The electro-encephalogram in patients with post-traumatic fat embolism

Electro-encephalograms (E.E.G.) have been studied in 7 patients with the fat embolism syndrome. Six patients recovered and 1 died. The E.E.G. abnormalities are diffuse and consist of delta and theta waves, sometimes with an absence of the alpha rhythm. There was nothing in the E.E.G. findings which could be considered specific for the fat embolism syndrome. The E.E.G.s primarily reflected the patient's state of consciousness. At follow-up examination the E.E.G.s were normal in all the six surviving patients.

ROENTGENOGRAPHIC SIGNS OF POSTTRAUMATIC FAT EMBOLISM

Recognizing the syndrome of posttraumatic fat embolism can be difficult. The onset of tachypnea, pulmonary rales, cyanosis, and stupor is frequently so rapid and dramatic that all efforts must be directed toward resuscitation. In such instances, the physician's use of pressure-or volume-cycled respirators to assist or control the patient's ventilation can be lifesaving. But among war casualties and accident victims, the signs of pulmonary injury from fat embolism may resemble those from thoracic trauma or heart failure. And, in older patients, thromboembolism must also be considered. Moreover, the cerebral injury from fat embolism may be confused with epidural hematoma or cerebral contusion. Tests for fat globules in the urine or elevated serum lipase activity are time consuming and occasionally misleading. The appearance of the patient's chest roentgenogram, however, often provides a clue to rapid diagnosis. Effect of Dextrose-Alcohol Mixture upon Pulmonary Fat

The therapy of post-traumatic pulmonary fat embolism has re-received little attention. We have studied the effect of various organic and inorganic substances upon the fat globules which lodge in the capillaries of the lungs after the intravenous injection of a known quantity of sterile fat from an animal of the same species.Twenty-four large Belgian hares and 4 dogs were used. Normal, healthy animals of the same litter were used in each series of experiments.The sterile fat was warmed to 37°C., and injected into either the marginal vein of the ear of the hares, or into the saphenous vein of the dogs. The injections were carried out at the uniform speed of 1 cc. per minute and without undue force. Four animals were used in each series of experiments. Into each animal we injected 1 cc. of the warmed, sterile fat per kilo of body weight. One animal of each series was kept for the control.
