Platelet-rich plasma does not influence magnetic resonance imaging findings of the gluteus muscles after total hip arthroplasty through the Hardinge approach.

Total hip arthroplasty through the Hardinge approach damages the hip abductor muscles. MRI can be used to assess adverse postoperative events. In this prospective randomized controlled trial, we evaluated MRI findings and whether platelet-rich plasma affected postoperative healing of the gluteal muscles (gluteus medius and minimus). Forty patients with hip osteoarthritis requiring treatment with total hip arthroplasty, aged between 60 and 76 years, were included. Patients were randomized into two groups: 19 patients in the platelet-rich plasma group and 21 in the placebo group. Platelet-rich plasma or placebo was injected into the gluteus medius tendon incision line during closure. Postoperative hip MRI and plain radiographs were taken 3 and 12 months after surgery. MRI showed fatty atrophy of the gluteal muscles in all 40 patients (100%), gluteal muscle tear in 11 patients (28%), and atrophy in 16 patients (40%) at both 3 and 12 months postoperatively. Fluid collections related to the operated hip joint were seen in 18 patients (45%) at 3 months and 13 patients (33%) at 12 months, heterotopic ossification formation in nine patients (23%) at 3 months, and 12 patients (31%) at 12 months. There were no significant differences in imaging findings between the two groups. MRI can be a valuable tool for evaluating postoperative healing after total hip arthroplasty. Fatty atrophy of the gluteal muscles was a common finding. Platelet-rich plasma injection into the gluteus medius tendon did not improve healing detected by MRI.

Clinical outcome and complications after simultaneous bilateral medial opening-wedge high tibial osteotomy

BackgroundThe literature provides limited evidence regarding postoperative outcomes and complications following simultaneous bilateral medial opening-wedge high tibial osteotomy (MOWHTO). This study aimed to investigate the clinical outcomes and complications associated with simultaneous bilateral MOWHTO. MethodsWe retrospectively evaluated 72 knees from 36 patients (mean age, 58.6 ± 8.2 years) who underwent simultaneous bilateral MOWHTOs between December 2011 and January 2021. Locking compression plates were used for simultaneous bilateral MOWHTOs. The Oxford Knee Score (OKS) was used to assess clinical outcomes at the last follow up. Complications (lateral hinge fracture (LHF), nonunion, and loss of correction) were evaluated using postoperative serial plain radiography and computed tomography (CT). ResultsAt the last follow up (range, 2.0–11.1 years), the mean OKS for the 72 knees was 36.8 ± 10.4. According to the OKS, 42 knees (58.3%) scored ’excellent’, 16 (22.2%) scored ‘good’, eight (11.1%) scored ‘moderate’, and six (8.3%) scored ‘poor’. LHFs occurred in 11 of the 72 knees (15.3%). Among these, five LHFs were identified on postoperative plain radiographs and CT scans (acute LHFs), whereas six fractures were identified only on follow up plain radiographs (delayed LHFs). Loss of correction occurred in one patient with delayed LHFs. The patient required an additional surgical treatment to achieve bone healing at the osteotomy site. ConclusionSimultaneous bilateral MOWHTO is a viable treatment option for patients with bilateral medial osteoarthritis of the knee joint. Serial plain radiographs at short-term intervals are recommended to detect delayed LHFs and prevent complications such as fixation failure and loss of correction.

Comparison of fusion rate, radiologic and clinical outcome between CaO-SiO2-P2O5-B2O3 bioactive glass-ceramics 7 (BGS-7) spacer and allograft spacer with iliac bone graft in multilevel ACDF.

CaO-SiO2-P2O5-B2O3 bioactive glass-ceramics7 (BGS-7) are known for their strong integration with bone and stability and are commonly used in spinal fusions. This study aimed to compare fusion rates and radiological and clinical outcomes between BGS-7 and allograft spacers with iliac bone grafts (IBG) in multilevel anterior cervical discectomy and fusion (ACDF) surgeries. This retrospective study was conducted at BRM Medical Center. We included patients who underwent multilevel ACDF at BRM Medical Center between January 2012 and December 2023. The patients had symptoms such as cervical radiculopathy and myelopathy due to cervical disc herniation, stenosis, and spondylosis. We evaluated the preoperative and postoperative Japanese Orthopedic Association (JOA) scores, neck disability index (NDI), functional rating index (FRI), and visual analog scale (VAS) scores for the neck, shoulder, and upper extremities at 6 months and 1 year after surgery. Fusion rates were assessed using dynamic radiography and computed tomography (CT) scans at 1 year postoperatively. Radiological measurements were obtained from preoperative and postoperative plain radiographs. At the 1-year follow-up, the fusion rates were 89.5% for BGS-7 and 92.2% for the allograft cage on dynamic radiographs (p=0.156) and 93.4% and 90.4%, respectively, on CT scans (p=0.319), confirming both internal and external osseointegration. Subsidence rates were 4% for BGS-7 and 10% for the allograft spacer group. Both groups showed increased cervical lordosis (CL), segmental lordosis (SL), and segmental height postoperatively, with maintained lower segmental height (LSH) in the BGS-7 group than in the allograft spacer group at postoperatively 1 year. No adjacent segmental disease (ASD) occurred in either group. The JOA, NDI, and FRI showed significant improvements in both groups. The VAS scores decreased significantly in both groups, indicating improved clinical outcomes. In multilevel ACDF, BGS-7 demonstrated fusion rates comparable to those of the allograft spacer with IBG, experiencing fewer instances of subsidence and cage fracture. Therefore, BGS-7 spacer can be safely utilized in multilevel ACDF as a substitute for traditional allograft spacers, without the need for additional IBG.

Change in Axial Rotation of the Talus Following Reconstruction of the Progressive Collapsing Foot Deformity

Category: Hindfoot; Other Introduction/Purpose: In addition to peri-talar subluxation of the foot, previous studies have demonstrated that the talus is more internally rotated with respect to the trans-malleolar axis of the ankle in PCFD patients compared with normal patients. However, no studies have investigated the change in rotation of the talus in PCFD postoperatively and its association with outcomes. The primary aim of this study was to investigate the preoperative to postoperative change in rotation of the talus following reconstruction of the flexible PCFD. Secondary aims were to determine whether changes in rotation of the talus were associated with radiographic measurements of PCFD or specific reconstructive procedures and whether postoperative talar rotation was associated with two-year patient-reported outcome scores. Methods: Thirty-four consecutive patients over the age of 18 years old who underwent reconstruction of a flexible PCFD with preoperative and at least 5-month postoperative weightbearing CT (WBCT) scans and plain radiographs and had preoperative and at least two-year postoperative PROMIS scores were included in the study. Patients were excluded if they underwent a talonavicular fusion. Talar axial rotation was the angle between the transmalleolar axis and talar axis on preoperative and postoperative WBCT scans as previously described by Kim et al (2023) with smaller angles representing more internal rotation. Hindfoot moment arm, Meary’s angle, and talonavicular angle were measured on plain radiographs. Due to small sample sizes, descriptive statistics are reported as median and interquartile ranges (IQRs). Wilcoxon signed rank and Mann Whiney U tests were used to investigate differences between groups. Spearman correlation coefficients were employed to detect associations between variables and reported using 95% confidence intervals (95% CIs). Results: Median preoperative and postoperative talar axial rotation were 39.9° (IQR 36.7°, 45.3°) and 49.7° (IQR 45.9°, 57.3°, P < 0.001), respectively. The talus externally rotated by a median of 8.3° (IQR 4.1°, 13.1°) following surgery. The preoperative to postoperative change in talar rotation was not associated with changes in any plain radiographic parameter (Table 1). Increasing external rotation of the talus was associated with an increase in postoperative PROMIS Pain Intensity (rho=0.38, 95% CI 0.00, 0.67) with similar trends in the PROMIS Physical Function (rho=-0.22, 95% CI -0.55, 0.18) and Pain Interference (rho=0.19, 95% CI -0.21, 0.53) domains (Figure 1). Lateral column lengthening and subtalar fusion procedures were not associated with changes in talar rotation (both P > 0.10). Conclusion: Reconstruction of the flexible PCFD distal to the talus results in external rotation of the talus within the ankle mortise. Talar rotation was not associated with common measures of deformity or specific procedures. Kim et al. found that control patients had approximately 40°-60° of talar axial rotation, which is similar to the corrected position of the talus in our study. However, increasing talar external rotation resulted in worse postoperative PROMIS Pain Intensity scores with a similar trend seen in other PROMIS domains and raises the possibility of overcorrecting the internal rotational deformity of the talus in flexible PCFD.

Paper 17: Does Higher Quality CAM Resection Correlate with Better Patient-Reported Outcomes?

Objectives: The relationships between CAM resection quality and patient-reported outcomes (PROs) are unknown in the literature. This study aimed to investigate the correlation of CAM resection quality with patient’s self-reported International Hip Outcome Tool (iHOT) scores collected annually post-surgery. Methods: A retrospective analysis was conducted including patients who underwent primary hip arthroscopy for femoroacetabular impingement (FAI) between 2012 and 2019. Inclusion criteria were pre-operative radiographic signs of CAM-type impingement (alpha angle > 55 °) and post-operative plain radiographs. Exclusion criteria were radiographic signs of a pincer or mixed impingement, history of femoral neck or proximal femur fracture, and treatment of concomitant hip pathologies. The following data were collected: demographic information, radiographic findings, and iHOT scores. CAM resection quality data were categorized based on findings from three plain radiographs (anteroposterior (AP) pelvis, frog leg lateral, and 45° Dunn views). High quality resections were defined as those post-operative alpha angle <55°. Results: This study included 148 participants who underwent CAM resection. From Frog leg views, 88% of patients had high-quality resections and presented superior post-operative iHOT scores compared to the 12% of patients with poor resection quality during the 1-year to 5-year follow-up period. Likewise, from Dunn views 82% of patients with a high-quality CAM resection displayed a similar trend in post-operative iHOT scores compared to 18% of patients with poor resection quality on Dunn views. However, this positive association between resection quality and post-operative iHOT scores was not evident in AP imaging. With AP views, 82% of patients with high-quality resections did not show a significant difference in iHOT scores compared to 18% of patients with poor resection quality. Conclusions: This study found that high quality CAM resection from arthroscopic surgery correlated with better iHOTs up to 5-years post-surgery from the Dunn and frog leg radiographic imaging views.

Closed reduction and internal fixation for tillaux fractures, based on pre-operative three-dimensional computed tomography.

This study aimed to propose treatment protocol and identify patterns of tillaux fractures using three-dimensional (3D) computed tomography (CT) analysis and to describe an effective reduction technique. Forty-two juvenile patients with tillaux fractures were evaluated with 3D-CT scan for fracture displacement pattern and received surgical treatment. Tillaux fragment was reduced by pushing the superomedial quadrant part of the fragment slightly downward towards the ankle joint from anterolateral to posteromedial through 5-mm skin incisions with mosquito forceps. A 4.0 cannulated screw was subsequently inserted from the anterolateral to the posteromedial side parallel to the ankle joint. We analysed the distance and direction of fracture displacement with 3D-CT before the surgery. Pre-operative and post-operative plain radiographs were evaluated. Pre-operative 3D-CT analysis revealed a common fracture pattern, varus tilt, and external rotation of fragment. We achieved satisfactory reduction with residual fracture gaps less than 2mm in 42 cases. Two cases had a 13-mm anterior gap that was reduced by mini-open reduction because of periosteal impingement. No significant clinical complications were found. The closed reduction technique developed based on the fracture pattern identified by 3D-CT anatomical analysis is safe and effective in treating tillaux fractures.

Posteroinferior glenosphere positioning is associated with improved range of motion following reverse shoulder arthroplasty with a 135° inlay humeral component and lateralized glenoid

BackgroundOptimal glenosphere positioning in a lateralized reverse shoulder arthroplasty (RSA) to maximize functional outcomes has yet to be clearly defined. Center of rotation (COR) measurements have largely relied on AP radiographs which allow assessment of lateralization and inferior position, but ignore scapular Y radiographs which may provide an assessment of posterior and inferior position relative to the acromion. The purpose of this study was to evaluate the COR in the sagittal plane and assess the effect of glenosphere positioning with functional outcomes utilizing a 135° inlay stem with a lateralized glenoid. MethodsA retrospective review was performed on a prospectively maintained multicenter database on patients who underwent primary RSA from 2015-2021 with a 135° inlay stem. The COR was measured on minimum 2-year postoperative sagittal plain radiographs using a perfect-circle fit method. A perfect circle was made on the glenosphere and the center was marked. From there, four measurements were made: 1) center to the inner cortex of the coracoid, 2) center to the inner cortex of the anterior acromion, 3) center to the inner cortex of the middle acromion, 4) center to the inner cortex of the posterior acromion. Regression analysis was performed to evaluate any association between the position of the COR relative to bony landmarks with functional outcomes. ResultsA total of 136 RSAs met the study criteria. There was no relation with any of the distances with outcome scores (ASES, VAS). In regards to range of motion (ROM), each distance had an effect on at least one parameter. The COR to coracoid distance had the broadest association with ROM with improvements in forward flexion (FF), external rotation (ER0), and internal rotation with arm at 90° (IR90) (p = <0.001, 0.031, <0.001; respectively). The COR to coracoid distance was also the only distance to affect the final FF and IR90. For every 1 mm increase in this distance, there was a 1.8° increase in FF and 1.5° increase in IR90 (ß = 1.78; 95% CI 0.85 – 2.72, p = <0.001, ß = 1.53; 95% CI 0.65 – 2.41, p = <0.001; respectively). ConclusionEvaluating the COR following RSA in the sagittal plane suggests that posteroinferior glenosphere position may improve ROM when using a 135° inlay humeral component and a lateralized glenoid.

Does the limb lengthening reduce the incidence of hip dislocation in patients with neurological disorders and insufficient muscle tension who undergoing hip arthroplasty?

The soft-tissue tension is closely associated with postoperative hip dislocation in patients undergoing total hip arthroplasty (THA), especially for those patients with neurological disorders and insufficient muscle tension. The aim of this study is to explore the effect of limb lengthening on the incidence of complications following THA in patients with neurological disorders and insufficient muscle tension. This retrospective analysis examines individuals with neurological disorders, such as ischemic stroke and poliomyelitis, who underwent primary total hip arthroplasty (THA) at our medical center between January 2015 and April 2021. Demographic and baseline characteristics (such as age, gender, muscle strength) were obtained from medical records. The limb length, offset and the positional parameters of both acetabular and femoral component were measured on pre- and postoperative plain radiograph. The primary outcome was the occurrence of hip dislocation. The secondary outcome included the incidence of other complications and the hip function (determined by Harris score). The correlation between the occurrence of hip dislocation and limb lengthening was analyzed. A total of 258 patients were finally analyzed. The hip dislocations were identified in 35 patients (overall incidence = 13.57%). The incidence of early dislocation was lower in patients whose limb-length discrepancy (LLD) was over 20 mm (incidence = 4.1% for LLD >20 mm, 12.2% for LLD 10 mm-20 mm and 17% for LLD <10 mm). The odds ratio (OR) was 0.206 and 95% confidence interval (CI) was 0.058-0.737 (compared between LLD <10 mm and LLD >20 mm). But the no difference was identified regarding on the incidence of late dislocation among patients with different LLD. Moreover, the overall incidence of other complications was elevated in patients with LLD >20 mm (incidence = 17.58% for LLD >20 mm, 11.11% for LLD 10 mm-20 mm and 3.19% for LLD <10 mm; OR = 6.464, 95% CI = 1.768-23.640). And the Harris scores, which reflected the hip function, was gradually decreased with the increasing in LLD. In terms of the relationship between the offset and dislocation rate, it was found that increased offset discrepancy was associated with decreased dislocation incidence (incidence = 4.71% for offset discrepancy >10 mm, 12.5% for offset discrepancy 5 mm-10 mm and 17.20% for offset discrepancy <5 mm; OR = 0.238, 95% CI = 0.076-0.742). Furthermore, increased offset discrepancy also bring a reduction in late dislocation. The incidences of late dislocation were 0%, 2.5% and 10.8% for offset discrepancy >10 mm, offset discrepancy 5 mm-10 mm and 17.20% for offset discrepancy respectively. Different from that of LDD, the incidences of other complications were similar among patients with different offset discrepancy. Besides, no influence of offset discrepancy on the hip function was identified in this study. Unfortunately, although increasing in limb length could partially reduce early dislocation postoperatively, it could not affect the incidence of late dislocation in those patients with neurological disorders and insufficient muscle tension. Moreover, over limb lengthening was associated with other postoperative complications and worse hip function. Instead, additional offset could reduce the probability of postoperative dislocation, without increasing the incidence of other complications. Therefore, femoral stem with lower cervico-diaphyseal angle (higher offset) should be recommended to patients with neurological disorders who were in high risk of postoperative dislocation. Isolated increasing in limb length should be avoided.

Multidrug-resistant tuberculosis of spine diagnosis and management: An institutional experience of 21 cases.

We aimed to establish a standardized protocol for managing multidrug-resistant (MDR) spinal tuberculosis (TB), addressing the surgical options, ranging from computed tomography-guided biopsy to intraoperative sampling. This study developed a treatment/management protocol based on an analysis of clinical, radiological, and postoperative outcomes for 21 patients with spinal MDR-TB. Over 24 months, 21 patients with multidrug-resistant spinal TB underwent the following testing: erythrocyte sedimentation rate (ESR), C-reactive protein (CRP), numerical rating scale (NRS), and the American Spinal Injury Association Scale. Radiological criteria were based upon a comparison of preoperative and 6-month to 2-year postoperative plain radiographs. The 21 patients underwent guided biopsies (35%) or intraoperative sampling (65%). For the surgical cases, dorsal vertebrae were most frequently involved (75%), and 90% underwent posterior surgical procedures. Postoperatively, ESR, CRP, kyphosis angle, and NRS score were significantly reduced. The 3 MDR patients who failed treatment were transitioned to the extensively drug-resistant (XDR) protocol wherein bedaquiline, linezolid, cycloserine, and clofazimine were given after drug sensitivity testing drug regimen, needed no further surgery, and none exhibited additional neurological deterioration. Regular clinical, laboratory, radiological, and outcome analysis is vital for following MDR spinal TB patients; early detection of relatively rare treatment failures (i.e., 3/21 patients in this series) allows for prompt initiation of XDR treatment, resulting in better outcomes.

MAGNITUDE OF FRACTURE NECK OF FEMUR IN PATIENTS WITH IPSILATERAL FRACTURE SHAFT OF FEMUR IN ADULT

Background: Magnitude of fracture neck of femur associated with ipsilateral fracture shaft of femur is rare and challenging in terms of diagnosis and management. Fracture neck of femur is associated with 0.8-9 % of ipsilateral fracture shaft of femur. Fracture neck of femur is often missed preoperatively with incidence ranging from 20-50%. This study was done with an objective of assessing the magnitude of fracture neck of femur in patients with ipsilateral fracture shaft of femur. Methods: This was descriptive study of retrospective data of 187 fracture shaft of femur. Data from all the patients with fracture shaft of femur in the age group of 17 to 70 years admitted and operated between September 2018 to September 2022 were analyzed with magnitude of ipsilateral fracture neck of femur diagnosed preoperatively or postoperatively as main variable. Descriptive statistics was used to calculate frequency and percentage. Results: Out of 187 fracture shaft of femur, 8.02% (n=15) cases were associated with ipsilateral fracture neck of femur. 73.33% (n=11) cases of fracture neck of femur were identified preoperatively on plain radiograph whereas diagnosis of 26.66% (n=4) cases of fracture neck of femur was missed on initial evaluation and diagnosed on postoperative plain radiograph. Conclusions: Current study showed the magnitude of fracture neck of femur as 8.02% associated with ipsilateral fracture shaft of femur. Careful clinical evaluation, proper radiography of hip, possible CT scan in doubtful cases and intraoperative fluoroscopic evaluation of hip may decrease the incidence of missed diagnosis.

Reprise d’une arthroplastie totale de la hanche chez les patients atteints de spondylarthrite ankylosante : résultats à moyen terme

BackgroundFew studies have reported on the outcome of revision total hip arthroplasty (THA) for ankylosing spondylitis (AS). Therefore, we sought to investigate the clinical and radiographic outcomes of revision THA in patients with AS. HypothesisThe mid-term outcome of revision THA in AS patients was comparable to conventional revision THA. Materials and methodsForty-one patients with AS who underwent revision THA (on a total of 51 hips) between April 2004 and June 2018 were included. The mean follow-up duration was 7.5±3.6 years (3.0–15.8 years). Clinical outcomes were assessed using the Bath Ankylosing Spondylitis Functional Index (BASFI), Harris Hip Score (HHS), and the Western Ontario and McMaster Universities Osteoarthritis (WOMAC) index. Complications were assessed at each follow-up. Radiological outcomes were examined by reviewing postoperative plain radiographs. ResultsSignificant improvements were seen in BASFI, HHS, and WOMAC scores (p<0.01). Postoperative combined range of motion (ROM) was 128.9±45.3° (range, 60–300°). Complications included heterotopic ossification, periprosthetic fractures, aseptic loosening, osteolysis, reinfection, residual pain or stiffness, delayed wound healing, numbness, deep vein thrombosis, leg length discrepancy, and posterior dislocation. Five (9.8%) of the 51 hips were treated with re-revision due to aseptic loosening, re-infection, or re-fracture. The incidence of dislocation was 1.9% at 7.5 years. Implant survival for all reasons was estimated to be 96.3% at 5 years, 85.9% at 10 years and 61.0% at 15 years. Forty-two (82.4%) hip revisions were rated “satisfactory” or “very satisfactory” by the patients. ConclusionsThe mid-term result of revision THA for patients with AS is comparable to conventional revisions in terms of implant survival, despite high complication rates. The benefit-risk ratio of these indications is acceptable but deserves specific information for these patients. Level of evidenceIV; case series.

Scoliosis in osteogenesis imperfecta: results of posterior spinal fusion in 39 patients.

To evaluate the outcomes of scoliosis corrective surgery in Osteogenesis Imperfecta (OI) patients with primarily pedicles screw fixation in terms of correcting and maintaining the correction of the spinal deformity, and to assess for several peri-operative parameters and complications associated with this surgical treatment. Retrospective case series of 39 consecutive patients with OI treated surgically for scoliosis. The surgeries were performed between 2002 and 2020 by three different surgeons. All patients' medical peri-operative and post-operative charts were evaluated. Radiological assessment was performed by evaluation of the pre-operative, immediate post-operative and last follow-up plain radiographs. There were 20 females and 19 males included in this review with a mean age of 14years (range, 6-20years) at the time of surgery. The median follow-up time was 7.9years. The mean pre-operative cobb angle (CA) of the major curve was 76.5 degrees (SD ± 18.9), decreasing to 42.6 (SD ± 17.4) in the long-term post-operative follow-up (P < 0.001). A total of 21 adverse events in 16 patients were noted. Only 4 patients required subsequent invasive surgical treatment or prolonged hospital stay. All other patients were treated conservatively with no lasting complication. Scoliosis surgical correction in OI patients seems to yield acceptable results, with maintained coronal plane surgical correction in the long-term follow-up. Even though a high peri-operative complications rate is observed in this series, there were no long-term sequelae or lasting complications. Level IV, Case series.

Early radiographic osseointegration of a novel highly porous 3D-printed titanium collar for megaprostheses compared to a previous generation smooth HA-coated collar

PurposeExtracortical osseointegration at the collar-bone interface of megaprostheses is associated with improved implant stability, lower rates of stem fracture and loosening. The use of hydroxy-apatite (HA-) coated collars showed mixed results in previously published reports. A novel collar system has recently become available utilizing additive manufacturing technology to create a highly porous titanium collar with a calcium-phosphate coated surface. The aim of this study was to evaluate our early experience with this novel collar and compare it to the previously used HA-coated model.MethodsTwenty patients who underwent megaprostheses implantation utilizing the novel collar system were case matched to 20 patients who had previously undergone a HA-coated collar. A minimum radiological follow-up of three months was available in all included patients. Osseointegration was evaluated using postoperative plain radiographs in two planes based on a previously published semi-quantitative score.ResultsCompared to the HA-coated collar the use of the novel highly porous collar was associated with a higher proportion of cases demonstrating osseointegration at the bone-collar interface (80% vs. 65%). Application of the highly porous collar led to a significantly shortened time to reach the final ongrowth score (173 ± 89 days vs. 299 ± 165 days, p < 0.05). At one year follow-up, 90% of the novel collars had reached their final osseoingration grade compared to 50% in the HA-coated collar group (p < 0.001). Radiological osseointegration was seen in 71% for highly porous collars where the indication was revision arthroplasty, compared to 27% in reported in the literature. ConclusionThese results indicate more reliable and accelerated osseointegration at the bone-collar interface of a novel highly porous collar system compared to a previously used HA-coated collar. Further studies are warranted to confirm these findings.

Revision total hip arthroplasty in patients with ankylosing spondylitis: Mid-term results

BackgroundFew studies have reported on the outcome of revision total hip arthroplasty (THA) for ankylosing spondylitis (AS). Therefore, we sought to investigate the clinical and radiographic outcomes of revision THA in patients with AS. HypothesisThe mid-term outcome of revision THA in AS patients was comparable to conventional revision THA. Materials and methodsForty-one patients with AS who underwent revision THA (on a total of 51 hips) between April 2004 and June 2018 were included. The mean follow-up duration was 7.5±3.6 years (3.0–15.8 years). Clinical outcomes were assessed using the Bath Ankylosing Spondylitis Functional Index (BASFI), Harris Hip Score (HHS), and the Western Ontario and McMaster Universities Osteoarthritis (WOMAC) index. Complications were assessed at each follow-up. Radiological outcomes were examined by reviewing postoperative plain radiographs. ResultsSignificant improvements were seen in BASFI, HHS, and WOMAC scores (p<0.01). Postoperative combined range of motion (ROM) was 128.9±45.3° (range, 60–300°). Complications included heterotopic ossification, periprosthetic fractures, aseptic loosening, osteolysis, reinfection, residual pain or stiffness, delayed wound healing, numbness, deep vein thrombosis, leg length discrepancy, and posterior dislocation. Five (9.8%) of the 51 hips were treated with re-revision due to aseptic loosening, re-infection, or re-fracture. The incidence of dislocation was 1.9% at 7.5 years. Implant survival for all reasons was estimated to be 96.3% at 5 years, 85.9% at 10 years and 61.0% at 15 years. Forty-two (82.4%) hip revisions were rated “satisfactory” or “very satisfactory” by the patients. ConclusionsThe mid-term result of revision THA for patients with AS is comparable to conventional revisions in terms of implant survival, despite high complication rates. The benefit-risk ratio of these indications is acceptable but deserves specific information for these patients. Level of evidenceIV; case series.

Predictive Factors for Lateral Hinge Fracture in Medial Closing Wedge Distal Femoral Osteotomy

Background: Recent studies have reported that lateral hinge fracture (LHF) has a negative effect on bone healing at the osteotomy site after medial closing wedge distal femoral osteotomy (MCDFO). However, limited evidence exists in the literature regarding the predictive factors for LHF in MCDFO. Hypothesis: A large medial closing gap and a lateral hinge position in the supracondylar area would increase plain radiography–based and/or computed tomography (CT)–based LHF in MCDFO. Study Design: Case control study; Level of evidence, 3. Methods: We retrospectively evaluated 67 knees of 53 patients (mean age, 37.4 ± 16.9 years) who underwent MCDFO between May 2009 and June 2021. The surgical indications for MCDFO were genu valgum deformity combined with either lateral compartment osteoarthritis or recurrent patellar dislocation. The presence of LHF was evaluated based on immediate postoperative plain radiography and CT scans. The predictive factors for LHF in MCDFO were investigated using multivariate logistic regression analysis. Results: LHFs were identified in 21 knees (31.3%) through plain radiography and in 40 knees (59.7%) through CT. Multivariate logistic regression analysis showed that the medial closing gap and lateral hinge position were predictive factors for plain radiography– and CT-based LHF after MCDFO. Controlling for other variables, we found that an increase in the medial opening gap by 1 mm increased the likelihood of plain radiography–based LHF by a factor of 1.805 (95% CI, 1.291-2.525; P = .001) and CT-based LHF by 1.333 (95% CI, 1.003-1.772; P = .048). Moreover, a lateral hinge position in the supracondylar area increased the likelihood of plain radiography–based LHF by a factor of 9.870 (95% CI, 2.179-44.720; P = .003) and CT-based LHF by 5.686 (95% CI, 1.124-28.754; P = .036). Conclusion: A large medial closing gap and lateral hinge position in the supracondylar area are associated with LHF in MCDFO. Care should be taken to prevent LHF in MCDFO with a large medial closing gap. Moreover, a lateral hinge position in the supracondylar area should be avoided to decrease the incidence of LHF in MCDFO.

Microscopic Unilateral Approach for Bilateral Decompression of Lumbar Spinal Stenosis.

This is a study based on single-surgeon data on spinal stenosis surgery via microscopic approach. The aim is to evaluate the effectiveness of the unilateral approach to bilateral decompression and the usage of Taylor retractors and brain spatula in patients with spinal stenosis. This is a retrospective study on bilateral decompression for lumbar spinal stenosis using a microscopic unilateral approach by a single surgeon, between April 2015 and March 2018. In total, 50 patients were operated due to single level lumbar spinal stenosis. All patients were evaluated by preoperative and postoperative plain radiographs and magnetic resonance (MR) images. Walking distance (WD), visual analog scale (VAS) for pain and Odom's criteria were evaluated for follow-up. One level of the lumbar spine was surgically decompressed in all patients. The median age of patients was 64.6 (51- 82). Of the patients, 72% (36) were women, and 28% (14) were men. Most patients had refractory low back pain (96%) after conservative treatment. The stenotic levels of the cases were as follows: L3-4, 23(46%); L4-5, 24(48%); and L5-S1, 3 (6%). VAS scores decreased in all patients after surgery. According to Odom's criteria, an excellent or good score was found in 43 patients at the 12th follow-up examination. WDs increased up to 1000 meters for 41 patients. The microscopic unilateral approach to bilateral decompression is an effective method for decompression in spinal stenosis. Via this approach, surgical trauma is reduced and surgically induced instability is avoided as much as possible.

Degree of Pelvic Rotation in the Coronal Plane on Postoperative Radiographs Obtained after Total Hip Arthroplasty.

There are many published cup anteversion measurements for postoperative total hip arthroplasty (THA), including Liaw's, Lewinnek's, and Murray's methods. However, most measurements ignore the potential pelvic rotation on radiographs except in Liaw's method. Without considering pelvic rotation, clinicians can miscalculate cup anteversion. Therefore, we aimed to quantify the mean degree of pelvic rotation. Herein, we collected 388 radiographs of 98 postoperative THA hips of 77 patients and measured pelvic rotation by measuring h, the horizontal displacement of the sacrococcygeal junction associated with the upper pole of the symphysis pubis, and ssd, the distance between the sacrococcygeal junction and pubic symphysis. The angle θ of pelvic rotation was defined as θ = arc sin (h/ssd) × (180°/π). The mean degree of pelvic rotation was then calculated. The standard deviation of h was 7.84 mm, and the mean ssd was 158 mm. The potential pelvic rotation was 2.50°. The p-values from the paired t-test were all &gt;0.05 when interobserver and intraobserver errors were assessed. This is the first study to quantify the potential pelvic rotation in the coronal plane on postoperative plain radiographs. The potential pelvic rotation was too large to be neglected during the measurement of cup anteversion.

Does a 1st Metatarsophalangeal Fusion in a Patient with Pes Planus Improve the Pes Planus Deformity? A Retrospective Case Series

Category: Midfoot/Forefoot; Bunion Introduction/Purpose: A pes planus deformity is a pathology relating to the loss of normality of the complex interaction of the bones and joints of the foot resulting in a flattening of the medial longitudinal arch with or without abduction of the midfoot. The distal aspect of the medial longitudinal arch can be a feature of the deformity. The fusion of the 1st metatarsophalangeal joint (MTPJ) is a common operation. In patients with concomitant pes planus deformity, it is unknown if reducing and stabilising the distal aspect of the medial ray through a 1st MTPJ fusion can subsequently improve the pes planus deformity. We sought to analyse the pes planus deformity pre and post 1st MTPJ fusion with the null hypothesis that there was no difference. Methods: We identified patients who had undergone 1st MTPJ fusion using our electronic database from January 2011 to October 2021. To meet the inclusion criteria, cases required pre- and post-operative weightbearing plain radiographs, with a Meary's angle of &gt;4 degrees on pre-operative imaging. Routine pes planus measurements were undertaken by 2 independent observers. Pre- and post-operative measurements were tested for significant change using Wilcoxon Signed Rank Test or Paired T-Test depending on the normality testing of the data. Routine demographic data was also collected. Data was analysed using IBM SPSS v.27. Results: Following radiographic analysis, 25 feet fitted the inclusion criteria. The mean patient age was 63 years. There was no pre-operative correlation between Meary's angle and the hallux valgus angle (Kendall's tau p=.441). Pre-operatively, Meary's line broke at the talo-navicular and navicular-cuneiform joints in 12 cases each; the remaining case broke at the tarsal-metatarsal joint. A statistically significant improvement between the pre- and post-operative measurements was identified in Meary's angle, calcaneal pitch angle, talo-navicular coverage angle, intermetatarsal angle and talo-navicular angle (see table). Significant changes were also noted in the medial cuneiform height (mean increase of 2.74mm; p=.022). A post-operative Meary's angle of &lt;4°was only achieved in 4/25 (16%) of cases. Of the remaining 21 cases, the point at which Meary's line broke moved proximally one joint in 4 and distally one joint in 3 cases; it remained at the same joint in 14 cases. Conclusion: Our results suggest that 1st MTPJ fusion improves axial alignment of the foot as well as reducing Meary's angle by a statistically significant amount, although diagnostic criteria for pes planus remained in most cases.

Reduction in Offset Is Associated With Worse Functional Outcomes Following Total Hip Arthroplasty

BackgroundConflicting reports exist about the effect of offset variation on functional outcomes following total hip arthroplasty. Reproducing native hip offset is thought to optimize function by restoring biomechanics and appropriately tensioning the hip abductor muscles. The aim of this study is to assess the effect of failing to restore global hip offset in comparison to the native contralateral hip. MethodsA retrospective analysis of a prospective patient cohort was performed on patients undergoing an elective primary total hip arthroplasty. A total of 414 patients who had a minimum of 12 months of follow-up were included. Postoperative plain radiographs were analyzed for offset and compared to the contralateral native hip. Western Ontario and McMaster Universities Arthritis Index (WOMAC) and Veterans RAND 12 (VR-12) scores were assessed preoperatively and at 12 months postoperatively. ResultsRegression analyses indicated that a reduction in offset of >20 mm resulted in worse WOMAC pain (P = .005) and motion (P = .015) scores compared to those with maintained offset. WOMAC function (P = .063), global (P = .025), and VR-12 scores were not affected (physical P = .656; mental P = .815). Reduction in offset up to 20 mm and increased offset were not significantly associated with patient-reported outcome measures (P-values ranged from .102 to .995). ConclusionThis study demonstrated an association between reduction in offset by >20 mm and worse WOMAC pain and motion scores following total hip arthroplasty. Surgeons should avoid decreases in offset >20 mm in order to optimize functional outcomes.

Delayed Repair of a Lesser Tuberosity Avulsion Fracture in an Adolescent 3 Years After Initial Injury.

This case study examines an adolescent athlete who had an avulsion fracture of the lesser tuberosity of the humerus. This is a relatively rare injury, although a collection of case studies have identified an increase in this type of injury in the past 15 years. Uniquely, the patient experienced a delay in appropriate diagnosis for approximately 3 years from the time of his initial injury. This is one of the most significant delays in diagnosis and treatment in the current body of literature regarding lesser tuberosity fractures. The authors include a detailed series of imaging studies, including preoperative plain radiographs, preoperative computed tomography, intraoperative arthroscopic images, and postoperative plain radiographs. In addition, a thorough description of the patient's surgery is presented. The degree of scarring to the axillary nerve present in this patient required conversion to a full open reduction. The authors' aim is that this case can be used as a reference for future surgical decision making, particularly in pediatric patients whose injuries are highly chronic or who are actively involved in athletic physical training programs.
