Image Guided Percutaneous Drainage of Intra-Abdominal Collections

Background: Intra-abdominal collections and abscesses are commonly encountered in surgical practice. They contribute to significant morbidity and mortality, hence, need to be addressed expeditiously. Percutaneous catheter drainage (PCD) is an important treatment modality that is now in vogue. Aim: This study was carried out to assess the outcomes of image guided PCD of intra-abdominal collections/ abscesses. Materials and Methods: This prospective study was carried out at a tertiary centre on patients who underwent image guided drainage of intra-abdominal collections/ abscesses from Jul 2014 to Dec 2016. Data was collected by following up the patients both clinically and radiologically. Results: 31 patients underwent PCD. Procedure was successful in 77.4%. Complete resolution was observed in all post-operative collections. Collections resolved after a single drainage procedure in 71% while the remaining 29% required multiple drainage procedures. Liver abscesses were the commonest, with multiple foci. Organisms were isolated in 13%, Escherichia coli being the commonest isolate. Higher leucocyte counts, lower levels of drain amylase and shorter duration of antibiotics were significantly associated with the success of the procedure. There were no complications other than an isolated case of biliary fistula following drainage of liver abscess. Ultrasonography was found to be at par with CT scan for PCD. Conclusion: Image guided PCD is a safe and efficacious procedure for the treatment of intra-abdominal collections and abscesses. It has a high success rate, especially in postoperative collections. The procedure has low morbidity and low complication rates. High leucocyte count, short duration of antibiotic exposure and low levels of drain fluid amylase are independent predictors of favourable outcome after percutaneous drainage.

Predicting outcomes after acute reperfusion therapy for basilar artery occlusion.

Basilar artery occlusion (BAO) leads to high rates of morbidity and mortality, despite successful recanalization. The discordance between flow restoration and long-term functional status clouds clinical decision-making regarding further aggressive care. We sought to develop and validate a practical, prognostic tool for the prediction of 3-month favorable outcome after acute reperfusion therapy for BAO. This retrospective, multicenter, observational study was conducted at four high-volume stroke centers in the USA and Europe. Multivariate regression analysis was performed to identify predictors of favorable outcome (90-day modified Rankin scale scores 0-2) and derive a clinically applicable prognostic model (the Pittsburgh Outcomes after Stroke Thrombectomy-Vertebrobasilar (POST-VB) score). The POST-VB score was evaluated and internally validated with regard to calibration and discriminatory ability. External validity was assessed in patient cohorts at three separate centers. In the derivation cohort of 59 patients, independent predictors of favorable outcome included smaller brainstem infarct volume on post-procedure magnetic resonance imaging (P<0.01) and younger age (P=0.01). POST-VB score was calculated as: age+(10×brainstem infarct volume). POST-VB score demonstrated excellent discriminatory ability [area under the receiver-operating characteristic curve (AUC)=0.91] and adequate calibration (P=0.88) in the derivation cohort (Center A). It performed equally well across the three external validation cohorts (Center B, AUC=0.89; Center C, AUC=0.78; Center D, AUC=0.80). Overall, a POST-VB score<49 was associated with an 88% likelihood of favorable outcome, as compared to 4% with a score≥125. The POST-VB score effectively predicts 3-month functional outcome following acute reperfusion therapy for BAO and may aid in guiding post-procedural care.

Local tirofiban infusion for remnant stenosis in large vessel occlusion: tirofiban ASSIST study

BackgroundCompared with embolic occlusions, intracranial atherosclerotic stenosis (ICAS)-related large vessel occlusions (LVOs) often require rescue treatment following mechanical thrombectomy (MT). Herein, we hypothesized that local tirofiban infusion can be effective and safe for remnant stenosis in LVO during endovascular treatment and can improve clinical outcomes.MethodsThis observational multicenter registry study (January 2011 to February 2016) included patients with ICAS who underwent endovascular treatment for LVO within 24 h after stroke onset. An underlying fixed focal stenosis at the occlusion site observed on cerebral angiography during and after MT was retrospectively determined as a surrogate marker of ICAS. Procedural and clinical outcomes were compared between the tirofiban and non-tirofiban groups.ResultsOf 118 patients, 59 received local tirofiban infusion. Compared to the non-tirofiban group, patients were older (non-tirofiban group versus tirofiban group; median, 63 years vs. 71 years, p = 0.015) and the onset-to-puncture time was longer (median, 275 min vs. 395 min, p = 0.036) in the tirofiban group. The median percent of residual stenosis prior to rescue treatment tended to be higher in the tirofiban group (80 [71–86] vs. 83 [79–90], p = 0.056). Final reperfusion success (modified Treatment In Cerebral Ischemic 2b–3) was more frequent (42.4%vs. 86.4%, p = 0.016) and post-procedure parenchymal hematoma type 2 and/or thick subarachnoid hemorrhages were less frequent (15.3%vs. 5.1%, p = 0.068) in the tirofiban group. The frequency of favorable outcomes 3 months after endovascular treatment (modified Rankin Scale 0–2) was significantly higher in the tirofiban group (32.2% vs. 52.5%, p = 0.025), and tirofiban administration was an independent predictor of favorable outcomes (odds ratio, 2.991; 95% confidence interval, 1.011–8.848; p = 0.048).ConclusionsLocal tirofiban infusion can be a feasible adjuvant treatment option for patients with ICAS-LVO.

Endovascular treatment of acute carotid atherosclerotic tandem occlusions: Predictors of clinical outcomes as technical aspects and location of tandem occlusions

BackgroundWe aimed to analyze angiographic and clinical outcomes according to the sequence of treatment (antegrade versus retrograde) in patients with acute ischemic stroke caused by tandem extracranial cervical carotid and intracranial large vessel occlusion. Materials and methodsAll eligible tandem occlusion patients from April 2012 to March 2019 undergoing carotid artery stenting (CAS) simultaneously with intracranial endovascular thrombectomy (EVT) were retrospectively reviewed. After dividing into 2 groups according to the treatment sequence for tandem lesions (antegrade, CAS first; retrograde, intracranial EVT first), baseline data, immediate angiographic results, and clinical outcome for eligible patients were analyzed and compared. In addition, the same analysis was performed after dividing into 3 groups based on the location of intracranial lesions (T-zone, M1, M2). ResultsA total of 76 patients with a tandem occlusion (mean age, 71.7 y± 11.1) were treated with CAS and intracranial EVT. The rate of successful recanalization (TICI 2BC) was 83% (63/76), and favorable functional outcome was achieved in 49% (37/76). When comparing antegrade and retrograde methods, there were no differences in baseline data and angiographic or clinical outcomes. Favorable functional outcome was significantly higher in the M2 occlusion group (P=0.011). In multivariate analysis, baseline NIHSS <15, age <80, and M2 occlusion were revealed as independent predictors of favorable outcome. ConclusionDifferent endovascular sequences for tandem extracranial cervical carotid and intracranial large vessel occlusion do not affect angiographic or functional outcomes. Intracranial M2 occlusion, age, and baseline NIHSS were independent predictors of good clinical outcome at 3 months.

P-270 Early tumor shrinkage as a predictor of favorable outcomes in patients with unresectable metastatic colorectal cancer treated with first-line chemotherapy plus bevacizumab

Obtaining early tumor shrinkage, defined as ≥ 20% shrinkage in tumor size per RECIST criteria at the first radiological assessment (8 weeks of treatment), has been shown to be prognostic for improved survival of patients with mCRC treated with cetuximab with or without chemotherapy. The aim of our study was to evaluate the impact of this parameter in patients treated with first-line of chemotherapy combining fluoropyrimidine, irinotecan, and bevacizumab. All pts who received first-line chemotherapy combining fluoropyrimidine, irinotecan, and bevacizumab for unresectable mCRC, between March 2012 and May 2015, were included in an exploratory analysis of survival based on the presence or not of ETS. The univariate analysis of the PFS and OS was based on the Kaplan-Meier method and the Log-Rank test. Multivariate analysis was performed using a Cox model including ETS, sex and Köhne prognostic score. Fifty-two patients were enrolled; median age was 59 years (range 32-75), performance status 0-1 was 84.6%. 69.2% had multi-organ involvement. The median follow-up was 40 months. Fifty-one patients were evaluable for response: ORR 39.2%, SD 43.1 for a clinical benefit of 82.3%. 33% qualified for early response. Median PFS and OS for the whole group was 11 months (95% CI: 7.8-14.2) and 20.8 months (95% CI: 16.5-25.1), respectively. ETS was associated with prolonged PFS and OS (22.5 vs 10.3 months, p = 0.001; 52 vs 20 months, p< 0.0001). Early response to chemotherapy may predict favorable outcomes in patients with metastatic CRC treated with first-line bevacizumab plus chemotherapy, but large prospective analysis and validation are mandatory.

More than five stentriever passes: real benefit or futile recanalization?

Clot extraction is associated with favorable outcome in patients with large vessel occlusions (LVO) undergoing endovascular thrombectomy (EVT). However, whether revascularization becomes futile or harmful with an increasing number of passes remains unknown. We performed a retrospective analysis of 271 consecutive patients with LVO who underwent stentriever-based EVT as the primary recanalization strategy. Primary outcomes including favorable recanalization, survival, and favorable functional outcomes were compared according to a dichotomized number of stentriever passes utilized with a cutoff of 4. In the entire cohort, 234 (86%) patients reached favorable recanalization and 46 (17%) patients had ≥ 5 passes (range 5-40). Patients that had ≤ 4 passes had significantly higher rates of favorable recanalization and favorable outcomes and a trend towards lower mortality rates compared with those that had ≥ 5 stentriever passes (92% vs. 61%; p < 0.001, 52% vs. 30%; p = 0.009 and 12% vs. 22%, p = 0.098). Among patients that received ≥ 5 stentriever passes, 30% reached favorable outcomes. Patients who achieved recanalization after ≥ 5 passes had higher rates of favorable outcome in comparison with those who did not (p = 0.009). Among patients that had ≥ 5 stentriever passes favorable recanalization (OR 97.3, 95%CI 2.8-3399.3) and admission NIHSS (OR 0.77, 95%CI 0.60-0.99) remained independent predictors of favorable outcome, whereas the number of passes did not. A substantial proportion of patients reach favorable outcomes even when ≥ 5 stentriever passes are performed. Treatment choices should be individualized based on personal preferences and expertise as well as on patient and clot-specific characteristics.

Association between mitral regurgitation and clinical outcome after endovascular thrombectomy in stroke patients

ABSTRACT Objective Some hyperacute stroke patients have unfavorable outcomes after endovascular thrombectomy (EVT) despite successful recanalization. We hypothesized that a cardiac parameter, moderate-to-severe mitral regurgitation (MR), might decrease the rate of favorable clinical outcome after EVT in patients with atrial fibrillation (AF). Method From our prospective EVT registry, AF patients who underwent transthoracic echocardiography (TTE) were retrospectively analyzed. Based on the presence of moderate-to-severe MR, patients were assigned to either significant MR or nonsignificant MR group. The severity of MR was determined by the ratio of the color Doppler jet area to the left atrial area in mid-systole. Moderate-to-severe MR was estimated to be at a ratio of >20%. Favorable outcome was defined as having a modified Rankin Scale score of 0–1 at 3 months. Result 127 patients with AF who underwent TTE were included in the study. TTE results found that 25 (20%) patients had significant MR. Patients with significant MR were older (p = 0.051) and had enlarged left (p = 0.015) and right (p = 0.002) atria. Tricuspid and aortic regurgitation (p = 0.007 and 0.043, respectively) were more severe in significant MR group. At 3 months, favorable outcomes were 11% in the significant MR group and 26% in the non-significant MR group (p = 0.031). Multivariate regression analysis reported that moderate-to-severe MR was a negative predictor of favorable outcome (odds ratio = 0.14; 95% confidence interval = 0.02, 0.84; p = 0.031). Conclusions Significant MR might prevent the clinical recovery of AF patients.

Large Infarct Volume Post Thrombectomy: Characteristics, Outcomes, and Predictors

Despite increasing interest in endovascular therapy (EVT) for large-core strokes, little is known about the predictors of good outcome in these patients. The aim of this study was to analyze patients with large-core strokes post-EVT and to define the predictors of favorable outcome in this population. A retrospective analysis of prospectively collected data on anterior circulation strokes undergoing EVT between January 2015 and February 2018 was performed. Patients with good baseline functional status who underwent EVT for occlusion of an anterior circulation artery and achieved successful recanalization (modified Treatment in Cerebral Ischemia score ≥2b) but had large follow-up infarct volume (FIV ≥70 cm3) were included in the study. Demographic characteristics, clinical and radiologic data, treatment and postprocedural outcomes were extracted and analyzed. The primary outcome was 90-day modified Rankin Scale (mRS) score, stratified by favorable (mRS 0-3) versus unfavorable (mRS 4-6). Of 355 patients meeting inclusion criteria, 85 (24%) had large FIV on follow-up imaging after EVT and constituted the study cohort. No patients achieved mRS score 0-2 at hospital discharge; 32% had 90-day mRS score 0-3. On multivariate logistic regression analysis, lower FIV (OR, -0.96 [0.95-0.99]; P= 0.007), male sex (OR, -1.29 [1.07-12.3]; P= 0.026), and intravenous tissue plasminogen activator use (OR, 3.6 [2.01-8.9]; P= 0.003) were independent predictors of favorable outcome. Independent predictors of mortality on multivariate analysis were higher FIV (OR, -1.01 [1.007-1.02]; P=0.001) and female sex (OR, 4.08 [1.25-13.3]; P= 0.02). For patients with large-core strokes (≥70 cm3) after EVT, approximately one third have favorable outcome at 90 days. Independent predictors of favorable 90-day outcomes include male sex, intravenous tissue plasminogen activator use, and lower FIV.

Comparison of Minimally Invasive and Pterional Craniotomies for Ruptured Anterior Circulation Aneurysms: A Propensity Score Matched Analysis

Minimally invasive craniotomy (MIC) for ruptured aneurysm remains a debatable issue because of unclear information regarding its safety and efficacy compared with standard approaches. Here, we compared the outcomes between MIC and pterional craniotomy (PTC) for the treatment of ruptured anterior circulation aneurysms. A database of patients with ruptured anterior circulation aneurysm who were treated with surgical clipping was reviewed. With the use of propensity score matching to balance the baseline characteristics of MIC and PTC groups, outcomes of the 2 groups were compared. Clinical predictors of favorable outcomes (modified Rankin scale score 0-2) were evaluated by using uni- and multivariate analyses. A total of 102 matched pairs were identified. MIC resulted in a significantly shorter operative time (2.8±0.9 vs. 4.2 ± 0.7 hours; P= 0.004) and hospital stay (14.2± 5.9 vs. 19.2 ± 9.1 days; P < 0.001), respectively. Both MIC and PTC had similar mortality and complication rates except for the incidences of intracranial hemorrhage (2% vs. 9.8%; P= 0.039) and brain injury (9.8% vs. 27.5%; P= 0.036), respectively. Use of MIC instead of standard surgery and lower World Federation of Neurosurgical Societies (WFNS) grade and absence of hydrocephalus were significant predictors of favorable outcome at 1 month, whereas higher WFNS grade and higher Fisher grade were significantly associated with a poor outcome at 6 months. For the treatment of ruptured anterior circulation aneurysms, MIC was comparable with PTC and presented additional advantage in terms of earlier recovery. Therefore, MIC can be considered an alternative surgical treatment in this setting.

Sudden Recanalization: A Game-Changing Factor in Endovascular Treatment of Large Vessel Occlusion Strokes.

Background and Purpose- First pass (FP) recanalization has been shown to be a predictor of favorable outcome in endovascular treatment of stroke. The reasons why FP recanalization leads to better outcome as compared with multiple passes (MP) are unknown. We aim to investigate the recanalization pattern and its relationship with outcome. Methods- Six hundred nine consecutive patients underwent endovascular treatment. Recanalization was defined as modified Thrombolysis in Cerebral Infarction score 2B-3. Favorable outcome was defined as modified Rankin Scale score of 0 to 2 at 90 days. Sudden recanalization (SR) was considered when modified Thrombolysis in Cerebral Infarction score varied from 0-1 to 2B-3 in a single pass. Progressive recanalization (PR) was considered if modified Thrombolysis in Cerebral Infarction score 2A was achieved at an interim pass before achieving recanalization. Patients were also categorized as recanalizers at FP, MP, or nonrecanalizers. Results- Five hundred nine (83.9%) patients achieved recanalization. SR was achieved in 378 (62.1%) patients; 280 (46%) were FP-SR, and 98 (16.1%) were MP-SR. MP-PR was achieved in 131 (21.5%) patients. Rates of favorable outcome were similar between patients with FP-SR (57.5%) and MP-SR (57.1%; odds ratio [OR], 0.9 [CI, 0.53-1.54]; P=0.7) but lower in MP-PR (29.8%; OR, 3.33 [CI, 1.71-5.63]; P<0.01). Patients with MP-PR had better outcome than nonrecanalizer (17%; OR, 2.93 [CI, 1.42-6.15]; P<0.01). In univariate analysis, both FP recanalization (OR, 1.91 [CI, 1.31-2.72]; P<0.01) and SR (OR, 3.19 [CI, 2.12-4.95]; P<0.01) were associated with favorable outcome. However multivariate analysis adjusting for FP recanalization showed that the only procedural predictor of favorable outcome was SR (OR, 3.12 [CI, 1.91-5.16]; P<0.01). Higher number of passes were associated with worse outcome in patients with recanalization (OR, 0.75 [CI, 0.66-0.85]; P<0.01) but not in those with SR (OR, 0.94 [CI, 0.59-1.61]; P=0.937). Conclusions- SR strongly predicts favorable outcome in patients undergoing endovascular treatment, even after previous unsuccessful attempts. PR may reflect clot fragmentation and embolization due to more friable composition, leading to worse outcomes. Prospective studies with independent assessment of recanalization pattern should validate these results.

Abstract WP36: National Trends in Utilization and Outcome of Endovascular Thrombectomy for Acute Ischemic Stroke in Elderly

Background: The efficacy of endovascular thrombectomy (EVT) for large vessel occlusion in elderly is controversial as this age group is often excluded or under-represented in major randomized clinical trials. We evaluated the real-world trends in utilization and outcome of EVT in patients ≥80 years in a nationally representative database. Methods: Using nationwide inpatient sample (2014-2016), we identified patients admitted to hospitals in the United States with acute ischemic stroke who underwent EVT. Baseline demographics and clinical outcome were assessed among patients with age ≥80 years and those with age &lt;80 years. The primary endpoint was favorable outcome defined as discharge to home or acute rehabilitation center. Hemorrhage and in-hospital mortality rates were assessed as secondary outcome measures. Multivariate analysis was performed to identify independent predictors of favorable clinical outcome in elderly. Results: A total of 6,230 patients were identified and 1,547 (24.83%) of them were ≥80. The number of patients ≥80 years who underwent EVT increased from 323 (22%) in 2014 to 695 (28%) in 2016. The rate of favorable outcome in elderly was 9% which was significantly lower than in younger patients (22%). In-hospital mortality was 19% in patients ≥80 years compared to 13% in their younger counterpart. There was no difference in the rate of hemorrhagic transformation between octogenarians and younger patients (3.2% vs 2.6%). Among patient ≥80, decreasing baseline comorbidity burden (Elixhauser Comorbidity Index) [OR; (95% CI): 1.24; (1.04-1.47)] was an independent predictor of favorable outcome. Conclusions: Two-fold increase in the number of EVT was seen from 2014 to 2016 among patients ≥80 years old. Although the rate of favorable outcome is lower in this age group, comparison to a historical control group suggests significant outcome benefit from EVT in these patients. Future prospectively designed studies are warranted for further characterization of the risks and benefit of EVT in elderly.

Abstract WP64: Perfusion Imaging Can Identify Basilar Occlusion Patients With a Favorable Response to Thrombectomy

Introduction: Perfusion imaging can identify patients who respond favorably to endovascular therapy (EVT) in the anterior circulation; no data are available for the posterior circulation. We evaluated perfusion patterns, assessed with RAPID software, in a consecutive cohort of patients with basilar artery occlusion treated with EVT and assessed the association between reperfusion and favorable clinical outcome based on the perfusion profile. Hypothesis: We hypothesized that patients with limited regions of severe hypoperfusion (Tmax &gt; 10s) would have a favorable response (mRS 0-2) to reperfusion (mTICI 2b-3) while patients with multiple critical brain regions severely hypoperfused would have poor outcome (mRS 4-6) despite reperfusion. Methods: From a multicenter cohort of perfusion imaging in posterior circulation stroke, we included patients with basilar artery occlusion and EVT. We pre-specified a Critical Area Perfusion Score (CAPS, 0 - 8 points) to identify severe hypoperfusion (Tmax &gt;10s) in the following regions: inferior and/or superior cerebellar hemisphere (1-4 points), pons (2 points), midbrain/thalamus (2 points). We compared the outcome between reperfusers and non reperfusers based on the CAP score with univariate and multivariate analysis. Results: 38 patients met the inclusion criteria. Mean age was 63±17, 34% female, NIHSS 17±11. In patients who reperfused (n=30, 79%) 63% had favorable outcome, while no patient without reperfusion survived, p=0.003 OR=29 (95%CI 1.5-547). Ninety percent (19/21) of reperfused patients with CAPS ≤2 had a favorable outcome, while none of the 9 with reperfusion and a score &gt;2 survived, p&lt;0.001, OR=148 (95%CI 6.5-3,333). In univariate analysis, favorable outcome was associated with NIHSS OR=0.87 (95% CI 0.80-0.96), p=0.003, and mismatch volume OR=0.98 (95% CI 0.97-0.997) p=0.013. In the multivariate analysis, only CAPS was an independent predictor of favorable outcome. Conclusions: Patients with limited regions of severe hypoperfusion (Tmax &gt; 10s) had a robust response to basilar artery EVT, however, all patients with multiple critical brain regions severely hypoperfused died despite successful reperfusion. Perfusion imaging profiles may help identify optimal patients for basilar EVT.

Prediction and differentiated approach in the treatment of patients with secondary peritonitis and abdominal sepsis

To identify the most important high risk criteria for predicting course of disease, as well as optimal preoperative preparation and surgical strategy in patients with secondary peritonitis. A prospective study enrolled 43 patients with diffuse secondary peritonitis. Significant predictors were Charlson's comorbidity index (p=0.001) and SOFA score of organ dysfunction within 3 days after admission. Rapid regression of organ dysfunction (SOFA1 - p=0.0001, SOFA2 - p=0.012, SOFA3 - p=0.017) and reduced time of examination and preoperative preparation (threshold value - 520 min after admission) are predictors of favorable outcome in patients with diffuse secondary peritonitis. There was no reliable correlation between the treatment outcome and preoperative preparation (infusion volume p=0.23, duration p=0.37, absence/presence of antibacterial therapy p=0.26). Elimination or control of infection is the fundamental principle of the management of patients with diffuse secondary peritonitis.

Noninvasive tri-level positive airway pressure ventilation: is it a new challenging mode in management of patients with chronic obstructive pulmonary disease?

Background Chronic obstructive pulmonary disease (COPD) is a serious issue of sickness and rise in global fatality rates. Noninvasive positive pressure ventilation is the first-line therapy in patients with COPD with respiratory fialure (RF). Tri-level positive airway pressure (Tri-PAP) is new challenging mode of noninvasive ventilation (NIV). Objectives To compare between two modes of noninvasive positive pressure ventilation [bi-level positive airway pressure (Bi-PAP) and Tri-PAP] and to spell out outcomes, complication, and predictors of favorable outcome of NIV in patients with COPD with respiratory failure. Patients and methods This prospective comparative study was accomplished on 30 patients with COPD with respiratory failure, who were subjected to NIV (Bi-PAP or Tri-PAP) and standard conventional therapy. Arterial-blood gases (ABGs) (initially, 6 h, and finally) were analyzed. NIV outcomes, complications, and predictors of success were encountered. Results Patients were categorized according to managerial therapeutic NIV into Bi-PAP (n=15) or Tri-PAP (n=15) groups. Initial, 6-h, and final ABGs analyses (pH, PO2, PCO2, HCO3, and O2 sat) and NIV outcomes (successful or failed weaning) were analogous, without considerable variation between both Bi-PAP and Tri-PAP groups. pH showed crucial advancement after 6 h of initiation of Tri-PAP NIV. Terminal ABG interpretation (pH, PaCO2, PaO2, and O2 sat) had better outcomes than initial and 6-h ABG analyses of Tri-PAP NIV group. Irritations and ulcerations of nasal bridge, dry nose or mouth, nasal congestion, hypotension, and PaCO2 increase were the most hazardous events in both Bi-PAP and Tri-PAP NIV groups. Shorter duration of NIV, lower BMI, higher pH, and lower PCO2 (initially, 6 h, and final ABGs) as well as higher O2 saturation (in final ABG) were predictors of successful weaning in NIV (Bi-PAP and Tri-PAP) groups collectively. Conclusion Bi-PAP and Tri-PAP are appropriate mode of NIV in patients with COPD with RF with minimal complication and better outcomes. Tri-PAP had no superiority of utilization than Bi-PAP mode. pH and PCO2 are the powerful predictors of favorable NIV outcomes.

Predictors of a Favorable Outcome after Emergent Carotid Artery Stenting in Acute Anterior Circulation Stroke Patients.

PurposeThis study aimed to identify independent predictors of favorable outcomes associated with emergent carotid artery stenting (CAS) in patients with acute anterior circulation stroke.Materials and MethodsThis study included 93 patients with acute stroke who underwent emergent CAS to treat stenoocclusive lesions in the cervical internal carotid artery (ICA) within 6 hours of the onset of the associated symptoms. Data were compared between patients with and without favorable outcomes. The independent predictors of a favorable outcome were determined via logistic regression analysis (modified Rankin Scale 0–2 at 90 days).ResultsIntracranial tandem occlusion was noted in 81.7% of patients (76/93) among which (76/93), 55 of whom underwent intracranial recanalization therapy. Intracranial reperfusion was successful in 74.2% (69/93) and favorable outcomes were noted in 51.6% of patients (48/93). The mortality rate was 6.5% (6/93). In logistic regression analysis, diffusion-weighted imaging-Alberta Stroke Program Early CT Score [odds ratio (OR), 1.487; 95% confidence interval (CI), 1.018–2.173, p = 0.04], successful reperfusion (OR, 5.199; 95% CI, 1.566–17.265, p = 0.007), and parenchymal hemorrhage (OR, 0.042; 95% CI, 0.003–0.522, p = 0.014) were independently associated with a favorable outcome.ConclusionBaseline infarct size, reperfusion status, and parenchymal hemorrhage were independent predictors of favorable outcomes after emergent CAS to treat stenoocclusive lesions in the cervical ICA in patients with acute anterior circulation stroke.

Aneurysmal Bone Cyst of the Cervical Spine

Aneurysmal bone cyst (ABC) is a non-neoplastic locally destructive primary bone lesion mostly occurring within the first two decades of life with female predominance. Cervical spine ABCs are a very rare clinical entity with particularly demanding surgical resection limited by the involvement of nearby neurovascular structures and a risk for cervical spine instability. We present a case of a 17-year-old patient with a symptomatic ABC of the cervical spine who underwent surgical removal of the osseous lesion with good postoperative outcome. There are different therapeutic strategies for ABC management, but complete surgical removal of the cyst remains one of the most important predictors of favorable outcome, although it can be often limited, especially in the cervical region. Previously described lesions have a very high recurrence rate and therefore different additional methods, such as radiotherapy, embolization and sclerosation have been used in addition to surgical resection. Further studies are necessary for development of evidence-based treatment regimens for cervical spine ABCs in pediatric population.

Management of subtrochanteric fractures of femur: A narrative review

Subtrochanteric fractures constitute complex proximal femoral fractures. These fractures are often associated with complications related to fracture union, loss of reduction, and implant failure. Reduction and stable fixation have been implicated as the major predictors for favorable outcomes. This narrative review briefly discusses various aspects of the management of subtrochanteric fractures along with various technical tips, which are helpful in achieving a satisfactory reduction of these fractures.

Decitabine-Induced Early Platelet Response, a Predictor of Favorable Outcome during Hypomethylating Treatment of MDS, Is Associated with In Vivo Megakaryocytic Differentiation

Introduction Despite broad use of hypomethylating agents (HMAs) in MDS/AML treatment, the number of established outcome predictors is still very limited (Stomper and Lübbert, Sem Hematol 2019). One of them, the early, isolated and sometimes massive increase of platelets, is recurrently observed in patients with MDS treated with HMAs. It is not observed with non-HMA cytidine analogs such as low-dose cytarabine. In HMA-treated MDS patients, an early platelet increase is a predictor of overall and leukemia-free survival (van den Bosch et al., Leuk Res 2004; van der Helm et al., Br J Haematol 2011). HMAs induce cellular differentiation in vitro, by gene reactivation in the malignant cells. However, evidence for HMA-induced in vivo differentiation is still very limited. We hypothesized that the megakaryocytic cell lineage in MDS is a target for HMA-induced cellular maturation in vivo. Methods We systematically analyzed the bone marrow morphology of 34 higher-risk MDS patients (median age: 71.5 years, range 51-79) before and after 1 cycle of treatment with the HMA decitabine (DAC). All patients had been treated at a single center within 3 prospective clinical trials (Wijermans et al., Ann Hematol 2005; Lübbert et al., J Clin Oncol 2011). One treatment cycle consisted of 45 mg/m2 DAC per day (15 mg/m2 intravenously over 4 hours every 8 hours) for 3 consecutive days, repeated 6 weeks later. The early platelet response was evaluated after 1 cycle of DAC treatment. Based on the criteria of the International Working Group, an absolute increase in platelet count of 30x109/l or more compared to the pre-treatment count was defined as a platelet response. The histological analysis of the bone marrow specimens was performed by an experienced hematopathologist blinded to the treatment timepoints. Up to 200 megakaryocytes (MK) per sample were quantified at a magnification of 400 x using chloroacetate esterase staining. Results Thirteen of 34 patients (38%) showed a platelet response already after 1 cycle of DAC treatment, 21 (62%) did not. The median pre-treatment platelet count did not differ in patients with or without an early platelet response (median of 34x109/l in both groups, range 7-169 and 8-265, respectively). After 1 cycle of DAC treatment, patients with a platelet response had a median platelet count of 117x109/l (range 78-281), patients without this response had a median platelet count of 32 x109/l (range 4-155). Overall survival (OS) was measured from the time of early platelet response assessment after completion of 1 treatment cycle, i.e. after 6 weeks. The presence of a platelet response after 1 DAC cycle was associated with a longer OS compared to the absence of this early platelet response: median of 26.6 versus 14.0 months (p=0.04). Both pre- and post-treatment bone marrow biopsies of patients with an early platelet response showed higher numbers of MK, as well as significant differences in MK morphology compared to biopsies from patients without an early platelet response. Regarding MK numbers, increased MK density in specimens of patients with an early platelet response was observed both before (mean MK number per high power field 6.2 vs. 2.6, p=0.02) and after the application of DAC (mean MK number 10.4 vs. 3.1, p=0.01). Regarding MK maturation stage, more pre-treatment juvenile MK (on average 32.4% vs. 20.5% of all MK, p=0.03) and MK with typical myeloproliferative stigmata (present in 5/13 vs. 2/21 biopsies) were observed in patients with an early platelet response, compared to patients without this response. Regarding the induction of megakaryocytic maturation during this early treatment phase, more post-treatment "naked", mature MK nuclei indicative of active platelet shedding (on average 9.5% vs. 3.8% of all MK, p=0.01), were noted in patients with an early platelet response than in patients without an early platelet response. Conclusions This is, to the best of our knowledge, the first systematic hematopathological analysis of changes in quantitative and morphological MK features in bone marrow specimens of MDS patients during HMA treatment. DAC, which has in vitro differentiation-inducing effects on megakaryoblastic cells, induced maturation also of dysplastic MK in vivo in higher-risk MDS patients with an early platelet response to this HMA. The predictive value of an early platelet increase, a very easy-to-determine parameter, during this type of treatment is confirmed. Disclosures No relevant conflicts of interest to declare.

PI3Kδ Inhibition Enhances Sensitivity of Primary High-Risk Childhood B-Cell Acute Lymphoblastic Leukemia Cells to Glucocorticoid Chemotherapy

Introduction: Despite excellent 5-year survival for most patients with B-cell acute lymphoblastic leukemia (B-ALL), those with high-risk (HR) B-ALL continue to have inferior outcomes. The prognosis is even worse for patients who respond poorly to chemotherapy, and improvements to current therapy approaches are needed. Glucocorticoids (GCs) are a key component of chemotherapy regimens. Rapid response to GC monotherapy is a well-documented predictor of favorable outcomes, while slow response to GCs correlates with poor outcomes. We hypothesized that enhancing specific GC toxicity to B-ALL cells can improve clinical outcomes for high-risk patients without increasing systemic toxicity. PI3Kδ is a promising target for this purpose. PI3Kδ is a critical component of the B-cell receptor cell-survival pathway; it restrains GC signaling, while GCs repress its expression. PI3Kδ expression is restricted to lymphoid cells, making it an appealing precision target to enhance GC toxicity in lymphoid cells while minimizing risk of increased systemic GC-associated toxicity. We previously demonstrated synergy between dexamethasone (dex) and the FDA-approved isoform-selective PI3Kδ inhibitor idelalisib (idela) in B-ALL cell lines, patient specimens, and patient-derived xenograft models (Kruth et al Blood 2017). In clinical practice, both dex and prednisone (pred) are essential components of childhood ALL chemotherapy. In this study, we sought to elucidate the extent to which idela in combination with dex or pred could induce synergistic killing of HR B-ALL specimens with various genetic backgrounds. Methods: Peripheral blood or bone marrow from patients ages 1-31 years with newly-diagnosed NCI HR or standard-risk (SR) B-ALL at the University of Iowa Stead Family Children's Hospital was obtained after informed consent on an Institutional Review Board-approved research study. Mononuclear cells were isolated from the specimens via Ficoll gradients and cultured in vitro in 384-well plates. Immediately after isolation, cells were treated with serial dilutions of the GCs dex or pred and idela in RPMI1640+10% FBS medium. Cell viability was measured with PrestoBlue (Invitrogen) after 72 hours of treatment. Survival curves were plotted to determine the LD50 of each drug using GraphPad. Dose effects around the LD50 for each drug were analyzed using Compusyn to determine if the drug combination was synergistic (combination index, CI &lt;1), additive (CI=1), or antagonistic (CI &gt;1). Results: As of August 1, 2019, 5 HR B-ALL patient specimens of various genetic alterations have been tested. Two patients were high-risk at diagnosis due to steroid pre-treatment (MAP011) or by NCI risk criteria (age &gt;10 years and/or WBC count &gt;50K/mm3; MAP014). Three patients with initially SR B-ALL were upstaged due to minimal residual disease (MRD) &gt;1% at day 8 in peripheral blood (MAP012) or marrow MRD ≥0.01% at end of induction (MAP009, MAP010). In our in vitro studies, only one sample (MAP014) showed reduced viability with GC alone, while 4 specimens (all except MAP012) responded to idela alone. Most specimens showed a synergistic response to dex or pred in combination with idela with greater reduction in cell viability compared to GC monotherapies. Surprisingly, one specimen (MAP010) had an antagonistic response with pred and idela combination. This near-haploid B-ALL specimen had reduced viability with idela monotherapy, but an antagonistic response with single-agent pred, raising the question of whether GC/idela synergy is influenced by genetic background. Conclusions: In pilot studies, idela sensitized HR B-ALL cells to GC chemotherapy and is a promising strategy for further evaluation in a larger cohort of specimens. Our results highlight the potential impact of genetic heterogeneity within childhood B-ALL upon differential therapeutic responses to standard steroid chemotherapy and demonstrate a potential precision medicine approach to augment GC sensitivity via combination with selective PI3Kδ inhibition. Brentuximab Vedotin Plus Bendamustine: As Salvage Regimen for Relapsed or Refractory Hodgkin Lymphoma, King Abdulaziz Medical City - Jeddah - KSA Experience

INTRODUCTION:In advanced stage Hodgkin's Lymphoma (HL), 30% of patients experience recurrence post upfront ABVD 1.Patients with sensitive disease to salvage chemotherapy typically proceed to ASCT with curative intent 2. Achievement of CR is one of the strongest predictor of favorable outcome after ASCT3-4. Recently brentuximab vedotin plus bendamustine (BvB) became an attractive option for relapsed/refractory HL with promising efficacy and non-overlapping toxicity profiles 5-6. CONTEXT: No PubMed searchable articles regarding use of BvB in relapsed/refractory Hodgkin lymphoma in Saudi Arabia could be found. OBJECTIVE:To determine overall response rate( ORR ) , PFS and OS. Secondary objective was to determine if there was a better response rate when BvB was used as a first salvage. DESIGN:Retrospective cohort analysis. PATIENTS:Patients with relapse /refractory hodgkin lymphoma received BvB as salvage between year 2016 and 2018 were included. Chemotherapy software and medical records were reviewed for clinical data. Twenty four patients were identified. One patient was excluded due to incomplete medical information. Patients were divided into two groups based on their indication for salvage therapy either refractory or relapse, and another two groups based on whether they received BvB as a first salvage or subsequent salvage. Brentuximab vedotin dose was 1.8 mg /kg on day 1 and bendamustine dose was 90 mg/m2 on day 1,2 every 21 days. RESULTS: Twenty-three patients (13 primary refractory and 10 relapsed) were included in this analysis. Median age at diagnosis was 28 years. After a median of 3 cycles of combination therapy BvB, the ORR was 82.6%. Ten patients (43%) achieved CR and 9 patients (39%) achieved PR. Patients who received BvB as a first salvage (65%) achieved 86.7% overall response , while ORR was 75% when BvB was given as subsequent salvage with border line P- value. After a median of 11 months follow-up, the estimated 1-year progression-free survival was 83% and 90% OS. Two patients (8.6%) experienced infusion-related reactions during second cycle. CONCLUSIONS:BvB represents a very effective regimen for high risk relapsed or refractory HL patients; in achieving higher CR rates when used as a first salvage, with manageable toxicity profile.
