IMAGE DIAGNOSIS IN EARLY BREAST CANCERS (WITHIN T 2.0 cm, TIS)

The image diagnosis in early breast cancers in clinically studied.(1) Echography(1) After Hitachi EUB2D 7.5 MHZ being used, the accurate diagnosis rate and false negative rate in invasive breast cancers are 90.9% and 9.1%, in tumors less than 1.0 cm in diameter, and 92.0%, 8.0% in those from 1.0 to 2.0 cm respectively. All tumors more than 0.5 cm in diameter can be represented as tumor echo. (2) The accurate rate in non invasive breast cancers is 69.2%.(2) Mammography(1) Accurate and false negative rates in invasive breast cancers are 47.1% and 52.9%, in tumors less than 1.0 cm in diameter, and 80.7% and 19.3% in those from 1.0 to 2.0 cm respectively. Diagnostic rate in tumors less than 1.0 cm is bad. It is as not so good as 57.1% in non invasive breast cancers. (2) The 52.9% of infiltrating breast cancer tumors less than 1.0 cm and 83.2% of those from 1.0 to 2.0 cm can be represented as tumor shadow. The previous date was bad. Tumor shadow can be visualized in 57.1% of non invasive tumors. It is 64.7% in invasive ductal carcinomas with a predominant intraductal component. (3) The appearance rates of fine calcification (FC) are 23.5% in less than 1.0 cm and 43.2% in from 1.1 to 2.0 cm invasive cancers, while 50.0% in non invasive ones. (4) The cases having no tumor shadow but FC are found in 25.0% of invasive cancers less than 1.0 cm and 26.7% of those in from 1.1 to 2.0 cm. It is 33.3% in non invasive breast ones.

Adenocarcinoma of the Pancreas with a Predominant Intraductal Component

A special variety of pancreatic cancer is characterized by a predominant intraductal proliferation with a low-grade proliferation rate. Immunohistochemical peculiarities and the lack of lymph node metastases despite an extended tumor, distinguish it from usual ductal pancreatic carcinoma. Adenocarcinoma of the pancreas with a predominant intraductal component is proposed for its classification.

ELASTOSIS IN NEOPLASTIC and NON‐NEOPLASTIC TISSUES FROM PATIENTS WITH MAMMARY CARCINOMA

We studied a series of 585 patients with non-invasive or invasive ductal carcinoma in an attempt to assess the significance of elastosis. Elastosis in the neoplasm was recognized in 60% of the 549 patients with invasive ductal carcinoma. The grade of elastosis was correlated with both the histologic grade of differentiation and the 5-year survival rate. The incidence of elastosis, however, was 17% in the 36 patients with non-invasive ductal carcinoma and 38% in the 21 with invasive ductal carcinoma with a predominant intraductal component. The increased elastic tissue may therefore be influenced by the stromal infiltration of cancer cells. Mastectomy specimens from another series of 100 patients with mammary carcinoma were examined with regard to the volume of elastic tissue. Increased periductal elastic fibers were also identified in the non-neoplastic tissue, but the volume density was far less than in the neoplasm. A significant correlation was found between the increased amount of periductal elastic fibers in the non-neoplastic tissue, periductal elastosis of the neoplasm and an increase in parity. We propose that cancer cells in mammary carcinoma exert an inductive effect on mesenchymal cells for the synthesis of elastic material, under the basic condition of an increased amount of elastic fibers with an increase in parity.
