Vitamin E Cup Cross-linked Polyethylene in Total Hip Arthroplasty: A Meta-analysis of Randomized Controlled Trials.

The goal of this study was to assess the efficacy and safety of vitamin E cup cross-linked polyethylene (VEPE) liners compared with standard polyethylene (PE) liners in total hip arthroplasty. A systematic search of the PubMed, EMBASE, Scopus, and Cochrane Collaboration Library databases was conducted. The evaluated outcomes were radiological parameters, functional scores (Harris Hip Score, Numeric Rating Scale, and Activity Scale from the University of California), and adverse events. Odds ratios and mean differences (MD) were calculated for the dichotomous and continuous variables. Meta-analysis was done using Review Manager. Seven randomized controlled trials (n = 990) were included. VEPE liners showed markedly lower penetration at the last follow-up (MD, -0.15; 95% confidence interval [CI], -0.18 to -0.11) and penetration rate at last follow-up (MD, -0.01; 95% CI, -0.01 to -0.00) than standard PE liners. However, no notable differences were observed in cup migration, inclination, anteversion, or patient-reported outcomes. The complication rate was not markedly different between the groups (odds ratio, 1.07; 95% CI, 0.42 to 2.73). This meta-analysis found that while VEPE demonstrated decreased penetration rates in total hip arthroplasty, no differences were observed in patient outcomes or complications compared with standard PE. However, the clinical relevance of penetration rate reduction remains uncertain, given the short follow-up period.

Long-term polyethylene wear rates and clinical outcomes of oxidized zirconium femoral heads on highly cross-linked polyethylene in total hip arthroplasty.

Oxidized zirconium (OxZi) and highly cross-linked polyethylene (HXLPE) were developed to minimize wear and risk of osteolysis in total hip arthroplasty (THA). However, retrieval studies have shown that scratched femoral heads may lead to runaway wear, and few reports of long-term results have been published. The purpose of this investigation is to report minimum ten-year wear rates and clinical outcomes of THA with OxZi femoral heads on HXLPE, and to compare them with a retrospective control group of cobalt chrome (CoCr) or ceramic heads on HXLPE. From 2003 to 2006, 108 THAs were performed on 96 patients using an OxZi head with a HXLPE liner with minimum ten-year follow-up. Harris Hip Scores (HHS) were collected preoperatively and at the most recent follow-up (mean 13.3 years). Linear and volumetric liner wear was measured on radiographs of 85 hips with a minimum ten-year follow-up (mean 14.5 years). This was compared to a retrospective control group of 45 THAs using ceramic or CoCr heads from October 1999 to February 2005, with a minimum of ten years' follow-up. Average HHS improved from 50.8 to 91.9 and 51.0 to 89.8 in the OxZi group and control group, respectively (p = 0.644), with no osteolysis in either group. Linear and volumetric wear rates in the OxZi group averaged 0.03 mm/year and 3.46 mm3/year, respectively. There was no statistically significant difference in HHS scores, nor in linear or volumetric wear rate between the groups, and no revision for any indication. The radiological and clinical outcomes, and survivorship of THA with OxZi femoral heads and HXLPE liners, were excellent, and comparable to CoCr or ceramic heads at minimum ten-year follow-up. Wear rates are below what would be expected for development of osteolysis. OxZi-HXLPE is a durable bearing couple with excellent long-term outcomes.

Safety of using a large femoral head on thin polyethylene for total hip arthroplasty based on different types of polyethylene

The use of a large femoral head in total hip arthroplasty (THA) to stabilize and reduce the incidence of dislocation is on the increase, but concerns arise when combining them with small acetabular components due to potential mechanical failures in thin polyethylene (PE) liners. A single-institution, retrospective cohort study was conducted on 116 patients with minimum 2-year follow-up who received 36-mm femoral heads and acetabular components ≤ 52 mm, using either remelted highly cross-linked polyethylene (remelted HXLPE) or vitamin E-infused HXLPE (VEPE). Osteolysis and implant loosening were not observed in either group. Although a fracture of the PE liner was observed in each group (1.7%), the clinical outcomes were excellent, as the mean modified Harris Hip Score (mHHS) at the last follow-up was 93.5. Moreover, the mean linear wear rates measured by digital imaging software in both groups were low, with 0.035 mm/y in remelted HXLPE and 0.030 mm/y in VEPE. In conclusion, The use of a large femoral head on a thin PE liner can be a viable treatment option in patients who need to prioritize stability; however, careful attention should be paid to mechanical fractures of the PE liner.

High Grade Osteosarcoma at the Site of a Ceramic on Cross-Linked Polyethylene Cementless Hip Replacement: A Case Report and Review of the Literature

The following case report describes a 50-year-old male with a history of post-traumatic arthritis of the right hip after a previous open reduction of a traumatic hip dislocation as a young adult. He underwent an uncomplicated ceramic on highly cross-linked polyethylene primary total hip arthroplasty (THA), with unremarkable intra-operative pathology at that time. Three years postoperatively, the patient developed acute onset of hip and groin pain and associated radiographic findings which proved to be related to a high-grade osteosarcoma within the soft tissue juxtaposed to the ceramic on highly cross-linked polyethylene THA. While metal-associated primary bone sarcomas have been previously reported, we are unaware of any prior description of an osteosarcoma in the setting of a ceramic on highly cross-linked polyethylene articulation. This current report represents the first case of a primary osteosarcoma associated with this type of hip arthroplasty articulation.

National Registry Data Analysis on a Unique Highly-Crosslinked Polyethylene for Total Hip Arthroplasty

BackgroundSeveral types of highly-crosslinked polyethylene with different manufacturing processes and mechanical properties are commercially available, including HIGHCROSS (Medacta). The aim of this registry study is to ascertain the long-term safety of the HIGHCROSS liners in total hip arthroplasty and compare the revision rates with its contemporaries using real-world data to establish their safety and generalizability when used by multiple surgeons. MethodsThe Australian Orthopaedic Association National Joint Replacement Registry (AOANJRR) and the Swiss National Joint Registry (SIRIS) were selected for the large number of users of Medacta implants and the availability of long-term results. Three reports from SIRIS and 4 reports from AOANJRR were examined to establish the overall and Kaplan-Meier (KM) cumulative revision rate. ResultsAccording to SIRIS, the HIGHCROSS liner was the most commonly used bearing surface with Medacta stems with overall revision rates for wear/osteolysis at 9.7 years of 0.04% and 0.03%, respectively, for AMIStem and Quadra. Based on AOANJRR, the KM cumulative revision rate for any reason of Medacta stems with HIGHCROSS liners was lower than that with ceramic liners at 3 years for MasterLoc and at 10 years for Quadra. The KM cumulative revision rate for any reasons of Medacta cementless cups with HIGHCROSS liners at 12 years was lower than the comparator made of all other implants and tribological couplings (1.6% vs 2.1%). ConclusionsThis real-world data proves that long-term HIGHCROSS survival rates are comparable to other modern bearing surfaces.

In vivo creep and wear performance of vitamin-E-diffused highly crosslinked polyethylene in total hip arthroplasty.

An acetabular liner thickness of around 6mm remains the "gold standard" in total hip arthroplasty. Some surgeons have been recommending the use of the thickest possible liner because contact stress and strain in articulating surfaces decrease with increasing the wall thickness. The purpose of this study was to determine whether in vivo creep and wear performance could be enhanced using a thicker liner over the standard thickness in vitamin-E-diffused highly crosslinked polyethylene (HXLPE). One hundred and twenty-two hips were allocated to age-matched, sex-matched, and body mass index-matched two subgroups implanted either with a 6.8- or 8.9-mm-thick vitamin-E-diffused HXLPE liner against 28-mm cobalt-chrome femoral head, and followed-up for 7years. Linear and volumetric penetration of femoral head into the liners attributed to creep and wear were analyzed for each group. Compressive creep strain generated at the initial 6months was significantly larger in the 6.8-mm group (2.6%) than in the 8.9-mm group (2.2%). The linear steady-state wear observed after 2years was 0.0019 and 0.0015mm/year, whereas the volumetric steady-state wear was 0.54 and 0.45 mm3/years in the 6.8- and 8.9-mm-thick groups, respectively. Although less strain in the thicker group resulted in a slightly less wear, it did not reach significant differences in the steady-state wear rates between the groups. No clinical significance for using a thicker liner over the standard thickness (6.8mm → 8.9mm) was confirmed in the vitamin-E-diffused HXLPE according to the 7-year follow-up. The wear rates for both thicknesses were very low enough to prevent osteolysis, and no mechanical failure was observed at any follow-up interval. Nevertheless, since the significantly higher strain was seen in the thinner liner, further follow-up is needed to compare the longer term wear and the incidence of osteolysis and component fracture.

Metal-on-crosslinked polyethylene in total hip arthroplasty - an excellent combination at fifteen to twentyyears of follow-up.

Cross-linked polyethylene (PE) has been used with great clinical success in total hip arthroplasty (THA) since its debut in the late 1990's. However, reports regarding this bearing couple near the end of its second decade of service are still scant. The aim of this study was to first determine the long term clinical and radiological results and second Investigate what factors affect wear rates using a metal-on-crosslinked PE bearing articulation. 55 THAs using a single brand of cross-linked liner, cementless cup and 28mm hip ball were performed in 44 patients. Age, sex, Charlson Comorbidity Index (CCI) and need for revision surgery were recorded. Linear and volumetric wear was determined using the Martell method. Mean age at operation was 51.2 (29-73 ± 12.1) years. Mean duration of follow-up was 16.9years (range 15.0-20.1 ± 1.1years). Osteolysis was not present in the latest follow-up radiographs. Median linear and volumetric wear rate was 0.038mm/year (95% CI 0.032-0.047) and 7.115mm3/year (95% CI 6.92-17.25) respectively. Acetabular component position was not found to be related to both linear and volumetric wear. No significant difference was found in the linear and volumetric wear rates of thinner and thicker liners (8mm or below and > 8mm) (p = 0.849 and p = 0.64 respectively). Metal-on-crosslinked PE is associated with low linear and volumetric wear rates which has virtually obviated osteolysis and has translated to excellent survivorship even at long term follow up. In-vivo oxidation does not appear to be of clinical concern at this point.

New polyethylenes in total hip arthroplasty : a 20- to 22-year follow-up study.

A significant reduction in wear at five and ten years was previously reported when comparing Durasul highly cross-linked polyethylene with nitrogen-sterilized Sulene polyethylene in total hip arthroplasty (THA). We investigated whether the improvement observed at the earlier follow-up continued, resulting in decreased osteolysis and revision surgery rates over the second decade. Between January 1999 and December 2001, 90 patients underwent surgery using the same acetabular and femoral components with a 28 mm metallic femoral head and either a Durasul or Sulene liner. A total of 66 hips of this prospective randomized study were available for a minimum follow-up of 20 years. The linear femoral head penetration rate was measured at six weeks, one year, and annually thereafter, using the Dorr method on digitized radiographs with a software package. In the Durasul group, no patients underwent revision due to loosening or showed radiological evidence of osteolysis. In the Sulene group, four patients (four hips) were revised due to femoral component loosening. The 20-year cumulative failure incidence in the presence of the competing event of death for revision surgery was 4.5% (95% confidence interval (CI) 0.8 to 13.6) in the Durasul group, and 8.9% (95% CI 2.8 to 19.5) in the Sulene group. The mean wear one year after surgery was 0.09 mm (SD 0.007) in the Durasul group and 0.24 (SD 0.015) in the Sulene group (p < 0.001). From one to 20 years after surgery, the mean total penetration was 0.32 mm (SD 0.045) in the Durasul group and 1.07 mm (SD 0.13) in the Sulene group (p < 0.001). Mean femoral head penetration at 20 years was approximately 70.0% less in the Durasul group than the Sulene group. The significant reduction in femoral head penetration obtained with the Durasul compared with Sulene in uncemented THA resulted in lower osteolysis and revision rates after 20 years.Cite this article: Bone Joint J2022;104-B(9):1032-1038.

Low Wear at 10-Year Follow-Up of a Second-Generation Highly Cross-Linked Polyethylene in Total Hip Arthroplasty.

Characterizations and factors influencing longer term performance of second-generation sequentially irradiated and annealed highly cross-linked polyethylene (HXLPE) are lacking. We evaluated patients who underwent total hip arthroplasty with HXLPE at mean 10-year follow-up for (1) linear and volumetric wear rates, (2) patient and implant characteristics, (3) implant survivorships, and (4) functional scores. We evaluated 130 hips (110 patients) that received HXLPE acetabular liners at a single center. The mean age was 56 years (range, 20-79 years), with a mean follow-up of 10 years (range, 8-15). Radiographic linear (millimeters/year) and volumetric (cubic millimeters/year) wear rates were quantified using radiographic analysis. Survivorship was assessed by all-cause and wear-related revision rates. Functional outcomes were assessed by Short Form 12 and modified Harris Hip Scores. The mean linear wear rate was 0.02 ± 0.03 mm/y, and the mean volumetric wear rate was 12.6 ± 5.3 mm3/y. Younger age had higher volumetric wear (total and yearly, P= .01). Increasing body mass index trended toward higher total and yearly linear (both, P ≤ .09) and volumetric wear (both, P ≤ .07). Ten patients required revisions, with an all-cause survivorship of 92% and a wear survivorship of 100%. The mean modified Harris Hip Scores was 84, and the mean Short Form 12 scores were 46 (physical) and 55 (mental). We observed low linear and volumetric wear rates for HXLPE at 10-year mean follow-up. Younger age and higher body mass index at the time of surgery may be important patient characteristics influencing long-term wear. These results illustrate the potential for this second-generation HXLPE to be an appropriate long-term total hip arthroplasty interface.

Psychiatric Disorders and Genotoxicity Following Primary Metal on Polyethylene Total Hip Arthroplasty and Their Correlation to Cobalt/Chromium Levels.

IntroductionHip arthroplasty (HA) using implantable metal components is among the commonest orthopedic interventions. However, it can be followed by several complications following corrosion and the release of metal ions. Several studies proved that damaged genomic DNA may contribute to the pathophysiology of mental disorders.AimThe current work aims to evaluate the psychiatric disorders in metal on polyethylene hip arthroplasty (MOP-HA) patients and its correlation to cobalt/chromium (Co/Cr) levels and genotoxicity.MethodsThe work was a longitudinal follow-up study including 34 adults with unilateral primary MOP-HA meeting the inclusion and exclusion criteria. Preoperatively, 6, 12-months-postoperatively, patients were examined for cognitive impairment using mini-mental-state-examination (MMSE), depression using major-depressive-inventory (MDI), and blood samples were collected for estimation of Co/Cr, detection of genotoxicity by single-cell-gel-electrophoresis (comet assay) and serum 8-hydroxy-2’–deoxyguanosine (8-OHdG).ResultsCognitive impairment was reported in 18.5% and 14.8% at 6-months, and 12-months postoperative, respectively. Depressive disorder was recorded in 22.2% at 6-months and 14.8% at 12-months postoperative. The marginal homogeneity tests proved a non-significant difference. There was a non-significant difference in preoperative, 6-months, 12-months postoperative MMSE, and MDI scores. There were significantly increased Co/Cr levels at 6-months postoperative. The levels decreased at 12-months postoperative, however, still significantly higher than preoperative values. There was a significant increase in serum 8-OHdG and the levels were positively correlated to cobalt levels at both 6 and 12-months-postoperative. There was a non-significant difference among preoperative, 6-months, and 12-months postoperative comet assay measurements.ConclusionFrom previous findings, we can conclude that will-functioning MOP hip arthroplasty can induce increased ion levels and positively correlated increase in biochemical markers of genotoxicity (8-OHdG).

Unexpected Wear of a Uniquely Designed Moderately Cross-Linked Polyethylene in Total Hip Arthroplasty.

A uniquely designed, non-heat-treated moderately cross-linked acetabular polyethylene liner used in total hip arthroplasty (THA) demonstrated excessive wear during routine follow-up, prompting an evaluation of the linear wear rate. All THAs were performed by the senior author. The study group included 38 THAs using the uniquely designed polyethylene in question, compared to a control group of 21 THAs using another moderately cross-linked polyethylene with good 10-year outcomes. Two-dimensional linear head penetration wear measurements were obtained using the Martell Hip Analysis Suite, and retrieval analysis was performed on two liners. The study group had a significantly higher average penetration rate of 0.089 mm/y than the control group average rate of 0.047 mm/y (P= .04). Forty-five percent of the study group had a wear rate above the osteolysis threshold (0.1 mm/y), compared to 24% in the control group. Macroscopic analysis of two retrieved liners validated the radiographic findings. The data suggest unexpectedly higher wear rates for a moderately cross-linked polyethylene design, with nearly half of the study group at risk for osteolysis. Further registry or database analyses of this particular moderately cross-linked polyethylene are warranted.

Long-Term Follow-Up of Conventional Polyethylene in Total Hip Arthroplasty in Young Patients: Heightened Wear-Related Complications Are Observed at the Beginning of the Third Decade

BackgroundConventional polyethylene (CPE) was used widely in the past as a bearing surface in total hip arthroplasty (THA). As CPE THAs age and the revision burden continues to grow, it is increasingly important to understand the durability and failure mechanisms of this bearing material. Currently, such long-term data remain limited, particularly in younger, more active patients in whom wear issues are of greater concern. MethodsWe retrospectively reviewed 90 hips (77 patients) that underwent primary THA with CPE bearings on cobalt chrome femoral heads at age ≤50 years at 20-year minimum follow-up (mean 21.6 [20-23]). We analyzed polyethylene wear rates, clinical outcomes (modified Harris Hip Score, University of California, Los Angeles Activity Score), and implant survivorship. ResultsWear analysis revealed a median linear wear rate of 0.113 mm/y (95% CI 0.102-0.148) and a median volumetric wear rate of 41.20 mm3/y (95% CI 43.5-61.0). Modified Harris Hip Scores remained 37 points above preoperative baseline (P < .001) and University of California, Los Angeles scores 1.4 points above baseline (P = .018) at 21.6-year mean follow-up. Twenty-nine hips (32.2%) were revised, 20 of which (22.2%) were wear-related at a median of 15.4 years (interquartile range 11.4-19.5). Survivorship free from wear-related revision was 95.6% (95% CI 88.7-98.3), 87.9% (78.6-93.3), 78.1% (49.6-66.5), and 61.1% (41.3-67.0) at 10, 15, 20, and 25 years. ConclusionWear-related issues developed at increasingly high rates after 15 years, suggesting the need for surveillance after this time. The long-term wear and survivorship data of this study may be used as a benchmark when evaluating the performance of contemporary bearings in young, active THA patients.

Wear rate and osteolysis in two types of second-generation annealed highly cross-linked polyethylene in total hip arthroplasty: A retrospective comparative study with a minimum of five years

BackgroundAs no previous study has directly compared the linear wear rate in two types of second-generation annealed highly cross-linked polyethylene, we performed a retrospective study with a minimum of 5-year follow-up to assess primary arthroplasties in the (1) wear rates and (2) incidence of osteolysis of the two types of HXLPE. HypothesisThere was no significant difference in the linear wear rate and the incidence of osteolysis between the two types of second-generation annealed highly cross-linked polyethylene. Patients and methodsIn this single-center study, we reviewed 257 cases of primary cementless total hip arthroplasties between 2011 and 2015, which were performed with 32mm delta ceramic on second-generation annealed highly cross-linked polyethylene (X3 and E1 were used in 105 and 103 cases, respectively.). The mean wear rate was evaluated using a computer-assisted method, and the incidence of osteolysis was evaluated based on the appearance of a localized area with loss of trabecular bone or cortical erosion adjacent to the implants during the latest follow-up. ResultsIn total, we evaluated 208 cases, followed postoperatively for over 5 years (mean, 6.1 years, range: 5.0-8.0). There were no significant differences between the two groups with respect to age (list in order of Group X, Group E, p value) (61.2±12.3, 62.7±12.1, p=0.36), sex (ratio of male: 17.1%, 14.6%, p=0.61), body mass index (22.9±3.7, 22.8±4.0, p=0.91), pre- (49.9±14.8, 48.5±13.8, p=0.49) and post-operative (91.3±9.1, 92.7±7.0, p=0.23) Japanese Orthopaedic Association Hip Score, cup size (50.8±3.0, 50.9±2.2, p=0.70), cup inclination (38.7±4.8, 37.6±4.8, p=0.10), and cup anteversion (18.7±6.9, 18.5±7.6, p=0.80). The mean linear wear rates of the X3 and E1 groups were 0.057±0.039 (range: 0–0.16) and 0.054±0.037mm/year (range: 0–0.15), respectively (p=0.61). No osteolysis was found on the final plain radiographs in both groups. DiscussionThis study revealed that both types of highly cross-linked polyethylene have excellent linear wear rates and were equally safe to use. However, the difference between the two materials in terms of the long-term wear rate should be further validated. Level of evidenceIII; retrospective case control study.

Highly cross-linked polyethylene still outperforms conventional polyethylene in THA: 10-year RSA results

Background and purpose — Cup wear in total hip arthroplasty (THA) can be affected by different manufacturing processes of the polyethylene (PE). We report the long-term wear pattern differences, as well as early creep behavior, between conventional PE and highly cross-linked PE (HXLPE) liners, as measured with radiostereometry (RSA) up to 10 years. We also compare migration and clinical outcome of 2 similar uncemented cups with different backside surface roughness. Patients and methods — We included 45 patients with primary osteoarthritis. 23 received a conventional liner and 22 an HXLPE liner in a similar uncemented cup, but with a slightly rougher surface. The patients were followed up with RSA and hip-specific outcome questionnaire (HOOS) at 3 months, 1, 2, 5, and 10 years. Results — During the first 3 months both liners showed expected deformation with mean proximal head penetration of 0.39 mm (conventional PE) and 0.21 mm (HXLPE). Between 3 months and 10 years there was a difference in annual wear with 0.12 mm/year for the conventional liner and 0.02 mm/year for the HXLPE liner. The cup with rougher surface had less initial migration but both types had stabilized after 3 months. The HOOS scores improved after surgery and remained high for both groups throughout the study period. Interpretation — Up to 10 years the HXLPE has consistent lower annual wear, possibly contributing to longer survival of the THA, compared with conventional PE. All patients reported good results regardless of liner type.

Second-Generation Highly Cross-Linked Polyethylene in Total Hip Arthroplasty.

Implant survival of 2,723 vitamin E-infused highly crosslinked polyethylene liners in total hip arthroplasty: data from the Finnish Arthroplasty Register

Background and purpose — The use of crosslinked polyethylene in total hip arthroplasty (THA) has decreased wear remarkably. It has been suggested that the antioxidative effects of vitamin E may enhance the wear properties of polyethylene even further. This study evaluates revision rates between vitamin E-infused polyethylene liners (E1 and E-poly, ZimmerBiomet, Warsaw, IN, USA) versus moderately crosslinked polyethylene (ModXLPE) liners from the same manufacturer used in primary THA. Patients and methods — We conducted a study based on data from the Finnish Arthroplasty Register. The study group consisted of 2,723 THAs with a vitamin E-infused liner and a reference group of 2,707 THAs with a moderately crosslinked polyethylene liner. Survivorship, revision risk, and re-revision causes were compared between groups. Results — The 7-year survival of the vitamin E-infused polyethylene liner group and of the reference group with revision for any reason as the endpoint was comparable (94% [95% CI 92.9–94.9] and 93% [CI 91.9–93.9], respectively). The adjusted hazard ratio (HR) for any revision was similar between the groups (0.7 [CI 0.4–1.1]). When revision for aseptic loosening was studied as the endpoint, the survival for the study group was 99% (CI 98.6–99.4) and for the reference group 99% (CI 98.7–99.5), and the risk of revision was comparable between the study groups (HR 1.3 [CI 0.7–2.5]). Interpretation — After an observation period of 7 years vitamin E-infused liners shows results equal to results obtained with crosslinked polyethylene liners.

The Lawrence D. Dorr Surgical Techniques & Technologies Award: Using Big Heads and Small Acetabular Components With Highly Cross-Linked Polyethylene in Total Hip Arthroplasty: Is It Safe?

BackgroundContemporary total hip arthroplasty (THA) employs larger femoral heads to optimize hip stability. However, the combination of large femoral heads and comparatively small acetabular components poses a potential risk for implant failure secondary to polyethylene (PE) liner fracture or dissociation. The purpose of this study was to evaluate the incidence of liner fracture or dissociation, implant survivorship, and PE wear rates in primary THAs using large femoral heads and small acetabular components. MethodsBetween 2000 and 2017, we retrospectively identified 882 primary THAs with 36 mm femoral heads and acetabular components ≤52 mm with highly cross-linked polyethylene liners. Mean age was 66 years, 88% were females, and mean body mass index was 30 kg/m2. We evaluated the cumulative incidences of dislocation, any revision, and any reoperation utilizing a competing risk model. Osteolysis and femoral head penetration were assessed with a validated radiographic technique at minimum 10-year follow-up (n = 18). Mean follow-up was 4 years. ResultsThere were no liner fractures or dissociations in the entire cohort. The 10-year cumulative incidences of dislocation, any revision, and any reoperation were 3.2%, 5.6%, and 9.3%, respectively. Mean linear femoral head penetration was 0.042 mm/y and mean volumetric wear rate was 44 mm3/y. No THAs demonstrated evidence of osteolysis or component loosening at long-term follow-up. ConclusionIn a large cohort of primary THAs pairing large femoral heads with small acetabular components, there was no evidence of liner fracture or dissociation. Cumulative incidences of dislocation, any revision, and any reoperation were low at mid-term. Level of EvidenceIV.

Early failure for wear after ceramic-on-highly cross-linked polyethylene total hip arthroplasty: a case report

BackgroundHighly cross-linked polyethylene (HXLPE) enhances the anti-wear characteristics of the conventional polyethylene (PE). Early failure for wear after ceramic-on-highly cross-linked polyethylene (CoHXLPE) total hip arthroplasty (THA) is extremely rare.Case presentationWe described the case of a 60-year-old man who underwent right CoHXLPE THA because of the developmental dysplasia hip (DDH) complained pain 32 months after this procedure. Plain radiographs showed that eccentric wear existed at the polyethylene insert. However, the patient refused surgery at that time and did not stop weight-bearing. The right hip pain continued for 7 months. Plain radiographs of the pelvis showed that the HXLPE liner was penetrated and partial inner wall of acetabular shell was worn. Acetabular cup revision was performed, and the ceramic head and HXLPE were exchanged.ConclusionsDifficult reduction during primary THA, especially for DDH, can result in higher abductor tension, which may lead to early eccentric wear of the prosthesis. Whenever eccentric wear of HXLPE liner was found, weight-bearing must be stopped to avoid the accelerated wear and adverse reactions to metal debris (ARMD).

Mid- to Long-Term Survivorship Analysis of a Second-Generation Highly Cross-Linked Polyethylene in Total Hip Arthroplasty

AbstractThe emergence of highly cross-linked polyethylene (HCLPE) has drastically improved survivorship of implants used in total hip arthroplasty (THA), as demonstrated by midterm outcomes. However, there is limited data evaluating long-term outcomes and wear rates of these liners. Therefore, the aim of this study is to evaluate the longest-to-date follow-up of a specific second-generation HCLPE liner, the progression of wear rate, and its relation to acetabular cup positioning. A retrospective study was conducted on patients who underwent THA between January 2001 and December 2005 using a specific second-generation HCLPE liner. Annual liner wear rate (mm/year) was calculated in this group as well as acetabular positioning through abduction angle (degrees). Failures and reason for failures such as periprosthetic fracture, dislocation, and osteolysis were noted when applicable. Forty hips of 35 patients were included in this study with a clinical and radiographic follow-up of 13.4 ± 2.2 years (range: 10.0–16.7 years). Linear and volumetric wear rates were calculated to be 0.037 ± 0.020 mm/year and 22.94 ± 12.07 mm3/year, respectively. No radiographic femoral or acetabular osteolysis was observed in any of the cases. One patient required revision due to complications unrelated to the liner. No significant relationship could be established for acetabular component abduction angle and linear wear rate (p = 0.690, Spearman's rho = 0.03). Our study demonstrates comparable liner wear rates to that of other second-generation HCLPE liners in long-term follow-up. The XLPE liner continues to exhibit minimal progression of linear wear when compared with previous midterm studies and maintains a low rate of postoperative complications requiring revision THA surgery. Further studies are warranted to assess long-term wear and survivorship between this specific liner and comparable HCLPE models.

Gross trunnion failure in metal on polyethylene total hip arthroplasty-a systematic review of literature.

Multiple cases of dissociation of the head from the neck termed as gross trunnion failure (GTF) in total hip replacement have been described. Very little quantitative data is available for patient and implant factors associated and predisposing to this complication. To systematically review and analyze all studies which have gross trunnion failure in case of metal on polyethylene (MoP) total hip replacement. PubMed database was searched. We also performed a secondary search by pearling bibliography of all full text articles obtained. Predefined inclusion and exclusion criteria were used for abstract screening by two independent observers. A total 46 cases met our inclusion criteria. These were included in the final analysis and data was pooled. Till date, 46 cases of GTF in MoP THR have been reported. The mean age at time of revision was 70.13years (range 50 to 89years). The mean time to revision surgery was 8.24years (range 4.7 to 14years). 91.4% cases were male. BMI was ≥ 25 in 38/41 cases and ≥ 30 in 21/41 cases. Pain (95.5%) and difficulty or inability to walk (97.7%) were the most common symptoms. A total of 19/44 cases described varying sounds like click, clunk, and pop before dislocation. Accolade TMZF/TMZF plus was the most common stem used in 34/46 cases. The stem neck angle was 127 degrees in 97% cases (32/33 cases). A positive neck offset of 4mm or more was used in 91.1% cases (40/44 cases). The head size ≥ 36mm in approximately 90% cases. The head material was cobalt chromium in all 45 cases, where data was available. Among the intra-operative findings, the most common findings were metallosis (41/44), black or brow coloured synovial fluid (21/44), pseudotumour (19/44), synovial hypertrophy (18/44), damage to the abductor musculature (17/44), proximal femoral osteolysis (5/44 cases), and heterotrophic ossification (2/44 cases). Histopathological analysis was available in 11 cases and was suggestive of fibrous tissue with chronic cellular infiltrate in all cases. The serum cobalt and chromium concentrations were raised above normal in 86.4% (19/22) and 21.7% cases (5/23) respectively. Gross trunnion failure may not be as uncommon as was previously thought. A number potential associations and predisposing factors of this complication have been highlighted in this review. But due to small sample size and weak level of evidence, further studies are needed in this field.
