The role of platelet-rich plasma in microlaryngeal surgery: a randomised, controlled trial.

Platelet-rich plasma has gained interest over the two last decades, mainly because of its role in regenerative medicine. This work aimed to assess the role of intra-operative local application of platelet-rich plasma gel in the improvement of quality of voice after microlaryngeal surgery. This was a prospective comparative study that included 40 patients undergoing microlaryngeal surgery for benign vocal fold lesions. There were two groups divided equally into study group A and control group B. The assessment of voice was performed by videostroboscopy and acoustic analysis pre-operatively and at two weeks and one and three months post-operatively. The data demonstrated that all the stroboscopic and acoustic parameters showed significant improvement in both groups. Group A showed significant improvement regarding acoustic parameters at the third post-operative follow up when compared with group B. Platelet-rich plasma has a beneficial effect on voice quality following microlaryngeal surgery based in particular on acoustic parameters.

Does Platelet-Rich Plasma (PRP) Improve Colonic Anastomotic Healing in Rats Undergoing Cisplatin-Paclitaxel-Based HIPEC?

Hyperthermic intraperitoneal chemotherapy (HIPEC) causes delays in wound healing. Platelet-rich plasma (PRP) gel has been shown to be effective in facilitating wound healing. We examined the effect of PRP gel on the healing of colonic anastomosis in rats that received HIPEC. Twenty-four rats were subdivided into three groups. In group 1, the rats only underwent colonic anastomosis. In group 2, the rats underwent colonic anastomosis and cisplatin-paclitaxel-based HIPEC. In group 3, the rats underwent colonic anastomosis and cisplatin-paclitaxel-based HIPEC. Afterwards, PRP gel was administered to the colonic anastomosis region. The rats were sacrificed on the 7th postoperative day, the anastomotic bursting pressure (ABP) was recorded, and tissue samples were taken from the anastomotic line. Compared to that in group 2, ABP was higher in group 1 and group 3 (p= 0.002 and 0.048, respectively). Tissue hydroxyproline levels were higher in group 1 (p= 0.001) and group 3 (p = 0.043) than in group 2. Histopathological evaluation revealed a significant decrease in fibroblast activity in group 2 compared to that in group 1 (p= 0.006), with no significant difference between group 3 and group 1 (p= 0.585). Collagen accumulation was significantly lower in group 2 than in group 1 and group 3 (p= 0.001 and p= 0.019, respectively). PRP gel application may aid wound healing by increasing colon ABP, tissue fibroblast activity and collagen accumulation; therefore, its use may reduce the cytotoxic effects of HIPEC and facilitate the anastomotic wound healing process.

Differences in results of infrabony pocket treatment with addition of platelet rich fibrin and platelet rich plasma gel in DFDBA bone graft

Treatment of infrabony pocket makes use of bone graft material demineralized freeze dried bone allograft (DFDBA) from different individuals which has undergone demineralization process and it is osteoinductive. Enhancement of growth factor was done by adding platelet rich fibrin (PRF) and platelet rich plasma (PRP). PRP is activated with an addition of calcium chloride CaCl2) to form gel. The method used to apply the bone grafting material is open flap debridement OFD). This research aimed to reveal the differences in the results of infrabony pocket treatment using PRF and PRP gel with an addition of DFDBA. The sample was taken from 20 infrabony pocket points divided into 2 groups, 10 infrabony pocket were treated with OFD+DFDBA+PRF and the other were treated with OFD+DFDBA+PRP gel. Pocket depth (PD) was measured on the baseline and the first and third month after treatment. Alveolar bone height was measured using cone beam computed tomography (CBCT) radiograph on the baseline to the third month after treatment. The results of this research showed that there was difference in the results of infrabony pocket treatment using PRF and PRP gel with an addition of DFDBA which could be observed from a reduction in PD from the baseline, month 1 and month 3 as well as reduction in alveolar bone height from the baseline to month 3. This research concluded that infrabony pocket treatment PRF application yields better results than PRP gel application in terms of PD and alveolar bone height reduction.

A Preliminary Report on Platelet‐Rich Plasma (PRP) Gel Application in Exofacial Parotidectomy

Objectives:Platelet‐rich plasma (PRP) is widely used in different kind of surgeries to improve hemostasis and wound healing. We set out to evaluate PRP gel efficacy, in drainage, drainage tube stay duration, and pain and length of hospital stay reduction after exofacial parotidectomy for pleomorphic adenoma.Methods:Between November and December 2012, exofacial parotidectomy was performed on 12 consecutive patients enrolled in this prospective randomized controlled clinical trial. Autologous PRP gel was applied on the remaining gland before wound closure in the treatment group (6 pts), while direct closure was applied in the controls (6 pts).Results:The mean 24‐hour cumulative drainage was 128.64 ml vs. 50.33 ml (P = 0.049), mean pain score at 4 hours was 5.3 vs 1.5 (P = 0.046) and 5.2 vs 1.5 (P = 0.037) at 8 hours in treatment and control group respectively; the drainage tube was removed after a mean of 5.6 days in control group and after 3.3 days in the treatment one (P = 0.037); PRP gel pts were discharged after a mean of 3.33 days whereas no‐PRP gel pts after a mean of 6.33 days (P = 0.046).Conclusions:These preliminary data suggest that PRP gel application in parotid surgery could be a feasible, low cost, and efficient tool in improving surgical outcomes and in reducing patients effort in terms of pain, drainage tube endurance and hospital staying.

Application of platelet-rich gel to enhance healing of transmetatarsal amputations in diabetic dysvascular patients.

Transmetatarsal amputation (TMA) represents an effective surgical procedure used to treat several clinical conditions such as forefoot infection, gangrene and chronic ulceration in diabetic patients. TMA permits walking without the need for prosthesis, but nevertheless is burdened with a high complications rate. The aim of this study was to evaluate the possibility to use platelet gel (PG) as an adjuvant therapy when performing TMA procedure in diabetic patients. In a 6-year period, 26 diabetic patients had undergone TMA procedure followed by autologous PG applications (group A) and 32 patients had undergone TMA as sole procedure (group B). After TMA procedure, the treatment is based on outpatient management and consists of a weekly platelet-rich plasma gel application on the surgical wound for 1 month in group A and on clinical evaluation only for group B. For group A, healing rate was of 96.15% and one patient (3.84%) presented wound dehiscence, and no postoperative wound infections occurred. For group B, healing rate was of 59.37%; severe infection of the stump prompted to the proximal amputations in 40.62% of patients during the follow-up period. PG application may be an effective adjuvant treatment to improve wound healing in diabetic dysvascular patients.

Application of PRP gel alone or in combination with guided bone regeneration does not enhance bone healing process: An experimental study in rabbits

Introduction The aim of this study was to assess the hypothesis that application of platelet-rich plasma (PRP) gel in mandibular defects in rabbits, alone or in combination with guided bone regeneration (GBR) techniques, could enhance the bone healing process. Materials and methods Thirty New Zealand white rabbits were used. Three groups of 10 animals each were assigned, and the animals were sacrificed after 12 weeks. During the operation, a rotating trephine bur was used to create circular defects 10-mm in diameter in the region anterior to the jaw angles. In group human fascia lata (HFL), a human fascia lata membrane was used. In group PRP, PRP gel was used to fill the defect, and in group HFL + PRP, PRP was used to fill the defect which after that was covered with a human fascia lata membrane. Results In general, none of the control sides and the PRP treated sides had full development of bone or filling of the defect through bone bridging. Conversely, the sides on which the fascia lata membrane or the combination of membrane and PRP had been applied were characterized mostly by development of newly formed bone that bridged the gap. Conclusions Our results suggest that the application of PRP gel alone or in combination with GBR does not enhance bone healing process.
