Event Abstract Back to Event MEG contribution for lesional epilepsy surgery : frequency domain approaches Romain Bouet1, Julien Jung2, Claude Delpuech3*, Philippe Ryvlin2, Jean Isnard2, Olivier Bertrand1 and François Mauguière2 1 INSERM U821, France 2 HCL, France 3 CERMEP/INSERM U821, France Epilepsy surgery is an established therapy for pharmacoresistant focal epilepsy.This study investigated the contribution of routinely used MEG to help surgery in lesional context.Some lesions are difficults to delineate (dysplasia), incomplete resection of this kind of lesion is the main predictor of poor postsurgical outcome. Also, irritative area might be diffuser than focal lesion (cavernoma).MEG is increasingly used in the presurgical management of patients with partial epilepsies refractory to combined pharmacotherapy. We investigated interictal epileptic spikes wich are defined as fast transient.Interictal epileptic spikes were associated with a short lasting, transient power increase in a relatively broad frequency. We recorded 60 lesionals patients with CTF-VSM 275 channels system. By recording 45 minutes, it can trace field changes with a time resolution below 1 ms and permits the localization of sources with an accuracy of several millimeters. It also allows the assessment of the frequency content of neural oscillations. It is ideal for the study of cerebral activity that changes quickly over space, time, and frequency. We show that by using advanced MEG source analysis, it is possible to identify the origin of spike, his volume and his propagation. Adaptive spatial filters as used in the proposed approach (Dynamic Imaging of Coherent Sources, DICS) use the spike frequency properties to calculate source power at each volume element in the brain. This seems to be particularly beneficial when several heterogeneous spikes originating from a source with relatively large extension are averaged or with intraspike propagation. In summary MEG is a useful tool in the routine workup for lesional epilepsy surgery contributing information to focus volumes hypothesis and help Stereotaxic Electroencephalography. To adresse these issues we analysed clinical, EEG, MRI and PET datas to extract specific MEG contribution.
