Prevalence and risk factors for diabetic peripheral neuropathy, neuropathic pain and foot ulceration in the Arabian Gulf region.

Aims/IntroductionThis study determined the prevalence and risk factors for diabetic peripheral neuropathy (DPN), painful DPN and diabetic foot ulceration (DFU) in patients with type 2 diabetes in secondary healthcare in Qatar, Kuwait and the Kingdom of Saudi Arabia.Materials and MethodsAdults aged 18–85 years with type 2 diabetes were randomly enrolled from secondary healthcare, and underwent clinical and metabolic assessment. DPN was evaluated using vibration perception threshold and neuropathic symptoms and painful Diabetic Peripheral Neuropathy was evaluated using the Douleur Neuropathique 4 questionnaire.ResultsA total of 3,021 individuals were recruited between June 2017 and May 2019. The prevalence of DPN was 33.3%, of whom 52.2% were at risk of DFU and 53.6% were undiagnosed. The prevalence of painful DPN was 43.3%, of whom 54.3% were undiagnosed. DFU was present in 2.9%. The adjusted odds ratios for DPN and painful DPN were higher with increasing diabetes duration, obesity, poor glycemic control and hyperlipidemia, and lower with greater physical activity. The adjusted odds ratio for DFU was higher with the presence of DPN, severe loss of vibration perception, hypertension and vitamin D deficiency.ConclusionsThis is the largest study to date from the Middle East showing a high prevalence of undiagnosed DPN, painful DPN and those at risk of DFU in patients with type 2 diabetes, and identifies their respective risk factors.

A Study on Peripheral Neuropathy and Its Related Risk Factors Associated With hba1c Levels

Diabetic peripheral neuropathy (DPN) patients frequently feel persistent pain, which is described as painful diabetic peripheral neuropathy (PDPN), which begins in both feet and frequently spreads to the calves, fingers, and hands. PDPN not only causes pain, but also affects patients' sleep, emotions, mental state, and everyday activities, resulting in a low quality of life and a significant financial burden. The goal of this study was to monitor if there was a link between the prevalence, pattern, and related risk factors of diabetic peripheral neuropathy and hemoglobin A1C (HbA1c) levels. In this crosssectional study, 150 type-2 diabetic patients were screened for DPN with PDPN and their HbA1c level was measured in every three months. DPN, PDPN and non-painful DPN were confirmed in patients displaying both clinical manifestations of neuropathy and neurological abnormalities assessment. DPN was detected in 24% (n = 36), while PDPN was found at 15% (n = 23) of the total patients. The prevalence of PDPN is 63.88% (n = 23) and non-painful DPN is 36.11% (n = 13) of total DPN (n = 36). Out of total PDPN (n = 23), the prevalence of symmetrical pain is 65% (n = 15), asymmetrical 35% (n = 8), sensory 26% (n = 6), motor 13% (n = 3), mixed (sensorimotor) 61% (n = 14), lower limb involvement 48% (n = 11), upper limb13% (n = 3) and both limb 39% (n = 9). In comparison to patients without DPN, both PDPN and non-painful DPN, patients had greater HbA1c levels (p<0.05). Furthermore, advanced age and longer diabetes duration were considerable and significant (p<0.05) risk factors for DPN with PDPN and non-painful DPN respectively. Overall, the findings imply that elevated HbA1c levels are closely linked to DPN, PDPN and non-painful DPN in type-2 diabetic patients and that HbA1c might be used as a predictive marker for DPN with PDPN and non-painful DPN in the patients studied. Prevalence and associated factors of painful diabetic peripheral neuropathy among diabetic patients on follow up at Jimma University Medical Center.

Despite having significant impact on the patient's quality of life, painful diabetic peripheral neuropathy (PDPN) is usually underdiagnosed. Screening for PDPN in patients with diabetes is needed in order to get timely identification and management. Hence, the purpose of this study was to assess the prevalence and determinants of PDPN among diabetes patients attending outpatient at Jimma University Medical Center from September 1 November 10, 2019. Hospital based cross-sectional study was conducted and douleur neuropathique-4 was used to identify the presence of PDPN. Data were collected using pretested structured questionnaire and entered into EPI data3.1 and exported to SPSS version 20 for analysis. Both bivariate and multivariate binary logistic regression was employed to identify factors associated with PDPN. Variable having a p value of <0.25 in the bivariate model were considered as candidates for multivariable regression. Adjusted odds ratios were calculated at 95%CI and considered significant with a p value of <0.05. A total of 366 diabetic patients were enrolled into the study and their mean duration of diabetes was 6.8 ± 5.3years. The study finding showed that the prevalence of PDPN was 14.5%. According to the multivariate logistic regression analysis smoking [current smoker(AOR = 6.17; 95%CI:2.25,16.86),former smoker(AOR = 3.22;95%CI:1.29,8.03)],diabetes duration[5 to 10years (AOR = 3.32;95%CI:1.29,8.53), ≥ 10years (AOR = 8.86;95% CI: 3.49,22.5)] and comorbid hypertension [AOR = 2.54; 95%CI:1.17,5.49] were independent predictors of PDPN among study participants. The overall prevalence of PDPN in this study was 14.5% and it was significantly associated with smoking, comorbid hypertension and diabetes duration of above 5years. Early detection and appropriate interventions are important for high risk patients identified in the current study.

Prevalence of painful diabetic peripheral neuropathy and its impact on quality of life among diabetic patients in Western region, Saudi Arabia.

Background:Diabetic neuropathy is the most common long-term complications of diabetes, frequently presenting as painful diabetic peripheral neuropathy (PDPN), which can significantly impair patients’ quality of life (QOL). This study set to estimate the prevalence of PNPD and health-related quality of life (HRQoL) in the setting of primary health care in Saudi Arabia.Methods:This study was conducted in primary health-care centers affiliated with the National Guard Health Affairs in Western Saudi Arabia. Arabic version of the Douleur Neuropathique 4 questionnaire was administered on diabetic patients to screen for neuropathic pain and short-form 12 questionnaire to assess HRQoL.Results:The study screened (n = 349) Type 2 diabetic patients. The prevalence of PDPN was 33.2%. PDRN was more likely to affect females (adjusted odds ratio [“AOR”] =1.96, P = 0.024), and those living with diabetes for over 15 years (AOR = 2.26, P = 0.039), and those on insulin treatment (AOR: 2.33, P = 0.010) alone or in combination (AOR = 1.78, P = 0.034). Both physical and mental components (MCs) of QOL scores were significantly higher in diabetic patients without PDPN compared to those with it; 49.57 ± 9.31 versus 40.77 ± 8.14 for physical component QOL and 51.72 ± 9.36 versus 44.35 ± 8.12 for MC QOL, P < 0.001.Discussion and Conclusion:Painful peripheral neuropathy is relatively common among type 2 diabetic patients in Western Saudi Arabia and impacts both physical and MCs of the QOL of affected patients.

PREVALENCE AND IMPACT OF PAINFUL DIABETIC PERIPHERAL NEUROPATHY IN PATIENTS WITH TYPE-2 DIABETES MELLITUS WITH LOW ECONOMIC STATUS

Objectives: The objectives of the study were to study the prevalence of painful diabetic peripheral neuropathy (PDPN) in patients with type-2 diabetes and to access the health-related quality of life of patient by collecting and documenting the information of patient using sf-15 questionnaire.Methods: The present study involves prospective analysis of quality of life in both men and women with PDPN with type-2 diabetes. The methodology involves collection and documentation of patients medical and medication history, blood glucose levels, and duration of diabetes. Further, the quality of life of patient is documented using Michigan neuropathic scaling instrument (sf-15 questionnaire) through which we can assess the impact of PDPN.Results: It can be seen that there are no significant changes in the prevalence of PDPN when compared gender wise and based on age, but there are an extremely statistically significant (*p&lt;0.05) values obtained when the presence of PDPN is compared between duration of diabetes.Conclusion: Patients with duration of diabetes type-2 between 5 and 10 years are mostly presented with PDPN. Early detection, nutritious diet, and patient counseling help in management of PDPN.
