AN EVALUATION OF PRESACRAL NEURECTOMY IN THE TREATMENT OF DYSMENORRHEA

Abstract The results of 255 cases of presacral neurectomy for dysmenorrhea of both the essential and the acquired types are reviewed. A comparative study is made between the 136 cases performed from 1931 to 1940, and the 119 cases from 1941 to 1945. Complete relief from what is commonly termed essential dysmenorrhea (cramps) was obtained in 78 per cent of cases. There was some degree of improvement of symptoms in a further 11 per cent. Complete relief from acquired dysmenorrhea occurred in only 33.3 per cent of cases. Total failure of the procedure was recorded in 11 per cent of the essential dysmenorrhea group, whereas this poor result obtained in 41.7 per cent of the acquired dysmenorrhea group. The offending pathologic conditions in the acquired cases are reviewed. It is pointed out that pelvic inflammation and endometriosis made up about 60 per cent of this group; 21 per cent of cases in these two categories came to total hysterectomy nine months to four years after presacral neurectomy was performed. We wish to emphasize that presacral neurectomy is still a court of last resort in the treatment of dysmenorrhea, that the operation carries with it the risks of laparotomy, and that good medical management should be attempted in each individual case before resorting to surgery unless other indications for laparotomy exist.

An evaluation of presacral neurectomy in the treatment of dysmenorrhea

STERILITY OF THE MALE AFTER SYMPATHECTOMIES

The surgical literature contains many references to the loss of the power of ejaculation from the seminal vesicles and the prostate gland when the first lumbar sympathetic ganglion is removed.¹The preganglionic motor fibers leave the spinal column through the upper lumbar white rami and join the presacral nerve through the lumbar sympathetic chain and the lateral roots of the superior hypogastric plexus. Presacral neurectomy destroys the postganglionic neuron, and, while libido and orgasm are unimpaired, the power of ejaculation is definitely lost.²With the wider application of high lumbar sympathectomy and section of the splanchnic nerves in our clinic for selected cases of Raynaud's disease, thrombo-angiitis obliterans, reflex dystrophy, hypertension and megacolon, the question of male sterility has frequently arisen. The clinical significance of endometriosis

The various hypotheses regarding the etiology of endometriosis have been briefly outlined. From the manner of the distribution of the implants, the hypothesis that endometrial tissue from the uterus is carried by lymph channels to points outside the uterus presents the least objections to its explanation. The hypothesis is extremely simple, logical and entirely possible.Pelvic pain, quite definitely related to menstruation, is the principal reason the patient seeks relief. There is usually a ten-year history from the onset of the disease and the symptoms have been progressive. There are most likely two types of pain, visceral or sympathetic and somatic or spinal sensory. The latter becomes evident when the peritoneum of the cul-de-sac, lateral pelvic wall, and round ligaments is involved.Surgical treatment is either radical or conservative, depending on the extent and involvement of the lesions. If conservatice treatment is performed it is necessary to carry out such procedures as will relieve pain as well as conserve the menstrual or reproductive functions. The pain may be relieved or alleviated by complete excision of the lesions from the myometrium plus a presacral neurectomy when the lesions are limited to the uterus. Other heterotopic lesions may be attacked by complete excision where possible by the surgical loop diathermy or partial resection when lesions are located in the sigmoid or the rectovaginal septum.

Presacral neurectomy in treatment of certain pelvic, bladder and bowel conditions

Eight patients with dysmenorrhea and seven with bladder and bowel dysfunction have received benefit from presacral neurectomy. The operation removes afferent pain fibers and the inhibitory elements which prevent emptying of the bladder and bowel. Extreme caution in the selection of cases must be exerted lest a useful therapeutic agent be thrown into disrepute by untimely and ill-advised operations.
