Endoscopic correction of vesicoureteric reflux secondary to neuropathic bladder.

Eleven patients with vesicoureteric reflux secondary to neuropathic bladder were treated by endoscopic injection of pyrolised polytetrafluoroethylene (Polytef) paste. Thirteen of the 15 ureters treated showed disappearance of reflux after one injection. The amount of Polytef paste injected varied from 0.2 to 0.9 ml (mean 0.45 ml). There were no complications, except in one ureter where distension of the collecting system was slightly increased compared with the pre-operative urogram.

Correction of Experimentally Produced Vesicoureteric Reflux in the Piglet by Intravesical Injection of Teflon

Vesicoureteric reflux was produced in eight piglets by opening their bladders and slitting the anterior intravesical wall of the ureter. Cystography confirmed the presence of bilateral reflux in six piglets and unilateral reflux in two. Six to eight weeks later the bladder was again opened and Teflon paste injected in the space behind the intravesical ureter, thereby creating a support for the submucosal ureter. Cystography four to six weeks after injection of Teflon paste showed absence of reflux in all animals. Intravenous pyelography showed obstruction at the vesicoureteric junction in only one of the 14 treated ureters and this was later confirmed at necropsy. Animals were followed up from one to six months and then were killed. Gross examination of the vesicoureteric region showed a well circumscribed subureteric Teflon mass of firm consistency, retaining its shape and position at the site of the injection. Histological examination showed encapsulation of the implant by a thin layer of fibrous tissue and a foreign body granulomatous reaction with histiocytes and giant cells within the implant. Hence it is technically feasible to correct experimentally produced vesicoureteric reflux in the piglet by intravesical injection of Teflon paste--a relatively inert material. It may now be possible to treat vesicoureteric reflux in man by endoscopic injection of Teflon behind the intravesical ureter.

An experimental endoscopic technique for reversing gastroesophageal reflux in dogs by injecting inert material in the distal esophagus

After the lower esophageal sphincters of large mongrel dogs had been made surgically incompetent, the degree of sphincter incompetency was determined by the grade of esophagitis present and the reflux volume. Five dogs were endoscopically injected in the distal esophagus with 5 to 30 ml of Teflon paste during four to six injection sessions, and all five dogs developed transient increases in the reflux volumes. Each of the five dogs sloughed a portion of injected Teflon during the 4-month study. Four additional dogs received between 5 and 105 ml of bovine dermal collagen injected in the distal esophagus. All have reversed their esophagitis and have retained a reflux volume at least 5 times the preinjection value without evidence of slough.

Treatment of vesicoureteric reflux by endoscopic injection of Teflon.

Thirteen girls with grade III-V vesicoureteric reflux were treated by endoscopic injection of Teflon paste behind the intravesical ureter. Fourteen of the 18 treated ureters showed complete absence of reflux after one injection of Teflon. Three ureters required a second injection of Teflon for successful treatment of the reflux. One ureter with grade IV reflux was converted to grade II reflux. Properly carried out, this procedure corrects reflux. It takes less than 15 minutes, may be done as a day procedure, and avoids open surgery. There have been no complications.

Periurethral Polytetrafluoroethylene Injections in Incontinent Female Subjects with Neurogenic Bladder Disease

Urinary incontinence in female subjects with bladder instability secondary to neurogenic bladder disease is difficult to manage. Many times pharmacologic manipulation and the standard anti-incontinence surgical procedures are unsuccessful. We have found that the urethral occlusive technique using polytetrafluoroethylene (Teflon) paste is highly effective in maintaining continence. We describe 6 patients who achieved complete urinary control and empty the bladder with the use of intermittent catheterization. All of the patients are extremely satisfied with the end results of urinary continence and have accepted self-catheterization well.

Surgical Therapy for Vocal Dysfunction

In general, misuse and abuse of the voice in the form of using the voice too loudly and too long and affecting a fundamental frequency that is too low produce chronic changes in the lamina propria while inhalation of irritants produces changes in the epithelium of the true vocal cords. Voice therapy plays an essential role in the preoperative and postoperative management of the patient. The use of the operative microscope under general anesthesia has permitted the regular preservation of the external elastic lamina, which is necessary for the restoration of the voice to its full dynamic and frequency ranges. Resection of the left recurrent laryngeal nerve is of great value in the treatment of spastic dysphonia. In the paralyzed larynx, injection of Teflon paste is of proven value in restoring the voice in permanent unilateral recurrent laryngeal nerve paralysis. Reinnervation procedures for unilateral and bilateral recurrent laryngeal nerve and superior laryngeal nerve paralysis must at this time be considered developmental.

Use of Teflon Paste for Patulous Eustachian Tube Challenged

To the Editor.— In the QUESTIONS AND ANSWERS section (1982;247:883) the response mentioned the use of Teflon paste for an excessively patent (patulous) eustachian tube. In patients with patulous eustachian tubes, good relief can be obtained by cautery with silver nitrate solution on wire wrapped with cotton. This treatment of autophony was reported in detail in an article published in Laryngoscope . 1 Injection of Teflon paste around the tube is dangerous and has resulted in cerebrovascular accidents and death. A former distributor of Teflon paste discontinued its investigational program and withdrew the material for this use after these complications were reported, although it is still available for injection into the vocal cords when they do not meet properly.

Endoscopic rehabilitation of the airway in laryngeal paralysis.

This paper deals with the three most commonly encountered laryngeal problems that are related directly or indirectly to laryngeal paralysis. These problems are compromised airway secondary to excessive injection of Teflon paste, abductor paralysis of the vocal cords and posterior commissure laryngeal webs.

Indications for Teflon Treatment of Unilateral Vocal Cord Paralysis

Unilateral vocal cord paralysis may result in severe voice insufficiency, characterized by a weak and breathy voice. The effect of extrusion on the mechanical properties of polytetrafluoroethylene paste

AbstractWe consider the connection between viscous effects experienced by polytetrafluoroethylene paste during extrusion, and the final strength of the extrudate. It is shown that by reducing the viscous contribution to the pressure drop across the extruder die, we not only save on extrusion pressure, but also increase the strength of the extrudate. The shape of die that gives the minimum pressure drop is derived.

Percutaneous teflon injection of a vocal cord under fibre optic laryngoscopic control.

The technique of percutaneous injection of Teflon paste into a paralysed abducted vocal cord using a fibre optic laryngoscope to observe and control the effect is described. Its use in vocal cord paralysis caused by inoperable bronchogenic carcinoma and the results in 14 patients are discussed.

Teflon injection of vocal cords under general anesthesia: a new method.

Use of general anesthesia permits the injection of Teflon paste into immobile vocal cords in an accurate and predictable manner, with the help of a special endotracheal tube and jetted O2 or O2/n2O mixture. The authors position a nonfunctioning vocal cord so that when the functioning cord is fully adducted , the two cords are in perfect approximation for phonation. This has allowed marked improvement in effective expulsive postoperative coughing and vocalization. Debris is blown away from the operating side and not into the patient's trachea. If excessive injection of Teflon is avoided, an uneventful postoperative course can be expected.

The use of spectrograms in the evaluation of vocal cord injection.

The utilization of voice spectrography can be an important adjunct to the assessment of vocal cord function by both laryngologists and speech pathologists. It is especially useful in determining the effectiveness of vocal cord Teflon paste injection procedures. Dysphonic characteristics are made visible, and their improvement can be monitored. As such, spectrographic analysis should be considered as a useful tool for the laryngologist.

THE CLINICAL entity of the abnormally patent auditory tube has been known since 1858.¹Attempts to treat this condition have included insufflation of solutions and powders²and surgical measures,³which proved to be unsatisfactory. Human Larynx Injected With Teflon Paste: Histologic Study of Innervation and Tissue Reaction

ONE OF US 1 recently reviewed the development of intrachordal injection procedures in the treatment of chronic vocal disability. After having experimented with various substances, 2,3 Arnold, 4 in 1962, proposed the injection of Teflon granules suspended in glycerin. The present designation of the implant material is PTFE Paste for Injection (modified polytetrafluoro-ethylene, 50% weight in weight w/w in glycerin). This technique made it possible to revive the original idea published by Brunings 5 in 1911 who injected paraffin for the correction of laryngeal paralysis. Numerous authors soon followed these suggestions and published their own experiences with the injection of granulated bone paste, silastic, 7 Teflon, 8-11 glycerin, 12 and silicone. 13 All these publications stressed the good functional results when suitable instances of this disability were treated with proper indication and correct techniques. Studies of the larynx after teflon injection.

ARNOLD and other authors,^1-10have stressed the feasibility of restoring voice in patients with dysphonias due to lateral fixation or adductor paralysis of the vocal cord. They advocated the injection of Teflon paste into the involved vocal fold, using the technique and instrument originally devised by Brünings.¹¹In his articles regarding technique of intracordal injection,¹²Arnold also reported the tissue tolerance of Teflon paste when this substance was injected into the quadriceps femoris muscle of rabbits. The original article described the histopathological results of these experiments two and four months after injection. For the past two years, experiments have been conducted by the departments of otolaryngology and pathology at the University of Kansas School of Medicine dealing with the tissue tolerance of the larynx of the dog to Teflon paste injection. This paper will describe the technique as well as the histopathological findings in these experiments. <h3>Materials</h3>
