Clinical practice guideline for transurethral plasmakinetic resection of prostate for benign prostatic hyperplasia (2021 Edition)

Benign prostatic hyperplasia (BPH) is highly prevalent among older men, impacting on their quality of life, sexual function, and genitourinary health, and has become an important global burden of disease. Transurethral plasmakinetic resection of prostate (TUPKP) is one of the foremost surgical procedures for the treatment of BPH. It has become well established in clinical practice with good efficacy and safety. In 2018, we issued the guideline “2018 Standard Edition”. However much new direct evidence has now emerged and this may change some of previous recommendations. The time is ripe to develop new evidence-based guidelines, so we formed a working group of clinical experts and methodologists. The steering group members posed 31 questions relevant to the management of TUPKP for BPH covering the following areas: questions relevant to the perioperative period (preoperative, intraoperative, and postoperative) of TUPKP in the treatment of BPH, postoperative complications and the level of surgeons’ surgical skill. We searched the literature for direct evidence on the management of TUPKP for BPH, and assessed its certainty generated recommendations using the grade criteria by the European Association of Urology. Recommendations were either strong or weak, or in the form of an ungraded consensus-based statement. Finally, we issued 36 statements. Among them, 23 carried strong recommendations, and 13 carried weak recommendations for the stated procedure. They covered questions relevant to the aforementioned three areas. The preoperative period for TUPKP in the treatment of BPH included indications and contraindications for TUPKP, precautions for preoperative preparation in patients with renal impairment and urinary tract infection due to urinary retention, and preoperative prophylactic use of antibiotics. Questions relevant to the intraoperative period incorporated surgical operation techniques and prevention and management of bladder explosion. The application to different populations incorporating the efficacy and safety of TUPKP in the treatment of normal volume (< 80 ml) and large-volume (≥ 80 ml) BPH compared with transurethral urethral resection prostate, transurethral plasmakinetic enucleation of prostate and open prostatectomy; the efficacy and safety of TUPKP in high-risk populations and among people taking anticoagulant (antithrombotic) drugs. Questions relevant to the postoperative period incorporated the time and speed of flushing, the time indwelling catheters are needed, principles of postoperative therapeutic use of antibiotics, follow-up time and follow-up content. Questions related to complications incorporated types of complications and their incidence, postoperative leukocyturia, the treatment measures for the perforation and extravasation of the capsule, transurethral resection syndrome, postoperative bleeding, urinary catheter blockage, bladder spasm, overactive bladder, urinary incontinence, urethral stricture, rectal injury during surgery, postoperative erectile dysfunction and retrograde ejaculation. Final questions were related to surgeons’ skills when performing TUPKP for the treatment of BPH. We hope these recommendations can help support healthcare workers caring for patients having TUPKP for the treatment of BPH.

Application of Two New Feature Fusion Networks to Improve Real-time Prostate Capsula Detection.

Excess prostate tissue is trimmed near the prostate capsula boundary during transurethral plasma kinetic enucleation of prostate (PKEP) and transurethral bipolar plasmakinetic resection of prostate (PKRP) surgeries. If a large portion of the tissue is removed, a prostate capsula perforation can potentially occur. As such, real-time accurate prostate capsula (PC) detection is critical for the prevention of these perforations. This study investigated the potential for using image denoising, image dimension reduction and feature fusion to improve real-time prostate capsula detection with two objectives. First, this paper mainly studied feature selection and input dimension reduction. Secondly, image denoising was evaluated, as it is of paramount importance to transient stability assessment based on neural networks. Two new feature fusion techniques, maxpooling bilinear interpolation single-shot multibox detector (PBSSD) and bilinear interpolation single shot multibox detector (BSSD) were proposed. Before original images were sent to the neural network, they were processed by principal component analysis (PCA) and adaptive median filter (AMF) for dimension reduction and image denoising. The results showed that the application of PCA and AMF with PBSSD increased the mean average precision (mAP) for prostate capsula images by 8.55% and reached 80.15%, compared with single shot multibox detector (SSD) alone. Application of PCA with BSSD increased the mAP for prostate capsula images by 4.6% compared with SSD alone. Compared with other methods, ours were proven to be more accurate for real-time prostate capsula detection. The improved mAP results suggest that the proposed approaches are powerful tools for improving SSD networks.

Medicated plaster at Dazhui (GV 14) for preventing perioperative hypothermia in patients undergoing PKRP and ureteroscopic laser lithotripsy

To observe the preventive effect of medicated plaster at Dazhui (GV 14) on preventing perioperative hypothermia in patients undergoing plasmakinetic resection of prostate (PKRP) and ureteroscopic laser lithotripsy. A total of 300 patients with ASA gradeⅠorⅡreceiving ureteroscopic laser lithotripsy or PKRP under total intravenous anesthesia (TIVA) were randomly divided into an observation group and a control group, 150 cases in each group. The patients in the control group received routine heat preservation measures, while on the basis of the control group, the patients in the observation group were treated with medicated plaster at Dazhui (GV 14) 30 min before operation. The temperature (ear temperature), mean arterial pressure (MAP), heart rate and blood oxygen saturation (SpO2) at each time point (resting state before operation, 5, 10 min into operation, at the end of operation, 5 and 10 min after operation) were recorded; the number of hypothermia, the classification of postoperative shivering degree, operation time, propofol dosage, intraoperative infusion volume and intraoperative flushing fluid volume were recorded. The temperature at 5 and 10 min into operation as well as at the end of operation in the two groups was all lower than that before operation and at previous time point (P<0.01). The temperature at 5 and 10 min after operation in the two groups was all higher than that at previous time point (P<0.01), but still lower than that before operation (P<0.01). The temperature at 10 min into operation, at the end of operation, 5 and 10 min after operation in the observation group was higher than that in the control group (P<0.01, P<0.05). The total incidence rate of hypothermia in the observation group was 2.0% (3/150), which was significantly lower than 13.3% (20/150) in the control group (P<0.01). The total incidence rate of shivering was 18.0% (27/150) in the observation group and 28.7% (43/150) in the control group (P>0.05). The medicated plaster at Dazhui (GV 14) can effectively prevent perioperative hypothermia and improve comfort in patients undergoing PKRP and ureteroscopic laser lithotripsy.

AB018. The Sectional Three lobes resection of transurethral plasmakinetic resection of prostate

BackgroundTo introduce the skill and key point of the sectional three lobes resection of transurethral plasmakinetic resection of prostate (PKRP).MethodsTwo thousand and one hundred twenty-six cases of benign prostate hyperplasia (BPH) underwent the sectional three lobes resection of PKRP. The prostate middle lobe at 6 o’clock was resected to mark channels. The prostate at 5 and 1 o’clock were resected, then the left lateral lobe was resected. The prostate at 7 and 11 o’clock were resected, then the right lateral lobe was resected. The top lobe at 12 o’clock was resected to mark channels. The residual lateral lobes to cut down blood supply of prostate could be resected without bleeding. The resection was along the border of the surgical capsule of prostate without cystostomy. Finally, the prostate around the seminal hillock was TriMed.ResultsAll the operations were finished successfully. The operation times were from 30–90 minutes. All patients were without major blood, TURS, cardiovascular accident. Removal the urinary tube after operations 4–7 days, all patients had no urinary incontinence.ConclusionsThe sectional three lobes resection of PKRP for BPH safe and effective, less complication.

Thulium laser VapoResection of the prostate versus traditional transurethral resection of the prostate or transurethral plasmakinetic resection of prostate for benign prostatic obstruction: a systematic review and meta-analysis.

To compare the efficacy and safety of thulium laser VapoResection of the prostate (ThuVaRP) versus standard traditional transurethral resection of the prostate (TURP) or plasmakinetic resection of prostate (PKRP) for benign prostatic obstruction. Systematic searches were performed in the Medline, EMBASE, the Cochrane Library, Web of Science, and CNKI in December 2017. The outcomes of demographic and clinical characteristics, perioperative variables, complications, and postoperative efficacy including International Prostate Symptom Score (IPSS), quality of life (QoL), maximum flow rate (Qmax), and postvoid residual (PVR) were assessed. 16 studies were selected in the meta-analysis including nine randomized controlled trials (RCTs) and seven non-RCTs. Among of them, nine studies compared ThuVaRP with PKRP, while seven studies compared ThuVaRP with TURP. It seemed that ThuVaRP needed longer operation time than TURP (WMD = 6.41, 95% CI 1.38-11.44, p = 0.01) and PKRP (WMD = 10.15, 95% CI 5.20-15.10, p < 0.0001). ThuVaRP was associated with less serum hemoglobin decreased, catheterization time, and the length of hospital stay compared with TURP (WMD = - 0.58, 95% CI - 0.77 to 0.38, p < 0.00001; WMD = - 1.89, 95% CI - 2.67 to 1.11, p < 0.00001; WMD = - 2.25, 95% CI - 2.91 to 1.60, p < 0.00001) and PKRP (WMD = - 0.28, 95% CI - 0.46 to 0.10, p = 0.002; WMD = - 1.88, 95% CI - 2.87 to 0.89, p = 0.0002; WMD = - 2.08, 95% CI - 2.63 to 1.54, p<0.00001). According to our assessment, there was no significantly difference in postoperative efficacy. The pooled data indicated that ThuVaRP had a nearly efficacy to TURP and PKRP based on IPSS, QoL, Qmax, and PVR. Although ThuVaRP was associated with longer operation time, it got distinct superiority on serum hemoglobin decreased, catheterization time, and hospital stay.

Analysis of clinical efficacy of transurethral plasmakinetic resection of prostate combined with pneumatic lithotripsy in the treatment of benign prostatic hyperplasia and bladder stones

Objective To analyse the clinical efficacy and adverse reactions of transurethral plasmakinetic resection of prostate(PKRP) combined with pneumatic lithotripsy(TULT) in the treatment of benign prostatic hyperplasia(BPH) and bladder stones. Methods 78 cases with BPH and bladder stones were selected as the study subjects, and they were randomly divided into two groups, 39 cases in each group. They were all treated with transurethral PKRP, on this basis, patients in the control group were treated with small incision incision, patients in the observation group were treated with TULT. The clinical effect and complications were analyzed. Results The gravel time of the observation group was (28.56±4.73)min, which was significantly shorter than that of the control group. The intraoperative blood loss in the observation group was (103.47±10.58)mL, which was significantly less than that of the control group, there were statistically significant differences between the two groups(t=6.44, 6.85, all P 0.05). The postoperative catheterization time, bladder irrigation time and hospitalization time in the observation group were (5.26±1.38)d, (1.27±0.84)d and (6.34±1.25)d, respectively, which were significantly shorter than those in the control group, the differences were statistically significant(t=7.84, 7.16, 8.01, all P 0.05). The incidence rate of complications of the observation group was 2.56%, which was significantly lower than 5.13% of the control group(χ2=2.36, P>0.05). Conclusion The clinical efficacy and adverse reactions of transurethral PKRP combined with TULT in the treatment of BPH and bladder stones has significant effect, the utility model has the advantages of simple operation, small trauma, quick recovery and less complications, it can be used as the first choice for the treatment of elderly patients with BPH and bladder stones. Effect of high-dose finasteride combined with transurethral plasmakinetic resection of prostate on prostatespecific antigen and inflammatory factors in patients with benign prostatic hyperplasia

Purpose: To ascertain the effect of perioperative application of high-dose of finasteride on prostatespecific antigen (PSA) in serum, tumor necrosis factor α (TNF-α) and interleukin-1β (IL-1β) in prostatic fluid of prostatic hyperplasia (BPH) patients with transurethral plasmakinetic resection of prostate (PKRP). Methods: In total, 141 BPH patients treated with PKRP in Hunan Provincial People’s Hospital, Changsha City, China from January 2015 to January 2017 were randomly divided into three groups of 47 cases each, viz, high-dose (10 mg of finasteride), low-dose (5 mg of finasteride), control (0 mg of finasteride). The levels of PSA in serum, as well as the levels of TNF-α and IL-1β in prostatic fluid were evaluated 7 days before and after surgery. Re-bleeding within 3 months after surgery in the three groups were performed and the patients observed. Results: Intra-operative bleeding volume, bleeding volume per unit resected tissue, and intra-operative volume of rinsing fluid were significantly less in high- and low-dose groups than those in the control group (p &lt; 0.05); furthermore, the operation time was significantly shorter than that for the control group (p &lt; 0.05). Seven days after surgery, international prostate symptom score (IPSS), serum PSA, and TNF-α and IL-1β in prostatic fluid of the high- and low-dose groups were significantly lower than those of the control group (p &lt; 0.05). Also, maximum urine flow rate was significantly higher than that of the control group. Serum PSA, and TNF-α and IL-1β levels in prostatic fluid were significantly lower than those of low-dose group (p &lt; 0.05). Serum PSA as well as TNF-α and IL-1β levels in prostatic fluid were positively correlated with IPSS score (r = 0.817, 0.838, 0.859, p &lt; 0.001). The admission rate due to rebleeding was lowest in the high-dose group, followed by the low-dose group. Conclusion: Finasteride combined with PKRP for BPH acts synergistically to reduce serum PSA levels and local inflammatory reaction. High-dose finasteride/PKRP combination is more efficacious than the low-dose combination, thereby improving short-term prognosis in patients. Comparison of improved continuous bladder irrigation device and traditional device after plasmakinetic resection of prostate

Objective To compare the effect of improved continuous bladder irrigation device and traditional device after plasmakinetic resection of prostate(PKRP). Methods A total of 110 patients underwent PKRP who were selected and randomly divided into the observation group and the control group with each group 55 cases. The observation group was given improved continuous bladder irrigation device. The control group was given traditional device. The adverse symptoms, recovery time of bowel movement, time of irrigation, time of clear rinse solution, urinary red cell count after 48 h of washout and urine bacterial count 24 h before operation and after 72 h of washout in two groups were recorded and compared. Results The adverse symptoms of the observation group were better than those of the control group. The difference was statistically significant (P 0.05). Conclusions Improved continuous bladder irrigation device compared with traditional device can effectively decrease the incidence of adverse symptoms and apply to patients with continuous bladder irrigation after PKRP. AB016. Transurethral PlasmaKinetic enucleation of prostate versus transurethral PlasmaKinetic resection of prostate for benign prostate hyperplasia

BackgroundTo compare the efficacy and safety of transurethral PlasmaKinetic enucleation of prostate (PKEP) versus transurethral PlasmaKinetic resection of prostate (PKRP) for benign prostate hyperplasia.MethodsThe Prospective and randomized controlled trials (RCTs) pertaining to PKEP and PKRP for benign prostate hyperplasia were retrieved from PubMed, Wanfang and so on. The level of trials was assessed by using Cochrane system.ResultsCompared with PKRP, the time of operation shorten 10.17 min, the time of catheterization and hospital stay shorten 22.30 h, 1.33 d respectively. The volume of intraoperative bleeding decreased 47.34 mL. The quantity of resectable prostate increase 10.76 g, difference is statistically significant (P<0.05). Compared with PKRP, the international prostate symptom score (IPSS) was lower after 3 months, the quality of life (QOL) and residual urine volume (RUV) was lower after 6 months, the maximum floe rate (Qmax) was higher after 3 and 6 months, difference is statistically significant (P<0.05). No significant differences were found in IPSS, QOL, Qmax, RUV, international index of erectile function 5 (IIEF-5) between PKRP and PKEP after 1 and 2 years. The incidence of transfusion and capsular perforation was lower than PKRP, but the incidence of postoperative Transient urinary incontinence was higher than PKRP.ConclusionsCompared with PKRP, PKEP cause less blood loss and requires a shorter operation time, a shorter catheterization time and a shorter hospital stay. It resects the proliferated prostate more cleanly with lower incidence of capsular perforation, but the incidence of postoperative transient urinary incontinence was higher. The short-term efficacy of PKEP was better than PKRP, but no significant differences were found in long-term efficacy and the postoperation influence of sex function between PKEP and PKRP.

Photoselective Vaporization of the Prostate vs Plasmakinetic Resection of the Prostate: A Randomized Prospective Trial With 12-Month Follow-up in Mainland China

To compare the short-term efficacy and safety results of photoselective vaporization of the prostate (PVP) and plasmakinetic resection of prostate (PKRP), 2 methods of treating benign prostatic hyperplasia (BPH)-related lower urinary tract symptoms (LUTS) in mainland China. One hundred twenty patients with LUTS secondary to BPH were randomly divided into 2 groups: PVP group (n = 61) and PKRP group (n = 59). Perioperative data and postoperative complications were recorded for both groups. Patients were followed up for 12 months after treatment. Compared with PKRP, PVP required a longer operation time (56 vs 41 minutes; P <.01). PVP also needed shorter catheterization time (2.4 vs 3.5 days; P <.01) and shorter length of hospitalization (3.5 vs 5.1 days; P <.01). International Prostate Symptom Score, Qmax, and quality of life score were significantly improved for both groups after surgery, and no significant differences were apparent between the 2 groups, for up to 12 months. Short-term complications were assessed in regard to necessity of blood transfusion (1 vs 1), clot retention (1 vs 0), reoperation (1 vs 0), incontinence (0 vs 0), urethral stricture (1 vs 0), and retrograde ejaculation (4 vs 6) in the PVP vs PKRP groups, respectively. In patients with BPH-related LUTS, PVP may be a superior treatment to PKRP as it relates to the endpoints of this study, with short-term results after a 12-month follow-up as length of hospital stay and catheterization time needed were both less than what was required for PKRP. Long-term comparative data are required to clearly define the role of PVP and PKRP in patients.

Plasma Kinetic Resection of Prostate Combined Terazosin for Treatment of BPH Patient with Coexisting Hypertension

Objective: To evaluate the efficacy and safety of terazosin and plasma kinetic resection of the prostate (PKRP) in the treatment of benign prostatic hyperplasia (BPH) patients with coexisting hypertension. Methods: A total of 205 men who suffered from BPH with concomitant hypertension were enrolled in this study. They all received PKRP surgery and terazosin medication from one week before to 3 months after the operation. The procedures of efficacy and safety were evaluated by analysis of the changes in blood pressures, IPSS, Qmax and the drug related adverse events 4 weeks and 3 months after the operation. A complete TUPVP with "both sides outflanking" method: a report of 30 cases

Objective To introduce a method and technique of transurethral plasmakinetic resection of prostate (TUPRP).Methods A total of 30 patients with benign prostatic hyperplasia (BPH) had undergoing TUPRP during the period of July 2004 to February 2013.Results The procedure was successfully performed on all the patients.The surgical duration was 25 to 120 min and the volume of bleeding was 30 to 400 ml.No serious complications including water intoxication occurred.Conclusions TUPVP is proven to be reasonable and easy to master.It greatly increases the efficiency and safety of the proceudure and is efficacious. Effect of modified continuous bladder irrigation device on prevention of urinary tract infection in patients after operation of plasmakinetic resection of prostate

Objective To explore the intervention effect of the modified continuous bladder irrigation device on preventing urinary tract infection after operation of plasmakinetic resection of prostate (PKRP).Methods A total of 261 patients from January 2011 to December 2012 were divided into two groups,126 cases from January 2011 to December 2011 used traditional device as control group,while 135 cases from January 2012 to December 2012 used modified device as observation group.The time of bladder irrigation,the total of white blood cells and bacteria,the time of urethral catheterization,ureter blockage,leakage urine,bladder spasm and urinary tract infection and so on after operation of PKRP were observed.Results After the intervention,the number of bladder spasm (15 vs 68),ureter blockage (11 vs 43),leakage urine (13 vs 58),and urinary tract infection(2 vs 12) in observation group was significantly less than that in control group(x2 =55.194,26.805,43.610,8.304,respectively; P ＜ 0.01).And the time of bladder irrigation and urethral catheterization in observation group respectively was (22.91 ± 10.181) h,(129.89 ± 46.632) h,and that in control group respectively was (35.63 ± 8.663) h,(190.18 ± 57.744) h,and the difference was statistically significant (t =-16.380,-11.511,respectively ; P ＜ 0.01).Conclusions The use of the modified continuous bladder irrigation device can reduce the chance of urinary tract infection,and prevent hospital infection,which is worth promoting widely. Comparison of Fluid Absorption between Transurethral Enucleation and Transurethral Resection for Benign Prostate Hyperplasia

Introduction: Although transurethral resection of prostate (TURP) remains the reference standard for benign prostate hyperplasia (BPH), the concern about complications promotes researchers to develop alternative surgical methods with fewer complications. In this study, we compared the safety and efficacy between the transurethral plasma kinetic enucleation of prostate (TUPKEP) and transurethral plasma kinetic resection of prostate (TUPKRP), mainly including absorption of irrigation fluid, the operation time, the weight of prostate tissue removed and severe complications. Methods: Sixty BPH patients were randomly and evenly assigned to the TUPKEP or TUPKRP group. The irrigation fluid used in both groups was 1% ethanol-containing saline solution. The ethanol concentrations in the subjects' end expiration were measured during operation. The volume of irrigation fluid absorbed was calculated accordingly. Results: No significant difference was found in operation time between two groups, whereas the weight of prostate tissue resection was significantly higher in the TUPKEP than that in the TUPKRP group. Conclusion: The study provides evidence for the safety, feasibility and effectiveness of both bipolar transurethral techniques. Further, compared to the TUPKRP group, the TUPKEP group has more efficient for resection of prostatic hyperplasia tissue, even though in terms of fluid absorption, no difference has been found in both groups. Ethanol monitoring is simple, safe and effective, which is beneficial for enhancing safety procedures.

METHODS A total of 512 patients were randomized to either TURP or PK resection of prostate between January 2004 to December 2005 . All of them were evaluated by detail medical history, physical examination , blood analysis, PSA, urinanalysis and culture, AUA Symptom Score, 3D-TRUS measurement of prostate volume , Omax, IIEF-5 and Quality of Life score. TURP was carried out using a 26 F resectoscope and a single wire loop for resection . Glycine was used as irrigation fluid. For the PK Superpulse system, the dedicated resectoscope was used, and normal saline was used as the irrigation fluid. RESULTS Both groups were comparable in all preoperative parameters. 501 patients had completed 5 years follow-up: 255 (of 261) in the PK group and 246(of 251)in the conventional TURP group. The blood loss observed in PK Superpulse arm (178.62±48.47mL) was significantly less than after conventional TURP (351.56±91.52) (P<0.01). The catheter time in patients operated on PK superpulse was 25.56±7.89 hours, less then that in the conventional TURP arm (40.37±10.48 hours) (P< 0.001). The operative time was less in PK group (74.74±12.34 vs 77±13.80 min, p=0.02). Both procedures resulted in statistically significant improvements in AUA symptom score, Qmax , Quality of life at each postoperative assessment compared with baseline values. All parameters did not differ significantly between the groups at any time. The Hematuria, Clot retension ,Blood transfusion and clinical TUR syndrome, Hyponatremia were statistically less in PK group while no statically difference noted in Capsular perforation ,Urinary tract infection , Urethral stricture and Bladder neck contracture. Perioperative nursing of transurethral bipolar plasmakinetic resection of prostate

Objective To evaluate the perioperative nursing of transurethral bipolar plasmakinetic resection of prostate(TUPKRP).Methods One hundred and thirty BPH patients underwent transurethral prostatectomy with the bipolar plasmakinetic technique.All patients received preoperative mental nursing,holistic nursing care and health education before operation.Results All patients passed through the surgery and were given nursing intervention.They were discharged after rehabilitation.Conclusions TUPKRP is an important miro-invasion operation for treatment of BPH.The important points are preoperative mental nursing,rational use of drug,postoperative bladder irrigation and prevention of hemorrhage. The clinical observation of bipolar transurethral plasma kinetic resection of prostate for high risk benign prostate hyperplasia

To evaluate therapeutic effect and reliability of bipolar transurethral plasma kinetic prostatectomy (TUPKP) for high risk level benign prostatic hyperplasia (BPH). A total of 230 cases of high risk of BPH were treated with TUPKP. Among them, 132 cases with the residual urine of 40 to 420 ml had accepted long term but inefficient medical therapy, 98 cases were suffered with repeating acute urinary retention. One hundred and seventy-three cases with the functional capacity>4 MET were performed the standard transurethral resection of the prostate (TURP), the other 57 cases with the functional capacity<4 MET were accepted the minimally invasive TURP. Among them 12 cases complicated with bladder stones accepted Ho: YAG lithotripsy priory. The international prostate symptom score (IPSS), The maximal urinary flow rate (Qmax) and residual urine of the 2 groups before and after operation were analyzed. There was no transurethral resection syndrome occurred in both groups. After 3 to 12 months of follow-up postoperatively, the IPSS of the two groups were reduced from (21.9+/-5.7) and (23.7+/-5.0) to (4.4+/-2.3) and (5.5+/-2.4), residual urine were reduced from (61.8+/-18.4) ml and (103.9+/-77.3) ml to (13.0+/-6.2) ml and (15.8+/-6.1) ml, respectively. The Qmax was increased from (5.7+/-3.0) ml/s and (4.8+/-2.8) ml/s to (20.9+/-6.3) ml/s and (16.8+/-3.9) ml/s, there existed significant differences (P<0.01). However the IPSS, Qmax and residual urine of the standard group had progressed more obviously than the minimally invasive TURP group (P<0.05). It is safe and effective to use TUPKP for treating high risk patients of BPH with classic TURP and minimally invasive TURP according to different functional capacity. When the functional capacity is more than 4 MET, the standard procedures is preferred.

Transurethral resection of prostate for BPH in high risk patients(report of 480 cases)

目的 探讨高龄高危前列腺增生症(BPH)患者的腔镜术治疗的疗效和安全性.方法 采用等离子双极电切术(PKRP)治疗高危BPH 356例,同期采用传统经尿道电切术(TURP)124例,对两种术式的临床效果及安全性进行评估.结果 两组患者术后国际前列腺症状评分(IPSS)、生活质量评分(QOL)、最大尿流率(Qmax)均得到显著改善(P＜0.01);但PKRP组病人术中出血更少,单位时间内组织切除率更高,无低钠血症及水中毒(TURS)等并发症.结论 两种经尿道(TUR)手术都是治疗BPH的有效方法;但PKRP具有止血效果好、安全性高、对机体生理功能影响小,并发症少等优点,对高危BPH患者是一极具优势的TUR手术方法。
