Oral lichen planus and systemic diseases: casual association or causal?

Lichen planus (LP) is a chronic inflammatory disease, mediated by T cells, with a multifactorial etiology, being associated with various diseases and systemic conditions. The objectives of this study are to determine, through the analysis of the literature, the casuistic or causal relationship in the concomitant presentation of Oral Lichen Planus (OLP) and other systemic diseases, and to analyze a series of cases treated at the University Hospital of Brasília (HUB) between 2015 and 2020. Methodology: This study was developed through a review of the literature published between 2016 and 2022, relating systemic diseases with OLP and the analysis of series of OLP cases treated at the HUB. Results: The review showed evidence of the relationship between OLP and Thyroid Disease (TD), Psychiatric Disease (PD), HPV virus and oxidative stress. Furthermore, it was observed that alterations in the oral bacterial microbiota are associated with the progression of OLP. Analysis of the case series identified a relationship between TD and PD with OLP. Conclusion: It is observed that there is strong evidence of the relationship between OLP and DT, DP, HPV virus, oxidative stress and the presence of periodontal bacteria in the oral cavity.

ATYPICAL MANIFESTATION OF ORAL LICHEN PLANUS

White plaques on oral mucosa associated with patient habits often receive clinical diagnosis of leukoplakia. A 55-year-old woman, ex-smoker and ex-alcoholic, sought dental care for oral rehabilitation. On intraoral examination, an asymptomatic white plaque in the tongue margin measuring approximately 2.5 × 2.0 cm of indeterminate time was observed. The diagnostic hypothesis was leukoplakia. Incisional biopsy was performed, and microscopic analysis revealed hyperplastic hyperparakeratinized stratified squamous epithelium, sawtooth-shaped rete ridges, mitotic figures, Civatte bodies, and an underlying lymphocytic inflammatory infiltrate in the band and juxta-epithelial invading and disorganizing the basal layer of the epithelium, cleft at the epithelial-connective interface, with final diagnosis of lichen planus. This report shows an atypical and rare presentation of oral lichen planus and shows that the diagnosis of white lesions becomes challenging when there are overlapping clinical features. Assessment of the Role of Vitamin D in the Treatment of Oral Lichen Planus

Lichen planus is an autoimmune disease that can affect the skin and mucous membranes, including the oral mucosa. Vitamin D plays a very important role in the bone health along with boosting immunity. This study was carried out to assess the role of vitamin D in the treatment of oral lichen planus. A sample size of 450 was taken which included patients showing clinical presentation of oral lichen planus. The patients were equally divided into three different groups based on their vitamin D levels. The data were analyzed using SPSS version 26.0. The patients who were given the vitamin D supplementation showed the maximum improvement. The comparison of the data were found to be statistically significant. It was concluded that vitamin D played an important role in the treatment of oral lichen planus. The role of vitamin D in lichen planus needs to be explored. This study may be useful in conducting further study to assess the role in vitamin D supplementation in the treatment of oral lichen planus.

An unusual presentation of oral lichen planus with desquamative gingivitis

Lichen planus (LP) is a chronic, autoimmune, mucocutaneous disease affecting the skin and mucosa. Oral LP (OLP) usually presents as a bilateral symmetrical lesion that affects approximately 1%–2% of the population. OLP exhibits a wide variety of clinical presentations. This case report deals with a 52-year-old female patient with OLP lesion isolated to the left buccal mucosa and desquamative gingivitis in relation to marginal gingiva of maxillary left premolars with white lacy striae extending from the left- to right-side central incisor region and a brownish-black-pigmented papule to the left angle of lip. Incisional biopsy was performed which revealed typical features of OLP. The patient was advised topical corticosteroids and antioxidants. She was subjected to oral prophylaxis to eliminate local irritating factors and was encouraged to follow meticulous oral hygiene measures to maintain periodontal health following which there was a resolution of the lesion and the symptoms.

Oral lichenoid lesions of the upper lip and gingiva: What we know so far.

The clinical presentation of oral lichen planus and oral lichenoid lesions is diverse. A special category of patients presents with lichenoid lesions affecting only the mucosa of their upper lip and the anterior upper gingiva. This is a concise review summarizing the specific characteristics of these patients.

Expression of heat shock protein (HSP70) in oral lichen planus and non-dysplastic oral leucoplakia.

The purpose of this study was to investigate the expression of heat shock protein (HSP70) in oral non-dysplastic leucoplakia and in relation to the clinical and pathological features of oral lichen planus. The expression of HSP70 was assessed in the epithelial compartment of normal mucosa (n = 5), oral lichen planus (n = 28) and benign leucoplakia (n = 11) using an immunohistochemical method. The immunostaining intensity distribution (IID) index was used to quantify the positivity of the staining. There was no association between HSP70 overexpression and clinical presentation of oral lichen planus. Oral lichen planus patients showed no statistically significant differences in the depth of the inflammatory infiltrate when expression of HSP70 was considered (X(i)- X(j) = 42.30; 95% confidence interval (95% CI) = -120.87-205.48). No statistically significant differences were identified in terms of HSP70 expression between oral lichen planus and normal buccal mucosal specimens (X(i)- X(j) = 4.07; 95% CI = -0.53-8.67). The IID index score for HSP70 expression in leucoplakia specimens was significantly higher than the one of the oral lichen planus group (X(i) - X(j)= 5.11; 95% CI = 1.73-8.48). It is concluded that there are no statistically significant differences in HSP70 expression between oral lichen planus and normal buccal mucosal specimens, suggesting that HSP70 does not play an obvious part in the pathogenesis of oral lichen planus. The expression of HSP70 was significantly higher in oral leucoplakia than in oral lichen planus, possibly because of differences in cellular activity or cell proliferation.

Evaluation of clinical types of cutaneous lichen planus in anti-hepatitis C virus seronegative and seropositive Nigerian patients.

The presentation of oral lichen planus in anti-hepatitis C virus (HCV) seropositive and seronegative patients was previously evaluated, and the keratotic form of oral lichen planus was found to be more prevalent in anti-HCV seropositive patients. This study evaluated the presentation of cutaneous lichen planus in anti-HCV seropositive and seronegative Nigerians. Fifty-seven Nigerians with cutaneous lichen planus were carefully examined to determine the form of lichen planus present. All were screened for the presence of anti-HCV by second-generation enzyme-linked immunosorbent assay (ELISA) and grouped as anti-HCV seropositive or anti-HCV seronegative patients. Nine patients were anti-HCV positive. Seven of these seropositive patients had hypertrophic lichen planus. Hypertrophic lichen planus in Nigerians is more prevalent with HCV infection.
