Implementation of Contextual learning Technology in the Formation of Professional and legal culture of Cadets at a Departmental University

The article actualizes the problem of forming the professional and legal culture of cadets of educational organizations of the Ministry of Internal Affairs of Russia using contextual technology. The topic under consideration is more relevant than ever, since the main task in training cadets of educational organizations of the Ministry of Internal Affairs of Russia at the present stage is not only the formation of relevant special knowledge in them, but also the development of certain personal qualities necessary for future employees of internal affairs bodies for the competent implementation of professional activities. A key position in the professional activity of an employee is played by professional and legal culture, which determines the degree to which students have mastered their profession and their readiness to perform it well. The article analyzes the implementation of contextual technology in educational organizations of the Ministry of Internal Affairs of Russia, aimed at developing a professional and legal culture among cadets, which is an integral quality of the personality of an internal affairs officer. The understanding of the categories “context” and “contextual technology” is analyzed, and some imperfections of the traditional approach to training cadets are noted. The article presents the leading forms of activity in the implementation of contextual technology: educational activities of the academic type, quasi-professional, educational-professional. The purpose of the study is to develop contextual technology that contributes to the formation of professional and legal culture of cadets of educational organizations of the Ministry of Internal Affairs of Russia. Methodology. The study was conducted using theoretical research methods and involved the development of contextual technology as an effective means of developing the professional and legal culture of cadets of educational organizations of the Ministry of Internal Affairs of Russia. In conclusion, it is concluded that the developed contextual technology makes it possible to improve the formation of professional legal culture among cadets as an integral quality of a specialist.

Emerging Trends in Lung Cancer Presentation at a Leading Tertiary Oncology Center and the Need for Lung Cancer Screening in Pakistan.

Background Lung cancer is a significant global health concern, with Pakistan lacking a national cancer registry. Methods A retrospective study analyzed 345 lung cancer patients at Shifa International Hospital from 2018 to 2020. Demographics, symptoms, cancer stage, site, and histological types were assessed using descriptive statistics and Pearson'schi-square test. Results Most patients were male (267, 77.4%) with a mean age of 45-75 years. Common symptoms included patients with cough (168, 49%), blood in sputum (114, 33%), and chest pain (120, 35%). Most cases presented at advanced stages (81% at stage 4). Adenocarcinoma (131, 38%) was the most common histological type, with significant differences noted between smokers and non-smokers. Conclusion Gender disparities exist in lung cancer incidence, with a higher proportion of male patients. Both smokers and non-smokers are affected, emphasizing the need for comprehensive screening. Early detection and intervention strategies are crucial, especially considering the advanced stage of presentation. Tailored approaches considering histological types are essential for effective management. This study underscores the urgency of implementing screening programs and raising awareness to mitigate the burden of lung cancer in Pakistan.

390. CONTINUED IMPLICATIONS OF THE COVID-19 PANDEMIC ENVIRONMENT ON ESOPHAGEAL CANCER CHARACTERISTICS AND TREATMENT IN THE UNITED STATES

Abstract Background The COVID-19 pandemic significantly impacted the stage of presentation and subsequent therapies for patients who were diagnosed with esophageal cancer during the first pandemic year. We aimed to assess for continued implications of the altered healthcare landscape on esophageal cancer presentation and treatment in the second year of the pandemic. Methods This was a retrospective review of the United States National Cancer Database (2019-2021). Demographic, cancer-related and treatment variables were compared between patients diagnosed in the pre-pandemic year (2019), pandemic year one (2020) and pandemic year two (2021). Multivariate logistic regression was performed to control for the impact of demographics on oncologic variables, and then for the impact of oncologic variables on treatment modalities and outcomes. Results Of 42,807 esophageal cancer cases, 15,127 (35.3%) were pre-pandemic, 13,975 (32.7%) were pandemic year 1, and 13,705 (32.0%) were pandemic year two. There were significant differences in the distribution of clinical stage at presentation by year (p=0.0001), see figure, suggesting more advanced stage presentation throughout the pandemic. Compared to the pre-pandemic year, patients diagnosed in pandemic year two had risk-adjusted increases in time to first course treatment (incidence rate ratio 1.057 [95% confidence interval [1.036–1.078]), to radiation (1.029 [1.008–1.050]), and to systemic therapy (1.035 [1.018–1.051]). Additionally, when compared to pandemic year one, patients in pandemic year two continued to experience risk-adjusted increases in time to first course therapy (1.084 [1.063–1.106]), to surgery (1.049 [1.000–1.097]), to radiation (1.046 [1.024–1.068]) and to systemic therapy (1.060 [1.043–1.077]). Conclusions Compared to the pre-pandemic year, patients diagnosed with esophageal cancer in in the U.S. during pandemic year two continued to experience delays to treatment including delays to first course therapy, including delays to surgery, radiation and systemic therapy. The treatment characteristics of esophageal cancer have not returned to pre-pandemic baseline in the United States possibly due to compounding delays to diagnosis and treatment and a growing back log of cases.

Forming inflection and word-formation operations in preschoolers with mental retardation

Introduction. At the present stage of societal development, great attention is paid to issues affecting the psychological and pedagogical support in the educational process of children with disabilities. One general pattern of dysontogenetic development is violation of speech mediation. The most important areas of assistance to children with special educational needs are: identifying speech defect specifics and creating additional special conditions that will facilitate assimilation of educational programs and the social development of each student’s personality. The study aims to develop and test a program for forming inflection and word-formation operations in preschoolers with mental retardation. Materials and methods. The study involved 92 preschoolers aged 6-7 with mental retardation. The research methods were: Methodology of psycholinguistic study of oral speech disorders in children by R.I. Lalaeva, Diagnostics of oral speech of children aged 4-7 by N.V. Nishcheva; mathematical statistics: χ2-Pearson criterion. Results. The ascertaining experiment showed that preschoolers with mental retardation are characterized by predominantly low level of formation of inflection and word-formation operations. Specific difficulties were identified: omission or incorrect use of prepositions in speech; errors in changing nouns in indirect cases; deficiencies in the agreement of an adjective with a noun in the neuter gender; lexical substitutions of nouns, verbs when forming new words using suffixes and prefixes; replacement and incorrect use of suffixes while forming relative and possessive adjectives, etc. After the formative experiment, significant shifts occurred both in the level of noun inflection operations (χ2 = 19.739; p ≤ 0.05) where the number of children at a low level decreased from 64% to 32%, and in the level of verb word-formation operations (χ2 = 30.61; p ≤ 0.05) where the number of children at an average level increased from 39% to 58%. Conclusion. Preschoolers with mental retardation experience difficulties in grammatically designing speech utterances. The data obtained in the experiment show that speech difficulties are due to the delayed development of inflection and word-formation operations and the qualitative peculiarity of the manifestation of the defect. Studying grammatical operations in young children with mental retardation and the specifics of the manifestation of this disorder in written speech is a promising further direction.

Main Directions of Modern Transformation and Modernization of Russian Society, Ways to Overcome the Systemic Crisis in the Context of Technological Revolution: Alternative Strategic Development Models

At the present stage of Russia’s development in the prevailing historical conditions, a period of deep conceptual transformations has begun, measures are being taken to free it from its continued dependence on crisis global processes, from the globalism policy of developed imperialist countries. Today, after three periods of permanent systemic crisis, through which Russia went from the early 80s to the early 2000s, the country is emerging from this dependence, returning to its mental ideological and cultural foundations, to state-political, economic, scientific, technical and technological sovereignty, to the status of a world power, renewed civilization, towards the consolidation of society. This period is characterized as a period of transformation and new modernization of society. The article, based on new approaches and methods of a comprehensive historical, sociological and formational-civilizational analysis of the problems of society’s development, examines the issues of the new modernization of society that has begun, analyzes and evaluates the persistent manifestations of the systemic crisis, issues of implementing an innovative anti-crisis management strategy, strategic planning and strategic management. Possible forecasts and alternatives for social development are considered. These processes require a clear, meaningful choice and implementation of a long-term strategy for the dynamic, innovative development of knowledge-intensive sectors of the economy, labor productivity growth, production recovery based on new reindustrialization and the industrial revolution, the development and implementation of achievements of the fifth technological order, scientific, technical, technological, information revolution, the formation of new scientific, human resources, including in the areas of governance of society and the state, ensuring comprehensive security and sovereignty of society and the state, regulatory support for these processes. This implies the choice of a new socio-economic, formational and civilizational model for further progressive development, taking into account positive domestic and foreign experience, and the corresponding ideological basis for these transformations. All this is becoming an urgent issue for the entire spectrum of social and social sciences, for educational practices.

Leasing impact on transformation processes in the Russian agricultural sector under sanctions pressure

At the present stage, the issues of sustainable development of the Russian agricultural sector and strengthening its technical potential in the context of sanctions pressure are becoming especially relevant. One of the most useful tools in solving these problems is leasing. The purpose of this article is to analyse promising directions for the development of leasing as the effective form of technical support for agricultural production in the current conditions. The study has used general scientific methods (analysis and synthesis, economic and statistical, graphical, etc.). The main trends now emerging in the domestic market are highlighted from the point of view of manufacturers of agricultural machinery and goods. With consideration to the current situation in the domestic market, decreased prices for agricultural products and rapid growth of prices for agricultural machinery, the most promising areas for the use of leasing mechanisms have been proposed to help accelerate technical re-equipment in the agricultural sector: application of leasing solutions in the agricultural industry that have already been tested and proven successful in other market segments (for example, subscription, agrocarsharing, etc.); combination of leasing mechanisms with trade-in programmes which make it possible to comprehensively solve issues not only of updating machinery and tractor fleet, but also of recycling worn-out agricultural equipment; use of leasing mechanisms for the development of machine and technological complexes as a modern form of joint use of machinery in agriculture which allows accelerating technical equipment of the industry. The proposed directions can help ensure systemic development and quicken the technical re-equipment of the Russian agricultural sector.

Effects of exosomes and inflammatory response on tumor: a bibliometrics study and visualization analysis via CiteSpace and VOSviewer

BackgroundTumor is a new organism formed by abnormal hyperplasia of local tissue cells under the action of various tumorigenic factors. Inflammation plays a decisive role in inducing tumorigenesis, promoting tumor development, invasion and migration. More and more evidence indicate that exosomes are involved in regulating the formation of tumor microenvironment in the process of proinflammatory carcinogenesis, leading to the stimulation of anti-tumor immune response or systemic immunosuppression, and exosomes play a crucial role in the development of tumor.MethodsThe articles on tumor-derived exosomes and inflammatory responses from January 2005 to January 2024 were collected through Web of Science (WOS), and the inclusion criteria were “Article”, “Review Article” and “Early Access”. Articles obtained after excluding “Book Chapters”, “Editorial Material”, “Proceeding Paper”, “Meeting ” and “Retracted Publication”. Bibliometrics and visualization analysis were carried out on the obtained articles using CiteSpace6.2.R6 and VOSviewer1.6.20.ResultsTotal of 703 articles were included. The number of published documents showed a fluctuating growth trend year by year. A total of 61 countries have participated in the research on the effects of exosomes and inflammatory responses on tumors, among which China and the United States have the largest influence in this field. The obtained articles have been published in 60 journals around the world, among which PLOS ONE and NAT REV IMMUNOL are the journals with the most published articles and the highest co-citations respectively. The article from French author THERY C was cited the most (202 times). As a major researcher on the basic function of exosomes, THERY C established the gold standard for extraction, separation and identification of exosomes, and found that exosomes promote tumor metastasis through direct regulation of miRNA. Her research has had a huge impact on the field. Keyword co-occurrence analysis indicate that extracellular vesicles, inflammation, cancer, miRNAs, mesenchymal stem cells, drug delivery, gastric cancer and circulating endothelial microparticles are the research hotspot at present stage. The main keywords of the cluster analysis show that extracellular vesicles, human papilloma virus, myeloid cells, tumor macro-environment are the current research hotspots and frontier. The research hotspots have developed over time from the time chart of keywords and clustering, especially after 2016, exosomes have established extensive links with drug delivery, cancer treatment, inflammatory response and other fields. Tumor-derived exosomes stimulate receptor cells to secrete pro-inflammatory cytokines and growth factors, enabling immune and inflammatory cells to perceive the intracellular environment of cancer cells even when cancer cells do not express any tumor-specific antigens. For example, in anoxic environment, cancer cells can secrete exosomes containing pro-inflammatory factors to promote the invasion and metastasis of cancer cells. In the complex tumor microenvironment, both tumor cells and various stromal cells will secrete specific exosomes, and promote the development of tumors through various ways, so that tumor cells have drug resistance, and bring adverse effects on the clinical treatment of tumor patients. MicroRNAs and long noncoding RNA as hot keywords play important roles in regulating and mediating tumor development, and their specificity makes them important biomarkers for cancer prediction and diagnosis. Highlighting word analysis shows that microRNAs secreted by leukemia patients can effectively promote the proliferation of malignant cells and the development of cardiovascular diseases. At the same time, exosomes can induce the secretion of some microRNAs in patients, leading to cardiac repair and regeneration. Therefore, the detection and screening of microRNAs plays a crucial role in predicting the incidence of cardiovascular diseases in patients.ConclusionExosomes have attracted increasing attention due to their significant heterogeneity and ability to regulate the tumor immune microenvironment. However, tumor cell-derived exosomes accelerate tumor progression by enhancing immunosuppression and inflammation, increasing oxidative stress, and promoting angiogenesis, which may lead to poor prognosis.Graphical abstract

Characteristics of Moderate Da'wah in The View of Abdul Mun'im

The purpose of this study is to identify the characteristics of moderate Islamic da'wah in the view of H. Abdul Mun'im. This qualitative study uses an analytical-narrative approach. The values of wasathiyah Islamic teachings become the theory of analysis of the subject matter of the study. Research data were obtained through observation techniques on Youtbue content about Ustadz H. Abdul Mun'im's da'wah entitled "Dakwah Wasathiyah" on the Tawaf TV Channel. Meanwhile, data analysis goes through the stages of data reduction, presentation, and verification. This study concludes that there are dimensions of wasathiyah Islamic values in the perspective of H. Abdul Mun'im in describing moderate Islamic preaching, including the value of tasamuth in preaching that respects differences in dialogue between religions and mass organizations. Then, the value of tawazun in da'wah that prohibits issues of extremism and gender discrimination. Furthermore, the value of tawasuth in da'wah that prohibits conflicts between mazhab differences and terrorism. The findings of this study confirm that the mainstreaming of moderate Islamic da'wah in Indonesia can be internalised through digital da'wah activities. The limitations of this study have not identified the concrete form of H Abdul Mun'im's moderate Islamic da'wah in the community.

Searching for scientific and practical solutions focuses on analyzing current trends, opportunities, and challenges associated with implementing innovative solutions in agribusiness. It is highlighted that global trends in the development of critical economic factors (labor, capital, technology) create both new opportunities and threats for the development of companies. It is substantiated that the reduction of working-age people threatens the development of rural areas and the provision of agricultural enterprises with labor. It is proved that most of the labor processes carried out in agricultural companies today require manual skills. The current trends in labor shortages in agricultural companies are highlighted, which is compensated for by the growth of labor productivity, but its level, due to the relatively low provision of enterprises with material and technical resources, still does not allow organizing agricultural business at the level corresponding to the leading countries. During the study, the methodology of theoretical generalization, as well as analysis and synthesis, was used, which allowed us to deepen and structure the understanding of the transformative potential of innovative technologies in agriculture at the present stage of development of innovative technologies. It has been found that innovative technologies play a crucial role in increasing the efficiency, competitiveness, and sustainability of agribusiness by optimizing production, and improving the transparency and traceability of operations and processes. Flat-top plasma operational space of the STEP power plant

Abstract STEP is a spherical tokamak prototype power plant that is being designed to demonstrate net electric power. The design phase involves the exploitation of plasma models to optimise fusion performance subject to satisfying various physics and engineering constraints. A modelling workflow, including integrated core plasma modelling, MHD stability analysis, SOL and pedestal modelling, coil set and free boundary equilibrium solvers, and whole plant design, has been developed to specify the design parameters and to develop viable scenarios. The integrated core plasma model JETTO is used to develop individual flat-top operating points that satisfy imposed criteria for fusion power performance within operational constraints. Key plasma parameters such as normalised beta, Greenwald density fraction, auxiliary power and radiated power have been scanned to scope the operational space and to derive a collection of candidate non-inductive flat-top points. The assumed auxiliary heating and current drive is either from electron cyclotron systems only or a combination of electron cyclotron and electron Bernstein waves. At present stages of transport modelling, there is a large uncertainty in overall confinement for relevant parameter regimes. For each of the two auxiliary heating and current drive systems scenarios, two candidate flat-top points have been developed based on different confinement assumptions, totalling to four operating points. A lower confinement assumption generally suggests operating points in high-density, high auxiliary power regimes, whereas higher confinement would allow access to a broader parameter regime in density and power while maintaining target fusion power performance.

PRINCIPLES OF SOCIAL MANAGEMENT AS A FACTOR IN MINIMIZING POLITICAL RISKS IN DECISION MAKING

The study draws conclusions from the analysis of the political risks of state power at the present stage of Russia’s development. The socio-political principles of management are analyzed, which are connected and interact with a group of organizational and technological mechanisms of political decision-making, which are integral elements of their common system of public administration. Organizational and technological mechanisms reflect the patterns of social management that are associated with the implementation of its organizational and technical functions and the minimization of political risks. They play an important role in creating the basis for the formation of managerial relations, determining essentially the distribution of power, the degree of centralization of the power of managers and the degree of real independence of the governed. Violation of these principles leads to political risks: unjustified fragmentation of the functions of management bodies, violation of the balance of rights and responsibilities of managers, increased demands on the managed and the unreasonableness of decisions. The peculiarities of personnel policy after the presidential elections of Russia are revealed. Proposals are made on mechanisms of social management based on the principles of social justice, as well as the use of existing potential based on the principles of decision-making and its justification in the context of political risks. The connection between the general picture of the world existing in culture and the way of attitude to the category of power in the public consciousness is substantiated.

Historical aspects of the creation and development of university clinics (literature review)

The study is relevant due to the special role of university clinics in maintaining and strengthening public health, since these medical institutions provide training future doctors, conduct clinical and scientific activities, develop and test advanced treatment methods. A systematic review of the literature sources (original research and reviews) selected using bibliographic and semantic search in the databases MedLine, The Cochrane Library, Global Health, Cyb,erLeninka, RSCI, devoted to the historical aspects of the creation and development of university clinics. The authors studied the specifics of the activities of university clinics, searched for solutions to the problem of improving the efficiency and quality of both medical and educational processes of their life. Despite the fact that university clinics originated in different ways, the main models of their activities are highlighted in the form of a single educational, scientific, and medical complex, which allows training of medical personnel at a qualitatively different level, providing practice-oriented training, as well as the possibility of developing scientific achievements of medical science. The literature data provided by the authors indicate university clinics today to be centers of advanced practical and scientific experience in providing medical care of the population. The study of historical patterns of development of university clinics makes it possible to use the most successful experience of their management in the structure of an educational institution and develop requirements that a university clinic must meet at the present stage to ensure strategic sectoral tasks of the state health system both within the framework of training medical personnel and in ensuring the process of providing medical care of the population in the Russian Federation.

This research aims to determine the extent of the relevance aspects of Incubator program towards formation of competencies and professional experience for students of Dian Nuswantoro University Semarang before they graduate from college and go into the real world of work. The method used in this research is descriptive. The data in this research were obtained by field studies through interviews, observation and documentation. The process of data analysis uses descriptive-qualitative analysis methods with the stages of data collection, data reduction, data presentation, decision making or verification. The results of this study, there is a relationship or relevance between TVKU Incubator programs with the formation of competencies and professional experience of students, proven by the existence of alumni who succeeded to work after joining the Incubator program.

Clinical manifestations of scarlet fever in children in the megalopolis at the present stage.

BACKGROUND: according to the literature, an increase in severe forms of scarlet fever among children was noted in a number of foreign countries in 2022-2023. AIMS: to identify the current clinical features of scarlet fever in children in a large metropolis of Russia. MATERIALS AND METHODS: an observational descriptive retrospective continuous study of the clinical features of the course of scarlet fever in children was conducted according to 295 case histories of patients diagnosed with Scarlet fever from 2020 to 2022 on the basis of the GBUZ "DGKB named after Z.A. The statistical processing of the material uses traditional methods for calculating extensive and intensive indicators (p1 and p2), errors of extensive indicators (m1 and m2) and standard deviations (rmax↔rmip), taking into account the confidence interval (t) and confidence probability (P=95%), a method for comparing indicators using a well-known formula, where at t ≥ 2.0 P ≥ 95% p ≤ 0.05, the definition of standard deviations, taking into account the confidence interval (t) and the confidence probability P = 95%. The comparison of percentages in the analysis of four-field conjugacy tables was performed using the exact Fisher criterion. The differences were considered statistically significant at p 0.05. RESULTS: as a result of the study, it was revealed that the abolition of strict anti-epidemic restrictions on COVID-19 in 2022 led not only to an increase in the incidence of scarlet fever among children from 0 to 18 years of age by 3.6 times in 2022 compared with 2021, but, as a result, to an increase in hospitalization of children in hospitals in Moscow. Thus, the number of hospitalized children with scarlet fever in 2022 increased by 18.0 times compared to 2020. The majority of those hospitalized (83.4%) were children from organized groups, including those aged 3 to 7 years (52.9%) and from 7 to 14 years (39.3%), with an average severity of the disease (93.6%). It was noted that that among the examined children, 53 children (18%) had a history of COVID-19, and among this group of children, one in five children had severe scarlet fever, while among children who did not have a history of COVID-19, these were significantly (p 0.001) 8.4 times less (respectively, 20.7% versus 2.5%). It was revealed that every 8-9 patients (12.2%) had various complications, mainly of a septic nature and those that arose in the first week of the disease, including otitis media, sinusitis, community-acquired pneumonia, parathonsillar and lateral pharyngeal abscesses and toxic shock syndrome. In the second week and later, complications of an infectious and allergic nature were reported, including tubulointerstitial nephritis, acute renal injury, post-infectious cardiomyopathy, acute post-streptococcal glomerulonephritis, toxic allergic vasculitis, post-streptococcal arthritis. Beta-lactam antibiotics remain the etiotropic drugs of choice. Antibiotics of the cephalosporin 53.6% and penicillin 46.4% series were used to treat scarlet fever. CONCLUSIONS: in modern conditions of the megalopolis, scarlet fever occurs with a typical clinical picture. The majority of those hospitalized were children of preschool and school age, only 12.2% of children were diagnosed with various complications of scarlet fever and the vast majority of children (93.6%) had a moderate severity of scarlet fever. It was noted that against the background of an increase in the number of hospitalized patients, the number of patients with severe scarlet fever decreases, while the incidence of complications in patients does not change (12.3% - 12.8%). It was shown that among patients who had suffered from COVID-19, scarlet fever was 8.3 times more likely to occur in severe form compared with children who had no history of COVID-19 disease (respectively, 20.7% vs. 2.5, p 0.0001).
