Round Window Accessibility Prediction in Cochlear Implant Surgery.

Clear identification of the round window (RW) through the facial recess is a key surgical step for successful cochlear implantation (CI) surgery, which may be very challenging in some cases. Objective is to predict round window (RW) accessibility during CI surgery using high-resolution computed tomography (HRCT). We retrospectively reviewed preoperative HRCT scans of 142 patients who underwent CI surgery via the standard posterior tympanotomy approach at our ENT Head and Neck Surgery department. Surgical accessibility of the RW was assessed according to 2 methods, similar to the ones introduced by Mandour et al and Elzayat et al. Pre-operative imaging findings were then compared to the actual surgical accessibility of the RW by reviewing surgical notes and video recordings. Difficult surgical access to the RW was correctly predicted in our series by Mandour's method in 81.8% of the cases, with a sensitivity and specificity of 56.3% and 96.4%, respectively, and by Elzayat's method in 72.2% of the cases, with a sensitivity and specificity of 50% and 94.5%, respectively. Combining both methods showed an increase in sensitivity levels (Se=71.9%). When the 2 methods both predicted difficult RW access, there was a strong probability that drilling a cochleostomy would be necessary for safe electrode insertion along the scala tympani of the basal turn of the cochlea (P < .001). These 2 methods are both simple and reliable tools that can help the surgeon anticipate difficult surgical access and prepare for the potential use of alternative techniques.

A Comparative Study between Preoperative Computed Tomography and Intraoperative Ossicular Chain Status in Patients of Chronic Suppurative Otitis Media at RIMS, Ranchi

ABSTRACT Background: Chronic suppurative otitis media (CSOM) is a persistent middle ear infection often leading to conductive hearing loss due to ossicular chain damage. Accurate preoperative assessment using high-resolution computed tomography (HRCT) is crucial for surgical planning. This study compares preoperative HRCT findings with intraoperative ossicular chain status in CSOM patients. Aims and Objectives: To evaluate the correlation between preoperative computed tomography scans and intraoperative ossicular chain status in CSOM patients. Methodology: The study included 60 CSOM patients aged 12 to 60 years attending the ENT department at Rajendra Institute of Medical Sciences, Ranchi. Preoperative HRCT was performed, and intraoperative findings were compared. Data analysis was conducted using SPSS version 17.0. Results: Ossicular erosion was observed in 61.7% of cases, predominantly affecting the incus. HRCT showed high sensitivity and specificity in detecting ossicular chain damage, aligning with intraoperative findings. Conclusion: HRCT provides valuable preoperative insights but should complement intraoperative assessment. Combining both approaches ensures accurate diagnosis and effective management of CSOM.

Evaluation High-Resolution Computed Tomography (HRCT) Of Temporal Bone In Cholesteatoma Of Middle Ear In Comparison With Histopathological Findings: A Prospective Study

Background: Cholesteatoma is relatively common and potentially dangerous disease of the ear. It is characterised by local destruction and osseous erosion leading to complications. Pre-operative assessment of middle ear cholesteatoma is a must for assessing the disease's location, extent, and complication. High resolution computed tomography (HRCT) is the imaging modality of choice for evaluation of middle ear structures and pathology. Therefore, in the present study we aimed to evaluate HRCT of temporal bone in cholesteatoma of middle ear cleft in comparison with histopathological results. Materials and Methods: This is a prospective study carried out on total of 30 patients who were undergoing HRCT of temporal bone for middle ear cleft pathology and found to have imaging evidence of chronic otomastoiditis on HRCT, subsequently undergoing surgery and histopathological validation. Results: The mean age of study subjects was found to be 23.97 ± 11.30 years. Majority of the study subjects were belonged to age group of 11-40 years (40%) with female predominance (53.3%) as compared to male (46.7%). Majority of the patients i.e., 60% showed signs of loss of aeration and sclerosis of mastoid air cells followed by soft tissue density lesions (43.3%), bony erosions (26.7%), and bony expansions (6.7%). Surgical findings and histopathological reports were correlated with pre-operative HRCT scans analyses with 95.88% accuracy. Conclusion: Compared to histopathological findings, HRCT offers a diagnostic capacity that is remarkably sensitive and specific in identifying cholesteatoma and the condition of the bone structures in the temporal bone

Selection of inner ear fenestration strategy and surgical effect of patients with oval window atresia accompanied by facial nerve aberration

Objective: To summarize the clinical features and postoperative efficacy of patients with oval window atresia accompanied by facial nerve aberration. Methods: The clinical data of patients with congenital middle ear malformation with facial nerve aberration admitted to our hospital from January 2015 to March 2023 were retrospectively analyzed. There were 97 cases (133 ears) in total. Among them, 39 patients (44 ears) had complete follow-up data, including 27 male patients and 12 females, aged 7-48 years old, with an average age of 17.8 years old. Of these, 14 cases (16 ears) were patients combined with facial nerve aberration, and 25 cases (28 ears) were without facial nerve aberration. The results of imaging examination, pure-tone audiometry, selection of surgical strategy, intraoperative findings and postoperative hearing improvement were summarized and analyzed. The malformations of malleus, incus, stapes, oval window and facial nerve were recorded. Prism 9 software was used to statistically analyze the mean bone conductance and air-bone gap of patients before and after surgery. Results: All the 14 patients (16 ears) with middle ear malformation accompanied by facial nerve aberration and oval window atresia showed poor hearing and no facial palsy since childhood. High resolution CT (HRCT) examination of temporal bone, pure tone audiometry and Gelle test were performed before surgery. The malformations of malleus, incus, stapes, oval window and facial nerve were recorded. Preoperative high-resolution CT (HRCT) examination of temporal bone found 12 ears with 4 or more deformities, accounting for 75.00%, in the group of patients with facial nerve malformation. The preoperative average bone conductive threshold was (15.3±10.4) dB and the average air-bone gap was (46.3±10.6) dB in pure-tone audiometry (0.5, 1, 2, 4kHz). According to the different degrees of facial nerve and ossicle malformation, we performed three different hearing reconstruction strategies for the 14 patients (16 ears) with facial nerve aberration and oval window atresia, including 7 ears of incus bypass artificial stape implantation, 7 ears of Malleostapedotomy (MS) and 2 ears of Malleus-cochlear-prothesis (MCP). After 3 months to 18 months of follow-up, all patients showed no facial paralysis. The postoperative mean bone conductive threshold was (15.7±7.9) dB and air-bone gap was (19.8±8.5) dB. There were significant differences in mean air-bone gap before and after operation (t=7.766, P<0.05), and there was no significant difference between the mean bone conductive threshold before and after surgery (t=0.225, P=0.824). There was no significant difference of mean reduction of air-bone gap between patients with and without facial nerve aberration (t=1.412, P=0.165). There was no significant difference between the three hearing reconstruction strategies. There was no significant displacement of the Piston examined by U-HRCT. Conclusion: For patients of middle ear malformation whose facial nerve cover the oval window partially, incus bypass artificial stape implantation or Malleostapedotomy (MS) can be selected according to the specific condition of auditory ossis malformation, and for patients whose facial nerve completely covers the oval window area, Malleus-cochlear-prothesis (MCP) can be selected. Three types of stapes surgery are safe and reliable for patients with oval window atresia accompanied by facial nerve aberration. There was no significant difference in efficacy between them. Preoperative HRCT assessment of middle ear malformation is effective. There is no significant difference of surgical effect with or without facial nerve aberration. The U-HRCT can be used to evaluate the middle ear malformation before surgery and the Piston implantation status after surgery. Due to the risks of surgery, those who do not want to undergo surgery can choose artificial hearing AIDS, such as hearing aid, vibrating soundbridge, bone bridge or bone-anchored hearing aid.

The Coexistence of Carotico-Clinoid Foramen and Interclinoidal Osseous Bridge: An Anatomo-Radiological Study With Surgical Implications.

The coexistence of complete carotico-clinoid bridge (CCB), an ossification between the anterior (ACP) and the middle clinoid (MCP), and an interclinoidal osseous bridge (ICB), between the ACP and the posterior clinoid (PCP), represents an uncommonly reported anatomic variant. If not adequately recognized, osseous bridges may complicate open or endoscopic surgery, along with the pneumatization of the ACP, especially when performing anterior or middle clinoidectomies. According to Preferred Reporting Items for Systematic Reviews and Meta-Analyses for Scoping Reviews guidelines, a systematic scoping review was conducted up to June 5, 2023. PubMed, Scopus, Web of Science databases, and additional citations were searched. Two hundred high-resolution noncontrast computed tomography (CT) scans (400 sides) and 41 dry skulls (82 sides) were analyzed to identify the different morphology of sellar bridges, focusing on the coexistence of complete CCF and ICB. Two embalmed latex-injected heads with coexisting CCF and ICB were dissected step-by-step to show the anatomic relationship with the surrounding structures from an endoscopic and microscopic perspective. A total of 19 articles were included. The review identified a complete CCF and ICB rate ranging from 4.92% to 6.3%. The analysis of 200 CT scans revealed a rate of coexistence in 4% of the cases, all encountered in White women. Two different types of interclinoid bridges were identified based on the degree of bone mineralization. Both endoscopic and macroscopic step-by-step dissections highlighted variability in morphology and consistency of the sellar bridges and the close relationship with the cavernous sinus neurovascular structures. The coexistence of CCF and ICB is an anatomic variation found in 4% of cases. Preoperative knowledge of the degree of mineralization and its relationship with surrounding structures is essential to performing safe surgery and minimizing cranial nerve and vascular injuries. Preoperative high-resolution CT scans can adequately identify these anatomic variations.

The Value of Radiomics Features Based on HRCT in Predicting whether the Lung Sub-Centimeter Pure Ground Glass Nodule is Benign or Malignant.

In this study, a radiomics model was created based on High-Resolution Computed Tomography (HRCT) images to noninvasively predict whether the sub-centimeter pure Ground Glass Nodule (pGGN) is benign or malignant. A total of 235 patients (251 sub-centimeter pGGNs) who underwent preoperative HRCT scans and had postoperative pathology results were retrospectively evaluated. The nodules were randomized in a 7:3 ratio to the training (n=175) and the validation cohort (n=76). The volume of interest was delineated in the thin-slice lung window, from which 1316 radiomics features were extracted. The Least Absolute Shrinkage and Selection Operator (LASSO) was used to select the radiomics features. Univariate and multivariable logistic regression were used to evaluate the independent risk variables. The performance was assessed by obtaining Receiver Operating Characteristic (ROC) curves for the clinical, radiomics, and combined models, and then the Decision Curve Analysis (DCA) assessed the clinical applicability of each model. Sex, volume, shape, and intensity mean were chosen by univariate analysis to establish the clinical model. Two radiomics features were retained by LASSO regression to build the radiomics model. In the training cohort, the Area Under the Curve (AUC) of the radiomics (AUC=0.844) and combined model (AUC=0.871) was higher than the clinical model (AUC=0.773). In evaluating whether or not the sub-centimeter pGGN is benign, the DCA demonstrated that the radiomics and combined model had a greater overall net benefit than the clinical model. The radiomics model may be useful in predicting the benign and malignant sub-centimeter pGGN before surgery.</p>.

Preoperative Assessment Using CT and MRI Scans of the Temporal Bone to Determine the Degree of Difficulty in Cochlear Implant Surgery.

Background Cochlear implant surgery is a complex procedure influenced by the anatomical structures of the temporal bone. Preoperative imaging using CT and MRI can provide critical insights into the surgical challenges that may be encountered. This study aims to evaluate the role of CT and MRI in preoperative assessment to predict the difficulty of cochlear implant surgery in terms of surgical time. Materials and methods A retrospectiveobservational study was conducted at Saveetha Medical College and Hospital, Chennai, from April 2022 to September 2023. Ninety patients with severe to profound sensorineural hearing loss who underwent cochlear implantation were included. Preoperative high-resolution CT (HRCT) and MRI of the temporal bone were performed to assess various anatomical parameters. Surgical difficulty was evaluated intraoperatively and correlated with preoperative imaging findings. Data were analyzed using IBM SPSS Statistics for Windows, V. 21.0 (IBM Corp., Armonk, NY). Results The mean age of participants was 7.4±10.9 years, with the majority (66.7%) in the 1-5-year age group. Out of 90 participants, 50 were male and 40 were female. HRCT and MRI revealed that 35.6% of participants had hypo-/non-pneumatized mastoids, 3.3% had narrow facial recesses, and 3.3% had high-riding jugular bulbs. Significant correlations were found between surgical time and associated congenital (p=0.006) and acquired (p=0.0001) anomalies of the temporal bone, as well as the total difficulty score (p=0.0001). The mean surgical time was 103.97±25.2 minutes, with a range from 45 to 220 minutes. Conclusion Preoperative HRCT and MRI are valuable tools in predicting the degree of difficulty in cochlear implant surgery. Specific anatomical features identified in imaging studies can significantly influence the surgical approach and duration. These findings underscore the importance of detailed preoperative imaging to enhance surgical planning and outcomes in cochlear implant procedures.

Importance of preoperative HRCT temporal bone in the orientation of the cochlea and its relation to intra-operative difficulties in cochlear implantation – predicting the grades of difficulty

Objective: This study emphasizes the role of preoperative high-resolution computed tomography (HRCT) temporal bone in evaluating the variation in cochlear orientation and proposes a grading system to determine the level of intraoperative difficulties encountered. Methods: Preoperative correlation of middle ear and inner ear structures along with the basal turn angle (BTA) was done to assess the orientation. Patients were divided into three groups depending on BTA and radiology findings. Results: Group A (BTA = 55°-60°) had statistically significant (p < 0.05) correlation between middle ear to inner ear structures on HRCT, whereas group B (BTA >60°) and C (BTA <55°) had variations in the alignment of middle ear structures to their corresponding inner ear structures. Conclusion: The association of BTA to the correlation between middle ear and inner ear structures can determine the orientation of the cochlea. This evaluation makes the surgeon aware of intraoperative challenges and helps in surgical planning.

Preoperative evaluation of visceral pleural invasion in peripheral lung cancer utilizing deep learning technology.

This study aimed to assess the efficiency of artificial intelligence (AI) in the detection of visceral pleural invasion (VPI) of lung cancer using high-resolution computed tomography (HRCT) images, which is challenging for experts because of its significance in T-classification and lymph node metastasis prediction. This retrospective analysis was conducted on preoperative HRCT images of 472 patients with stage I non-small cell lung cancer (NSCLC), focusing on lesions adjacent to the pleura to predict VPI. YOLOv4.0 was utilized for tumor localization, and EfficientNetv2 was applied for VPI prediction with HRCT images meticulously annotated for AI model training and validation. Of the 472 lung cancer cases (500 CT images) studied, the AI algorithm successfully identified tumors, with YOLOv4.0 accurately localizing tumors in 98% of the test images. In the EfficientNet v2-M analysis, the receiver operating characteristic curve exhibited an area under the curve of 0.78. It demonstrated powerful diagnostic performance with a sensitivity, specificity, and precision of 76.4% in VPI prediction. AI is a promising tool for improving the diagnostic accuracy of VPI for NSCLC. Furthermore, incorporating AI into the diagnostic workflow is advocated because of its potential to improve the accuracy of preoperative diagnosis and patient outcomes in NSCLC.

Development and Validation of a Nomogram for Axillary Lymph Node Metastasis Risk in Breast Cancer.

Purpose: Preoperative assessment of axillary lymph node (ALN) status is essential for breast cancer treatment planning. This study prospectively analyzed risk factors for ALN metastasis by comparing high-resolution computed tomography (HRCT) imaging with pathology and developed a nomogram to aid in diagnosis. Methods: From April 2023 to May 2024, breast cancer patients confirmed by pathology participated in the study. All had chest HRCT before surgery, and ALN samples were anatomically matched to HRCT imaging and pathology. The least absolute shrinkage and selection operator (LASSO) regression helped refine metastasis features, and a nomogram was constructed using the final selected features determined by multivariate logistic regression. The nomogram's performance was evaluated with concordance index (C-index), calibration plot, and decision curve analysis, with internal validation through bootstrapping. Results: A total of 302 ALN from 98 patients were included in this study. The predictors included in the nomogram encompassed the mean CT value, short diameter, border, and shape of ALN, as well as the Ki-67 status and histological grade of the primary tumor. The model exhibited satisfactory discrimination, with a C-index of 0.869 (95% CI: 0.826-0.912) and an AUC of 0.862 (95% CI, 0.815-0.909). The calibration curve demonstrated a high degree of concordance between the predicted and actual probabilities. The decision curve analysis demonstrated that the nomogram was clinically useful when the threshold for intervention was set at the metastasis possibility range of 1% to 86%. Conclusion: The nomogram combined with preoperative pathology and HRCT imaging have the potential to improve the evaluation of ALN status.

Morphological Predictors of Primary Lung Cancer among Part-Solid Ground-Grass Nodules on High-Resolution CT.

Recent advancements in computed tomography (CT) scanning have improved the detection rates of peripheral pulmonary nodules, including those with ground-glass opacities (GGOs). This study focuses on part-solid pure ground-glass nodules (GGNs) and aims to identify imaging predictors that can reliably differentiate primary lung cancer from nodules with other diagnoses among part-solid GGNs on high-resolution CT (HRCT). A retrospective study was conducted on 609 patients who underwent surgical treatment or observation for lung nodules. Radiological findings from pre-operative HRCT scans were reviewed and several CT imaging features of part-solid GGNs were examined for their positive predictive value to identify primary lung cancer. The proportions of the nodules with a final diagnosis of primary lung cancer were significantly higher in part-solid GGNs (91.9%) compared with solid nodules (70.3%) or pure GGNs (66.7%). Among CT imaging features of part-solid GGNs that were evaluated, consolidation-to-tumor ratio (CTR) < 0.5 (98.1%), pleural indentation (96.4%), and clear tumor border (96.7%) had high positive predictive value to identify primary lung cancer. When two imaging features were combined, the combination of CTR < 0.5 and a clear tumor border was identified to have 100% positive predictive values with a sensitivity of 40.8%. Thus we conclude that part-solid GGNs with a CTR < 0.5 accompanied by a clear tumor border evaluated by HRCT are very likely to be primary lung cancers with an acceptable sensitivity. Preoperative diagnostic procedures to obtain a pathological diagnosis may potentially be omitted in patients harboring such part-solid GGNs.

Correlative Study of High-Resolution Computed Tomography Temporal Bone with Intraoperative Findings in Tympanomastoid Surgery

Abstract Introduction: The global incidence of chronic otitis media (COM) is approximately 4.76 per thousand, with 31 million cases causing various complications. Most of these patients require one of the various forms of mastoid surgeries. High-resolution computed tomography (HRCT) has greatly improved the ability to assess both bony architecture and soft-tissue status. Aim: The aim is to assess the correlation of HRCT temporal bone with intraoperative findings in tympanomastoid surgery and to evaluate the sensitivity, specificity, positive predictive value, and negative predictive value of HRCT temporal bone. Materials and Methods: This prospective study was conducted in the Department of Otorhinolaryngology, SDM College of Medical Sciences and Hospital, Dharwad from November 2012–April 2014. Fifty patients of COM were taken up for study who underwent preoperative HRCT temporal bone followed by appropriate tympanomastoid surgery. Results: HRCT temporal bone showed increased sensitivity and specificity in detecting ossicular chain erosion and soft tissue abnormality within the middle ear and mastoid air cells. Facial nerve canal erosion was correctly detected in 62.50%, and overall agreement between surgical findings and the scan was high (90%). It also showed a very high specificity of 95.24%. For semicircular canal erosion and dural plate erosion, sensitivity was 50% each, specificity was 100% and had a very good correlation with surgical findings. Two cases of erosion of the posterior bony wall of the external auditory canal were correctly detected by the HRCT scan. Conclusion: HRCT temporal bone was found to have a high overall correlation with surgical findings and can be a useful method for early diagnosis of complications of COM and preoperative assessment.

High-Resolution Computed Tomography in Middle Ear Cholesteatoma: How Much Do We Need It?

Background and Objectives: The diagnosis of cholesteatoma is usually clinic, and the only efficient treatment is surgical. High-resolution computed tomography (HRCT) is not considered absolutely necessary for the management of an uncomplicated cholesteatoma, but unsuspected situations from a clinical point of view can be discovered using the scans, warning the surgeon. Our objective is to compare HRCT scan information with intraoperative findings in patients with cholesteatoma and analyze the usefulness of a preoperative HRCT scan from a surgical point of view. Materials and Methods: This is a prospective descriptive study conducted in the Department of Otolaryngology, Victor Babes University of Medicine and Pharmacy Timisoara, Romania, from May 2021 to April 2022. It was carried out on 46 patients with a clinical diagnosis of cholesteatoma who were consequently operated on in our department. All patients received full clinical and audiological examinations. In all cases, an HRCT scan was performed preoperatively as a mandatory investigation. Preoperative HRCT scans were analyzed, and their findings were compared to the intraoperative notes. The two sets of observations were analyzed using standard statistical methods. Results: Extensive cholesteatoma was the most common type of disease, involving 46% of the patients, followed by pars flaccida cholesteatoma (35%) and pars tensa cholesteatoma (19%). Eroded scutum was the most frequent lesion involving 70% of the patients, followed by incus erosion (67%). Comparison of the HRCT and intraoperative findings revealed a very good correlation for tegmen tympani erosion, sigmoid plate erosion, scutum and malleus erosion, and a moderate-to-good correlation for lateral semicircular canal erosion, incus and stapes erosion, and fallopian canal erosion. Conclusions: HRCT is a valuable tool in the preoperative assessment of cholesteatoma, helping in making surgical decisions. It can accurately predict the extent of disease and is helpful for detecting unapparent dangerous situations. However, it is not very accurate in detecting fallopian canal and stapes erosion.

Evaluation of Medial Retrotympanum by High-Resolution Computed Tomography and Endoscopy - A Descriptive Study.

The aim of this study was to determine and compare the capability of the High-Resolution Computed Tomography (HRCT) and endoscope in detecting hidden areas of medial retrotympanum in cases with chronic otitis media (COM). Per-operative endoscopic evaluation of structures in medial retrotympanum was done in 74 patients suffering from COM and was compared with pre-operative HRCT of the temporal bone in 50 patients. HRCT revealed type-A Sinus Tympani (ST) in 61.2%, type-B in 34.7%, and type-C in 4.1% of the patients; endoscope could reveal type-A in 71.6%, type-B in 25.7% and type-C in 1.4% of the patients. Ponticulus was discovered in 84.1%, subiculum in 100%, finiculus in 97.3% and sinus subtympanicus (SST) in 100% of the cases using endoscope. HRCT could detect ponticulus in 38.6% and subiculum in 4.0% of the cases. HRCT could not very clearly detect finiculus and SST. HRCT could not differentiate the content of the ST as compared with endoscopy. HRCT is a useful diagnostic tool in predicting the presence and type of ST, but its accuracy in detecting SST, finer bony ridges of ponticulus, subiculum, and finiculus and the contents is poorer than endoscope.

Accuracy of High-Resolution Computed Tomography Compared to High-Definition Ear Endoscopy to Assess Cholesteatoma Extension.

To correlate radiographic evidence of cholesteatoma in the retrotympanum with intraoperative endoscopic findings in cholesteatoma patients and to evaluate the clinical relevance of radiographic evidence of cholesteatoma in the retrotympanum. Case series with chart review. Tertiary referral center. Seventy-six consecutive cases undergoing surgical cholesteatoma removal with preoperative high-resolution computed tomography (HRCT) were enrolled in this study. A retrospective analysis of the medical records was conducted. The extension of cholesteatoma regarding different middle ear subspaces, into the antrum and mastoid were reviewed radiologically in preoperative HRCT and endoscopically from surgical videos. Additionally, facial nerve canal dehiscence, infiltration of the middle cranial fossa, and inner ear involvement were documented. Comparison of radiological and endoscopic cholesteatoma extension revealed statistically highly significant overestimation of radiological cholesteatoma extension for all retrotympanic regions (sinus tympani 61.8% vs19.7%, facial recess 69.7% vs43.4%, subtympanic sinus 59.2% vs7.9%, and posterior sinus 72.4% vs4.0%) and statistically significant overestimation for mesotympanum (82.9% vs56.6%), hypotympanum (39.5% vs9.2%), and protympanum (23.7% vs6.6%). No statistically significant differences were found for epitympanum (98.7% vs90.8%), antrum (64.5% vs52.6%), and mastoid (26.3% vs32.9%). Statistically significant radiological overestimation of facial nerve canal dehiscence (54.0% vs25.0%) and invasion of tegmen tympani (39.5% vs19.7%) is reported. Radiologic cholesteatoma extension in different middle ear subspaces is overestimated compared to the intraoperative extension. The preoperative relevance of radiological retrotympanic extension might be limited in the choice of approach and transcanal endoscopic approach is always recommended first.

Differentiation of geniculate ganglion venous malformation from schwannoma: dynamic T1-weighted imaging provides unique diagnostic value.

To distinguish geniculate ganglion venous malformation (GGVM) from schwannoma (GGS) by using high-resolution CT (HRCT), routine MRI, and dynamic T1-weighted imaging (T1WI) characteristics. Surgically confirmed GGVMs and GGSs between 2016 and 2021 were retrospectively included. Preoperative HRCT, routine MR, and dynamic T1WI were performed on all patients. Clinical data, imaging characteristics including lesion size, involvement of facial nerve (FN), signal intensity, enhancement pattern on dynamic T1WI, and bone destruction on HRCT were evaluated. Logistic regression model was developed to identify independent factors for GGVMs, and the diagnostic performance was accessed by receiving operative curve (ROC) analysis. Histological characteristics were explored for both GGVMs and GGSs. Twenty GGVMs and 23 GGSs with mean age of 31 were included. On dynamic T1WI, 18 GGVMs (18/20) showed "pattern A" enhancement (a progressive filling enhancement), while all 23 GGSs showed "pattern B" enhancement (a gradual whole-lesion enhancement) (p < 0.001). Thirteen GGVMs (13/20) showed the "honeycomb" sign whereas all GGS (23/23) showed extensive bone changes on HRCT (p < 0.001). Lesion size, involvement of FN segment, signal intensity on non-contrast T1WI and T2-weighted imaging (T2WI), and homogeneity on enhanced T1WI were obviously differed between two lesions (p < 0.001, p = 0.002, p < 0.001, p = 0.01, p = 0.02, respectively). Regression model showed the "honeycomb" sign and "pattern A" enhancement were independent risk factors. Histologically, GGVM was characterized by interwoven dilated and tortuous veins, while GGS was characterized by abundant spindle cells with dense arterioles or capillaries. The "honeycomb" sign on HRCT and "pattern A" enhancement on dynamic T1WI are the most promising imaging characteristics for differentiating GGVM from GGS. The characteristic sign and enhancement pattern on HRCT and dynamic T1-weighted imaging allow preoperative differentiation of geniculate ganglion venous malformation and schwannoma feasible, which will improve clinical management and benefit patient prognosis. • The "honeycomb" sign on HRCT is a reliable finding to differentiate GGVM from GGS. • GGVM typically shows "pattern A" enhancement (focal enhancement of the tumor on early dynamic T1WI, followed by progressive contrast filling of the tumor in the delayed phase), while "pattern B" enhancement (gradual heterogeneous or homogeneous enhancement of the whole lesion) is observed in GGS on dynamic T1WI.

Survival impact of fibrotic interstitial lung abnormalities in resected stage IA non-small cell lung cancer.

To assess the association between fibrotic interstitial lung abnormalities (ILAs) and long-term survival in patients with resected Stage IA non-small cell lung cancer (NSCLC). Data of patients who underwent curative resection of pathological Stage IA NSCLC between 2010 and 2015 were retrospectively analysed. ILAs were evaluated using pre-operative high-resolution CT scans. The association between ILAs and cause-specific mortality was assessed via Kaplan-Meier analysis and the log-rank test. Cox proportional hazards regression was performed to determine the risk factors for cause-specific death. Overall, 228 patients were identified (63.27 ± 8.54 years, 133 men [58.3%]). ILAs were detected in 24 patients (10.53%). Fibrotic ILAs were observed in 16 patients (7.02%), and there was a significantly higher cause-specific mortality rate among patients with fibrotic ILAs compared with patients with no ILAs (p < 0.001). Patients with fibrotic ILAs had a significantly higher cause-specific mortality rate than patients without ILAs at 5 post-operative years (survival rate: 61.88% vs 93.03%, p < 0.001). The presence of afibrotic ILA was an independent risk factor for cause-specific death (adjusted hazard ratio = 3.22; 95% confidence interval: 1.10, 9.44; p = 0.033). The presence of afibrotic ILA was a risk factor for cause-specific death in patients with resected Stage IA NSCLC. Radiologists and clinicians should be familiar with the relatively new concept of ILAs and understand the close association between ILA status and long-term survival in resected Stage IA NSCLC. Patients presenting fibrotic ILAs should receive appropriate surveillance and management to optimise prognosis. Fibrotic ILAs are important findings implicated inthe long-term survival of patients with resected Stage IA NSCLC. Specific management is required for this group.

Establishment and Verification of Benign and Malignant Prediction Model of Subcentimeter Pulmonary Ground Glass Nodules Based on HRCT

Pre-operative accuracy of subcentimeter ground glass nodules (SGGNs) is a difficult problem in clinical practice, but there are few clinical studies on the benign and malignant prediction model of SGGNs. The aim of this study was to help identify benign and malignant lesions of SGGNs based on the imaging features of high resolution computed tomography (HRCT) and the general clinical data of patients, and to build a risk prediction model. This study retrospectively analyzed the clinical data of 483 patients with SGGNs who underwent surgical resection and were confirmed by histology from the First Affiliated Hospital of University of Science and Technology of China from August 2020 to December 2021. The patients were divided into the training set (n=338) and the validation set (n=145) according to 7:3 random assignment. According to the postoperative histology, they were divided into adenocarcinoma group and benign lesion group. The independent risk factors and models were analyzed by univariate analysis and multivariate Logistic regression. The receiver operator characteristic (ROC) curve was constructed to evaluate the model differentiation, and the calibration curve was used to evaluate the model consistency. The clinical application value of the decision curve analysis (DCA) evaluation model was drawn, and the validation set data was substituted for external verification. Multivariate Logistic analysis screened out patients' age, vascular sign, lobular sign, nodule volume and mean-CT value as independent risk factors for SGGNs. Based on the results of multivariate analysis, Nomogram prediction model was constructed, and the area under ROC curve was 0.836 (95%CI: 0.794-0.879). The critical value corresponding to the maximum approximate entry index was 0.483. The sensitivity was 76.6%, and the specificity was 80.1%. The positive predictive value was 86.5%, and the negative predictive value was 68.7%. The benign and malignant risk of SGGNs predicted by the calibration curve was highly consistent with the actual occurrence risk after sampling 1,000 times using Bootstrap method. DCA showed that patients showed a positive net benefit when the predictive probability of the predicted model probability was 0.2 to 0.9. Based on preoperative medical history and preoperative HRCT examination indicators, the benign and malignant risk prediction model of SGGNs was established to have good predictive efficacy and clinical application value. The visualization of Nomogram can help to screen out high-risk groups of SGGNs, providing support for clinical decision-making.

Assessment of the ability of the radiological incudo-stapedial angle to predict the stapedotomy technique type: a prospective case-series study.

This study aimed to propose a radiological classification of the incudo-stapedial angle by preoperative high-resolution computed tomography (HRCT) images and to highlight its importance for predicting the use of reversal-steps stapedotomy (RSS) rather than the traditional non-reversal technique. We included 83 candidates for stapedotomy operation. Two physicians measured the radiological incudo-stapedial joint angle in the preoperative HRCT. According to this measurement, the radiological incudo-stapedial joint was classified into three types: obtuse, right, and acute. In addition, this radiological classification was correlated with the intraoperative use of the stapedotomy technique, either reversal or non-reversal. The RSS technique was used in forty-two (97.7%) cases with an obtuse angle and twenty-six (89.7%) with a right angle. At the same time, the traditional non-reversal technique was used in all patients with an acute angle. The three groups differed significantly regarding the method used for stapedotomy (P value < 0.001). Moreover, Spearman's correlation coefficient revealed a significant correlation between the used technique and the radiological type of the incudo-stapedial angle (P value < 0.001). This prospective study proposed a preoperative radiological classification of the incudo-stapedial angle. This classification was significantly correlated with the type of stapedotomy technique. The RSS technique was feasible in most cases with an obtuse and right radiological incudo-stapedial angle. In contrast, the non-reversal method was used in all patients with an acute radiological incudo-stapedial angle. This radiological classification could predict the choice for the stapedotomy technique with an accuracy of 95.18%, a sensitivity of 73.33%, and a specificity of 100%.

Facial skeleton dysmorphology in syndromic craniosynostosis: differences between FGFR2 and no-FGFR2-related syndromes and relationship with skull base and facial sutural patterns.

To assess the role of FGFR2 mutations and sutural synostotic patterns on facial skeleton dysmorphology in children with syndromic craniosynostosis. Preoperative high-resolution CT images in 39 infants with syndromic craniosynostosis were evaluated. Patients were divided into infants with and without FGFR2 mutations; each group was split according to synostotic involvement of minor sutures/synchondroses: isolated or combined involvement of middle (MCF) and posterior cranial fossae (PCF). Quantitative analysis of the midface and mandible measures was performed. Each subgroup was compared with a group of age-matched healthy subjects. Twenty-four patients with FGFR2 related syndromes were clustered in 3 subgroups: MCF + PCF (8 patients, 5.4 ± 1.75months), MCF (8 patients, 3.62 ± 1.68months), and PCF (8 patients, 2.75 ± 0.46months). Fifteen no-FGFR2 patients were clustered in 2 subgroups: MCF + PCF (7 patients, 9.42 ± 0.78months) and PCF (8 patients, 7.37 ± 2.92months). Both FGFR2 and no-FGFR2 groups with involvement of minor sutures coursing in MCF showed more facial sutural synostoses. Children with minor suture/synchondrosis synostosis of MCF (MCF-PCF and MCF subgroups) showed altered position of glenoid fossa and mandibular inclination ([Formula: see text]), but children in the FGFR2 group had also reduced midfacial depth and maxillary length ([Formula: see text]). Children with minor suture/synchondrosis synostosis of PCF (PCF subgroups) had reduced posterior mandibular height, but those children in the FGFR2 group also showed reduced intergonion distance ([Formula: see text]). In children with syndromic craniosynostosis, both skull base and facial suture synostosis affect facial dysmorphology/hypoplasia. FGFR2 mutations may worsen facial hypoplasia both acting on bone development and causing an earlier premature closure of facial sutures.
