Comparison of Dynamic Postural Stability in Autografts Versus Allografts Following Anterior Cruciate Ligament Reconstruction With Remnant Preservation: A Prospective Study With 1-Year Follow-up.

Graft selection is an important part of preoperative planning for anterior cruciate ligament reconstruction (ACLR). In addition, ACLR with the remnant preservation technique has recently gained attention due to potential benefit in bone-tendon healing, graft revascularization, and proprioceptive nerve remodeling. However, the ideal graft choice remains controversial, and there is limited research comparing autograft and allograft in ACLR with remnant preservation. To compare knee muscle strength, neuromuscular control, and patient-reported outcomes between hamstring (HS) tendon autografts and tibialis anterior (TA) allografts in patients undergoing ACLR with remnant preservation through posterior transseptal portal for >1 year. Cohort study; Level of evidence, 3. A total of 70 patients who underwent ACLR with remnant preservation between 2018 and 2021 using HS tendon autograft (n = 35) or TA allograft (n = 35) were analyzed at 3 different time points (preoperative, 6 months, and 12 months). Quadriceps and HS muscle strengths were measured with an isokinetic device. Proprioception was evaluated with reproduction of passive positioning for joint position sense and overall stability index (OSI) for dynamic postural stability. Patient-reported outcomes included the Lysholm score and the International Knee Documentation Committee score. There were no significant differences in demographic information including age, body mass index, concomitant meniscal injuries, and sports activities (P > .05) between HS tendon autograft and TA allograft groups. Quadriceps and HS strength in both groups, HS strength in deep knee flexion in both groups, and OSI in the HS tendon autograft group significantly improved at 12 months (P < .05). Repeated-measures analysis of variance revealed significant interactions between time and grafts for OSI (F[2, 136] = 5.08; P = .007). The subsequent independent t test demonstrated that the OSI was significantly higher in the TA allograft group (2.7 ± 1.0) than that of the HS tendon autograft group (2.1 ± 0.8) at 12 months (P = .009), indicating better dynamic postural stability in the HS tendon autograft group. No significant differences were found in knee muscle strength or patient-reported outcomes between HS tendon autograft and TA allograft after ACLR with remnant preservation for up to 1 year except for dynamic postural stability favoring HS tendon autograft.

Remnant Preserving Posterior Transseptal Technique Provide Safe Passage for Posterior Cruciate Ligament Reconstruction

Introduction: General reconstruction procedure of the PCL usually sacrificed its remnant and footprints to allow for better tunnel placement. Better visualization, uncomplicated graft passing, dan elimination of the “killer turn” were mentioned as the possible benefits of utilizing the posterior transseptal portal, while preservation of the PCL remnant was widely encouraged for restoring knee kinematics and healing of the graft. Objectives: This study is aimed to evaluate the functional and clinical outcomes of PCL reconstruction using posterior transseptal remnant preservation technique. Materials &amp; Methods: Five cases were included in this study. Follow-up was performed at four weeks and consisted of International Knee Documentation Committee (IKDC) score, Knee Injury and Osteoarthritis Outcome Score (KOOS), Lysholm knee scoring scale, and post-operative complications. All patients underwent similar post-operative rehabilitation program. Results: Improvement in IKDC, KOOS, and Lysholm score was recorded in all cases. There were no post-operative complications found at the time of reporting. Conclusions: Posterior transseptal portal, which was created through the posterior septum from the posteromedial to the posterolateral compartment, or conversely, is a safe arthroscopic procedure. It offers great visualization of the posterior compartment, without damaging the capsule and neurovascular bundle, while also provides satisfactory early outcomes when used in conjunction with the remnant preservation technique.

Outcome of Arthroscopic All-Inside Posterior Cruciate Ligament Reconstruction Using the Posterior Trans-Septal Approach.

Posterior Cruciate Ligament (PCL) reconstruction is a complex surgical procedure and often challenging. The newer posterior trans-septal portal technique is thought to make tibial tunnel preparation easier with better visualization of the tibial attachment site. It is also thought to lower the risk of neurovascular injuries. The aim of this study was to evaluate the functional and clinical outcomes of patients who underwent arthroscopic all-inside PCL reconstruction using the posterior trans-septal portal at our institute. This was a retrospective study with prospectively collected data between 2016 and 2020. Data collected were age, gender, types of graft used, range of movement, posterior drawer test grade, KOOS score, Lysholm knee scoring scale, and post-operative complications. All patients underwent pre- and post-operative PCL rehabilitation. A total of 36 patients (26 males and 10 females) were identified from our database. The mean age was 35.2years. Mean time from injury to surgery was 20months. Mean follow-up was 41.2months (range, 13-72months). Twenty cases involved multi-ligament injuries and another 16 patients had isolated PCL injury. Post-operative mean posterior drawer test grade improved from 2.7 to 0.7 (p < 0.001). Knee range of movement was 116.3 degrees pre-operatively and 115.6 degrees postoperatively (p = 0.814). Lysholm knee scoring scale improved from 50.9 to 91.0 (p < 0.001). KOOS score improved from 65.1 to 77.2 (p = 0.196). One patient required manipulation under anesthesia for stiffness. No patients needed any additional surgical procedures. All PCLs were clinically intact at the final follow-up. Greater visualization of the PCL tibial attachment minimizes the 'killer turn' giving a huge advantage to this technique. Arthroscopic all-inside PCL reconstruction using the posterior trans-septal portal technique is a safe, reliable and reproducible procedure. From our study, it shows that post-operative clinical and functional outcomes improved significantly.

The use of posteromedial portal for arthroscopic treatment of synovial chondromatosis of the knee: a case report

The synovial chondromatosis is an uncommon proliferative metaplastic process of the synovial cells that can develop in any synovial joint. An isolated primary chondromatosis of the posterior compartment of the knee is uncommon and few cases are reported in literature. Our purpose is to describe a rare case of primary chondromatosis of the knee posterior compartment and report the arthroscopic loose bodies excision through a difficult posteromedial portal, avoiding the use of the accessory posterior portal, most commonly reported for approaching this disease. We report a rare case of a 35-year-old Caucasian male patient with diagnosis of chondromatosis of the posterior knee compartment. The radiographs showed multiple loose bodies of the posterior compartment. The MRI revealed minimal synovial hypertrophy areas, multiple osteophytes in the intercondylar notch, and loose bodies in the posteromedial compartment. The CT allowed us to assess the bony structures, the morphology of the intercondylar notch, and the presence osteophytes of the medial and lateral femoral condyles. The CT images were crucial to plan how to reach the posterior compartments of the knee through a trans-notch passage. The patient underwent arthroscopic surgery using anteromedial, anterolateral, and posteromedial portals. The tunneling through the intercondylar osteophytes was performed to allow the arthroscope to pass trans-notch. To avoid additional accessory posterior portals, we used a 70° arthroscope to better explore the posterior knee compartment. The cartilage-like bodies were removed and synovectomy of the inflamed areas was performed. The clinical and radiological follow-up was 12months and the patient showed excellent clinical outcomes, returning to his activities of daily living and sport activity. Our case report highlights the importance of the arthroscopic approach to treat synovial chondromatosis, despite the involvement of the posterior compartment of the knee. An optimal preoperative imaging allows to plan for the proper surgical procedure even in patients with severe osteoarthritis. Moreover, the adoption of an intercondylar notch tunneling and a 70° arthroscope can help surgeons to better explore the posterior knee compartment, avoiding an accessory posterior trans-septal portal. Therefore, a synovectomy of the inflamed foci may be useful to prevent recurrence.

Arthroscopic Synovectomy for Tenosynovial Giant Cell Tumor/Pigmented Villonodular Synovitis in the Posterior Knee Using the Posterior Trans-Septal Portal Technique.

The patient is positioned such that the contralateral leg will not obstruct the ability to work in the posteromedial portal. Diagnostic arthroscopy is performed through standard anteromedial and anterolateral portals. Next, with visualization from the anterolateral portal and the knee in 90° of flexion, the posteromedial portal is created with use of a transilluminated spinal needle. The posterolateral portal is made in the same fashion as the posteromedial portal, with use of a trans-notch view from the anteromedial portal. With the arthroscope in the posteromedial portal, a blunt instrument or motorized shaver can be placed through the posterolateral portal to perforate the posterior septum and create the TSP. The mass can then be identified, biopsied, and removed with use of a motorized shaver or tissue grasper. Arthroscopic exploration through the TSP can then be done to confirm adequate excision. Alternatives include synovectomy either by arthrotomy, arthroscopy via a posteromedial or posterolateral portal with trans-notch views, or a combination of both. To limit the risk of recurrent diffuse PVNS, radiosynovectomy with yttrium-90 or phosphorus-32, either combined with surgery or alone, has been described2,17. External beam radiation has also been utilized, but radiation toxicity is seen as a major limitation17. Macrophage-colony stimulating factor (M-CSF) or CSF-1 inhibitors have recently been developed. In 2019, the FDA approved the use of CSF-1 inhibitors, and they are considered an acceptable treatment for patients who are not candidates for surgical resection17. Advantages involve increased posterior anatomy visualization to ensure adequate synovectomy, more working capacity for instruments, and decreased disruption of anatomical planes and scar tissue formation around neurovascular structures compared with open dissection10-16. Excellent clinical results (defined by return to full knee function) have been reported for the TSP technique for PVNS synovectomy. In a study of 10 cases of posterior-knee PVNS masses removed via arthroscopic synovectomy with use of a TSP, Shekhar et al. reported good functional outcomes and no operative complications2. Keyhani et al. reported a series of 21 patients who underwent the same procedure for diffuse PVNS with similar findings9. Patients can expect to retain close to full knee function following this procedure2,9. Baseline magnetic resonance imaging is recommended for all patients at 3 to 6 months after excision, as asymptomatic recurrence can occur, and patients should be followed for a minimum of 2 years post-excision2,3,7,9,18. Keeping the knee in 90° of flexion provides the furthest distance from the saphenous vein on the medial side, the peroneal nerve on the lateral side, and the popliteal artery near the posterior septum when making the posterior portals10-16.Transillumination of the posterior portals is recommended10-16.Perforation of the septum should be in the posterolateral to posteromedial direction, allowing surgeons to have a wider "safe zone" to decrease the chance of vascular injury to the popliteal artery14. Femoral tunnel placement in single-bundle, remnant-preserving anterior cruciate ligament reconstruction using a posterior trans-septal portal

Anatomic tunnel formation and remnant preservation are valuable aspects of anterior cruciate ligament (ACL) reconstruction. However, anatomic landmarks are difficult to observe during remnant-preserving ACL reconstruction (ACLR). The aims of this study were to evaluate the: 1) femoral tunnel location created with guidance from the apex of the deep cartilage margin (ADC) and footprint compared to anatomical reference; and 2) relationship between femoral tunnel location and outcomes of ACLR. A total of 109 ACLR patients without revision ACLR, multi-ligament reconstruction, peri-knee fracture, and osteotomy were included. The femoral tunnel was formed at the most proximal corner of the femoral footprint using a posterior trans-septal portal as the viewing portal. The distance from the tunnel center to ADC was measured by computed tomography and arthroscopy. The two measurements were then compared. Finally, femoral tunnel location was compared to the anatomic reference and correlated with the outcomes. The average distance from ADC to the femoral tunnel center was 7.0 ± 1.4 mm as measured by arthroscopy, and 7.2 ± 2.0 mm using three-dimensional computed tomography. There was no statistically significant difference between the two methods (P = 0.420). Clinical and stability outcomes were significantly improved postoperatively. Clinical outcome was not related to femoral tunnel location; however, stability outcome was related to femoral tunnel location: the more proximally located femoral tunnels showed better stability. The ADC can be a possible landmark in remnant-preserving ACLR using a trans-septal portal. A more proximal femoral tunnel, which is located at the proximal corner of the ACL remnant, can provide stability advantage during remnant-preserving ACLR.

Medial Meniscal Root Repair Using Curved Guide and Soft Suture Anchor.

Medial meniscal root tears have been repaired using various methods. Arthroscopic all-inside repair using a suture anchor is one of the popular methods. However, insertion of the suture anchor into the proper position at the posterior root of the medial meniscus is technically difficult. Some methods have been reported to facilitate suture anchor insertion through a high posteromedial portal, a posterior trans-septal portal, or a medial quadriceptal portal. Nevertheless, many surgeons still have difficulty during anchor insertion. We introduce a technical tip for easy suture anchor insertion using a 25° curved guide and a soft suture anchor through a routine posteromedial portal.

Comparison of the Accuracy and Reproducibility of the Femoral Tunnel Location Between Anterior and Posterior Viewing During Outside-In Anterior Cruciate Ligament Reconstruction

Accuracy and reproducibility of the femoral tunnel location could be improved with a TS portal viewing using a 30° arthroscope. During outside-in anterior cruciate ligament (ACL) reconstruction, anteromedial (AM) or anterolateral (AL) portals are usually used for the viewing of the femoral foot print. However, observing the posterior portion of the ACL femoral foot print (AM bundle footprint) using an AM or AL portal could be difficult due to blocking by remnant ACL or ridge on the medial wall of the lateral femoral condyle. In this view point, using a 70° arthroscope through posterolateral (PL) portal, posterior part of the ACL footprint could be seen directly. However, this view could be a little distorted because the wall of the ACL insertion should be seen from posterior to anterior direction. The posterior trans-septal portal (TS) could allow direct visual access to the posterior part of the ACL femoral footprint through the posteromedial (PM) portal. The purpose of this study was to compare the accuracy and reproducibility of the femoral tunnel location between three different viewing techniques (observed through an AL or AM portal using a 30° arthroscope (A group) vs. PL portal using a 70° arthroscope (PL group) vs. TS portal using a 30° arthroscope (TS group) by 3D CT in outside-in ACL reconstruction. The hypothesis of this study was that femoral tunnel locations would be more accurate and reproducible in the posterior viewing group (PL and TS), and the TS viewing technique would be the best because it offers the most direct view of the medial wall of the lateral femoral condyle. One-hundred and six outside-in ACL constructions patients were recruited. Patients were divided into three groups according to the viewing techniques (36 A vs. 35 PL vs. 35 TS patients). Femoral tunnel locations were evaluated with a quadrant method and anatomic coordinate axes measurement (ACAM) method in the medial wall of the lateral femoral condyle using 3D reconstructed CT. The accuracy and reproducibility of the femoral tunnel locations were compared between three techniques. The accuracy of the tunnel location was higher in the TS group by the quadrant method (A group vs. TS group: p<0.001, PL group vs. TS group: p<0.001) as well as the ACAM (A group vs. TS group: p<0.001, PL group vs. TS group: p=0.02). The reproducibility of the femoral tunnel position in the TS group was the highest and femoral tunnel locations of the TS group were more compactly distributed compared with those of the A and PL group (standard deviation: A group [4.68%], PL group [3.75%], and TS group [1.34%] by the quadrant method; A group [5.18%], PL group [3.85%], and TS group [1.92%] by the ACAM method). Accuracy and reproducibility of the femoral tunnel location could be improved with a TS portal viewing using a 30° arthroscope and anterior and PL portal viewing using a 70° arthroscope showed no difference.

Adjustable Button Devices for All-Arthroscopic Posterior Cruciate Ligament Reconstruction Using the Hamstrings Tendons and the “Forgotten” Transseptal Approach

Posterior cruciate ligament (PCL) ruptures account for nearly 20% of all ligamentous knee injuries. These may be either isolated or in the setting of a more complex knee trauma. Isolated tears with moderate posterior laxity (grades I or II) are commonly treated conservatively; nevertheless, symptomatic grade III injuries frequently require surgical intervention. PCL reconstruction remains a challenging surgery for multiple reasons like the neurovascular structures' proximity, the difficult passage of the graft with the “killer turn” angle, or the risk of poor graft fixation. We describe an all-inside operative technique using hamstrings tendon autografts with tibial and femoral adjustable buttons cortical fixation and the visualization of the posterior transseptal portal.

Arthroscopic suture bridge fixation technique with multiple crossover ties for posterior cruciate ligament tibial avulsion fracture.

This study examined the clinical outcomes of a newly developed technique, arthroscopic suture bridge fixation with crossover ties of PCL tibial avulsion fracture using two tibial tunnels and a posterior trans-septal portal. Records were reviewed of 18 patients (median age 33.5years, range 13-55years) with PCL tibial avulsion fractures treated with an arthroscopic suture bridge technique. Knee function before surgery and at last follow-up was evaluated by Lysholm and Tegner scores. A KT-2000 arthrometer was used to evaluate knee stability, and fracture union was assessed by plain radiographs. Mean postoperative Lysholm (P<0.001) and Tegner (P=0.011) scores showed significant improvements compared with preoperative scores. Arthrometry showed that the mean side-to-side difference improved significantly, from 7.8±0.8mm preoperatively to 3±1.2mm postoperatively (P=0.012). Radiographic evaluation showed solid union at the fracture site in all 18 patients at last follow-up. This new arthroscopic double-tunnel pull-out suture bridge fixation with multiple crossover ties and posterior trans-septal technique for PCL tibial avulsion fracture yielded good clinico-radiological outcomes, including satisfactory stability and fracture site healing. This technique can be a useful treatment option for PCL tibial avulsion fracture even with small comminuted fracture due to compression by the unique crossover configuration mesh of multiple fixation sutures. Loose body as an obstacle for posterolateral arthroscopic portal formation in the knee joint.

In this report, we present a case with difficult arthroscopic posterolateral portal formation due to loose body located in posterior compartment. These loose bodies are responsible for pain, decreased range of motion and cartilage damage in the knee joint. By making the posterior trans-septal portal prior, posterolateral portal could be made without difficulty completing the planned arthroscopic procedure.

Femoral Footprint Reconstruction With a Direct Viewing of the Posterior Insertion Using a Trans-Septal Portal in the Outside-In Anterior Cruciate Ligament Reconstruction

We established a technique for femoral tunnel preparation through direct vision of the femoral footprint with maximum preservation to the native anterior cruciate ligament (ACL) remnant using a posterior trans-septal portal. Anterior arthroscopy is difficult for the proper tunnel placement without sacrificing the ACL remnant. Posterior arthroscopy could be helpful for viewing the posterior insertion of the ACL remnant that provides excellent femoral footprint exposure without sacrificing the native ACL remnant. Therefore, a posterolateral portal technique using a 70° arthroscope through a posterolateral portal is introduced. However, using the 70° arthroscope, an oblique view is displayed and distorted view could be seen. Therefore, to achieve the goal of posterior arthroscopy and avoid obstacles of the posterolateral view with the 70° arthroscope, we introduce this technique that uses the posterior trans-septal portal with a 30° arthroscope that provides an excellent viewing to the femoral footprint through a hole of the posterior septum.

Arthroscopic approach to the posterior compartment of the knee using a posterior transseptal portal.

Arthroscopic surgery of the posterior compartment of the knee is difficult when only two anterior portals are used for access because of the inaccessibility of the back of the knee. Since its introduction, the posterior transseptal portal has been widely employed to access lesions in the posterior compartment. However, special care should be taken to avoid neurovascular injuries around the posteromedial, posterolateral, and transseptal portals. Most importantly, popliteal vessel injury should be avoided when creating and using the transseptal portal during surgery. Purpose of the present study is to describe how to avoid the neurovascular injuries during establishing the posterior three portals and to introduce our safer technique to create the transseptal portal. To date, we have performed arthroscopic surgeries via the transseptal portal in the posterior compartments of 161 knees and have not encountered nerve or vascular injury. In our procedure, the posterior septum is perforated with a 1.5-3.0-mm Kirschner wire that is protected by a sheath inserted from the posterolateral portal and monitored from the posteromedial portal to avoid popliteal vessel injury.

Arthroscopic Removal of a Wire Fragment from the Posterior Septum of the Knee following Tension Band Wiring of a Patellar Fracture.

Tension band wiring with cerclage wiring is most widely used for treating displaced patellar fractures. Although wire breakage is not uncommon, migration of a fragment of the broken wire is rare, especially migration into the knee joint. We describe here a rare case of migration of a wire fragment into the posterior septum of the knee joint after fixation of a displaced patellar fracture with tension band wiring and cerclage wiring. Although it was difficult to determine whether the wire fragment was located within or outside the knee joint from the preoperative plain radiographs or three-dimensional computed tomography (3D CT), we found it arthroscopically through the posterior transseptal portal with assistance of intraoperative fluoroscopy. Surgeons who treat such cases should bear in mind the possibility that wire could be embedded in the posterior septum of the knee joint.

Anatomic Graft Passage in Remnant-Preserving Posterior Cruciate Ligament Reconstruction

Posterior cruciate ligament (PCL) reconstruction with preservation of the remnant PCL fibers has been performed under the assumption that preserving the fibers contributes to knee kinematics, grafted tendon healing, and recovery of proprioception. This technical note presents a single-bundle, transtibial PCL reconstruction with anatomic graft passage between the remnant PCL fibers. The operation is performed using the posterior trans-septal portal, which can provide excellent visualization while preserving a large amount of remnant PCL fibers. In addition, this technique allows for anatomic graft passage without soft-tissue impingement, and it minimizes the risk of nonanatomic positioning of the PCL grafts.

Arthroscopic posterior approach for reconstruction of posterior cruciate ligament

Objective To evaluate the clinical results of arthroscopic reconstruction of posterior cruciate ligament (PCL) via posteromedial,posterolateral and posterior trans-septal portals with the preservation of intact meniscofemoral ligaments and remnant PCL fibers.Methods Nine patients with PCL injuries were treated with autogenous hamstring tendons arthroscopically through routine arthroscopic portal,posteromedial portal,posterolateral portal and posterior trans-septal portal with preservation of intact meniscofemoral ligaments and remnant PCL fibers.Tibial tunnel of 1-1.5 cm was made below the lateral articular surface of PCL tibial attachment via arthroscopic posterolateral approach.Femoral tunnel of 1 cm was made posterior to the articular cartilage of the medial femoral condyle through anterolateral approach.Autogenous tendon graft was positioned in the knee joint through the navigation of tibial and femoral double-folded silk loops that traversed the bone tunnels and was fixed with bioabsorbable interface screws at both ends.Knee function was evaluated with Lysholm scale six months postoperatively.Results All patients were followed up for average 8.6 months (range,7-14 months).None of the patients had knee extension limitation six months postoperatively,but there were two patients with 10°-15° flexion limitation and one withⅠ degree positive result in posterior drawer test.Lysholm knee score was increased from preoperative (47.6 ± 14.9)points to (92.9 ±4.6) points at six months postoperatively (P ＜0.01).Conclusion PCL reconstruction via posteromedial,posterolateral and posterior trans-septal portals can obtain clear vision (with no blind area),safe operation,accurate positioning of tibial attachment and affirmative short term treatment results. Arthroscopic Partial Meniscectomy of a Posteriorly Flipped Superior Leaflet in a Horizontal Medial Meniscus Tear Using a Posterior Transseptal Portal

This article describes a case of an arthroscopic partial meniscectomy of a posteriorly flipped superior leaflet in a horizontal medial meniscus tear using the posterior transseptal portal. An arthroscopic partial meniscectomy for bucket handle or flap tears in medial or lateral compartments using ordinary portals is a relatively common procedure in irreparable cases. However, the posterior compartment of the knee is not readily accessible through ordinary arthroscopic portals. Therefore, it has been considered a blind spot. Through the posterior transseptal portal, surgeons can achieve excellent arthroscopic visualization of the posterior compartment and easily perform arthroscopic procedures of the posterior compartment of the knee. A 48-year-old woman presented with a 1-year history of pain in the medial aspect of the right knee joint. Preoperative magnetic resonance imaging revealed a thinning of the medial meniscus posterior horn in coronal images and a sharp-edged triangle arising from the medial meniscus posterior horn between the medial femoral condyle and medial meniscus posterior horn on sagittal images (flipped-over sign). During the arthroscopic procedure, we found that the flipped leaflet was displaced posteriorly and was not mobile between the medial femoral condyle and medial meniscus posterior horn. Partial meniscectomy for a posteriorly displaced fragment can be performed successfully using the posterior transseptal portal. The posterior transseptal portal is useful for an arthroscopic partial meniscectomy of a posteriorly flipped leaflet in the posterior compartment of the knee.

Arthroscopic treatment of synovial chondromatosis at the knee posterior septum using a trans-septal approach: Report of two cases

Synovial chondromatosis of the knee joint rarely develops in the posterior septum. Such cases are difficult to treat arthroscopically since the posterior septum is not readily accessible via standard portals. We report two cases of localized synovial chondromatosis in the posterior septum. We used a posterior trans-septal portal to arthroscopically remove the loose bodies and perform a concomitant synovectomy. The two cases were followed-up for 24 and 16 months after surgery, respectively, and the patients were found to be pain-free, have a full range of knee motion and showed no evidence of recurrence. A posterior trans-septal portal may be essential in some cases for treatment of synovial chondromatosis at the posterior septum.
