Google Trends Analysis Shows Increasing Public Interest in Platelet-Rich Plasma Injections for Hip and Knee Osteoarthritis

BackgroundOsteoarthritis is a chronic musculoskeletal condition that frequently affects the hip and knee joints. Given the burden associated with surgical intervention for hip and knee osteoarthritis, patients continue to search for potential nonoperative treatments. One biologic therapy with mixed clinical and basic science evidence for treating osteoarthritis is platelet-rich plasma injections into the affected joint. We used the Google Trends tool to provide a quantitative analysis of national interest in platelet-rich plasma injections for hip and knee osteoarthritis. MethodsGoogle Trends parameters were selected to obtain search data from January 2009 to December 2019. Various combinations of “arthritis,” “osteoarthritis,” “PRP,” “platelet-rich plasma,” “knee,” and “hip” were entered into the Google Trends tool and trend analyses were performed. ResultsThree linear models were generated to display search volume trends in the United States for platelet-rich plasma and osteoarthritis, hip osteoarthritis, and knee osteoarthritis, respectively. All models showed increased Google queries as time progressed (P < .001), with R2 ranging from 0.837 to 0.940. Seasonal, income-related, and geographic variations in public interest in platelet-rich plasma for osteoarthritis were noted. ConclusionOur results demonstrate a significant rise in Google queries related to platelet-rich plasma injections for osteoarthritis of the hip and knee since 2009. Surgeons treating hip and knee osteoarthritis patients can expect continued interest in platelet-rich plasma, despite inconclusive clinical and basic science data. Trends in public interest may inform patient counseling, shared decision-making, and directions for future clinical research.

Comparison of the short-term results of single-dose intra-articular peptide with hyaluronic acid and platelet-rich plasma injections in knee osteoarthritis: a randomized study

Introduction/objectivesIntra-articular injections may be useful in terms of pain and functional status, in knee osteoarthritis (OA). Besides hyaluronic acid (HA) and platelet-rich plasma (PRP), peptide molecules recently begin to be used. The aim of this study was to compare the efficacy of intra-articular peptide Prostrolane® (CAREGEN Co. Ltd.) injection with that of the HA and PRP in the persons with OA.MethodFifty-four patients with OA were included in this prospective, randomized study. Patients were randomized into three groups as intra-articular HA, peptide, and PRP groups. Paracetamol was permitted three times a day to all groups. All the patients were evaluated by the Western Ontario and McMaster Universities Arthritis Index (WOMAC), Health Assessment Questionnaire (HAQ), and visual analogue scale (VAS) at rest and during movements. Measurements performed at the baseline, after the first week of injection, and at the first and third months of follow-up.ResultsMean age was 55.8 ± 8.9 years. Forty-four (81.6%) were women. A week after the injections, rest and movement pain severity was measured by VAS decreased significantly in all the study groups (p < 0.05). There were no statistically significant differences between the groups in terms of first week pain relief (p > 0.05). WOMAC pain, stiffness, function, and total scores were improved significantly in all the groups a week after the injections (p < 0.05). Improvement continued at the third month control; however, the improvement in the WOMAC pain score was significantly better in the peptide group at the third month control (p < 0.05). The decrease in the rest and movement pain was continued for 3 months except the HA group’s rest pain. There were no differences among the groups for all measurements, except for the WOMAC pain score at 3 months after treatment, which was significantly lower in the peptide group.ConclusionAs a result, pain relief and functional improvement were obtained after the intra-articular HA, peptide, and PRP injections in OA, and decrease in pain was better in the peptide group.Key Points• The short-term effects of intra-articular HA, peptide, and PRP injections were compared in knee osteoarthritis.• HA, peptide, and PRP injections may be useful in pain relief and functional improvement in knee osteoarthritis.

Efficacy and Safety of Platelet-Rich Plasma Injections for Osteoarthritis

Therapeutic options for the treatment of osteoarthritis (OA), the most common type of arthritis, are limited, leaving many patients symptomatic and resulting in extensive disability and economic cost. Platelet-rich plasma (PRP) has become a therapeutic treatment offered by a number of practitioners for the management of a variety of conditions from osteoarthritis to wound healing, dental procedures, soft tissue injuries, and alopecia. As PRP is a blood product rather than a pharmaceutical agent and is often administered autologously, it has not undergone standard pharmaceutical approval processes. However, the use of PRP has thus far been grandfathered in jurisdictions such as Canada due to its wide use for many years. As a result, the clinical utilization of PRP for osteoarthritis has extended rapidly despite largely heterogeneous and often inconclusive studies of its efficacy. In the last year, studies with a higher level of evidence have been published that indicate that PRP may result in improved pain and function at later timepoints (e.g., 6 and 12 months) compared with either hyaluronic acid or saline. This review will provide updated information of the scientific basis, clinical studies, and future studies of PRP.

Regenerative Rehabilitative Medicine for Joints and Muscles

The purpose of this paper was to review regenerative medicine treatments for joints and muscles. There is level 1 evidence supporting the use of platelet-rich plasma (PRP) injections for the treatment of osteoarthritis (OA). Few studies have investigated the efficacy and safety of bone marrow aspirate concentrate (BMAC), microfragmented fat (MF), and placental and amniotic product (PAP) injections for OA. Preliminary studies have not found a benefit of PRP injections for acute muscle injury. No studies have been performed evaluating the therapeutic effect of BMAC, MF, or PAP injections for muscle injuries. Based upon the currently available research, PRP injections for OA provide more significant pain relief and functional improvement than placebo or hyaluronic acid and are more effective in the knee than in the hip. There is insufficient evidence to determine whether BMAC, MF, or PAP injections are safe or efficacious for the treatment of OA or whether any regenerative medicine injection is safe or effective for muscle injuries.
