Fully Digital Workflow for Planning Static Guided Implant Surgery: A Prospective Accuracy Study.

The accuracy of static guided implant surgery (sGIS) using conventional planning workflow has been extensively examined; however, more information is required to justify the application of fully digital planning protocol. The purpose of this study was to investigate the clinical accuracy of sGIS with a fully digital planning workflow. Twenty-one partially edentulous patients were enrolled in this prospective study. Cone-beam computed tomography (CBCT) and intraoral scans were taken and superimposed by matching the dental surface images directly (surface registration protocol) or by matching fiducial markers on a stereolithographic (SLA) radiographic template fabricated from the digital data of the intraoral scan (fiducial marker registration protocol). Virtual implant treatment plans were then determined, and tooth-supported SLA surgical guides were fabricated according to the plans. Twenty-six implant surgeries were performed via the surgical guide by one surgeon. Pre- and post-operative CBCT images were superimposed, and the positional and angular deviations between placed and planned implants were measured with metrology software. A total of 43 fully guided implants were placed, in which 25 implants were planned with the surface registration protocol. Implants planned based on the surface registration protocol had a larger mean angular deviation than the fiducial marker registration protocol. No significant differences were found for any deviations of the examined variables. Within the limits of this study, we concluded that the clinical accuracy of the sGIS planned with a fully digital workflow was consistent with the conventional workflow for partially edentulous patients.

Biomechanical study on the changes of stress in temporomandibular joints after the orthognathic surgery in patients with mandibular prognathism: a 3D finite element study

This study aimed to analyze the changes of the stress distributions in TMJs for the pre- and postoperative patients with mandibular prognathism under unilateral occlusions, a frequent occlusion in mastication. Pre- and six-mouth postoperative cone-beam computed tomography images of thirteen patients diagnosed with mandibular prognathism were scanned and used to construct complete maxillofacial models, assigned as the Pre and Post group, respectively. Another ten asymptomatic individuals were defined as the Control group. The inhomogeneous properties were assigned to the models. The muscle forces and boundary conditions corresponding to left and right unilateral occlusions were applied on the models. The analysis of variation (ANOVA) was chosen for the comparison among the groups. The results showed that the Pre group had abnormal stress distributions ang higher stress level in TMJs, compared with those of the Post and Control groups. Moreover, from clinical cases, symptoms of temporomandibular disorders (TMDs) always followed with increased stresses. Generally, orthognathic surgeries could improve the stress distribution in TMJs of the patients with mandibular prognathism under the unilateral occlusions. However, the postoperative complications, especially symptoms of TMD, were closely related to changes of stress for patients with mandibular prognathism after orthognathic surgeries. Individual virtual surgery and finite element analysis should be conducted to prevent complications in TMJ.

Targeted endodontic microsurgery and endodontic microsurgery: a surgical simulation comparison.

To compare surgical time, bevel angle and site volumetric profiles of osteotomy and resection accomplished by targeted endodontic microsurgery (TEMS) and traditional endodontic microsurgery (EMS) in a surgical simulation model. An 80x80-mm cone beam computed tomography (CBCT) file was imported into Mimics software where artificial periapical lesions were created encompassing twelve root apices. Maxillary and mandibular models were 3D-printed. TEMS surgical guides were designed and 3D-printed for each surgical site. Three board-certified endodontists used the original CBCT to plan and perform EMS on models of six maxillary and six mandibular teeth. Next, the same endodontists performed TEMS on duplicate 3D-printed models for the same teeth. All surgeries were timed. Postoperative CBCT images of experimental models were made and imported into Amira software for measurement of bevel angle and site volumetric profiles. Paired t-tests compared the mean differences between EMS and TEMS groups. A Bonferroni correction determined data to be significant at P<0.004. TEMS significantly reduced surgical time (P<0.00001), had bevel angles more closely approaching zero degrees (P<0.01) and had significantly less volume of over-resection (P<0.001) and length of root resection (P<0.01). In this surgical simulation scenario, TEMS provided more efficient completion of osteotomy and resection, with a more appropriate root-end resection volume and bevel angle.

Accuracy and precision of 3D-printed implant surgical guides with different implant systems: An in vitro study

Statement of problemImplant guided surgery systems promise implant placement accuracy and precision beyond straightforward nonguided surgery. Recently introduced in-office stereolithography systems allow clinicians to produce implant surgical guides themselves. However, different implant designs and osteotomy preparation protocols may produce accuracy and precision differences among the different implant systems. PurposeThe purpose of this in vitro study was to measure the accuracy and precision of 3 implant systems, Tapered Internal implant system (BioHorizons) (BH), NobelReplace Conical (Nobel Biocare) (NB), and Tapered Screw-Vent (Zimmer Biomet) (ZB) when in-office fabricated surgical guides were used. Material and methodsA cone beam computed tomography (CBCT) data set of an unidentified patient missing a maxillary right central incisor and intraoral scans of the same patient were used as a model. A software program (3Shape Implant Studio) was used to plan the implant treatment with the 3 implant systems. Three implant surgical guides were fabricated by using a 3D printer (Form 2), and 30 casts were printed. A total of 10 implants for each system were placed in the dental casts by using the manufacturer's recommended guided surgery protocols. After implant placement, postoperative CBCT images were made. The CBCT cast and implant images were superimposed onto the treatment-planning image. The implant positions, mesiodistal, labiopalatal, and vertical, as well as implant angulations were measured in the labiolingual and mesiodistal planes. The displacements from the planning in each dimension were recorded. ANOVA with the Tukey adjusted post hoc pairwise comparisons were used to examine the accuracy and precision of the 3 implant systems (α=.05). ResultsThe overall implant displacements were −0.02 ±0.13 mm mesially (M), 0.07 ±0.14 mm distally (D), 0.43 ±0.57 mm labially (L), and 1.26 ±0.80 mm palatally (P); 1.20 ±3.01 mm vertically in the mesiodistal dimension (VMD); 0.69 ±2.03 mm vertically in the labiopalatal dimension (VLP); 1.69 ±1.02 degrees in mesiodistal angulation (AMD); and 1.56 ±0.92 degrees in labiopalatal angulation (ALP). Statistically significant differences (ANOVA) were found in M (P=.026), P (P=.001), VMD (P=.009), AMD (P=.001), and ALP (P=.001). ZB showed the most displacements in the M and vertical dimensions and the least displacements in the P angulation (P<.05), suggesting statistically significant differences among the M, VMD, VLP, AMD, and ALP. NB had the most M variation. ZB had the least P deviation. NB had the fewest vertical dimension variations but the most angulation variations. ConclusionsDimensional and angulation displacements of guided implant systems by in-office 3D-printed fabrication were within clinically acceptable limits: <0.1 mm in M-D, 0.5 to 1 mm in L-P, and 1 to 2 degrees in angulation. However, the vertical displacement can be as much as 2 to 3 mm. Different implant guided surgery systems have strengths and weaknesses as revealed in the dimensional and angulation implant displacements.

Scalar localisation of peri-modiolar electrodes and speech perception outcomes.

To identify the intracochlear electrode position in cochlear implant recipients and determine the correlation to speech perception for two peri-modiolar electrode arrays. Post-operative cone-beam computed tomography images of 92 adult recipients of the 'CI512' electrode and 18 adult recipients of the 'CI532' electrode were analysed. Phonemes scores were recorded pre-implantation, and at 3 and 12 months post-implantation. All CI532 electrodes were wholly within scala tympani. Of the 79 CI512 electrodes intended to be in scala tympani, 58 (73 per cent) were in scala tympani, 14 (17 per cent) were translocated and 7 (9 per cent) were wholly in scala vestibuli. Thirteen CI512 electrodes were deliberately inserted into scala vestibuli. Speech perception scores for post-lingual recipients were higher in the scala tympani group (69.1 per cent) compared with the scala vestibuli (54.2 per cent) and translocation (50 per cent) groups (p < 0.05). Electrode location outside of scala tympani independently resulted in a 10.5 per cent decrease in phoneme scores. Cone-beam computed tomography was valuable for demonstrating electrode position. The rate of scala tympani insertion was higher in CI532 than in CI512 electrodes. Scala vestibuli insertion and translocation were associated with poorer speech perception outcomes.

Three-dimensional region-based study on the relationship between soft and hard tissue changes after orthognathic surgery in patients with prognathism.

Both deep understanding and reliable prediction of postoperative soft tissue changes are crucial for planning orthognathic surgery. Instead of estimating soft tissue responses by measuring individual landmark changes, this study aimed to investigate the relationship (ratio) between soft and hard tissue movements in different facial regions through three-dimensional cone-beam computed tomography (CBCT). Preoperative and postoperative CBCT images were superimposed using the surface registration method on the basis of the cranial base, and 10 facial regions of interest were defined. Region-based volumetric subtractions between the preoperative and postoperative segments were performed. The volumetric differences and surface of each region were used to estimate the average movement. Correlation and regression analyses were performed to examine the relationships between the corresponding soft and hard tissue movements. An overall pattern of facial soft tissue movement was observed in patients with prognathism who underwent orthognathic surgery. The experiment results have shown that mean ratios for the average soft-to-hard tissue movements in the facial regions varied, which may not exactly be similar to the published reports because of the population biocharacteristics and study methods, but the trend is in agreement with the previous studies. Additionally, the prediction capability of the regression model was significantly high, ranging from 0.786 to 0.857, in upper lip, upper vermilion, and chin regions, thus demonstrating that the skin outline changes in these critical regions could be reliably predicted from the underlying bone movements. These results could likely be applied in future soft tissue simulation in orthognathic surgery.

Palatal orthodontic miniscrew insertion using a CAD-CAM surgical guide: description of a technique

The aim of this report was to describe a new computer-guided technique for a controlled site preparation and palatal orthodontic miniscrew insertion using a dedicated software. A surgical guide was designed after planning the appropriate insertion sites on three-dimensional images created by the fusion of cone-beam computed tomography (CBCT) and digital dental model images. Pre- and postoperative CBCT images were compared and the angular, coronal, and apical deviations between the planned and the placed miniscrews were calculated. The mean coronal and apical deviations were 1.38mm (range: 3.48–0.15mm; standard deviation (SD): 0.65) and 1.73mm (range: 5.41–0.10mm; SD: 1.03), respectively, while the mean angular deviation was 4.60° (range: 15.23–0.54°; SD: 2.54). The present surgical guide allows a controlled and accurate palatal miniscrew placement in three dimensions.

Pooled analysis of 1-year recall data from three root canal treatment outcome studies undertaken using cone beam computed tomography.

To provide an estimate of the proportion of successful outcomes of primary and secondary root canal treatments (retreatments) determined by periapical radiographs and cone beam computed tomography (CBCT), a pooled analysis of the data collected from three previous prospective clinical outcome studies was undertaken. The analysis pooled the 1-year results for 354 teeth, including 123 primary treatments and 231 retreatments. All root canals were instrumented with ProTaper Universal and filled using a warm vertical condensation technique. Comparisons of favourable results between root canal treatments and retreatments and between different tooth types were made using chi-square/Fisher's exact test. The overall percentage of favourable results was 91% using periapical radiographs and 80% for CBCT (P<0.001). With CBCT, the percentage of favourable results for primary treatments (84.7%) was not significantly different (P=0.316) from that of retreatments (77.9%). When assessed by tooth group, the overall percentage of favourable results with CBCT was 75.5%, 90.6% and 91.1% for molar, premolar and anterior teeth, respectively. When CBCT is used to assess the outcome, the proportion of favourable outcomes in molars was significantly lower than that of premolars and anterior teeth (P<0.05). Teeth with root fillings terminating more than 2mm short of the radiographic apex had less favourable outcomes (73%) compared to long (83%) and adequate root filling length (84%). The proportions of favourable outcomes of primary root canal treatments and retreatments assessed with CBCT were lower when compared to periapical radiographs, and also lower than those historically reported by periapical radiograph-based outcome studies. Considering the very high favourable outcome of anterior teeth and premolars compared to molar teeth, future studies assessing the effect of new materials and techniques on the outcome of root canal treatments should be based on preoperative and postoperative CBCT images, and focus on molar teeth.

Are Biodegradable Plates Applicable in Endoscope-Assisted Open Reduction and Internal Fixation of Mandibular Subcondyle Fractures?

To investigate whether biodegradable plates are applicable in endoscope-assisted open reduction and internal fixation (EAORIF) of mandibular subcondyle fractures. This retrospective case-series study included patients with mandibular subcondyle fractures treated with EAORIF using an unsintered hydroxyapatite particles/poly-l-lactide biodegradable plate system, with at least 6months of clinical follow-up data available. The outcome variables were fracture healing with postoperative stability and postoperative complications. Other variables included age, gender, fracture site, cause of injury, accompanying mandibular fracture, total follow-up period, fracture classification, extent of displacement, preoperative status of occlusion, preoperative mandibular movements, fixation materials in accompanying mandibular fracture, location and number of fixation plates, periods of intermaxillary fixation/elastic bands, and postoperative mandibular movements. Fracture healing in these patients was assessed by comparing the immediate postoperative cone-beam computed tomography (CBCT) images with those obtained at least 3months after surgery. A total of 11 patients, 9 male and 2 female, with a mean±standard deviation age of 35.3±15.9years, were included. The mean follow-up period was 18.8±7.8months. Four patients had an accompanying mandibular fracture. Two 4-hole, 2.0-mm biodegradable plates were fixed with 6-mm screws along the posterior border of the mandibular ramus and near the sigmoid notch. Complete bone formation around the fracture lines or fading of the fracture lines, with no change in the position of the fractured segments, was observed on the postoperative CBCT images at 3months. With the exception of 2 patients, no patient complained of plate palpability, deviation in occlusion, or discomfort during the postoperative follow-up period. EAORIF using biodegradable plates for mandible subcondylar fractures is a stable and reliable method, with considerable advantages compared with titanium plates.

Guidelines for Selecting the Implant Diameter During Immediate Implant Placement of a Fresh Extraction Socket: A Case Series.

This prospective case series proposes a method for selecting the diameter of immediate postextraction anterior implants and guiding the placement position of the implant based on the buccolingual dimension with the goal of preserving the buccal bone wall. The socket buccolingual distance was measured to determine the appropriate implant diameter, considering a 3-mm gap to the buccal wall. Pre- and postoperative cone-beam computed tomography images were compared to evaluate the buccal bone. The socket width measurements were not significantly different (P = .931). The mean widths of the postoperative buccal bone were 3.01 ± 0.18 mm, 2.92 ± 0.38 mm, and 2.83 ± 0.42 mm for the crestal bone at the implant platform and at 2 and 4 mm apically, respectively, after 35 months of follow-up. This new diameter selection method for implants demonstrated predictable buccal plate preservation.

Value of intra- and post-operative cone beam computed tomography (CBCT) for positioning control of a sphenopalatine ganglion neurostimulator in patients with chronic cluster headache

IntroductionThe objective of this study was to determine whether postoperative control of the neurostimulator placement within the pterygopalatine fossa (PPF) by means of 3-dimensional (3D) cone beam computed tomography (CBCT) was of therapeutic relevance compared to intraoperative CBCT imaging alone. Material and methodsImmediately after implantation of the sphenopalatine ganglion (SPG) neurostimulator, intraoperative CBCT datasets were generated in order to visualize the position of the probe within the PPF. Postoperatively, all patients received a CBCT for comparison with intraoperatively acquired radiographs. ResultsTwenty-four patients with cluster headache (CH) received an SPG neurostimulator. In 4 patients, postoperative CBCT images detected misplacement not found in intraoperative CBCT. In 3 cases, electrode tips were misplaced into the maxillary sinus and in 1 case into the apex of the PPF superior to the suspected location of the SPG. Immediate revision with successful repositioning within 3 days was done in 2 patients and a deferred reimplantation in 1 patient within 6 months. One patient declined revision. ConclusionWe were able to demonstrate the clinical value of postoperative dental CBCT imaging with a wide region of interest (ROI) due to a superior image quality compared with that achieved with intraoperative medical CBCT. Although intraoperative 3D CBCT imaging of electrode placement is helpful in the acute surgical setting, resolution is, at present, too low to safely exclude misplacement, especially in the maxillary sinus. High-resolution postoperative dental CBCT allows rapid detection and revision of electrode misplacement, thereby avoiding readmission and recurrent tissue trauma.

Cone-Beam Computed Tomography Comparison of Canal Transportation after Preparation with BioRaCe and Mtwo Rotary Instruments and Hand K-Flexofiles.

The aim of this in vitro study was to evaluate the transportation of mesiobuccal canals of mandibular first molars prepared with either BioRaCe or Mtwo rotary instruments or hand K-Flexofile, by means of cone-beam computed tomography (CBCT). Forty-five mandibular molars were selected and randomly divided into three groups (n=15). Mesiobuccal roots of these teeth were prepared by BioRaCe, Mtwo, or hand K-Flexofile. Transportation was measured by pre- and post-operative CBCT images. Two-way ANOVA analysis was applied to detect any differences between the groups followed by the post hoc Tukey's tests. The level of significance was set at 0.05. The non-parametric Friedman test was used to compare the behavior of each file at 3-, 6- and 9-mm levels. There were no significant differences between different levels in Mtwo group (P=0.15); however, the differences in K-Flexofile and BioRaCe groups were significant (P>0.05). The post hoc Tukey's test revealed significant differences between BioRaCe and K-Flexofile and also between Mtwo and K-Flexofile, both in the 3-mm depths (P<0.05). Under the limitations of the present study, BioRaCe and Mtwo rotary instruments are considered suitable for canal preparation to greater apical sizes provided that the recommended sequences are observed.

The Efficacy of FlexMaster's IntroFile, PreRaCe and Gates Glidden Drills in Straight-Line Access: A CBCT Assessment.

An overlooked but important part of successful root canal treatment is a straight-line access (SLA). The purpose of this in vitro study was to compare the efficacy of IntroFile and PreRaCe rotary instruments with Gates Glidden (GG) drills in gaining SLA by cone-beam computed tomography (CBCT). A total of forty five extracted mandibular first molars were selected and mounted in dental like arches. Subsequently, they were randomly classified into three groups (n=15). After preparation of a standard access cavity, orifices of the mesiobuccal canal was reached and a #10 file was inserted to explore the canals until the file tip was visible at the apex. Then, preoperative CBCT images were taken. SLA was gained in three groups; group 1, FlexMaster's IntroFile (FM); group 2, PreRaCe (RC) and group 3, GG. Again, the first binding file at the working length (WL) was placed in the canal and postoperative CBCT images in similar positions were taken. The pre/post operative morphology of the canal was evaluated for changes. Data was analyzed using the one-way ANOVA and post-hoc Bonferroni analysis. The average amount of reduction in coronal canal curvature in FM, RC and GG groups was 2.43±1.79, 3.17±2.05 and 8.7±3.45, respectively. This descending trend was statistically significant. The difference between pre/post SLA changes in FM and RC groups was significant compared to GG group, while there were no significant differences between RC and FM. GG drills produced extraordinary results in reducing coronal curvature of the canal and achieving SLA. They are also more effective than nickel-titanium (NiTi) rotary instruments in canals with coronal curvature.

Three-dimensional surgical simulation

In this article, we discuss the development of methods for computer-aided jaw surgery, which allows us to incorporate the high level of precision necessary for transferring virtual plans into the operating room. We also present a complete computer-aided surgery system developed in close collaboration with surgeons. Surgery planning and simulation include construction of 3-dimensional surface models from cone-beam computed tomography, dynamic cephalometry, semiautomatic mirroring, interactive cutting of bone, and bony segment repositioning. A virtual setup can be used to manufacture positioning splints for intraoperative guidance. The system provides further intraoperative assistance with a computer display showing jaw positions and 3-dimensional positioning guides updated in real time during the surgical procedure. The computer-aided surgery system aids in dealing with complex cases with benefits for the patient, with surgical practice, and for orthodontic finishing. Advanced software tools for diagnosis and treatment planning allow preparation of detailed operative plans, osteotomy repositioning, bone reconstructions, surgical resident training, and assessing the difficulties of the surgical procedures before the surgery. Computer-aided surgery can make the elaboration of the surgical plan a more flexible process, increase the level of detail and accuracy of the plan, yield higher operative precision and control, and enhance documentation of cases.

S192 – 3D Diagnosis of Vocalizing Larynx Using Cone Beam CT

Objectives Cone beam computed tomography (CBCT) was developed for the office-based quick and precise 3D visualization of maxillofacial region. CBCT requires scanning time of less than 10 seconds and provides isotropic 3D image with high resolution. In this study, we examined the potential role of CBCT as a tool for 3-dimensional evaluation of the glottal insufficiency. Methods CBCT was performed in 2 normal controls and in 14 patients with unilateral vocal fold paralysis (13 cases) or paresis (1 case). Laryngeal CBCT images were taken in the resting state and during phonation. Furthermore, preoperative and postoperative images were evaluated to assess the impact of phonosurgeries in 5 cases. Results CBCT of the vocalizing vocal fold was successfully performed in all the cases, producing clear coronal images. During phonation, no remarkable differences in levels between the paralyzed and normal vocal folds were observed in 10 cases, proving the 2-dimensional glottal gap (2D cases). On the other hand, the levels of paralyzed vocal folds stayed higher during phonation compared to the normal side in 4 cases, proving the 3-dimensional glottal gap (3D cases). 4 of the 2D cases had injection laryngoplasty and 1 of the 3D cases had arytenoid adduction procedure to correct their glottal gaps. Postoperative CBCT images verified the successful correction of these gaps. Conclusions Our study proved the feasibility of CBCT for 3D evaluation of vocalizing larynx. This quick and easy system may have an informative role to suggest the proper phonosurgical procedure for glottic insufficiency.
