Positive predictive value of Birads IV Lesions on mammogram in detection of breast carcinoma.

To determine the positive predictive value of Breast Imaging Reporting and Data System category IV lesions, and to evaluate interobserver agreement among radiologists in interpreting the subcategories. The cross-sectional study was conducted at the Dow Institute of Radiology, Dow University of Health Sciences, Karachi, from September 2018 to February 2019, and comprised adult females aged 30-60 years diagnosed with Breast Imaging Reporting and Data System IV. Already diagnosed breast carcinoma patients were excluded. The positive predictive value of Breast Imaging Reporting and Data System category IV was calculated using histopathology findings as the gold standard. Data was analysed using SPSS 22. There were 191 patients with a mean age of 49.01±11.79 years. Positive family history of breast cancer was found in 33(17.3%) subjects. Category IV-A was noted in 53(28.0%) subjects, IV-B in 45(23.6%) and IV-C in 93(48.7%). The overall positive predictive value of category IV was 151(79.1%), IV-A 27(50.9%), IV-B 35(77.7%) and IV-C 89(95.6%). Malignant disease was significantly associated with architectural distortion and suspicious microcalcifications (p<0.001). A good agreement was observed in the categorisation of Breast Imaging Reporting and Data System category IV between observers 1 and 2 (kappa 0.85, 95% confidence interval: 0.70-0.99), between observers 1 and 3 (kappa 0.77, 95% confidence interval: 0.61-0.94), and between observers 2 and 3 (kappa 0.63, 95% confidence interval: 0.43-0.83). Breast Imaging Reporting and Data System BI-RADS IV was found to have a high positive predictive value in the diagnosis of breast cancer. Besides the interobserver agreement in terms of identifying BI-RADS IV subcategories was good.

Positive predictive values of Breast Imaging Reporting and Data System (BI-RADS®) categories 3, 4 and 5 in breast lesions submitted to percutaneous biopsy

Objective To evaluate the BI-RADS as a predictive factor of suspicion for malignancy in breast lesions by correlating radiological with histological results and calculating the positive predictive value for categories 3, 4 and 5 in a breast cancer reference center in the city of São Paulo. Materials and Methods Retrospective, analytical and cross-sectional study including 725 patients with mammographic and/or sonographic findings classified as BI-RADS categories 3, 4 and 5 who were referred to the authors' institution to undergo percutaneous biopsy. The tests results were reviewed and the positive predictive value was calculated by means of a specific mathematical equation. Results Positive predictive values found for categories 3, 4 and 5 were respectively the following: 0.74%, 33.08% and 92.95%, for cases submitted to ultrasound-guided biopsy, and 0.00%, 14.90% and 100% for cases submitted to stereotactic biopsy. Conclusion The present study demonstrated high suspicion for malignancy in lesions classified as category 5 and low risk for category 3. As regards category 4, the need for systematic biopsies was observed.

Inter-observer variability of Breast Imaging Reporting and Data System(BI-RADS) ultrasound final assessment

Objective To evaluate the inter-observer variability of static breast sonogram final assessment among observers with different breast imaging experience, using the first edition of the Breast Imaging Reporting and Data System(BI-RADS) for ultrasound. Methods Thirty patients with 30 breast lesions were included who underwent beast lesions resection operation. A pathological diagnosis was available for all 30 lesions:16 (53%) malignant and 14 (47%) benign. Twelve radiologists independently reviewed two sonograms of each lesion, and assigned a final BI-RADS assessment category. Inter-observer variability was measured using kappa statistic. Positive predictive value(PPV) and negative predictive value (NPV) for final assessment were also calculated. Results As for the experienced observers,kappa values of categories 3,4 and 5 were 0.72,0.28 and 0.60,NPV of category 3 was 93% ,PPV of category 5 was 97% ,all of which decreased as the breast imaging experience reduced. PPVs of categories 4a,4b and 4c were 56 % ,88% and 69%, respectively. Conclusions Using BI-RADS final assessment, radiologists with sufficient breast imaging experience can provide accurate and consistent assessment for breast ultrasonography,but the agreement of diagnosis decreased as the breast imaging experience reduced. The clinical feasibility of 4a,4b and 4c subcategories is uncertain. Key words: Ultrasonography; Breast diseases

Correlação radiológica e histológica de lesões mamárias não-palpáveis em pacientes submetidas a marcação pré-cirúrgica, utilizando-se o sistema BI-RADS

OBJETIVO: Avaliar as categorias 3, 4 e 5 da classificação BI-RADS (Breast Imaging Reporting and Data System) como fator preditivo para malignidade, correlacionando os achados mamográficos e histológicos em lesões não-palpáveis da mama. MATERIAIS E MÉTODOS: Estudo analítico descritivo de 169 mulheres submetidas a biópsia cirúrgica, após localização estereotáxica de lesões mamárias não-palpáveis. As mamografias dessas pacientes foram classificadas de acordo com a quarta edição do BI-RADS, avaliando-se as categorias 3, 4 (A, B e C) e 5. Correlacionaram-se os achados mamográficos com os exames histológicos das lesões, avaliando-se o valor preditivo positivo em cada categoria. RESULTADOS: No total de 169 casos, foram diagnosticados 42 casos de câncer (24,8%). Destes, houve apenas um caso na categoria 3, 19 casos na categoria 4 e 22 casos na categoria 5. Os valores preditivos positivos para as categorias 3, 4A, 4B, 4C e 5 foram, respectivamente, de 3,4%, 10,3%, 11,3%, 36% e 91,7%. As microcalcificações foram o achado mais freqüente relacionado à doença maligna, ocorrendo em 61,5% do total. CONCLUSÃO: Este estudo demonstrou que a classificação BI-RADS permite predizer com segurança que há alta suspeição de malignidade para achados classificados na categoria 5 e diminuta chance para os achados da categoria 3. Quanto à categoria 4, foi constatada elevação progressiva dos valores preditivos positivos nas subcategorias A, B e C, mostrando que esta subdivisão contribui de forma mais detalhada e precisa na indicação de lesões suspeitas para malignidade.
