Pandemics: past, present, and future: multitasking challenges in need of cross-disciplinary, transdisciplinary, and multidisciplinary collaborative solutions.

The extensive history of pandemics has spanned many centuries, profoundly impacting societies, economies, and public health, and thereby shaping the course of history in various ways. Advances in medicine, science, and public health practices have played a pivotal role in mitigating the effects of pandemics over time. This review explores the scientific landscape of contemporary pandemics, examining their diverse and complex nature. It goes beyond the biological aspects of pandemics to consider socioeconomic, environmental, and technological factors. Through a scientific lens, this study aims to understand the complexities of pandemics and contribute to the expanding knowledge base that helps humanity strengthen its defenses against global health threats. By elucidating the enigmas of pandemics, the study hopes to foster a more resilient and prepared global health environment. Highlighting the importance of a multidisciplinary, cross-disciplinary, and transdisciplinary approach, this exploration emphasizes the critical need to integrate biological, socioeconomic, environmental, and technological domains to develop more robust defenses against these global health challenges.

The Role of Optimistic Bias and Affect on Social Media Searches About COVID-19

ABSTRACT The preventive health behavior people adopt is partly a result of the risk they perceive from the threat, and health behavior theory has shown that risk communication is a critical part of that outcome. But risk to self and risk to others are often judged differently. Optimistic bias, which describes an unrealistic level of optimism about a threat, is a well described and frequently observed phenomenon in the study of health behavior. Traditional measurements of this construct have typically used the difference in self and other risk levels, which may obscure the impact. This study used a moderated mediation path with other-risk as a moderator of self-risk to study how optimistic bias and emotion about a rapidly changing risk may impact information seeking about it through social medial channels, which represent a still nascent but evolving media for credible health information. Results showed that optimistic bias about developing symptoms of COVID was indeed present and that the effect of perceived self-risk was mediated by fear and anxiety to predict social media searches about the threat. Further, affect and social media search behavior decreased with increasing levels of perceived other risk, indicating optimistic bias served to dampen a person’s motivation to seek information. The implications of the results on health behavior theory, risk communication, and public health practice are discussed.

Using Civic Service Design Methods to Redevelop a Data Communication Website With a Health Literacy Lens.

Public health agencies routinely publish data in hopes that data influence public health policy and practice. However, data websites can often be difficult to use, posing barriers to people trying to access, understand, and use data. Working to make data websites easier to use can add value to public health data communication work. The New York City Department of Health and Mental Hygiene (DOHMH) redesigned its Environment and Health Data Portal, a website used to communicate environmental health data, with the goal of making data more accessible and understandable to a broader audience. The DOHMH used Civic Service Design methods to establish priorities and strategies for the redesign work, to build a data communication website that emphasizes a high level of usability, and content that explains data. By following a Civic Service Design process, the DOHMH synthesized findings from health communications, data visualization and communication, and web usability to create an easy-to-use website with explanations of data and findings alongside datasets. On the new site, automated dataset visualizations are supplemented with narrative content, explanatory content, and custom interactive applications designed to explain data and findings. Web analytics showed that, in its first year of operation, the site's web traffic grew substantially, with the last 12 weeks recording weekly page views 150% higher than the first 12 weeks of operation (7185 average weekly page views compared with 2866 average weekly page views). Two-thirds (66.3%) of page views include recorded user engagement. Additional evaluations to measure specific aspects of usability compared with the previous version of the site are planned. By following a Civic Service Design process, the DOHMH redesigned a vital data communication platform to increase its usability and saw significant increase in engagement in its first year of operations. By designing data material with usability in mind, public health departments have the potential to improve public health data communication work.

The Role of Nursing in the Management of Chemotherapy Extravasation: A Systematic Review Regarding Public Health.

The scientific literature was reviewed with the aim of analysing the state of the art on the role of nursing in the management of chemotherapy extravasation, recognising the possible risk factors and identifying effective training programmes for nurses. WOS, Scopus, and PubMed databases were used to perform the searches. Papers that met the inclusion criteria and that had been published in the last 9 years were selected. The Effective Public Health Practice Project (EPHPP) instrument was applied to the selected studies. In addition, this research was registered in the International Prospective Register of Systematic Reviews (PROSPERO) (ID: 512480). Out of the 23 initially selected articles, a total of 9 articles were eventually included, since they met the eligibility criteria that were established after a more exhaustive analysis, which included reading their abstracts and full texts. The results show that the management of chemotherapy extravasation is closely related to factors that largely depend on the nursing staff. Among the most relevant findings are factors including the identification of the nursing role in the management of extravasation due to chemotherapy; risk factors; and effective training programmes for nursing. Nurses play a crucial role throughout the entire process of treatment, prevention, and health education in chemotherapy treatment. Training programmes for nurses are fundamental, as they increase their professional competence and improve the safety of the patient. Adequate knowledge of chemotherapy treatment and the risk factors of extravasation are basic elements for the prevention of this type of injury, as well as for the improvement of the quality of life of patients under this kind of intravenous therapy.

Food Proteins as Functional Ingredients in the Management of Chronic Diseases: A Concise Review.

Chronic diseases have emerged as a formidable global health concern, with their prevalence steadily rising over the years. Several approaches to addressing these concerns include the use of medications, which are often expensive, contain synthetic chemical substances, and have reported adverse effects. The use of foods, especially proteins, as an alternative approach to addressing chronic health concerns by treating and managing chronic diseases is increasing. This review evaluates the intriguing role of food proteins in mitigating chronic diseases and improving our understanding of the therapeutic potential of different protein types, including those derived from legumes, nuts, and seeds, dairy, fish, and numerous other sources. They have been reported to offer promising avenues for managing chronic diseases, including cardiovascular diseases, diabetes, chronic inflammation, weight management, bone health, glycemic control, muscle preservation, and many other health benefits. Although the exact mechanisms for these actions are still not properly elucidated, it is, however, understood that food proteins exert these health-beneficial effects by their unique nutritional and bioactive profiles, especially their bioactive peptides and amino acids. Practical applications are also discussed, including dietary interventions that are tailored towards incorporating protein-rich foods and the development of functional foods for disease prevention and management. Food proteins are a promising approach to combating chronic diseases that can turn around public health practices.

Frailty and its association with long-term mortality among community-dwelling older adults aged 75 years and over

BackgroundFrailty, a significant risk factor for adverse outcomes and mortality, poses an emerging challenge with profound implications for public health and clinical practice. The measurement of frailty offers potential enhancements in healthcare services for older adults. The prevalence of frailty and its association with long-term mortality in a nationwide, unselected population of community-dwelling older adults, particularly those aged 75 and over, has not been previously studied on a large scale in Israel.MethodsA retrospective cohort study was conducted at Meuhedet Health Maintenance Organization, Israel’s third largest healthcare service provider, serving 1,276,000 people (13.8% of Israelis). The prevalence of frailty and its association with all-cause mortality were studied among older adults aged 75 years and over who were followed for 2–8 years. Frailty, defined by the cumulative deficit method, utilized clinical data from the preceding 10-year period, comprising 28 chronic diseases and age-related health deficits.ResultsThe cohort included 43,737 older adults, with a median age of 77 years (IQR 75–82 years); among them, 19,300 (44.1%) were males. Overall, 19,396 (44.3%) older adults were frail: 12,260 (28.0%) mildly frail, 5,533 (12.7%) moderately frail and 1,603 (3.7%) severely frail. During the follow-up period 15,064 (34.4%) older adults died: 4,782 (39.0%) mildly frail, 3,016 (54.5%) moderately frail and 1,080 (67.4%) severely frail. Cox regression analysis demonstrated that mortality was associated with severe frailty (HR 2.63, 95%CI 2.45–2.80), moderate frailty (HR 2.05, 95%CI 1.96–2.14), and mild frailty (HR 1.45, 95%CI 1.39–1.51), independent of age, gender, and population sector. Among patients aged 90 years and over, no significant differences in cumulative survival were found between those with moderate and severe frailty (p = 0.408).ConclusionsFrailty is prevalent among community-dwelling Israeli older adults aged 75 years and over, and it is associated with long-term mortality. Considering its association with long-term mortality across frailty levels until the age of 90, early identification and intervention for frailty are recommended within this population. Policymakers should consider the use of the cumulative deficit method for evaluating frailty at both the population health and clinical levels.

Oral health promotion and dental care of children with microcephaly in the context of public health

The university's fundamental mission is to be a source of scientific knowledge, technological innovation, and academic public health practices, all the while being mindful of social responsibility. But when faced with children in vulnerable clinical conditions, what would this public and professional knowledge and practice be? Currently, there is a lack of oral health education programs aimed at children with microcephaly, including a protocol for preventive dental care. This study aims to describe the field experiences of the academic team of the Dental School of the State University of Southwest Bahia in oral public health practices in children with microcephaly based on the principles of universality, equity, and integrality. Extracurricular activities provide a dialogue between the university and society, where both environments teach and learn from each other, making the university not the only place where knowledge is shared. Consequently, professors and students must understand that in order to achieve a high quality education, the implementation of extracurriculars in their academic curriculum is extremely necessary.

Two years in a post- Dobbs world: clinical, social and professional consequences.

Dobbs v Jackson Women's Health Organization revoked the decades-old precedent that pregnancy termination is a constitutional right. This review article explores the research landscape describing the consequences of overturning Roe v Wade for patients and providers. To date, fourteen states have enforced total bans on abortion, with seven more restricting abortion access to levels not seen since before Roe. Dobbs has had immediate and swift consequences from clinical, social and professional perspectives, with increases in maternal mortality and demand for long-acting and permanent contraception, matched by declines in both access to methotrexate and applications to Obstetrics & Gynecology training programs. Eighteen million patients now live in states where abortion access is highly if not completely inaccessible. Abortion restrictions have profound implications beyond those desiring pregnancy termination; future research should continue to explore the ways Dobbs has affected clinical care, public health and social practices.

Perinatally Acquired HIV: Viral Evolution and its Implications for Adolescents

This literature review explores the implications of viral evolution on the health outcomes of adolescents with perinatally acquired HIV, offering recommendations for enhancing public health practices. Since the emergence of HIV in the 1980s, perinatal transmission from parent to child has become increasingly prevalent in developing countries. Antiretroviral therapy (ART) has greatly improved the life expectancy of children with perinatally acquired HIV, allowing them to reach adolescence. ART plays a critical role in suppressing HIV and reducing the risk of transmission. Nevertheless, low adherence or lack of treatment can lead to viral evolution and drug resistance, posing significant threats to adolescents. Challenges faced by adolescents with perinatally acquired HIV are multifaceted, including increased viral evolution and difficulties in adhering to ART. The impact of stigma, behavioral factors, and social and familial dynamics contribute to low adherence rates. Global inequities and systems-level challenges further complicate the situation, particularly in resource-limited settings. Adequate public health infrastructure and innovative approaches, such as long-acting injectable ART, are imperative to address these structural barriers and reduce drug resistance among adolescents. Comprehensive data collection and longitudinal studies are needed to understand the long-term consequences of perinatally acquired HIV and its impact on viral evolution. A multidisciplinary approach involving healthcare providers, virologists, and social scientists is crucial to meeting global HIV goals and addressing the specific needs of this vulnerable population.

Reformed theology critique of public health policies and practices in KwaZulu-Natal

Churches are currently successfully involved in community development and empowerment, providing health workers and agencies with support and helping meet tangible needs of the people. Four fundamental areas are undertaken in responding to the reformed critique of public health policies and practices in KwaZulu-Natal (KZN), namely: public health policies and practices in light of Reformed theology, public health policies and practices in KZN, the role of religion and religious entities in contributing to health in communities and to investigate the possibilities of collaboration between church and state to improve the public health system in KZN. The article aims to establish a common framework of strengths, barriers, and make recommendations for positive church responses to public health issues in order to inform an improved collaborative strategy.

Complex intertwined association between breastfeeding practices and household food insecurity: Systematic review and meta-analysis.

Breastfeeding offers ideal food and nutrition for infants; however, structural barriers may amplify breastfeeding inequities. We aimed to identify whether household food insecurity (HFI) is associated with exclusive and continued breastfeeding (EBF and CBF) as recommended by the World Health Organization/United Nations Children's Fund (UNICEF) Following the Preferred Reporting Items for Systematic Reviews and Meta-Analysis, literature searches using 'breastfeeding', 'food insecurity' and 'infant' terms were conducted in PubMed/MEDLINE, Embase, CINAHL, Global Health and LILACS from inception through November 2023, without language restrictions, yielding 1382 publications (PROSPERO:CRD42022329836). Predetermined eligibility criteria yielded the 12 included studies (nine cross-sectional and three cohorts). The risk of bias was assessed through the Effective Public Health Practice Project. Meta-analysis was performed for studies assessing EBF (n = 10), and metaregression was used to explore heterogeneity across studies. Prevalence of EBF ranged from 1.6% to 85.3%, with a lower prevalence among HFI. The pooled effect of the association between HFI and EBF presented an odds ratio (OR) of 0.61 (95% CI = 0.49-0.76) with consistent results across marginal (OR = 0.72, 95% CI = 0.55-0.94), moderate (OR = 0.59, 95% CI = 0.41-0.84) and severe HFI (OR = 0.49, 95% CI = 0.32-0.76). High heterogeneity was found only when HFI was dichotomized. The prevalence of CBF ranged from 35.4% to 78.0%, with inconsistent prevalence among HFI; a meta-analysis was not performed due to the low number of studies (n = 3). We concluded that HFI levels are associated with lower odds of EBF. Integrating service and policy-level strategies, such as screening, referrals, skilled breastfeeding counseling and access to comprehensive nutrition and social programs, could reduce structural inequities and promote adherence to the World Health Organization/UNICEF breastfeeding recommendations among food-insecure families.

Utilizing rapid qualitative assessment and thematic analysis methods to identify and share promising case investigation and contact tracing practices with people in refugee, immigrant, and migrant communities during COVID-19.

In the early months of the COVID-19 pandemic The National Resource Center for Refugees, Immigrants and Migrants (NRC-RIM) was established. NRC-RIM initially sought to rapidly identify promising case investigation and contact tracing (CICT) practices within refugee, immigrant, and migrant communities. Between September 2020 and April 2021, the team conducted 60 interviews with individuals from cross-sector organizations (i.e., public health, health systems, community experts/organizations) working with refugee, immigrant and migrant communities in health and public health capacities related to COVID-19. The overarching aim was to identify and amplify innovative promising and best practices for CICT with refugee, immigrant, and migrant communities, including an exploration of barriers and facilitators. We utilized layered methods to rapidly assess, summarize and disseminate promising practices while simultaneously completing four thematic analyses including: (1) public health organizations; (2) health system organizations; (3) community leaders and organizations; and (4) vaccine planning and access across the three sectors. The primary objective of this article is to describe the project design, applied methods, and team science approach we utilized. We found that rapid identification and dissemination of promising practices, and barriers and facilitators for CICT with refugee, immigrant and migrant communities was feasible during a public health emergency. This approach was essential for identifying and widely sharing culturally and linguistically concordant public health practices.

Decoding the Future: Genomic Sequencing's Vital Role in Communicable Disease Prevention within Public Health Practice - A Scoping Review

Precision public health using genomic sequencing is a new field that has gained interest from public health practitioners for prevention and control measures. This study aims to identify the various applications of genomic sequencing for the prevention of communicable diseases in public health practice. Articles containing relevant keywords were determined using a systematic search strategy applied in Medline, Scopus, and Springer electronic databases. Full text included in the study was retrieved and categorised. A total of 24 articles were included in the final review. The main themes regarding the application of genomic sequencing in the prevention of communicable disease that were found in the articles were describing transmission patterns, investigating outbreaks, diagnosing infection, developing and evaluating interventions including vaccines, outcomes response treatment, and monitoring antimicrobial resistance. In conclusion, genomic sequencing has the potential to enhance the prevention and control of communicable diseases globally.

Gerontology in Public Health: A Scoping Review of Current Perspectives and Interventions.

The intersection of gerontology and public health is increasingly vital due to the global aging population and its implications for health systems. This scoping review aims to map existing literature on gerontology within public health, identify current perspectives, and evaluate interventions tailored to the needs of older adults. A systematic search was performed using predefined keywords across multiple databases, including PubMed, Google Scholar, Scopus, and Web of Science. The review included 42 studies that employed various designs, all focusing on public health interventions targeting the aging population. Key findings indicate a pressing need to integrate gerontological principles into public health practice, recognizing the heterogeneous nature of older adults and the significance of social determinants of health. Interventions ranged from preventive health measures and chronic disease management programs to health promotion activities and caregiver support, including the application of technology to improve health outcomes. However, there was a notable lack of research on diverse populations and mental health interventions. The review also uncovered critical gaps in the literature, such as economic barriers to care access and the necessity for comprehensive policies addressing the aging population's diverse needs. In conclusion, this review emphasizes the importance of a multidimensional approach to effectively addressing older adults' health needs. While several effective interventions exist, there is an urgent need to tackle identified gaps, particularly concerning diverse populations and mental health, to enhance overall health strategies for the aging demographic.

The potential and translational application of infant genetic research.

In the current genomic revolution, the infancy life stage is the most neglected. Although clinical genetics recognizes the value of early identification in infancy of rare genetic causes of disorders and delay, common genetic variation is almost completely ignored in research on infant behavioral and neurodevelopmental traits. In this Perspective, we argue for a much-needed surge in research on common genetic variation influencing infant neurodevelopment and behavior, findings that would be relevant for all children. We now see convincing evidence from different research designs to suggest that developmental milestones, skills and behaviors of infants are heritable and thus are suitable candidates for gene-discovery research. We highlight the resources available to the field, including genotyped infant cohorts, and we outline, with recommendations, special considerations needed for infant data. Therefore, infant genetic research has the potential to impact basic science and to affect educational policy, public health and clinical practice.

Artificial Intelligence in Public Health: A Review Article

Background: Artificial intelligence (AI) is the simulation of human intelligence processes by machines, such as learning, reasoning, and problem-solving. AI has the potential to transform the field of public health, which is concerned with promoting and protecting the health of populations and preventing diseases. AI can help public health organizations perform their essential functions more efficiently, effectively, and equitably; AI can transform the public health field, but it also poses some challenges and risks that must be addressed carefully and responsibly. Objectives: This paper reviews the current and potential applications of AI in public health, discusses the opportunities and challenges of AI for public health, and provides recommendations for the ethical and responsible use of AI in public health. Results: AI can improve the speed and accuracy of diagnosis, screening, and treatment of various diseases and support disease surveillance, outbreak response, and health systems management. However, AI poses significant challenges and risks, such as ethical, legal, and social implications, data quality and security, algorithmic bias and fairness, and environmental impact. Conclusion: AI has the potential to revolutionize public health, but it comes with risks that must be addressed. Promoting digital literacy, establishing modern data governance frameworks, and investing in advanced data infrastructure and procedures are essential. Public health organizations must also train their workforce to collaborate with AI. By doing so, they can improve health outcomes, reduce health disparities, and advance public health science and practice.

Sociopolitically and Trauma-Informed Public Health Practice With Latinx Families: Conceptual Framework and Best Practices.

Public health practitioners working with Latinx families in the United States must consider the historical contexts of colonization and slavery that have created conditions of violence, displacement, and social and economic marginalization throughout Latin America. Although shared experiences of colonization, dispossession, and migration affect all Latinxs, diverse national histories and sociopolitical contexts, migration patterns, and intersecting identities (e.g., gender, social class, race) complicate efforts to develop a uniform approach to this heterogeneous population. We provide a critical analysis of (1) how past experiences contribute to collective trauma and motivate migration, and (2) how these experiences are replicated in the United States through immigration-related adversities that deprive and threaten children and families through marginalization, fear of detention and deportation, and family separation brought on by a parent's deportation. This knowledge is imperative to advance research, practice, and policymaking with US Latinx populations. We provide best practice recommendations for a sociopolitically and trauma- informed public health workforce interfacing with Latinxs in the United States. The Effectiveness of Imagery Rescripting Interventions for Military Veterans With Nightmares and Sleep Disturbances: A Systematic Review and Meta-Analysis.

Imagery rescripting (ImRs) interventions have been found effective in improving sleep outcomes, although research has mostly focused on civilian, rather than military, samples. The aim of this review was to estimate the overall effectiveness of ImRs interventions for military veterans on primary outcomes of nightmare frequency and sleep quality. A systematic search was conducted in CINAHL, MEDLINE, PsycArticles, PsycINFO, Psychology and Behavioural Sciences Collection and the PTSDpubs database and was completed on 1 November 2021. Randomised controlled trials, nonrandomised trials and pre-post studies of ImRs interventions in veterans with sleep disturbances or nightmares were included. The methodological quality of the studies was assessed using the Effective Public Health Practice Project (EPHPP) tool, and meta-analysis was performed using Stata. Nineteen articles from 15 empirical studies were included in the review, and data from the 15 studies (involving 658 participants) were included in the meta-analysis. Meta-analysis findings indicated that ImRs interventions are associated with significant positive changes from pretreatment to posttreatment for nightmare and sleep quality. Significantly greater improvements were found in ImRs interventions compared to control groups for sleep quality (Hedges' g = -0.65, 95% CI [-1.20, -0.10]) but not for nightmare frequency (Hedges' g = -0.10, 95% CI [-0.34, 0.14]). Overall, the meta-analysis included a relatively small number of studies with poor methodological quality and considerable heterogeneity; therefore, findings should be cautiously interpreted. Further research should focus on veteran participants with larger samples and from a broader range of sources to determine effectiveness more confidently.

Practical Year in Public Health: 10 Years of Experience at the Public Health Department in Frankfurt

Since the winter semester 2013/14, the Public Health Department in Frankfurt has been the first to offer an internship in public health to medical students in their practical year in Germany. In this study, we analyzed how students evaluated their practical experience and the logbook within clinical training at the Public Health Department. A rotation procedure was set up and a logbook with subject-specific learning objectives was developed. After the internship, students participated in an evaluation using a standardized questionnaire. The results of the last 10 years were evaluated quantitatively. The evaluation results of 15 medical students of the last 10 years showed a very high level of internship satisfaction. There was a high acceptance of the logbook regarding structure, learning objectives and skills. The internship at the Frankfurt Public Health Department provides the opportunity to be involved in daily activities of public health practice, gives an overview of learning objectives, and underlines good public health training.

The aim of this study is to analyze the key modifiable and non-modifiable risk factors associated with the development of lung cancer and to discuss existing prevention strategies. Materials and methods: A literature review was conducted using the "PubMed" database with the keywords "lung cancer", "lung cancer risk factors", "lung cancer prevention”. Results: Lung cancer risk factors can be divided into two groups: modifiable and non-modifiable. Modifiable risk factors include: smoking, radon, diet, chronic lung diseases, asbestos and diesel fumes. Non-modifiable risk factors include: age, race, genetic factors and gender. In primary prevention of lung cancer it is important to reduce exposure to risk factors, especially cigarette smoke. Screening using low-dose computed tomography plays a major role in secondary prevention. Conclusion: Lung cancer is one of the most frequent malignant tumors. Over the years, enormous progress has been made in the fight against lung cancer. Despite a large reduction in smoking prevalence, cigarette smoking remains the most important risk factor of lung cancer. Several other factors have been identified, with e.g. asbestosis exposure and air pollutants being among top-5 risk factors with the highest impact. Effective methods of primary and secondary prevention such as health campaigns and screening tests have led to a reduction in the incidence of lung cancer. Smoking cessation and low-dose chest CT screening were proven to be the most effective in the lung cancer prevention. However, lung cancer remains the most common cause of death in cancer patients so further research and effective and practical public health policy are crucial in decreasing incidence and mortality.
