Impact of Caries Experience on the Oral Health-Related Quality of Life of Pre-school Children and their Families in an Indian City - An Evaluative Study.

The presence of dental caries may have a negative impact on well-being of children and their parents, causing pain and discomfort, leading to poor oral health-related quality of life (OHRQoL) in them. The objective of this study was to evaluate the impact of different levels of caries experience on the OHRQoL of pre-school children and their families in Odisha, assessed using Odia version of Early Childhood Oral Health Impact Scale (Od-ECOHIS). A cross-sectional survey was carried out on a sample of 369 pre-school children using Od-ECOHIS. Dental caries was recorded using International Caries Detection and Assessment System (ICDAS) index. The total score of ECOHIS ranged from 0 to 42 with a mean of 2.4 (±5.3) and a median score of 0. The items bearing high mean scores in the child impact section were related to "pain", "difficulty in drinking hot or cold beverages", "difficulty in eating some food", and "irritable or frustrated". Though half of children (50.4%) had cavitated carious lesions, the prevalence of negative impact was 30.6%. Compared to the child being caries-free, the expected count of total Od-ECOHIS score increased by 1.54 times if the child had white spot lesion and 1.94 times if the child had a cavitated lesion. Presence of dental carious lesions has a significant impact on OHRQoL of pre-school children and their families measured by using Od-ECOHIS even after adjusting for SES and caregivers' perception of their children's oral health.

Premature loss of primary molars: impact on the oral health-related quality of life of schoolchildren.

Evaluate the impact of the premature loss of primary molars due to caries on oral health-related quality of life (OHRQoL) in schoolchildren. A cross-sectional study was conducted with children eight to ten years of age at public schools in the city of Florianópolis selected through cluster sampling. The collection of the clinical data occurred in the school setting by four trained and calibrated examiners (Kappa > 0.7) DMFT/dmft index, PUFA/pufa index, accentuated overjet, and anterior open bite. Sex and age of the child and guardian's schooling (classified as ≤ 8 or > 8years) were collected through a questionnaire answered by the guardians. OHRQoL was assessed using the Child Perceptions Questionnaire 8-10. The data were submitted to bivariate, multivariate, and hierarchical Poisson analyses. A total of 1456 children participated in the study. The prevalence of premature tooth loss due to caries was 5.8% (n = 85). The prevalence of negative impact on OHRQoL was 47.4% (n = 691). Negative impact on OHRQoL was significantly associated with the premature loss of primary molars (p < 0.001) as well as the sex (girls), guardian's schooling (> 8years) and the presence of untreated caries (p < 0.001). The premature loss of primary molars due to caries was associated a negative impact on the OHRQoL of children eight to ten years of age. Negative impact is also associated with untreated caries, sex, and the level of schooling of guardians.

Tooth Loss, Sociodemographic Conditions and Oral Health-Related Quality of Life in the Elderly

Objective: To verify the association between impacts produced by tooth loss and sociodemographic variables in the oral health-related quality of life (OHRQoL) in the elderly. Material and Methods: This is an analytical cross-sectional study carried out in the municipality of Vitória/ES, Brazil, which sample was composed of 402 older adults. The Oral Health Impact Profile (OHIP-14) was used to measure the individuals' OHRQoL. For data analysis, descriptive and bivariate analyses were performed using the Fisher's exact test and the Mantel-Haenzsel test to assess the effects of combined dimensions. The significance level adopted was 5%. To assess the strength of association between independent variables and dimensions, Odds-Ratio (OR) with 95% confidence interval (CI) was calculated. Results: The prevalence of negative impact on OHRQoL in the elderly was 32.6%. The greatest impact perception was found in individuals with up to 10 years of schooling (p=0.021 and OR=1.602), with need for removable partial dentures (p=0.000 and OR=2.873) and those who did not accept the loss of dental elements (p=0.000 and OR=3.064). Conclusion: Older female adults with socioeconomic class C/D-E, with up to 10 years of schooling, who lost 11 or more teeth, who declared the need for removable partial dentures, suffered greater impacts caused by tooth loss on OHRQoL.

Impact of temporomandibular disorder on oral health-related quality of life in adolescents

The aim of this cross-sectional study was to assess the impact of temporomandibular disorder (TMD) on oral health-related quality of life (OHRQoL) of adolescents. The OHRQoL of adolescents undergoing dental treatment at a University clinic in 2019 was measured using the Brazilian version of the Oral Heath Impact Profile – 14 (OHIP-14). The diagnosis of TMD was performed using Research Diagnostic Criteria for Temporomandibular disorders (RDC/TMD). The patients were examined for other oral conditions and the parents/guardians answered questions addressing socioeconomic/demographic characteristics and the general health of the adolescents. Statistical analysis involves simple and multiple logistic regression models. Ninety male and female adolescents between 13 and 18 years of age participated in the study. The prevalence of negative impact on OHRQoL was 34%. In the unadjusted analysis, negative impact on OHRQoL was associated with a poorer self- perception of general and oral health of the adolescent, nonspecific symptoms including pain, and generalized anxiety disorder, caries, reports of dental pain, muscle disorders and disc displacement, and chronic pain related to TMD. In the adjusted model, negative impact on OHRQoL was associated with all diagnoses related to TMD on the RDC/TMD, except signs of depression. Adolescents with at least one diagnosis related to TMD were 4.13-fold more likely (95% CI:1.08-15.80) to have negative impact on OHRQoL than adolescents without a diagnosis of TMD. The different diagnostic categories of TMD had a negative impact on the OHRQoL of the adolescents analyzed in the present study.

Early childhood caries and oral health-related quality of life of Brazilian children: Does parents' resilience act as moderator?

Early childhood caries (ECC) is the most common chronic disease in childhood. Measures to reduce the prevalence of ECC cannot be taken without recognizing that oral health is influenced by biopsychosocial factors on individual and community levels. To evaluate the impact of ECC on OHRQoL and moderation effect of parents' resilience. Population-based, cross-sectional study with 497 children (4-6years old) at preschools in Ribeirão das Neves, MG, Brazil. Parents self-completed the Brazilian version of the early childhood oral health impact scale (B-ECOHIS), the Resilience Scale and questionnaires about socioeconomic and oral health behaviour factors. Calibrated dentists conducted examinations for ECC (ICDASepi) and pulp consequences (pufa). Data analysis used multivariate Poisson regression for complex sample (P<.05). Model#1, adjusted for parents' resilience and socioeconomic factors, revealed that preschoolers with pulp involvement had 2.36 (95% CI: 1.60-3.49) and fistula/abscess had 3.57 (95% CI: 2.23-5.72) more prevalence of negative impact on OHRQoL than preschoolers with ECC without pulp consequences. In Model#2, resilience was removed from the analysis and the strength of associations almost did not change (OHRQoL vs pulp involvement RP=2.33;95% CI: 1.58-3.43; OHRQoL vs fistula/abscess RP=3.65;95% CI: 2.22-5.99). Early childhood caries with pulp consequences had negative impact on OHRQoL of preschoolers and families, and it is not moderated by parents' resilience.

Impact of oral health conditions on the quality of life of quilombola and non-quilombola rural adolescents in the countryside of Bahia, Brazil: a cross-sectional study

BackgroundThis study aimed to estimate the prevalence of negative impacts of oral health conditions on the quality of life of quilombola and non-quilombola rural adolescents and identify associated factors.MethodsThis cross-sectional study was carried out in a rural area in the countryside of Bahia, Brazil, in 2015. Participants were asked to complete the Oral Impacts on Daily Performance Questionnaire. Prevalence and prevalence ratios (PR) were estimated together with their respective 95% confidence intervals. Multiple analysis was conducted using Poisson regression with robust error variance and hierarchical entry of variables.ResultsOf the 390 rural adolescents who took part in the study, 42.8% were quilombolas, and 45.6% of all participants reported a negative impact of their oral health conditions on their quality of life. The most prevalent impact was difficulty eating (32.6%). After adjusted analysis, the following factors were found to be associated with the negative impact of oral health conditions on quality of life: age (PR = 1.04), feeling lonely (PR = 1.42), worst evaluation of oral health (PR = 1.52), need of dental care (PR = 1.33), and occurrence of toothache in the last 6 months (PR = 1.83). Quilombolas and non-quilombolas presented with a different prevalence of discomfort when brushing their teeth and had different factors associated with the negative impact of oral health conditions on their quality of life. Both quilombola and non-quilombola rural adolescents showed a high prevalence of negative impact of oral health conditions on their quality of life.ConclusionsThese results support the need for improved oral healthcare for specific populations like the quilombolas. Furthermore, the results illustrate the importance of incorporating oral healthcare strategies that take into consideration the sociocultural context of adolescents.

Oral health-related quality of life in breast cancer survivors.

The aim of the present study was to evaluate oral health-related quality of life (OHRQoL) among breast cancer survivors and identify possible factors associated with the outcome. A cross-sectional study was conducted with 151 women in follow-up at a hospital after treatment for breast cancer. Data were collected on socio-demographic characteristics, general health, and breast cancer. Clinical examinations were performed to determine caries experience using the decayed, missing, and filled teeth (DMFT) index. The Oral Health Impact Profile (OHIP-14) questionnaire was administered for the assessment of OHRQoL. Logistic regression analysis was performed to identify associated factors after adjustments for confounding variables. The mean OHIP-14 score was 12.8 (SD 10.92). The prevalence of negative impact was 58.9%. A negative impact was associated with depression, the diagnosis of breast cancer, chemotherapy, number of restored teeth, and xerostomia (p < 0.05). In the adjusted analysis, only the oral variables remained significantly associated with the outcome. A greater number of restored teeth and xerostomia exert a negative impact on the OHRQoL of women who have survived breast cancer, which suggests the need for special attention to the oral health of this population.

Anxiety symptoms have a direct effect on oral health perception in young women.

Assess the influence of anxiety symptoms on oral health related-quality of life in young women from a cohort study in Southern Brazil. A sample of 535 young mothers were analyzed. Interviews and psychological evaluations were carried out by trained psychologists. The Brazilian version of the Corah's dental anxiety scale and the Beck anxiety inventory were used to evaluate dental anxiety and anxiety symptoms, respectively. The oral health-related quality of life (OHRQoL) was assessed by the Oral Impacts on Daily Performance instrument. Dental examinations were performed by trained dentists to assess oral health status (DMFT). The effect of anxiety symptoms on oral health-related quality of life was estimated using the parametric g-formula. The prevalence of negative impact on OHRQoL was of 46.3 and 28% of the women presented anxiety symptoms. Unadjusted analysis showed that women with anxiety symptoms had 2.5 higher impact on OHRQoL (OR 2.55; CI 95% 1.72-3.79). The parametric g-formula revealed that anxiety had a direct effect on oral health perception (OR 1.16; CI 95% 1.04-1.30), not mediated by dental anxiety. Oral health-related quality of life is influenced by anxiety symptoms, regardless of dental anxiety.

Association between oral conditions and functional limitations in childhood.

The aim of this study was to evaluate the impact of oral conditions on functional limitations among preschoolers. A preschool-based, cross-sectional study was carried out with 843 preschoolers in Campina Grande, Brazil. Parents/caregivers answered a questionnaire addressing socio-demographic characteristics and perceptions regarding the general/oral health of their children as well as the Brazilian version of the Early Childhood Oral Health Impact Scale. The nonparametric Kruskal-Wallis test followed by Mann-Whitney test (α = 5%) was used to compare mean children's quality-of-life scores for each independent variable. Poisson regression analysis was used to test associations between the independent and dependent variables (difficulties eating, drinking and speaking) (α = 5%). The multivariate regression model involved a hierarchical approach with four levels (distal to proximal determinants): (i) socio-demographic aspects; (ii) health perceptions; (iii) oral conditions; and (iv) pain conditions. The prevalence of negative impact on function was 24.7% for eating/drinking and 8.0% for speaking. Significant associations were found between toothache and negative impact on eating/drinking (PR = 5.38; 95%CI: 3.20-9.02) as well as between high severity dental caries and negative impact on speaking (PR = 14.91; 95%CI: 1.98-112.32). Dental caries, traumatic dental injury and malocclusion were not significantly associated with a negative impact on eating or drinking. However, toothache was an indicator of negative impact on eating/drinking and dental caries severity was an indicator of negative impact on speaking.

Malocclusion and quality of life in Brazilian preschoolers

The aim of the present study was to assess the impact of malocclusion on oral health-related quality of life (OHRQoL) among preschool children and their families. This study involved 732 preschoolers. Parents/caregivers filled out the Brazilian version of the Early Childhood Oral Health Impact Scale (B-ECOHIS) and a questionnaire addressing sociodemographic data. Overbite, overjet, and crossbite were recorded during the clinical examination. Bivariate and multiple Poisson regression analyses were performed and prevalence ratio (PR) were calculated (α = 5%). The prevalence of negative impact from malocclusion on OHRQoL was 27.6% among the children and 22.3% among the families. Mother's schooling (PR = 1.37; 95% CI: 1.10-1.70), parent's/caregiver's assessment of child's oral health (PR = 2.07; 95% CI: 1.60-2.58), history of toothache (PR = 3.84; 95% CI: 2.34-6.30), and visits to the dentist (PR = 0.59; 95% CI: 0.37-0.94) remained significantly associated with OHRQoL in the final model for the children, whereas parent's/caregiver's assessment of child's oral health (PR = 2.32; 95% CI: 1.71-3.14) and history of toothache (PR = 2.28; 95% CI: 1.69-3.09) remained significantly associated with OHRQoL for the families. Malocclusion was not associated with a negative impact on OHRQoL. In contrast, parents'/caregivers' perceptions regarding the oral health of their children and a history of toothache were predictors of a negative impact on the OHRQoL.

Impact of Traumatic Dental Injury on the Quality of Life of Brazilian Preschool Children

This study aimed to assess the impact of traumatic dental injury (TDI) on the quality of life of preschoolers and their families. A cross-sectional study was carried out, with a sample of 814 children, aged three to five years old, in Campina Grande, Brazil. Parents/caregivers were asked to complete the Brazilian Early Childhood Oral Health Impact Scale and a questionnaire on socio-demographic data. Oral examinations of the children were performed by three previously calibrated dentists. Bivariate and multiple Poisson regression analyses were performed (α = 5%). The prevalence of negative impact from oral conditions on quality of life was 31.1% among the children and 24.7% among the families. TDI was not associated with a negative impact on quality of life. Parent/caregiver’s assessment of the child’s oral health (PR = 1.210; 95% CI: 1.027–1.426) and history of toothache (PR = 4.997; 95% CI: 2.943–8.493) remained in the final model for the child section, whereas only a history of toothache (PR = 2.791; 95% CI: 1.801–4.325) remained in the final model for the family section. TDI exerted no negative impact on quality of life in the present sample. A history of toothache was the only variable associated with a negative impact on the quality of life of the preschoolers and their families.
