgroup analyses were performed, and interaction tests were undertaken. In 2,361 female participants over 20, WWI and SUI showed a positive association. Model 3 found that for each one-unit increase in WWI, SUI prevalence increased by 38% [1.38 (1.20, 1.59)]. The prevalence of SUI was 84% higher among participants in the highest quartile of WWI compared to those in the lowest quartile [1.84 (1.32, 2.57)]. Observed nonlinear positive correlation between WWI and SUI was revealed through smoothed curve fitting. In addition, we found that WWI was more strongly associated with SUI than traditional indicators of obesity, including BMI and weight. Females with higher weight-adjusted waist index levels were more likely to develop stress urinary incontinence and have a stronger correlation than traditional indicators of obesity. According to our research, WWI may help detect women's stress urinary incontinence (SUI), and managing obesity through the WWI index may reduce SUI prevalence rates.

Stress Urinary Incontinence in Young Female Athletes- From Prevalence to Treatment- A Review

Urinary incontinence is a prevalent issue among females aged 15-65, with prevalence rates ranging from 10% to 55%. Stress Urinary Incontinence (SUI) is the most common form in young athletes, with rates ranging from 0% (golf) to 80% (trampolinists). Activities like jumping, landing, and running can cause leakage due to an increase in intraabdominal pressure. This study aimed to identify management strategies and the prevalence of SUI in teenage female athletes. A literature search was conducted on studies conducted between 1990 and the present, excluding abstracted and non-open access studies. The research found that the chance of urinary stress incontinence in young and nulliparous female athletes in sports ranges from 0% (golf) to 80% (trampolinists). These activities, including gymnastics, long jumps, track and field, and ball activities, significantly impact their standard of existence, psychological situation, and athletic performance. Effective conservative treatment options for SUI include behavioral and lifestyle modifications, pelvic floor exercises, and core strengthening exercises. Female athletes require significantly stronger pelvic floor muscles than inactive females, and contraction of these muscles should occur as the inner core muscles are engaged. Future studies, instruction, and focused therapies are needed for female athletes with SUI.

Cost Effectiveness of Concurrent Midurethral Sling at the Time of Prolapse Repair: Results From a Randomized Controlled Trial

ABSTRACT After pelvic reconstructive surgery for pelvic organ prolapse, postoperative stress urinary incontinence (SUI) commonly occurs, which leads many women to choose to have a midurethral sling placed at the time of surgery, even in cases without symptomatic preoperative urinary incontinence. Approximately 27.3% of women with a sling had de novo SUI despite this intervention. Full evaluation of the societal and economic implications brought by a midurethral sling placement attempting to prevent postoperative de novo SUI at the time of a pelvic reconstructive surgery have yet to be evaluated. This study aimed to evaluate the 1-year cost-effectiveness of a midurethral sling in the prevention of SUI at the time of prolapse surgery. This assessment was a randomized controlled trial based on patient resource utilization and patient-reported effectiveness. Study data were obtained via the OPUS randomized clinical trial, which was performed through the Pelvic Floor Disorders Network (a cooperative agreement network sponsored through the National Institute of Child Health and Human Development). A total of 337 women with anterior vaginal prolapse and without SUI symptoms set to undergo treatment from May 2007 to January 2011 were included in the study. Patients were randomly assigned to receive either sham incisions or a midurethral sling during vaginal prolapse surgery. Follow-up occurred at 3, 6, and 12 months postsurgery, with surveys, physical examinations, and medical history. Cost data were collected, and overall health utility in quality-adjusted life-years (QALYs) was calculated. Secondary outcomes were urinary incontinence-specific quality of life and cases of urinary incontinence, as defined by bothersome incontinence symptoms and positive cough stress tests. Initial surgical procedures and subsequent urinary incontinence-related health care were all used for recording health care sector costs. Total costs for study participants were collected via health care resource utilization data, including office visits, additional surgical procedures, and related procedures. The incremental cost-effectiveness ratio was the difference between the sham incision and midurethral sling groups in mean cost, divided by difference in mean QALYs. Data for health care resource utilization were collected during the trial period. Questionnaires at baseline, 3, 6, 9, and 12 months were used to collect other costs. At 1 year, one-way sensitivity analysis was performed for assessing the varying effectiveness of midurethral slings at 1 year, by noting the urinary incontinence-associated patient costs and by using varied QALY for the sling group. Results were taken from a study population of 337 women who underwent randomization to be included in the analysis of cost-effectiveness. No great variance of characteristics existed between groups. One year after surgery, a lower rate of urinary incontinence existed for those in the midurethral sling group. Notably, QALYs were not statistically different between midurethral sling and sham incision groups at baseline, but at 12 months, there was an improvement in both the UDI stress subscale and an overall reduction in the Incontinence Severity Index for the midurethral sling group. In conclusion, although prophylactic midurethral sling placements during vaginal prolapse surgery does reduce the rate of de novo SUI, based on this analysis of date from the OPUS trial, it is not a cost-effective intervention. The decision to place a sling in this setting is often driven by cost savings on the patient side, whereas midurethral sling health care costs are higher than when sling is not done.

Menopause and stress urinary incontinence: The risk factors of stress urinary incontinence in perimenopausal and postmenopausal women.

The prevalence of stress urinary incontinence (SUI) increases around menopause. The quality of life of perimenopausal and postmenopausal women with SUI is significantly affected. This study aimed to investigate the prevalence of SUI and the associated risk factors in a population of Chinese perimenopausal and postmenopausal women. A total of 273 perimenopausal and postmenopausal women were enrolled, and a cross-sectional study was conducted. SUI was defined as an involuntary loss of urine with increases in abdominal pressure. Data including personal characteristics, menopause information, estrogen levels, and pelvic floor muscle strength levels were statistically analyzed. The study enrolled 158 (57.9%) perimenopausal and 115 (42.1%) postmenopausal women. Sixty-six (41.8%) perimenopausal women and 56 (48.7%) postmenopausal women complained of SUI. The mean age was 49.42 ± 5.58 years. Body mass index over 24 kg/m2 (odds ratio [OR] 2.02, 95% confidence interval [CI] 1.07-3.81), vaginal delivery (OR 2.47, 95% CI 1.33-4.58), and diabetes (OR 4.65, 95% CI 1.23-17.62) were high-risk factors for SUI. Climacteric symptoms (evaluated by Kupperman index scores) were statistically related to SUI, and among the 13 symptoms, insomnia, nervousness, weakness and fatigue, arthralgia and myalgia, headache, palpitation, and sexual complaints were all correlated with SUI in perimenopausal and postmenopausal women. Several factors are associated with SUI in Chinese perimenopausal and postmenopausal women. Obesity, vaginal delivery, climacteric symptoms, and diabetes were identified as the most notable risk factors. The management strategy could focus on the prevention and management of risk factors.

Prevalence of Stress Urinary Incontinence and Risk Factors among Saudi Females.

Background and Objectives: Stress urinary incontinence (SUI) is involuntary urine leakage upon effort or physical exertion, sneezing, or coughing, and it is the most prevalent type of urinary incontinence (UI) in women. We aimed to estimate the prevalence of SUI and its risk factors among Saudi females. Materials and Methods: A descriptive cross-sectional study was conducted in the Kingdom of Saudi Arabia between March 2022 and July 2022, with a total of 842 respondents. We included Saudi females over the age of 20 years. Data were collected through an online questionnaire distributed to the target group and analyzed using SPSS software. Results: The prevalence of SUI was found to be 3.3% among Saudi women. Moreover, only 41.8% of the participants had at least one pregnancy; the majority had five or more pregnancies (29%). According to our findings, the majority of the participants diagnosed with SUI had the following risk factors: increased age, widowhood, a family history of SUI, and a history of pregnancy. The results revealed that the odds of SUI increased among Saudi females with a family history of SUI by 19.68-fold compared with those who had no family history of SUI, and this was statistically significant (p < 0.001). Conclusion: The prevalence of SUI among Saudi females was found to be relatively low. The above-listed associated factors should be considered in future research and interventions.

Postpartum stress urinary incontinence: a clinical study of 6,302 cases in Jiangsu Province.

To investigate the prevalence and influencing factors of stress urinary incontinence (SUI) within 6-8 weeks postpartum in Jiangsu Province. We designed a multi-center cross-sectional study involving seven hospitals in Jiangsu province, and enrolled women who underwent postpartum examination at 6-8 weeks in these hospitals between July 2019 and June 2021. According to the presence or absence of SUI, the enrolled patients were divided into two groups: the SUI group and the non-SUI group, respectively. We assessed the general health status, noted the details of delivery, and checked the pelvic floor electromyographic parameters of the postpartum women in both groups. Among 6,302 cases of postpartum women in Jiangsu province, there were 1,579 cases of SUI, with a prevalence of 25.06%. The prevalence of SUI increased significantly with age, BMI, increasing parity, coexisting constipation, organ prolapse, and diastasis recti abdominis. Compared to the non-SUI group, the SUI group had a lower mean value of the pre-baseline rest phase, shorter rise and fall times of fast muscle contractions, and a lower mean value of the endurance contraction phase. Multiple regression analysis revealed associations with weight (especially overweight and obesity), coexisting organ prolapse, constipation, parity, gestational week of delivery, mode of delivery, and mean value of endurance contraction phase. The prevalence of postpartum stress urinary incontinence in Jiangsu Province was 25.06%, and was linked to being overweight, parity > 2, coexisting organ prolapse, constipation, and a decrease in the mean value of the endurance contraction phase of the electromyograph. In this report, we offer a theoretical basis for the effective prevention of postpartum SUI clinically.

The Role of Gene Expression in Stress Urinary Incontinence: An Integrative Review of Evidence.

Stress urinary incontinence (SUI) is defined as unintentional urine leakage occurring as a consequence of increased intraabdominal pressure due to absent or weak musculus detrusor contractility. It affects postmenopausal women more often than premenopausal and is associated with quality of life (QoL) deterioration. The complex SUI etiology is generally perceived as multifactorial; however, the overall impact of environmental and genetic influences is deficiently understood. In this research report, we have disclosed the upregulation of 15 genes and the downregulation of 2 genes in the genetic etiology of SUI according to the accessible scientific literature. The analytical methods used for the analysis of gene expression in the studies investigated were immunohistochemistry, immunofluorescence staining, PCR, and Western blot. In order to facilitate the interpretation of the results, we have used GeneMania, a potent software which describes genetic expression, co-expression, co-localization, and protein domain similarity. The importance of this review on the genetic pathophysiology of SUI lies in determining susceptibility for targeted genetic therapy, detecting clinical biomarkers, and other possible therapeutic advances. The prevention of SUI with the timely recognition of genetic factors may be important for avoiding invasive operative urogynecological methods.

In women, SUI is associated with pelvic organ prolapse (POP) and in men after a radical prostatectomy. A safety review started in 2011 by the FDA of POP mesh resulted in the 2019 recall. This study reviews the prevalance and procedure trends for SUI between 2012 to 2020. METHODS: Using the 100% Optum® Clinformatics® Data Mart data and 100% national Medicare Fee-for-Service data, we identified subjects claims for urinary incontinence (UI) and any procedures performed for UI. Results reported as mean ± standard deviation. RESULTS: From 2012 - 2020, the mean prevalence of any UI in the 18-64 age group was 37,529 ± 3292 or 0.62% of the population. In the Medicare population (aged 65+), it was 1,439,221 ± 90507, or 5.7% of the group. The Female to Male ratio in the <65 yr group was 2.52:1 and in the 65+ was 5.31:1. The Medicare mean SUI prevalence was 212223 ± 14292 (0.84% population), and the Optum group was 13,179 ± 1,535 (0.22%).38,677 Medicare patients received procedures for UI in 2012. This increased to 54,122 by 2019, falling to 45,667 during COVID. In 2012, 12,286 patients received SUI procedures, which plateaued at 8,670/yr for 2015 to 2019. In 2020, 6508 patients had a SUI procedure. (Breakdown in Figure 1A). 4020 patients with UI aged 18-64 got a procedure in 2012, which decreased by 45% to 2635 in 2019 with a dip for COVID to 2020. The numbers plateaued from 2014 to 2019 at 2500 patients/yr approximately. The SUI patient numbers for this group decreased from 2501 in 2012 to 967 in 2020, plateaued between 2014 and 2019 at 1250 patients/yr approximately. (Breakdown in Figure 1B) There was a 50% decrease in patients getting sling procedures. Patients obtaining artificial urinary sphincter, and injectables remained constant. Men receiving artificial urinary sphincter, slings, and injectables has remained even in the 65+ age group. However, in the 18-64 age group, men obtaining slings decreased. CONCLUSIONS: The claims prevalence for UI has increased in older age and decreased in younger patients. Sling use has decreased in all female patients and younger men. The overall decrease in procedures for SUI, appears partly due to decreased reporting of incontinence claims in the younger population, together decreased Sling procedures in female patients. Prevalence of urinary incontinence in pregnant women in Turkey: A systematic review and meta-analysis

Background and objectiveUrinary incontinence is a common complaint in pregnant women. The prevalence of urinary incontinence increases as the week of gestation progresses. This study was conducted to determine the prevalence of urinary incontinence in pregnant women, types of urinary incontinence during pregnancy, and the prevalence of urinary incontinence by trimesters in Turkey. Study desingThis study is a systematic review and meta-analysis study. The publications meeting the inclusion criteria were searched between the dates September 1–30, 2022. A search was done in the PubMed and ScienceDirect, MEDLINE, Ovid, EBSCO CINAHL Plus, and Cochrane Library databases. The methodological quality of the studies was examined with the checklist developed by the Joanna Briggs Institute. ResultsTwenty articles were included in this study. According to the study results, the estimated urinary incontinence prevalence in pregnant women was 35 % (95 % CI: 0.288–0.423 Z:-3.984, p = 0.000, I2: 96.574), urinary incontinence was most frequently observed in the third trimester, and its estimated prevalence was 32 % (95 % CI:0.230–0.419 Z:-3.428, p = 0.001, I2: 96.400). Considering types of urinary incontinence during pregnancy, the prevalence of stress urinary incontinence was found in 10 studies, and according to the combined results of these studies, the estimated prevalence of stress urinary incontinence during pregnancy was 29 % (95 % CI:0.223–0.365, Z:-5.077, p = 0.000, I2: 94.678). ConclusionThe present study revealed that pregnancy increased the probability of urinary incontinence. While it is determined to be mostly experienced in the third trimester, approximately-one-third of pregnant women experience stress urinary incontinence.PROSPERO Registration no: CRD42022338643.

Prevalence and severity of urinary incontinence and associated factors in Iranian postmenopausal women: a cross-sectional study

BackgroundUrinary incontinence (UI) is one of the most common symptoms during menopause, leading to a decreased quality of life and limited social activities. This study aimed to determine the prevalence and severity of urinary incontinence and associated risk factors in postmenopausal women.MethodsIt was a cross-sectional study using cluster sampling on 433 postmenopausal women in Tabriz-Iran, 2021–2022. Data were collected using questionnaires of socio-demographic characteristics, Questionnaire for Urinary Incontinence Diagnosis (QUID), and International Consultation on Incontinence Questionnaire-Urinary Incontinence Short Form (ICIQ-UISF). Multivariate logistic regression was used to determine factors related to urinary incontinence.ResultsThe overall prevalence of urinary incontinence was 39.5%; 20.6% stress urinary incontinence (SUI), 10.4% urgency urinary incontinence (UUI), and 8.5% mixed urinary incontinence (MUI). Multivariate logistic regression analysis showed that the prevalence of SUI (aOR 0.38; 95% CI 0.18–0.77) and UUI (aOR 0.38; 95% CI 0.15–0.94) was significantly lower in women with three childbirths than the ones with fewer childbirths. Also, the odds of UUI increased significantly in women at the 50–55 age range (aOR 3.88; 95% CI 1.16–12.93) than those less than 50 years.ConclusionDue to the high prevalence of urinary incontinence in postmenopausal women, caregivers should screen for early diagnosis and appropriate treatment of urinary incontinence to prevent its destructive impact on the quality of life.

Gestational diabetes mellitus and pelvic floor function 6 weeks postpartum in Chinese women.

With the significant increase in its incidence, gestational diabetes mellitus (GDM) has received growing attention for its effect on pelvic floor function. This study was aimed at investigating the association of GDM with pelvic floor function and diastasis recti abdominis (DRA) in postpartum women. This is a retrospective cohort study. At 6 weeks postpartum, 1,133 postpartum women with vaginal delivery underwent routine examinations including measurement of the pelvic floor muscle (PFM) strength and endurance, determination of the stress urinary incontinence (SUI) by questionnaire, quantification of pelvic organ prolapse (POP) and assessment of DRA. Statistical analysis was performed using binary logistic regression and multiple linear regression analysis. One hundred and seventy-six (176) of the 1,133 women were confirmed to be suffering from GDM, with a rate of 15.53% (176 out of 1,133). The age and pre-pregnancy body mass index of the GDM group were significantly higher than those without GDM (p < 0.05). The GDM group was more likely to have smaller gestational age and a higher chance of having to undergo a lateral episiotomy. No statistically significant differences are found in PFM endurance (B: -0.025, p = 0.462) or PFM strength (B: -0.001, p = 0.979) between women with and without GDM. And these two groups are not significantly different in terms of the prevalence of SUI (19.3% vs 20.4%), POP (35.8% vs 37.5%) and DRA (29.0% vs 25.8%; p > 0.05). Pelvic floor muscle function and SUI/POP/DRA prevalence of women at 6 weeks postpartum are not significantly affected by GDM.

A Retrospective Analysis of the Effects of Second - Stage Labor Arrest on Postpartum Women’s Stress Urinary Incontinence

Background: Stress Urinary Incontinence (SUI), has been always linked to normal vaginal delivery (NVD) or operative vaginal delivery (OVD). Many women get concerned about this issue before giving birth and tend to deliver by cesarean delivery. However, the increased frequency of cesarean deliveries poses health concerns especially in the absence of solid risk factors. Methods: This is a retrospective observational cohort study conducted at Rafic Hariri University Hospital (RHUH) between 2011 and 2013. Forty primiparous women undergoing cesarean delivery were involved. They were evenly divided into 2 groups. The first involved those who underwent elective cesarean delivery and those with failure to progress. The second group involved women who underwent cesarean delivery for arrested second stage of labor. Statistical analysis was performed using the statistical program SPSS version 20. Results: The prevalence of SUI in group 1 was 25% compared to 35% in group 2. Cesarean delivery was not found to have any protective role against SUI in group 1 patients and the frequency of SUI in selective cesarean patients was nearly the same as those performing cesarean for failure to progress. However, patients undergoing cesarean for arrested second stage of labor &gt; 2hours had a significant higher frequency of SUI as compared to patients undergoing cesarean delivery for arrested second stage of labor &lt; 2hours. Conclusion: Cesarean delivery cannot be proposed to women who fear to suffer from SUI post-partum. However, it should be done if the cervix fully dilated for &gt; 2hours in order to decrease the risk of SUI

Comparison Of Prevalence Of Stress Incontinence Among Multiparous And Primiparous Women After One Year Of Delivery

Objective: To find out and compare the frequency of Stress Urinary Incontinence among multiparous and primiparous women.Place of Study: Nishter Hospital, Multan.Study Design: A Cross-Sectional Study.Duration of Study: Sept 2021 to Dec 2021Methodology: After permission from the institutional review committee of Nishter Medical University, Multan, a hospital-based, face-to-face, cross-sectional study was carried out by employing 250 participants as a sample size. The convenience sampling technique was utilized for the recruitment of participants from Nishter Hospital, Multan. All the patients visiting the OPD of the Gynecology department after approximately one year of the last delivery were included in the study. Participants with red flags including urinary tract infection, bladder calculus, prostatic hyperplasia, and neoplasm were omitted from the survey. Data was collected by using a well-developed questionnaire for prevalence containing questions pertaining to age, parity, and symptoms like Loss of urine while, lifting weights, exercising, coughing, sneezing, laughing, and during sexual intercourse to find out the association of symptoms with the parity status of the women. Statistical Package for Social Sciences (SPSS) Version 21 was used to analyze the data. The chi-square test was used to assess the association between urinary incontinence and parity.Results: In this study, 57 women had single children, 39 women had two children, 47 had three children and 107 women had &gt;3 children. Of the women who reported the loss of urine while lifting weights 17.5% (primiparous), 30.8 % (2 children), 36.2 % (3 children), and 47.6% were multiparous. The primiparous Women who reported urine leakage during exercise were 12.2% and multiparous was 30.8%. 17.7% of women &gt;3 children stated un-intentional loss of urine during coughing, sneezing, or laughing while only 8.8% primiparous reported the un-intentional urine loss. During sexual intercourse none of the primiparous experienced leakage of urine while 4.6% of multiparous complained about the loss of urine during sexual intercourse.Conclusion: The prevalence of stress urinary incontinence is strongly associated with the parity status of women.
